The marked increase in cases and deaths by Covid-19 has led to a significant overload on the health system in Brazil, especially in the cities of Manaus and Rio de Janeiro, and São Paulo. The description of the pandemic's impact has been based on absolute numbers or crude mortality rates without considering the pattern of distribution of age groups in different regions of the country. This study aims to compare the crude mortality rates for Covid-19 with standardized rates for age in the Brazilian states' capitals and in the Federal District. Information on death was accessed in the Influenza Surveillance Information System (SIVEP-Gripe), and the population denominators were based on the estimates available by the Ministry of Health. The age structure of Brazil's estimated population for 2020 was used as standard to calculate the age-standardized rates. The results show that the highest crude rates were observed in Manaus (253.6/100,000) and Rio de Janeiro (253.2/100,000). After age-standardization, there was a significant increase in rates in the North region. The highest adjusted rate was seen in Manaus (412.5/100,000), where 33% of deaths by Covid-19 occurred among individuals under 60. The mortality over 70 years old doubled if compared to Rio de Janeiro and tripled if compared to Sao Paulo. The use of age-standardized mortality rates eliminates interpretive biases, exposing the even greater weight of Covid-19 in the northern region of country.

O crescimento acentuado de casos e óbitos por Covid-19 tem levado a grande sobrecarga do sistema de saúde no Brasil em especial em cidades com Manaus e Rio de Janeiro e São Paulo. A descrição do impacto da pandemia tem se baseado em números absolutos ou taxas de mortalidade brutas não considerando o padrão de distribuição das faixas etárias nas diferentes regiões do país. Este estudo tem por objetivo comparar a taxas de mortalidade brutas por Covid-19 com taxas padronizadas por idade nas capitais dos estados brasileiras e no Distrito Federal. As informações sobre óbito foram acessadas no Sistema de Informação de Vigilância da Gripe (SIVEP-Gripe) e os denominadores populacionais foram baseados nas estimativas disponibilizadas pelo Ministério da Saúde. Para o cálculo das padronizadas por idade utilizou-se a estrutura etária da população do Brasil estimada para 2020. Os resultados mostram que as maiores taxas brutas foram em Manaus (253,6/100.000) e Rio de Janeiro (253,2/100.000). Após padronização por idade, houve aumento expressivo das taxas na região Norte. A maior taxa ajustada foi vista em Manaus (412,5/100.000) onde 33% de óbitos por Covid-19 ocorreram entre menores de 60 anos. A mortalidade acima de 70 anos foi o dobro se comparada à do Rio  de Janeiro e o triplo se comparada à de São Paulo. A utilização de taxas de mortalidade padronizadas por idade elimina vieses interpretativos expondo de forma marcante o peso ainda maior da Covid-19 na região Norte do país.

A fonte de dados utilizada foi o registro de óbitos pelos Cartórios de Registro Civil. Os dados foram desagregados por capitais e demais municípios das 26 unidades federativas e do Distrito Federal segundo sexo. A razão de mortalidade ajustada para o ano de 2020 foi calculada tendo como padrão os coeficientes de mortalidade do ano de 2019. Os resultados indicaram excesso de 39.146 óbitos para o período estudado, sendo maior entre homens do que nas mulheres. Esse aumento foi maior nas capitais das regiões Norte, Nordeste e Sudeste. Nos demais municípios dessas regiões o incremento foi observado em maio, indicando possível interiorização da transmissão da COVID-19. Evidencia-se a necessidade de se aprimorar a detecção e o registro de casos para viabilizar o monitoramento eficiente da pandemia.

O objetivo deste artigo é identificar a associação entre os fatores reprodutivos e o excesso de peso em mulheres de 20 a 59 anos de áreas cobertas pela Unidade de Atenção Primária à Saúde. Trata-se de um estudo transversal com captação domiciliar e coleta de dados em duas Unidades de Atenção Primária à Saúde, no município de Juiz de Fora, em mulheres de 20 a 59 anos, cujo desfecho foi o Excesso de Peso medido por meio do Índice de Massa Corporal. Entre as 2018 mulheres incluídas na análise, a prevalência de Excesso de Peso foi de 61,0%. Na análise multivariada, o Excesso de Peso apresentou associação com as variáveis idade da menarca antes dos 12 anos, ter filhos, idade maior ou igual a 30 anos e relato de hipertensão arterial. A prevalência de excesso de peso em mulheres que tiveram menarca antes dos 12 anos foi 12,4% maior do que naquelas que a tiveram com 12 anos ou mais; enquanto que a prevalência de Excesso de Peso em mulheres que tiveram filhos foi 58,2% maior do que entre as que nunca tiveram. Foi observada elevada prevalência de Excesso de Peso na população feminina adulta, com ênfase na influência dos fatores reprodutivos.

ABSTRACT OBJECTIVE: to describe the trend of mortality from general cancer and more frequent types among men and women living in the Capitals and other municipalities of the five macro-regions of Brazil between 1978 and 2017. METHODS: Time series study with mortality data corrected by redistribution of ill-defined causes. Proportional cancer mortality was calculated for Brazil and regions. The annual percentage change in rates for total cancer and specific types in each segment and in the selected unit of analysis was calculated by generalized linear regression with Gaussian binding. RESULTS: the proportion of cancer increased progressively for both sexes from 1978 to 2017. Important differences between the Capitals and the interior of the macro-regions were seen with disaggregated data. The greatest declines occurred for stomach cancer, except in the northern and interior regions of the Northeast, and for the cervix cancer, with a generalized fall, with the exception of the interior of the northern region. Lung cancer decreased among men in the Southeast and South regions and had a generalized increase among women. Breast and prostate cancers tended to decrease in the Southeast and South regions and among residents of the Capitals but showing an increase in the interior of the North and Northeast regions. Colorectal cancer had a general tendency to increase; with stability among men in the Capitals of the South region and among women of the Southeast and Midwest regions and, since 2007, a decrease among women in the South region. CONCLUSIONS: Cancer mortality showed great variation among residents of capitals and the interior of the country's major regions. Clear decrease in mortality was seen for the main types in the Southeast and South regions. The North and Northeast regions have patterns compatible with cancers associated with poverty, while the large increase of the cancers related to sedentary lifestyle stand out.

RESUMO OBJETIVO: Descrever a tendência da mortalidade por câncer geral e tipos mais frequentes entre homens e mulheres residentes nas capitais e demais municípios das cinco macrorregiões do Brasil entre 1978 e 2017. MÉTODOS: Estudo de séries temporais com dados de mortalidade corrigidos por redistribuição das causas maldefinidas. Foi calculada a mortalidade proporcional de câncer para Brasil e regiões. A variação percentual anual das taxas para o total de câncer e tipos específicos em cada segmento e na desagregação selecionada foi calculada por regressão linear generalizada com ligação gaussiana. RESULTADOS: A proporção de câncer aumentou progressivamente para ambos os sexos de 1978 a 2017. Diferenças importantes entre as capitais e o interior das macrorregiões foram vistas com dados desagregados. Os maiores declínios ocorreram para o câncer de estômago, exceto nas regiões Norte e interior da Nordeste, e de colo do útero, com queda generalizada, com exceção do interior da região Norte. O câncer de pulmão teve queda entre homens nas regiões Sudeste e Sul e aumento generalizado entre mulheres. Os cânceres de mama e de próstata tenderam a diminuir nas regiões Sudeste e Sul e entre residentes das capitais, mostrando, porém, aumento no interior das regiões Norte e Nordeste. O câncer colorretal teve tendência geral de aumento; nas capitais houve estabilidade entre homens da região Sul e entre mulheres das regiões Sudeste e Centro-Oeste e queda entre mulheres da região Sul a partir de 2007. CONCLUSÕES: A mortalidade por câncer apresentou grande variação entre os residentes de capitais e do interior das grandes regiões do país. Diminuição clara da mortalidade foi vista para os principais tipos nas regiões Sudeste e Sul. As regiões Norte e Nordeste apresentam padrões compatíveis com cânceres associados à pobreza ao mesmo tempo que se destacam pelo grande aumento daqueles relacionados ao estilo de vida sedentário.

This text seeks to contribute to this analysis based on the results of studies on the developing health conditions of the population and the Brazilian health system in the last 30 years. It identifies the strengthening of the private sector and capital in the health sector, to the detriment of the public interest and the SUS. Finally, it discusses the strategies of the struggle for the right to health necessary and possible in the current context.

Abstract: Our objectives with this study were to describe the spatial distribution of mammographic screening coverage across small geographical areas (micro-regions) in Brazil, and to analyze whether the observed differences were associated with spatial inequities in socioeconomic conditions, provision of health care, and healthcare services utilization. We performed an area-based ecological study on mammographic screening coverage in the period of 2010-2011 regarding socioeconomic and healthcare variables. The units of analysis were the 438 health micro-regions in Brazil. Spatial regression models were used to study these relationships. There was marked variability in mammographic coverage across micro-regions (median = 21.6%; interquartile range: 8.1%-37.9%). Multivariable analyses identified high household income inequality, low number of radiologists/100,000 inhabitants, low number of mammography machines/10,000 inhabitants, and low number of mammograms performed by each machine as independent correlates of poor mammographic coverage at the micro-region level. There was evidence of strong spatial dependence of these associations, with changes in one micro-region affecting neighboring micro-regions, and also of geographical heterogeneities. There were substantial inequities in access to mammographic screening across micro-regions in Brazil, in 2010-2011, with coverage being higher in those with smaller wealth inequities and better access to health care.

Resumo: O estudo teve como objetivos descrever a distribuição espacial do rastreamento por mamografia entre áreas geográficas pequenas (microrregiões) no Brasil, além de investigar se as diferenças observadas estavam associadas a inequidades espaciais nas condições socioeconômicas, na prestação de assistência à saúde e no uso de serviços de saúde. Este foi um estudo ecológico de base territorial, comparando a cobertura do rastreamento por mamografia em 2010-2011 com fatores socioeconômicos e de cuidados de saúde. O estudo usou 438 microrregiões sanitárias brasileiras como as unidades analíticas. Foram utilizados modelos de regressão espacial para estudar as associações. Houve uma importante variabilidade na cobertura por mamografia entre microrregiões (mediana = 21,6%; variação interquartil: 8,1%-37,9%). A análise multivariada identificou: forte desigualdade na renda familiar, número baixo de radiologistas/100 mil habitantes, número baixo de aparelhos de mamografia/10 mil habitantes e número baixo de mamografias realizadas com cada aparelho enquanto correlatos independentes da baixa cobertura mamográfica no nível microrregional. Houve evidência de forte dependência espacial nessas associações, em que as mudanças em uma microrregião afetavam as microrregiões vizinhas, além de heterogeneidade geográfica. O estudo revelou importantes inequidades no acesso ao exame de mamografia entre microrregiões brasileiras em 2010-2011, com cobertura mais alta nas microrregiões com menor desigualdade de renda e melhor acesso geral aos cuidados de saúde.

Resumen: Los objetivos de este estudio fueron describir la distribución espacial de la cobertura del cribado mamográfico, a través de pequeñas áreas geográficas (microrregiones) en Brasil, y examinar si las diferencias observadas estuvieron asociadas con inequidades espaciales, en términos de condiciones socioeconómicas, sistema de atención de salud, y utilización de servicios de salud. Se trata de un estudio ecológico, basado en áreas incluidas en la cobertura de cribado mamográfico durante 2010-2011 y relacionadas con variables socioeconómicas y de salud. Las unidades de análisis fueron 438 microrregiones de salud en Brasil. Se utilizaron modelos de regresión espacial para estudiar estas relaciones existentes. Hubo una variabilidad marcada en relación con la cobertura mamográfica a través de las microrregiones (media = 21.6%; rango intercuartílico: 8,1%-37,9%). Los análisis multivariables identificaron una alta inequidad en los ingresos por hogar, bajo número de radiólogos/100,000 habitantes, bajo número de máquinas de mamografía/10.000 habitantes, y un bajo número de mamografías realizadas por cada máquina, lo que está independiente correlacionado con la baja cobertura de mamografías en el nivel de microrregión. Hubo evidencias de una dependencia espacial fuerte de estas asociaciones, con cambios en una microrregión afectando a microrregiones vecinas, y también de heterogeneidades geográficas. Hubo inequidades sustanciales en el acceso al cribado mamográfico a través de las microrregiones en Brasil, en 2010-2011, con una cobertura superior en aquellas con pequeñas inequidades respecto a la riqueza y mejor acceso a los servicios de salud.
