A cardiomiopatia hipertrófica (CMH) é uma forma de doença do músculo cardíaco de causa genética, caracterizada pela hipertrofia das paredes ventriculares. O diagnóstico requer detecção por métodos de imagem (Ecocardiograma ou Ressonância Magnética Cardíaca) de qualquer segmento da parede do ventrículo esquerdo com espessura > 15 mm, sem outra causa provável. A análise genética permite identificar mutações de genes codificantes de diferentes estruturas do sarcômero responsáveis pelo desenvolvimento da CMH em cerca de 60% dos casos, permitindo o rastreio de familiares e aconselhamento genético, como parte importante do manejo dos pacientes e familiares. Vários conceitos sobre a CMH foram recentemente revistos, incluindo sua prevalência de 1 em 250 indivíduos, não sendo, portanto, uma doença rara, mas subdiagnosticada. A vasta maioria dos pacientes é assintomática. Naqueles sintomáticos, a obstrução do trato de saída do ventrículo esquerdo (OTSVE) é o principal distúrbio responsável pelos sintomas, devendo-se investigar a sua presença em todos os casos. Naqueles em que o ecocardiograma em repouso ou com Manobra de Valsalva não detecta gradiente intraventricular significativo (> 30 mmHg), devem ser submetidos à ecocardiografia com esforço físico para detecção da OTSVE. Pacientes com sintomas limitantes e grave OTSVE, refratários ao uso de betabloqueadores e verapamil, devem receber terapias de redução septal ou uso de novas drogas inibidoras da miosina cardíaca. Por fim, os pacientes adequadamente identificados com risco aumentado de morta súbita podem receber medida profilática com implante de cardiodesfibrilador implantável (CDI).
La miocardiopatía hipertrófica (MCH) es una forma de enfermedad cardíaca de origen genético, caracterizada por el engrosamiento de las paredes ventriculares. El diagnóstico requiere la detección mediante métodos de imagen (Ecocardiograma o Resonancia Magnética Cardíaca) que muestren algún segmento de la pared ventricular izquierda con un grosor > 15 mm, sin otra causa probable. El análisis genético permite identificar mutaciones en genes que codifican diferentes estructuras del sarcómero responsables del desarrollo de la MCH en aproximadamente el 60% de los casos, lo que permite el tamizaje de familiares y el asesoramiento genético, como parte importante del manejo de pacientes y familiares. Varios conceptos sobre la MCH han sido revisados recientemente, incluida su prevalencia de 1 entre 250 individuos, por lo tanto, no es una enfermedad rara, sino subdiagnosticada. La gran mayoría de los pacientes son asintomáticos. En los casos sintomáticos, la obstrucción del tracto de salida ventricular izquierdo (TSVI) es el trastorno principal responsable de los síntomas, y su presencia debe investigarse en todos los casos. En aquellos en los que el ecocardiograma en reposo o la maniobra de Valsalva no detecta un gradiente intraventricular significativo (> 30 mmHg), deben someterse a ecocardiografía de esfuerzo para detectar la obstrucción del TSVI. Los pacientes con síntomas limitantes y obstrucción grave del TSVI, refractarios al uso de betabloqueantes y verapamilo, deben recibir terapias de reducción septal o usar nuevos medicamentos inhibidores de la miosina cardíaca. Finalmente, los pacientes adecuadamente identificados con un riesgo aumentado de muerte súbita pueden recibir medidas profilácticas con el implante de un cardioversor-desfibrilador implantable (CDI).
Hypertrophic cardiomyopathy (HCM) is a form of genetically caused heart muscle disease, characterized by the thickening of the ventricular walls. Diagnosis requires detection through imaging methods (Echocardiogram or Cardiac Magnetic Resonance) showing any segment of the left ventricular wall with a thickness > 15 mm, without any other probable cause. Genetic analysis allows the identification of mutations in genes encoding different structures of the sarcomere responsible for the development of HCM in about 60% of cases, enabling screening of family members and genetic counseling, as an important part of patient and family management. Several concepts about HCM have recently been reviewed, including its prevalence of 1 in 250 individuals, hence not a rare but rather underdiagnosed disease. The vast majority of patients are asymptomatic. In symptomatic cases, obstruction of the left ventricular outflow tract (LVOT) is the primary disorder responsible for symptoms, and its presence should be investigated in all cases. In those where resting echocardiogram or Valsalva maneuver does not detect significant intraventricular gradient (> 30 mmHg), they should undergo stress echocardiography to detect LVOT obstruction. Patients with limiting symptoms and severe LVOT obstruction, refractory to beta-blockers and verapamil, should receive septal reduction therapies or use new drugs inhibiting cardiac myosin. Finally, appropriately identified patients at increased risk of sudden death may receive prophylactic measure with implantable cardioverter-defibrillator (ICD) implantation.

ABSTRACT: Pythiosis is the disease caused by aquatic oomycetes of the genus Pythium. In dogs the typical course of the disease involves the gastrointestinal tract, characterized by transmural thickening of the stomach or intestines. However, infection by Pythium spp. has only rarely been recognized as a cause of esophagitis in dogs. Thus, the present reports a case of esophageal and gastric pythiosis in an eight-month-old female pit bull dog. The dog was attended at the hospital after two months presenting regurgitation and dyspnea. It was reported that the dog lived in an urban area and had no previous sanitary issues. At clinical examination it was noted that the dog presented crackling sounds at pulmonary auscultation. A support therapy accompanied by antibiotics has been employed; however, 22 days after hospitalization the clinical condition worsened, and the dog died. At necropsy, the wall of the distal segment of the esophagus and the cardia and part of the fundus of the stomach were expanded by a focal extensive irregular intramural annular mass. Additionally, there was a transmural esophageal fistula. At histology, the walls of the esophagus and stomach were extensively expanded by multifocal extensive areas of necrosis, associated with a pyogranulomatous infiltrate and abundant granulation tissue containing multiple negative images of hyphae that were highlighted by silver impregnation (Grocott). Furthermore, immunohistochemistry and PCR for P. insidiosum were both positive in samples of paraffin-embedded esophageal tissue.
RESUMO: A Pitiose é uma doença causada por oomicetos aquáticos do gênero Pythium. Em cães o curso típico da doença envolve o trato gastrointestinal, caracterizado por espessamento transmural do estômago ou intestinos. No entanto, a infecção por Pythium spp. raramente foi reconhecida como causa de esofagite em caninos. Assim, o presente trabalho tem como objetivo relatar um caso de pitiose esofágica e gástrica em um canino Pit Bull fêmea de oito meses de idade. O cão foi recebido no hospital com histórico de regurgitação e dispneia durante dois meses. Foi relatado que o animal era de um domicílio de área urbana e não possuía histórico pregresso de enfermidades. No exame clínico contatou-se ainda crepitações à ausculta pulmonar. Foi instituído tratamento de suporte e antibioticoterapia, porém após 22 dias de internação, o cão evoluiu para o óbito. Na necropsia, as paredes do segmento distal do esôfago e o cárdia e parte do fundo do estômago estavam expandidos por uma massa anular intramural focalmente extensiva e irregular. Além disso, havia uma fístula esofágica transmural. À histologia, as paredes do esôfago e do estômago apresentavam-se difusamente expandidas por áreas multifocais extensas de necrose, associadas a infiltrado piogranulomatoso, abundante proliferação de fibroblastos imaturos, neovascularização e contendo múltiplas imagens negativas de hifas realçadas pela impregnação por prata (Grocott). Além disso, tanto a imuno-histoquímica como PCR para P. insidiosum foram positivas em amostras de esôfago embebidas em parafina.

Fantastic Lifestyle Questionnaire applied to undergraduate medical students during the COVID-19 pandemic: a factor analysis

SUMMARY OBJECTIVE: To identify what structure represents life style in medical students, in a public university, with the application of the Fantastic Lifestyle Questionnaire. METHODS: This is a cross-sectional exploratory study. The Fantastic Lifestyle Questionnaire was applied online to medical students. Factorial analysis was realized and factors were extracted by principal components method. Rotation was conducted by Varimax method. RESULTS: Sixty-one questionnaires were analyzed. Students were male in 45.9%, single in 88.5%. The model was composed of three factors that explained 63.5% of all observed variance. The factors related to sleep perception, nutrition and stress, followed by relationships and entertainment and then, activity. Cronbach's alpha was 0.81, which was considered good. CONCLUSION: The model composed by the three factors observed in this group of students represented the construct quality of life, evaluated by the Fantastic Lifestyle Questionnaire. This result may provide substrate to actions that aim to improve quality of life and well-being in medical students from this university.

Experimental resin-based dual-cured calcium aluminate and calcium titanate materials for vital pulp therapy

Abstract: This paper evaluates the physicochemical and biological properties of experimental resin-based dual-cured calcium aluminate (CA) and calcium titanate (CTi) materials for vital pulp therapy (VPT). The experimental dual-cured materials were obtained as two pastes: a) Bis-EMA 10, PEG 400, DHEPT, EDAB, camphorquinone, and butylated hydroxytoluene; and b) fluoride ytterbium, Bis-EMA 10, Bis-EMA 30, benzoyl peroxide, and butylated hydroxytoluene. The materials were divided into six groups based on the added calcium component: MTA (MTA®, Angelus); CLQ (Clinker-Fillapex®, Angelus); CA (calcined at ,1200°C in pastes a and b); CA800 (calcined at 800°C in paste a); CA1200 (calcined at 1,200°C in paste a); and CTi (paste a). The real-time degree of conversion and rate of polymerization (n = 3), diametral tensile strength (n = 10), hydrogen potential (n = 15), calcium ion release (n = 10), water sorption and solubility (n = 10), and cell viability (n = 6) were evaluated. One- and two-way ANOVA and Tukey’s post hoc test were used in the analysis of the parametric data, and Kruskal-Wallis and Dunn’s multiple tests were used to analyze the nonparametric data (α = 0.05). CLQ, CA800 and CA1200 had the highest diametral tensile strength. The water solubility of MTA was similar to that of CA800, CA1200 and CTi. CA800 and CA1200 resulted in cell viabilities similar to those of MTA and CLQ. The experimental dual-cured CA-based material that calcined at 800°C showed physicochemical and biological properties suitable for VPT, and similar to those of MTA.

Abstract The objective of this study was to formulate an experimental light-cured periodontal dressing containing alpha-humulene and to compare its physical, antimicrobial, and cytotoxicity properties with commercial gold standards (Barricaid® and Periobond®). Two periodontal dressing formulations were developed (a and b). The formulations were divided into 5 groups according to the alpha-humulene concentration as follows: Ea - control group, Ea1 - 1%, Ea5 - 5%, Ea10 - 10%, and Ea20 - 20%; Eb - control group, Eb1 - 1%, Eb5 - 5%, Eb10 - 10%, and Eb20 - 20%. Materials characterization was performed using the degree of conversion, cohesive strength, sorption, and solubility assays. Antimicrobial assay was performed using the modified direct contact test against E. faecalis and S. aureus. Cytotoxicity was assessed by the cell viability experiment using L929 fibroblasts. In general, the cohesive strength values of materials decreased as the alpha-humulene concentration increased. All the experimental dressings showed antimicrobial activity against both bacteria tested. Cell viability results for the Ea, Ea1, Eb, and Eb1 groups showed moderate cytotoxic effect. The formulations containing alpha-humulene showed similar behavior to the commercial references. Thus, formulations containing alpha-humulene have potential to be used as periodontal dressing.

Resumo: Este estudo buscou identificar a composição da ictiofauna do rio Muzambinho, um tributário de terras altas do Rio Paraná. Também buscamos investigar se as cachoeiras da região podem servir de barreiras para dispersão de espécies de peixes. Para isso, foram realizadas coletas em 34 pontos do Rio Muzambinho, com diferentes técnicas. Ao todo foram registradas 37 espécies de peixes, sendo algumas endêmicas e a maioria nativa. Dentre essas espécies algumas são predominantes em locais degradados e podem ser usadas como bioindicadores. Os resultados também demonstram que há uma zonação na composição ictiofaunística no Muzambinho que segrega a fauna em três sessões separadas por cachoeiras, Baixo Muzambinho, Alto Muzambinho e São Domingos. Concluímos que para a ideal preservação da ictiofauna do rio Muzambinho é necessária a preservação independente de suas sessões o que garantiria a manutenção de linhagens naturalmente isoladas.
Abstract: This study sought to identify the ichthyofauna composition of the Muzambinho River, an upland tributary of the Paraná River. We also investigate whether waterfalls in the region can serve as barriers to the dispersal of fish species. For this purpose, collections were carried out at 34 points on the Muzambinho River using different techniques. In all, 37 species were recorded, some of which were endemic, and the majority were native. Among these species, some are predominant in degraded places and can be used as bioindicators. The results also demonstrate zonation in the ichthyofauna composition in Muzambinho that segregates the fauna into three sessions separated by waterfalls: Lower Muzambinho, Upper Muzambinho, and Sao Domingos. We conclude that, for the ideal preservation of the ichthyofauna of the Muzambinho River, it is necessary to preserve its sections independently, which would guarantee the maintenance of naturally isolated strains.

ABSTRACT Patients with Chagas disease have reduced health-related quality of life (HRQoL). Hence, we aimed to identify the factors that mostly affected their HRQoL. This was a systematic review of qualitative studies. The Latin American and Caribbean Health Sciences Literature, Medical Literature Analysis and Retrieval System Online, Excerpta Medica Database, Web of Science, and SciVerse Scopus databases were searched for relevant studies without language or date restrictions. The search and data analysis were performed by independent reviewers; all qualitative studies that reported the factors that had an impact on the HRQoL of patients with Chagas disease were included. The risk of bias was assessed using the Critical Appraisal Skills Program Qualitative Study Checklist; confidence in the evidence was evaluated using the Grading of Recommendations Assessment, Development and Evaluation-Confidence in the Evidence from Reviews of Qualitative approach. Five studies were included in this review: four in Brazil and one in California, United States, with immigrants from Central and South America. The sample consisted of 207 patients with chronic Chagas disease. Stigma, physical limitations, work absenteeism, emotional or mental aspects, fear of treatment, and fear of the future had the strongest impact on the HRQoL. All items showed moderate confidence except for fear of treatment (low confidence). The physical, emotional, mental, and cultural aspects affected the HRQoL of patients with chronic Chagas disease. Identification of these factors is important in the development of strategies aimed at improving the HRQoL of this population.

VERTEBROPLASTY IN BONE FRAGILITY FRACTURES AND TUMOR FRACTURES: RISKS AND BENEFITS

RESUMO Objetivo: Avaliar os resultados da vertebroplastia percutânea (VP) em fraturas por fragilidade da coluna (osteoporose/tumoral), analisando possíveis complicações. Método: Foram avaliados 33 pacientes com fratura da coluna vertebral (FXV) por osteoporose ou tumor, entre janeiro e novembro de 2021, submetidos à VP. Foi realizado exame físico junto à obtenção da história e fatores de risco para fragilidade óssea / tumor, além de avaliação radiológica da coluna para constatação de FXV. O método semiquantitativo de Genant foi empregado para a classificação no pós-operatório, além do score EVA e do questionário de incapacidade (ODI). O controle tomográfico foi avaliado por médico radiologista para quantificação do preenchimento vertebral e extravasamento, determinando para onde ocorreram. Resultados: Foram operadas 46 vértebras de 33 pacientes, como média de idade de 71 anos, sendo 11 pacientes com mais de um nível operado. Do total, 13 pacientes apresentavam fraturas tumorais e 20 possuíam fraturas por insuficiência. Observou-se extravasamento do PMMA em 31 vértebras, mais frequentemente para Plexo Venoso Vertebral Externo (23), Corpo Discal (9), Recesso Epidural Anterior (4) Vasos Pulmonares (4), Plexo Venoso Vertebral Interno (3), Cava Inferior (2), Plano Adiposo (2) e Veia Ázigos (1). Nenhum paciente apresentou complicações clínicas. Ainda, o EVA pré-operatório médio foi 8 e o pós-operatório de 3, enquanto o ODI pré-operatório médio foi de 56 e o pós-operatório de 30. Conclusão: O extravasamento de PMMA foi frequente em diversos locais e níveis, sem nenhuma complicação clínica. A VP mostrou-se eficaz na melhora de dor e função. ABSTRACT Objective: To evaluate the results of percutaneous vertebroplasty (PV) in spinal fragility fractures (osteoporosis/tumor), analyzing possible complications. Method: We evaluated 33 patients with spinal fractures (FXV) due to osteoporosis or tumor who underwent PV between January and November 2021. A physical examination was performed, obtaining the history and risk factors for bone fragility/tumor and a radiological evaluation of the spine to verify FXV. Genant’s semiquantitative method was used for postoperative classification, the VAS score, and a disability questionnaire (ODI). A radiologist evaluated tomographic control to quantify vertebral filling and extravasation, determining where they occurred. Results: 46 vertebrae of 33 patients were operated on, with a mean age of 71 years, and 11 patients with more than one level of surgery. Of the total, 13 patients had tumor fractures, and 20 had fractures due to insufficiency. PMMA extravasation was observed in 31 vertebrae, most frequently in the External Vertebral Venous Plexus (23), Discal Body (9), Anterior Epidural Recess (4), Pulmonary Vessels (4), Internal Vertebral Venous Plexus (3), Inferior Cava (2), Adipose Plane (2) and Azygos Vein (1). No patient had clinical complications. Furthermore, the mean preoperative VAS was eight, the postoperative one was 3, the mean preoperative ODI was 56, and the postoperative one was 30. Conclusion: PMMA extravasation was frequent in several locations and levels without any clinical complications. VP proved to be effective in improving pain and function. RESUMEN Objetivo: Evaluar los resultados de la vertebroplastia percutánea (PV) en fracturas por fragilidad de columna (osteoporosis/tumor), analizando posibles complicaciones. Método: Se evaluaron 33 pacientes con fractura de columna (FXV) por osteoporosis o tumor, entre enero y noviembre de 2021, que fueron sometidos a PV. Se realizó examen físico junto con obtención de antecedentes y factores de riesgo de fragilidad ósea/tumor, además de evaluación radiológica de columna para verificar FXV. Para la clasificación postoperatoria se utilizó el método semicuantitativo de Genant, además de utilizar la escala EVA y un cuestionario de discapacidad (ODI). El control tomográfico fue evaluado por un radiólogo para cuantificar el llenado vertebral y la extravasación, determinando dónde se producían. Resultados: Se operaron 46 vértebras de 33 pacientes, con una edad promedio de 71 años, 11 pacientes con más de un nivel de cirugía. Del total, 13 pacientes presentaron fracturas tumorales y 20 fracturas por insuficiencia. Se observó extravasación de PMMA en 31 vértebras, con mayor frecuencia en el Plexo Venoso Vertebral Externo (23), Cuerpo Discal (9), Receso Epidural Anterior (4), Vasos Pulmonares (4), Plexo Venoso Vertebral Interno (3), Cava Inferior (2), Plano Adiposo (2) y Vena Azygos (1). Ningún paciente presentó complicaciones clínicas. Además, la EVA preoperatoria media fue de 8 y la postoperatoria de 3, mientras que la ODI preoperatoria media fue de 56 y la postoperatoria de 30. Conclusión: La extravasación de PMMA fue frecuente en varias localizaciones y niveles, sin complicaciones clínicas. VP demostró ser eficaz para mejorar el dolor y la función. Resumo O objetivo deste trabalho foi determinar a herança do teor de betacaroteno em melão (Cucumis melo). O acesso AC-16 (Cucumis melo subsp. melo var. acidulus) – com baixo teor de betacaroteno e mesocarpo branco – foi cruzado com a cultivar Vedrantais (C. melo subsp. melo var. cantalupensis) – com alto teor de betacaroteno e mesocarpo de cor salmão –, para obtenção das gerações F1, F2, RC1 e RC2. Avaliaram-se os genitores AC-16 e 'Vedrantais', as gerações F1 e F2, e os retrocruzamentos de cada genitor RC1 e RC2. A quantificação do betacaroteno foi realizada em sistema de cromatografia líquida de alto desempenho. Foram estimados os componentes de média relacionados aos efeitos aditivos e de dominância, as variâncias aditiva e de dominância e a herdabilidade. O teor de betacaroteno foi alto (17,78 µg g-1) em 'Vedrantais' e baixo em AC-16 (0,34 µg g-1). Observaram-se os seguintes resultados: efeito aditivo e de dominância no controle genético do caráter, dominância de caráter incompleta, número estimado de loci próximo de dois, maior variância para populações segregantes (F2 e retrocruzamentos), e valores de herdabilidade nos sentidos amplo (87,75%) e restrito (64,19%). O teor de betacaroteno em melão é controlado por um gene de efeito maior, com efeitos aditivos e de dominância associados a poligenes com efeitos aditivos.
Abstract The objective of this work was to determine the inheritance of beta-carotene content in melon (Cucumis melo). The AC-16 accession (Cucumis melo subsp. melo var. acidulus) – with a low beta-carotene content and white mesocarp – was crossed with the Vedrantais cultivar (C. melo subsp. melo var. cantalupensis) – with a high beta-carotene content and salmon colored mesocarp –, to obtain the F1, F2, BC1, and BC2 generations. The AC-16 and 'Vedrantais' parents, the F1 and F2 generations, and the BC1 and BC2 backcrosses of each parent were evaluated. The quantification of beta-carotene was carried out in a high-performance liquid chromatography system. Mean components related to the additive and dominance effects, additive and dominance variances, and heritability were estimated. The beta-carotene content was high in 'Vedrantais' (17.78 µg g-1) and low in AC-16 (0.34 µg g-1). The following results were observed: additive and dominance effects on the genetic control of the character, incomplete character dominance, estimated number of loci close to two, greater variance for segregating populations (F2 and backcrosses), and heritability values in the broad (87.75%) and narrow (64.19%) senses. The beta-carotene content in melon is controlled by a major effect gene, with additive and dominance effects associated with polygenes with additive effects.

ABSTRACT Studies about seed dispersal and germination are important to understand patterns of plant distribution and abundance, and help establish strategies for environmental conservation. We evaluated the role of two different dispersers, the ant Atta laevigata and the maned wolf Chrysocyon brachyurus, in the germination of Copaifera arenicola seeds, a characteristically myrmecochorous plant species. Germination was evaluated for seeds subjected to four treatments: (1) seeds manipulated by the ant, (2) seeds ingested by the maned wolf, (3) seeds that had the elaiosome removed manually, and (4) seeds with elaiosome (unmanipulated seeds). Seeds manipulated by the ant and seeds that had the elaiosome removed manually required less time to germinate and had higher germination percentages (98.6% and 95.8%, respectively) than the other treatments. However, seeds ingested by the maned wolf also had a high germination percentage (87.7%), above that of unmanipulated seeds (34.7%). Probably, elaiosomes of unmanipulated seeds and small remnants of this structure that resist digestion by maned wolf can decrease to some extent seed germination. We argue that dispersal of C. arenicola seeds by the ant and by the maned wolf result in different patterns of seed distribution in the environment and that they have complementary roles in structuring plant populations.

ABSTRACT Background and aims: Treatment of hepatitis C with direct antiviral agents (DAA) is associated with almost 95% of sustained virological response. However, some patients need retreatment. In Brazil, it should be done according to the Ministry of Health guidelines, frequently updated to include newly available drugs. This study aimed to conduct a national survey about the characteristics and outcomes of retreatment of hepatitis C in previously non-responders to DAAs. Patients and methods: Institutions from all over the country were invited to participate in a national registry for retreatment, including information about clinical and epidemiological characteristics of the patients, type and outcomes of retreatment regimens. Only patients previously treated with interferon-free regimens were included. Results: As previous treatments the distribution was: SOF/DCV (56%), SOF/SIM (22%), 3D (11%), SOF/LED (6%) and SOF/RBV (5%). For retreatment the most frequently used drugs were SOF/GP (46%), SOF/DCV (23%) and SOF/VEL (11%). From 159 patients retreated, 132/159 (83%) had complete information in the registry and among them only seven patients were non-responders (SVR of 94.6%). All retreatments were well tolerated, without any serious adverse events or interruptions. Conclusion: The retreatment of patients previously non-responders to DAAs was associated with high rate of SVR in this sample of Brazilian patients. This finding allows us to conclude that the retreatment options available in the public health system in Brazil are effective and safe and are an important component of the strategy of elimination of hepatitis C in our country.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.

We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.
