ABSTRACT Objective To calculate the positive likelihood ratio to determine whether telemedicine is able to optimize referral to the emergency department. Methods Unicenter study with 182 consecutive patients admitted to Hospital Israelita Albert Einstein due to respiratory symptoms. All patients were submitted to oxygen saturation measurement using the standard method Welch Allyn finger device vital sign monitor and a 2-minute evaluation (Binah.ai mobile application). The reproducibility of oxygen saturation measurements made with both methods was investigated using interclass correlation coefficients and analysis of dispersion. Bland-Altman plots were constructed and kappa concordance coefficients used to examine data normality. Accuracy was also estimated. Results Oxygen saturation measurement differences between methods were ≤2% in more than 85% of cases. The mean difference (bias) between methods was near zero (0.835; Bland-Altman analysis). Oxygen saturation measurements made using the Binah.ai mobile application had an average ability to detect patients with altered oxygen saturation levels compared to the conventional method (ROC analysis). The positive likelihood ratio of the mobile application was 6.23. Conclusion Mobile applications for oxygen saturation measurement are accessible user-friendly tools with moderate impact on clinical telemedicine evaluation of patients with respiratory symptoms, and may optimize referral to the emergency department.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

This work investigated the effect of adding ceria-stabilized tetragonal zirconia (Ce-TZP) on the fatigue behavior of alumina-based ceramic composites. Alumina powder (control group) and mixtures containing 5 wt.% (group A) and 20 wt.% (group B) of a commercial m-ZrO2/Al2O3/CeO2 powder mixture were milled/homogenized, compacted, sintered at 1600°C-2h, and submitted to hydrothermal degradation. The samples were characterized by relative density, microstructure, crystalline phases, and static mechanical properties. The cyclic fatigue strength was determined using the modified staircase method in 4-point bending tests. The results indicate that adding the m-ZrO2/Al2O3/CeO2 powder mixture to the Al2O3-matrix increases the tetragonal-ZrO2 grains (Ce-TZP) content, presenting 2.9 wt.% of Ce-TZP and 11.9 wt.% of Ce-TZP for group A and group B, respectively. Furthermore, the addition of Ce-TZP improves densification (98.5% → 99.1%) with a slight reduction in hardness and modulus of elasticity and a significant KIC increase of the composite (KIC = 6.7 MPa.m1/2, group B) when compared to monolithic alumina (KIC=2.4 MPa.m1/2). The fatigue strength limit of the control group was around 100 MPa, while the composites (groups A and B) presented the values of 279 MPa and 239 MPa, respectively. The results indicated that the incorporation of Ce-TZP significantly improves the fracture toughness of alumina-based ceramics. On the other hand, regarding the fatigue behavior, there was an increase in fatigue resistance in group A, resulting from the benefits of the t→m Ce-TZP grains transformation, which occurs during cyclic loading, producing a zone shielding that involves the tip of the crack, slowing its growth. The increase in the amount of Ce-TZP (group B) leads to an increase in the internal residual stresses between the phases due to anisotropy and difference in the thermal expansion coefficients, which accelerates the phase transformation and formation of microcracks at grain boundaries, reducing the fatigue strength of composites of group B.

Resumo A identificação e seleção de talentos no futebol são feitas majoritariamente pela avaliação subjetiva dos treinadores. Sistematizar o “olho do treinador” pode otimizar este processo. O objetivo do presente estudo foi comparar as habilidades táticas, a competência percebida e a orientação motivacional de jovens futebolistas classificados como atletas de alto e baixo potencial esportivo, de acordo com a opinião dos treinadores. Participaram 101 futebolistas brasileiros sub15 (com média de 14,6±0,7 anos; e média de tempo de prática de 7,6±2,6 anos). Foram utilizados diferentes questionários para avaliar o conhecimento tático (TACSIS - Tactical Skills Inventory for Sports), a orientação esportiva (SOQ - Sport Orientation Questionnaire) e a competência percebida. Os futebolistas de alto potencial (n=44), quando comparados aos futebolistas de baixo potencial (n=57), apresentaram maior posicionamento e decisão (4,52±0,73 vs. 4,18±0,63; p=0,02; d=0,50), maior conhecimento sobre os outros (4,25±0,96 vs. 3,81±0,83; p=0,02; d=0,49) e um valor maior no somatório de habilidades táticas (4,43±0,72 vs. 4,14±0,65; p=0,04; d=0,46); os atletas de alto potencial apresentaram maior competitividade (4,76±0,27 vs. 4,55±0,33; p=0,01; d=0,72) e maior competência percebida (7,79±1,32 vs. 7,12±1,13; p=0,01; d=0,54), respectivamente. Conclui-se que os futebolistas sub15 avaliados pelos treinadores como alto potencial esportivo apresentaram resultados melhores em indicadores de habilidades táticas, motivação e competência percebida, quando comparados aos de baixo potencial.
Abstract Coaches’ subjective evaluations are predominantly responsible for talent identification and selection in soccer; therefore, systematizing the "coach's eye" can optimize this process. Our objective was to compare tactical skills, perceived competence, and sport achievement orientation of young soccer players classified as having high or low sporting potential, according to their coaches’ opinion. 101 Brazilian U15 soccer players (14.6 ± 0.7 years; practice time: 7.6 ± 2.6 years) participated in the study. Questionnaires were used to assess their tactical knowledge (TACSIS – Tactical Skills Inventory for Sports), sport orientation (SOQ - Sport Orientation Questionnaire), and perceived competence. Compared with the low-potential soccer players (n = 57), the high-potential soccer players (n = 44) showed higher positioning and deciding skills (4.52 ± 0.73 vs. 4.18 ± 0.63; p = 0.02; d = 0.50), knowledge about others (4.25 ± 0.96 vs. 3.81 ± 0.83; p = 0.02; d = 0.49), and sum of tactical skills (4.43 ± 0.72 vs. 4.14 ± 0.65; p = 0.04; d = 0.46) In turn, high-potential athletes showed higher competitiveness (4.76 ± 0.27 vs. 4.55 ± 0.33; p = 0.01; d = 0.72) and greater perceived competence (7.79 ± 1.32 vs. 7.12 ± 1.13; p = 0.01; d = 0.54), respectively. We concluded that the U15 soccer players evaluated by their coaches as having high sporting potential presented better results in indicators of tactical skills, motivation, and perceived competence than their low-potential peers.

Abstract Background: The VI Brazilian Consensus on Autoantibodies against HEp-2 cells for determination of autoantibodies against cellular constituents on HEp-2 cells was held on September, 2019, in Fortaleza (CE, Brazil). The guidelines in this edition were formulated by the group of Brazilian experts discussing the classification of complex patterns, the classification of the nuclear discrete dots (few and multiple), the identification of the discrete fine speckled pattern (AC-4a) and improvements on the ANA report. Mainbody: Sixteen Brazilian researchers and experts from universities and clinical laboratories representing the various geographical regions of Brazil participated in the meeting. Four main topics were discussed: (1) How to classify patterns with fluorescence in more than one cell compartment considering three relevant categoris: composite patterns, mixed patterns and multiple patterns; (2) The splitting of the discrete nuclear dots pattern into the multiple discrete nuclear dots (AC-6) and few discrete nuclear dots (AC-7) patterns, respectively; (3) Inclusion of a novel nuclear pattern characterized by discrete fine speckled pattern highly associated with antibodies to SS-A/Ro60, classified as AC-4a. In addition, adjustments on the Brazilian Consensus nomenclature were implemented aiming to harmonize the designation of some patterns with the International Consensus on ANA Patterns (ICAP). Furthermore, the designations of the PCNA-like pattern (AC-13), CENP-F-like pattern (AC-14) and Topo I-like pattern (AC-29) were adjusted in accordance to ICAP. Finally, there was a recommendation for adjustment in the test report in order to address the status of nuclear envelope staining. For all topics, the aim was to establish specific guidelines for laboratories and clinicians. All recommendations were based on consensus among participants. All recommendations from the V Consensus were maintained and there was relevant progress in the BCA/HEp-2 guidelines and further harmonization with ICAP. Conclusion: The VI BCA/HEp-2 edition was successful in establishing important recommendations regarding the classification of complex patterns, in supporting the identification of a novel pattern within the AC-4 group and in the harmonization process with the ICAP terminology.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Abstract The multicriteria decision-making process is still an open problem, especially when the decision criteria are not numerical or fully outlined. Several numerical, intelligent, or hybrid techniques have been developed, creating contributions to this problem's solution. This paper is another step in this direction. Based on the Modified Analytic Hierarchy Process (MAHP), a methodology for diagnosis and performance analysis is presented for the dispatch ranking of generating units in a thermoelectric plant. The problem is complex because it covers a power plant, where 99 generating units may be dispatched, according to 10 possible evaluation criteria, which should be used together. This article also presents details of the implementation of the sensors necessary to add to the supervisory system existing in the Palmeiras de Goias Thermal Power Plant.

ABSTRACT: Vitamin A deficiency is the third most important micronutrient deficiency having a significant impact on public health worldwide. The aim of this study was to develop a carotenoid extraction process with edible vegetable oils, using a central composite design which, concurrently, enriches the oils with provitamin A carotenoids. The stability of carotenoids in the oil and their shelf life was also evaluated. The process also aimed to meet the Recommended Daily Intake for vitamin A (600 μg retinol equivalent for an adult) with the enriched oil. The results showed that 20 % of the vegetable matrix (dried carrot) in vegetable oil and 5 min of extraction time was an optimum combination for both soy oil (64.43 μg retinol equivalent mL−1) and olive oil (62.98 μg retinol equivalent mL−1). Furthermore, the ingestion of just a spoonful (∼10 mL) of the enriched oils would meet the Recommended Daily Intake for vitamin A (600 μg retinol equivalent), and, thus, it was possible to affirm that a dose of vegetable oils enriched with provitamin A contributes to meeting the Recommended Daily Intake for vitamin A. The proposed process is based on simple and low-cost technology that can be used to enrich edible oils (soybean and olive oil) with provitamin A.
