Abstract Essential oil (EO) of Cannabis sativa (C. sativa) was evaluated against the egg, larval, pupal, and adult stages of the flea Ctenocephalides felis felis. The chemical composition of EO was determined by gas chromatography with flame ionization and mass spectrometry. EO mainly comprised γ-elemene (16.2%) and caryophyllene oxide (14.2%) as major compounds. To evaluate the mortality of flea stages in vitro, filter paper tests were performed at different concentrations. EO of C. sativa showed insecticidal activity (100% mortality at the highest concentrations) for flea control at egg, larval, pupal, and adult stages, with lethal concentrations (LC50) of 32.45; 91.61; 466.41 and 927.92 μg/cm2, respectively. EO of C. sativa indicated the potential for the development of ectoparasiticide for veterinary use, especially for fleas in egg and larval stages.
Resumo O óleo essencial (OE) de Cannabis sativa (C. sativa) foi avaliado contra os estágios de ovo, larva, pupa e adulto da pulga Ctenocephalides felis felis. A composição química do OE foi determinada por cromatografia gasosa com ionização de chama e espectrometria de massa. O OE foi composto principalmente de γ-elemeno (16,2%) e óxido de cariofileno (14,2%) como compostos majoritários. Para avaliar a mortalidade dos estágios de pulgas in vitro, foram realizados testes de papel filtro em diferentes concentrações. O OE de C. sativa apresentou atividade inseticida (100% de mortalidade nas maiores concentrações), para controle de pulgas nos estágios de ovo, larva, pupa e adulto, com concentrações letais (CL50) de 32,45; 91,61; 466,41 e 927,92 μg/cm2, respectivamente. O OE de C. sativa indicou potencial para o desenvolvimento de ectoparasiticida para uso veterinário, principalmente para pulgas em fase de ovo e larva.

ABSTRACT Subsistence hunting is the main source of protein for forest reserve dwellers, contributing to the development of spurious infections by Calodium hepaticum, frequently associated with the consumption of the liver from wild mammals. The prevalence of infections by soil-transmitted helminths (STHs) and intestinal protozoa is considered an indicator of the social vulnerability of a country, besides providing information on habits, customs and quality of life of a given population. Intestinal parasites mostly affect poor rural communities with limited access to clean water and adequate sanitation. This study reports the results of a parasitological survey carried out in 2017 and 2019, in two municipalities (Xapuri and Sena Madureira) in Acre State. Stool samples were collected from 276 inhabitants. Upon receipt, each sample was divided into two aliquots. Fresh samples without preservative were processed and examined by the Kato-Katz technique. Samples fixed in 10% formalin were processed by the spontaneous sedimentation and the centrifugal sedimentation techniques. Calodium hepaticum eggs were found in three stool samples. The overall STH prevalence was 44.9%. The hookworm prevalence (19.2%) was higher than that of Ascaris lumbricoides (2.5%) and Trichuris trichiura (0.7%), an unexpected finding for municipalities belonging to the Western Brazilian Amazon. When considering parasites transmitted via the fecal-oral route, Endolimax nana and Entamoeba coli showed the highest positivity rates, of 13% and 10.9%, respectively. This study is the first report of spurious infection by C. hepaticum among forest reserve dwellers that consume undercooked liver of lowland pacas. Additionally, this is the first report of Blastocystis sp. in Acre State.

OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

Experimental and Theoretical Study of LiMn2O4 Synthesized by the Solution Combustion Method Using Corn Starch as Fuel

Lithium manganese oxide, LiMn2O4, was synthesized in two temperature stages, where the first consisted by an ecofriendly solution combustion method at 300 °C. Finally, the as-burned powders were thermal treated at 500 and 700 °C. The structural and morphological changes were evaluated by the Rietveld method and density functional theory (DFT) calculations. The Rietveld refinement indicates obtaining the spinel cubic phase LiMn2O4 and a small amount of Mn2O3. The analyses by scanning electron microscopy (SEM) and transmission electron microscopy (TEM) show a porous microstructure composed of nano-sized crystallites for the sample treated at 500 °C. In cyclic voltammetry, it was possible to observe that the reduction-oxidation reaction is reversible due to the shape of voltammograms and the anodic and cathodic peaks of Mn ions. The theoretical calculations considered the experimental crystallographic parameters. The unit cell volume change was evaluated according to distinct amounts of lithium ions in the structure. The removal of the Li+ cations from the oxides promotes a volume contraction. Therefore, it was possible to evaluate the participation of the Mn3+ ions in the frontier region between the valence and conduction bands. The density of states (DOS) calculation shows a predominant contribution of the O 2p and Mn 3d orbitals in the frontier orbitals.

Synthesis of Nerol Derivatives Containing a 1,2,3-Triazole Moiety and Evaluation of Their Activities against Cancer Cell Lines

In the present investigation, a collection of twenty two nerol derivatives, containing 1,2,3-triazolic appendages, was synthesized and screened in vitro for their cytotoxic activity against HL60, Nalm6, and Jurkat human leukemia cells as well as against B16F10 (melanoma cell line). In most cases, derivatives were able to reduce cell viability. The most potent compound (Z)-4-(((3,7-dimethylocta-2,6-dien-1-yl)oxy)methyl)-1-(4-(trifluoromethoxy)benzyl)-1H-1,2,3 triazole showed antiproliferative activity against Jurkat cells and reduced B16F10 cell migration. Physicochemical properties of the compounds were calculated in order to evaluate their potential for drug development. Most of the evaluated physicochemical parameters seemed to be favorable for drug development. In addition, for a better understanding of the biological activity results, 3D quantitative structure-activity relationship (QSAR) studies were carried out. 3D-QSAR studies indicate that the anticancer activities observed for the cell lines HL60 and Jurkat may occur by a similar mechanism of action and the same was found for the Nalm6 and B16F10 cell lines.

Resumo Fundamento: O implante valvar aórtico transcateter (TAVI) está bem estabelecido, porém, o bloqueio atrioventricular, com necessidade de marca-passo definitivo (MPD), é complicação frequente. Objetivo: Determinar a incidência, preditores e desfechos clínicos de implante de MPD após TAVI, focando em como a evolução da fração de ejeção do ventrículo esquerdo (FEVE), após o TAVI, é afetada pelo MPD. Métodos: No registro brasileiro foram incluídos 819 pacientes submetidos a TAVI por estenose aórtica severa em 22 centros entre janeiro/2008 e janeiro/2015. Após exclusões, os preditores de implante de MPD foram avaliados em 670 pacientes por regressão multivariada. Análise da curva ROC foi utilizada para medir a habilidade dos preditores; p < 0,05 foi considerado significativo. Resultados: Aos 30 dias, 135 pacientes (20,1%) necessitaram de MPD. Tais pacientes eram mais velhos (82,5 vs. 81,1 anos; p = 0,047) e predominantemente homens (59,3% vs 45%; p = 0,003). A permanência hospitalar foi maior no Grupo MPD (média= 15,7 ± 25,7 vs. 11,8 ± 22,9 dias; p < 0,001), mas o implante não afetou morte por qualquer causa (26,7% vs. 25,6%; p = 0,80) nem morte cardiovascular (14,1% vs. 14,8%; p = 0,84). Por análise multivariada, a presença prévia de bloqueio de ramo direito (BRD) (OR, 6,19; 3,56–10,75; p ≤ 0,001), o uso da prótese CoreValve® (OR, 3,16; 1,74–5,72; p ≤ 0,001) e gradiente transaórtico basal > 50 mmHg (OR, 1,86; 1,08–3,2; p= 0,025) foram preditores de implante de MPD. O risco estimado de implante de MPD foi 4%, quando nenhum dos fatores de risco estava presente, e 63% na presença de todos. O modelo mostrou boa habilidade de prever a necessidade de MPD: 0,69 (IC95%: 0,64 – 0,74) na curva ROC. Subestudo de 287 ecocardiogramas durante o seguimento de 1 ano mostrou pior evolução da FEVE no Grupo MPD (p = 0,01). Conclusão: Em crianças de 6 a 11 anos, circunferência da cintura aumentada está associada à PA elevada, mesmo quando o IMC é normal. (Arq Bras Cardiol. 2017; [online].ahead print, PP.0-0)
Abstract Background: Transcatheter aortic valve implantation (TAVI) is a well-established procedure; however, atrioventricular block requiring permanent pacemaker implantation (PPI) is a common complication. Objectives: To determine the incidence, predictors and clinical outcomes of PPI after TAVI, focusing on how PPI affects left ventricular ejection fraction (LVEF) after TAVI. Methods: The Brazilian Multicenter TAVI Registry included 819 patients submitted to TAVI due to severe aortic stenosis from 22 centers from January/2008 to January/2015. After exclusions, the predictors of PPI were assessed in 670 patients by use of multivariate regression. Analysis of the ROC curve was used to measure the ability of the predictors; p < 0.05 was the significance level adopted. Results: Within 30 days from TAVI, 135 patients (20.1%) required PPI. Those patients were older (82.5 vs. 81.1 years; p = 0.047) and mainly of the male sex (59.3% vs 45%; p = 0.003). Hospital length of stay was longer in patients submitted to PPI (mean = 15.7 ± 25.7 vs. 11.8 ± 22.9 days; p < 0.001), but PPI affected neither all-cause death (26.7% vs. 25.6%; p = 0.80) nor cardiovascular death (14.1% vs. 14.8%; p = 0.84). By use of multivariate analysis, the previous presence of right bundle-branch block (RBBB) (OR, 6.19; 3.56-10.75; p ≤ 0.001), the use of CoreValve® prosthesis (OR, 3.16; 1.74-5.72; p ≤ 0.001) and baseline transaortic gradient > 50 mm Hg (OR, 1.86; 1.08-3.2; p = 0.025) were predictors of PPI. The estimated risk of PPI ranged from 4%, when none of those predictors was present, to 63%, in the presence of all of them. The model showed good ability to predict the need for PPI: 0.69 (95%CI: 0.64 - 0.74) in the ROC curve. The substudy of 287 echocardiograms during the 1-year follow-up showed worse LVEF course in patients submitted to PPI (p = 0.01). Conclusion: BRD prévio, gradiente aórtico médio > 50 mmHg e CoreValve® são preditores independentes de implante de MPD pós-TAVI. Ocorreu implante de MPD em aproximadamente 20% dos casos de TAVI, o que prolongou a internação hospitalar, mas não afetou a mortalidade. O implante de MPD afetou negativamente a FEVE pós-TAVI.

O objetivo deste estudo é analisar o perfil das trabalhadoras domésticas brasileiras em Massachusetts, Estados Unidos, valendo-se da descrição de suas condições de trabalho e percepções a respeito do seu estado de saúde. Trata-se de estudo transversal com 198 domésticas de Massachusetts, recrutadas por amostragem do tipo "bola-de-neve". O instrumento utilizado abordou características demográficas, condições de trabalho e percepção do estado de saúde dos participantes. Os dados foram analisados por meio do software SPSS 21.0. Dentre os entrevistados, 95,5% eram mulheres, 62,1% com idade entre 30-49 anos e 55,6% não legalizados. Foram observadas diferenças estatisticamente significativas entre participantes legalizadas e não legalizadas em variáveis sociodemográficas, de condições de trabalho e de saúde. O status imigratório irregular parece afetar negativamente as condições de vida e saúde das trabalhadoras domésticas.
Abstract: Brazilian immigrants in the United States experience various social, labor, and health challenges. This study aimed to analyze the profile of female Brazilian domestic workers in Massachusetts, USA, through a description of their working conditions and self-rated health. This was a cross-sectional study of 198 domestic workers in Massachusetts, recruited with "snowball" sampling. The instrument addressed participants' demographic characteristics, work conditions, and self-rated health. Data were analyzed with SPSS 21.0. Among the interviewees, 95.5% were women, 62.1% were 30 to 49 years of age, and 55.6% were undocumented. Documented and undocumented participants showed statistically significant differences in demographics, work conditions, and health. Irregular immigrant status appears to have a negative impact on domestic workers' living and health conditions.
Resumen: Los inmigrantes brasileños han vivido diversos desafíos en los ámbitos social, laboral y de salud en los Estados Unidos. El objetivo de este estudio es analizar el perfil de las trabajadoras domésticas brasileñas en Massachusetts, Estados Unidos, valiéndose de la descripción de sus condiciones laborales y percepciones respecto a su estado de salud. Se trata de un estudio transversal con 198 empleadas domésticas de Massachusetts, reclutadas por un muestreo tipo "bola de nieve." El instrumento utilizado abordó características demográficas, condiciones de trabajo y percepción del estado de salud de los participantes. Los datos se analizaron mediante el software SPSS 21.0. Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Introdução: O implante por cateter de prótese aórtica (TAVI, do inglês transcatheter aortic valve implantation) constitui tratamento alternativo para pacientes com estenose aórtica de alto risco cirúrgico ou inoperáveis. Para adquirir competência, o grupo multidisciplinar deve receber treinamento específico e acumular experiência na execução do TAVI. Contudo, sua curva de aprendizado não está bem estabelecida. Nosso objetivo foi analisar o impacto da curva de aprendizado na seleção de pacientes, nos aspectos técnicos e nos resultados clínicos do TAVI. Métodos: Estudo observacional e prospectivo dos primeiros 150 pacientes submetidos a TAVI por via femoral, entre janeiro de 2009 e dezembro de 2013 divididos em tercis (n = 50) de acordo com a data do procedimento. Os desfechos foram definidos conforme os critérios Valve Academic Research Consortium-2 (VARC-2). Resultados: A idade foi de 82,5 ± 6,7 anos, sendo 44% homens e 75% em classe NYHA III/IV. O EuroSCORE (24,2 ± 13% vs. 21,2 ± 10,8% vs. 23,4 ± 14,3%) e o STS Score (5,9 ± 2,9% vs. 6,7 ± 4,3% vs. 5,8 ± 3,1%) foram similares entre os grupos. Observou-se redução gradativa nos tempos do procedimento (107,2 ± 48,1 minutos vs. 90,3 ± 42,2 minutos vs. 76,6 ± 37,7 minutos; p < 0,01) e de fluoroscopia (31,3 ± 9,6 minutos vs. 25,4 ± 8,7 minutos vs. 17,2 ± 6,2 minutos; p = 0,01), e no volume de contraste (145,5 ± 70,9 mL vs. 123,2 ± 87,8 mL vs. 101,1 ± 50 mL; p = 0,01). A mortalidade reduziu-se de forma progressiva (20% vs. 10% vs. 4%; p = 0,047), e menores taxas de sangramentos e de refluxo paraprotético de grau moderado/grave foram observadas no terceiro tercil (14% e 4%, respectivamente). Não ocorreram casos de reintervenção, acidente vascular cerebral e nem outros eventos clínicos entre a alta hospitalar e o 30º dia pós-implante. Conclusões: A competência para o TAVI aumentou progressivamente com o número de pacientes tratados, associando-se a melhores desfechos clínicos.

A substituição cirúrgica da valva aórtica é o tratamento de eleição para pacientes com estenose aórtica sintomática acentuada. Entretanto, o risco cirúrgico aumenta expressivamente com o avançar da idade e com a associação de comorbidades. Nessa situação clínica, o implante por cateter de bioprótese valvar aórtica deve ser considerado uma alternativa terapêutica válida e eficaz. O propósito deste Consenso é oferecer uma revisão crítica do conjunto de evidências e dos avanços mais recentes relacionados ao implante por cateter de bioprótese valvar aórtica e sumarizar o estado da arte atual, com o intuito de melhor guiar a tomada de decisão terapêutica. A seleção dos pacientes para o implante por cateter de prótese valvar aórtica deve obedecer aos seguintes critérios: 1) presença de estenose valvar aórtica acentuada, sintomática, definida como orifício valvar aórtico com área < 1 cm² (ou < 0,6 cm²/m²); 2) idade > 75 anos e alta probabilidade de morbidade e mortalidade cirúrgicas pela presença de comorbidades; e 3) presença de condição anatômica e morfológica favorável para o procedimento por cateter. As evidências científicas atuais indicam que esse procedimento melhora os sintomas e reduz a mortalidade de pacientes com estenose aórtica e com contraindicação cirúrgica, quando comparado ao tratamento conservador. Adicionalmente, em pacientes com estenose aórtica acentuada e alto risco cirúrgico, o implante por cateter apresenta-se como alternativa terapêutica eficaz, capaz de alterar o curso natural da doença de forma equivalente ao tratamento cirúrgico convencional.
Surgical replacement of the aortic valve is the therapy of choice for patients with severe symptomatic aortic stenosis. However, surgical risk increases significantly with age and with the association of comorbidities. In this clinical context, transcatheter aortic-valve implantation must be considered as a valid and effective therapeutic alternative. The purpose of this Consensus is to provide a critical review of all the evidence and recent advances related to transcatheter aortic-valve implantation and summarize the current state of the art, in order to better guide therapeutic decision-making. Patient selection for transcatheter aortic-valve implantation must meet the following criteria: 1) presence of severe symptomatic aortic valve stenosis, defined as aortic valve orifice area < 1 cm² (or < 0.6 cm²/m²); 2) age > 75 years and a high probability of surgical morbidity and mortality due to pre-existing comorbidities; and 3) the presence of anatomical and morphological conditions favorable for transcatheter interventions. Current scientific evidence indicates that this procedure improves symptoms and reduces mortality in patients with aortic stenosis who were not suitable candidates for surgery, as compared to standard therapy. Additionally, in patients with severe aortic stenosis and high surgical risk, transcatheter aortic-valve implantation is an alternative effective treatment, able to alter the natural course of disease in a manner equivalent to conventional surgical treatment.
