Resumo A pandemia de COVID-19 no Brasil atingiu níveis alarmantes. Na cidade do Rio de Janeiro, ela encontrou um cenário de desmonte da Atenção Primária à Saúde (APS) em meio à uma crise política, o que teve grande impacto nos territórios de maior vulnerabilidade. O objetivo desse estudo é analisar de que forma as favelas e equipes da APS organizaram-se para desenvolver ações comunitárias, ocupando espaços deixados pela falta de outras políticas públicas. Os resultados fazem parte da pesquisa qualitativa multicêntrica “Estratégias de abordagem dos aspectos subjetivos e sociais na Atenção Primária no contexto da pandemia”, onde foram analisados documentos orientadores públicos e 36 entrevistas em profundidade com trabalhadores e usuários da APS, organizadas em grades interpretativas. Como resultado, observou-se que houve iniciativas cogestoras dos trabalhadores e usuários da APS, a partir do surgimento de coletivos organizados e ativismo social, para o enfrentamento da pandemia, independente das normativas da Secretaria Municipal de Saúde e demais instâncias governamentais. A APS se apresentou como único equipamento público nos territórios de alta vulnerabilidade, onde a violência armada esteve presente mesmo durante a pandemia.

Abstract The COVID-19 pandemic has reached alarming levels in Brazil. In Rio de Janeiro city, it arrived in a scenario in which Primary Health Care (PHC) was being dismantled in the midst of a political crisis, which had major impact on the most vulnerable territories. This study examined how favelas and PHC teams organised community-based action and occupy the vacuum left by the lack of public policies. The results form part of the multi-centre qualitative study “Strategies for approaching subjective and social aspects of Primary Care in the pandemic context”, using public guidance documents and 36 in-depth interviews of PHC workers and users, which were categorised into interpretive grids. Co-management initiatives by PHC workers and users were found to have arisen out of organised groups and social activism, to face the pandemic, independently of regulations from the Municipal Health Department and other government bodies. PHC figured as the only public facility in highly vulnerable territories, where armed violence was ongoing even during the pandemic.

Abstract This study aimed to make a longitudinal analysis of interexaminer calibration reproducibility in diagnosing dental caries in posterior teeth, by examiners without previous experience in epidemiological studies. A group of 11 inexperienced examiners underwent theoretical-practical training and calibration assessments, assisted by a standard examiner. An examiner who did not participate directly in the research selected 5-year-old children with and without caries. The D3 diagnostic threshold was used to evaluate dental caries, based on the World Health Organization (WHO) criteria. The initial calibration (baseline) was performed after the theoretical-practical training session, and consisted of examining 20 children; the second calibration occurred three months later, and involved evaluating another 18 children. The interexaminer agreement was obtained by kappa statistics, and by overall percentage agreement. The paired t-test was applied to compare the values for kappa means and overall percentage agreement between the time points studied. At baseline, the values for kappa (> 0.81) and overall percentage agreement (> 95.63%) were considered high. At the 3-month calibration assessment, all the examiners showed some decrease in both kappa (p < 0.0001) and overall percentage agreement (p = 0.0102). The calibration process currently proposed by the WHO is effective. However, reproducibility was not maintained over time for inexperienced examiners evaluating the posterior teeth of 5-year-old children, under epidemiological conditions.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

ABSTRACT Introduction: The mandibular buccal bifurcation cyst is a rare inflammatory odontogenic cyst that typically develops at the buccal region of the first or second permanent mandibular molars of children aged 6 to 15 years. The tooth involved is vital and shows an increased probing depth on the buccal surface. Radiographically, it is characterized by a unilocular radiolucent area, well circumscribed on the buccal osseous cortical, that it is difficult to visualize on periapical radiography. The diagnosis is primarily clinical and radiographic, and the treatment is usually the cystic enucleation without extraction of the tooth. Objective: To report a case of spontaneous involution of a unilateral mandibular buccal bifurcation cyst. Case presentation: This report describes the clinical and radiographic characteristics of an unusual case of unilateral buccal bifurcation cyst in a 6-year-old patient, presenting as a slight expansion in the buccal region of the first permanent mandibular molar. The treatment of choice was the clinical and radiographic follow-up due to the spontaneous involution of the cyst. There were no recurrences during follow-up and the tooth remained vital and erupted normally. Conclusions: The possibility of spontaneous involution is a characteristic that must always be observed, in order to avoid unnecessary surgical procedures.

RESUMEN Introducción: El quiste de bifurcación bucal mandibular es un quiste odontogénico inflamatorio poco frecuente que, en general, se desarrolla en la región bucal de primeros y segundos molares mandibulares permanentes en niños de 6-15 años. El diente involucrado es vital y muestra una mayor profundidad de sondeo en la superficie bucal. Radiográficamente, se caracteriza por un área radiolúcida unilocular, bien circunscrita en la cortical ósea bucal, que es difícil de visualizar en la radiografía periapical. El diagnóstico es principalmente clínico y radiográfico y el tratamiento suele ser la enucleación quística sin extracción del diente. Objetivo: Informar un caso de involución espontánea de un quiste de bifurcación vestibular mandibular unilateral. Presentación del caso: Caso inusual de quiste de bifurcación bucal unilateral en un paciente de 6 años, que se presenta como una ligera expansión de la región vestibular del primer molar mandibular permanente. El tratamiento de elección fue el seguimiento clínico y radiográfico debido a la involución espontánea del quiste. No hubo recurrencias durante el seguimiento y el diente permaneció vital y brotó normalmente. Conclusiones: La posibilidad de involución espontánea es una característica que siempre se debe observar para evitar procedimientos quirúrgicos innecesarios.

Resumo Introdução O desenvolvimento do complexo craniofacial resulta da interação de fatores genéticos e ambientais. Dentre as ocorrências que podem afetar esse desenvolvimento, estão as odontológicas. A cárie dentária e a má oclusão são de etiologia multifatorial e atingem grande parte da população, podendo impactar a qualidade de vida. Objetivo O objetivo deste estudo foi avaliar o impacto de fatores clínicos, funcionais e sociais na qualidade de vida relacionada a saúde bucal de crianças e suas famílias. Material e método A amostra consistiu de 753 crianças matriculadas nas escolas públicas da cidade de Araraquara-SP. Foram realizados exames bucais para avaliar a ocorrência cárie dentária (ceo-d e CPO-D) e a má oclusão (Foster, Hamilton) e aplicado um questionário socioeconômico aos pais, além do questionário para avaliação da qualidade de vida relacionada a saúde bucal (ECOHIS). Os dados foram avaliados por meio de análise bivariada e, em seguida, pelo modelo de regressão logística múltipla, considerando o nível de significância de 5%. Resultado A condição social, além das variáveis clínicas de cárie dentaria e trauma dental, apresentou associação com o impacto negativo na qualidade de vida relacionada a saúde bucal nas três sessões do questionário: criança, pais e geral, respectivamente. Conclusão A experiência da cárie dentária, o nível socioeconômico e o trauma dental foram variáveis preditoras para o impacto negativo na qualidade de vida relacionada a saúde bucal de crianças e suas famílias. Os hábitos estão mais relacionados ao índice ceo-d e ao nível socioeconômico do que com a má oclusão.

Abstract Introduction The development of the craniofacial complex results from the interaction of genetic and environmental factors. Dental caries and malocclusion have a multifactorial etiology and affect a large part of the population, which may impact the quality of life. Objective The aim of this study was to evaluate the impact of clinical, functional and social factors on oral health-related quality of life of children and their families. Material and method The sample consisted of 753 children enrolled in public schools in the city of Araraquara-SP. Oral examinations were performed to assess dental caries (dmft and DMFT) and malocclusion (Foster, Hamilton) and a socioeconomic questionnaire was administered to the parents, in addition to the quality assessment questionnaire. health-related life expectancy (ECOHIS). Data were evaluated by bivariate analysis and then by multiple logistic regression model, considering the significance level of 5%. Result Social condition, besides the clinical variables dental caries and dental trauma were associated with the negative impact on oral health-related quality of life in the three questionnaire sessions, child, parent and general respectively. Conclusion The experience of dental caries, socioeconomic status and dental trauma were predictive variables for the negative impact on oral health-related quality of life of children and their families. Habits seem to be more related to the dmft index and socioeconomic level than to malocclusion.

RESUMO Objetivo: Avaliar o impacto do perfil facial nos domínios da qualidade de vida relacionada à saúde bucal (QVRSB) de jovens adultos. Métodos: Estudo observacional transversal realizado com 205 jovens adultos, com idade média de 23,1 anos. Os indivíduos responderam questões sobre QVRSB (OHIP-14) e autoestima (Global Negative Self-Evaluation). O Componente de Saúde Bucal (DHC) do Índice de Necessidade de Tratamento Ortodôntico (IOTN) avaliou as necessidades normativas de tratamento ortodôntico e definiu clinicamente a má oclusão dentária. O perfil facial foi analisado por meio de fotografias e dicotomizado em dois níveis: perfil facial normal (reto) e perfil facial alterado (convexo ou côncavo). Um pesquisador calibrado realizou os exames clínicos. A associação entre as variáveis independentes e o desfecho (QVRSB) foi determinada pela análise de regressão linear múltipla hierarquizada para cada domínio do OHIP-14. Considerando a variável de interesse (perfil facial), o domínio incapacidade psicológica foi o item mais afetado. Resultados: Indivíduos com perfil facial alterado tiveram 2,47 (1,04-5,85) vezes mais chances de relatar impacto na incapacidade psicológica do que aqueles com perfil normal (p > 0,05). A associação foi modulada pela má oclusão dentária e a autoestima. Conclusões: O perfil facial alterado (convexo ou côncavo) apresentou impacto negativo nos aspectos psicológicos da qualidade de vida relacionada à saúde bucal de adultos jovens.

ABSTRACT Objective: To assess the impact of facial profile on young adults’ oral health-related quality of life (OHRQoL) item levels. Methods: A cross-sectional study was carried out with a population-based sample of 205 young adults, with a mean age of 23.1 years. The individuals answered questions about OHRQoL (OHIP-14) and self-esteem (Global Negative Self-Evaluation). The Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN) was used to evaluate normative orthodontic treatment needs and define dental malocclusion clinically. Facial profile was analyzed using photographs and dichotomized into two levels: normal (straight) and altered facial profile (convex or concave). A calibrated researcher performed the clinical examination. Association between the independent variables and the outcome (OHRQoL) was established by hierarchical multiple linear regression analysis for each item level. Considering the variable of interest (facial profile), the psychological incapacity domain was the most affected item. Results: Individuals with changed facial profile had 2.47 (1.04-5.85) times higher chances of reporting impacts on psychological incapacity than those with a normal profile (p> 0.05). The association was modulated by dental malocclusion and self-esteem. Conclusions: The convex and concave facial profile showed a negative impact on the psychological aspects of young adults’ quality of life.

Resumo Introdução A dor orofacial é um sintoma crítico da DTM que pode influenciar a capacidade física e social. Objetivo Avaliar a associação dos sintomas das desordens têmporomandibulares (DTM) com as relações afetivas e variáveis demográficas em adultos jovens. Material e método Estudo transversal envolvendo 395 adultos jovens foi realizado. Os critérios diagnósticos para DTM, ansiedade e depressão foram coletados a partir de questionários. O Componente de Saúde Bucal do Índice de Necessidade de Tratamento Ortodôntico mensurou a necessidade de tratamento ortodôntico. Os questionários também continham questões relacionadas ao tratamento ortodôntico anterior. Modelos de regressão logística foram ajustados, estimando odds ratio bruto com os intervalos de confiança de 95%. As variáveis com p<0.20 nas análises foram avaliadas em modelo de regressão logística múltipla, permanecendo as variáveis com p≤0.10. Resultado Não houve associação significativa dos sintomas de DTM com sexo, idade, uso de medicamentos para dor, tratamento ortodôntico prévio, necessidade de tratamento ortodôntico, ansiedade e depressão (p>0.05). Indivíduos sem relacionamento afetivo têm 1.78 (IC95%:0.99-3.17) vezes mais chance de relatar sintomas de DTM. Conclusão Os relacionamentos afetivos mostraram associação com os sintomas de DTM em adultos jovens.

Abstract Introduction Orofacial pain is a critical TMD symptom that can influence physical and social capacity. Objective To evaluate the association of temporomandibular disorders (TMD) symptoms with affective relationships and demographic variables in young adults. Material and method A cross-sectional study involving 395 young adults was developed. Diagnostic Criteria for TMD, anxiety, and depression were collected from questionnaires. The Dental Health Component of the Index of Orthodontic Treatment Need measures the orthodontic treatment need. Questionnaires also contained questions related to the previous orthodontic treatment. Logistic regression models were adjusted, estimating crude odds ratio with the 95% confidence intervals. The variables with p<0.20 in the analyses were assessed in a multiple logistic regression model, remaining with p≤0.10. Result There was no significant association of TMD symptoms with sex, age, medication use for pain, previous orthodontic treatment, orthodontic treatment need, anxiety, and depression (p>0.05). Individuals without an affective relationship are 1.78 (95%CI: 0.99-3.17) times more likely to report TMD symptoms. Conclusion Affective relationships showed an association with TMD symptoms in young adults.

ABSTRACT Objective: To analyze the difference in the on-line searches for terms related to hand hygiene during the COVID-19 pandemic in developed and middle-income countries. Material and Methods: The cross-sectional study analyzed the digital data through the Google Trends website to obtain the variation of the relative search volume (RSV) through the terms "alcohol gel" and "handwashing." According to socio-economic development, the countries were divided into two groups: countries from different continents and hemispheres, with more than 15 million inhabitants, with more than 50% of the population with access to the Internet network and over 1,000 confirmed cases of infected with COVID-19. The paired t-test was applied to compare the means. The significance value adopted was p<0.010. Results: The searches related to the term "hand washing" were more significant when compared to the term "alcohol gel," and the term "alcohol gel" presented a higher average volume of research in developed countries (p<0.010). The developed countries had a higher average relative volume of research than middle-income countries (p<0.010). Developed countries sought more for the term "alcohol gel," and the term "hand washing" showed no difference in the volume of research about the country's socio-economic aspect. Conclusion: Developed countries have a higher volume of search for hand hygiene terms. The middle-income countries must create proposals for raising awareness outside the on-line environment so that this information reaches the entire population during the pandemic.

Abstract Next-generation sequencing (NGS) has altered clinical genetic testing by widening the access to molecular diagnosis of genetically determined rare diseases. However, physicians may face difficulties selecting the best diagnostic approach. Our goal is to estimate the rate of possible molecular diagnoses missed by different targeted gene panels using data from a cohort of patients with rare genetic diseases diagnosed with exome sequencing (ES). For this purpose, we simulated a comparison between different targeted gene panels and ES: the list of genes harboring clinically relevant variants from 158 patients was used to estimate the theoretical rate of diagnoses missed by NGS panels from 53 different NGS panels from eight different laboratories. Panels presented a mean rate of missed diagnoses of 64% (range 14%-100%) compared to ES, representing an average predicted sensitivity of 36%. Metabolic abnormalities represented the group with highest mean of missed diagnoses (86%), while seizure represented the group with lowest mean (46%). Focused gene panels are restricted in covering select sets of genes implicated in specific diseases and they may miss molecular diagnoses of rare diseases compared to ES. However, their role in genetic diagnosis remains important especially for well-known genetic diseases with established genetic locus heterogeneity.

RESUMO Objetivo: construir um banco de termos relevantes para a prática de enfermagem no contexto de infecções por COVID-19. Métodos: estudo metodológico, de natureza documental, realizado no período de março a junho de 2020 no Centro CIPE®/Universidade Federal da Paraíba, considerado referência para pesquisa e disseminação da CIPE® no Brasil. Os achados foram coletados em bases de dados e analisados utilizando a ferramenta PorOnto, a técnica de consenso e o mapeamento dos termos com a CIPE® versão 2019/2020. Resultados: identificaram-se 1.134 termos relevantes na literatura, que, quando submetidos à técnica de mapeamento com os termos do Modelo de Sete Eixos da CIPE®, resultando em 531 termos constantes e 603 termos não constantes nessa Classificação. Considerações Finais: comprova-se que os termos da prática de enfermagem, mesmo em um contexto específico, estão presentes na literatura e têm representatividade na CIPE®, o que possibilitará o desenvolvimento futuro de subconjunto terminológico no contexto de infecções por coronavírus.

ABSTRACT Objective: to build a term database relevant to nursing practice in the context of COVID-19 infections. Final considerations: It is proven that nursing practice terms, even in a specific context, are present in the literature and are representative in ICNP®, which will enable the future development of a terminological subset in the context of coronavirus infections.

RESUMO Objetivo: O objetivo do presente estudo experimental in vitro foi avaliar se sessões de clareamento dental antes do tratamento ortodôntico alteram a resistência da união ao cisalhamento (RUC) de braquetes estéticos dos tipos monocristalino e policristalino. Métodos: Sessenta (60) dentes incisivos bovinos foram utilizados e divididos aleatoriamente nos seguintes seis grupos (n = 10): SCP (sem clareamento/braquetes policristalinos); SCM (sem clareamento/braquetes monocristalinos); 1CP (uma sessão de clareamento/braquetes policristalinos); 1CM (uma sessão de clareamento/braquetes monocristalinos); 3CP (três sessões de clareamento/braquetes policristalinos); e 3CM (três sessões de clareamento/braquetes monocristalinos). Os braquetes foram colados sete dias após as sessões de clareamento. As amostras foram submetidas ao teste de RUC em máquina universal de ensaios (Instron modelo 4411) na velocidade de 1 mm/min. Foi realizada análise de variância de dois fatores (ANOVA) e teste de Tukey, com nível de significância de 5%. Após o ensaio mecânico, as amostras foram avaliadas para determinação do índice de adesivo remanescente (ARI), de acordo com o método proposto por Artun e Bergland, com o auxílio de uma lupa com lente de aumento de 25x. Resultados: Os valores de RUC foram significativamente maiores para os braquetes monocristalinos do que para os policristalinos (p< 0,0001) e significativamente maiores com três sessões de clareamento do que sem clareamento (p< 0,0436). O ARI mostrou predomínio de falhas entre o braquete e a resina para todos os grupos (escore 3). Conclusão: Três sessões de clareamento dental têm efeito positivo na RUC. Os braquetes monocristalinos apresentaram valores de RUC superiores aos do tipo policristalino.

ABSTRACT Objective: The aim of this experimental in vitro study was to evaluate whether dental bleaching performed before orthodontic treatment change the shear bond strength (SBS) of monocrystalline and polycrystalline esthetic brackets. Methods: Sixty (60) bovine incisors teeth were used and randomly divided into the following six groups (n=10): SCP (without bleaching/polycrystalline brackets); SCM (without bleaching/monocrystalline brackets); 1CP (one bleaching session/polycrystalline brackets); 1CM (one bleaching session/monocrystalline brackets); 3CP (three bleaching sessions/polycrystalline brackets); and 3CM (three bleaching sessions/monocrystalline brackets). The brackets were bonded seven days after the bleaching sessions. The samples were submitted to the SBS test in a universal testing machine (Instron model 4411) at 1 mm/min crosshead speed. The two-way analysis of variance (ANOVA) and the Tukey tests were performed at a 5% level of significance. After the mechanical test, samples were evaluated to determine the adhesive remnant index (ARI). Results: The SBS values were significantly higher for the monocrystalline brackets, when compared with the polycrystalline type (p< 0.0001), and significantly higher with three bleaching sessions than without bleaching (p< 0.0436). The ARI showed predominance of failures between the bracket and resin for all the groups (score 3). Conclusion: Three dental bleaching sessions increased the SBS values. Monocrystalline brackets showed higher SBS values than the polycrystalline type.

Resumo Objetivo Avaliar a prevalência e fatores associados à polifarmácia em pessoas idosas de uma área rural. Método Estudo transversal, quantitativo, realizado com pessoas idosas residentes de uma área rural no município de Ponta Grossa, Paraná, Brasil (n=80). Para a coleta de dados utilizou-se um questionário estruturado. Considerou-se como variável dependente a polifarmácia e, como variáveis independentes as características sociodemográficas, de estilo de vida, presença de doenças crônicas, autopercepção em saúde, quedas no último ano, sugestivo de comprometimento cognitivo e nível de fragilidade. Realizou-se teste de qui-quadrado e regressão logística. Resultados A prevalência de polifarmácia foi de 40,0%. A maioria era do sexo feminino (52,5%), com idade entre 60 e 74 anos (61,2%), cor branca (76,2%), casados (62,5%), e ensino fundamental incompleto (66,2%). A polifarmácia esteve associada ao nível de fragilidade (OR=3,73;IC95%=1,09-12,74; p=0,036), indicativo para sarcopenia (OR=5,02; IC95%=1,39-18,13; p=0,014) e diabetes (OR=9,20; IC95%=2,37-36,05; p=0,001). Conclusão Verificou-se alta prevalência de polifarmácia nas pessoas idosas residentes na zona rural, sendo essa condição atrelada a fatores inerentes ao maior grau de fragilidade, indicativo de sarcopenia e diabetes.

Abstract Objective Evaluate the prevalence and factors associated to polypharmacy in older adults from a rural area. Method Cross-sectional, quantitative study carried out with older adults residents from a rural area in Ponta Grossa city, Paraná, Brazil (n = 80). For data collection, a structured questionnaire was used. Polypharmacy was considered as a dependent variable and sociodemographic, lifestyle characteristics, presence of chronic diseases, self-perceived health, falls in the last year, suggestive of cognitive impairment and level of fragility were considered as independent variables. Chi-Square and Logistic Regression tests were performed. Results The prevalence of polypharmacy was 40.0%. Most were female (52.5%), aged between 60 and 74 years old (61.2%), white (76.2%), married (62.5%), and incomplete Elementary School (66,2%). Polypharmacy was associated to level of frailty (OR=3.73; CI95%=1.09-12.74; p=0.036), indicative for sarcopenia (OR=5.02; CI95%=1.39-18.13; p=0.014) and diabetes (OR=9.20; CI95%=2.37-36.05; p=0.001). Conclusion There was a high prevalence of polypharmacy in rural older adults, and this condition is linked to factors inherent to a greater degree of frailty, indicative of sarcopenia and diabetics.

RESUMO Objetivo: Avaliar a influência do traumatismo dentário na qualidade de vida relacionada à saúde bucal (QVRSB) de crianças e suas famílias. Métodos: 571 crianças de 5 anos de idade foram aleatoriamente selecionadas de escolas públicas. O trauma foi avaliado clinicamente de acordo com a classificação de Andreasen. A experiência de cárie na região anterior e a presença de overjet foram determinadas com base nos critérios da Organização Mundial da Saúde. A versão brasileira do questionário ECOHIS (Escala de Impacto na Saúde Oral na Primeira Infância) avaliou a QVRSB e foi respondida pelos pais; além disso, foram avaliados aspectos socioeconômicos. Realizou-se uma regressão logística simples, bem como as razões de chances brutas com os respectivos intervalos de confiança de 95%. As variáveis com p <0,20 foram testadas nos modelos de regressão logística múltipla, e aquelas com p≤0,05 permaneceram no modelo. Resultados: A renda mostrou uma magnitude de associação de 1,56 e 2,70 com a QVRSB das crianças e famílias, respectivamente. A variável avulsão apresentou chance 9,65 e 8,25 vezes maior de influenciar a QVRSB de crianças e famílias, respectivamente. A experiência de cárie mostrou chance 3,80 e 2,42 vezes maior de influenciar a QVRSB de crianças e famílias, respectivamente. Conclusões: O trauma dental não influenciou negativamente a QVRSB das crianças e suas famílias. Entretanto, especificamente a avulsão, e a experiência de cárie nos dentes anteriores em famílias de baixa renda estiveram associadas a uma percepção negativa da QVRSB.

ABSTRACT Objective: To evaluate the influence of dental trauma on oral health-related quality of life (OHRQoL) of children and their families. Methods: A total of 571 children aged five years were randomly selected at public schools. Trauma was clinically evaluated in accordance with the Andreasen classification. Caries experience in the anterior region and increased overjet were determined according to the World Health Organization criteria. The Early Childhood Oral Health Impact Scale (ECOHIS) was answered by the parents and used to evaluate OHRQoL. In addition, this questionnaire has aspects related to socioeconomic status. Simple logistic regression was performed, and the raw Odds Ratios with the respective 95% confidence intervals (95%CI) were estimated. The variables with p<0.20 were tested in multiple logistic regression models, and those with p≤0.05 remained in the model and the adjusted odds ratio with respective 95%CI was estimated. Results: Income showed a magnitude of association of 1.56 and 2.70 with the OHRQoL of children and families, respectively. The avulsion variable showed 9.65- and 8.25-times greater chance of influencing the OHRQoL of children and families, respectively. The experience of caries showed 3.80- and 2.42-times greater chance of influencing the OHRQoL of children and families, respectively. Conclusions: Dental trauma did not influence OHRQoL of children and their families negatively. However, avulsion and caries experience in low-income families was associated with a negative perception of OHRQoL.

Resumo Introdução Na Atenção Primária à Saúde, o acesso pode ser compreendido como o fato de oportunizar diversas alternativas para adentrar aos serviços de saúde. Objetivo Avaliar o acesso aos serviços de atenção primária à saúde do Sistema Único de Saúde/SUS, identificando a presença de um conjunto proposto de elementos importantes na rede de serviços do município estudado percebidos e relatados pelos usuários. Método Pesquisa de método dedutivo realizada por meio de um estudo quantitativo de base populacional, com levantamento de dados através de um inquérito realizado em uma amostra de usuários do SUS em três Gerências Distritais da cidade de Porto Alegre/RS, sobre o acesso aos serviços de Atenção Primária no SUS. Resultados A brevidade para ser consultado após agendamento, o atendimento por demanda espontânea e a adscrição do domicílio foram os elementos mais importantes para caracterizar um adequado/bom acesso. Entretanto, apenas 18,8% do conjunto de elementos que caracterizam um bom acesso foi relatado pelos referidos usuários. Conclusão Com o desenvolvimento deste estudo, foi possível visualizar de forma mais adequada as características que devem ser mantidas, priorizadas e/ou melhoradas no que se refere ao acesso aos serviços de saúde de uma cidade de grande porte.

Abstract Background In Primary Health Care, access can be understood as enabling alternatives to obtain health services. Objective Analyze access to primary health care services in the Unified Health System (Acronym in Portuguese is SUS), to identify a proposed set of important characteristics in the service network of the city studied through users report. Method Deductive research, performed through a population-based quantitative study, with data collection through a survey conducted in a sample of SUS users, in three administrative districts in Porto Alegre/RS on access to primary care services in the SUS. Results The waiting time after scheduling, service by spontaneous demand and the domicile ascription were the most important elements to characterize good access. However, only 18.8% of the elements that characterize good access was reported by the users. Conclusion This study allowed a better visualization of the characteristics that should be maintained, prioritized and improved regarding access to health services in a big city.
