ABSTRACT This article deals with the ideology of medical professionals in order to understand the meanings attributed to their profession, taking Thompson’s theory as a starting point and centering on the categories of autonomy, norms and values, and social recognition. Twenty-four semi-structured interviews were conducted with medical professionals in Amecameca, a municipality in the State of Mexico, from March to October 2019, in four sectors of service provision: public institutional, private institutional, private practice, and mixed practice. The analysis of meanings expressed by medical professionals with respect to each category of interest reveals that medical professionals are conditioned by the structure of state control and by the commodified economy, where private forms of medical practice are more highly regarded. In the ideological conformation of the medical profession, there remains a practical component imbued with a liberal romanticism in an extinct scenario that still exists today, without being articulated to more general social processes.

RESUMEN Se aborda la ideología de las y los profesionales de la medicina, desde la teoría de Thompson, para entender los significados de la profesión médica, a partir de las categorías: autonomía, normas y valores, y reconocimiento social. Se realizaron 24 entrevistas semiestructuradas a profesionales de la medicina en cuatro ámbitos de trabajo: institucional público, institucional privado, ejercicio privado y ejercicio mixto, en Amecameca, municipio del Estado de México, en el período comprendido de marzo a octubre de 2019. El análisis de los significados expresados por las y los profesionales de la medicina en cada una de las categorías de interés muestra una base condicionada por la estructura del control estatal y por la economía mercantilizada, que valora con mayor significación formas de práctica médica privada. En la conformación ideológica de la profesión médica subsiste un componente práctico inmerso en un romanticismo liberal en un escenario casi extinto que subsiste en la actualidad, sin articularlo a los procesos sociales más generales.

In this article the authors reflect on the consequences of assuming experience and subjectivity in order to understand and recognize passion and desire; as well as on the necessity of recognizing the critique of empiricism, cartesianism and neopositivism as starting points in constructing the process of meaning. Experience and subjectivity are inscribed in complex, interrelated and historically situated institutional groups, and global processes. The experience of instances or collective actions answer the excluding globalization process, new readings and new meanings about themselves and about the sense of their actions and their relationships with other subjects; that is, to produce subjectivities in a permanent resignificaction. Understanding experience leads us to the interpretation problematic; it implies the development of understanding forms that consider discourse as a dialectical process within meaning, as well as, between meaning and reference. Interpretation implies the consideration of the infuence that meanings generated in specific historic moments and exert on the social sciences.

En este artículo se reflexiona sobre las consecuencias de asumir que el entendimiento de la experiencia y la subjetividad implica el reconocimiento de la pasión y el deseo; así como sobre la necesidad de ubicar la crítica al empirismo, al cartesianismo y al neopositivismo como punto de partida para comprender los procesos de construcción de sentido. Experiencia y subjetividad se inscriben en procesos grupales, institucionales y globales complejos, interrelacionados e históricamente situados. La experiencia de formar parte de instancias y acciones colectivas que responden a procesos globalizatorios excluyentes, genera relecturas y resignificaciones de los propios sujetos sobre sí y sobre el sentido de su accionar y de sus relaciones con otros sujetos, es decir, produce subjetividades en permanente resignificación. Dar cuenta de la experiencia nos conduce a la problemática de la interpretación, lo cual implica desarrollar formas de comprensión que consideren el discurso como proceso dialéctico entre acontecimiento y sentido, así como entre sentido y referencia. La interpretación implica considerar la influencia que sobre las ciencias sociales ejercen los sentidos que se generan en momentos históricos determinados.

Background: Cerebral palsy involving spasticity is a highly handicapping illness, especially among children, the treatment of which is limited to reducing the handicaps involved without any full cure. There is a drug treatment which affords the possibility of providing patients with a better quality of life, regarding which a cost effectiveness analysis is made, note being made of the individual achievements and social benefits. Methods: This analysis is based on nationwide information from Mexico which was obtained from secondary sources. The base data is set out within the framework of trends in the growth of this health problem, and the calculations affording the possibility of sizing up this problem are employed in order to ascertain the benefits thereof based on the following indicators: relative impact of this disease and years of healthy life lost and gained, based on which the social and economic benefit entailed in the drug treatment under analysis is estimated. Results: The data reveals that the drug treatment in question is directly beneficial, because it lowers the relative impact of this disease from 0.92 to 0.40. At the individual level, the greatest benefit takes the form of the improved quality of life among the afflicted children within the 6-12 age range, they having gone from 0.08 years to 0.60 years of life in terms of quality. At the nationwide level, the 595,817 years of healthy life which would be lost without any treatment would be recouped. The monetary value involved in this treatment is that of 2,725.00 Mexican pesos (US $283.00) for administering the drug over a one-year period to each child suffering from this disability Conclusions: This is a low-cost treatment, because it is currently impossible to reverse the neurological and physiological processes of cerebral palsy, this treatment therefore improving the quality of life of the children afflicted with this disease is the best possible benefit. The cost effectiveness analysis taking into account these indicators reveals a clear-cut relationship between the monetary values and the health benefits.

Fundamento: La parálisis cerebral con espasticidad produce un alto grado de minusvalía, especialmente en los niños/as menores, su tratamiento se circunscribe a la disminución de las incapacidades pero no se cura totalmente. Existe una intervención farmacológica que permite una calidad de vida mejor, por lo cual se hace una evaluación de costo-beneficio de esa intervención, observando los logros individuales y los beneficios sociales. Métodos: El punto de partida es información nacional de México, obtenida de fuentes secundarias. A los datos base se les proyecta en escenarios tendenciales de crecimiento del problema de salud y se le aplican los cálculos que permiten dimensionar el problema, para derivar sus beneficios a partir de los indicadores: peso de la enfermedad y años de vida saludables perdidos y ganados, con lo cual se estima el beneficio social y económico que significa la intervención farmacológica que se analiza. Resultados: Los datos muestran que la intervención farmacológica produce un beneficio inmediato porque se disminuye el peso de la enfermedad de 0,92 a 0,40. En el nivel individual el máximo beneficio se expresa en el incremento de la calidad de vida de los menores entre 6 y 12 años que padecen la minusvalía, al pasar de 0,08 a 0,60 años de vida ajustados por calidad. En el nivel nacional se recuperarían 595.817 años de vida saludables, que serían perdidos al realizar la intervención. El valor monetario que significa la intervención corresponde a $ 2.725.00 pesos mexicanos ($ 283.00 dólares de Estados Unidos de América) por administración del medicamento durante un año a cada niño con la minusvalía. Conclusiones: La intervención significa un coste bajo, porque revertir los procesos neurológicos y fisiológicos de la parálisis cerebral es imposible en la actualidad, por lo tanto incrementar la calidad de vida de los menores enfermos es el máximo beneficio posible. El análisis coste-beneficio considerando estos indicadores ofrece una clara relación entre valores monetarios y beneficios a la salud.

BACKGROUND: The Hegemonic Medical Model is discussed and is articulated with the postulates of the sociology of the professions. The information takes Aids as a specific topic, because it involves the aspects of recent scientific development, of clinical and epidemiological importance and due to the research which is being done for its treatment and the production of immunizations. METHOD: This is a case study of students enrolled in different years of study for medical degrees at the three main universities in México City. A closed-question questionnaire was handed out, checked and corrected, the variables of which were related to three types of knowledge, that is, basic, technical and general. RESULTS: The students were revealed to master this subject to only a minor extent the extentions of the knowledge of the disciplines of the profession however being applied to new cases without the suitable accuracy. A certain degree of dissociation was found to exist among the different types of knowledge which were researched, the professional know-how having been noted to clearly conform within specialized, segmented, curative logic of the Hegemonic Medical Model. CONCLUSIONS: The technical, problem-solving knowledge employed in clinical use prevails over the systematic, abstract knowledge of the general knowledge of medicine. A clear notion of the processes based on the dominant medical pattern is revealed in the single-causal relations of the disease. There is an extension of the basic knowledge a the resolving of clinical problems. The medical students are oriented toward taking in knowledge which will be useful to them in clinical practice.

FUNDAMENTO: Se considera el modelo médico hegemónico y se le articula con los postulados de la sociología de las profesiones. La información tiene al sida como tema concreto, porque presenta características de avance científico reciente, de importancia clínica y epidemiológica y por la investigación que se realiza para su tratamiento y la producción de vacunas. MÉTODO: Es un estudio con estudiantes de distintos cursos de la carrera de medicina de las tres principales universidades del país. Se aplicó un cuestionario con preguntas cerradas que se probó y corrigió; las variables se refieren a tres tipos de conocimiento: básico, técnico y general. RESULTADOS: Entre los 120 estudiantes que respondieron al cuestionario hubo predominio del tercer (30%) y cuarto (32%) años de estudios. El 54% eran del sexo femenino. El primer año de estudios, con un intervalo de confianza del 95%, presenta diferencias significativas (p<0.05) con respecto de los estudiantes de 3° y 4° años de estudio. Al considerar la categorización por tipo de conocimiento se encontró que el conocimiento técnico tuvo mayor porcentaje de aciertos (51%), seguido del conocimiento básico y el conocimiento general. El total de las respuestas a las 16 preguntas alcanzó una media de 42.7% de aciertos. CONCLUSIONES: Mostrando poco dominio sobre el tema, los conocimientos técnicos, resolutivos, de aplicación clínica, predominan sobre los sistemáticos, abstractos, del conocimiento general de la medicina. Se presenta una clara noción de los procesos con base en el modelo médico hegemónico, en las relaciones unicausales de la enfermedad. Existe una extensión de los conocimientos básicos a la resolución de problemas clínicos. Los estudiantes de medicina se orientan a la asimilación de conocimientos que les resulten útiles en la práctica clínica
