Abstract This publication is an update of the “Consensus on the therapeutic management of atopic dermatitis - Brazilian Society of Dermatology” published in 2019, considering the novel, targeted-oriented systemic therapies for atopic dermatitis. The initial recommendations of the current consensus for systemic treatment of patients with atopic dermatitis were based on a recent review of scientific published data and a consensus was reached after voting. The Brazilian Society of Dermatology invited 31 experts from all regions of Brazil and 2 international experts on atopic dermatitis who fully contributed to the process. The methods included an e-Delphi study to avoid bias, a literature search and a final consensus meeting. The authors added novel approved drugs in Brazil and the indication for phototherapy and systemic therapy for AD. The therapeutical response to systemic treatment is hereby reported in a suitable form for clinical practice and is also part of this updated manuscript.

ABSTRACT The aim of this study was to determine the spontaneous decolonization period and characteristics in a prospective cohort of newborns colonized by multidrug-resistant organisms, after their discharge from the neonatal intensive care unit. Multidrug resistance is defined as bacterial non-susceptibility to ≥ 1 agent of ≥ 3 antimicrobial categories. In total, 618 newborns were included in the study, of which 173 (28.0%) presented a positive culture for multidrug-resistant microorganisms, and of these, 52 (30.1%) were followed up in this study. The most frequent intrinsic factors were be born by cesarean section (86.5%), prematurity (84.6%), and very low birth weight (76.9%). The extrinsic factors were having remained hospitalized for an average of 27 days, during which 67.3% were submitted to invasive procedures and 88.5% received antimicrobials. The intrinsic and extrinsic factors of newborns were not associated to a decolonization period longer or shorter than 3 months, which was the average period of decolonization found in the present study. From the totality of colonization cultures sampled at hospital discharge, the Gram-negative Extended Spectrum β-lactamase producing bacteria were the most common, with 28.9% of babies colonized by Klebsiella spp. The median period of decolonization by multidrug-resistant microorganisms in the newborns population after hospital discharge was 3 months, but was highly dependent on the microbial species, and this period was not associated to any intrinsic and extrinsic factors of the newborn.

ABSTRACT This review aims to demonstrate the possible formation of Ethyl carbamate (EC) in different production stages of cachaça. EC is a carcinogen confirmed in animal experiments and possibly carcinogenic to humans. EC incidence below relevant health limits naturally affects many fermented foods and beverages. Concentrations above 1 mg L–1 are often detected in certain spirits derived from cyanogenic plants. In Brazil, cachaça is widely consumed and therefore consumers are at health risk if they drink EC beverages at high concentrations, since the amount of EC in cachaça (sugar cane brandy) constantly exceeds the threshold 0.21 mg L–1, damaging the health of consumers and hindering exportation to North America and Europe. Many studies have reported on EC formation at different stages of the cachaça production process. Most studies refer to EC formation during fermentation and distillation. In fermentation, reaction occurs between ethanol and nitrogen precursors, such as urea (H2NCONH2) and cyanide (CN–) to form EC. In distillation, cupric ions catalyze cyanide conversion from EC; however, in discontinuous distillation, the bottling fraction of cachaça reduced EC concentration. Although this review describes the possible routes of EC formation at different production stages of cachaça, it presents some studies on EC reduction in alcoholic beverages.

ABSTRACT Zika virus (ZIKV) clinical presentation and frequency/duration of shedding need further clarification. Symptomatic ZIKV-infected individuals identified in two hospitals in Sao Paulo State, Brazil, were investigated regarding clinical characteristics, shedding in body fluids, and serodynamics. Ninety-four of 235 symptomatic patients (Site A: 58%; Site B: 16%) had Real-Time PCR-confirmed ZIKV infection; fever, headache and gastrointestinal symptoms were less frequent, and rash was more frequent compared to ZIKV-negative patients. Real-Time PCR in serum had worse performance compared to plasma, while urine had the highest sensitivity. Shedding in genital fluids and saliva was rare. IgM positivity was the highest <14 days after the symptoms onset (86%), decreasing >28 days (24%); IgG positivity increased >14 days (96%) remaining positive in 94% of patients >28 days. ZIKV prevalence varied importantly in two neighboring cities during the same transmission season. Urine Real-Time PCR can improve diagnostic sensitivity; serum testing is less useful. Accurate serological tests are needed to improve diagnosis and surveillance.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

The objective of this study was to conduct a review of the legal proceedings involving liability lawsuits filed against dentists in Ribeirão Preto, São Paulo, Brazil. A search was carried out of professionals duly registered with the Regional Council of Dentistry of São Paulo seeking professionals and clinics involved in civil proceedings in Ribeirão Preto - São Paulo. The results noted a significant increase (73.3%) in the number of lawsuits filed in the last five years, with the specialty in dental prostheses (35.6%) being the most frequent. It can be concluded that it is important for professionals to have information in reference to the increasing number of lawsuits in professional/patient relationship and that prevention measures are necessary for a safer dental practice.
El objetivo del trabajo fue realizar una revisión de los procesos judiciales que involucran demandas de responsabilidad civil interpuestas contra cirujanos-dentistas en el Forum de Ribeirão Preto, Estado de São Paulo, Brasil. Fue realizada una búsqueda nominalde profesionales y clínicas, debidamente inscritos en el Consejo Regional de Odontología de São Paulo, involucrados en procesos civiles en el municipio de Ribeirão Preto - São Paulo. Como resultado fue observado un significativo aumento (73,3%) en el número de demandas presentadas en los últimos cinco años, siendo la especialidad de prótesis dental (35,6%) la más involucrada. Se puede concluir que es importante el conocimiento por parte del profesional en cuanto al aumento de divergencias en la relación profesional/paciente y algunas medidas son necesariaspara su prevención, para que los cirujanos-dentistas busquen una práctica profesional más segura.

O suporte ventilatório artificial invasivo e não invasivo ao paciente grave tem evoluído e inúmeras evidências têm surgido, podendo ter impacto na melhora da sobrevida e da qualidade do atendimento oferecido nas unidades de terapia intensiva no Brasil. Isto posto, a Associação de Medicina Intensiva Brasileira (AMIB) e a Sociedade Brasileira de Pneumologia e Tisiologia (SBPT) - representadas por seu Comitê de Ventilação Mecânica e sua Comissão de Terapia Intensiva, respectivamente, decidiram revisar a literatura e preparar recomendações sobre ventilação mecânica, objetivando oferecer aos associados um documento orientador das melhores práticas da ventilação mecânica na beira do leito, com base nas evidências existentes, sobre os 29 subtemas selecionados como mais relevantes no assunto. O projeto envolveu etapas que visaram distribuir os subtemas relevantes ao assunto entre experts indicados por ambas as sociedades, que tivessem publicações recentes no assunto e/ou atividades relevantes em ensino e pesquisa no Brasil, na área de ventilação mecânica. Esses profissionais, divididos por subtemas em duplas, responsabilizaram-se por fazer uma extensa revisão da literatura mundial. Reuniram-se todos no Fórum de Ventilação Mecânica, na sede da AMIB, na cidade de São Paulo (SP), em 3 e 4 de agosto de 2013, para finalização conjunta do texto de cada subtema e apresentação, apreciação, discussão e aprovação em plenária pelos 58 participantes, permitindo a elaboração de um documento final.

Perspectives on invasive and noninvasive ventilatory support for critically ill patients are evolving, as much evidence indicates that ventilation may have positive effects on patient survival and the quality of the care provided in intensive care units in Brazil. For those reasons, the Brazilian Association of Intensive Care Medicine (Associação de Medicina Intensiva Brasileira - AMIB) and the Brazilian Thoracic Society (Sociedade Brasileira de Pneumologia e Tisiologia - SBPT), represented by the Mechanical Ventilation Committee and the Commission of Intensive Therapy, respectively, decided to review the literature and draft recommendations for mechanical ventilation with the goal of creating a document for bedside guidance as to the best practices on mechanical ventilation available to their members. The document was based on the available evidence regarding 29 subtopics selected as the most relevant for the subject of interest. The project was developed in several stages, during which the selected topics were distributed among experts recommended by both societies with recent publications on the subject of interest and/or significant teaching and research activity in the field of mechanical ventilation in Brazil. The experts were divided into pairs that were charged with performing a thorough review of the international literature on each topic. All the experts met at the Forum on Mechanical Ventilation, which was held at the headquarters of AMIB in São Paulo on August 3 and 4, 2013, to collaboratively draft the final text corresponding to each sub-topic, which was presented to, appraised, discussed and approved in a plenary session that included all 58 participants and aimed to create the final document.

Cultivares de batata com bom desempenho produtivo e cujos tubérculos sejam resistentes ao esverdeamento, são de grande importância econômica para o setor produtivo. Objetivou-se com esse trabalho avaliar a produtividade, a classificação, o teor de proteína e a resistência ao esverdeamento pós-colheita de tubérculos de cultivares de batata, produzidos na safra de inverno. O experimento foi conduzido durante a safra de inverno de 2008, no município de Itaí, SP. O delineamento experimental utilizado foi o de blocos casualizados, com quatro repetições. Os tratamentos consistiram de cinco cultivares de batata (Ágata, Asterix, Atlantic, Markies e Mondial). Foram avaliados: produtividade, classificação pelo diâmetro transversal, peso médio, teor de proteína e esverdeamento dos tubérculos. As cultivares Mondial, Asterix e Ágata apresentaram maior produtividade total e comercial de tubérculos. A cultivar Mondial apresentou maior produtividade de tubérculos graúdos. A cultivar Atlantic apresentou menor produtividade de tubérculos, porém, com alta porcentagem de tubérculos comercializáveis. As cultivares Markies, Atlantic e Asterix apresentaram maior teor de proteínas nos tubérculos. A cultivar Markies apresentou menor suscetibilidade ao esverdeamento na fase inicial de armazenamento. A cultivar Asterix apresentou maior intensidade de esverdeamento dos tubérculos.
Potato cultivars with good production performance and with resistance to greening are of great economic importance to the productive sector. This study aimed to evaluate yield, classification, protein content and resistance to postharvest greening of potato tubers of different cultivars. The experiment was carried out during the winter cropping season of 2008 in Itaí, São Paulo State, Brazil. The experimental design was the completely randomized block with four replications. Treatments consisted of five potato cultivars (Ágata, Asterix, Atlantic, Markies and Mondial). Yield, classification by diameter, weight, protein content and greening of tubers were evaluated. Mondial, Asterix and Ágata cultivars showed higher total and commercial tuber yield. Mondial showed higher yield of large tubers. Atlantic showed lower tuber yield, with high percentage of commercial tubers. Markies, Atlantic and Asterix cultivars showed higher protein content of tubers. Markies showed lower susceptibility to the greening in the initial stage of storage. Asterix showed higher intensity of greening in the tubers.

Foram estabelecidos critérios de interpretação do teste de tuberculinização aplicado ao diagnóstico da tuberculose em grupos de caprinos experimentalmente sensibilizados. Dos 30 animais utilizados, dez foram sensibilizados com Mycobacterium avium - amostra D4 (grupo A) e dez com Mycobacterium bovis - amostra AN5 (grupo B). Dez caprinos não foram sensibilizados e constituíram o grupo controle (grupo C). No teste cervical simples, realizado com tuberculina bovina (M. bovis) e lido às 72 horas pós-tuberculinização (p.t.), as reações positivas foram aquelas em que houve aumento da espessura da dobra de pele igual ou superior a 3,9mm; reações inconclusivas, quando situadas entre 1,8 e 3,8mm, e negativas quando iguais ou menores que 1,7mm. A análise dos resultados do teste cervical comparativo, realizado com o M. avium e M. bovis e lido às 72 horas p.t., indicou reação positiva quando o aumento da espessura da dobra da pele induzida pela tuberculina bovina superou a reação à aviária em pelo menos 2,5mm; reação inconclusiva quando a diferença entre a reação à tuberculina bovina e à tuberculina aviária ficou situada entre 1,9 e 2,4mm; e negativa quando a reação bovina ultrapassou a aviária em até 1,8mm. Às 96 horas após a injeção da tuberculina, foi efetuada a avaliação histológica do local das reações tuberculínicas colhendo-se amostras de pele de cinco caprinos dos grupos A e B e de quatro animais do grupo controle; os resultados confirmaram a presença, nos grupos sensibilizados, de infiltrado inflamatório, constituído, preferencialmente, por células mononucleares.
The tuberculin skin test was established with the aim to be applied in the diagnosis of tuberculosis in experimentally sensitized goats. Thirty goats were alocated into three groups with ten animals each. The animals in group A were sensitized with Mycobacterium avium sample D4; group B with Mycobacterium bovis sample AN5; and group C (control) was inoculated with saline solution. The results of the simple cervical test after 72h of bovine tuberculin inoculation was interpreted as follow: positive reaction, when there was an increase in the skin fold thickness greater than 3.9mm; suspicious, when from 1.8 to 3.8mm; and negative when less than 1.7mm. The analysis of the results of the comparative cervical test between M. avium and M. bovis and analysed, 72h after tuberculin, indicated positive reactions, when the increase in skin fold thickness induced by M. bovis was greater than that one induced by the avian tuberculin at least 2.5mm; it was considered suspicious when the difference between the bovine and avian tuberculin reactions was from 1.9 to 2.4mm; and negative when this difference was smaller than 1.8mm. The histological evaluation of the local specific tuberculin response, were performed in skin samples collected from five goats in group A, five in group B, and four in group C, the results were the presence of mononuclear inflammatory infiltrate at 96h after tuberculin inoculation.
