ABSTRACT Introduction. School victimization is a current problem in Peru. Gender approach in analysis allows the adaptation of differentiated intervention strategies. Objective. To determine the risk factors for victimization according to gender approach in school adolescents from cities of Huamanga and Sicuani. Design. Analytical cross-sectional observational. Participants. 1160 students from first to fifth grade from two schools (private and public) in Huamanga and Sicuani. Intervention. Validated survey to identify victims of bullying. Analysis. Desciptive, chi-square, OR (95% CI) and multivariate logistic regression analysis. The SPSS vs. 15 program was used. Main outcome measures. Percentage and risk factors for school victimization. Results. 37.3% of students were found victims. In male gender, "has witnessed a violent act" 1.72 (1.122 - 2.636); in female gender "to live with one of parent" 1.682 (1.072 - 2.641); "live in Huamanga" 1.767 (1.119 - 2.790); and in both genders "having a physical defect" and "parents react with violence when they misbehave" were the most important risk factors. The most reported forms of victimization in both genders were "they gave you nicknames" and "you have been discriminated against". Direct forms such as "they called you homosexual" and "you have been beaten" prevailed in men, and indirect forms in women. Conclusions. The school victimization is an important problem. In female gender is related to individual and family characteristics and in male gender to school safety. The direct forms of victimization are related to male gender.

RESUMEN Introducción. La victimización escolar es un problema vigente en el Perú. El enfoque de género en el análisis de sus características permitirá adaptar estrategias de intervención diferenciadas. Objetivo. Determinar los factores de riesgo de victimización según enfoque de género en adolescentes escolares de las ciudades de Huamanga y Sicuani. Diseño. Estudio observacional transversal analítico. Participantes. 1160 alumnos de primero a quinto de secundaria de dos colegios (privado y público) en Huamanga y Sicuani. Intervención. Encuesta validada para identificar víctimas de bullying. Análisis. Descriptivo, chi-cuadrado, OR (IC95%) y análisis multivariado de regresión logística. Se utilizó SPSS v. 15. Principales medidas de resultados. Porcentaje y factores de riesgo de victimización escolar. Resultados. El 37,3% de estudiantes fueron victimizados. En varones, "ha presenciado algún acto violento" 1,72 (1,122 - 2,636); en mujeres "vivir con uno de los padres" 1,682 (1,072 - 2,641); en "Huamanga" 1,767 (1,119 - 2,790); en ambos sexos "tener algún defecto físico" y "padres reaccionan con violencia cuando se portan mal" fueron los factores de riesgo más importantes. Formas de victimización más reportadas en ambos sexos fueron "te pusieron apodos" y "has sido discriminado". Predominó en varones, las formas directas como "te llamaron homosexual" y "has sido golpeado"; y en mujeres las indirectas. Conclusiones. La victimización escolar es problema importante. En mujeres estuvo asociada con características individuales y familiares; y en varones con la seguridad escolar. Las formas directas de victimización se relacionaron con varones.

ABSTRACT We analyzed the presence of the mcr-1 gene in 165 extended-spectrum beta-lactamase-producing enterobacterales (ESBL-PE) obtained during 2017, from blood (40), urine (57), lower respiratory secretions (12) and rectal swabs (56) of patients hospitalized in the Instituto Nacional de Enfermedades Neoplásicas (Peru). Antimicrobial identification and susceptibility were determined by the Phoenix M50 automated system; colistin resistance by Colistin Agar-Spot (CAS); mrc-1 detection by colistin pre-diffusion and inhibition with EDTA test (CPD-E) and by polymerase chain reaction (PCR). We found that from the 165 ESBL-PE, 25 were positive for mcr-1 by the CPD-E method and confirmed by PCR. Colistin resistance was found in 20/165 by using the CAS method. Additionally, they showed resistance to fluoroquinolones and gentamicin, while remaining sensitive to amikacin; two isolates presented metallo-carbapenemases. Obtaining data on resistance to last-line antimicrobials (colistin) is crucial to establish measures for its control.

RESUMEN Se analizó la presencia del gen mcr-1 en 165 enterobacterales productores de betalactamasas de espectro extendido (EP-BLEE) recuperados en 2017 de sangre (40), orina (57), secreciones respiratorias bajas (12) e hisopados rectales (56) de pacientes hospitalizados en el Instituto Nacional de Enfermedades Neoplásicas (Perú). La identificación y la susceptibilidad antimicrobiana se determinaron por el sistema automatizado Phoenix M50; la resistencia a colistina por Colistin Agar-Spot (CAS); la detección de mrc-1 por el método fenotípico de predifusión de colistina e inhibición con EDTA (CPD-E) y por reacción en cadena de la polimerasa (PCR, por sus siglas en inglés). De los 165 EP-BLEE 25 fueron positivos para mcr-1 por el método CPD-E y se confirmó por PCR. Por el método CAS, 20/165 fueron resistentes a colistina. Además, mostraron resistencia a las fluoroquinolonas y a la gentamicina, y permanecieron sensibles a la amikacina; dos aislamientos presentaron metalocarbapenemasas. La obtención de datos sobre la resistencia a antimicrobianos considerados de última línea (colistina) es crucial para establecer medidas para su control.

Introduction. The primary objective of national programs is the suppression of viral replication to undetectable levels and the sustaining of this virological response over time. The objective of the study was to analyze the duration of viral suppression in adult patients with HIV who started antiretroviral treatment in Peru during the period 2004-2006 with follow-up until 2012 using cumulative survival rate analysis. Methods. A retrospective observational study was conducted based on the historical cohort of patients who linked to the antiretroviral treatment national program during the period 2004 to 2006 in Peru with follow-up until December 31st, 2012. Results. Of the 6289 patients, 5142 (81.7%) initiated some treatment scheme and 4530 (72%) reached viral suppression which was sustained by variable time during the observation period. According to the cumulative survival rate 91.1% sustained viral suppression up to 1 year, 84.6% up to 2 years, 80.2% up to 3 years, 77.1% up to 4 years, 74.1% up to 5 years and 70.1% up to 6 years. Conclusions. The Peruvian cohort that began antiretroviral treatment in the initial phase of implementation of the national program, presented comparable results or above other Latin American countries in the duration of sustained viral suppression. The duration of viral suppression is a longitudinal indicator of the performance of national and local programs, which could also help to estimate the time of use of the therapeutic schemes of antiretrovirals and validate adherence.

Introducción. El objetivo primordial de los programas nacionales es la supresión de la replicación viral hasta niveles indetectables y el sostenimiento de esta respuesta virológica a lo largo del tiempo. El objetivo del estudio fue analizar la duración de la supresión viral en pacientes adultos con VIH que iniciaron tratamiento antiretroviral en el Perú durante el periodo 2004-2006 con seguimiento hasta el 2012 mediante análisis de sobrevida acumulada. Métodos. Se realizó un estudio observacional retrospectivo realizado en base a la cohorte histórica de pacientes que accedieron al programa nacional de tratamiento antirretroviral durante el periodo 2004 al 2006 en el Perú y que tuvo seguimiento hasta el 31 de diciembre del 2012. Resultados. Del total de 6289 pacientes, 5142(81,7%) iniciaron algún esquema de tratamiento y 4530(72%) alcanzaron supresión viral, la cual se sostuvo por tiempo variable durante el periodo de observación. Según la tasa de sobrevida acumulada el 91,1% mantuvieron supresión viral hasta 1 año, 84,6% hasta 2 años, 80,2% hasta 3 años, 77,1% hasta 4 años, 74,1% hasta 5 años y 70,1% hasta 6 años. Conclusiones. La cohorte peruana que empezó el tratamiento antirretroviral en la etapa inicial de implementación del programa nacional, presentó duración de supresión viral sostenida con tasas comparables o por encima a otros países latinoamericanos. La duración de la supresión viral es indicador longitudinal del desempeño de los programas nacionales y locales, que puede contribuir a estimar la duración del uso de un solo tipo de esquema de terapia antiretroviral y validar adherencia.
