ABSTRACT Purpose To describe the main quantitative methods used for morphometric analysis of the vocal tract in singers, their applications and the main segments studied. Research strategy This is an integrative review guided by the guiding question “What are the main quantitative methods used for morphometric analysis of the vocal tract, their applications and the main segments studied in singers?”. The electronic databases PubMed, Scopus and VHL were used through the search key (Vocal tract OR Oropharynx) AND (Morphology OR Geometry) AND (Evaluation OR Diagnosis) AND (voice), without restriction of years of publication, including articles in three languages: Portuguese, English and Spanish. Selection criteria The selection took place independently through reading by pairs and subsequent application of exclusion and inclusion criteria. Results Of the 380 studies, 30 duplicates were excluded. After reading the abstracts, applying the selection criteria, 18 studies were selected for full reading, of which 12 were included in this review. Magnetic resonance imaging was the main instrument used and the analyzed segments ranged from the length and volume of the vocal tract to isolated segments and their morphological particularities. Conclusion Vocal tract morphological quantification methods are important for the instrumental evaluation of the vocal tract and its segments, a technological update leading to better understanding of singers’ voice and therapeutical intervention.

RESUMO Objetivo descrever os principais métodos quantitativos utilizados para análise morfométrica do trato vocal em cantores, suas aplicações e os principais segmentos estudados. Estratégia de pesquisa trata-se de uma revisão integrativa guiada pela pergunta condutora: “Quais os principais métodos quantitativos utilizados para análise morfométrica do trato vocal, suas aplicações e os principais segmentos estudados em cantores?”. Foram utilizadas as bases eletrônicas PubMed, Scopus e BVS, por meio da chave de busca Vocal tract OR Oropharynx AND Morphology OR Geometry AND Evaluation OR Diagnosis AND voice, sem restrições de ano de publicação, sendo incluídos artigos em três idiomas: português, inglês e espanhol. Critérios de seleção: a seleção se deu de forma independente, por meio da leitura por pares e posterior aplicação dos critérios de exclusão e inclusão. Resultados dos 380 estudos, foram excluídos 30 duplicados. A partir da leitura dos resumos, aplicando-se os critérios de seleção, foram selecionados 18 estudos para leitura na íntegra, dos quais, 12 foram incluídos nesta revisão. A ressonância magnética foi o principal instrumento utilizado e os segmentos analisados incluíram desde o comprimento e volume do trato vocal, como segmentos isolados e suas particularidades morfológicas. Conclusão os métodos de quantificação morfológica do trato vocal integram instrumentos importantes para a avaliação instrumental do trato vocal e de seus segmentos, colaborando na atualização tecnológica em voz para melhor compreensão e intervenções fonoaudiológicas na voz cantada

Abstract Thiazolidinedione, often shortened to TZD or glitazone, helps lower insulin resistance, which is the underlying problem for many people with type 2 diabetes. The two most known glitazones are pioglitazone (PGZ), with the brand name medicine Actos®, and rosiglitazone (RSG), which is Avandia®. This study presented a multivariate optimization in the microextraction procedure employing Fractional Factorial Design (FFD) combined with Desirability Function (DF) to determine TZD and metabolites in biological samples. Microextraction requires several parameters to be optimized; however, most of them still use univariate optimization. Finding optimum conditions by simple response is relatively simple, but the problems, in case of microextractions, are often more complex when it has more responses. For example, changing one factor that promotes one response may suppress the effect of the others. Thus, this multivariate optimization was applied for two bioanalytical methods for determination of TZD and metabolites, one by HPLC and other by CE, both using Hollow Fiber Liquid-Phase Microextraction (HF-LPME). The results establish the optimal values and elucidate how the factors that affect HF-LPME procedure perform in extraction efficiency for TZDs. Additionally, this study demonstrates that DF can be an important tool to optimize microextraction procedures.

TEMA: o objetivo deste estudo foi descrever os aspectos clínico, comportamental, cognitivo e comunicativo de indivíduos com o diagnóstico genético da Síndrome Smith-Magenis. PROCEDIMENTOS: participaram dois indivíduos do sexo masculino, de nove e 19 anos. Realizou-se a avaliação genética clínica e laboratorial (teste FISH, utilizando sonda para região 17p11.2). A avaliação psicológica constou da observação comportamental e aplicação da Escala Wechsler de Inteligência. A avaliação Fonoaudiológica foi realizada por meio de procedimentos formais e informais e avaliação auditiva periférica. RESULTADOS: a análise genética clínica evidenciou as características fenotípicas da síndrome Smith-Magenis, confirmada pela avaliação laboratorial. A avaliação psicológica evidenciou o fenótipo comportamental peculiar da síndrome Smith-Magenis e comprovou a deficiência intelectual de grau moderado nos dois indivíduos. A avaliação fonoaudiológica mostrou alterações no desempenho linguístico, com alterações nos níveis fonológico, semântico, sintático e pragmático e nas habilidades psicolinguísticas, interferindo nas habilidades comunicativas e de aprendizagem. A avaliação auditiva indicou audição periférica dentro de parâmetros de normalidade. CONCLUSÃO: a avaliação multidisciplinar favoreceu a descrição dos aspectos clínicos, comportamentais, cognitivos que pertencem ao fenótipo comportamental da síndrome Smith-Magenis e permitiu verificar que estes apresentam graves alterações da linguagem oral, das habilidades psicolinguísticas e do processamento das informações visuais e auditivas com reflexos marcantes no desenvolvimento das habilidades comunicativas e processos de aprendizagem.

BACKGROUND: this study aimed to describe the clinical, behavioral, cognitive and communicative features of subjects with Smith-Magenis Syndrome genetic diagnosis. PROCEDURES: the subjects were two males, 09 and 19 year old. We performed a clinical and laboratory genetic evaluation (FISH assay using probes for the region 17p11.2). The psychological evaluation consisted of behavioral observation and application of the Wechsler Intelligence Scale. Speech evaluation was performed by means of formal and informal procedures and peripheral hearing evaluation. RESULTS: the clinical genetic analysis showed the phenotypic characteristics of Smith-Magenis syndrome, confirmed by laboratory evaluation. The psychological evaluation revealed the peculiar phenotype behavioral of Smith-Magenis syndrome and confirmed the moderate intellectual disabilities in two subjects. Speech evaluation showed changes in language performance, with changes in phonological, semantic, syntactic and pragmatic levels and psycholinguistic skills, interfering with communication and learning skills. The hearing test showed peripheral hearing within normal parameters. CONCLUSION: the multidisciplinary approach made easier the description of clinical, behavioral, cognitive aspects, belonging to the behavioral phenotype of Smith-Magenis syndrome and showed that these changes have severe oral language alterations in skills and psycholinguistic processing of visual and auditory information with remarkable consequences on the development of communicative skills and learning processes.

FUNDAMENTO: As alterações cardíacas na fase de transição do coração fetal para a vida extrauterina vêm sendo exploradas por inúmeras pesquisas em animais, e os mecanismos celulares responsáveis por essas modificações ainda não estão bem documentado em seres humanos. OBJETIVO: Avaliar o mecanismo de diferenciação celular em cardiomiócitos ocorridas nos primeiros dias de vida, por meio da análise imunoistoquímica de proteínas envolvidas com processos de proliferação e contração muscular, em amostras de miocárdio de recém-natos humanos. MÉTODO: Estudo transversal de amostras parafinadas de miocárdio provenientes de banco de necropsias de recémnascidos humanos, divididos em dois grupos amostrais: recém-nascidos a termo que foram a óbito com no máximo dois dias de vida (NEO1) com 10 casos, e recém- nascidos a termo que foram a óbito entre três e 10 dias de vida (NEO2) com 14 casos, a fim de seguir uma linha de tempo que contemplasse a fase de transição da circulação fetal a vida extrauterina. As amostras foram estudadas em tissue microarray e os anticorpos utilizados foram o Ki67, PCNA, PTEN, Bcl2 (proliferação) e HHF35 e actina sarcomérica (proteínas contráteis). RESULTADOS: Foi encontrada diferença com o Ki67 p = 0,02, HHF35 p < 0,01 e actina sarcomérica p = 0,02, e a expressão do Ki67 foi mais alta no grupo NEO1 e a expressão do HHF35 e da actina sarcomérica foi mais alta no grupo NEO2. CONCLUSÃO: Os resultados sugerem que os cardiomiócitos apresentam uma característica proliferativa (Ki67) nos NEO1 e que essa vai, seguindo uma linha temporal, sendo substituída por um caráter de diferenciação (HHF35 e actina sarcomérica) nos NEO2.

BACKGROUND: The cardiac alterations during the fetal heart transition to extrauterine life have been explored by several animal studies and the cell mechanisms responsible for these modifications are not well documented in humans. OBJECTIVE: To evaluate the mechanism of cell differentiation into cardiomyocytes that occur in the first days of life, through immunohistochemical analysis of proteins involved in proliferation and muscle contraction processes, in samples of human neonate myocardium. METHODS: Cross-sectional study of paraffin-sample sections of myocardium from an autopsy database of human neonates, divided into two sample groups: full-term neonates who died after a maximum of two days of life (NEO1) with 10 cases, and full-term infants who died between 3 and 10 days of life (NEO2) with 14 cases, in order to follow a temporal line that would contemplate the transition from fetal circulation to extrauterine life. The samples were studied in tissue microarray and the antibodies used were Ki67, PCNA, PTEN, Bcl2 (proliferation), HHF35 and sarcomeric actin (contractile proteins). RESULTS: Difference was observed regarding Ki67, p = 0.02; HHF35, p <0.01 and sarcomeric actin, p = 0.02, with Ki67 expression being higher in NEO1 group, whereas HHF35 and sarcomeric actin expression was higher in the NEO2 group. CONCLUSION: The results suggest that cardiomyocytes have a proliferation characteristic (Ki67) in NEO1 which, following a temporal line, will be replaced by a differentiation characteristic (HHF35 and sarcomeric actin) in NEO2.

OBJETIVO: Descrever o desempenho de indivíduos com mielomeningocele quanto às habilidades psicolinguísticas e escolares. MÉTODOS: Participaram cinco indivíduos, com idade cronológica entre 9 anos e 10 meses e 11 anos e 7 meses, com diagnóstico de mielomeningocele e lesão lombo-sacral (Grupo 1 - G1), e cinco indivíduos com desenvolvimento típico (Grupo 2 - G2), que foram pareados ao G1 quanto a idade, gênero e grau de escolaridade. A avaliação constou de entrevista com os responsáveis e aplicação dos seguintes testes: Teste de Illinois de Habilidades Psicolinguísticas; Teste de Desempenho Escolar; Teste de Velocidade de Leitura; e Teste de Nomeação Automática Rápida. RESULTADOS: A comparação entre os grupos nos subtestes do Teste de Illinois de Habilidades Psicolinguísticas indicou que os valores máximos obtido pelo G1 corresponderam aproximadamente aos valores mínimos obtidos pelo G2, confirmando a diferença entre eles, exceto para o subteste closura auditiva. No Teste de Desempenho Escolar foram observadas alterações significativas no desempenho do G1 em todas as provas. Nos testes de Velocidade de Leitura e de Nomeação Rápida, os indivíduos do G1 também apresentaram prejuízos consideráveis, cometendo mais erros nas tarefas e realizando a atividade de modo mais lento. CONCLUSÃO: Indivíduos com mielomeningocele apresentam alterações nas habilidades psicolinguísticas, no desempenho escolar, na velocidade de leitura e na nomeação automática rápida.

PURPOSE: To describe the performance of individuals with myelomeningocele regarding psycholinguistic and scholastic abilities. METHODS: Participants were five individuals with myelomeningocele and lumbar sacral abnormalities, and chronological age between 9 years and 10 months and 11 years and 7 months (Group 1 - G1); five subjects with typical development (Group 2 - G2), matched to G1 for age, gender, and educational level. The evaluation consisted of interview with parents/caregivers, and application of the following tests: Illinois Test of Psycholinguistic Abilities (ITPA); School Performance Test; Speed Reading Test; and Rapid Automatized Naming Test. RESULTS: The between-groups comparison in the ITPA subtests showed that the maximum values obtained by G1 corresponded approximately to the minimum values obtained by G2, confirming the difference between the groups, except for the auditory closure subtest. In the Scholastic Performance Test, significant alterations were observed on the performance of G1 in all tasks. In the Speed Reading and Rapid Automatized Naming tests, individuals in G1 also presented considerable deficits, making more mistakes and spending more time than G2 to perform the same tasks. CONCLUSION: Individuals with myelomeningocele present deficits in psycholinguistic abilities, school performance, reading speed, and rapid automatized naming.

TEMA: desempenho de crianças com PKU no Teste de Screening de Desenvolvimento Denver - II. Introdução: a fenilcetonúria é uma desordem autossômica recessiva resultante da mutação do gene localizado no cromossomo 12q22.24.1. OBJETIVO: caracterizar o desempenho de crianças com fenilcetonúria diagnosticadas e tratadas precocemente por meio do Teste de Screening de Desenvolvimento Denver II e dos níveis de fenilalanina sanguíneos. MÉTODO: participaram 20 crianças, dez com fenilcetonúria, diagnosticadas e tratadas desde o nascimento, de idade cronológica entre três a seis anos, e dez crianças do grupo típico, pareadas quanto ao sexo, idade e nível socioeconômico. Os níveis sanguíneos e as informações neurológicas, psicológicas e sociais foram obtidas no banco de dados do Programa de Triagem Neonatal para Erros Inatos do Metabolismo. A avaliação constou da aplicação do Teste de Screening de Desenvolvimento Denver-II. Utilizou-se estatística descritiva e aplicação do teste estatístico de Mann Whitney para a caracterização das habilidades. Para as medições dos níveis plasmáticos sanguíneos de fenilalanina considerou-se os valores abaixo de 2mg/dL, acima de 4mg/dL, os valores de referência entre 2 e 4mg/dL, de todos os exames realizados no decorrer da vida dos participantes, os valores mínimos e máximos e o valor obtido na época da avaliação fonoaudiológica. Resultado: A comparação entre os grupos foi estatisticamente significante nas áreas pessoal-social e de linguagem. CONCLUSÃO: crianças com fenilcetonúria diagnosticadas e tratadas precocemente apresentaram prejuízo nas áreas pessoal-social e de linguagem e, mesmo com o acompanhamento periódico, apresentaram dificuldades para manter os níveis de normalidade de fenilalanina, embora realizassem o tratamento recomendado.

BACKGROUND: phenylketonuria is an autosomal recessive disorder resulting from the mutation of a gene located in chromosome 12q22-24.1. AIM: to describe the performance of children with classic phenylketonuria, who were diagnosed and treated early, in the Development Screening Test Denver - II. METHOD: participants were 20 children with phenylketonuria, ranging in age from 3 and 6 years, and 10 children with typical language development, paired by gender, age and socioeconomic level to the research group. The plasmatic phenylalanine measure and the neurological, psychological and social information were gathered in the data base of the Neonatal Screening Programs for Metabolic disorder. Assessment consisted on the application of the Development Screening Test Denver II. A descriptive statistical analysis and the Mann Whitney test were used in order to characterize the tested skills. For the measurements of the plasmatic phenylalanine blood levels the values considered for analysis were: below 2mg/dL, above 4mg/dL, reference values between 2 and 4mg/dL, of all exams performed during the participants'lives; maximum and minimum values and values obtained on the day of the screening application. RESULTS: comparison between the groups indicated statistically significant differences for the personal-social and language areas. CONCLUSION:children who were diagnosed and treated early for phenylketonuria present deficits in the personal-social and language areas. Also, even when receiving follow-up and undergoing treatment, these children presented difficulties in maintaining normal plasmatic phenylalanine levels.

TEMA: a fenilcetonúria é manifestada por deficiência parcial ou total da enzima hepática fenilalanina hidroxilase que, em excesso, tem efeito tóxico para as funções do sistema nervoso central, refletindo no desenvolvimento global do indivíduo. OBJETIVO: apresentar as alterações no desenvolvimento verificadas em estudos científicos com indivíduos portadores de fenilcetonúria e refletir sobre as habilidades relacionadas ao desenvolvimento da linguagem. CONCLUSÃO: indivíduos com fenilcetonúria são de risco para alterações nas funções cognitivas, linguísticas, motoras e comportamental-social. Déficits nas funções executivas e habilidades neuropsicolinguísticas são comuns e acarretam defasagens para o desenvolvimento das habilidades de linguagem. Os achados justificam o encaminhamento de proposta para o Ministério da Saúde com vistas à contratação de Fonoaudiólogos nos Programas de Triagem Neonatal credenciados.

BACKGROUND: phenylketonuria is manifested by partial or total deficiency of the hepatic enzyme phenylalanine hydroxylase that, in excess, has a toxic effect on the central nervous system functions, reflecting in the individual's global development. PURPOSE: to submit the alterations in the development verified in scientific studies with individuals with phenylketonuria, and to contemplate the abilities related to language development. CONCLUSION: individuals with phenylketonuria are risky for alterations in the cognitive, linguistics, motor and social-behavior functions. Deficits in the executive functions and neuropsychological abilities are common and imply in discrepancies as for language abilities development. The findings justify the proposal forwarding to the Ministry of Health with views to contracting a Speech and Language Pathologist in the accredited Neonatal Screening Programs.

Este estudo teve como objetivo caracterizar o perfil de habilidades comunicativas de cinco irmãos com Desordens do Espectro Alcoólico Fetal. O diagnóstico de Desordens do Espectro Alcoólico Fetal foi realizado a partir do histórico gestacional positivo para álcool e identificação de sinais clínicos. A avaliação fonoaudiológica constou da Observação do Comportamento Comunicativo, Escala de Desenvolvimento Comportamental de Gesell e Amatruda, Teste de Vocabulário por Imagens Peabody e avaliação audiológica. Todos os participantes apresentaram alterações nos comportamentos motor grosso, motor delicado, adaptativo, pessoal-social e de linguagem em graus variados. As habilidades comunicativas estavam comprometidas para todos os participantes e S4 apresentava comportamentos autísticos. As Desordens do Espectro Alcoólico Fetal foram confirmadas em S1, S2 e S5 e o diagnóstico de Síndrome Alcoólica Fetal foi confirmado para S3 e S4. Os resultados apresentaram variabilidade no desenvolvimento das habilidades de desenvolvimento dos irmãos com as Desordens do Espectro Alcoólico Fetal. A variabilidade dos achados, principalmente nas habilidades comunicativas e comportamentais, sugere a necessidade de acompanhar crianças com histórico de uso de álcool pela mãe, visto o impacto destas desordens no desenvolvimento global destes indivíduos, com impacto nas atividades de vida diária e escolaridade.

The present study had the aim to characterize the communicative abilities profile of five siblings with Fetal Alcohol Spectrum Disorders. This diagnosis was carried out based on the positive report of prenatal alcohol exposure and identification of clinical signs. The Speech-Language Pathology evaluation consisted of the Communicative Behavior Observation, the Behavioral Development Scale of Gesell and Amatruda, the Peabody Picture Vocabulary Test, and hearing evaluation. Participants presented various degrees of alterations in gross motor, fine motor, adaptative, personal-social and language behaviors. The communicative abilities were altered for all the participants, and S4 presented autistic behaviors. Fetal Alcohol Spectrum Disorders were confirmed in S1, S2 and S5 and the diagnosis of Fetal Alcohol Syndrome was confirmed for S3 and S4. The results showed variability in the development of the studied abilities among the siblings with Fetal Alcohol Spectrum Disorders. The variability of the findings, especially in communicative abilities and behavior, suggests the need to follow-up children with reports of alcohol use by the mother, considering the impact of these disorders on these individuals' global development, including daily life activities and schooling.

The Neonatal Screening for Inborn Errors of Metabolism of the Association of Parents and Friends of Special Needs Individuals (APAE) - Bauru, Brazil, was implanted and accredited by the Brazilian Ministry of Health in 1998. It covers about 286 cities of the Bauru region and 420 collection spots. Their activities include screening, diagnosis, treatment and assistance to congenital hypothyroidism (CH) and phenylketonuria (PKU), among others. In 2005, a partnership was established with the Department of Speech-Language Pathology and Audiology, Bauru School of Dentistry, University of São Paulo, Bauru, seeking to characterize and to follow, by means of research studies, the development of the communicative abilities of children with CH and PKU. OBJECTIVE: The aim of this study was to describe communicative and psycholinguistic abilities in children with CH and PKU. MATERIALS AND METHODS: Sixty-eight children (25 children aged 1 to 120 months with PKU and 43 children aged 1 to 60 months with CH) participated in the study. The handbooks were analyzed and different instruments were applied (Observation of Communication Behavior, Early Language Milestone Scale, Peabody Picture Vocabulary Test, Gesell & Amatruda's Behavioral Development Scale, Portage Operation Inventory, Language Development Evaluation Scale, Denver Developmental Screening Test, ABFW Child Language Test-phonology and Illinois Test of Psycholinguistic Abilities), according to the children's age group and developmental level. RESULTS: It was observed that the children with PKU and CH at risk for alterations in their developmental abilities (motor, cognitive, linguistic, adaptive and personal-social), mainly in the first years of life. Alterations in the psycholinguistic abilities were also found, mainly after the preschool age. Attention deficits, language and cognitive alterations were more often observed in children with CH, while attention deficits with hyperactivity and alterations in the personal-social, language and motor adaptive abilities were more frequent in children with PKU. CONCLUSION: CH and PKU can cause communicative and psycholinguistic alterations that compromise the communication and affect the social integration and learning of these individuals, proving the need of having these abilities assisted by a speech and language pathologist.
