ABSTRACT Nutri-priming is a low-cost and highly effective way of supplying seeds with micronutrients. The aim of this study was to evaluate the physiological quality of seeds of the Pérola cultivar of the common bean under nutrient solutions containing molybdenum, cobalt and zinc for different imbibition times. The experiment was carried out in the laboratory and greenhouse of the State University of Goiás UEG - UnU Ipameri, using a completely randomised experimental design, with eight micronutrient treatments (T1. Control; T2. Mo; T3. Zn; T4. Co; T5. Mo + Co; T6. Mo + Zn; T7. Zn + Co; T8. Mo + Co + Zn) and six imbibition times (6, 8, 12, 16, 20 and 24 hours). The variables under analysis were seed imbibition, germination test, first germination count, accelerated ageing, seedling emergence, emergence speed index, shoot length, root length, total length, fresh weight, dry weight, and shoot and root biomass. The micronutrients afforded an increase in germination in the seed batches under evaluation; on the other hand, imbibition times of greater than four hours reduced the germination capacity of the seeds. It can be concluded that nutri-priming in the common bean is only beneficial for an imbibition time of up to four hours.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

ABSTRACT Coronary artery fistula draining into the coronary sinus is a rare vascular malformation, and its diagnosis and clinical manifestations usually occur late. We describe the case of a 72-year-old female patient with dyspnea on exertion (New York Heart Association Class III) associated with palpitations. The transthoracic echocardiogram showed significant tricuspid insufficiency. Cardiac catheterization showed aneurysm of the circumflex coronary artery and fistula of this artery draining into the coronary sinus. The patient underwent fistula ligation and tricuspid valve repair, with excellent surgical results.

ABSTRACT. The aim of this study was to assess the effect of increasing periods of coexistence of weed plants with Urochloa ruziziensis on the canopy structure and productivity of a pasture already established with this forage species. The experiment was a randomized blocks design with four replications, and treatments consisted of seven increasing periods of coexistence of forage grass with weed plants: 0 (control), 15, 30, 45, 60, 75 and 90 days after regrowth (DAR). The main morpho-structural and productive characteristics of the forage plants were determined at the end of the experimental period (90 DAR). The ratio of the first green leaf height to the tiller height increases, while the leaf to stem ratio diminishes as the period of interaction with the infesting community increases. The number of green leaves per tiller and the tiller height diminishes as the period of coexistence with weed plants increases. The presence of weed plants interferes negatively with all parameters of the grass canopy structure and productivity of a grazing land already established with Urochloa ruziziensis, suggesting that measures of control of the infesting community should be adopted up to 17 days of regrowth of the forage plant.

ABSTRACT The determination of leaf area is fundamental for studies related to plant growth and physiology. Thus, non-destructive methods allow an accurate estimate of the leaf area through linear dimensions of the leaves. The research objective was to construct allometric equations to estimate the leaflet area of peanut cultivars. Then, 2,605 leaflets were collected from six peanut cultivars (IAC Caiapó, IAC 8112, Runner IAC 886, BRS Havana, BRS 151 L7, and IAC Tatuí), with more than 400 leaflets sampled for each cultivar. We measured the length, width, product between length and width, and leaflet area. Linear and non-linear models (linear, linear without intercept, power, and exponential) were built, and the best equation was chosen using the statistical criteria: highest coefficient of determination (R2), Pearson’s linear correlation coefficient (r), Willmott’s agreement index (d), lowest Akaike information criterion (AIC), and root mean square of the error (RMSE). It was found that the models that used the product between length and width were the most suitable for estimating the leaflet area of peanut cultivars. Given the little intraspecific morphological variability, it was possible to group the cultivars, and model y ^ = 0.875 * LW0.929 was indicated to estimate the peanut leaflet area accurately, regardless of the cultivar.

Abstract Objective To describe the clinical, laboratory, and radiological characteristics, as well as the outcomes of children with MIS-C. Method Multicenter, prospective cohort study, conducted in 17 pediatric intensive care units in five states in Brazil, from March to July 2020. Patients from 1 month to 19 years who met the MIS-C diagnostic criteria were included consecutively. Results Fifty-six patients were included, with the following conditions: Kawasaki-like disease (n = 26), incomplete Kawasaki disease (n = 16), acute cardiac dysfunction (n = 10), toxic shock syndrome (n = 3), and macrophage activation syndrome (n = 1). Median age was 6.2 years (IQR 2.4−10.3), 70% were boys, 59% were non-whites, 20% had comorbidities, 48% reported a contact with COVID-19 cases, and 55% had a recent SARS-CoV-2 infection confirmed by RT-PCR and/or serology. Gastrointestinal symptoms were present in 71%, shock symptoms in 59%, and severe respiratory symptoms in less than 20%. -Dimer was increased in 80% and cardiac dysfunction markers in more than 75%. Treatment included immunoglobulin (89%); corticosteroids, antibiotics, and enoxaparin in about 50%; and oseltamivir and antifungal therapy in less than 10%. Only 11% needed invasive mechanical ventilation, with a median duration of five days (IQR 5–6.5). The median length of PICU stay was six days (IQR 5–11), and one death occurred (1.8%). Conclusions Most characteristics of the present MIS-C patients were similar to that of other cohorts. The present results may contribute to a broader understanding of SARS-CoV-2 infection in children and its short-term consequences. Long-term multidisciplinary follow-up is needed, since it is not known whether these patients will have chronic cardiac impairment or other sequelae.

ABSTRACT BACKGROUND: Coronavirus disease-19 (COVID-19) has imposed a new reality that presents several challenges for healthcare professionals. The main challenge has been the lack of proper training in relation to an unknown disease. OBJECTIVE: To investigate healthcare professionals’ acquisition of knowledge of a new airway management protocol for COVID-19 through their participation in simulation training. DESIGN AND SETTING: Pre and post-test study with purpose sampling, carried out in a tertiary-level hospital in the city of Campinas, state of São Paulo, Brazil. METHODS: This was a cross-sectional pre and post-test intervention among healthcare professionals working in the intensive care unit and emergency department of a large hospital. The training was carried out using an in situ simulation scenario and the participants answered pre and post-tests consisting of a 20-item questionnaire about the new protocol. RESULTS: The paired-sample t test demonstrated that there was a significant increase in test score (t = −19.06; P < 0.001), from before the training (M = 8.62; standard deviation, SD = 3.53) to after the simulation training (M = 17.02; SD = 1.76). CONCLUSIONS: The simulated training had a positive impact on the healthcare professionals’ acquisition of the COVID-19 protocol. We also demonstrated that in situ simulation training was an efficient tool for implementing new protocols, thus bringing benefits to healthcare systems, professionals and patients.

Resumo A soja é uma das culturas de maior impacto no abastecimento global de alimentos, por isso é importante estudar os fatores relevantes que afetam sua produção e até mesmo o desenvolvimento de suas sementes no campo. Os macro e micronutrientes são importantes, especificadamente em relação à soja, o boro é extremamente importante para a fixação biológica do nitrogênio, participando dos processos fisiológicos de formação das sementes. No entanto, o nutriente não é muito móvel nas plantas, sendo assim alguns autores recomendam a pulverização foliar para melhorar a produtividade e a qualidade das sementes. A dosagem e o momento das aplicações são fatores importantes nas decisões internas relacionadas à produção e ao manejo da safra. Portanto, o objetivo deste trabalho foi avaliar os efeitos de tempos e doses de aplicação de boro no solo sobre a qualidade da soja, e sua influência nas características fisiológicas após o armazenamento. Dois experimentos foram conduzidos um na Universidade Estadual de Goiás e outro na Universidade Federal de Pelotas. A primeira fase foi em campo, conduzida em blocos casualizados em esquema fatorial 5 x 6, com cinco estágios (V0, V3, V6, V9 e R1) de aplicação de boro e seis diferentes doses (0, 1, 2, 3, 4 e 5 kg ha-1) dela, quatro repetições, foram avaliandos o teor de boro nas folhas, o índice de clorofila e a produtividade de sementes em kg ha-1. A segunda fase foi elaborada em laboratório com o objetivo de avaliar a qualidade fisiológica e titulação de nutrientes das sementes após 6 meses de armazenamento, em delineamento inteiramente casualizado em esquema fatorial 5 x 6 com quatro repetições, como no campo com 5 estágios e 6 doses. As variáveis analisadas foram: germinação, comprimento da raiz, comprimento da parte aérea, índice de taxa de crescimento, razão de desenvolvimento da raiz, razão de desenvolvimento da parte aérea e concentração de fósforo, nitrogênio, potássio e boro. O estádio reprodutivo apresentou as melhores médias para as variáveis de qualidade das sementes e concentração dos nutriente na semente, enquanto que para as doses, os níveis intermediários apresentaram resultados positivos para a maioria dos testes utilizados no estudo. No entanto, não há justificativa para a fertilização com boro para manter a qualidade das sementes armazenadas.
Abstract Soybeans is one of the crops with significant impact on global food supply, so it’s important to study those relevant factors affecting its production and even the development of its seeds in the field. The macro and micronutrients are important, and continuously soybeans is concerned, boron is extremely important for biological nitrogen fixation, and participating in the physiological seed formation processes. Since the nutrient is not very mobile in the plant, some authors recommend foliar spraying to improve seed productivity and quality. Dose and timing of applications are important factors inner decisions related to crop production and management. Therefore, the goal of this work was to evaluate effects of times and doses of soil boron application on soybean quality, and its influence on the physiological characteristics after storage. Two experiments were conducted at the State University of Goiás and Federal University of Pelotas. The first in the field, conducted in a randomized block in a 5 x 6 factorial scheme, with five stages (V0, V3, V6, V9 and R1) of boron application and six different doses (0, 1, 2, 3, 4 and 5 kg ha-1) of it, four replications, evaluating leaf boron content, chlorophyll index and seed yield in kg ha-1. The second phase was contrived in the laboratory aimed to evaluate the physiological quality and nutrient titration of seeds after 6 months of storage, with a completely randomized design in a 5 x 6 factorial arrangement with four replications as in the field with 5 stages and 6 doses. The variables analyzed were: germination, root length, shoot length, growth rate index, root development ratio, shoot development ratio and concentration of phosphorus, nitrogen, potassium and boron. The reproductive stage showed the best averages for seed quality variables and concentration nutrients, whereas for doses, the intermediate levels showed positive results for most of the tests used in the study. However, there isn’t justification for fertilization with boron to maintain the stored seeds quality.

RESUMO Objetivo: Definir o perfil epidemiológico e os principais determinantes de morbimortalidade dos pacientes cirúrgicos não cardíacos de alto risco no Brasil. Métodos: Estudo prospectivo, observacional e multicêntrico. Todos os pacientes cirúrgicos não cardíacos admitidos nas unidades de terapia intensiva, ou seja, considerados de alto risco, no período de 1 mês, foram avaliados e acompanhados diariamente por, no máximo, 7 dias na unidade de terapia intensiva, para determinação de complicações. As taxas de mortalidade em 28 dias de pós-operatório, na unidade de terapia intensiva e hospitalar foram avaliadas. Resultados: Participaram 29 unidades de terapia intensiva onde foram realizadas cirurgias em 25.500 pacientes, dos quais 904 (3,5%) de alto risco (intervalo de confiança de 95% - IC95% 3,3% - 3,8%), tendo sido incluídos no estudo. Dos pacientes envolvidos, 48,3% eram de unidades de terapia intensiva privadas e 51,7% de públicas. O tempo de internação na unidade de terapia intensiva foi de 2,0 (1,0 - 4,0) dias e hospitalar de 9,5 (5,4 - 18,6) dias. As taxas de complicações foram 29,9% (IC95% 26,4 - 33,7) e mortalidade em 28 dias pós-cirurgia 9,6% (IC95% 7,4 - 12,1). Os fatores independentes de risco para complicações foram Simplified Acute Physiology Score 3 (SAPS 3; razão de chance − RC = 1,02; IC95% 1,01 - 1,03) e Sequential Organ Failure Assessment Score (SOFA) da admissão na unidade de terapia intensiva (RC =1,17; IC95% 1,09 - 1,25), tempo de cirurgia (RC = 1,001; IC95% 1,000 - 1,002) e cirurgias de emergências (RC = 1,93; IC95% 1,10 - 3,38). Em adição, foram associados com mortalidade em 28 dias idade (RC = 1,032; IC95% 1,011 - 1,052) SAPS 3 (RC = 1,041; IC95% 1,107 - 1,279), SOFA (RC = 1,175; IC95% 1,069 - 1,292) e cirurgias emergenciais (RC = 2,509; IC95% 1,040 - 6,051). Conclusão: Pacientes com escores prognósticos mais elevados, idosos, tempo cirúrgico e cirurgias emergenciais estiveram fortemente associados a maior mortalidade em 28 dias e mais complicações durante permanência em unidade de terapia intensiva.
ABSTRACT Objective: To define the epidemiological profile and the main determinants of morbidity and mortality in noncardiac high surgical risk patients in Brazil. Methods: This was a prospective, observational and multicenter study. All noncardiac surgical patients admitted to intensive care units, i.e., those considered high risk, within a 1-month period were evaluated and monitored daily for a maximum of 7 days in the intensive care unit to determine complications. The 28-day postoperative, intensive care unit and hospital mortality rates were evaluated. Results: Twenty-nine intensive care units participated in the study. Surgeries were performed in 25,500 patients, of whom 904 (3.5%) were high-risk (95% confidence interval - 95%CI 3.3% - 3.8%) and were included in the study. Of the participating patients, 48.3% were from private intensive care units, and 51.7% were from public intensive care units. The length of stay in the intensive care unit was 2.0 (1.0 - 4.0) days, and the length of hospital stay was 9.5 (5.4 - 18.6) days. The complication rate was 29.9% (95%CI 26.4 - 33.7), and the 28-day postoperative mortality rate was 9.6% (95%CI 7.4 - 12.1). The independent risk factors for complications were the Simplified Acute Physiology Score 3 (SAPS 3; odds ratio - OR = 1.02; 95%CI 1.01 - 1.03) and Sequential Organ Failure Assessment Score (SOFA) on admission to the intensive care unit (OR = 1.17; 95%CI 1.09 - 1.25), surgical time (OR = 1.001, 95%CI 1.000 - 1.002) and emergency surgeries (OR = 1.93, 95%CI, 1.10 - 3.38). In addition, there were associations with 28-day mortality (OR = 1.032; 95%CI 1.011 - 1.052), SAPS 3 (OR = 1.041; 95%CI 1.107 - 1.279), SOFA (OR = 1.175, 95%CI 1.069 - 1.292) and emergency surgeries (OR = 2.509; 95%CI 1.040 - 6.051). Conclusion: Higher prognostic scores, elderly patients, longer surgical times and emergency surgeries were strongly associated with higher 28-day mortality and more complications during the intensive care unit stay.

Abstract INTRODUCTION: The number of syphilis cases among pregnant women in Brazil has increased. This study aimed to analyze the temporal trend of syphilis indicators among pregnant women in Northeast Brazil. METHODS: A time-series study was performed. RESULTS: We observed an increase in the detection rate of syphilis among pregnant women, those aged 15-19 years, and those of brown ethnicity. A strong correlation was observed between the detection rate of syphilis and family health strategy coverage. CONCLUSIONS: Despite an increase in primary care coverage, The increase in cases of syphilis among pregnant women is still considered a challenge.

Objetivou-se com esta pesquisa avaliar a resposta produtiva e os ciclos de colheita do capim-marandu irrigado e adubado com combinações contendo ureia e esterco bovino em diferentes períodos. Utilizou-se delineamento em blocos casualizados, em esquema de parcelas subdivididas no tempo com 5 tratamentos e 5 repetições. Foram testados cinco níveis de substituição da ureia por esterco bovino (0; 25; 50; 75 e 100% de esterco bovino) em três épocas de crescimento. O acúmulo de forragem total e de folhas com a inclusão do esterco em substituição a ureia reduziu em 59,24% e 61,23%, respectivamente. A produção acumulada e o número de ciclos de colheita decresceram com a inclusão de esterco bovino, de 44,7% e 46,66% respectivamente. As porcentagens de folha, colmo e morto foram influenciadas somente pela época, sendo os melhores resultados observados no segundo período. A produção acumulada e o número de ciclos de colheita decresceram com a inclusão de esterco bovino. A produção de forragem e os ciclos de colheita são afetados negativamente pela inclusão do esterco em substituição à ureia.
The objective of this work was to evaluate the productive response and harvest cycles of irrigated and fertilized marandu grass with combinations containing urea and bovine manure in different periods. A randomized complete block design was used, in a plot scheme subdivided in time with 5 treatments and 5 replications. Five levels of urea replacement by bovine manure (0, 25, 50, 75 and 100% bovine manure) were tested at three growing seasons. The accumulation of forage and leaves with the inclusion of manure in substitution of urea decreased by 59.24% and 61.23%, respectively. The accumulated production and the number of harvest cycles decreased with the inclusion of bovine manure, 44.7% and 46.66%, respectively. The percentages of leaf, stem and dead were influenced only by the season, and the best results were observed in the second period. The accumulated production and the number of harvest cycles decreased with the inclusion of bovine manure. Forage production and harvest cycles are negatively affected by the inclusion of manure as a substitute for urea.
