ABSTRACT Objective: This study aims to investigate the ability of the six-minute walk distance (6MWD) as a prognostic marker for midterm clinical outcomes three months after coronary artery bypass grafting (CABG), to identify possible predictors of fall in 6MWD in the early postoperative period, and to establish the percentage fall in early postoperative 6MWD, considering the preoperative baseline as 100%. Methods: A prospective cohort of patients undergoing elective CABG were included. The percentage fall in 6MWD was assessed by the difference between preoperative and postoperative day (POD) five. Clinical outcomes were evaluated three months after hospital discharge. Results: There was a significant decrease in 6MWD on POD5 compared with preoperative baseline values (percentage fall of 32.5±16.5%, P<0.0001). Linear regression analysis showed an independent association of the percentage fall of 6MWD with cardiopulmonary bypass (CPB) and preoperative inspiratory muscle strength. Receiver operating characteristic curve analysis revealed that the best cutoff value of percentage fall in 6MWD to predict poorer clinical outcomes at three months was 34.6% (area under the curve = 0.82, sensitivity = 78.95%, specificity = 76.19%, P=0.0001). Conclusion: This study indicates that a cutoff value of 34.6% in percentage fall of 6MWD on POD5 was able to predict poorer clinical outcomes at three months of follow-up after CABG. Use of CPB and preoperative inspiratory muscle strength were independent predictors of percentage fall of 6MWD in the postoperative period. These findings further support the clinical application of 6MWD and propose an inpatient preventive strategy to guide clinical management over time.

RESUMO Alternativas que possibilitem a melhoria da retenção de água no solo devem ser buscadas visando maior aproveitamento de água por plantas cultivadas. Objetivou-se avaliar o efeito de doses de resina vegetal natural e estratégias de irrigação nas culturas do algodão (ciclo intermediário) e rabanete (ciclo curto). Foram desenvolvidos dois experimentos, em fatorial 5 x 5, em delineamento inteiramente casualizado, com três repetições e cinco doses de resina vegetal (0; 0,67; 1,33; 2,00; e 2,67 g planta-1), cinco tensões de água no solo (15; 30; 45; 60; e 75 kPa) para o algodoeiro e cinco reposições de água (40; 60; 80; 100; e 120 % da ETc) para o rabanete. Aos 93 dias após a semeadura (DAS), foram avaliados a altura, diâmetro de caule e número de folhas do algodoeiro. Aos 30 DAS, foram medidos a altura, diâmetro de raiz e número de folhas do rabanete, bem como quantificada a matéria seca da parte aérea e das raízes de ambas as culturas. A resina vegetal reduz a suscetibilidade da cultura ao déficit hídrico, com a dose máxima de 2,7 g planta-1 sendo recomendada para se obter melhor desenvolvimento das plantas. A resina aumenta a eficiência do uso de água nas culturas de algodão e rabanete, correspondendo a até 0,68 g L-1 e 69,6 g L-1, respectivamente.

ABSTRACT The search for alternatives that enable the improvement of the soil water retention capacity should focus on a better water use by cultivated plants. This study aimed to evaluate the effect of natural tree resin rates and irrigation levels on the cotton (intermediate cycle) and wild radish (short cycle) crops. Two experiments were conducted in a completely randomized design, in a 5 × 5 factorial arrangement, with three replications and five tree resin rates (0, 0.67, 1.33, 2.00 and 2.67 g plant-1), five soil water tensions (15, 30, 45, 60 and 75 kPa) for cotton plants and five irrigation water levels (40, 60, 80, 100 and 120 % of the ETc) for wild radish plants. The plant height, stem diameter and number of leaves were evaluated at 93 days after sowing (DAS) for the cotton plants; while the plant height, root diameter and number of leaves were evaluated at 30 DAS for the wild radish plants, as well as quantified the shoot and root dry weights of both plant species. The application of tree resin reduces the susceptibility of the crop to water deficit, being 2.7 g plant-1 the highest recommended rate to improve the plant development. The use of tree resin increases the water-use efficiency in cotton and wild radish crops in up to 0.68 g L-1 and 69.6 g L-1, respectively.

BACKGROUND The high demand for adequate material for the gold standard reverse transcription real-time polymerase chain reaction (RT-qPCR)-based diagnosis imposed by the Coronavirus disease 2019 (COVID-19) pandemic, combined with the inherent contamination risks for healthcare workers during nasopharyngeal swab (NP) sample collection and the discomfort it causes patients, brought the need to identify alternative specimens suitable for the diagnosis of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). OBJECTIVES The aim of this work was to compare saliva and gingival fluid swabs to NP swabs as specimens for RT-qPCR-based SARS-CoV-2 diagnosis. METHODS We compared gingival fluid swabs (n = 158) and saliva (n = 207) to the rayon-tipped NP swabs obtained from mild-symptomatic and asymptomatic subjects as specimens for RT-qPCR for SARS-CoV-2 detection. FINDINGS When compared to NP swabs, gingival fluid swabs had a concordance rate of 15.4% among positive samples, zero among inconclusive, and 100% among negative ones. For saliva samples, the concordance rate was 67.6% among positive samples, 42.9% among inconclusive, and 96.8% among negative ones. However, the concordance rate between saliva and NP swabs was higher (96.9%) within samples with lower cycle threshold (Ct) values (Ct > 10 ≤ 25). MAIN CONCLUSIONS Our data suggests that whereas gingival fluid swabs are not substitutes for NP swabs, saliva might be considered whenever NP swabs are not available or recommended.

RESUMO Objetivo: correlacionar dados clínicos e epidemiológicos com a análise patológica dos explantes hepáticos dos pacientes submetidos ao transplante hepático por hetapocarcinoma no HC da UNICAMP e verificar se os scores MELD e MELD-Na apresentam diferenças estatísticas para predizer pior prognóstico pós-transplante. Métodos: considerando os transplantes hepáticos ocorridos entre maio/2010 e novembro/2017, após excluir 38, 87 pacientes foram incluídos, analisando-se dados clínicos e laboratoriais desses pacientes para verificar se há independência entre esses e desfecho Invasão Microvascular (IMV). Posteriormente, realizou-se o cálculo do MELD e MELD-Na, a análise descritiva dos dados clínicos e laboratoriais e, por fim, curvas ROC para avaliar a associação entre esses parâmetros laboratoriais e o desfecho óbito nestes pacientes. Resultados: a maior parte dos pacientes foi do sexo masculino (78,30%), com idade, em média, de 58,53 anos. A maior parte das hepatopatias foi causada pelo VHC (53,26%). Não foi encontrado preditores para o desfecho IMV entre os parâmetros laboratoriais. As curvas ROC para o desfecho óbito identificaram o score MELD como ponto de corte de maior sensibilidade (90,91%) e especificidade (37,50%) simultâneas com o valor de 10 pontos, ao passo que no MELD-Na o ponto de corte foi 7 pontos, com sensibilidade de 90,91% e especificidade de 33,33%, sendo ambas as escalas significativas. Conclusões: não foram encontrados preditores para IMV entre critérios clínicos, laboratoriais e epidemiológicos. O score MELD-Na é mais sensível que o MELD para predizer mortalidade nos pacientes submetidos a transplante hepático.

ABSTRACT Objective: to correlate clinical and epidemiological data with the pathological analysis of liver explants from patients undergoing liver transplantation for hetapocarcinoma in the UNICAMP HC and to verify whether the MELD and MELD-Na scores are reliable factors to predict a worse post-transplant prognosis. Methods: we studied liver transplants carried out between May 2010 and November 2017. After excluding 38 patients, we included 87, analyzing clinical and laboratory data for correlation with the outcome Microvascular Invasion (MVI). Subsequently, we computed the MELD and MELD-Na scores and performed a descriptive analysis of clinical and laboratory data and, finally, calculated ROC curves to assess the association between these laboratory parameters and mortality in these patients. Results: most patients were male (78.30%), with an average age of 58.53 years. Most liver diseases were caused by HCV (53.26%). We found no predictors for MVI among the laboratory parameters. The ROC curves for death identified the MELD score as the cutoff point with the highest combined sensitivity (90.91%) and specificity (37.50%), with a value of 10 points, whereas in the MELD-Na the cutoff point was 7 points, with a sensitivity of 90.91% and a specificity of 33.33%, both scores being significant. Conclusions: there were no reliable predictors of MVI between clinical, laboratory, and epidemiological variables. The MELD-Na score is more sensitive than the MELD one for predicting mortality in patients undergoing liver transplantation.

ABSTRACT Oebalus poecilus is one of the most important pests of irrigated rice in Brazil. However, the impact of this species on the cultivar IRGA 424 RI, which is the most used, is unknown. Hence, the objective of this work is to evaluate the damage caused by O. poecilus on cv IRGA 424 RI. Panicles of this cultivar were infested for seven days at the R5 stage, using a randomized block design, considering the following factors sex, insect reproductive stage and insect density (number/panicle). In order to analyze the effect of these factors, the qualitative and quantitative damage caused by stink bugs to panicles were evaluated. An interaction was found between the sex and developmental stage for quantitative variables, in which reproductive females were responsible for increasing the damage in comparison to pre-reproductive females and the males, while the greatest qualitative damage was caused by reproductive insects regardless of sex and by females, despite the reproductive stage. Density of only one infesting insect has already increased the qualitative damage, while significant losses on the weight of the grain were verified with the infestation of two insects, demonstrating that IRGA 424 RI is susceptible to the attack of these stink bugs.

Abstract Ti-6Al-4V and TiAl-based alloys are widely used for fabricating the implantable orthopedic devices and automotive components, respectively. Ti6Si2B-based alloys are attractive for use in orthopedic components because their higher hardness, superior biocompatibility and corrosion resistance in simulated body fluid than Ti and Ti-6Al-4V alloy. Limited information on Ti6Si2B stability in Co-dopped 67Ti-22Si-11B alloys are available in literature. This work presents the effect of cobalt doping and milling time on microstructure and Vickers microhardness of 65Ti-2Co-22Si-11B and 61Ti-6Co-22Si-11B (at-%) alloys produced by spark plasma sintering at 1100 oC for 12min using 20MPa. Samples were characterized by X ray diffraction, scanning electron microscopy, energy dispersive spectrometry, laser particle size analysis, and Vickers microhardness. Sintered alloys with 2 and 6at-%Co indicated the major presence of Ti6Si2B and Ti5Si3 dissolving up to 2.7 and 4.2 at-%Co, respectively, besides the minor precipitates of CoTi2 (4.4-16.7at-%Si) and CoTi (4.6-4.7at-%Si). Vickers microhardness of the sintered 65Ti-2Co-22Si-11B and 61Ti-6Co-22Si-11B alloys were in the range of 950-1050 and 1050-1150HV, respectively. Although the increase from 2 to 6at-%Co has reduced the Ti6Si2B stability, the Co-rich phases increased their hardness values up to 1150HV (11.3GPa), which are superior than those of commercial Ti alloys used for joint orthopedic components and automotive rotating parts.

Resumo Cochliomyia hominivorax (Coquerel, 1858), a mosca-da-bicheira, causa miíase primária em animais silvestres e domésticos em regiões tropicais e subtropicais do Brasil. Embora esta espécie seja considerada de ocorrência em todo o país, informações organizadas sobre sua distribuição com base em registros não estavam disponíveis até o momento. Este artigo teve como objetivo fornecer uma revisão abrangente dos dados históricos e atuais publicados sobre o registro de estágios imaturos (miíase) e adultos de C. hominivorax, no Brasil. De 174 artigos encontrados, 141 relataram casos de miíase em bovinos (146 relatos), humanos (68 relatos) e outros mamíferos hospedeiros (40 relatos) e capturas de adultos de C. hominivorax foram registradas em 33 artigos. C. hominivorax encontra-se amplamente distribuída no Brasil, tendo sido registrada em 208 municípios brasileiros em todos os principais biomas do país.

Abstract Cochliomyia hominivorax (Coquerel, 1858), the New World screwworm, causes primary myiasis in wild and domestic animals in tropical and subtropical regions of Brazil. Although this species is considered to occur throughout the country, organized information about its recorded distribution has not been available until now. This article aimed to provide a comprehensive review of the historical and current data published on both immature (myiasis) and adult stages of C. hominivorax in Brazil. A total of 174 articles were found; of these, 141 articles reported myiasis cases in cattle (146 records), humans (68 records), and other mammalian hosts (40 records), and captures of adult flies were reported in 33 articles. C. hominivorax is widespread in Brazil, having been recorded in 208 municipalities in all major biomes of the country.

Abstract Introduction: Coronary artery bypass grafting (CABG) is the most frequently performed heart surgery in Brazil. Recent international guidelines recommend that national societies establish a database on the practice and results of CABG. In anticipation of the recommendation, the BYPASS Registry was introduced in 2015. Objective: To analyze the profile, risk factors and outcomes of patients undergoing CABG in Brazil, as well as to examine the predominant surgical strategy, based on the data included in the BYPASS Registry. Methods: A cross-sectional study of 2292 patients undergoing CABG surgery and cataloged in the BYPASS Registry up to November 2018. Demographic data, clinical presentation, operative variables, and postoperative hospital outcomes were analyzed. Results: Patients referred to CABG in Brazil are predominantly male (71%), with prior myocardial infarction in 41.1% of cases, diabetes in 42.5%, and ejection fraction lower than 40% in 9.7%. The Heart Team indicated surgery in 32.9% of the cases. Most of the patients underwent cardiopulmonary bypass (87%), and cardioplegia was the strategy of myocardial protection chosen in 95.2% of the cases. The left internal thoracic artery was used as a graft in 91% of the cases; the right internal thoracic artery, in 5.6%; and the radial artery in 1.1%. The saphenous vein graft was used in 84.1% of the patients, being the only graft employed in 7.7% of the patients. The median number of coronary vessels treated was 3. Operative mortality was 2.8%, and the incidence of cerebrovascular accident was 1.2%. Conclusion: CABG data in Brazil provided by the BYPASS Registry analysis are representative of our national reality and practice. This database constitutes an important reference for indications and comparisons of therapeutic procedures, as well as to propose subsequent models to improve patient safety and the quality of surgical practice in the country.
