Abstract To assess the in vitro and in situ effect of experimental combined fluoride and calcium nanocomposite solutions on dental caries prevention. Nanocompound mesoporous silica (MS) with calcium (Ca) and sodium fluoride (NaF) - (MSCaNaF); MS with NaF (MSNaF), NaF solution (positive control), and deionized water (negative control - CG) were studied. The specimens (n=130) were submitted in vitro to a multispecies biofilm in the presence of 2% sucrose. After 24 h and 48 h, the culture medium pH, the percent of surface mineral loss (%SML), and lesion depth (ΔZ) were analyzed. In the in situ study, 10 volunteers participated in four phases of 7-days each. The products were applied on the specimens (n=240) before 20% sucrose solution drips. The polysaccharides (SEPS and IEPS), %SML and roughness (Sa) were evaluated. There was an in vitro decrease in pH values in 24h and 48h, compared to baseline. The MSCaNaF and MSNaF groups obtained lower values of %SML and ΔZ (p < 0.05) than CG and NaF after 24h and were similar to NaF after 48h (p<0.05). In situ results showed similar SEPS and IEPS among all groups after 48h. An after 7-days, the nanocomposites had similar values (p>0.05), while NaF was similar to CG (p>0.05). After 48h, the MSCaNaF and MSNaF reduced the %SML (p<0.05). After 7-days, both experimental nanocomposites were similar to NaF (p>0.05). Regarding Sa, MSCaNaF was better than NaF for both periods (p<0.05). The nanocomposites controlled the in vitro and in situ enamel demineralization, mainly in the initial periods.

Doxycycline to treat levodopa-induced dyskinesias in Parkinson’s disease: a preliminary study

Abstract Background Levodopa-induced dyskinesia (LID) is a common motor complication of levodopa therapy in patients with Parkinson’s disease (PD). Doxycycline is a widely used and inexpensive tetracycline with anti-inflammatory properties. Objective To evaluate the efficacy and safety of doxycycline in patients with PD and LID. Methods This was an open-label, uncontrolled, single-arm, single-center, phase 2 proof-of-concept study in patients with PD with functional impact of dyskinesia, which used levodopa three times daily, in a movement disorders clinic in Brazil. Participants were treated with doxycycline 200 mg/day for 12 weeks, with evaluations at baseline, week4, and week 12 of treatment. The primary outcome measure was the change from baseline in the Unified Dyskinesia Rating Scale (UDysRS) total score at week 12, evaluated by two blinded raters. Key secondary outcomes measures were OFF time and ON time with troublesome dyskinesia in the PD home diary. Results Eight patients with PD were treated and evaluated. Doxycycline 200 mg/day reduced the UDysRS total score at week 12, compared with baseline (Friedman χ2 = 9.6; p = 0.008). Further, doxycycline reduced the ON time with troublesome dyskinesia (Friedman χ2 = 10.8; p = 0.004) without worsening parkinsonism. There were no severe adverse events, and dyspepsia was the commonest event. Conclusion In this preliminary, open-label and uncontrolled trial, doxycycline was effective in reducing LID and safe after a 12-week treatment. Further well-designed placebo-controlled clinical trials with a longer duration and a larger number of participants are needed. Resumo Antecedentes A discinesia induzida por levodopa (DIL) é uma complicação motora comum da terapia com levodopa em pacientes com doença de Parkinson (DP). A doxiciclina é uma tetraciclina amplamente usada e barata, com propriedade anti-inflamatória. Objetivo Avaliar a eficácia e segurança da doxiciclina em pacientes com DP e DIL. Métodos Este foi um estudo aberto, não-controlado, de braço único, monocêntrico, fase 2 e de prova de conceito, em pacientes com DP e impacto funcional das discinesias, que usavam levodopa três vezes ao dia, em um ambulatório de distúrbios de movimento no Brasil. Os participantes foram tratados com doxiciclina 200 mg/dia por 12 semanas, com avaliações na base, na semana 4 e na semana 12 do tratamento. A medida de desfecho primário foi a mudança no escore total da Unified Dyskinesia Rating Scale (UDysRS) da base à semana 12, avaliada por dois avaliadores cegos. As medidas-chave de desfecho secundário fora o tempo em OFF e tempo em ON com discinesia problemática. Resultados Oito pacientes com DP foram tratados e avaliados. A doxiciclina 200 mg/dia reduziu o escore total da UDysRS na semana 12, comparado com a avaliação inicial (χ2 de Friedman = 9.6; p = 0.008). Além disso, a doxiciclina reduziu o tempo em ON com discinesia problemática (χ2 de Friedman = 10.8; p = 0.004) sem piorar o parkinsonismo. Não houve eventos adversos graves, e dispepsia foi o evento mais comum. Conclusão No presente estudo preliminar, aberto e não-controlado, a doxiciclina foi eficaz em reduzir as DIL e segura após tratamento por 12 semanas. Estudos clínicos bem-desenhados e placebo-controlados adicionais, com duração mais longa e maior número de participantes, são necessários.

Resumo O objetivo deste artigo é estudar o conhecimento de mulheres que fazem sexo com mulheres sobre Infecções Sexualmente Transmissíveis e Aids. Estudo transversal com 260 mulheres, sendo 81 mulheres que fazem sexo com mulheres ou com mulheres e homens (MSM) e 179 mulheres que fazem sexo exclusivamente com homens (MSH). Dados obtidos entre 2019 e 2020, por meio de aplicação de formulário e de instrumentos validados. Para estudar a associação entre parceria sexual e nível de conhecimento sobre Infecções Sexualmente Transmissíveis e Aids foram ajustados modelos de regressão múltipla de Cox, sendo significativas as associações com p<0,05. A mediana do percentual de acerto das questões foi inferior entre às MSM quando comparadas às das MSH [68% (18-96) vs 75% (14-96); p=0,023]. Fazer sexo com mulher [RP=2,36 (1,07-5,21); p=0,033] e escolaridade menor que 11 anos de estudo [RP=2,64 (1,12-6,21); p=0,026] associaram-se independentemente ao baixo conhecimento. As MSM apresentaram menor nível de conhecimento sobre Infecções Sexualmente Transmissíveis e Aids do que as MSH, assim como a menor escolaridade associou-se independentemente a esse desfecho.

Abstract This article aims to study the knowledge of women who have sex with women about Sexually Transmitted Infections and AIDS. Cross-sectional study of 260 women, 81 women who either have sex with women or with men and women (WSW) and 179 women who have sex exclusively with men (WSM). Data were collected in 2019/2020 by means of a form and validated instruments. To study the association between sexual partnership and level of knowledge about Sexually Transmitted Infections and AIDS, multiple Cox regression models were adjusted, producing significant associations of p<0,05. The median percentage of correct answers was lower among WSW when compared to WSM [68% (18-96) vs 75% (14-96); p=0.023]. Having sex with woman [PR=2.36 (1.07-5.21); p=0.033] and less than 11 years of schooling [PR=2.64 (1.12-6.21); p=0.026] were independently associated with low knowledge. WSW had a lower level of knowledge about Sexually Transmitted Infections and AIDS than WSM, and lower education was independently associated with this finding.

Abstract Chemical and pharmacological potentials of botanical species found in the Cerrado are well-known and widely studied. Chemical diversity of secondary metabolites produced by plants that belong to this Brazilian biome has triggered several studies in the fields of farming, pharmaceutical and cosmetic industries. Therefore, this study aimed at evaluating the chemical composition of essential oils (EOs) from fresh leaves of three species found in the Brazilian Cerrado: Cardiopetalum calophyllum Schltdl. (EO-CC), Campomanesia adamantium (Cambess.) O. Berg (EO-CA) and Protium ovatum Engl. (EO-PO) and at determining their anti-Malassezia furfur and anti-inflammatory activities. Both GC-FID and GC-MS showed that the most abundant chemical class of the oils was the one of sesquiterpene hydrocarbons. The major constituents identified in EO-CC were germacrene D (34.9%) and bicyclogermacrene (26.8%). EO-CA exhibited mainly germacrene D (21.7%) while EO-PO had high concentrations of germacrene D (25.0%) and γ-muurolene (18.6%). EOs were tested by the broth microdilution method on 96-well microplates and exhibited satisfactory activity against M. furfur, i. e., EO-CA had MIC = 375 µg/mL while EO-CC and EO-PO had MIC = 750 µg/mL. The chemotaxis model, which was used for evaluating their anti-inflammatory activity, showed that EOs exhibited effective results: C. calophyllum (EO-CC; IC50 = 24.4 µg/mL), C. adamantium (EO-CA; IC50 = 15.7 µg/mL) and P. ovatum (EO-PO; IC50 = 32.5 µg/mL). In short, biological activities of EO-CC, EO-CA and EO-PO, such as anti-Malassezia furfur and anti-inflammatory ones, were investigated and described for the first time.

ABSTRACT BACKGROUND: Although bariatric surgery is today’s gold standard treatment for obesity, weight regain affects the success rate of the procedure. Recent studies have identified psychiatric and neurological factors as possible causes. AIMS: The aim of this study was to evaluate the influence of psychiatric diseases on the outcome and long-term success of bariatric surgeries and find a weight regain threshold that has an acceptable sensibility to mental health-related issues to be used in research and clinical studies. METHODS: This is a observational study of bariatric patients submitted to Roux-en-Y bypass or sleeve gastrectomy, with a postoperative time of 2–10 years to access weight regain, depression, and binge-eating disorder. RESULTS: Of 217 patients studied, 163 were women and 54 were men, with an average postoperative time of 5.2±2.6 years. Weight regain was experienced in 35% of the patients, binge-eating disorder in 24.9%, and depression in 24%. The greater weight before surgery, body mass index (BMI), percentage increase to maximum weight loss, and time postoperatively all have a significant positive correlation with weight regain (p=0.045, p=0.026, p<0.001, and p<0.001, respectively). A significant association between binge-eating disorder, depression, and anxiety with weight regain (p=0.004, p=0.008, and p=0.001, respectively) was found. CONCLUSIONS: The significant weight regain rates with significant impact on psychiatric disorders highlight the need for continuous postoperative monitoring focused on the psychiatric aspects of obesity to aid surgeries’ long-term success.

RESUMO INTRODUÇÃO: Embora a cirurgia bariátrica atualmente é considerada o tratamento padrão ouro para a obesidade, o reganho de peso afeta a taxa de sucesso do procedimento. Estudos recentes identificaram fatores psiquiátricos e neurológicos como possíveis causas. OBJETIVOS: Avaliar a influência de transtornos psiquiátricos no resultado a longo prazo das cirurgias bariátricas; testar a sensibilidade e correlação das fórmulas de reganho de peso e de seus respectivos pontos de corte para questões relacionadas à saúde mental. MÉTODOS: Estudo observacional de pacientes pós bariátricos submetidos à by-pass em Y de Roux ou gastrectomia vertical com pós-operatório de 2 a10 anos avaliados quanto a reganho de peso, depressão e transtorno da compulsão alimentar. RESULTADOS: Foram avaliados 217 pacientes, 163 mulheres e 54 homens com pós-operatório de 5,2±2,6 anos. O reganho de peso foi registrado em 35% dos pacientes, o transtorno da compulsão alimentar (TCA) foi encontrado em 24,9% e depressão em 24%. O ganho de peso pré-operatório, o índice de massa corporea (IMC), o aumento percentual para perda máxima de peso e tempo de pós-operatório, apresentaram correlação positiva significativa com o reganho de peso (p=0,045), (p=0,026), (<0,001), (<0,001). Foi encontrada associação significativa entre TCA, depressão e ansiedade com reganho de peso (p=0,004), (p=0,008) e (p=0,001). CONCLUSÕES: As taxas significativas de reganho de peso associado ao impacto dos transtornos psiquiátricos reforçam a necessidade de acompanhamento pós-operatório contínuo focado nos aspectos psiquiátricos da obesidade, para sucesso do tratamento cirúrgico em longo prazo.

ABSTRACT This study aimed to evaluate the effect of oregano extract added to commercial diets on zootechnical performance parameters, carcass yield, immunological conditions, morphometry, and intestinal pH for free-range broiler chickens raised under sanitary challenge conditions. Three hundred chicks of the strain ‘Heavy red’ were used and distributed in a completely randomized experimental design with five treatments and six replications, totaling 30 experimental units, each consisting of 10 birds. The treatments were: T1: Basal diet without oregano extract (OE); T2: Basal diet with OE (150 mg/kg); T3: Basal diet with OE (250 mg/kg); T4: Basal diet with OE (350 mg/kg); T5: and Basal diet with OE (450 mg/kg). There was a significant effect on feed intake, weight gain, feed conversion, feed efficiency, heterophil/lymphocyte ratio (H/L), and intestinal morphometry. No significant differences were observed in viability, carcass yield, intestinal pH, lymphoid organs, gizzard, heart, and intestine. Oregano extract influenced the liver yield and abdominal fat of broilers. Overall, the inclusion of oregano extract at the level of 350 mg/kg provided better results.

RESUMO O objetivo desse trabalho foi avaliar o efeito do extrato de orégano adicionado à ração comercial, considerando os parâmetros de desempenho zootécnico, rendimento de carcaça, condições imunológicas, morfometria e pH intestinal de frangos de corte de linhagem caipira, criados sob condição de desafio sanitário. Foram utilizados 300 pintos da linhagem vermelho pesadão, que foram distribuídos em um delineamento experimental inteiramente casualizado - DIC, com cinco tratamentos e seis repetições, totalizando 30 unidades experimentais, constituídas por 10 aves cada. Os tratamentos foram: T1: Ração basal, sem Extrato de Orégano (EO); T2: Ração basal com EO (150 mg/Kg); T3: Ração basal com EO (250 mg/Kg); T4: Ração basal com EO(350 mg/Kg); T5: Ração basal com EO (450 mg/Kg). Houve efeito significativo no consumo de ração, ganho de peso, conversão alimentar, eficiência alimentar, relação heterofilo/linfócito (H/L) e morfometria intestinal. Não foram observadas diferenças significativas na viabilidade, no rendimento de carcaça, no pH intestinal, nos órgãos linfoides, moela, coração e intestino. Promoveu influência no rendimento de fígado e gordura abdominal dos frangos. A inclusão no nível de 350mg/kg proporciona melhores resultados.

Abstract Background Studies show that among the drugs most commonly used in judicial litigation in Brazil, are those used to treat diabetes mellitus, especially insulin analogues. Objective Evaluate the use of the Unified Health System (SUS) by patients with type 1 diabetes mellitus (T1DM), who receive insulin analogues through judicial action, before and after this process. Method In a retrospective longitudinal observational study, secondary data was used from these patients in Minas Gerais, Brazil, in 2018. Socio-demographic information was collected and related to the follow-up of these patients in the SUS. The McNemar χ2 test was used to compare the proportions of the variables. Results Of the 89 patients analyzed, women (53.9%) were predominant. Most patients were aged between 20 and 39 years (52.8%), and more than half, 55.1%, use only a private health system. After the judicial action, there was a significant increase (p <0.05) in the number of patients who had consultations in primary health care (from 19.1% to 30.3%) and emergency medical appointments (from 1.1% to 9.0%). Conclusion It is observed that the majority of patients with T1DM via judicial action in the SUS are not monitored by this health system through examinations, consultations, and hospitalizations.

Resumo Introdução Estudos mostram que, dentre os medicamentos mais adquiridos via ação judicial, estão os utilizados para o tratamento do Diabetes Mellitus, especialmente os análogos de insulina. Objetivo Avaliar a utilização do Sistema Único de Saúde (SUS) pelos pacientes com Diabetes Mellitus tipo 1 (DM1), que recebem insulina por meio de judicialização, antes e após este processo. Método Em um estudo observacional longitudinal retrospectivo, foram utilizados dados secundários de pacientes com DM1, que adquiriram insulinas por processos judiciais em Divinópolis-MG, Brasil, em 2018. Foram coletadas informações sociodemográficas e referentes ao acompanhamento destes pacientes no SUS Realizou-se o teste χ2 de McNemar para a comparação das proporções das variáveis utilizadas para a avaliação do acompanhamento antes e após a judicialização. Resultados Dos 89 pacientes analisados, predominou-se o sexo feminino (53,9%), com idade entre 20 e 39 anos (52,8%). 55,1% destes utilizam apenas o sistema privado de saúde. Após a judicialização, houve um aumento significativo (p< 0,05) no número de pacientes que realizaram consultas na atenção primária à saúde (de 19,1% para 30,3%) e consultas médicas de emergência (de 1,1% para 9,0%). Conclusão A maioria dos pacientes com DM1 que judicializam medicamentos no SUS não são acompanhados por este sistema de saúde através de realização de exames, consultas e hospitalizações.

ABSTRACT Objective: to assess Pap smear performance in bacterial vaginosis diagnosis in women of childbearing age. Method: a cross-sectional, retrospective study carried out in a health unit in a city in the interior of São Paulo. The study included information from 1,173 women who underwent collection of Pap smear and Gram-stained vaginal smears (reference standard test for bacterial vaginosis diagnosis) and analysis based on the Nugent score, from January 2013 to June 2020. Secondary data collected from the information and internal control systems were used. Results: the prevalence of bacterial vaginosis in the sample was 31.8%. The degree of agreement between the two diagnostic methods by the Kappa index was 0.54, considered moderate, with a value <0.001. Sensitivity was 55.2% and specificity was 94.1%, with accuracy of 81.7% and positive predictive value of 81.4% and negative of 81.8%. Conclusion: Pap smear showed agreement with the reference standard of 0.77, however, does not replace the Gram-stained vaginal smear, but can be used as an auxiliary method for bacterial vaginosis diagnosis.

RESUMEN Objetivo: evaluar el desempeño del examen citopatológico en el diagnóstico de vaginosis bacteriana en mujeres en edad fértil. Método: estudio transversal, retrospectivo, realizado en una unidad de salud de una ciudad del interior de São Paulo. El estudio incluyó información de 1173 mujeres a las que se les realizó recolección de pruebas citopatológicas y frotis vaginales teñidos con la técnica de Gram (prueba estándar de referencia para el diagnóstico de vaginosis bacteriana) y análisis con base en el puntaje de Nugent, desde enero de 2013 hasta junio de 2020. Datos secundarios recolectados de los sistemas de información y control interno. Resultados: la prevalencia de vaginosis bacteriana en la muestra fue de 31,8%. El grado de concordancia entre los dos métodos diagnósticos por el índice Kappa fue de 0,54, considerado moderado, con un valor <0,001. La Sensibilidad fue del 55,2% y la Especificidad del 94,1%, con una Precisión del 81,7% y un Valor Predictivo Positivo del 81,4% y Negativo del 81,8%. Conclusión: el examen citopatológico mostró concordancia con el patrón de referencia de 0,77, sin embargo, no sustituye al frotis vaginal teñido de Gram, pero puede utilizarse como método auxiliar para el diagnóstico de la vaginosis bacteriana.

RESUMO Objetivo: avaliar o desempenho do exame citopatológico no diagnóstico de vaginose bacteriana de mulheres em idade fértil. Método: estudo transversal, retrospectivo, realizado em uma unidade de saúde de um município do interior paulista. Foram inclusas no estudo as informações de 1173 mulheres submetidas à coleta dos exames citopatológico e esfregaço vaginal corado com técnica de Gram (exame padrão referência para diagnóstico de vaginose bacteriana) e análise a partir do score de Nugent, no período de janeiro de 2013 a junho de 2020. Utilizaram-se dados secundários, coletados a partir dos sistemas de informação e controle interno. Resultados: a prevalência de vaginose bacteriana na amostra foi de 31,8%. Abstract Background The factors associated with bacterial vaginosis in women with homosexual, bisexual and heterosexual practices are still poorly explored. Thus, the aim of this study was to analyze the factors associated with bacterial vaginosis in women with different sexual practices. Methods Cross-sectional study that included 453 women, 149 Women with Homosexual practice (WSW); 80 bisexual Women (WSWM) and 224 Women with heterosexual practice (WSM). The diagnosis of bacterial vaginosis was performed by microscopic examination of the vaginal smears stained by Gram method and classified according to the Nugent et al. (1991) score. Data analysis was performed by Cox multiple regression. Results Bacterial vaginosis was associated to years of education among WSW (0.91 [95% CI 0.82‒0.99]; p= 0.048) and non-white skin color (2.34 [95% CI 1.05‒5.19]; p= 0.037) between WSWM. Changing partners in the last 3-months (2.09 [95% CI 1.14‒3.82]; p= 0.017), inconsistent use of condoms (2.61 [95% CI 1.10‒6.20]; p= 0.030) and positive diagnosis of Chlamydia trachomatis (2.40 [95% CI 1.01‒5.73]; p= 0.048) were associated with bacterial vaginoses only in WSH. Conclusions The factors associated to bacterial vaginosis differ between different sexual practices, suggesting that the type of sexual partner may influence the risk of developing this classic dysbiosis.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

ABSTRACT Objective: To determine the clinical profile of COVID-19 inpatients who were vaccinated prior to hospitalization and to compare the risk factors for death and the 28-day survival rate of between those inpatients vaccinated with one, two, or three doses and unvaccinated COVID-19 inpatients. Methods: This was a retrospective observational cohort study involving COVID-19 patients admitted to a referral hospital in the city of Recife, Brazil, between July of 2020 and June of 2022. Results: The sample comprised 1,921 inpatients, 996 of whom (50.8%) were vaccinated prior to hospitalization. After adjusting the mortality risk for vaccinated patients, those undergoing invasive mechanical ventilation (IMV) had the highest mortality risk (adjusted OR [aOR] = 7.4; 95% CI, 3.8-14.1; p < 0.001), followed by patients > 80 years of age (aOR = 7.3; 95% CI, 3.4-15.4; p < 0.001), and those needing vasopressors (aOR = 5.6; 95% CI, 2.9-10.9; p < 0.001). After adjusting the mortality risk for all patients, having received three vaccine doses (aOR = 0.06; 95% CI, 0.03-0.11; p < 0.001) was the most important protective factor against death. There were progressive benefits of vaccination, reducing the frequency of ICU admissions, use for IMV, and death (respectively, from 44.9%, 39.0% and 39.9% after the first dose to 16.7%, 6.2% and 4.4% after the third dose), as well as significant improvements in survival after each subsequent dose (p < 0.001). Conclusions: Vaccines were effective in reducing illness severity and death in this cohort of COVID-19 inpatients, and the administration of additional doses conferred them with accumulative vaccine protection.

RESUMO Objetivo: Traçar o perfil clínico de pacientes internados com COVID-19 que haviam sido vacinados antes da hospitalização e comparar os fatores de risco para óbito e a taxa de sobrevida em 28 dias entre esses internados vacinados com uma, duas ou três doses e pacientes internados com COVID-19 não vacinados. Métodos: Estudo de coorte observacional retrospectivo envolvendo pacientes com COVID-19 internados em um hospital de referência na cidade do Recife (PE) entre julho de 2020 e junho de 2022. Resultados: A amostra foi composta por 1.921 pacientes internados, dos quais 996 (50,8%) haviam sido vacinados antes da hospitalização. Após ajuste do risco de mortalidade para os pacientes vacinados, aqueles submetidos à ventilação mecânica invasiva (VMI) apresentaram o maior risco de mortalidade (OR ajustada [ORa] = 7,4; IC95%: 3,8-14,1; p < 0,001), seguidos pelos pacientes > 80 anos (ORa = 7,3; IC95%: 3,4-15,4; p < 0,001) e aqueles que necessitam de vasopressores (ORa = 5,6; IC95%: 2,9-10,9; p < 0,001). Após ajuste do risco de mortalidade para todos os pacientes, o recebimento de três doses de vacina (ORa = 0,06; IC95%: 0,03-0,11; p < 0,001) foi o fator de proteção mais importante contra o óbito. Houve benefícios progressivos da vacinação, com redução da frequência de internações em UTI, de uso de VMI e de óbitos (de 44,9%, 39,0% e 39,9% após a primeira dose para 16,7%, 6,2% e 4,4% após a terceira dose, respectivamente), bem como melhora significativa na sobrevida após cada dose subsequente (p < 0,001). Conclusões: As vacinas foram efetivas na redução da gravidade da doença e dos óbitos nesta coorte de pacientes internados com COVID-19, e a aplicação de doses adicionais conferiu-lhes proteção vacinal cumulativa.

ABSTRACT Tephrosia egregia Sandwith is a species that develops in some States of the Northeast region of Brazil. It is propagated by seeds and has considerable potential for restoration. Thus, studies related to seed germination are important for the species. The aims were to characterize the post-seminal development and evaluate the germination response of the seeds under different temperatures and water stress and salt stress. Germination tests were performed at 20, 25, 30 and 20-30 °C and under PEG 6000 and NaCl concentrations (-0.2, -0.4, -0.6 and -0.8 MPa). The germination test should be conducted in a paper roll at 25 ºC for 14 days. Seed germination declines from the water potential of -0.2 MPa on; the species is sensitive to water stress. Under salt stress, there is more accentuated decline in germination as from -0.4 MPa, and this is an indication have a certain tolerance to salinity.

RESUMO Tephrosia egrégia Sandwith é uma espécie que se desenvolve em alguns Estados do Nordeste. Sua propagação é realizada por meio de sementes, sendo uma espécie potencial para restauração. Os objetivos foram caracterizar o desenvolvimento pós-seminal e avaliar a germinação das sementes sob diferentes temperaturas e sob estresse hídrico e salino. Testes de germinação foram conduzidos a 20, 25, 30 e 20-30 °C e sob concentrações de PEG6000 e NaCl (-0.2, -0.4, -0.6 e -0.8 MPa). O teste de germinação deve ser conduzido em rolo de papel sob temperatura constante de 25 ºC durante 14 dias. A germinação das sementes é reduzida a partir do potencial hídrico de -0.2 MPa, sendo a espécie sensível ao estresse hídrico. Sob estresse salino, a redução mais acentuada na germinação, a partir de -0.4 MPa, sendo isso um indicativo de uma certa tolerância à salinidade.

Abstract In Brazil, insulin analogs stand out as one of the most demanded medications by judicial means. However, the guarantee of judicial access does not guarantee rational use. In context, pharmacotherapeutic follow-up (PF) is shown to be clinical effective strategy for patients with diabetes. To evaluate direct medical costs one year after performing PF in patients with type 1 diabetes mellitus using insulin analogs ordered by court in Public Health System (Sistema Único de Saúde - SUS). This is a partial economic analysis, nested within a quasi-experimental study. Patients with T1DM who receive insulin analogs by judicialization in a medium-sized Brazilian city participated. The PF was conducted following the method adapted from the Pharmacotherapy workup (PW). Data were collected considering the period of one year before the start of the intervention and one year after the start of the intervention. Direct medical costs were evaluated and the difference in costs was calculated. 28 patients participated in the intervention. After PF, direct costs were -$3,696.78. Sensitivity analysis showed that there is a 33.4 % chance for PF to present cost savings when compared to baseline. The PF has the potential to reduce direct medical costs from the perspective of the SUS.

ABSTRACT Objective: To investigate sociodemographic factors, non-communicable diseases and conditions, and behavioral risk factors associated with negative self-rated health among Brazilian women of childbearing age. Method: Cross-sectional study with 26,071 Brazilian women of reproductive age. Estimated prevalence of self-rated health according to sociodemographic characteristics, non-communicable diseases and conditions, and behavioral risk factors. Poisson regression was used to estimate adjusted and unadjusted prevalence ratios. Results: Occurrence of two or more of the diseases and conditions presented a prevalence of negative self-rated health almost three times higher than none. There was a positive association between negative self-rated health and older age groups, lower education, black or brown skin color/race, living in the north and northeast regions, physical inactivity, being a smoker, and presence of one or more of the diseases and conditions. Conclusion: There are differences in self-rated health, reflecting social inequalities.

RESUMEN Objetivo: Investigar factores sociodemográficos, enfermedades y condiciones no transmisibles y factores de riesgo conductuales asociados con la autoevaluación negativa de la salud entre mujeres brasileñas en edad fértil. Método: Estudio transversal con 26.071 mujeres brasileñas en edad reproductiva. Prevalencia estimada de salud autovalorada según características sociodemográficas, enfermedades y trastornos no transmisibles y factores de riesgo conductuales. Se utilizó la regresión de Poisson para estimar las tasas de prevalencia ajustadas y no ajustadas. Resultados: La aparición de dos o más de las enfermedades y agravios mostró una prevalencia de autoevaluación negativa de la salud casi tres veces mayor que ninguna. Hubo una asociación positiva entre la autoevaluación negativa de la salud y los grupos de mayor edad, menor educación, color de piel/raza negra o morena, vivir en las regiones norte y noreste, inactividad física, ser fumador y la presencia de una o más de las enfermedades y condiciones. Conclusión: Existen diferencias en la autoevaluación de la salud, lo que refleja desigualdades sociales.

RESUMO Objetivo: Investigar fatores sociodemográficos, doenças e agravos não transmissíveis e fatores de risco comportamentais associados à autoavaliação de saúde negativa das mulheres brasileiras em idade reprodutiva. Método: Estudo transversal com 26.071 mulheres brasileiras em idade reprodutiva. Estimadas prevalências da autoavaliação de saúde segundo características sociodemográficas, doenças e agravos não transmissíveis e fatores de risco comportamentais. Utilizou-se regressão de Poisson para estimar as razões de prevalência ajustadas e não ajustadas. Resultados: Ocorrência de duas ou mais das doenças e agravos apresentou prevalência de autoavaliação de saúde negativa quase três vezes maior do que nenhuma. Houve associação positiva entre autoavaliação de saúde negativa e maiores faixas etárias, menor escolaridade, cor da pele/raça preta ou parda, viver nas regiões norte e nordeste, inatividade física, ser fumante e presença de uma ou mais das doenças e agravos. Conclusão: Existem diferenças na autoavaliação de saúde, refletindo em iniquidades sociais.
