ABSTRACT: This study assessed the efficacy of an intra-articular injection of allogeneic adipose tissue-derived from mesenchymal stem cells (AD-MSCs) for the treatment of hip dysplasia in dogs. The study group included 12 otherwise healthy dogs of different breeds, ages, weights, and degrees of hip dysplasia diagnosed using radiography. An orthopedic assessment was performed on all dogs before and at 30, 60, and 90 days after infusion of AD-MSCs(2 × 106cells). On the same days, each dog’s owner answered a questionnaire based on theHelsinkiChronic Pain Index. The data were converted to ordinal data based on the score for each variable, and the Friedman test for multiple comparisons was used to verify the results. Compared with the corresponding values on day 0, orthopedic and gait assessments and owners’ reported pain indexes improved over the 90-day observation period. These results suggested that treatment with allogeneic AD-MSCs significantly reduced the clinical signs associated with hip dysplasia during the study period. However, long-term studies are needed to determine the optimal therapeutic protocol for routine clinical use of AD-MSCs in hip dysplasia. RESUMO: O objetivo deste estudo foi avaliar a eficácia clínica da aplicação intra-articular de células-tronco mesenquimais alógenas derivadas do tecido adiposo (AD-CTM) no tratamento de cães portadores de osteoartrite secundária a displasia do coxofemoral (DCF). Doze cães de ambos os sexos, diferentes raças, idades e peso, portadores de graus variados de DCF comprovada em radiografia e livres de quaisquer outras alterações clínicas ou ortopédicas, foram utilizados no estudo. Todos os cães foram submetidos a avaliação ortopédica nos dias 0, 30, 60 e 90 após aplicação de AD-CTM na dose de 2 x 106. Além disso, os tutores preencheram a um questionário, baseado no Índice de dor crônica de Helsinque (IDCH) nos mesmos intervalos. Em comparação com o dia 0, observou-se melhora significativa na avalição em locomoção e físico-ortopédica assim como na avaliação dos tutores pelo IDCH ao longo dos 90 dias. Os resultados permitem inferir que as AD-CTM alógenas contribuíram significativamente para a redução dos sinais clínicos comumente associados a DCF durante o período de estudo. Entretanto, há necessidade de estudos de longo prazo para melhor determinação de protocolos terapêuticos baseados no uso de AD-CTM na rotina clínica. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. ABSTRACT Objective Consequently, in this study, we aimed to evaluate 1,203 cases of referral to a quaternary vascular surgical service, in São Paulo, Brazil, over a 6-year period, to assess the appropriate need for referral; in addition to the prevalence of surgical indications. Methods In this retrospective analysis, we reviewed the institutional records of participants referred from Basic Healthcare Units to a vascular surgical service inside the Brazilian Unified Health System, between May 2015 and December 2020. Demographic and clinical data were collected. The participants were stratified, as per the reason for referral to the vascular surgical service, previous imaging studies, and surgical treatment indications. Referral appropriateness and complementary examinations were evaluated for each disease cohort. Finally, the prevalence of cases requiring surgical treatment was defined as the outcome measure. Results Of the 1,203 referrals evaluated, venous disease was the main reason for referral (53%), followed by peripheral arterial disease (19.4%). A considerable proportion of participants had been referred without complementary imaging or after a long duration of undergoing an examination. Referrals were regarded as inappropriate in 517 (43%) cases. Of these, 32 cases (6.2%) had been referred to the vascular surgical service, as the incorrect specialty. The percentage of referred participants who ultimately underwent surgical treatment was 39.92%. Carotid (18%) and peripheral arterial diseases (18.4%) were correlated with a lower prevalence of surgical treatments. Conclusion The rate of referral appropriateness to specialized vascular care from primary care settings was low. Abstract Background and objective: Lower limb varicose veins are a prevalent disease associated with several available treatment options, including conventional surgery and polidocanol foam sclerotherapy. However, few studies have analyzed therapeutic modality outcomes based on Patient-Reported Outcome Measures (PROMs). This large sample-size study was designed to evaluate the outcomes of polidocanol foam sclerotherapy compared to conventional surgery based on an analysis of PROMs. Methods: This was a prospective, observational, and qualitative study of 205 patients who underwent varicose vein treatment with either polidocanol foam sclerotherapy (57 patients, 90 legs) or conventional surgery (148 patients, 236 legs). Patients were preoperatively assessed and re-evaluated 30 days after the procedure using the Venous Disease Severity Score (VCSS) and specific venous disease quality-of-life questionnaires (VEINES-QoL/Sym). Results: Both treatments significantly improved VCSS and VEINES results 30 days after the procedure (p < 0.05). However, surgery promoted greater improvements in VCSS (on average 4.02-points improvement, p < 0.001), VEINES-QoL (average 8-points improvement, p < 0.001), and VEINES-Sym (average 11.66 points improvement, p < 0.001) than did sclerotherapy. Postoperative pain and aesthetic concerns about the legs were the domains of the questionnaires in which the results varied the most between the treatment modalities, with worse results for sclerotherapy. ABSTRACT The focus of this case report is to technically describe a noninvasive diagnostic evaluation of bow Hunter's syndrome using a dynamic computed tomography angiography protocol and discuss its advantages. In addition, we aimed to exemplify the quality of the study by presenting images of a 3D-printed model generated to help plan the surgical treatment for the patient. The dynamic computed tomography angiography protocol consisted of a first image acquisition with the patient in the anatomic position of the head and neck. This was followed by a second acquisition with the head and neck rotated to the side that triggered the symptoms, with technical parameters similar to the first acquisition. The acquired images were used to print a 3D model to better depict the findings for the surgical team. The dynamic computed tomography angiography protocol developed in this study helped visualize the vertebrobasilar arterial anatomy, detect vertebral artery stenosis produced by head and neck rotation, depict the structure responsible for artery stenosis (e.g., bony structure or membranes), and study possible complications of the disease (e.g., posterior cerebral circulation infarction). Additionally, the 3D-printed model better illustrated the findings of stenosis, aiding in surgical planning. In conclusion, dynamic computed tomography angiography for the evaluation of bow Hunter's syndrome is a feasible noninvasive technique that can be used as an alternative to traditional diagnostic methods. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Background: Patients with peripheral arterial disease have an increased risk of developing cardiovascular complications in the postoperative period of arterial surgeries known as Major Adverse Cardiac Events (MACE), which includes acute myocardial infarction, heart failure, malignant arrhythmias, and stroke. The preoperative evaluation aims to reduce mortality and the risk of MACE. However, there is no standardized approach to performing them. The aim of this study was to compare the preoperative evaluation conducted by general practitioners with those performed by cardiologists. Methods: This is a retrospective analysis of medical records of patients who underwent elective arterial surgeries from January 2016 to December 2020 at a tertiary hospital in São Paulo, Brazil. The authors compared the preoperative evaluation of these patients according to the initial evaluator (general practitioners vs. cardiologists), assessing patients’ clinical factors, mortality, postoperative MACE incidence, rate of requested non-invasive stratification tests, length of hospital stay, among others. Results: 281 patients were evaluated: 169 assessed by cardiologists and 112 by general practitioners. Cardiologists requested more non-invasive stratification tests (40.8%) compared to general practitioners (9%) (p < 0.001), with no impact on mortality (8.8% versus 10.7%; p = 0.609) and postoperative MACE incidence (10.6% versus 6.2%; p = 0.209). The total length of hospital stay was longer in the cardiologist group (17.27 versus 11.79 days; p < 0.001). Conclusion: The increased request for exams didn’t have a significant impact on mortality and postoperative MACE incidence, but prolonged the total length of hospital stay. Abstract Marginal reefs can provide meaningful information about the structure and dynamics of ecosystems under suboptimal environmental conditions. In addition to their different characteristics, these environments can also occur in urbanized areas. In this note, we characterize marine animal forests (MAFs) on turbid-zone reefs on an urban coast in the equatorial southwestern Atlantic. Overall, the sandstone ferruginous reefs (6-10 m depth) exhibited a flat topography and gentle slope (1-2 m above the seabed). Benthic cover is composed mainly of sponge gardens and ascidians. In addition, we found a low-relief coral carpet with only one massive reef-building coral (Siderastrea stellata) and Zoantharia. The ascidians and sponges had a higher diversity (at least 15 species) than the cnidarians (two species) in these forests. The main animals forming this seascape are weedy and stress-tolerant species adapted to challenging environmental conditions, such as swell waves, mesotidal regimes, moderate turbidity waters, and periodic burial. In this regard, these conditions and human impacts have shaped a unique MAF. Remarkably, the studied formations seem similar to high-latitude marginal reefs or low-latitude reefs under the influence of upwelling, which sustains soft corals and non-framework building coral communities along with sponges and ascidians. In particular, the shallow MAFs along the semi-arid coast of Brazil seem to lack some of the characteristics of low-latitude reefs under high sedimentation, whose structure was described as coral rubble within sedimentary matrices. This suggests that factors other than periodic burials and low light availability affected these MAFs. These overlooked forests are widespread in this area and have been neglected in studies, despite their richness (> 31 taxa) and valuable ecosystem goods and services. Abstract We evaluate how the concentrations of inorganic nutrients and chlorophyll a vary in a heterogeneous area (Equatorial SW Atlantic), covering a gradient from stations closer to the coast to others more distant associated or not with turbid-zone reefs. Vertical temperature (27.9 ± 0.10 °C; mean ± standard deviation) and salinity (36.2 ± 0.14) profles showed that the water column is well mixed (0-30 m depth). The oligotrophic condition was marked by low concentrations of phosphate (0.30 ± 0.22 µM) and dissolved inorganic nitrogen (0.64 ± 0.74 µM). Moreover, dissolved reactive silicon (DSi) was low in most samples (< 2.0 µM), but higher (>10 µM) in nearshore stations, probably related to continental runoff and/or resuspension of the bottom sediments. The pelagic phytoplankton biomass indicated that chlorophyll a (0.25 ± 0.08 µg L-1) was low, positively correlated with light and negatively correlated with nutrients, indicating possible phytoplankton uptake. Chlorophyll a concentrations were lower in stations closer to the coast and higher in some stations near the reefs, indicating that the latter could be more prone to phytoplankton development and depletion of nutrients, especially DSi. Therefore, although oligotrophy is present along this coast, we found some unexpected heterogeneity of nutrient and chlorophyll a distributions, which were probably infuenced by benthic-pelagic coupling due to the presence of extensive reefs, sponge gardens (18-30 m depth), and the proximity to the coast. These results highlight the importance of understanding the heterogeneity of ocean productivity, especially in lesser known low-latitude areas, which showed distinct nutrient and chlorophyll a levels related to the occurrence of tropical reefs that are capable of supporting important fsh stocks and unique biological communities. Abstract In the Amazon, local electrical energy production is very important due to the large number of isolated communities, usually far from the conventional grid. In this context, steam turbines are relevant technologies to be used in the region, since waste biomass is widely available in several communities, besides, where sustainable agricultural activities take place, such as the use of açaí seeds, sugarcane bagasse, among others. Currently, there are not many relevant studies in the literature for small steam turbines operating at low pressure, conditioned to the reality of the Amazon region. Hence, the main goal of this work is to develop an experimental study of a small steam turbine operating at low pressure, in order to apply it to small energy demand, typically found in the Amazon. We developed measurements on the behavior of mechanical and electrical powers in relation to the shaft rotational speed of the small steam turbine, taking into account pressures of 0.1, 0.2 and 0.3 MPa. Resumo Na Amazônia, a produção local de energia elétrica é muito importante devido ao grande número de comunidades isoladas, geralmente distantes da rede convencional. Nesse contexto, as turbinas a vapor são tecnologias relevantes a serem utilizadas na região, uma vez que a biomassa está amplamente disponível em diversas comunidades, inclusive, onde ocorrem atividades agrícolas sustentáveis como a utilização do caroço do açaí, bagaço de cana, entre outros. Atualmente, não existem muitos estudos relevantes na literatura para pequenas turbinas a vapor operando em baixa pressão, condicionadas à realidade da região amazônica. Assim, o objetivo principal deste trabalho é desenvolver um estudo experimental de uma pequena turbina a vapor operando em baixa pressão, a fim de aplicá-la a pequenas demandas de energia, tipicamente encontradas na região. Desenvolvemos medições do comportamento das potências mecânica e elétrica em relação à velocidade de rotação do eixo da pequena turbina a vapor, levando em consideração as pressões de 0,1, 0,2 e 0,3 MPa. Concluímos que o levantamento experimental preliminar, feito neste trabalho, demonstra um bom comportamento mecânico da pequena turbina e pode ser uma alternativa para o sistema de geração de energia elétrica suprir pequenas demandas em comunidades isoladas na Amazônia, aproveitando a biomassa oriunda de atividades agrícolas sustentáveis.
