Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

The essay aims to elucidate the potential impact of ChatGPT and other artificial intelligence on academic research across various aspects: information retrieval and review of scholarly literature, data analysis and presentation, as well as writing and translation. We explore potential consequences, risks, and paradoxes in utilizing AI for research, encompassing issues such as authorship dilemmas, erosion of research integrity, limitations in methodological approaches, expected shifts in the dynamics of knowledge production, and changes in the relations between academic center and peripheries. We conclude by calling for comprehensive dialogue on regulation and the development of technologies that align with our evolving needs.

O ensaio procura elucidar as transformações que o Chat GPT e outras inteligências artificiais tendem a causar na pesquisa acadêmica, como: busca, seleção e leitura de literatura acadêmica, análise e apresentação de dados; escrita e tradução. Discutimos algumas possíveis consequências, riscos e paradoxos no uso de IAs para a pesquisa, tais como dilemas de autoria, potencial deterioração da integridade da pesquisa, limitação das abordagens metodológicas, possíveis modificações nas dinâmicas de produção de conhecimento e nas relações centro-periferia no ambiente acadêmico. Concluímos demandando por um diálogo aprofundado sobre regulação e criação de tecnologias adaptadas às nossas necessidades.

SUMMARY OBJECTIVE: This study aimed to evaluate postoperative pain and quality of life in patients undergoing median sternotomy. METHODS: A cohort study was carried out on a sample of 30 patients who underwent elective cardiac surgery by longitudinal median sternotomy. Patients were interviewed at Intensive Care Unit discharge and hospital discharge, when the Visual Numeric Scale and the Brief Pain Inventory were applied, and 2 weeks after hospital discharge, when the World Health Organization Quality of Life-Bref questionnaire was administered. The normality of the results was analyzed by the Shapiro-Wilk test, and Wilcoxon Rank Sum and McNemar tests were utilized for the analysis of numerical and categorical variables. For correlation between numerical variables, Spearman's linear correlation test was applied. To compare numerical variables, Mann-Whitney U and Kruskal-Wallis tests were applied. Differences between groups were considered significant when the p-value was <0.05. RESULTS: Between Intensive Care Unit and hospital discharge, there was a reduction in median pain intensity assessed by the Visual Numeric Scale from 5.0 to 2.0 (p<0.001), as well as in eight Brief Pain Inventory parameters: worst pain intensity in the last 24 h (p=0.001), analgesic relief (p=0.035), and pain felt right now (p=0.009); and in interference in daily activities (p<0.001), mood (p=0.017), ability to walk (p<0.001), relationship with other people (p=0.005), and sleep (p=0.006). Higher pain intensity at Intensive Care Unit discharge was associated with worse performance in the psychological domain of quality of life at out-of-hospital follow-up. CONCLUSION: Proper management of post-sternotomy pain in the Intensive Care Unit may imply better quality of life at out-of-hospital follow-up.

RESUMO Introdução: O objetivo deste estudo foi avaliar se uma única sessão de treinamento intervalado de alta intensidade (HIIT) promoveria efeito hipotensor e risco cardiovascular em mulheres hipertensas, bem como aumentar a biodisponibilidade de óxido nítrico. Métodos: A amostra foi composta por 10 mulheres hipertensas (63,7 ± 10,34 anos; 66, ± 7,67 kg e 153,7 ± 9,08 cm) e a carga de treinamento foi estabelecida em 60% da velocidade aeróbica máxima. Resultados: Observamos um efeito hipotensor muito alto entre os momentos de interação durante a intervenção (Int. Pré: 122,40 ± 18,58; Int. Pós: 143,00 ± 24,90; Int. Pós 60 min.: 121,40 ± 13,87; p < 0,001, η2P = 0,569). Nenhum risco cardiovascular foi observado durante a intervenção (DP = Int. Pré: 9138,20 ± 1805,34; Int. Pós: 14849,70 ± 3387,94; Int. Pós: 60 min.: 9615,90 ± 1124,41, p < 0,001, η2P = 0,739) e não houve aumento da biodisponibilidade de óxido nítrico. Conclusões: Em conclusão, este trabalho revela que uma sessão de HIIT é capaz de gerar efeito hipotensor sem apresentar risco cardiovascular em mulheres hipertensas. Nível de evidência I; Estudo clínico randomizado de alta qualidade com ou sem diferença estatisticamente significativa, mas com intervalos de confiança estreitos .

Results: We observed a very high hypotensive effect between the interaction moments during the intervention (Int. Pre: 122.40 ± 18.58; Int. Post: 143.00 ± 24.90; Int. Post 60min: 121.40 ± 13.87; p<0.001, η2P = 0.569). No cardiovascular risk was observed during the intervention (DP = Int. Pre: 9138.20 ± 1805.34; Int. Post: 14849.70 ± 3387.94; Int. Post 60min: 9615.90 ± 1124.41, p< 0.001, η2P = 0.739) and there was no increase in the bioavailability of nitric oxide. Conclusion: In conclusion, this work reveals that an HIIT session is capable of generating a hypotensive effect while not posing cardiovascular risk in hypertensive women. Level of evidence I; High-quality randomized clinical trial with or without statistically significant difference, but with narrow confidence intervals .

Métodos: La muestra fue compuesta por 10 mujeres hipertensas (63,7 ± 10,34 años; 66 ± 7,67 kg y 153,7 ± 9,08 cm) y la carga de entrenamiento se estableció en el 60% de la velocidad aeróbica máxima. Resultados: Se observó un efecto hipotensor muy elevado entre los momentos de interacción durante la intervención (Int. Pre: 122,40 ± 18,58; Int. Post: 143,00 ± 24,90; Int. Post 60 min: 121,40 ± 13,87; p <0,001, η2P = 0,569). No se observó ningún riesgo cardiovascular durante la intervención (DP = Int. Pre: 9138,20 ± 1805,34; Int. Post: 14849,70 ± 3387,94; Int. RESUMO: O objetivo deste estudo foi avaliar o efeito da suplementação proteica materna sobre o metabolismo energético do músculo esquelético de bezerros de corte. Dezesseis matrizes gestantes foram divididas em três grupos: CONTROLE (não suplementado); MÉDIO (suplementados entre 30 e 180 dias de gestação); e FINAL (suplementado entre 181 e 281 dias de gestação). O suplemento continha 30% de proteína bruta e foi fornecido em quantidades totais iguais aos tratamentos. Trinta dias após o nascimento, amostras de sangue e músculo dos bezerros foram coletadas para análises de expressão gênica, abundância de proteínas e metabólitos. Não foram observadas diferenças (P ≥ 0,15) no peso ao nascimento ou parâmetros de desempenho ao desmame, bem como na expressão dos genes avaliados (P ≥ 0,21). Os bezerros nascidos de matrizes do tratamento CONTROLE apresentaram menor proporção (P = 0,03) de proteína p-AMPK/AMPK no músculo esquelético. Os bezerros nascidos de matrizes do tratamento MÉDIO apresentaram concentração de glicose menor (P = 0,04) do que aqueles nascidos de matrizes do tratamento FINAL. Os resultados observados indicam que a ausência de suplementação proteica em matrizes gestantes pode alterar o metabolismo energético da progênie e refletir em uma inflexibilidade metabólica, a qual pode ocasionar limitações quanto à eficiência energética do tecido muscular esquelético e consequentemente, limitar o desempenho da progênie ao longo da fase pós-natal.

ABSTRACT: This study evaluated the effect of maternal protein supplementation during mid or late gestation on energy metabolism of the skeletal muscle of beef calves. Sixteen pregnant cows were divided into 3 groups: CTRL (not supplemented); MID (supplemented from 30 to 180 days of gestation); and LATE (supplemented from 181 to 281 days of gestation). The supplement contained 30% crude protein. Thirty days after birth, blood and muscle samples of the calves were collected for analyses of gene expression, proteins, and metabolites. No differences (P ≥ 0.15) in birth weight, performance at weaning, or muscle expression of the genes evaluated (P ≥ 0.21) were observed. Calves born to CTRL cows had a lower ratio (P = 0.03) of p-AMPK/AMPK protein in the skeletal muscle. Calves born to MID cows had lower (P = 0.04) glucose concentration than those born to LATE cows. Changes in p-AMPK/AMPK protein, indicated a possible metabolic inflexibility in the skeletal muscle of calves born to CTRL cows. These results indicated that lack of protein supplementation in pregnant cows alter the energy metabolism of their calves and reflect in a metabolic inflexibility.

ABSTRACT BACKGROUND: Bishop-Koop ileostomy has been widely used in pediatric patients with the intention of including as much bowel as possible in the intestinal transit early in the management of children with meconium ileus and intestinal atresia. In recent years, we have been using it as an alternative to test the distal bowel function before closure of a previously constructed ostomy in selected children with questionable distal bowel motility. AIMS: The aim of this study was to present our experience with this alternative use of the Bishop-Koop ostomy. METHODS: This is a cross-sectional retrospective review of hospital records, combined with a comprehensive literature review. RESULTS: Seven children were included: five had suspected aganglionosis, one had gastroschisis complicated with ileal atresia, and one had a colonic stricture secondary to necrotizing enterocolitis. In this short series of patients, motility of the distal bowel was correctly assessed in six patients and partially correctly assessed in one patient. One patient did not pass stools per anus after the Bishop-Koop, and he was later confirmed to have Hirschsprung disease. Four patients resumed normal evacuation pattern after closure of the Bishop-Koop. One patient had a Bishop-Koop colostomy because of recurrent enterocolitis after a transanal pull-through. Although he evacuated normally while having the colostomy, the diarrhea recurred after the ostomy was closed. An additional patient, with a severe behavioral problem, did not evacuate per anus after her colostomy was transformed in a Bishop-Koop-type ostomy, despite the apparent presence of normal ganglia in the bowel wall. CONCLUSIONS: Data from the present series allow us to affirm that Bishop-Koop-type ostomy is a safe and efficient procedure that can be used to assess distal bowel function before a definitive transit reconstruction, in children with uncertain motility issues.

RESUMO RACIONAL: A ileostomia Bishop-Koop foi amplamente utilizada em pacientes pediátricos com a intenção de incluir o máximo de intestino possível no trânsito intestinal no manejo inicial de recém-nascidos com íleo meconial e atresia intestinal. Nos últimos anos, temos usado-a como alternativa para testar a função intestinal distal antes do fechamento de uma ostomia, em algumas crianças com motilidade intestinal distal questionável. OBJETIVOS: Apresentar nossa experiência com este uso alternativo da ostomia Bishop-Koop. MÉTODOS: Revisão retrospectiva dos registros hospitalares, combinada com uma revisão abrangente da literatura. RESULTADOS: Sete crianças foram incluídas: cinco tinham suspeita de aganglionose, uma tinha gastrosquise complicada com atresia ileal e uma tinha estenose de colon secundária à NEC. Nesta pequena série de pacientes, a motilidade do intestino distal foi corretamente avaliada em 6 pacientes e parcialmente avaliada em um. Um paciente não evacuou por ânus após o Bishop-Koop e mais tarde foi confirmado que ele tinha doença de Hirschsprung. Seis pacientes retomaram o padrão normal de evacuação após o fechamento do Bishop-Koop. Um paciente que fez uma colostomia Bishop-Koop por causa de enterocolite recorrente após um abaixamento transanal, recidivou a enterocolite após o fechamento definitivo. CONCLUSÕES: A ostomia tipo Bishop-Koop é um procedimento seguro e eficaz que pode ser utilizado para avaliar a função intestinal distal antes de uma reconstrução definitiva do trânsito em crianças com problemas de motilidade intestinal.

ABSTRACT Introduction: in Brazil, trauma is responsible for 40% of deaths in the age group between 5 and 9 years old, and 18% between 1 and 4 years, and bleeding is the leading cause of preventable death in the traumatized child. Conservative management of blunt abdominal trauma with solid organs injury - started in the 60s - is the current world trend, with studies showing survival rates above 90%. The objective was to assess the efficacy and safety of conservative treatment in children with blunt abdominal trauma treated at the Clinical Hospital of the University of Campinas, in the last five years. Methods: retrospective analysis of medical records of patients classified by levels of injury severity, in 27 children. Results: only one child underwent surgery for initial failure of conservative treatment (persistent hemodynamic instability), resulting in a 96% overall success rate of the conservative treatment. Five other children (22%) developed late complications that required elective surgery: a bladder injury, two cases of infected perirenal collections (secondary to injury of renal collecting system), a pancreatic pseudocyst and a splenic cyst. Resolution of the complications was attained in all children, with anatomical and functional preservation of the affected organ. There were no deaths in this series. Conclusion: the conservative initial approach in the treatment of blunt abdominal trauma was effective and safe with high resolution and low rate of complications leading to a high preservation rate of the affected organs. RESUMO Introdução: no Brasil, o trauma é responsável por 40% dos óbitos na faixa etária entre 5 e 9 anos, e 18% entre 1 e 4 anos, e o sangramento é a principal causa de prevenção morte na criança traumatizada. O manejo conservador de trauma abdominal contuso com lesão de órgãos sólidos - iniciado na década de 60 - é a tendência mundial atual, com estudos mostrando taxas de sobrevivência acima de 90%. O objetivo do presente trabalho foi avaliar a eficácia e segurança do tratamento conservador em crianças com trauma abdominal contuso tratado no Hospital das Clínicas da Universidade de Campinas, nos últimos cinco anos. Métodos: análise retrospectiva de prontuários de pacientes classificados por níveis de gravidade da lesão, em 27 crianças. Resultados: apenas uma criança foi submetida a cirurgia por falha inicial do tratamento conservador (instabilidade hemodinâmica persistente), resultando em uma taxa de sucesso global de 96% do tratamento conservador inicial. Outras cinco crianças (22%) desenvolveram complicações tardias que exigiram cirurgias eletivas: lesão na bexiga, dois casos de coleção perirenal infectada (secundária à lesão de sistema de coleta renal), um pseudocisto pancreático e um cisto esplênico. Resolução da complicação foi atingida em todas as crianças, com preservação anatômica e funcional do órgão afetado. Não houve mortes nesta série. Conclusão: a abordagem inicial conservadora no tratamento de trauma abdominal contundente foi eficaz e segura com alta resolução e baixa taxa de complicações levando a uma alta taxa de preservação dos órgãos afetados. Nível de evidência III - estudo prognóstico e terapêutico.

ABSTRACT: This article analyzed the international competitiveness of exports of forest products from 2008 to 2018. The information is available in the Food and Agriculture Organization (FAO) and the indicators used were: revealed comparative advantage (RCA), trade openness index (Oi) and the contribution to trade balance index (CTB). Results showed that the exportation of forest products are competitive, in addition gains in competitiveness have been observed by the RCA and in Europe, countries had losses in detriment of the financial crises and austerity measures adopted. All countries showed a low degree of openness by the Oi and had a comparative advantage by the CTB, except for China and Germany. It is concluded that the countries with bigger comparative advantages in the exportation of forest products were the ones that had contributed more for its commercial balance.

RESUMO: Este artigo analisou a competitividade internacional das exportações de produtos florestais, de 2008 e 2018. As informações estão disponíveis na Organização das Nações Unidas para Agricultura e Alimento (FAO) e os indicadores utilizados foram: vantagem comparativa revelada (RCA), o índice de abertura do comércio (Oi) e o índice de contribuição ao saldo comercial (CTB). Os resultados mostram que as exportações de produtos florestais são competitivas, ademais foram observados pelo RCA ganhos em competitividade e na Europa, os países tiveram perdas em detrimento das crises financeiras e medidas de austeridade adotadas. Todos os países apresentaram baixo grau de abertura pelo Oi e teve vantagem comparativa pelo CTB, com exceção da China e Alemanha. Conclui-se que os países com maiores vantagens comparativas nas exportações de produtos florestais foram os que mais contribuíram para o seu saldo comercial.

This research was aimed to obtain biodegradable polymeric films with antifungal action against Aspergillus niger, through the addition of copper oxide nanoparticles (CuO-NPs) in the commercial polymer (Ecoflex®) and to evaluate its mechanical, morphological, biodegradation, and copper release properties. The microbiological results showed that the films with 4 wt% CuO-NPs inhibited 97.57% the growth of the fungus growth of 1×105 spores·mL-1 compared to the control. The morphological characterization revealed that the CuO-NPs presented irregular form with particles size of 50 to 400 nm and the XRD showed characteristic peaks of copper oxide. The addition of CuO-NPs to Ecoflex® provided increased maximum and rupture tension, strain, and elasticity modulus the obtained films with 1 wt% CuO-NPs and did not alter the biodegradability of the films. It can be said that, through the addition of CuO-NPs to Ecoflex®, it was possible to add antifungal activity for Aspergillus niger without compromising its biodegradability, meeting the safety standards established by legislation regarding the release of copper.

The Montmorillonite-Erucamide nanocomposite (MMT-EU) can be used to reduce and to control the values of coefficient of friction of low-density polyethylene films (LDPE), widely used by the food industry. In this work, the effect of the addition of 5 wt% and 10 wt% of MMT-EU on the mechanical properties of LLDPE films involving the weld region was investigated. The MMT-EU concentration range was defined considering its possible technological application as an additive to stabilize the friction coefficient of polymeric films. The result shows a significant improvement in the values of stiffness, strength and ductility of polymeric films. In addition, it did not change the coefficient of friction, but the brightness of the LLDPE films decreased and the opacity increased, due to the increase in the crystallinity degree of the LLDPE polymer promoted by MMT-EU, not being a problem for the application of these films.

Abstract Objective: To review the literature on the diagnosis and treatment of vestibular schwannoma. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on vestibular schwannoma were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: (1) Diagnosis – audiologic, electrophysiologic tests, and imaging; (2) Treatment – wait and scan protocols, surgery, radiosurgery/radiotherapy, and systemic therapy. Conclusions: Decision making in VS treatment has become more challenging. MRI can diagnose increasingly smaller tumors, which has disastrous consequences for the patients and their families. It is important to develop an individualized approach for each case, which highly depends on the experience of each surgical team.

Composites based on green high-density polyethylene (G-HDPE) and hydroxyapatite (HA) compatibilized with maleic anhydride grafted high-density polyethylene (HDPE-g-MAH) were developed in this work. The main objective was to evaluate the effects of HA and HDPE-g-MAH contents on the mechanical and thermal properties and hydrolytic degradation of composites through design of experiments and statistical analysis, as well as to evaluate the hemolytic stability. Hydroxyapatite acts as a reinforcing agent for the G-HDPE matrix while the HDPE-g-MAH acts as a compatibilizing agent, improving the dispersion of the HA particles in the polymer matrix and thus increasing the mechanical properties. The crystallization temperature and the degree of crystallinity of the polymer matrix were increased with the addition of HA, suggesting the filler acts as a heterogeneous nucleating agent. Hemolysis tests performed on a composite sample with best mechanical performance did not indicate significant hydrolytic degradation, which suggests this composite is a promising material to be used in bone tissue engineering, for application in implants and bone grafts. highdensity high density GHDPE G HDPE (G-HDPE (HA HDPEgMAH g MAH (HDPE-g-MAH work analysis stability engineering grafts

RESUMO: A angiomatose cutânea bovina (ACB) é uma condição rara caracterizada por lesões vasoproliferativas na pele que afetam principalmente bovinos do hemisfério norte. Uma novilha Holandesa de oito meses de idade apresentou dois nódulos cutâneos protuberantes na epiderme, que apresentavam fácil sangramento localizados na fronte e próximo à região pré-escapular direita. As lesões cutâneas dérmicas tinham uma superfície irregular e um parênquima avermelhado heterogêneo macio composto por numerosas arteríolas, veias e capilares de tamanho variável circundadas por tecido fibroso colagenoso abundante. O exame imuno-histoquímico evidenciou células endoteliais revestindo a estrutura vascular proliferativa imunomarcadas para CD31 e fator de Von Willebrand, e células vasculares de músculo liso imunomarcadas para actina de músculo liso. Todas as características clínicas, patológicas e imuno-histoquímicas observadas na novilha Holandesa foram características da ACB. Considerando seu potencial de disseminação hereditária, a ACB deve ser incluída no diagnóstico diferencial dos nódulos cutâneos de fácil sangramento que não respondem aos tratamentos tópicos de rotina para evitar sua disseminação nos rebanhos brasileiros. ABSTRACT: Bovine cutaneous angiomatosis (BCA) is a rare condition characterized by vasoproliferative skin lesions mainly affecting cattle in the northern hemisphere. An eight-month-old Holstein heifer showed two skin easy-bleeding nodules bulging from the epidermis in the forehead and close to the right prescapular region. Skin lesions were within the dermis and had an irregular surface and a soft heterogeneous reddish parenchyma composed of numerous variable-sized arterioles, veins, and capillaries surrounded by abundant collagenous fibrous tissue. Immunohistochemical assays evidenced endothelial cells lining proliferative vascular structure immunolabeled for CD31 and Von Willebrand factor, and vascular smooth muscle cells immunostained for smooth muscle actin. All clinical, pathological, and immunohistochemical features observed in the Holstein heifer were hallmarks of BCA. Considering its potential for hereditary spread, BCA must be included in the differential diagnosis of easily bleeding skin nodules that do not respond to routine topical wound treatments to prevent its spread in Brazilian herds. Abstract The Pyroligneous extract is a product from the combustion of plant biomass with applications in the fields of health, industrial chemistry, and agriculture. The discovery of new molecules with therapeutic potential and of natural origin continues to be one of the great challenges for research centres around the world. The following work aims to analyze, through a technological prospection, the use of pyroligneous extracts for therapeutic purposes. To carry out the study, searches were carried out in documents deposited in Brazil, Europe, and the United States and searched on platforms specialized in patents. The number of inventions using pyroligneous extract with therapeutic applications is still quite small, however, innovations have been observed for the treatment of diseases of great clinical relevance such as cancer and hypertension.
