ABSTRACT: This research evaluated different rice genotypes regarding physicochemical and instrumental parameters of grain quality and associated the data with sensory analysis to support the creation of rules for classification of the culinary quality of rice (texture), based on isolated or combined parameters. The combination of amylose content and gelatinization temperature was able to predict the rice quality. According to the sensorial panel, the instrumental stickiness was able to segregate rice with very low amylose content or waxy to the other ones. Regarding pasting properties, rice that presented high final viscosity (310-480 RVU), setback (165-245 RVU), and pasting temperature (78 - 88 °C), and low values for breakdown (15-120 RVU), associated with a high stickiness (>-5N) was desirable by the Brazilian consumers. The classification rules created through the relationship between the physicochemical parameters and the texture profile evaluated by the sensory panel will help to verify the culinary profile of the rice samples (through free software), which makes it easier to predict the probability of rice meeting the desired quality standards.

RESUMO: Este trabalho avaliou diferentes genótipos de arroz quanto a parâmetros físico-químicos e instrumentais de qualidade do grão e associou os dados à análise sensorial para subsidiar a criação de regras de classificação da qualidade culinária do arroz (textura), com base em parâmetros isolados ou combinados. A combinação do teor de amilose e temperatura de gelatinização foi capaz de predizer a qualidade do arroz. De acordo com o painel sensorial, a pegajosidade instrumental foi capaz de segregar arroz com baixo teor de amilose ou ceroso dos demais. Em relação às propriedades de pasta, o arroz que apresentou alta viscosidade final (310-480RVU), retrogradação (165-245RVU) e temperatura de pasta (78-88 °C) e baixos valores de quebra pasta (15-120RVU), associados a uma alta pegajosidade (> -5N) será desejável pelos consumidores brasileiros. As regras de classificação criadas por meio da relação entre os parâmetros físico-químicos e o perfil de textura avaliado pelo painel sensorial ajudarão a verificar o perfil culinário das amostras de arroz (por meio de um software livre), o que torna mais fácil prever a probabilidade de o arroz atender aos padrões de qualidade desejados.

Resumen Este estudio tuvo como objetivo evaluar el efecto de la inoculación con L. buchneri liofilizado y/o activado sobre las poblaciones de bacterias de ácido láctico en ensilajes de maíz. Los tratamientos experimentales consistieron en ensilaje de maíz sin aditivo y ensilaje con los inoculantes L. buchneri 1 × 10 5 ufc/g, aplicados según las recomendaciones del fabricante (1 g/tonelada de forraje) en las formas de inoculante liofilizado e inoculante preactivado. Se identificaron aislados purificados de ensilajes de maíz con y sin inocu lación, y el 93% de los aislados correspondieron a bacterias de ácido láctico de la especie L. plantarum. Entre los aislados no se detectaron bacterias de la especie L. buchneri. La aplicación de L. buchneri liofilizado o activado en la masa ensilada promueve un aumento considerable de la población de bacterias de ácido lácticas en los ensilajes de maíz durante todo el período de fermentación.

Abstract This study aimed to evaluate the effect of inoculation with lyophilised and/or activated Lactobacillus buchneri on lactic acid bacteria populations in corn silage. Experimental treatments consisted of corn silage without additives or silage with the inoculants of L. buchneri (1 x 10 5 cfu/g) applied according to the manufacturer’s recommendations (1 g/tonne fodder) in the forms of the lyophilised inoculant and pre-activated inoculant. Purified isolates from corn silage with and without the inoculant were identified, and 93% of the isolates corresponded to the lactic acid bacteria of the species Lactobacillus plantarum. Among the isolates, no bacteria of the species L. buchneri were detected. The application of lyophilised or activated L. buchneri improved the microbiological profile and reduced ethanol production in corn silage, even without being identified among the isolates captured 70 days after ensilage.

ABSTRACT Introduction: Chronic kidney disease (CKD) has been increasing significantly. There is evidence that a large part of the population does not have enough knowledge on the subject. Objective: To investigate the level of knowledge about CKD in the general population, its risk factors and means of prevention. Methods: We ran a cross-sectional study in the population of Fortaleza, Ceará – Brazil, between 2017 and 2020, with the application of a questionnaire on CKD, risk factors and prevention. Results: we interviewed 735 volunteers, with a mean age of 38 years, of which 55% were female. Only 17.2% correctly responded to the concept of CKD, and 5.8% knew the concept of creatinine. Low water intake was the most cited risk factor by respondents (79.3%). The main risk factors and direct causes of CKD (diabetes and hypertension) were mentioned less frequently (13.2% and 15.1%, respectively). Men were more correct regarding risk factors and ways to prevent CKD. Older respondents answered more correctly the questions about the definition of CKD (n = 22; 28.6%) and creatinine (n = 7; 9.0%). With regards to education there was a statistically significant correlation in all the questions (p < 0.05). Conclusion: There is little knowledge about CKD in the general population. Higher level of education is associated with better knowledge. More health education actions are needed so that the population becomes better acquainted with CKD and, consequently, can adopt more adequate prevention and control measures.

Resumo Introdução: A doença renal crônica (DRC) vem aumentando de forma significativa. Há evidências de que grande parte da população não tem conhecimento suficiente sobre o assunto. Objetivo: Investigar na população geral o nível de conhecimento sobre a DRC, seus fatores de risco e meios de prevenção. Métodos: Foi realizado estudo transversal na população de Fortaleza, Ceará, Brasil, entre 2017 e 2020, com aplicação de um questionário sobre DRC, fatores de risco e prevenção. Resultados: Foram entrevistados 735 voluntários, com média de idade de 38 anos, dos quais 55% eram do sexo feminino. Apenas 17,2% responderam corretamente ao conceito de DRC, e 5,8% sabiam o conceito de creatinina. A baixa ingestão hídrica foi o fator de risco mais citado pelos entrevistados (79,3%). Os principais fatores de risco e as causas diretas de DRC (diabetes e hipertensão) foram mencionados com menor frequência (13,2% e 15,1%, respectivamente). Os homens tiveram maior acerto com relação aos fatores de risco e às formas de prevenção da DRC. Os entrevistados com idade mais avançada responderam mais corretamente às perguntas sobre a definição de DRC (n = 22; 28,6%) e creatinina (n = 7; 9,0%). No que diz respeito à escolaridade, em todas as perguntas houve correlação estatisticamente significativa (p < 0,05). Conclusão: Há um baixo nível de conhecimento sobre a DRC na população geral. Maior nível de escolaridade associa-se a um melhor conhecimento. São necessárias mais ações de educação em saúde para que a população conheça melhor a DRC e consequentemente possa adotar medidas de prevenção e controle mais adequadas.

Resumo: A Estratégia de Prevenção e Atenção à Obesidade Infantil (PROTEJA) visa promover a implementação de um pacote de ações para prevenção da obesidade no Brasil. Este estudo apresenta as etapas de desenho operacional, proposta geral, avaliação e monitoramento do PROTEJA realizadas pela coordenação técnica do Ministério da Saúde. Trata-se de um estudo qualitativo baseado em análise descritiva documental. A formulação da estratégia envolveu análise de dados epidemiológicos de crianças brasileiras, análise de políticas existentes, e uma revisão da literatura científica e das recomendações O PROTEJA foi então formulado e aprovado pelo Ministério da Saúde e 1.320 municípios se comprometeram a implementar 20 ações essenciais e 5 complementares das 41 ações, incluindo algumas estruturais para melhoria dos ambientes. A estratégia é coordenada pelo Ministério da Saúde em parceria com governos subnacionais e universidades. Sua implementação conta com apoio de uma equipe em nível local, e prevê a avaliação da implementação e de impacto. As ações serão monitoradas anualmente, e os indicadores também implicarão nos incentivos financeiros. O PROTEJA é uma estratégia forte, baseada em evidências e inovadora que visa promover ambientes saudáveis nas cidades, mas sua implementação é desafiadora. Não obstante, apresenta um possível caminho para a reversão da obesidade infantil. Também pode agregar à ciência de implementação e contribuir para o desenvolvimento e aprimoramento de políticas públicas de prevenção da obesidade.

Resumen: La Estrategia Brasileña para la Prevención y Atención de la Obesidad Infantil (PROTEJA) tiene como objetivo promover la implementación de un conjunto de acciones para prevenir la obesidad en Brasil. Este estudio presenta las etapas del diseño operativo, la propuesta general, la evaluación y el seguimiento de PROTEJA realizadas por la coordinación técnica del Ministerio de Salud brasileño. Se trata de un estudio cualitativo basado en el análisis descriptivo de documentos. La formulación de la estrategia implicó el análisis de datos epidemiológicos de niños brasileños, el análisis de las políticas existentes y una revisión de la literatura científica y de las recomendaciones. PROTEJA fue entonces formulado y aprobado por el Ministerio de Salud, y 1.320 municipios se comprometieron a implementar 20 acciones esenciales y 5 complementarias de las 41 acciones, incluidas algunas estructurales para mejorar los entornos. La estrategia es coordinada por el Ministerio de Salud en colaboración con gobiernos subnacionales y universidades. Su implementación cuenta con el apoyo de un equipo a nivel local, y prevé una evaluación de la implementación y de impacto. Las acciones se monitorearán anualmente, y los indicadores también implicarán incentivos económicos. PROTEJA es una estrategia sólida, basada en evidencias e innovadora que busca promover entornos saludables en las ciudades, pero su implementación es un desafío. Sin embargo, presenta una posible vía para revertir la obesidad infantil. También puede agregar a la ciencia de implementación y contribuir al desarrollo y a la mejora de políticas públicas para prevenir la obesidad.

Abstract: The Brazilian Strategy for the Prevention and Care of Childhood Obesity (PROTEJA) aims to implement a set of actions to prevent obesity in Brazil. As such, this qualitative and descriptive documentary study presents the Strategy’s stages of the operational design, general proposal, evaluation and monitoring conducted by the Brazilian Ministry of Health’s technical coordination. After analyzing the epidemiological data on children and the existing policies aimed at obesity prevention, and reviewing the scientific literature and recommendations, PROTEJA was formulated and approved by the Brazilian Ministry of Health, and 1,320 municipalities committed to implementing 20 essential and 5 complementary actions, from 41, including some structural to improve environments. Coordinated by the Brazilian Ministry of Health in partnership with subnational governments and universities, the Strategy also relies on a local team for implementation support, as well as implementation and impact evaluations. Actions will be monitored annually, and the indicators will impact financial incentives. As a strong, evidence-based and innovative strategy aiming to promote healthy environments in cities, PROTEJA has the potential to open a path to childhood obesity reversal, as well as add to the implementation science and contribute to the development and improvement of public policies for obesity prevention; however, its implementation remains a challenge.

RESUMO OBJETIVOS Avaliar a qualidade dos dados antropométricos de crianças registradas no Sistema de Vigilância Alimentar e Nutricional (Sisvan) no período 2008-2017. MÉTODOS Estudo descritivo sobre a qualidade dos dados antropométricos de crianças menores de 5 anos atendidas nos serviços de atenção primária do Sistema Único de Saúde, a partir das bases de dados individuais do Sisvan. A qualidade dos dados foi avaliada anualmente por meio dos indicadores: cobertura, completude, razão entre sexos, distribuição da idade, preferência por dígitos de peso e estatura, valores de escore-z implausíveis, desvio-padrão e normalidade dos escores-z. RESULTADOS No total, 73.745.023 registros e 29.852.480 crianças foram identificados. A cobertura aumentou de 17,7% em 2008 para 45,4% em 2017. A completude da data de nascimento, peso e estatura correspondeu a quase 100% para todos os anos. A razão entre sexos foi equilibrada e aproximadamente similar a razão esperada, variando entre 0,8 e 1. A distribuição da idade revelou maiores percentuais de registros entre as idades de 2 a 4 anos até meados de 2015. Uma preferência pelos dígitos terminais “zero” e “cinco” foi identificada entre os registros de peso e estatura. As porcentagens de escores-z implausíveis excederam 1% para todos os índices antropométricos, com redução dos valores a partir de 2014. Uma alta dispersão dos escores-z, incluindo desvios-padrão entre 1,2 e 1,6, foi identificada principalmente nos índices incluindo estatura e nos registros de crianças menores de 2 anos e residentes das regiões Norte, Nordeste e Centro-Oeste. A distribuição dos escores-z foi simétrica para todos os índices e platicúrtica para estatura/idade e peso/idade. CONCLUSÕES A qualidade dos dados antropométricos do Sisvan para crianças menores de 5 anos melhorou substancialmente entre 2008 e 2017. Alguns indicadores requerem atenção, sobretudo para medidas de estatura, cuja qualidade foi principalmente inferior entre os grupos mais vulneráveis a agravos nutricionais.

ABSTRACT OBJECTIVE To evaluate the quality of anthropometric data of children recorded in the Food and Nutrition Surveillance System (SISVAN) from 2008 to 2017. METHOD Descriptive study on the quality of anthropometric data of children under five years of age admitted in primary care services of the Unified Health System, from the individual databases of SISVAN. Data quality was annually assessed using the indicators: coverage, completeness, sex ratio, age distribution, weight and height digit preference, implausible z-score values, standard deviation, and normality of z-scores. RESULTS In total, 73,745,023 records and 29,852,480 children were identified. Coverage increased from 17.7% in 2008 to 45.4% in 2017. Completeness of birth date, weight, and height corresponded to almost 100% in all years. The sex ratio was balanced and approximately similar to the expected ratio, ranging from 0.8 to 1. The age distribution revealed higher percentages of registrations from the ages of two to four years until mid-2015. A preference for terminal digits “zero” and “five” was identified among weight and height records. The percentages of implausible z-scores exceeded 1% for all anthropometric indices, with values decreasing from 2014 onwards. A high dispersion of z-scores, including standard deviations between 1.2 and 1.6, was identified mainly in the indices including height and in the records of children under two years of age and residents in the North, Northeast, and Midwest regions. The distribution of z-scores was symmetric for all indices and platykurtic for height/age and weight/age. CONCLUSIONS The quality of SISVAN anthropometric data for children under five years of age has improved substantially between 2008 and 2017. Some indicators require attention, particularly for height measurements, whose quality was lower especially among groups more vulnerable to nutritional problems.

Abstract This descriptive ecological study sought to analyze the inductive role of a federal financial incentive passed on to Brazilian municipalities in 2020, during the COVID-19 pandemic, in increasing the number of individual consultations for the condition of obesity in primary health care (PHC). Secondary data obtained from the Health Information System for Primary Care and from the Food and Nutrition Surveillance System (SISVAN) were used. In 2021, 74.8% of the 5,504 municipalities that received the financial incentive, showed an increase in the number of individual obesity consultations in PHC, compared to 2020. The number of people identified with obesity and the number of individual visits for this condition were higher in 2021 than in the other years analyzed (2017 to 2020). Comparing 2021 with 2020 (year of receipt of the financial incentive), it was observed that the percentage of increase in the number of individual consultations for the condition of obesity was higher than the increase in the number of people identified with obesity in the SISVAN (77.6 % vs. 39.1%). In conclusion, federal financial incentives for municipalities, channeled to enhance care for people with obesity within the scope of PHC, are important mechanisms for inducing actions at the local level.

Resumo O presente estudo ecológico descritivo objetivou analisar o papel indutor de um incentivo financeiro federal repassado aos municípios brasileiros em 2020, em meio à pandemia de COVID-19, no aumento do número de atendimentos individuais para a condição de obesidade na atenção primária à saúde (APS). Utilizaram-se dados secundários, obtidos no Sistema de Informação em Saúde para a Atenção Básica e no Sistema de Vigilância Alimentar e Nutricional (SISVAN). Em 2021, 74,8% dos 5.504 municípios que receberam o incentivo financeiro apresentaram aumento no número de atendimentos individuais para a condição avaliada como obesidade na APS, em relação a 2020. O número de pessoas identificadas com obesidade e o número de atendimentos individuais para esta condição foram maiores em 2021 do que nos demais anos analisados (2017 a 2020). Na comparação de 2021 com 2020 (ano de recebimento do incentivo financeiro), observou-se que o percentual de aumento de atendimentos individuais foi superior ao percentual de aumento de pessoas com obesidade registradas no SISVAN (77,6% vs. 39,1%). Em conclusão, incentivos financeiros federais, canalizados para o fortalecimento do cuidado às pessoas com obesidade no âmbito da APS, figuram como importantes mecanismos de indução de ações em nível local.

ABSTRACT Objective: To estimate the prevalence of Sexually Transmitted Infections (STIs) in immigrants and refugees living in the metropolitan region of Goiânia, Goiás. Method: This is a cross-sectional and analytical study. Data collection was carried out from July 2019 to January 2020 and 308 immigrants and refugees were included in the sample. All were underwent face-to-face interviews and were tested for HIV, Syphilis, and Hepatitis B, using rapid tests. Results: The general prevalence for any of the STIs investigated was 8.8% (95%CI 6.0% – 12.3%), being 5.8% (95%CI 3.6% – 8.9%) for Hepatitis B, 2.3% for Syphilis (95%CI 1.00% – 4.4%) and 0.7% for HIV (95%CI 0.1% – 2.1%). Multiple analysis, using logistic regression, showed that the variables male gender (OR = 2.7) and length of time living in Brazil (OR = 2.6) were significantly associated with STIs (p < 0.05). Conclusion: The results of this study suggest that STIs are a health problem in immigrants/refugees, which appear to be enhanced with the length of migration in the country. Public policies that guarantee health care for this population shall be considered.

RESUMEN Objetivo: Estimar la prevalencia de Enfermedades de Transmisión Sexual (ETS) en inmigrantes y refugiados residentes en la región metropolitana de Goiânia, Goiás. Método: Se trata de un estudio transversal y analítico. La recolección de datos se llevó a cabo desde julio de 2019 hasta enero de 2020 y se incluyeron en la muestra 308 inmigrantes y refugiados. Todos fueron entrevistados cara a cara y sometidos a pruebas de VIH, Sífilis y Hepatitis B, mediante pruebas rápidas.. Resultados: La prevalencia general para cualquiera de las ETS investigadas fue de 8,8% (IC95% 6,0% – 12,3%), siendo 5,8% (IC95% 3,6% – 8,9%) para Hepatitis B, 2,3% para Sífilis (IC95% 1,00% – 4,4%) y 0,7% para VIH (IC95% 0,1% – 2,1%). El análisis múltiple, mediante regresión logística, mostró que las variables género masculino (OR = 2,7) y tiempo de residencia en Brasil (OR = 2,6) se asociaron significativamente con las ETS (p < 0,05). Conclusión: Los resultados de este estudio sugieren que las ETS son un problema de salud en inmigrantes/refugiados, que parecen exacerbarse con la duración de la migración en el país. Se deben considerar políticas públicas que garanticen la atención de la salud de esta población.

RESUMO Objetivo: Estimar a prevalência de Infecções Sexualmente Transmissíveis (IST) em imigrantes e refugiados residentes na região metropolitana de Goiânia, Goiás. Método: Trata-se de um estudo transversal e analítico. A coleta de dados foi realizada no período de julho de 2019 a janeiro de 2020 e integraram a amostra 308 imigrantes e refugiados. Todos foram entrevistados face-a-face e testados para HIV, Sífilis e Hepatite B, por meio de testes rápidos. Resultados: A prevalência geral para alguma das IST investigadas foi de 8,8% (IC95% 6,0% – 12,3%), sendo 5,8% (IC95% 3,6% – 8,9%) para Hepatite B, 2,3% para Sífilis (IC95% 1,00% – 4,4%) e 0,7% para HIV (IC95% 0,1% – 2,1%). A análise múltipla, por regressão logística, mostrou que as variáveis sexo masculino (OR = 2,7) e tempo de moradia no Brasil (OR = 2,6) foram associadas significativamente às IST (p < 0,05). Conclusão: Os resultados deste estudo sugerem que as IST são um problema de saúde em imigrantes/refugiados, que parecem ser exacerbadas com o tempo de migração no país. Políticas públicas que garantam a assistência à saúde dessa população devem ser consideradas.

Abstract Introduction The physiotherapists were one of the health professional categories on the Coronavirus (COVID-19) pandemic front line, however little is known how the pandemic has affected the mental health of these professionals. Objective To analyze the relation-ship between the presence of common mental disorder (CMD) and elements related to the work of physiotherapists who have been assisting patients with COVID-19. Methods: This is a cross-sectional survey study composed by physiotherapists from four states of Brazil: Alagoas, Paraíba, Pernambuco and Rio Grande do Norte. Working conditions evaluated were type of contract, weekly worked hours, time working, job satisfaction, and psychosocial aspects related to work. The CMD assessment was performed using the Self-Reporting Questionnaire-20 (SRQ-20). Results A total of 288 physiotherapists answered the electronic form, out of which 76.2% had CMD. Fear of reinfection (OR 2.75, 95%CI: 1.35-5.63) and previous infection by COVID-19 (OR: 2.03, 95%CI: 1.09-3.78) were factors related to a higher chance of risk of CMD. However, those who reported being satisfied with the work (OR: 0.28, 95%CI 0.11-0.72) and being more than 30 years old (OR: 0.46, 95%CI 0.24-0.88) had a lower chance of CMD risk. Conclusion Physiotherapists showed a higher prevalence of CMD. Fear of reinfection and previous infection by COVID-19 were risk factors to CMD, while satisfaction with the work and being more than 30 years old were protective factors to CMD.

Resumo Introdução Os fisioterapeutas foram uma das categorias profissionais da linha de frente do combate à pandemia do coronavírus (COVID-19), todavia, pouco se sabe como esta pandemia afetou a saúde mental destes profissionais. Objetivo Analisar a relação entre a presença de transtorno mental comum (TMC) e elementos relacionados ao trabalho de fisioterapeutas que assistiram pacientes com COVID-19. Métodos Trata-se de um estudo transversal composto por fisioterapeutas de quatro estados do Brasil: Alagoas, Paraíba, Pernambuco e Rio Grande do Norte. As condições de trabalho avaliadas foram tipo de contrato, jornada semanal, tempo de trabalho, satisfação e aspectos psicossociais relacionados ao trabalho. A avaliação do TMC foi realizada por meio do Self-Reporting Questionnaire-20 (SRQ-20). Resultados Um total de 288 fisioterapeutas responderam ao formulário eletrônico, dos quais 76,2% tinham TMC. Medo de reinfecção (OR 2,75, IC 95%: 1,35-5,63) e infecção anterior por COVID-19 (OR: 2,03, IC 95%: 1,09-3,78) foram fatores relacionados a uma maior chance de risco de TMC. No entanto aqueles que relataram estarem satisfeitos com o trabalho (OR: 0,28, IC 95% 0,11-0,72) e os que tinham até 30 anos (OR: 0,46, IC 95% 0,24-0,88) tiveram menor chance de risco de TMC. Conclusão Os fisioterapeutas apresentaram maior prevalência de TMC. Medo de reinfecção e infecção prévia por COVID-19 foram considerados fatores de risco para TMC, enquanto a satisfação com o trabalho e ter mais que 30 anos foram fatores protetores ao TMC.

ABSTRACT The urine bag is a thermo-labile medical-hospital device used in clinical/surgical procedures in urine drainage and collection. As it is a thermolabile material, it is commonly sterilized by ethylene oxide, a highly toxic, mutagenic and carcinogenic cyclic ether gas. Currently, several countries require, through documents and standards, the quantification of ethylene oxide in medical and hospital devices. Thus, this work describes the development and validation of a method employed for the quantification of ethylene oxide (ETO) in urine bags. The method was developed based on the guidelines of ISO 10993-7, ANVISA and INMETRO. The work range, linearity, limits of detection and quantification, repeatability, intermediate precision and selectivity were evaluated. Statistical tools such as Levene's test, Grubbs, residual analysis, F-test of two variances, paired sample T test and ANOVA table, were also applied to evaluate the method. The linearity of the work range (10 – 400 mg.L-1) showed an adequate correlation coefficient (r > 0.9993), with a homoscedastic profile with absence of outliers while the limit of detection and quantification were 1,95 and 6,5 mg.L-1 respectively. After validation, 45 samples of urine bags of different batches were evaluated, which demonstrated levels of ETO below the limit of detection (1,95 mg.L-1). The results highlight a simple method that meets several regulations with a wide working range, high sensitivity and capability to quantify ETO not only in urine bags but also in other medical devices.

Abstract Background The food environment can influence food and beverage choice by the population and their nutritional status. Objective To characterize the food environment of campuses at the Federal University of Rio de Janeiro. Method A cross-sectional study of food and beverage establishments within the university was carried out. The assessment of the food environment was performed with the application of an instrument to collect information: type, food or preparations, mode, characteristics, menu and nutritional information, promotions, advertisements, availability of food or drinks, variety, and price. The observed foods and preparations, the presence of an approach such as nutritional information, advertisements, and promotions were classified as facilitators/barriers for healthy eating, according to the Food Guide for the Brazilian Population. Results Of the 5 establishments that participated, 4 were commercial and 1 was a university restaurant (RU). All establishments accepted cash and cards, and most had a menu and price. None had nutritional information. The supply of fruits and vegetables was found in 4 establishments. The availability of artificial sweetener was found in all locations. A wide variety of candies (8 ± 7.1), soft drinks (9 ± 3.7), chocolate (8 ± 7.1), salted biscuits (8 ± 12.3), and sweets (7 ± 9.1) were observed. Fruits had a higher average price, R$3.83, when compared to savory foods, R$ 2.85, and biscuits, R$ 1.50. Conclusion The university food environment presented a greater offer of unhealthy foods, presenting barriers to the promotion of more appropriate eating practices.

Resumo Introdução O ambiente alimentar pode influenciar as escolhas alimentares e de bebidas pela população e seu estado nutricional. Objetivo Caracterizar o ambiente alimentar dos campi de universidade federal do Rio de Janeiro. Método Estudo transversal com estabelecimentos que comercializavam alimentos e bebidas dentro dos campi da universidade. A avaliação do ambiente alimentar foi realizada com a aplicação de um instrumento para coleta das informações: tipo de estabelecimento, alimentos/preparações, modalidade, características, cardápio e informações nutricionais, promoções, propagandas, disponibilidade de alimentos/bebidas, variedade e preço. Os alimentos e preparações observados, a presença de estratégias como informação nutricional, propagandas e promoções foram classificados em facilitadores/ barreiras para alimentação saudável, de acordo com o Guia Alimentar para a População Brasileira. Resultados Participaram do estudo cinco estabelecimentos, quatro eram comerciais e um o restaurante universitário (RU). Todos os estabelecimentos aceitavam dinheiro e cartão, havia cardápio e preço na maioria. Nenhum apresentava informação nutricional. A oferta de frutas/hortaliças foi encontrada em quatro estabelecimentos. A disponibilidade de edulcorante artificial foi encontrada em todos os locais. Observou-se grande variedade de balas (8 ± 7,1), refrigerantes (9 ± 3,7), chocolate (8 ± 7,1), biscoito salgado (8 ± 12,3) e doce (7 ± 9,1). Frutas apresentaram preço médio superior, R$ 3,83, quando comparadas aos salgados, R$ 2,85 e biscoitos, R$ 1,50. Conclusão O ambiente alimentar universitário apresentou maior oferta de alimentos não saudáveis, apresentando barreiras na promoção de práticas alimentares mais adequadas.

ABSTRACT: The objective of this research was to estimate genetic parameters and genetic trends (GT) for 305-day milk yield (MY305) and 305-day fat yield (FY305) of purebred Dairy Gir animals of the National Dairy Gir Breeding Program. The restricted maximum likelihood method was used in an animal model. GT were obtained via linear regression and divided into two periods (1935-1992 and 1993-2013 for PL305; 1935-1992 and 1993-2010 for MY305). The estimated heritabilities were 0.23 (MY305) and 0.10 (FY305). The GT (kg/year) values for MY305 in the 2nd period for measured females (25.49), females (26.11), and males (35.13) were higher than those found in the 1st period (2.52; 2.06, and 1.00, respectively). The heritability estimated for MY305 confirmed the possibility of genetic improvement by selection and indicated a lower additive genetic effect on FY305 of purebred animals. The genetic progress for MY305 in all purebred population is denoted by the more expressive gains found from 1990’s, when the first bull catalogs were published.

RESUMO: Objetivou-se estimar os parâmetros genéticos e tendências genéticas (GT) para produção de leite (MY305) e produção de gordura (FY305), ambas em 305 dias, de animais puros Gir Leiteiro, integrantes do Programa Nacional de Melhoramento do Gir Leiteiro. Foi utilizada a metodologia da máxima verossimilhança restrita em modelo animal. As GT foram obtidas via regressão linear e divididas em dois períodos (1935-1992 e 1993-2013 para PL305; 1935-1992 e 1993-2010 para MY305). As herdabilidades foram de 0,23 (MY305) e 0,10 (FY305). Para PL305, as GT (kg/ano) do 2º período para fêmeas mensuradas (25,49), fêmeas (26,11) e, machos (35,13) foram claramente superiores às do 1º período (2,52; 2,06 e 1,00; respectivamente). A estimativa de herdabilidade para MY305 reafirma ser possível melhoramento genético por meio de seleção, enquanto para FY305 sugere uma menor influência genética aditiva em animais puros. O progresso genético para MY305 em toda a população pura está evidenciado pelos ganhos mais expressivos, observados a partir da década de 90, quando foram divulgados os primeiros sumários de touros.

ABSTRACT Background: The rate of tuberculosis (TB) infection among the prison population (PP) in Brazil is 28 times higher than that in the general population, and prison environment favors the spread of TB. Objective: To describe TB transmission dynamics and drug resistance profiles in PP using whole-genome sequencing (WGS). Methods: This was a retrospective study of Mycobacterium tuberculosis cultivated from people incarcerated in 55 prisons between 2016 and 2019; only one isolate per prisoner was included. Information about movement from one prison to another was tracked. Clinical information was collected, and WGS was performed on isolates obtained at the time of TB diagnosis. Results: Among 134 prisoners included in the study, we detected 16 clusters with a total of 58 (43%) cases of M. tuberculosis. Clusters ranged from two to seven isolates with five or fewer single nucleotide polymorphism (SNP) differences, suggesting a recent transmission. Six (4.4%) isolates were resistant to at least one anti-TB drug. Two of these clustered together and showed resistance to rifampicin, isoniazid, and fluoroquinolones, with 100% concordance between WGS and phenotypic drug-susceptibility testing. Prisoners with clustered isolates had a high amount of movement between prisons (two to eight moves) during the study period. Conclusions: WGS demonstrated the recent transmission of TB within prisons in Brazil. The high movement among prisoners seems to be related to the transmission of the same M. tuberculosis strain within the prison system. Screening for TB before and after the movement of prisoners using rapid molecular tests could play a role in reducing transmission.

ABSTRACT Cystic fibrosis (CF) is a genetic disease that results in dysfunction of the CF transmembrane conductance regulator (CFTR) protein, which is a chloride and bicarbonate channel expressed in the apical portion of epithelial cells of various organs. Dysfunction of that protein results in diverse clinical manifestations, primarily involving the respiratory and gastrointestinal systems, impairing quality of life and reducing life expectancy. Although CF is still an incurable pathology, the therapeutic and prognostic perspectives are now totally different and much more favorable. The purpose of these guidelines is to define evidence-based recommendations regarding the use of pharmacological agents in the treatment of the pulmonary symptoms of CF in Brazil. Questions in the Patients of interest, Intervention to be studied, Comparison of interventions, and Outcome of interest (PICO) format were employed to address aspects related to the use of modulators of this protein (ivacaftor, lumacaftor+ivacaftor, and tezacaftor+ivacaftor), use of dornase alfa, eradication therapy and chronic suppression of Pseudomonas aeruginosa, and eradication of methicillin-resistant Staphylococcus aureus and Burkholderia cepacia complex. To formulate the PICO questions, a group of Brazilian specialists was assembled and a systematic review was carried out on the themes, with meta-analysis when applicable. The results obtained were analyzed in terms of the strength of the evidence compiled, the recommendations being devised by employing the GRADE approach. We believe that these guidelines represent a major advance to be incorporated into the approach to patients with CF, mainly aiming to favor the management of the disease, and could become an auxiliary tool in the definition of public policies related to CF.

RESUMO A fibrose cística (FC) é uma doença genética que resulta em disfunção da proteína reguladora de condutância transmembrana da FC (CFTR), que é um canal de cloro e bicarbonato expresso na porção apical de células epiteliais de diversos órgãos. A disfunção dessa proteína resulta em manifestações clínicas diversas, envolvendo primariamente os sistemas respiratório e gastrointestinal com redução da qualidade e expectativa de vida. A FC ainda é uma patologia incurável, porém o horizonte terapêutico e prognóstico é hoje totalmente distinto e muito mais favorável. O objetivo destas diretrizes foi definir recomendações brasileiras baseadas em evidências em relação ao emprego de agentes farmacológicos no tratamento pulmonar da FC. As perguntas PICO (acrônimo baseado em perguntas referentes aos Pacientes de interesse, Intervenção a ser estudada, Comparação da intervenção e Outcome [desfecho] de interesse) abordaram aspectos relativos ao uso de moduladores de CFTR (ivacaftor, lumacaftor + ivacaftor e tezacaftor + ivacaftor), uso de dornase alfa, terapia de erradicação e supressão crônica de Pseudomonas aeruginosa, e erradicação de Staphylococcus aureus resistente a meticilina e do complexo Burkholderia cepacia. Para a formulação das perguntas, um grupo de especialistas brasileiros foi reunido e realizou-se uma revisão sistemática sobre os temas, com meta-análise quando aplicável. Os resultados encontrados foram analisados quanto à força das evidências compiladas, sendo concebidas recomendações seguindo a metodologia GRADE. Os autores acreditam que o presente documento represente um importante avanço a ser incorporado na abordagem de pacientes com FC, objetivando principalmente favorecer seu manejo, podendo se tornar uma ferramenta auxiliar na definição de políticas públicas relacionadas à FC.

ABSTRACT Objective To identify eye conditions associated to social distancing and related increase in screen time exposure on children and teenagers. Methods Observational, descriptive and analytical study, with a cross-section design, carried out between July and August 2021, by questionnaire survey. Results Headaches related to excessive screen time exposure were reported by 26.51% of the students surveyed, while 38.8% reported suspect convergence insufficiency. Regarding the questionnaire about dry eye, 18.8% were symptomatic and 7.5% reported hard-to-manage symptoms. Computers were the devices most used for schoolwork (71.5%), followed by cellphones on second place (66.3%). Symptomatic cases of dry eye were mostly reported by participants that did not make use of computers (36.4%). It was not possible to establish a connection between convergence insufficiency and device usage time. Those participants that reported more screen time exposure also reported an increased percentage of hard-to-manage symptoms (42.9%), regarding dry eye. Conclusion Dry eye and convergence insufficiency symptoms were more prevalent in the surveyed population.

RESUMO Objetivo Identificar as alterações oculares em crianças e adolescentes secundárias ao fator do isolamento social e ao consequente aumento do uso de telas. Métodos Estudo observacional, descritivo e analítico, do tipo transversal realizado no período de julho a agosto de 2021, por meio da aplicação de questionários. Resultados Apresentaram cefaleia associada ao uso excessivo de tela 26,51% dos estudantes e 38,8% relataram suspeita de insuficiência de convergência. Em relação ao questionário de olho seco, 18,8% foram sintomáticos e 7,5% sintomáticos de difícil manejo. O computador foi o dispositivo mais utilizado em atividades escolares (71,5%), e, em segundo lugar, esteve o celular (66,3%). Casos sintomáticos de olho seco foram mais relatados em participantes que não faziam uso de computador (36,4%). Não foi possível identificar relação entre insuficiência de convergência e tempo de uso de dispositivos. Em relação ao olho seco, aqueles que passaram mais tempo em frente a tela relataram maior porcentagem de sintomas de difícil manejo (42,9%). Conclusão Foram mais prevalentes olho seco e sintomas de insuficiência de convergência na população estudada.
