Abstract There are countless proven benefits of breastfeeding, and the demand for such a right in health for transfeminine people is rarely addressed in the literature, reinforcing inequities in health. The article aims to conduct a scoping review of lactation induction for transfeminine people in the health care context. Systematic literature review in six selected databases, looking for articles with terms related to lactation and transfeminine people. Data were extracted and analyzed, summarizing the main results in tables. Three hundred ninety articles were found. After the exclusion of the duplicates there was a selection by title/abstract and a following selection by the full reading of the remaining articles, considering the pre-determined exclusion and inclusion criteria. Twenty-one articles were included, published between 2018 and 2023. Among them, six are case reports with unprecedented information on the topic, and the others are publications in various formats. Lactation induction was achieved in all the case reports. There is a fragile and recent body of evidence affirming the success of lactation induction in transgender women. Resumo São inúmeros os benefícios comprovados do aleitamento materno, e a demanda por tal direito em saúde para as pessoas transfemininas é pouco abordada na literatura, reforçando as iniquidades em saúde. O artigo objetiva realizar uma revisão de escopo sobre a indução da lactação para pessoas transfemininas no contexto de assistência à saúde. Revisão sistemática da literatura em seis bases de dados selecionadas, buscando artigos com termos relacionados à lactação e pessoas transfemininas. Os dados foram extraídos e analisados, resumindo os principais resultados em tabelas. Foram encontrados 390 artigos. Após a exclusão dos duplicados, procedeu-se à seleção por título/resumo e posterior seleção pela leitura na íntegra, considerando os critérios de exclusão e inclusão. Foram incluídos 21 artigos, publicados entre 2018 e 2023. Entre eles, seis são relatos de casos com informações inéditas sobre o tema, e os demais são publicações em diversos formatos. A indução da lactação foi alcançada em todos os relatos de casos. Existe um corpo de evidências frágil e recente que afirma o sucesso da indução da lactação em mulheres trans. ABSTRACT Purpose: Timely screening and treatment are essential for preventing diabetic retinopathy blindness. Improving screening workflows can reduce waiting times for specialist evaluation and thus enhance patient outcomes. This study assessed different screening approaches in a Brazilian public healthcare setting. Methods: This retrospective study evaluated a telemedicine-based diabetic retinopathy screening implemented during the COVID-19 pandemic and compared it with in-person strategies. The evaluation was conducted from the perspective of a specialized referral center in an urban area of Central-West Brazil. In the telemedicine approach, a trained technician would capture retinal images by using a handheld camera. These images were sent to specialists for remote evaluation. Patient variables, including age, gender, duration of diabetes diagnosis, diabetes treatment, comorbidities, and waiting time, were analyzed and compared. Results: In total, 437 patients with diabetes mellitus were included in the study (mean age: 62.5 ± 11.0 years, female: 61.7%, mean diabetes duration: 15.3 ± 9.7 years, insulin users: 67.8%). In the in-person assessment group, the average waiting time between primary care referral and specialist evaluation was 292.3 ± 213.9 days, and the referral rate was 73.29%. In the telemedicine group, the average waiting time was 158.8 ± 192.4 days, and the referral rate was 29.38%. The telemedicine approach significantly reduced the waiting time (p<0.001) and significantly lowered the referral rate (p<0.001). Conclusion: The telemedicine approach significantly reduced the waiting time for specialist evaluation in a real-world setting. Employing portable retinal cameras may address the burden of diabetic retinopathy, especially in resource-limited settings. ABSTRACT Objective: To analyze characteristics of the home visit (HV) in Brazil, 2012 and 2017. Methods: Ecological study, with panel data whose units of analysis were the Primary Health Care teams in Brazil, participants of the 1st and 3rd cycles of the Program to Improve Access and Quality of Primary Care of the Unified Health System. Descriptive, inferential and spatial analyzes (alpha=5%) were performed. Results: There was an increase in the proportion of teams that carried out home visits at a frequency defined based on risk and vulnerability analysis and actively searched for people with respiratory symptoms and women with delayed and altered cytopathological examination. In the heat maps, the Northeast, Southeast and South regions had a higher concentration of teams that carried out the HV and carried out an active search. RESUMO Objetivo: Analisar características da visita domiciliar (VD) no Brasil em 2012 e 2017. Métodos: Estudo ecológico com dados em painel cujas unidades de análise foram as equipes de saúde da Atenção Primária à Saúde no Brasil, participantes do 1º e 3º ciclos do Programa de Melhoria do Acesso e Qualidade da Atenção Básica do Sistema Único de Saúde. Foram realizadas análises descritivas, inferenciais e espaciais (alpha=5%). ABSTRACT Objective: to analyze national and international scientific production on gait speed associated with signs, symptoms, and clinical factors in elderly people in primary health care. Method: integrative literature review carried out in five data sources (Virtual Health Library, Embase, Web of Science, Cumulative Index To Nursing And Allied Health Literature (CINAHL), and PubMed) with publications from January 2017 to June 2023. Results: 13 publications met the eligibility criteria, with a predominance of studies in English. Studies indicated that reduced gait speed is associated with fractures (p<0.05), dementia (p<0.01), cardiovascular (p<0.001) and neuropsychiatric (p<0.01) diseases, falls (p<0.05) and polypharmacy (p<0.001). Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Resultados Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). As crianças também devem ser submetidas ao rastreio de de anticorpo contra a glicoproteína da mielina de oligodendrócitos (anti-myelin oligodendrocyte glycoprotein, anti-MOG, em inglês). O tratamento deve ser iniciado dentro das primeiras 4 semanas dos sintomas, sendo as opções de primeira linha metilprednisolona combinada com imunoglobulina intravenosa (IGIV) ou plasmaférese. O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Em relação à encefalite antirreceptor de N-metil-D-aspartato (anti-N-methyl-D-aspartate receptor, anti-NMDAR, em inglês), os especialistas concordaram que agentes imunossupressores orais não devem ser usados. Os pacientes devem ser avaliados na fase aguda e pós-aguda mediante escalas funcionais e cognitivas, como Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Modified Rankin Scale (mRS), e Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. Patients should be evaluated at the acute and postacute stages using functional and cognitive scales, such as the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment (MoCA), the Modified Rankin Scale (mRS), and the Clinical Assessment Scale in Autoimmune Encephalitis (CASE). Conclusion The present study provides tangible evidence for the effective management of AIE patients within the Brazilian healthcare system. Abstract Background Genes and their variants associated with environmental factors contribute to the development of the hypertensive phenotype. The G protein beta 3 subunit gene (GNB3) is involved in the intracellular signaling process, and its variants have been related to susceptibility to arterial hypertension. Objective To determine the association of the GNB3 variant (rs5443:C>T) with arterial hypertension, biochemical parameters, age, and obesity in hypertensive and normotensive individuals from Ouro Preto, Minas Gerais, Brazil. Method The identification of variants was performed by real-time PCR, using the TaqMan® system, in 310 samples (155 hypertensive and 155 normotensive). Biochemical analyses (renal function, lipid profile and glycemia) were performed from the serum using UV/Vis spectrophotometry and ion-selective electrode. A multiple logistic regression model was used to identify factors associated with arterial hypertension. The analysis of continuous variables with normal distribution was performed using the unpaired Student’s t test; non-normal data were analyzed using Mann-Whitney. P < 0.05 was considered significant. Results The rs5443:C>T variant was not associated with arterial hypertension in the evaluated population (p = 0.88). Regarding biochemical measures, the T allele was associated with high levels of triglycerides, glucose and uric acid in hypertensive individuals (p < 0.05). Resumo Fundamento Genes e suas variantes associadas a fatores ambientais contribuem para o desenvolvimento do fenótipo hipertenso. O gene da subunidade beta 3 da proteína G ( GNB3 ) está envolvido no processo de sinalização intracelular e suas variantes têm sido relacionadas à suscetibilidade à hipertensão arterial. Objetivo Determinar a associação da variante GNB3 (rs5443:C>T) com a hipertensão arterial, parâmetros bioquímicos, idade e obesidade em indivíduos hipertensos e normotensos de Ouro Preto, Minas Gerais. Método A identificação das variantes foi realizada por PCR em tempo real, utilizando o sistema TaqMan®, em amostras de 310 pacientes (155 hipertensos e 155 normotensos). ABSTRACT Models that simulate the process of stomatal conductance (gs) for a given set of environmental conditions are important, as this process is the main mechanism that controls the gas exchange of terrestrial plants absorbing atmospheric CO2 in tropical forests. Simulations were performed for the Tapajós National Forest, in the western Brazilian Amazon, observing the gs process under the current climate scenario (control) and under the scenarios RCP4.5 and RCP8.5 (2071 - 2100), using the ED2.2 ecosystem demography model. The results showed that the lower availability of soil water for the plants reduced photosynthesis due to the closing of the stomata. The model results for gross primary productivity (GPP) are similar to those observed in the field, varying about ≈24 MgC ha-1 year-1 for the rainy season and ≈23 MgC ha-1 year-1 for the dry season (average 2002 to 2010) in the control scenario. In the RCP4.5 scenario, simulated GPP was 30.7 and 30 MgC ha-1year-1 for the rainy and dry season, respectively (30.5 and 25 MgC ha-1year-1, respectively, for the RCP8.5 scenario). Our results also show that there may be a limitation on the increase in biomass carbon with the concentration of CO2, as GPP was lower in RCP8.5, despite this scenario having a higher value of atmospheric CO2 relative to RCP4.5. RESUMO Modelos que simulam o processo de condutância estomática (gs) para um determinado conjunto de condições ambientais são importantes, pois esse processo é o principal mecanismo que controla as trocas gasosas das plantas terrestres ao absorver o CO2 atmosférico em florestas tropicais. Realizamos simulações para a Floresta Nacional do Tapajós, na Amazônia Ocidental brasileira, observando o processo da gs sob o cenário climático atual (controle) e sob os cenários RCP4.5 e RCP8.5 (2071 - 2100) usando o modelo demográfico de ecossistema ED2.2. Os resultados mostraram que a menor disponibilidade de água no solo para as plantas reduziu a fotossíntese devido ao fechamento dos estômatos. Os resultados do modelo para produtividade primária bruta (PPB) foram semelhantes aos observados em campo, variando cerca de ≈24 MgC ha-1 ano-1 para a estação chuvosa e ≈23 MgC ha-1 ano-1 para a estação seca (média 2002 a 2010) no cenário controle. ABSTRACT This article analyzes the lived experiences of a group of babies in a collective context of education and care. Based on the dialogue between Cultural-Historical Theory and Ethnography in Education, we followed a group of 13 babies, aged between 10 months to 1 year and 2 months, for six months, seeking to understand their experiences in this context. The empirical material was built through filmic and photographic records, fieldnotes and semi-structured interviews with adults. The babies approached each other provoked by physical touch, cultural artifacts and initial play in the infant’s room. We argue that approaching each other can be understood as the movement performed by babies, constituting a semantic field through the lived experiences in that context and enabling the relationship between them. In this context, a representative history of the lived experiences constituted in the infant’s room was selected, which revealed acts of care through which two babies, Ícaro and Giulia, initiated and sustained their relationships in the educational space. The analyzes reveal (i) the attractive force of cultural artifacts; (ii) the diversity of activities listed in the nursery routine; (ii) the great value in the relationship with the teachers; (iii) the fleeting nature of babies’ interactive dynamics. RESUMO Este artigo analisa as vivências de um grupo de bebês inseridos/as em um contexto coletivo de educação e cuidados. Com base no diálogo entre a Teoria Histórico-Cultural e a Etnografia em Educação, acompanhamos um grupo de 13 bebês, com idades entre 10 meses a 1 ano e 2 meses, durante seis meses, visando o conhecimento das suas vivências nesse contexto. O material empírico foi construído por meio de registros fílmicos, fotográficos, anotações em diário de campo e entrevistas semiestruturadas com professoras. Os/as bebês se aproximaram provocados pelo toque corporal, pelos artefatos e iniciaram brincadeiras no berçário. Argumentamos que a aproximação pode ser compreendida como o movimento realizado pelos/as bebês, constituindo um campo de sentidos por meio das vivências naquele contexto e possibilitando a relação entre eles/as. Selecionamos um caso expressivo das vivências de dois bebês, Ícaro e Giulia. Tais vivências revelaram atos de cuidado pelos quais os dois bebês iniciaram e sustentaram suas relações no espaço educacional. RESUMEN Objetivo: describir la experiencia en la elaboración e implementación de un curso a distancia de Reanimación Cardiopulmonar para profesionales de la salud mediante Telesimulación. Desarrollo: informe de experiencia de un curso de capacitación en Reanimación Cardiopulmonar, realizado con 227 profesionales de la salud entre junio y julio de 2021, de forma remota y sincrónica, en el estado de Río de Janeiro, Brasil. Se dictó el curso a 35 grupos y se los sometió a: pretest; video clase y postest 1; y Telesimulación y postest 2 con evaluación de la percepción. En la evaluación, la Telesimulación se destacó como estrategia para motivar un mayor aprendizaje sobre el tema, como revela el aumento del número de aciertos entre los tests. Conclusión: el curso fue de interés para los participantes y demostró ser una estrategia de aprendizaje, además de ser considerado una posibilidad de innovación para la educación permanente en salud. ABSTRACT Objective: to describe the experience in the elaboration and implementation of a remote course on Cardiopulmonary Resuscitation for health professionals by means of Telesimulation. Development: an experience report about a course for Cardiopulmonary Resuscitation training, developed remotely and synchronously with 227 health professionals between June and July 2021 in the state of Rio de Janeiro - Brazil. Abstract Scientific evidence about genetic and molecular changes in oral squamous cell carcinoma (OSCC) among smokers and non-smokers is inconclusive. This systematic review and meta-analysis assessed the effects of tobacco on the DNA of individuals with OSCC based on protein mutations. Electronic searches were conducted on PubMed, Ovid, Web of Science, and Scopus to identify observational studies published up to January/2022. The Joanna Briggs Institute tool was used for the critical appraisal of studies. The certainty of the evidence was evaluated. Twenty-three studies assessing 4,060 individuals (2,967 smokers vs. 1,093 non-smokers) were included in this review. Fifteen groups of proteins/genes were investigated. Analysis of the quality of articles revealed low risk of bias in most studies. The certainty of the evidence was very low. The meta-analysis confirmed no significant difference between smokers and non-smokers with respect to damage to GSTM1 (OR: 0.60; 95%CI: 0.30–1.18), GSTT1 (OR: 1.18; 95%CI:0.49–2.83), hydrolase proteins (Ku70 and Ku80) (OR: 0.74; 95%CI: 0.18–3.05), and transferase proteins (GSTM1, GSTT1, GSTM3) (OR: 0.74; 95%CI: 0.47–1.18). Most of the studies included showed that smokers are more likely to exhibit genetic instability. Abstract Background Patients admitted with acute decompensated heart failure (HF) are subject to developing worsening episodes that require more complex interventions. The Acute Decompensated Heart Failure National Registry (ADHERE) risk model was developed in the United States to predict the risk of in-hospital worsening HF. Objective To use the ADHERE risk model in the assessment of risk of in-hospital worsening HF and to determine its sensitivity and specificity in hospitalized patients. Methods This cohort study was conducted at a Brazilian public university hospital, and data from 2013 to 2020 were retrospectively collected. P values < 0.05 were considered statistically significant. Results A total of 890 patients with a mean age of 74 ± 8 years were included. The model showed that, in the group of 490 patients at risk, 254 (51.8%) developed in-hospital worsening HF. In the group of 400 patients not at risk, only 109 (27.2%) experienced worsening HF. The results demonstrated a statistically significant curve (area under the curve = 0.665; standard error = 0.018; P < 0.01; confidence interval = 0.609 to 0.701), indicating good accuracy. The model had a sensitivity of 69.9% and a specificity of 55.2%, with a positive predictive value of 52% and a negative predictive value of 72.7%. Resumo Fundamento Pacientes hospitalizados com insuficiência cardíaca (IC) aguda descompensada estão sujeitos a desenvolver episódios de piora que requerem intervenções mais complexas. O modelo de predição de risco “Acute Decompensated Heart Failure National Registry” (ADHERE) foi desenvolvido nos Estados Unidos para prever o risco de piora intra-hospitalar da IC. Objetivo Utilizar o modelo de predição de risco ADHERE para avaliar o risco de piora intra-hospitalar da IC e determinar a sua sensibilidade e especificidade em pacientes hospitalizados. Métodos O presente estudo de coorte foi realizado em um hospital universitário público brasileiro e os dados de 2013 a 2020 foram coletados retrospectivamente. Foram considerados estatisticamente significativos valores de p < 0,05. Resultados Foram incluídos 890 pacientes com idade média de 74 ± 8 anos. O modelo mostrou que no grupo de 490 pacientes de risco, 254 (51,8%) desenvolveram piora intra-hospitalar da IC. No grupo de 400 pacientes sem risco, apenas 109 (27,2%) apresentaram piora da IC. Os resultados demonstraram uma curva estatisticamente significativa (área sob a curva = 0,665; erro padrão = 0,018; p < 0,01; intervalo de confiança = 0,609 a 0,701), indicando boa precisão. O modelo apresentou sensibilidade de 69,9% e especificidade de 55,2%, com valor preditivo positivo de 52% e valor preditivo negativo de 72,7%. Conclusões Na presente coorte, demonstramos que o modelo de predição de risco ADHERE foi capaz de discriminar pacientes que, de fato, desenvolveram piora da IC durante o período de internação daqueles que não desenvolveram. RESUMEN Objetivo: Analizar las estrategias utilizadas por el personal de enfermería de un hospital universitario para asegurar la continuidad asistencial al alta hospitalaria de pacientes recuperadas de Covid-19, a la luz de los principios de gestión clínica. Método: Estudio descriptivo con abordaje cualitativo, realizado con siete enfermeras de guardia en las consultas de medicina y ginecología/obstetricia de un hospital universitario de la región centro-oeste del país. Los datos se procesaron con el programa IRaMuTeQ y se analizaron mediante Análisis de Contenido. Resultados: Los datos resultaron en cinco clases por la Clasificación Jerárquica Descendente (CJD), que constituyeron dos categorías: “Prácticas desarrolladas por las enfermeras para la continuidad de los cuidados en el ambiente hospitalario” y “Continuidad de los cuidados durante el alta al domicilio”. Las estrategias utilizadas por las enfermeras fueron: cuidados diarios sistematizados en el proceso de enfermería y orientación tanto para la preparación como para el día del alta. Conclusión: La ausencia de un protocolo institucional para el alta segura, así como del cargo de enfermera coordinadora para gestionar el alta de pacientes con covid-19, puede comprometer la continuidad de cuidados de estos pacientes. ABSTRACT Objective: To analyze the strategies used by nurses at a university hospital to ensure continuity of care at hospital discharge for patients recovered from Covid-19, under the angle of the principles of clinical management. Method: A descriptive study with a qualitative approach, carried out with seven nurses on duty in the medical and gynecology/obstetrics clinics of a university hospital in the Midwest region of the country. The data was processed using IRaMuTeQ software and analyzed using Content Analysis. Results: The data resulted in five classes by the Descending Hierarchical Classification (DHC), which made up two categories: “Practices developed by nurses for continuity of care in the hospital environment” and “Continuity of care during discharge to the home”. The strategies used by the nurses were: daily care systematized in the nursing process and guidance both for preparation and for the day of discharge. Método: Estudo descritivo de abordagem qualitativa, realizado com sete enfermeiros plantonistas das clínicas médicas e ginecologia/obstetrícia de um hospital universitário da região centro-oeste do país. Os dados foram processados pelo software IRaMuTeQ e analisados pelo método de Análise de Conteúdo. Resultados: Os dados resultaram em cinco classes pela Classificação Hierárquica Descendente (CHD), que compôs duas categorias: “Práticas desenvolvidas pelos enfermeiros para a continuidade do cuidado no ambiente hospitalar” e “Continuidade do cuidado durante a alta hospitalar para o domicílio”. As estratégias utilizadas pelos enfermeiros foram: cuidados diários sistematizados no processo de enfermagem e orientações tanto para o preparo quanto para o dia de alta. Résumé L’objectif de cet article est de démontrer que, dans des conditions historiques et culturelles données, les jeux produisent des subjectivités, des significations, des récits et, par conséquent, une conscience humaine. À partir de la théorie historico-culturelle et de l’ethnographie de l’éducation, nous considérons que les jeux sont un acte de création de possibilités, non seulement de participation dans le groupe social mais aussi de production des significations pour tout ce qui se passe dans la vie collective. En partant de l’événement intitulé «Não é uma caixa!» [«Ceci n’est pas une boîte!»], la recherche vise à analyser la création de significations pour une boîte en carton. Nous concluons que les jeux créent des contradictions dans les domaines perceptif, narratif et imaginaire qui élargissent les possibilités d’action, de langage et de relations sociales, favorisant ainsi le développement culturel des enfants. Resumen Este artículo tiene como objetivo argumentar que el juego es una actividad de producción de subjetividades, significados, narrativas y, por tanto, conciencia humana en condiciones histórica y culturalmente situadas. A partir de la teoría histórico-cultural y de la etnografía en educación, discutimos que el juego es un acto de creación de posibilidades, tanto de participación en el grupo social como de producción de significados para lo que sucede en la vida colectiva, a través del análisis de la creación de significados para un caja de cartón en el evento “¡Não é uma caixa!” [“¡No es una caja!”]. Abstract This article argues that play is an activity that produces subjectivities, meanings, narratives, and, therefore, human consciousness in historically and culturally situated conditions. From the historical-cultural theory and ethnography in education, we argue that play is an act of creating possibilities, both for participation in the social group and for the production of meanings for what happens in collective life, through the analysis of creation meanings for a cardboard box in the event “Não é uma caixa!” [“Not a box!”].
