Abstract Over the last three decades, stroke care has undergone significant transformations mainly driven by the introduction of reperfusion therapy and the organization of systems of care. Patients receiving treatment through a well-structured stroke service have a much higher chance of favorable outcomes, thereby decreasing both disability and mortality. In this article, we reviewed the scientific evidence for stroke reperfusion therapy, including thrombolysis and thrombectomy, and its implementation in the public health system in Brazil.

Resumo Nas últimas três décadas, o tratamento do AVC sofreu transformações significativas, impulsionadas principalmente pela introdução das terapias de reperfusão e pela organização dos serviços de AVC. Os pacientes que recebem tratamento em um serviço de AVC bem estruturado têm uma probabilidade muito maior de resultados favoráveis, diminuindo assim a incapacidade funcional e a mortalidade. Neste artigo, revisamos as evidências científicas para as terapias de reperfusão do AVC, incluindo trombólise e trombectomia e sua implementação no sistema público de saúde no Brasil.

ABSTRACT Objective: To report outcomes from the largest multicenter series of penile cancer patients undergoing video endoscopic inguinal lymphadenectomy (VEIL). Materials and Methods: Retrospective multicenter analysis. Authors of 21 centers from the Penile Cancer Collaborative Coalition-Latin America (PeC-LA) were included. All centers performed the procedure following the same previously described standardized technique. Inclusion criteria included penile cancer patients with no palpable lymph nodes and intermediate/high-risk disease and those with non-fixed palpable lymph nodes less than 4 cm in diameter. Categorical variables are shown as percentages and frequencies whereas continuous variables as mean and range. Results: From 2006 to 2020, 210 VEIL procedures were performed in 105 patients. Mean age was 58 (45-68) years old. Mean operative time was 90 minutes (60-120). Mean lymph node yield was 10 nodes (6-16). Complication rate was 15.7%, including severe complications in 1.9% of procedures. Lymphatic and skin complications were noted in 8.6 and 4.8% of patients, respectively. Histopathological analysis revealed lymph node involvement in 26.7% of patients with non-palpable nodes. Inguinal recurrence was observed in 2.8% of patients. 10y- overall survival was 74.2% and 10-y cancer specific survival was 84.8%. CSS for pN0, pN1, pN2 and pN3 were 100%, 82.4%, 72.7% and 9.1%, respectively. Conclusion: VEIL seems to offer appropriate long term oncological control with minimal morbidity. In the absence of non-invasive stratification measures such as dynamic sentinel node biopsy, VEIL emerged as the alternative for the management of non-bulky lymph nodes in penile cancer.

ABSTRACT Brackish waters has been increasingly used in hydroponic systems for the cultivation of vegetables. However, its use can cause significant losses in crop production. Therefore, new alternatives to enhance the tolerance of plants to salt stress are being studied, including seed priming with hydrogen peroxide (H2O2). Thus, this study aimed to assess the seed priming with H2O2 at different periods of exposure for enhancing the production, water status and pigments concentration of crisp lettuce grown under salt stress. The experiment was carried out under protected conditions, in a completely randomized design, with four replicates. The plants were cultivated in a floating hydroponic system, containing nutrient solution. Five treatments were tested: control (absence of H2O2 and absence of NaCl); salt control (absence of H2O2 and presence of 100 mM NaCl); 0.1 mM H2O2 (12 hours) + 100 mM NaCl; 0.1 mM H2O2 (24 hours) + 100 mM NaCl, and 0.1 mM H2O2 (36 hours) + 100 mM NaCl. In general, salinity reduced the height, production of the fresh and dry mass of the shoot, relative water content, and chlorophylls concentration of lettuce plants. However, the application of 0.1 mM H2O2 for 12 and 36 hours on the seeds, enhanced the growth, water status, and chlorophylls concentration of the plants. Seed priming with H2O2 at a 0.1 mM concentration for 12 hours can be recommended to increase tolerance of lettuce plants grown in a hydroponic system under salt stress.

RESUMO As águas salobras vêm sendo cada vez mais utilizadas em sistemas hidropônicos para o cultivo de hortaliças. Entretanto, seu uso pode provocar prejuízos significativos na produção das culturas. Assim, este trabalho teve como objetivo avaliar o condicionamento fisiológico de sementes com H2O2 em diferentes períodos de exposição para melhorar a produção, estado hídrico e teor de pigmentos de alface crespa cultivada sob estresse salino. O experimento foi conduzido em condições protegidas, em delineamento inteiramente casualizado, com quatro repetições. As plantas foram cultivadas em sistema hidropônico do tipo floating, contendo solução nutritiva de Furlani. Foram testados cinco tratamentos: controle (ausência de H2O2 e ausência de NaCl); controle salino (ausência de H2O2 e presença de 100 mM NaCl); 0,1 mM H2O2 (12 horas) + 100 mM NaCl; 0,1 mM H2O2 (24 horas) + 100 mM NaCl e 0,1 mM H2O2 (36 horas) + 100 mM NaCl. Em geral, a salinidade reduziu a altura, a produção de massa fresca e seca da parte aérea, o teor relativo de água e o teor de clorofilas das plantas de alface. No entanto, a aplicação de 0,1 mM de H2O2 por 12 e 36 horas na semente melhorou o crescimento, o estado hídrico e o teor de clorofila das plantas. O condicionamento de sementes com H2O2 na concentração de 0,1 mM por 12 horas pode ser recomendado para aumentar a tolerância de plantas de alface cultivadas em sistema hidropônico sob estresse salino.

Hematological problems are associated with Coronavirus disease 2019 (COVID-19). Respiratory impairment is the higher point studied, although without experimental studies related to the oxygen transport performed by erythrocytes. Therefore, we decided to investigate if erythrocytes from COVID-19 patients have their functionality changed. The case-control study included hospitalized patients with a positive real-time polymerase chain reaction (RT-PCR) result admitted to University Hospital. Volunteers (negative RT-PCR results) were recruited as a control group. Thus, we assessed different erythrocytes parameters, oxidative stress markers, and biophysical studies using whole blood and isolated hemoglobin. We found a decrease of 51% in oxygen uptake and reduced antioxidant enzyme activity in COVID-19 patients compared to controls. Raman spectrometry showed structural changes in the hemoglobin and lipids of the erythrocytes from COVID-19 patients; thus, these results were consolidated with an increase in Young’s modulus in erythrocytes followed by morphology changes. Besides, isolated hemoglobin from COVID-19 patients has a distinct interaction profile using a ligand model compared to the control. COVID-19 leads to structural, functional, and morphological damage to human erythrocytes. Our data showed structural and molecular changes and induction of oxidative stress in erythrocytes by COVID-19, a new perspective on the contribution of erythrocytes to a respiratory commitment in COVID-19.

Progenesis QI (PQI) is a multiplatform bioinformatics tool that facilitates the identification workflow for metabolomics experiments. PQI uses fragmentation data provided by MassBank of North America (MoNA) libraries, among others, for metabolite annotation. However, PQI does not officially support MoNA libraries and other libraries based on structure-data files (.sdf). This paper describes the development and application of a software named MoNA to Progenesis QI Library Converter, allowing PQI and MoNA by correcting the fragmentation data of the library for Progenesis readability. We evaluated several public experimental datasets, including human plasma, plant extracts, cultured cells, bacteria, rat serum, and rat hippocampus. The results showed that it is mandatory to proceed with file conversion of each library to allow PQI to access fragmentation information from .msp (main spectra profile) files. This step is highly recommended to improve the identification level of the metabolites.

Abstract Background Leprosy presents itself heterogeneously, which requires the recognition of profile and spatial distribution for the effectuation of control actions. Objective To describe the epidemiological profile and the spatial distribution of leprosy cases in the Paraíba, Brazil. Method Ecological study on a secondary basis with a quantitative approach. The study site was the 223 municipalities in the state of Paraíba. The data were collected in the Notifiable Diseases Information System/Leprosy for the period from 2015 to 2019. Descriptive statistics and spatial analysis were used with the aid of the R software. Results 3,218 cases were analyzed for the epidemiological profile and 3,212 for the spatial analysis. There was a greater record in the year of 2019 (n = 778;24.2%); in males (n = 1,783; 55.5%); age group 40 to 59 years (n = 1,236; 38.4%); multibacillary classification (n = 2,095; 65.2%); dysmorphic form (n = 970; 30.2%); and degree of physical disability 0 (n = 1,611; 50.2%). 171 (76.7%) municipalities that reported cases were identified, and simple and compound conglomerates were detected involving 31 municipalities located further east and west of the state. Conclusion It should be noted that leprosy remains an active transmission chain in Paraíba and has a heterogeneous geographic distribution, reaffirming the importance of planning and executing more resolute control actions.

Resumo Introdução A hanseníase se apresenta de forma heterogênea, o que requer o reconhecimento do perfil e distribuição espacial para a efetivação de ações de controle. Objetivo Descrever o perfil epidemiológico e a distribuição espacial dos casos de hanseníase na Paraíba. Método Estudo ecológico, de base secundária e abordagem quantitativa. Os loci do estudo foram os 223 municípios do estado. Os dados foram coletados no Sistema de Informação de Agravos de Notificação/Hanseníase correspondentes ao período de 2015 a 2019. Utilizou-se de estatística descritiva e análise espacial com auxílio do software R. Resultados Foram analisados 3.218 casos para o perfil epidemiológico e 3.212 para a análise espacial. Verificou-se maior registro no ano de 2019 (n=778;24,2%); no sexo masculino (n=1.783; 55,5%); faixa etária 40 a 59 anos (n=1.236; 38,4%); classificação multibacilar (n=2.095; 65,2%); forma dimorfa (n= 970; 30,2%), e grau de incapacidade física 0 (n=1.611; 50,2%). Identificaram-se 171 (76,7%) municípios que notificaram casos, e houve detecção de conglomerados simples e compostos envolvendo 31 municípios, situados mais ao Leste e Oeste do estado. Conclusão Ressalta-se que a hanseníase permanece com cadeia de transmissão ativa na Paraíba e com distribuição geográfica heterogênea, reafirmando a importância de planejar e executar ações de controle mais resolutivas.

Abstract This study compares the morphology, thermal, and dynamic-mechanical properties of composites based on polybutylene adipate terephthalate/polylactide biocomposites with sponge gourd waste treated code as R, and non-treated sponge gourd, coded as NR, by mechanical disc refining after milled process. Extrusion followed by compression molding was used to produce biocomposites with fiber contents of 0, 2.5, 5, 10, and 15% wt/wt for R and NR sponge gourd fibers. Scanning electron microscopy analysis reveals that NR has the morphology of a rigid tubular shape, whereas R is a thinner, twisted, and fibrillated fiber. Regardless of the type of sponge gourd fiber used, the thermal stability of the composite decreases as the sponge gourd content increases. At 25°C, the biocomposite with 10%wt/wt R fiber has the highest storage modulus value. The comparison of Tangent peak values reveals that the presence of sponge gourd fibers reduces the energy dissipation of the biocomposites. The analysis of the loss modulus at 25°C reveals that R fiber contributes more to the reduction of energy dissipation of the biocomposites than NR. Furthermore, the Cole-Cole plot shows that R and NR fibers are dispersed and do not significantly change the homogeneity of the biopolymer systems.

ABSTRACT The objective of this study was to evaluate the effects of replacing choline chloride with a plant source of choline on the locomotor system, liver health, and development of breast myopathies in broilers aged 1-42 days of age. We allocated 1,120 one-day-old Cobb broilers to four treatment groups and fed them commercial diets based on corn and soybean meal. The treatments included choline in the form of 1,800.00 mg/kg choline chloride; 1,350.00 mg/kg choline chloride + 450.00 mg/kg herbal choline; 900.00 mg/kg choline chloride + 900.00 mg/kg herbal choline; and 1,000.00 mg/kg herbal choline. Each treatment group had eight replications. Throughout the experiment, gait score, footpad dermatitis, hock burn, and leg deformities (valgus and varus) were evaluated in the birds at 28 and 35 days of age. After slaughter, parameters such as breast myopathies, tibial dyschondroplasia score, and histological slides of the pectoral muscle, liver, and proximal tibial epiphysis were assessed. The results demonstrated good hepatic and locomotor health in the broilers, as no classical signs of choline deficiency were observed. Statistical analyses indicated no significant differences between treatments in terms of liver and locomotor health, suggesting that broilers fed diets supplemented with the plant source did not experience choline deficiency. Additionally, no statistically significant differences were found between treatments regarding breast myopathies. Overall, the tested choline plant source can effectively replace choline chloride in broiler diets.

ABSTRACT Cyrtopodium R. Br. is a neotropical genus with 50 species, of which 37 are present in Brazil, especially in the Cerrado, its center of diversity and endemism. Works on the genus for the country are limited to classical literature, a synoptical treatment, descriptions of new species, and floristic listings. We present a taxonomic synopsis for Cyrtopodium species occurring in the municipality of Niquelândia, Goiás State, Brazil. Fourteen species were found (C. blanchetii, C. braemii, C. brandonianum, C. brunneum, C. cristatum, C. confusum, C. eugenii, C. paludicolum, C. poecilum, C. saintlegerianum, C. triste, C. vernum, C. virescens and C. withnerii), among which C. confusum constitutes a new record for the Goiás State. The species are contrasted by means of a key, have their geographic distributions and morphological relationships commented and are represented by images. Cyrtopodium confusum, the novelty registered here, is described and its distribution mapped.

RESUMO Cyrtopodium R. Br. é um táxon neotropical com 50 espécies, das quais 37 estão presentes no Brasil, especialmente no Cerrado, seu centro de diversidade e endemismo. Trabalhos sobre o gênero no país resumem-se à literatura clássica, a um tratamento sinóptico, a descrição de novas espécies e a listagens florísticas. Apresentamos uma sinopse taxonômica para os taxa de Cyrtopodium ocorrentes no munucípio de Niquelândia, Estado de Goiás, Brasil. Foram encontradas 14 espécies (C. blanchetii, C. braemii, C. brandonianum, C. brunneum, C. cristatum, C. confusum, C. eugenii, C. paludicolum, C. poecilum, C. saintlegerianum, C. triste, C. vernum, C. virescens e C. withnerii), dentre as quais C. confusum é interpretada como a primeira ocorrência para o Estado de Goiás. Tais espécies são contrastadas por meio de uma chave de identificação, têm suas distribuições geográficas e relações morfológicas comentadas e são representadas por imagens. Cyrtopodium confusum, a novidade aqui registrada, é descrita e tem sua distribuição mapeada.

Abstract The objective of this study was to establish populations to start the breeding program of three species of Corymbia that are not widely planted in Brazil yet. To this end, basic density, bark content and growth of seedlots/provenances were evaluated three years after planting. The experiments consisted of 14 treatments for Corymbia citriodora subsp. variegata (CCV), 15 for C. maculata (CM), and five for C. henryi (CH). The species exhibited high survival, indicating adaptation to the Cwa climate while average coefficients of determination of seedlots exhibited intermediate values. The CCV from the Richmond Range (28º 55’ S) exhibited the highest productivity. The mean annual increment was low (22 to 26 m3ha-1), but the basic density (573 to 613 kg m-³) was high compared to the standard for eucalypts in Brazil. The bark content was close to 15% and varied between and within species.

Abstract Background: The observation that 2-deoxy-2[18F]fluoro-D-glucose-positron emission tomography/magnetic resonance imaging ([18F]F-FDG-PET/MRI) revealed high-grade arterial wall FDG uptake, without arterial wall thickening with contrast-enhancement, in a considerable number of c-TA patients in our previous study, encouraged us to compare patients with both PET and MR angiography (MRA) positives, with those with PET positive but MRA negative. Our aim was to evaluate the relevance of these two imaging modalities together. Methods: A three-center cross-sectional study with 17 patients who fulfilled the EULAR/PRINTO/PReS criteria for c-TA and who underwent [18F]F-FDG-PET/MRI was previously performed. Herein we compared patients/vessels with positive PET (arterial wall 18F-FDG uptake higher than liver) and positive MRA (arterial wall thickening with contrast-enhancement)—group 1, with those with positive PET but negative MRA—group 2. Results: Median disease duration of 17 c-TA patients was 10.4 years. Nine patients were classified as group 1 and six as group 2. Median of metabolic inflammatory volume (MIV) of all arterial segments was significantly higher in group 1 (2346 vs. 1177 cm3; p = 0.036). Fifty-four (19%) from 284 available arterial segments presented positive findings in vessel wall in one or both images. Positive findings were concordant between PET and MRA in only 13% arterial segments (group 1); most changes (28–59.6%) that were discordant between both images, were positive in PET and negative in MRA (group 2). Conclusions: Our study demonstrated that [18F]F-FDG-PET/MRI added information about inflammation in vessel wall of c-TA patients. Prospective multicenter studies are needed in order to get solid data to guide immunosuppressive tapering and withdrawal.

Abstract Kavain is one of the main kavalactones of Piper methysticum (Piperaceae) with anxiolytic, analgesic, and antioxidant activities. Therefore, the aim of the study was to evaluate the cytotoxic, mutagenic, and antimutagenic potential of kavain in Allium cepa cells. Roots of A. cepa were transferred to the negative (2% acetone) and positive (10 µg/mL of Methylmethanesulfonate, MMS) controls and to the concentrations of kavain (32, 64 and 128 µg/mL) for 48 h. A total of 5,000 meristematic cells were analyzed under an optical microscope to determine the mitotic index, mean number of chromosomal alterations and percentage of damage reduction. Data were analyzed by Kruskal-Wallis test (p <0.05). All concentrations of kavain were not cytotoxic and did not show significant chromosomal changes when compared to 2% acetone. Kavain showed a cytoprotective effect in the pre (128 μg/mL) and in the post-treatment (32 and 64 μg/mL) and reduced damage against the mutagenic action of MMS in all concentrations of the pre and simultaneous and at the highest of post (128 μg/mL). Kavain promoted a significant reduction in micronuclei, nuclear buds and chromosomal losses in relation to MMS. The observed data indicate the importance of kavain for the inhibition of damage and chemoprevention.

Ambuic acid (AA) is a highly-modified cyclohexenone and known as a promising inhibitor of quorum sensing in methicillin-resistant Staphylococcus aureus, and is thus a candidate as an antivirulence drug. This molecule is mainly produced by the species of Pestalotiopsis and, since its discovery twenty years ago, only a restricted amount of AA-derivatives have been described. Despite being a promising subject, methods for the analysis of modified AA-analogues via mass spectrometry remain unexplored. In order to adress this question, the marine fungus Pestalotiopsis sp. 4A11 associated with the ascidian Didemnum perlucidum was grown in a solid rice medium and its crude extract was chemically studied. From this extract, AA and 10-hydroxy ambuic acid (10-HAA) were isolated and identified using NMR spectroscopy with the aim of obtaining model compounds for the MS analysis. These served as reference compounds (seeds) to guide the dereplication of other AA-analogues via LC-MS/MS-based molecular networking. Based on the manual interpretation of the fragmentation pathways of the seeds and related compounds observed in the networks, six AA-derivatives were dereplicated in the extract. Furthermore, three analogues with unprecedented chemical formulas were proposed as putative unprecedented AA-derivatives. The fragmentation annotation proposed represents a fast and feasible method for characterizing AA-derivatives.

ABSTRACT The use of Precision Agriculture tools and soil attributes has contributed to local agricultural management in the identification of different productive potentials and quality of pastures. The present study aimed to use Precision Agriculture tools to investigate the spatial variability of soil organic matter fractions and soil chemical and physical attributes and to delineate management zones in pasture soils cultivated under tropical conditions. The study was conducted on a hay production farm located in Seropédica, Brazil. Fifty points were collected on a irregular grid and soil samples were taken at depths of 0-0.20 and 0.20-0.40 m and crop productivity was evaluated. Chemical and physical soil organic matter fractionation was performed to obtain data on fulvic acid, humic and humin fractions, mineral-associated and particulate organic carbon, and light organic matter. Geostatistics and multivariate analysis (principal component analysis and k-means clustering) were performed to define the management zones. The results obtained contributed to the division of the pasture area into two regions that can be managed in different ways aiming to increase soil organic matter in a localized manner with the possibility of reducing the use of inputs and directed management that respects the productive potential of the pasture on the farm.

RESUMO Objetivo: Fornecer orientações sobre a frequência e os componentes dos exames oftalmológicos para crianças saudáveis de 0 a 5 anos. Métodos: Essas diretrizes foram desenvolvidas com base em revisão bibliográfica e experiência clínica de um comitê de especialistas. Foram realizadas buscas PubMed/Medline; documentos selecionados não se restringiram a revisões sistemáticas, ensaios clínicos randomizados e estudos observacionais. Quando adequado, o perfil GRADE foi aplicado para graduá-los e o consenso de especialistas foi usado nos tópicos sem evidência científica. Também foram revisadas as recomendações pela Academia Americana de Pediatria, Associação Americana de Oftalmologia Pediátrica e Estrabismo, Academia Americana de Oftalmologia, Royal College of Ophthalmologist e Sociedade Canadense de Oftalmologia. O documento final foi aprovado pela Sociedade Brasileira de Oftalmologia Pediátrica e Sociedade Brasileira de Pediatria. Resultados: Os recém-nascidos devem ser submetidos ao teste do reflexo vermelho e inspeção dos olhos e anexos pelo pediatra dentro de 72 horas de vida ou antes da alta da maternidade. O teste do reflexo vermelho deve ser repetido pelo pediatra durante as consultas de puericultura pelo menos três vezes ao ano durante os primeiros 3 anos de vida. Se factível, um exame oftalmológico completo pode ser feito entre 6 a 12 meses de vida. Até os 36 meses de idade, os marcos visuais, função visual apropriada para a idade, fixação e alinhamento ocular também devem ser avaliados pelo pediatra ou médico da família. Pelo menos um exame oftalmológico completo deve ser realizados entre 3 e 5 anos de idade. O exame deve conter pelo menos inspeção dos olhos e anexos, avaliação da função visual apropriada para a idade, avaliação da motilidade e alinhamento ocular (testes de cobertura), refração sob cicloplegia e avaliação do fundo de olho dilatado. Conclusões: As diretrizes sobre a frequência da avaliação oftalmológica são ferramentas importantes para orientar os médicos sobre a melhor prática a fim de evitar problemas visuais tratáveis na infância, que poderiam comprometer seu desenvolvimento social, escolar e global, além de causar perda permanente da visão.

ABSTRACT Purpose: To provide guidance on the frequency and components of eye examinations for healthy children aged 0 to 5 years. Methods: These guidelines were developed based on the medical literature and clinical experience of an expert committee. PubMed/Medline searches were performed, with selected publications not restricted to systematic reviews, randomized controlled trials, or observational studies. The Grading of Recommendations Assessment, Development, and Evaluation profile was applied when suitable, and for issues without scientific evidence, recommendations were based on expert consensus. Recommendations by the American Academy of Pediatrics, American Association of Pediatric Ophthalmology and Strabismus, American Academy of Ophthalmology, Royal College of Ophthalmologists, and Canadian Ophthalmological Society were also reviewed. The final guideline document was approved by the Brazilian Pediatric Ophthalmology Society and by the Brazilian Pediatric Society. Results: Newborns must undergo the red reflex test and inspection of the eyes and adnexa by a pediatrician within 72 hours of life. The red reflex test should be repeated by the pediatrician during childcare consultations at least three times per year during the first 3 years of life. If feasible, a comprehensive ophthalmologic examination may be performed between 6 and 12 months of age. Until 36 months of age, the pediatrician should assess the infant’s visual development milestones, age-appropriate assessment of visual function, ocular fixation, and eye alignment. At least one comprehensive ophthalmologic examination should be performed at 3 to 5 years of age. The examination should minimally include inspection of the eyes and adnexa, age-appropriate visual function assessment, evaluations of ocular motility and alignment (cover tests), cycloplegic refraction, and dilated fundus. Conclusions: Guidelines concerning the frequency of ophthalmic assessment are important tools for directing physicians regarding best practices that avoid treatable vision problems that affect children’s development, school, and social performance and cause unnecessary permanent vision loss.
