Resumo Fundamento: O fondaparinux é um anticoagulante eficaz e seguro usado no tratamento de síndromes coronarianas agudas (SCAs). No entanto, devido à baixa representatividade de indivíduos obesos em ensaios clínicos, os efeitos de se aplicar os resultados desse medicamento nesta população continuam incertos. Objetivos: Comparar o fondaparinux à enoxaparina no tratamento de obesos com SCA. Métodos: Este é um estudo do tipo coorte retrospectivo, incluindo indivíduos obesos (IMC ≥ 30 Kg/m2) internados com Infarto do Miocárdio sem Elevação do Segmento ST (IAMSSST) ou Angina Instável (AI) e tratados com fondaparinux ou enoxaparina entre 2010 e 2020. Os grupos que receberam fondaparinux e enoxaparina foram comparados quanto suas características clínicas e laboratoriais usando o teste do qui-quadrado e o teste de Mann-Whitney, conforme apropriado. A incidência dos desfechos primários (morte, reinfarto, acidente vascular cerebral, sangramento maior) foi comparada entre os grupos. Um p<0,05 foi considerado estatisticamente significativo em todas as análises. Resultados: Um total de 367 pacientes obesos com IAMSSST ou AI foi incluído, dos quais 258 usaram fondaparinux e 109 usaram enoxaparina. A idade média foi 64 ± 12 anos, 52,9% eram do sexo masculino. A prevalência e diabetes, hipertensão, dislipidemia, doença arterial coronariana prévia, acidente vascular cerebral prévio, e implementação de estratégia invasiva foi similar entre os grupos. A incidência do desfecho primário foi 4,7% no grupo fondaparinux e 5,5% no grupo enoxaparina (p = 0,729). Não houve diferença entre os grupos quando os componentes do desfecho primário foram analisados separadamente. Conclusão: Em uma amostra de pacientes obesos com IAMSSST ou AI, não houve diferença na ocorrência do desfecho composto (morte, acidente vascular cerebral, reinfarto, sangramento maior) entre os pacientes que utilizaram fondaparinux ou enoxaparina.

Abstract Background: Fondaparinux is an effective and safe anticoagulant in the treatment of acute coronary syndromes (ACS). However, due to the low representation of obese individuals in clinical trials, the effects of applying the results of this drug to this population remain uncertain. Objectives: To compare Fondaparinux to Enoxaparin in the treatment of obese patients with ACS. Methods: This is a retrospective cohort study, including obese individuals (BMI ≥ 30 Kg/m2) admitted with non-ST-segment elevation myocardial infarction (NSTEMI) or unstable angina (UA) and treated with Fondaparinux or Enoxaparin between 2010 and 2020. The Fondaparinux and Enoxaparin groups were compared for their clinical and laboratory characteristics using chi-square and Mann-Whitney tests, as appropriate. The incidence of primary outcomes (death, reinfarction, stroke, major bleeding) was compared between groups. P-value < 0.05 was considered significant for all analyses. Results: A total of 367 obese patients with NSTEMI or UA were included, of whom 258 used Fondaparinux and 109 used Enoxaparin. Mean age was 64 ± 12 years, and 52.9% were male. The prevalence of diabetes, hypertension, dyslipidemia, prior coronary artery disease, prior stroke, and implementation of invasive strategy was similar between groups. The incidence of the primary outcome was 4.7% in the Fondaparinux group and 5.5% in the Enoxaparin group (p = 0.729). There was no difference between groups when analyzing the components of the primary outcome separately. Conclusion: In a sample of obese patients with NSTEMI or UA, there was no difference in the occurrence of the composite outcome (death, stroke, reinfarction, major bleeding) between patients who used Fondaparinux or Enoxaparin.

RESUMEN El artículo relata la experiencia de una acción de educación permanente en salud sobre racismo institucional con profesionales de la salud del Sistema Único de Salud (SUS), que ocupan cargos de gestión en un municipio del interior de Bahía, región nordeste de Brasil. Se realizó un taller en septiembre de 2022, basado en una metodología problematizadora denominada Arco de Maguerez, llevada a cabo por residentes del Programa Multiprofesional en Salud Familiar. El taller contó con la participación de diez gestores, siendo la mayoría mujeres. Los gestores, a pesar de reconocer la presencia y el impacto del racismo en los servicios de salud, encuentran dificultades para visibilizar y combatir el racismo en su dimensión institucional. Fue notable el discurso recurrente alineado con el mito de la democracia racial, y se observó un mayor compromiso de las mujeres negras a lo largo del taller. La educación permanente en salud se muestra como una importante estrategia de combate al racismo, necesaria para ampliar los debates de este tipo dentro de los servicios de salud.

ABSTRACT This article discusses the experience of a permanent health education initiative focused on institutional racism with health professionals from the Brazilian Unified Health System (SUS) holding management positions in a municipality in the interior of Bahia, in Northeastern Brazil. A workshop was held in September 2022, based on a problematizing methodology called the Maguerez Arc, conducted by residents of the Multiprofessional Family Health Program. Ten managers participated in the workshop, most of whom were women. Despite acknowledging the presence and impact of racism in health services, managers find it challenging to recognize and address racism in its institutional dimension. A recurring discourse aligned with the myth of racial democracy was notable, and a higher level of commitment among Black women was observed throughout the workshop. Permanent health education emerges as an essential strategy to combat racism, necessary to expand such discussions within health services.

Resumo Atualmente, o teste não invasivo padrão para diagnosticar a doença arterial periférica é o índice tornozelo-braquial, porém, em uma determinada população, este torna-se inviável. Novas evidências propõem o tempo de aceleração plantar, que é um índice ultrassonográfico, como alternativa. Para investigá-lo no contexto do estudo da perfusão do pé, uma revisão bibliográfica integrativa foi realizada entre 03/06/2022 e 08/01/2023. Artigos em inglês, português ou espanhol, publicados entre 2012 e 2022 foram pesquisados através das ferramentas PubMed, Google Scholar e SciELO, com as palavras-chave “Peripheral Arterial Disease” AND “Acceleration Time” AND (Pedal OR Plantar). Pesquisas que não avaliaram a perfusão do pé pelos métodos de interesse, sem dados em humanos e relatos ou séries de casos, foram excluídos. Dos 66 artigos, 7 foram selecionados; todos com notáveis limitações metodológicas. O tempo de aceleração plantar aparenta ser capaz de diagnosticar, estratificar e prognosticar os doentes.

Abstract Currently, the standard non-invasive test for diagnosing Peripheral Arterial Disease is the Ankle-Brachial Index. However, this test becomes unfeasible in a certain population. New evidence proposes the pedal acceleration time, an ultrasound index, as an alternative test. An integrative bibliographic review was carried out between June 3, 2022 and January 8, 2023, to investigate this new index as a tool to assess foot perfusion. Papers published in English, Portuguese, or Spanish between 2012 and 2022 were searched on PubMed, Google Scholar, and Scielo, using the keywords “Peripheral Arterial Disease” AND “Acceleration Time” AND (Pedal OR Plantar). Research that didn’t assess foot perfusion using the methods of interest or did not present human data and also case series or reports were excluded. Seven out of the sixty-six articles identified in the searches were selected for the review, all of which had notable methodological limitations. Pedal acceleration time seems to be able to diagnose and stratify and may reflect prognosis.

RESUMO Objetivo: Traduzir o Hammersmith Infant Neurological Examination (HINE) para o português brasileiro e analisar a confiabilidade da versão traduzida em lactentes brasileiros. Métodos: Estudo metodológico, aprovado por Comitê de Ética, realizado entre junho de 2020 e maio de 2021. O HINE é um exame clínico neurológico padronizado, utilizado para detecção precoce de paralisia cerebral. A seção quantitativa, “exame neurológico”, contém 26 itens pontuados de 0 a 3, divididos em 5 categorias: função dos nervos cranianos; postura; movimentos; tônus muscular e reflexos; e reações. A tradução do HINE seguiu quatro etapas: tradução; síntese; retrotradução; e avaliação por um comitê de especialistas. Dois examinadores independentes avaliaram 43 lactentes, entre 3 e 22 meses, utilizando a versão HINE-Br (versão em português brasileiro), para verificar sua confiabilidade. A consistência interna foi verificada pelo coeficiente Alpha de Cronbach e a confiabilidade interexaminadores pelo coeficiente de correlação intraclasse (CCI). Resultados: A versão traduzida foi semelhante à versão original e poucos ajustes semânticos e idiomáticos foram necessários. Encontrou-se consistência interna adequada (Apha=0,91) para os 26 itens do HINE-Br, bem como forte confiabilidade interexaminadores para o escore total (CCI2,1=0,95) e também para as cinco categorias (CCI2,1=0,83–0,95). Conclusões: O HINE-Br apresenta índices adequados de consistência interna e confiabilidade interexaminadores, podendo ser utilizada para avaliação de crianças com risco de apresentar paralisia cerebral, entre 3 e 24 meses de idade, por pediatras e fisioterapeutas infantis.

ABSTRACT Objective: The current study aimed to translate the Hammersmith Infant Neurological Examination (HINE) into Brazilian Portuguese and analyze the reliability of the translated version for a population of Brazilian infants. Methods: This was a methodological study, approved by the Ethics Committee, carried out between June 2020 and May 2021. HINE is a standardized clinical neurological examination used for the early detection of cerebral palsy. The quantitative section, “neurological examination”, contains 26 items scored from 0 to 3 points, divided into five categories: cranial nerve function, posture, movements, muscle tone and reflexes, and reactions. The HINE translation followed four steps: translation, synthesis, back-translation, and evaluation by an expert committee. To verify the reliability of the HINE-Br (Portuguese-Brazil version) two independent examiners evaluated 43 infants, between 3 and 22 months of age. Internal consistency was verified by Cronbach’s Alpha coefficient and interrater reliability by the intraclass correlation coefficient (ICC). Results: The translated version was similar to the original version and a few semantic and idiomatic adjustments were necessary. Appropriate internal consistency (Alpha=0.91) was found for the 26 items of the HINE-Br, as well as strong interrater reliability for the total score (ICC2.1=0.95), and also for the five categories (ICC2.1=0.83–0.95). Conclusions: The HINE-Br presents adequate rates of internal consistency and interrater reliability, and can be used for the evaluation of children at risk for cerebral palsy, between 3 and 24 months of age, by pediatricians and pediatric physical therapists.

RESUMO A maioria das pessoas com demência vive em países de baixa/média renda, onde recursos essenciais para a saúde cerebral, como educação de qualidade, ainda não são amplamente acessíveis. No Brasil, o analfabetismo ainda é frequente, especialmente em comunidades de baixo nível socioeconômico. O estudo PROAME teve como objetivo explorar a educação básica tardia em pessoas analfabetas como ferramenta para o aumento da reserva cognitiva. Objetivo: Investigar a relação entre nível socioeconômico com aprendizado e com desempenho em testes cognitivos, em adultos analfabetos. Métodos: Este estudo clínico de seis meses (NCT04473235) contou com 108 participantes inscritos no projeto Educação para Jovens e Adultos (EJA), dos quais 77 completaram os testes. O nível socioeconômico de cada participante foi medido usando-se: o Índice de Qualidade de Vida Urbana, o Índice de Desenvolvimento Humano Municipal e o nível socioeconômico doméstico. Avaliações cognitivas incluíram: o Teste de Recordação Seletiva Livre e Guiada (TRSLG), uma lista de palavras para avaliar leitura e a matriz Beta III. Resultados: A amostra era predominantemente feminina, com idade média de 58,5. Os participantes melhoraram a leitura (p=0,01) e o TRSLG (p=0,003). Com relação à memoria episódica, as mulheres tiveram resultados superiores aos dos homens (p=0,007) e participantes mais jovens melhoraram mais que seus colegas mais velhos (p=0,001). Não foi observada nenhuma relação entre o nível socioeconômico e o desempenho cognitivo. Conclusão: Independentemente do nível socioeconômico, participantes obtiveram resultados positivos após frequentar a educação básica. Isso sugere que a educação tardia pode ser uma medida preventiva não farmacológica importante, especialmente em países de baixa/média renda.

ABSTRACT The majority of people with dementia live in low or middle-income countries (LMICs) where resources that play a crucial role in brain health, such as quality education, are still not widely available. In Brazil, illiteracy remains a prevalent issue, especially in communities with lower socioeconomic status (SES). The PROAME study set out to explore basic education in illiterate adults as a means to improve cognitive reserve. Objective: This manuscript aims to explore the relationship between SES and learning, as well as cognitive outcomes, in an older illiterate population. Methods: This six-month clinical trial (NCT04473235) involved 108 participants, of which 77 concluded all assessments, enrolled in late-life basic education. SES assessments included Quality of Urban Living Index, Municipal Human Development Index and Household SES calculated for each participant. Cognitive assessments encompassed the Free and Cued Selective Reminding Test (FCSRT), a word list to assess reading, and the Beta III matrix. Results: The sample consisted primarily of women, with a mean age of 58.5. Participants improved their reading (p=0.01) and their FCSRT (p=0.003). Regarding episodic memory, women outperformed men (p=0.007) and younger participants improved more than their older counterparts (p=0.001). There was no association observed between SES and cognitive outcomes. Conclusion: Irrespective of SES, participants demonstrated positive outcomes after attending basic education. These findings highlight that late life education could be an important non-pharmacologic preventative measure, especially in LMICs.

RESUMO Ligas Al-Cu-Li têm apresentado vantagens em relação às ligas Al-Cu-Mg convencionais usadas na indústria aeroespacial devido as suas propriedades, como alta resistência à fadiga e baixa densidade. As propriedades mecânicas destas ligas dependem de sua microestrutura. Neste trabalho, a atividade eletroquímica associada a microestrutura e comportamento à corrosão da liga Al-Cu-Li 2198-T8 foi investigada por testes eletroquímicos e ensaios de imersão em soluções contendo íons cloreto (Cl–). Os resultados obtidos mostraram que o comportamento eletroquímico e a susceptibilidade a corrosão desta liga são fortemente influenciados pela fase T1(Al2CuLi), a qual é a principal responsável pelo endurecimento da mesma. Foi observado ainda que esta fase é eletroquimicamente mais ativa do que a matriz de alumínio e, que quando exposta ao meio corrosivo, resulta em corrosão localizada severa (CLS), devido ao ataque cristalográfico. Nestas regiões foi observada intensa acidificação, principalmente dentro dos pites, resultando na evolução de gás hidrogênio, o que foi confirmado pelos ensaios de visualização em gel. Os resultados obtidos por técnicas eletroquímicas globais e técnica de varredura do eletrodo vibratório (SVET) confirmaram as observações dos ensaios de imersão e de visualização em gel.

ABSTRACT Al-Cu-Li alloys present interesting advantages over conventional Al-Cu-Mg alloys used in the aerospace industry, due to their excellent properties, such as high fatigue resistance and low density. The mechanical properties of these alloys depend on their microstructure. In this work, the electrochemical activity associated with the microstructure and corrosion behavior of the AA2198-T8, Al-Cu-Li alloy, was investigated through electrochemical and immersion tests, in chloride ions (Cl–) containing solutions. Results obtained showed that the electrochemical behavior and susceptibility to corrosion of this alloy are strongly influenced by the T1(Al2CuLi) phase density which is mainly responsible for its hardening. It was also observed that this phase is electrochemically more active than the aluminum matrix and when exposed to corrosive environments, it results in severe localized corrosion (SLC), due to crystallographic attack. In these regions, intense acidification was observed, within the pits, resulting in hydrogen gas evolution, which was confirmed by gel visualization tests. The results obtained through global electrochemical techniques and scanning vibrating electrode technique (SVET) confirmed the observations from the immersion and gel visualization tests, where increased values of anodic current densities were related to the SLC regions.

Resumo O objetivo do estudo foi avaliar a substituição de virginiamicina por produtos à base de levedura em dietas de alto grão contendo monensina, sobre a digestibilidade dos nutrientes e parâmetros ruminais. Cinco novilhos cruzados Nelore x Holandês (peso inicial 281 kg ± 9,2 kg) distribuídos em um delineamento Quadrado Latino 5 x 5. As dietas foram compostas por silagem de milho (355 g kg-1) e concentrado composto por: milho moído, farelo de soja, casca de soja, mistura mineral, calcário, sal e ureia (645 g kg-1). Os tratamentos foram: 1. Controle, 18 mg kg-1 MS de virginiamicina (VM); 2. Cultura de levedura enriquecida (Saccharomyces cerevisiae) 7g animal dia-1 (CLE 7); 3. Cultura de levedura enriquecida (Saccharomyces cerevisiae) 14g animal dia-1 (CLE 14); 4. Levedura autolisada (Saccharomyces cerevisiae) 7g animal dia-1 (LA 7) e Levedura autolisada (Saccharomyces cerevisiae) 14g animal dia-1 (LA 14). A monensina (18 mg kg-1 MS) estava presente na mistura mineral vitamínica fornecida em todos tratamentos. Não houve diferença significativa (P>0,05) para o consumo de matéria seca (média 8,36 kg MS); digestibilidade da matéria orgânica (média 58,39%) e média de pH ruminal (média 6,75). Além disso, não foram encontrados efeitos significativos de tratamentos para concentração de ácidos graxos de cadeia curta e nitrogênio amoniacal. Estes resultados indicam a possibilidade de substituir a virginiamicina por produtos à base de levedura nos menores níveis (7g animal dia-1 de cultura de levedura enriquecida ou 7g animal dia-1 de levedura autolisada) em dietas de alto grão com monensina.

Abstract The aim of this study was to evaluate the effect of substituting virginiamycin with yeastbased products in high-energy diets containing monensin on the nutrient digestibility and ruminal parameters of cattle. Five crossbred Nellore × Holstein steers (initial body weight 281 kg ± 9.2 kg) were allotted to a 5 × 5 Latin square design. The diets were composed in dry matter basis of corn silage (355 g kg-1), and concentrate composed of ground corn, soybean meal, soybean hulls,mineral mix, limestone, salt and urea (645 g kg-1). The treatments were: 1 - control, 18 mg kg-1 dry matter (DM) virginiamycin (VM); 2 - enriched yeast culture (Saccharomyces cerevisiae) at 7 g animal day-1 (EYC 7); 3 - enriched yeast culture (S. cerevisiae) at 14 g animal day-1 (EYC 14); 4 - autolyzed yeast (S. cerevisiae) at 7 g animal day-1 (AY 7); and 5 - autolyzed yeast (S. cerevisiae) at 14 g animal day-1 (AY 14). Monensin (18 mg kg-1 DM) was added to the mineral mix in all treatments. No significant differences (P > 0.05) were found for DM intake (mean 8.36 kg DM), organic matter (OM) digestibility (average of 58.39%), or mean ruminal pH (mean 6.75) among treatments. Moreover, the treatment did not have a significant effect on the short-chain fatty acid or ammonia nitrogen concentrations. These results indicated the possibility of replacing virginiamycin with yeast-based products at the lowest level (7 g of enriched yeast culture or autolyzed yeast per animal day-1) in high-energy diets containing monensin.

Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Resultados Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). As crianças também devem ser submetidas ao rastreio de de anticorpo contra a glicoproteína da mielina de oligodendrócitos (anti-myelin oligodendrocyte glycoprotein, anti-MOG, em inglês). O tratamento deve ser iniciado dentro das primeiras 4 semanas dos sintomas, sendo as opções de primeira linha metilprednisolona combinada com imunoglobulina intravenosa (IGIV) ou plasmaférese. O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Em relação à encefalite antirreceptor de N-metil-D-aspartato (anti-N-methyl-D-aspartate receptor, anti-NMDAR, em inglês), os especialistas concordaram que agentes imunossupressores orais não devem ser usados. Os pacientes devem ser avaliados na fase aguda e pós-aguda mediante escalas funcionais e cognitivas, como Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Modified Rankin Scale (mRS), e Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusão Esta pesquisa oferece evidências tangíveis do manejo efetivo de pacientes com EAIs no sistema de saúde Brasileiro.

Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. Patients should be evaluated at the acute and postacute stages using functional and cognitive scales, such as the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MoCA), the Modified Rankin Scale (mRS), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusion The present study provides tangible evidence for the effective management of AIE patients within the Brazilian healthcare system.

Resumo Antecedentes A disfunção olfatória (DO) é uma manifestação frequente da doença do coronavírus 2019 (COVID-19). A apolipoproteína E (APOE) é uma proteína que interage com o receptor da enzima conversora de angiotensina, essencial para a entrada viral na célula. Publicações anteriores sugeriram um possível papel da APOE na gravidade da COVID-19. Até onde sabemos, nenhuma publicação encontrou associações significativas entre a gravidade dessa doença, DO e polimorfismos da APOE (E2, E3 e E4). Objetivo Analisar a epidemiologia da DO e sua relação com os polimorfismos do gene APOE em uma coorte de pacientes com COVID longa. Métodos Um estudo de coorte prospectiva com pacientes acompanhados em ambulatório neurológico pós-COVID, com DO sendo definida como uma redução subjetiva da função olfativa após a infecção e a DO persistente sendo definida quando a queixa durou mais de 3 meses após a resolução da infecção por COVID-19. Este estudo transversal é parte de uma pesquisa maior com dados anteriormente relatados, focando na performance cognitiva dos pacientes. Resultados Foram selecionados 221 pacientes para esse estudo, dos quais 186 haviam coletado amostras de sangue para genotipagem APOE. O grupo DO persistente foi mais jovem e apresentou menor taxa de internação na fase aguda da doença (p < 0,001). Além disso, a frequência do alelo E4 da APOE foi menor nesse grupo (p = 0,035). Este estudo avaliou a DO em uma população com COVID longa. Na literatura atual sobre essa doença, a anosmia está associada a melhores desfechos clínicos e o alelo E4 está associado a piores desfechos. Conclusão Nosso estudo acrescenta novas informações a essas correlações, sugerindo a APOE E4 como um fator de proteção para DO.

Abstract Background Olfactory dysfunction (OD) represents a frequent manifestation of the coronavirus disease 2019 (COVID-19). Apolipoprotein E (APOE) is a protein that interacts with the angiotensin-converting enzyme receptor, essential for viral entry into the cell. Previous publications have suggested a possible role of APOE in COVID-19 severity. As far as we know, no publications found significant associations between this disease's severity, OD, and APOE polymorphisms (E2, E3, and E4). Objective To analyze the epidemiology of OD and its relationship with APOE polymorphisms in a cohort of Long-COVID patients. Methods We conducted a prospective cohort study with patients followed in a post-COVID neurological outpatient clinic, with OD being defined as a subjective reduction of olfactory function after infection, and persistent OD being defined when the complaint lasted more than 3 months after the COVID-19 infection resolution. This cross-sectional study is part of a large research with previously reported data focusing on the cognitive performance of our sample. Results The final sample comprised 221 patients, among whom 186 collected blood samples for APOE genotyping. The persistent OD group was younger and had a lower hospitalization rate during the acute phase of the disease (p < 0.001). Furthermore, the APOE variant E4 allele frequency was lower in this group (p = 0.035). This study evaluated OD in an outpatient population with COVID-19. In the current literature on this disease, anosmia is associated with better clinical outcomes and the E4 allele is associated with worse outcomes. Conclusion Our study provides new information to these correlations, suggesting APOE E4 as a protective factor for OD.

RESUMO Objetivos: avaliar o risco relativo de abandono do tratamento do tabagismo durante sua fase intensiva. Métodos: estudo de coorte retrospectivo e quantitativo, desenvolvido a partir da consulta aos prontuários eletrônicos de pessoas que iniciaram o tratamento do tabagismo entre os anos de 2015 e 2019 em um ambulatório de especialidades de uma cidade do interior de São Paulo, Brasil. O risco relativo de abandonar o tratamento foi calculado utilizando o modelo de regressão de Poisson. Resultados: observou-se que, das 396 (100,0%) pessoas que iniciaram o tratamento, 109 (27,5%) o abandonaram antes do término da fase intensiva. A cada um ano de aumento na idade, o risco de abandono do tratamento do tabagismo diminuiu, em média, 2%. Conclusões: o risco de abandono do tratamento do tabagismo é maior em pessoas mais jovens. Faz-se necessário repensar o cuidado ofertado a adultos mais jovens, de forma a favorecer a continuidade do tratamento.

ABSTRACT Objectives: to evaluate the relative risk of smoking cessation treatment dropout during its intensive phase. Methods: a retrospective and quantitative cohort study was developed from the electronic medical records of individuals who started smoking cessation treatment between 2015 and 2019 at a specialty clinic in a city in the interior of São Paulo, Brazil. The relative risk of dropping out of treatment was calculated using the Poisson regression model. Results: it was observed that out of the 396 (100.0%) individuals who started the treatment, 109 (27.5%) abandoned it before the end of the intensive phase. For each one-year increase in age, the risk of dropping out of smoking cessation treatment decreased by an average of 2%. Conclusions: the risk of dropping out of smoking cessation treatment is higher among younger individuals. It is necessary to rethink the care offered to younger adults to promote the continuity of treatment.

RESUMEN Objetivos: evaluar el riesgo relativo de abandono del tratamiento para dejar de fumar durante su fase intensiva. Métodos: un estudio de cohorte retrospectivo y cuantitativo se desarrolló a partir de los registros médicos electrónicos de individuos que comenzaron el tratamiento para dejar de fumar entre 2015 y 2019 en una clínica de especialidades en una ciudad del interior de São Paulo, Brasil. El riesgo relativo de abandonar el tratamiento se calculó utilizando el modelo de regresión de Poisson. Resultados: se observó que, de los 396 (100.0%) individuos que iniciaron el tratamiento, 109 (27.5%) lo abandonaron antes del final de la fase intensiva. Por cada aumento de un año en la edad, el riesgo de abandonar el tratamiento para dejar de fumar disminuyó en un promedio del 2%. Conclusiones: el riesgo de abandonar el tratamiento para dejar de fumar es mayor entre los individuos más jóvenes. Es necesario repensar el cuidado ofrecido a los adultos más jóvenes para promover la continuidad del tratamiento.

ABSTRACT The herbicide tembotrione may be seletivity to sorghum, which is of utmost importance in grain production systems. Thus, the present study aimed to evaluate the selectivity of tembotrione + atrazine combinations applied post-emergence at different phenological stages of grain sorghum. Four experiments were carried out in Rio Verde and Montividiu (state of Goiás) in succession to soybean cultivation in 2017 and 2018. The experiments were carried out in randomized block design with six replications, both in a (3x2) + 1 factorial arrangement. The first factor referred to three vegetative stages of application (V3, V5 and V7) associated with two doses of tembotrione (90 and 180 g ha-1) combined with atrazine (1,000 g ha-1)and and an additional treatment with no herbicide application. Tembotrione + atrazine applications at the V7 stage caused less damage to grain yield and dry mass accumulation in relation to V3 and V5. The dose of 90 g ha-1 tembotrione + atrazine combined with atrazine resulted in higher grain yields compared to 180 g ha-1, although still at lower levels compared to the treatment with no herbicides. Thus, herbicide tembotrione at doses of 90 and 180 g ha-1 combined with atrazine did not have selectivity for grain sorghum in applications at V3, V5 and V7 stages.

Abstract The aim of the study was to evaluate the use of Acustic Radiation Force Impulse (ARFI) elastography in mammary parenchyma and supramammary lymph nodes, for detection of active mastitis in sheep with naturally infected chronic fibrous lesions. 27 female sheep were included and B-mode ultrasound and ARFI elastography images were obtained, acquiring qualitative (echogenicity and echotexture) and quantitative (shear rate, depth and short/long axis ratio) variables of 48 mammary glands. The glands were divided into three experimental groups: control group (CG) - healthy animals; LSCC- animals that presented fibrous lesions and SCC (somatic cell count) less than 500 x 103 cls/mL; HSCC: animals that presented fibrous lesions and SCC (somatic cell count) more than 500 x 103 cls/mL; The qualitative variables using B-mode ultrasonography, including echotexture and echogenicity, showed no significant differences between the evaluated groups and tissues (p = 0.9336 and p = 0.233, respectively) .In healthy areas of the gland, it was an increasing in shear wave velocity (SWV) in LSCC than in HSCC (p=0.04). When comparing the fibrosis in the LSCC and HSCC groups with their respective normal areas, the velocity increased in both groups: LSCC (p= 0,0007) and HSCC (p= 0,0001). When comparing the areas of fibrosis in LSCC and HSCC with the CG parenchyma, there was an increase in LSCC (p=0.001) and HSCC (p=0.0001). B-mode ultrasound indicate predominance of hypoechoic echogenicity in lymph nodes and reduced short/long axis ratio in cases of active subclinical mastitis. The supramammary lymph node showed increased SWV when comparing the CG with HSCC groups (p=0.02) and GC with LSCC (p=0.04). B-mode ultrasonography is useful for evaluating the mammary parenchyma, however, its application as a standalone diagnostic technique is not recommended. ARFI elastography indicates potential cutoff points for differentiating subclinical mastitis from healed mastitis, highlighting its importance as a tool for distinguishing normal areas from fibrous parenchymal areas. While this study did not establish specific cutoff points due to sample size limitations, further research with larger sample sizes could explore and define these critical thresholds.

ABSTRACT Objective: To investigate the potential impact of the pandemic on the number of sexological forensic examinations among children and adolescents in a state capital of Northeastern Brazil, as well as to outline the profile of victims and perpetrators. Material and Methods: Cross-sectional study with retrospective analysis of medical-legal/forensic dental examinations conducted from March 2019 to March 2023, focusing on cases of sexual intercourse/sexual assault suffered by children and adolescents examined in the municipality of João Pessoa, Brazil. Results: Of the 1254 reports included, 370 occurred during the pre-pandemic period, 272 during the critical pandemic, 280 during the stabilization, and 332 during the post-pandemic. One thousand and eighty-eight cases involved females, and 692 affected children, with the pre-pandemic period showing the highest number of occurrences. 86.36% of perpetrators were male and acted alone, and 51.20% had a relationship with the victim. Three hundred and fifty-one cases occurred at the victim's residence. No statistically significant association was found among cases across the pandemic periods. Conclusion: Despite the apparent decrease in cases of child sexual violence, it does not necessarily imply a real reduction due to widespread underreporting during COVID-19. Abstract In this article, the authors propose changes to a microwave system for measuring the relative humidity of hollow concrete blocks presented in the literature, aiming to improve the system’s sensitivity. The operating frequency range was changed from 2.5 GHz to 5.8 GHz. This reduced the dimensions of the antenna arrays used in the system. Five block samples were used in this work. The frequency selective surface (FSS) used as a sensor has been modified, from a double circular ring to a square loop, which presents a single-band frequency response. The results obtained in this work showed a high correlation between the frequency variation and the relative humidity level. First, second, and third-order functions were compared to see which would better relate resonance frequency to relative humidity. The second-order function presented the best results, considering the Mean Absolute Percentage Error (MAPE), the Mean Square Error (RMS), and the coefficient of determination. We performed a sensor sensitivity analysis showing that the new proposed system is more sensitive than others presented in the literature.
