Abstract Background: It is estimated that more than 30% of the Brazilian population has systemic arterial hypertension (SAH), and mostly as an uncontrolled disease. The most recent Brazilian Guideline of Hypertension recommends the practice of self-measurement of blood pressure (BP) as one of the strategies for a better control of SAH, but there is no consensus about the efficiency of this tool. Objective: To assess the control of SAH and the practice of non-targeted self-measured BP (SMBP) among hypertensive users of the Unified Health System (SUS) and the Supplementary Network (SN). Methods: This is a cross-sectional, observational, analytical study, with a stratified probability sample. One thousand volunteers were investigated, being 500 from SUS and 500 from the SN. Uni and multivariate analyses were performed considering a 5% significance level. Results: Patients from SUS presented inferior sociodemographic data (schooling, social status) in relation to those of the SN (p < 0.001), and showed lower control of SAH (p = 0.014), as well as more visits to the emergency room in the past year due to hypertension (p = 0.002), and fewer regular appointments with the cardiologist (p = 0.004). SMBP was equally present in both assessed groups (p = 0.567), even though users of the SN have been more advised to not conduct such a practice (p = 0.002). SMBP (p < 0.001) was an independent factor for uncontrolled SAH both in SUS (OR = 3.424) and in the SN (OR = 3.474). Conclusion: Patients in SUS presented lower SAH control. The practice of SMBP, mostly practiced with an uncalibrated digital device, was equally present in both groups and became an independent factor of uncontrolled SAH.

ABSTRACT The Heart Failure (HF) is responsible for a large number of hospitalizations in Brazil, but there is still a considerable gap in the scrutiny of the public health system (SUS) regarding the quality of care provided. Objective: to describe the methodology used in the VICTIM-CHF study, which aims to characterize and compare the nutritional, clinical, lifestyle, treatment received, outcomes, as well as the possible disparities observed among patients in Brazil. Methodology: This is a cross-sectional, descriptive and prospective study, carried out with patients with heart failure (HF) admitted to 4 hospitals in Sergipe, Brazil. Clinical, sociodemographic, nutritional, quality of life and outcomes (death and readmission) parameters will be investigated up to 30 days after hospital discharge. Preliminary results of ten volunteers: 70% were male, 60% adults. The most frequent etiology was ischemic (50%), 50% had HF with preserved ejection fraction (EF) and the remainder, with intermediate EF. Malnutrition, according to BMI, was present in 20% and the hospital stay was significantly (p= 0.036) longer in SUS users. Conclusion: The VICTIM-CHF Registry aims to identify opportunities for improvement in the line of care for HF users of the SUS and the supplementary health system. It is hoped that the findings of this investigation can help to optimize the strategic planning of more efficient and equitable health policies.
RESUMO A Insuficiência Cardíaca (IC) é responsável por um grande número de internações no Brasil, mas ainda existe considerável lacuna escrutinizando o sistema de saúde pública (SUS) no tocante a qualidade assistencial praticada. Objetivo: descrever a metodologia empregada no estudo VICTIM-CHF que tem como objetivo caracterizar e comparar os aspectos nutricionais, clínicos, estilo de vida, tratamento recebido, desfechos, bem como as possíveis disparidades observadas entre os pacientes Brasil, mas ainda existe considerável lacuna escrutinizando o sistema de saúde pública (SUS) no tocante a qualidade assistencial praticada. Métodologia: Trata-se de estudo transversal, descritivo e prospective, realizado com portadores de insuficiência cardíaca (IC), internados em 4 hospitais de Sergipe, Brasil. Serão investigados parâmetros clínicos, sociodemográficos, nutricionais, qualidade de vida e desfechos (morte e reinternação) até 30 dias após a alta hospitalar. Resultados preliminaries de dez voluntários: 70% eram do sexo masculine, sendo 60% adultos. A etiologia mais frequente foi a isquêmica (50%), 50% exibia IC pcom fração de ejeção (FE) preservada e o restante, com FE intermediária. Desnutrição, segundo o IMC, esteve presente em 20% e o período de internação foi, significativamente (p= 0,036) superior nos usuários do SUS. Conclusão: O Registro VICTIM-CHF visa identificar oportunidades de melhoria na linha de cuidado para a IC de usuários do SUS e do Sistema de saúde suplementar. Espera-se que os achados desta investigação possam ajudar a otimizar o planejamento estratégico de políticas de saúde mais eficientes e equânimes.

Abstract Birds have a unique anatomy and physiology. Their digestive system consists of esophagus, stomach (proventricle and ventricle), small intestine and large intestine. The esophagus has a dilatation called ingluvies in most birds, which stores food. Injuries and masses in these places can occur for several reasons, causing dietary difficulties and weight loss. Ingestion of inappropriate substances can form a mass, rest in the crop and cause intoxication. The aim of this study was to describe a case of ingluviotomy resulting from the ingestion of a foreign body in cockatiel (Nymphicus hollandicus). Ingluviotomy is an urgent surgical procedure, and it can be chosen to remove a foreign body in the ingluvium. The surgical resolution of the problem and clinical recovery occur without major problems as long as an appropriate clinical-surgical approach is used.
Resumen Los pájaros tienen una anatomía y fisiología únicas. Su sistema digestivo consta de esófago, estómago (proventrículo y ventrículo), intestino delgado e intestino grueso. En la mayoría de los pájaros, el esófago tiene una dilatación llamada ingluvie que almacena alimento. En este lugar pueden ocurrir lesiones y masas debido a varias razones, ocasionando dificultades dietarias y pérdida de peso. La digestión de sustancias inadecuadas puede formar una masa, quedarse ahí hasta germinar e intoxicarse. El objetivo de este estudio es hacer un informe de una ingluviotomía debido a la ingestión de un cuerpo extraño en una cacatúa ninfa (Nymphicus hollandicus). La ingluviotomía es un procedimiento quirúrgico de urgencia y se puede escoger para remover un cuerpo extraño en el ingluvium. La resolución quirúrgica del problema y la recuperación clínica ocurren sin mayores problemas si se usa un enfoque clínico-quirúrgico adecuado.
Resumo As aves apresentam anatomia e fisiologia únicas. Seu sistema digestório é composto por esôfago, estômago (proventrículo e ventrículo), intestino delgado e intestino grosso. O esôfago apresenta uma dilatação chamada de inglúvio na maioria das aves, que armazena alimentos. Lesões e massas nesses locais ocorrem por diversos motivos, provocando dificuldades alimentares e emagrecimento. A ingestão de substâncias inadequadas pode formar uma massa, repousar no papo e gerar possível intoxicação. O objetivo do trabalho foi descrever um caso de ingluviotomia decorrente de ingestão de corpo estranho em calopsita (Nymphicus hollandicus). A ingluviotomia é um procedimento cirúrgico de urgência, podendo ser optado para a remoção de corpo estranho no inglúvio, sendo que a resolução cirúrgica do problema e a recuperação clínica ocorre sem maiores problemas desde que empregada um conduta clínico-cirúrgica adequada.

Sensitivity of different DNA extraction methods and PCR to detect resistance in patients with leprosy stratified by the bacilloscopic index

ABSTRACT Introduction: Antimicrobial resistance in leprosy is an emerging problem, and the quantitative impact of low bacilloscopic indexes (BIs) on the sensitivity of molecular tests is unknown. We aimed to evaluate the sensitivity of gene sequencing for the detection of mutations related to antimicrobial resistance in Mycobacterium leprae in patients with low BIs using an analytical model. Methods: Patients with leprosy were included and divided into two groups depending on their BIs (≥ 2+ and < 2+). The sensitivities of the two DNA extraction methods were compared after amplifying and sequencing the repetitive element (RLEP), folP1, rpoB and gyrA in M. leprae. Results: We included 56 patients with leprosy: 35 had BIs less than 2+ (22 had negative slit-skin smear [SSS] results) and 21 patients had BIs greater than or equal to 2+. The sensitivity of the amplification of the RLEP target and the gene sequencing of folP1, rpoB and gyrA was 50 to 70% lower in patients with a BI less than 2+ and was significantly reduced in patients with lower BIs for all targets (p < 0.001). One patient had a mutation in the folP1 gene, and 14 patients had mutations in the gyrA gene, but no mutations related to antimicrobial resistance were found. Conclusions: We can conclude that the sensitivity of molecular tests is directly related to the BI, but these tests can still detect up to 20% of the targets in patients with BIs < 2+. New strategies to improve the sensitivity for detecting antimicrobial resistance in leprosy patients and reasonable clinical criteria for follow-up and the introduction of alternative treatments must be developed.

RESUMO O DC de Neuroimunologia da ABN e o BCTRIMS trazem, nesse documento, as recomendações sobre vacinação da população com doenças desmielinizantes do sistema nervoso central (SNC) contra infecções em geral e contra o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2), causador da COVID-19. Destaca-se a gravidade do atual momento frente ao avanço da COVID-19 em nosso País, o que torna mais evidente e importante a criação de guia de referência para orientação aos médicos, pacientes e autoridades de saúde pública quanto à vacinação, meio efetivo e seguro no controle de determinadas doenças infecciosa. O DCNI/ABN e o BCTRIMS recomendam que os pacientes com doenças desmielinizantes do SNC (ex., EM e NMOSD) sejam constantemente monitorados, quanto a atualização do seu calendário vacinal, especialmente, no início ou antes da mudança do tratamento com uma droga modificadora de doença (DMD). É importante também salientar que as vacinas são seguras e os médicos devem estimular o seu uso em todos os pacientes. Evidentemente, deve ser dada especial atenção às vacinas com vírus vivos atenuados. Por fim, é importante que os médicos verifiquem qual DMD o paciente está em uso e quando foi feita a sua última dose, pois cada fármaco pode interagir de forma diferente com a indução da resposta imune.
ABSTRACT The Scientific Department of Neuroimmunology of the Brazilian Academy of Neurology (DCNI/ABN) and Brazilian Committee for Treatment and Research in Multiple Sclerosis and Neuroimmunological Diseases (BCTRIMS) provide recommendations in this document for vaccination of the population with demyelinating diseases of the central nervous system (CNS) against infections in general and against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes COVID-19. We emphasize the seriousness of the current situation in view of the spread of COVID-19 in our country. Therefore, reference guides on vaccination for clinicians, patients, and public health authorities are particularly important to prevent some infectious diseases. The DCNI/ABN and BCTRIMS recommend that patients with CNS demyelinating diseases (e.g., MS and NMOSD) be continually monitored for updates to their vaccination schedule, especially at the beginning or before a change in treatment with a disease modifying drug (DMD). It is also important to note that vaccines are safe, and physicians should encourage their use in all patients. Clearly, special care should be taken when live attenuated viruses are involved. Finally, it is important for physicians to verify which DMD the patient is receiving and when the last dose was taken, as each drug may affect the induction of immune response differently.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

RESUMO Apesar de a cirurgia bariátrica (CB) auxiliar na redução do processo inflamatório e risco cardiometabólico, poucos estudos avaliaram o impacto desse procedimento em fatores ambientais, bioquímicos, hemodinâmicos, cardiorrespiratórios e na composição corporal, de forma prospectiva, sobretudo escrutinando usuários do Sistema Único de Saúde (SUS). Portanto, o principal objetivo deste estudo é analisar a evolução do risco cardiometabólico em pacientes submetidos a CB e a sua associação com os fatores ambientais, bioquímicos, hemodinâmicos, cardiorrespiratórios e de composição corporal. Trata-se de estudo analítico, longitudinal, prospectivo, de coorte com dois grupos e abordagem quantitativa. Serão considerados elegíveis, aqueles candidatos à CB que atendam os critérios da Sociedade Brasileiro de Cirurgia Bariátrica e Metabólica (grupo caso) e voluntários com diabetes mellitus 2 e obesidade grau II (grupo controle). Serão coletados dados de composição corporal, hemodinâmicos, bioquímicos, de consumo alimentar e cardiorrespiratórios antes da cirurgia e decorridos: um, três, seis e doze meses do procedimento. Além disso, serão realizados: teste ergoespirométrico, ecocardiograma transtorácico e densitometria óssea (DEXA). Esta investigação pretende produzir informações a respeito do impacto da CB na integridade metabólica, física e psicossocial de usuários do SUS. Assim, espera-se municiar os gestores públicos de informações técnicas que embasem políticas de saúde eficientes e equânimes.
ABSTRACT Despite the fact that bariatric surgery (CB) reduces the inflammatory process and cardiometabolic risk, few studies have evaluated the impact of this procedure on environmental, biochemical, hemodynamic, cardiorespiratory factors on body composition, prospectively, especially by scrutinizing users of the Unified Health System. Health (SUS). Therefore, this study aims to analyze the evolution of cardiometabolic risk in patients undergoing CB and its association with environmental, biochemical, hemodynamic, cardiorespiratory, and body composition factors. It was an analytical, longitudinal, prospective, cohort study with two groups and a quantitative approach. Candidates for CB who meet the criteria of the Brazilian Society of Bariatric and Metabolic Surgery (case group) and volunteers with diabetes mellitus 2 and obesity grade II (control group) will be considered eligible. Body composition, hemodynamic, biochemical, food consumption, and cardiorespiratory data will be collected before surgery and after one, three, six, and twelve months after the procedure. In addition, an ergospirometric test, transthoracic echocardiogram, and bone densitometry (DEXA) was performed. This investigation intends to produce information about the impact of CB on the metabolic, physical, and psychosocial integrity of SUS users. Thus, it is expected to provide public managers with technical information to support efficient and equitable health policies.

RESUMO Este esudo busca descrever a metodologia do estudo AnOPAR em hipertensos bem como a associação da autoaferição da pressão arterial (PA) com o seu controle, presença de transtornos de ansiedade, de visitas não programadas à urgência e automedicação. Estudo transversal, observacional, analítico no qual se pretende avaliar 1000 voluntários, portadores de hipertensão arterial sistêmica, acompanhados ambulatorialmente, em hospital público e privado, localizados em Aracaju-Sergipe, Brasil. A coleta de dados será realizada mediante a aplicação de um questionário específico para a pesquisa; ao controle da PA, mediante Monitorização Ambulatorial da Pressão Arterial (MAPA) ou média das aferições nas três últimas consultas; a autoavaliação da ansiedade por meio do Inventário de Ansiedade Traço-Estado (IDATE), a determinação da qualidade de vida, por intermédio do SF-36. Na análise estatística serão empregados testes para hipótese de indepedência entre variáveis categóricas, igualdade de médias, regressão logística, tamanhos de efeito D e h de Cohen e nível de significância de 5%. A análise dos resultados pretende produzir informações a respeito do impacto da autoaferição da pressão arterial, não orientada, da população hipertensa. Com isso, espera-se municiar os gestores públicos de informações que embasem políticas de saúde mais eficientes em relação a prática da autoaferição realizada pelos hipertensos.
ABSTRACT We describe the methodology of the MBPPG study in hypertensive patients as well as the association of blood pressure self-measurement (BP) with its control, the presence of anxiety disorders, unplanned visits to urgency and self-medication. This is a cross-sectional observational analytical study in which it is intended to evaluate 1000 volunteers with systemic arterial hypertension followed up on an outpatient basis in public and private hospitals located in Aracaju-Sergipe, Brazil. Data collection will be conducted applying a specific questionnaire for the research; BP control through Ambulatory Blood Pressure Monitoring (ABPM) or an average of measurements in the last three consultations; the self-assessment of anxiety through the State-Trait Anxiety Inventory (STAI), the determination of quality of life through the SF-36. In the statistical analysis, tests will be used for the hypothesis of independence between categorical variables, equality of means, logistic regression, effect sizes Cohen’s D and h, and a significance level of 5%. The results analysis intends to produce information about the impact of the self-measurement of blood pressure, not oriented, of the hypertensive population. With this, it is expected to provide public managers with information that will support more efficient health policies in relation to the practice of self-assessment performed by hypertensive patients.

Abstract INTRODUCTION: Although supervised doses are essential for reducing leprosy treatment failure, the impact of specific drug interactions has rarely been assessed. This study aimed to estimate the risk of leprosy treatment suspension in patients receiving polypharmacy. METHODS We performed this case-control study in which the primary outcome was defined as the need to discontinue multibacillary leprosy treatment for at least one supervised dose, and the main risk factor was the detection of polypharmacy. Multivariate analysis by logistic regression was used for calculating odds ratio (OR). RESULTS: This study included 103 patients, of whom 43 needed to discontinue leprosy treatment (hemolysis = 26, hepatitis = 2, hemolysis associated with hepatitis = 6, and suspected treatment resistance = 9) and the rest did not. The severity of drug interactions had no effect on treatment discontinuation. Patients who used five or more drugs in addition to leprosy treatment had almost a 4-fold greater risk of treatment suspension (OR, 3.88; 95% confidence interval: 1.79-9.12; p < 0.001). The number of drugs used also positively influenced the occurrence of hemolysis (p < 0.001). No patient presented evidence of molecular resistance to rifampicin, dapsone, or ofloxacin treatment, as evidenced by genetic sequencing detection of rpoB, folp1, and gyrA mutations. CONCLUSIONS: Polypharmacy has deleterious effects on the already difficult-to-adhere-to treatment of leprosy and polypharmacy induces hemolysis. Additional measures must be taken to avoid the undesirable effects of inadequate polypharmacy.

ABSTRACT Immunosuppression is an important risk factor for leishmaniasis. We assessed the clinical profile, geographic distribution and prevalence of leishmaniasis in patients undergoing immunosuppressive therapy for dermatological, rheumatological or gastroenterological autoimmune diseases. We identified relevant studies in PubMed, EMBASE, Scopus, Web of Science and LILACS on July 3rd, 2018. We included articles that reported at least one case of leishmaniasis in patients undergoing immunosuppressive treatment for dermatological, rheumatological or gastroenterological diseases. Our protocol was registered in PROSPERO (CRD42018103050). We assessed the quality of the included studies with the Joanna Briggs Institute Critical Appraisal Tool. After the removal of duplicates, 5,431 articles were collected and screened. We included 138 articles; the prevalence of leishmaniasis in six methodologically similar studies varied from three to 1,282 cases per 100,000 patients using anti-TNFα drugs, but the results were significantly heterogeneous . Leishmaniasis in patients treated with immunosuppressive drugs is a health problem mostly reported in European countries bordering the Mediterranean Sea; sporadic activities, such as travelling, seem not to be associated with a significant risk of leishmaniasis, although effective control measures must always be observed.

ABSTRACT The use of Ti-Ni memory alloys covers several fields of application such as medicine, dentistry, actuators, sensors among others. Because of the thermomechanical behavior of these materials, they are an object of continuous studies and their properties are related to the occurrence of thermoelastic martensitic phase transformations. The thermal analyzes were performed using dilatometry and differential scanning calorimetry to determine the temperatures that occurred transformations during the cooling and heating, as well as to evaluate the influence of the thermal treatments performed by dilatometry. The thermomechanical behavior was also evaluated by the internal friction that measured behavior damping considering the structural and microstructural changes. The calorimetry measurements indicated suppression of the R phase through decomposition of precipitates of Ti3N4after the thermal treatments, that were corroborated by dilatometry and microhardness. This phase was also studied by the technique of internal friction, which showed that the mechanical damping coefficient increased as a function of temperature, due to the movement of the defects induced by tension.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Predictors of in-hospital mortality in patients with ST-segment elevation myocardial infarction undergoing pharmacoinvasive treatment

OBJECTIVES: To identify predictors of in-hospital mortality in patients with acute myocardial infarction undergoing pharmacoinvasive treatment. METHODS: This was an observational, prospective study that included 398 patients admitted to a tertiary center for percutaneous coronary intervention within 3 to 24 hours after thrombolysis with tenecteplase. ClinicalTrials.gov: NCT01791764 RESULTS: The overall in-hospital mortality rate was 5.8%. Compared with patients who survived, patients who died were more likely to be older, have higher rates of diabetes and chronic renal failure, have a lower left ventricular ejection fraction, and demonstrate more evidence of heart failure (Killip class III or IV). Patients who died had significantly lower rates of successful thrombolysis (39% vs. 68%; p = 0.005) and final myocardial blush grade 3 (13.0% vs. 61.9%; p<0.0001). Based on the multivariate analysis, the Global Registry of Acute Coronary Events score (odds ratio 1.05, 95% confidence interval (CI) 1.02-1.09; p = 0.001), left ventricular ejection fraction (odds ratio 0.9, 95% CI 0.89-0.97; p = 0.001), and final myocardial blush grade of 0-2 (odds ratio 8.85, 95% CI 1.34-58.57; p = 0.02) were independent predictors of mortality. CONCLUSIONS: In this prospective study that evaluated patients with ST-segment elevation myocardial infarction treated by a pharmacoinvasive strategy, the in-hospital mortality rate was 5.8%. The Global Registry of Acute Coronary Events score, left ventricular ejection fraction, and myocardial blush were independent predictors of mortality in this high-risk group of acute coronary syndrome patients.

INTRODUÇÃO: O ultrassom intracoronário (USIC) é um método capaz de fornecer medida sensível e reprodutível das dimensões da artéria coronária, da placa aterosclerótica e do lúmen arterial. Avanços em sua tecnologia permitem agora a caracterização da composição e da morfologia das placas ateroscleróticas. Embora estudos prévios tenham reportado dados utilizando USIC com análise de radiofrequência, o uso de uma nova modalidade (iMap®, Boston Scientific, Santa Clara, Estados Unidos) de caracterização da placa aterosclerótica é muito pouco conhecida. Nosso objetivo será analisar as características morfológicas, teciduais e fenotípicas das placas ateroscleróticas consideradas angiograficamente "culpadas" e "não-culpadas" em pacientes submetidos a angiografia coronária decorrente de infarto agudo do miocárdio (IAM). MÉTODOS: Estudo prospectivo, transversal, em único centro (Hospital São Paulo - Escola Paulista de Medicina - Universidade Federal de São Paulo/UNIFESP, São Paulo, SP, Brasil). Serão selecionados 50 pacientes para análise ultrassonográfica, de acordo com os seguintes critérios de inclusão: idade < 75 anos, IAM sem supradesnivelamento do segmento ST ou IAM com supradesnivelamento do segmento ST recente, com ou sem uso de fibrinolítico prévio. CONCLUSÕES: O presente estudo objetivará a caracterização morfológica, tecidual e fenotípica da placa aterosclerótica utilizando uma nova modalidade de imagem ainda pouco estudada em pacientes com IAM.
BACKGROUND: Intravascular ultrasound (IVUS) provides a sensitive and reproducible measurement of coronary artery dimensions, atherosclerotic plaque, and arterial lumen. Advances in IVUS technology now allow the characterization of the composition and morphology of atherosclerotic plaques. Although previous studies have reported data using IVUS radiofrequency analysis, the use of a new modality (iMap®, Boston Scientific, Santa Clara, USA) of atherosclerotic plaque characterization is very little known. Our objective is to analyze the morphological, phenotypic and tissue characteristics of "culprit" and "non-culprit" atherosclerotic plaques determined by angiography in patients undergoing coronary angiography due to acute myocardial infarction (AMI). METHODS: Prospective, cross-sectional, single center study (Hospital São Paulo - Escola Paulista de Medicina - Universidade Federal de São Paulo/UNIFESP, São Paulo, SP, Brazil). Fifty patients will be selected for ultrasound analysis according to the following inclusion criteria: age < 75 years, non-ST-segment elevation AMI or recent ST-segment elevation AMI, with or without previous fribrinolytic agents. CONCLUSIONS: The present study is aimed at obtaining morphological, phenotypic and tissue characteristics of the atherosclerotic plate using an imaging modality not extensively evaluated in patients with AMI.

OBJETIVO: Neste estudo, o objetivo foi revisar o papel de um possível processo inflamatório na gênese da esquizofrenia. MÉTODO: Foram selecionados os trabalhos publicados em revistas indexadas nas bases de dados Lilacs e MedLine, sob os unitermos "esquizofrenia", "inflamação" e "estresse oxidativo", nos últimos 10 anos até dezembro de 2009, nos idiomas inglês e português. Foram excluídos os artigos que tratavam de aspectos fisiopatológicos da doença fora do interesse da psiquiatria. RESULTADOS: Sessenta e um artigos foram selecionados. Doze abordavam o envolvimento do estresse oxidativo na esquizofrenia, nove tratavam de alterações no sistema imunológico de pacientes esquizofrênicos, dezesseis da infecção pré-natal como desencadeador da doença e sete mostravam a ação antioxidante e anti-inflamatória de fármacos antipsicóticos. CONCLUSÃO: Os estudos enfatizam o envolvimento do sistema imunológico (isto é, interleucinas e ação anti-inflamatória dos antipsicóticos), das infecções, do estresse oxidativo e da função mitocondrial na fisiopatologia da esquizofrenia. Portanto, esses novos achados são importantes para a melhor compreensão e, consequentemente, a elaboração de terapias mais específicas e eficazes no combate dessa doença mental.
OBJECTIVE: We aimed at reviewing about the influence of the inflammatory process in the genesis of schizophrenia. METHOD: A search for papers published in Lilacs and MedLine databases during the last 10 years until December 2009 was made using the terms "schizophrenia", "inflammation" and "oxidative stress". The papers concerning other pathophysiologic aspects of schizophrenia not exclusively related to psychiatry were excluded. RESULTS: Sixty-one articles were selected: twelve were involved the role of oxidative stress, nine dealt with changes in the immune system, and sixteen referred to prenatal infection as the trigger of schizophrenia. Seven articles showed the anti-inflammatory and antioxidant action of antipsychotic drugs. CONCLUSION: The studies emphasized the importance of the mitochondrial function, oxidative stress, immunological system (interleukin, anti-inflammatory action of the antipsychotics) and infections in the pathophysiology of schizophrenia. These findings are important for a better understanding and consequently the development of more specific and effective therapies for schizophrenia.
