Resumo Introdução: A doença pulmonar obstrutiva crónica (DPOC) é a terceira causa de mortalidade a nível global. O tratamento foca-se na melhoria da qualidade de vida, atraso da progressão da doença e redução das exacerbações. Neste contexto, o benefício da reabilitação respiratória tem vindo a ser verificado. Esta medida não farmacológica pode ser implementada na comunidade, no contexto dos cuidados de saúde primários. Objetivos: Esta revisão sistemática propôs-se a fazer um levantamento da evidência atual sobre o impacto da reabilitação pulmonar na qualidade de vida dos doentes com DPOC, num contexto de cuidados de saúde primários. Métodos: Foram pesquisados estudos randomizados controlados em três bases de dados. Foi incluída a reabilitação pulmonar realizada na comunidade e excluídas intervenções em contexto hospitalar. Foi considerado como resultado primário a qualidade de vida relacionada com a saúde comparando com a terapêutica padrão da DPOC. A extração e triagem de dados foi efetuada por três autores, de forma independente. Avaliou-se o viés pela Cochrane Risk of Bias Tool. Resultados: Quatro estudos corresponderam aos critérios de pesquisa. Três estudos reportaram benefício em pelo menos um dos resultados. A frequência e duração da reabilitação pareceram ter algum impacto nos resultados. A mortalidade foi mais alta nos grupos de controlo. Os estudos incluídos tinham limitações importantes, nomeadamente a incapacidade de realizar intervenções cegas. Conclusões: A reabilitação pulmonar em cuidados de saúde primários melhora a qualidade de vida e capacidade funcional na DPOC. A duração, frequência e modalidade ótimas da intervenção não estão definidas e poderão depender de características individuais. Embora o paradigma não se altere, espera-se que esta revisão fomente o interesse na intervenção.

Abstract Introduction: Chronic obstructive pulmonary disease (COPD) is the third leading cause of death worldwide, and its management involves improving quality of life, delaying progression, and minimizing exacerbations. The benefit of pulmonary rehabilitation in COPD has been documented in the literature. This intervention can be applied to a primary healthcare setting, using community resources. Objectives: This systematic review sought to compile current evidence implying the impact of pulmonary rehabilitation in a primary healthcare setting on the quality of life of patients with COPD. Methods: Three databases were searched for randomized controlled trials. We included community-based pulmonary rehabilitation and excluded interventions in a hospital setting. The primary outcome was health-related quality of life; secondary outcomes were functional exercise capacity and mortality. Data extraction and triage were performed independently by three authors, and bias was evaluated using the Cochrane Risk of Bias Tool. Results: Four studies met the search criteria. Three studies reported benefits in at least one of the outcomes; a fourth reported no differences between groups. Rehabilitation frequency and duration seemed to have some impact on all outcomes. Mortality was higher in the control groups when available. The included studies had important limitations, namely the inability to blind interventions. Conclusions: Pulmonary rehabilitation in a primary health care setting benefits the quality of life and functional exercise capacity in COPD patients. Optimal treatment modality, frequency, and duration are yet to be defined and are perhaps reliant on individual patient attributes. Though unable to add to previous findings, we hope this review fosters interest in this potentially life-changing intervention in a primary healthcare context.

Resumo Introdução: A dissecção endoscópica da submucosa (ESD) é uma técnica endoscópica com demonstrada eficácia nas lesões neoplásicas superficiais colorectais. No entanto, a evidência da sua eficácia nas lesões localizadas na junção ano-rectal é escassa. O nosso objectivo foi avaliar a segurança e eficácia da ESD nas lesões da junção ano-rectal (menos de 2 cm da linha pectínea), em comparação com as lesões mais proximais do recto. Métodos: Análise retrospectiva de registos colhidos prospectivamente de dois centros europeus de referência, entre 2015 e 2021. Resultados: Foram incluídas 252 lesões. Sessenta (24%) localizavam-se na junção ano-rectal, e as restantes 192 noutro local do recto. O sucesso técnico foi de 98% (n = 248) e foi semelhante nas 2 localizações (p = 0.246). A maioria das lesões eram displasias de alto grau/Tis (54%); 36 (15%) tinham adenocarcinoma submucoso, tendo 20 invasão submucosa superficial (sm1) e 16 invasão profunda (>SM1). Não foram encontradas diferenças entre as duas localizações relativamente às taxas de ressecção em bloco (100% vs. 96%, p = 0.204), R0 (76% vs. 75%, p = 0.531), ou curativa (70% vs. 70%, p = 0.920), duração da ESD (mediana 120 min vs. 90 min, p = 0.072), velocidade da ESD (14 vs. 12 mm2/min, p = 0.415) ou resultado histológico (p = 0.053), assim como na necessidade de cirurgia por ESD não curativa (5% vs. 3%, p = 0.739). Além disso, as taxas de hemorragia tardia (7% vs. 8%, p = 0.709), perfuração (0% vs. 5%, p = 0.075) e necessidade de internamento por complicações (2% vs. 2%, p = 0.939) não revelaram diferenças estatisticamente significativas. A estenose ano-rectal (5% vs. 0%, p = 0.003) e a dor ano-rectal (9% vs. 1%, p = 0.002) foram mais frequentes nas lesões da junção ano-rectal. Conclusão: A ESD é uma técnica segura e eficaz no tratamento das lesões do recto localizadas na junção ano-rectal.

Abstract Introduction: Endoscopic submucosal dissection (ESD) is a well-established resection technique for colorectal superficial tumors, but its role in the treatment of anorectal junction (ARJ) lesions still remains to be determined. With this study, we aimed to evaluate the feasibility, safety, and efficacy of ESD for the resection of ARJ lesions, in comparison to more proximal rectal lesions. Methods: We performed a retrospective analysis of prospectively collected data concerning all consecutive rectal ESD procedures performed in two European centers, from 2015 to 2021. Results: A total of two hundred and fifty-two rectal lesions were included. Sixty (24%) were ARJ lesions, and the remaining 192 (76%) were located proximally. Technical success was achieved in 248 procedures (98%), and its rate was similar in both locations (p = 0.246). Most of the lesions presented high-grade dysplasia/Tis adenocarcinoma (54%); 36 (15%) had submucosal adenocarcinoma, including 20 superficial (sm1) and 16 deeply invasive (>SM1) T1 cancers. We found no differences between ARJ and rectal lesions in regard to en bloc resection rate (100% vs. 96%, p = 0.204), R0 resection rate (76% vs. 75%, p = 0.531), curative resection rate (70% vs. 70%, p = 0.920), procedures’ median duration (120 min vs. 90 min, p = 0.072), ESD velocity (14 vs. 12 mm2/min, p = 0.415), histopathology result (p = 0.053), and the need for surgery due to a non-curative ESD (5% vs. 3%, p = 0.739). Also, there was no statistically significant difference that concerns delayed bleeding (7% vs. 8%, p = 0.709), perforation (0% vs. 5%, p = 0.075), or the need for readmission (2% vs. 2%, p = 0.939). Nevertheless, anorectal stenosis (5% vs. 0%, p = 0.003) and anorectal pain (9% vs. 1%, p = 0.002) were significantly more frequent in ARJ lesions. Conclusion: ESD is a safe and efficient resection technique for the treatment of rectal lesions located in the ARJ.

Abstract An adopted animal is considered a grand gesture with significant social responsibility since it needs daily care and affection from the adopter. Far beyond appearance and aesthetic issues, the adopter must consider their natural conditions, respecting the specific characteristics of the animal. The lack of expertise in choosing the animal can lead to an irresponsible and fruitless adoption, compromising the animal’s quality of life. Animal adoption portals are prone to this problem if they do not provide controls that try to approximate the adopter’s real motivations and conditions with the animal’s characteristics. Faced with this problem, this article proposes a content-based recommendation system to suggest animals for users to consume drugs, applying the weighted cosine technique. The weighting is fundamental in calculating the similarity between the users’ emotions and the animal’s characteristics. The experiments increased the accuracy of the recommendations when weighting is applied over the traditional method without considering the prescribed method of users by specific characteristics of the animals to be adopted.

Resumo A adoção animal é considerada um gesto grandioso que carrega consigo grande responsabilidade social, visto que um animal precisa dos cuidados e carinho diário do adotante. Muito além da aparência e questões estéticas, é essencial que o adotante considere suas reais condições respeitando as características específicas do animal. A falta de perícia na escolha do animal pode acarretar uma adoção irresponsável e infrutuosa, comprometendo a qualidade de vida do animal. Portais de adoção de animais são suscetíveis a esse problema se não fornecerem mecanismos que tentem aproximar as reais preferências e condições do adotante com as características do animal. Diante dessa problemática, este artigo propõe um sistema de recomendação baseado em conteúdo que visa sugerir animais para adoção considerando as preferências dos adotantes, aplicando a técnica do cosseno ponderado. A ponderação é fundamental no cálculo de similaridade entre as preferências dos adotantes e das características do animal. Os experimentos realizados apontam um aumento de precisão das recomendações quando a ponderação é aplicada sobre o método tradicional sem considerar as preferências do adotante por determinadas características dos animais a serem adotados.

ABSTRACT In Brazil, the COVID-19 burden was substantial, and risk factors associated with higher in-hospital mortality rates have been extensively studied. However, information on short-term all-cause mortality and the factors associated with death in patients who survived the hospitalization period of acute SARS-CoV-2 infection is limited. We analyzed the six-month post-hospitalization mortality rate and possible risk factors of COVID-19 patients in a single center in Brazil. This is a retrospective cohort study focused on a six-month follow-up. The exclusion criteria were death during hospitalization, transference to another hospital, and age under 18. We collected data from the charts of all hospitalized patients from March 2020 to December 2020 with a positive RT-PCR test for SARS-CoV-2, resulting in a sample size of 106 patients. The main outcome was death after hospitalization, whereas comorbidities and demographics were evaluated as risk factors. The crude post-hospitalization death rate was 16%. The first 30 days of follow-up had the highest mortality rate. In a Cox regression model for post-hospitalization mortality, previous chronic kidney disease (HR, 4.06, 95%CI 1.46 – 11.30) and longer hospital stay (HR 1.01, 95%CI 1.00 – 1.02) were the only factors statistically associated with death. In conclusion, a high six-month all-cause mortality was observed. Within the six-month follow-up, a higher risk of death was observed for patients who had prior CKD and longer hospital stay. These findings highlight the importance of more intensive medical surveillance during this period.

Abstract This work aims the assessment of the dynamic structural response of reinforced concrete floors subjected to rhythmic human activities, based on the use of biodynamic models, in order to consider the people-structure interaction effect. Initially, an experimental modal analysis on the investigated floor , with dimensions of 16m x 35m, was performed, in order to identify and assess the global dynamic behaviour of the structure. In sequence, a finite element model was developed and calibrated through experimental results. After that, based on forced vibration analyses, the floor dynamic response was determined (displacements and accelerations). It is concluded that the biodynamic systems modelling induced significant attenuations on the structural response when compared to those calculated based on the use of traditional “only force” models.

Resumo Este trabalho tem como objetivo a avaliação da resposta dinâmica de pisos de concreto armado submetidos a atividades humanas rítmicas, com base no uso de sistemas biodinâmicos, a fim de analisar o efeito de interação pessoa-estrutura. Inicialmente, realizou-se uma análise modal experimental sobre o piso investigado, com dimensões de 16m x 35m, visando identificar e avaliar o comportamento dinâmico global da estrutura. Em seguida, um modelo de elementos finitos foi desenvolvido e calibrado com base nos resultados experimentais. Posteriormente, a partir das análises de vibração forçada, a resposta dinâmica do piso foi determinada (deslocamentos e acelerações). Conclui-se que a modelagem dos sistemas biodinâmicos induziu atenuações significativas sobre a resposta estrutural quando comparadas àquelas calculadas via utilização de modelos tradicionais de força-rítmica.

Abstract Objective To describe the prevalence of newly graduated nurses as second victims of adverse events and to know the conditions of support received in health institutions. Methods Cross-sectional, descriptive, quantitative study. The population consisted of newly graduated nurses who agreed to answer the online questionnaire with questions related to being hired at the institution, involvement in adverse events and management of the second victim condition. Contacted was by email, intermediated by the Regional Nursing Council of São Paulo. Descriptive data analysis and the test of equality of proportions were performed. Results The final sample consisted of 138 nurses, 54.3% were unaware of the term ‘second victim’, 44.9% were unaware of the existence of institutional protocols for emotional support to professionals and 26.8% were involved in adverse events. Of these, 94.6% presented emotional distress, frustration, guilt, sadness, stress, inability, embarrassment and insecurity to perform their duties at work as an outcome of the event; 59.5% received some type of support and 21.6% received institutional punishment. Conclusion The prevalence of newly graduated nurses involved in adverse events was 26.8%, and among those who experienced this incident, the majority presented negative feelings and insecurity in performing their work as an outcome. After the event, most of the time, the support received came from work colleagues and significant others. Regarding institutional support, the need for programs for emotional support is also highlighted, so that these professionals can overcome when finding themselves in the place of the second victim.

Resumen Objetivo Describir la prevalencia de enfermeros recién graduados como segundas víctimas de eventos adversos y conocer las condiciones de apoyo recibidas en las instituciones de salud. Métodos Estudio transversal, descriptivo y de enfoque cuantitativo, cuya población estuvo compuesta por enfermeros recién graduados, que aceptaron responder un cuestionario digital con preguntas relacionadas con la admisión en la institución, la participación en eventos adversos y la gestión de la condición de segunda víctima, contactados por correo electrónico e intermediado por el Consejo Regional de Enfermería de São Paulo. Se realizó el análisis descriptivo de los datos y prueba de igualdad de proporciones. Resultados La muestra final fue de 138 enfermeros. El 54,3 % desconocía el término segunda víctima, el 44,9 % desconocía la existencia de protocolos institucionales para apoyo emocional a profesionales y el 26,8 % estuvo involucrado en eventos adversos. De estos, el 94,6 % presentó, como consecuencia del evento, sufrimiento emocional, frustración, culpa, tristeza, estrés, incapacidad, vergüenza e inseguridad para realizar sus funciones en el trabajo; el 59,5 % recibió algún tipo de apoyo, y el 21,6 % recibió sanción institucional. Conclusión La prevalencia de enfermeros recién graduados involucrados en eventos adversos fue del 26,8 % y, de los que pasaron por estos incidentes, la mayoría presentó, como consecuencia, sentimientos negativos y de inseguridad en la conducción de su trabajo. Después del evento, el apoyo recibido, la mayoría de las veces, surgió de compañeros de trabajo y personas importantes. Respecto al apoyo institucional, también se observa la necesidad de programas para apoyo emocional para que estos profesionales se sobrepongan cuando se encuentren en condición de segunda víctima.

Métodos Estudo transversal, descritivo e de abordagem quantitativa, cuja população foi constituída por enfermeiros recém-formados, que aceitaram responder ao questionário online, com perguntas relacionadas à admissão na instituição, envolvimento em eventos adversos e gerenciamento da condição de segunda vítima, contatados por e-mail, intermediado pelo Conselho Regional de Enfermagem de São Paulo. Foi realizada a análise descritiva dos dados e teste de igualdade de proporções. Resultados A amostra final foi de 138 enfermeiros, 54,3% desconheciam o termo segunda vítima, 44,9% desconheciam a existência de protocolos institucionais para apoio emocional aos profissionais e 26,8% estiveram envolvidos em eventos adversos. Destes, 94,6% apresentaram como desfecho diante do evento o sofrimento emocional, frustração, culpa, tristeza, estresse, incapacidade, constrangimento e insegurança para realizar suas funções no trabalho; 59,5% receberam algum tipo de apoio e 21,6% receberam punição institucional. Conclusão A prevalência de enfermeiros recém-formados envolvidos em eventos adversos foi de 26,8%, e, entre os que vivenciaram esse incidente, a maioria apresentou como desfecho, sentimentos negativos e de insegurança na condução do trabalho. Após o evento, o apoio recebido partiu, na maioria das vezes, de colegas de trabalho e pessoas significativas, e, quanto ao apoio institucional, destaca-se ainda a necessidade de programas para suporte emocional, a fim de que esses profissionais superem quando se encontram na condição de segunda vítima.

Abstract The construction of high-rise buildings has emerged as a constructive trend worldwide, and excessive vibration problems due to wind actions are becoming increasingly frequent. The Brazilian design standard NBR 6123 recommends that the transfer of wind actions used for structural analysis be carried out based on the pressure coefficients along the building facades for static analyses and considers their non-deterministic dynamic behaviour through a stochastic modelling method of the wind velocity field. To present an alternative approach to this methodology, this study aims to investigate the non-deterministic dynamic structural response of a real reinforced concrete building, considering the soil-structure interaction effect, using the pressure coefficients obtained through different methodologies such as numerical simulations using Computational Fluid Dynamics (CFD), international databases, and the recommendations of the Brazilian standard NBR 6123.

Resumo A construção de edifícios altos tem se mostrado uma tendência construtiva ao redor do mundo, e vibrações excessivas oriundas das ações do vento tem se tornado cada vez mais frequentes. A norma brasileira de projeto NBR 6123 recomenda que a transferência das ações de vento utilizadas para análise estrutural seja realizada com base nos coeficientes de pressão ao longo das fachadas do edifício, para análises estáticas, e considera o seu comportamento dinâmico não determinístico através de um método de modelagem estocástica do campo de velocidade do vento. Objetivando apresentar uma abordagem alternativa para esta metodologia, este estudo tem como objetivo investigar a resposta estrutural dinâmica não determinística de um edifício real de concreto armado, considerando o efeito da interação solo-estrutura, utilizando coeficientes de pressão obtidos por intermédio de metodologias distintas, tais como: simulações numéricas utilizando CFD (Computational Fluid Dynamics), bases de dados internacionais, e utilizando as recomendações da norma brasileira NBR 6123.

Resumo Lajes com fôrmas metálicas utilizadas durante a concretagem são um sistema estrutural misto composto de aço e concreto. O modo de falha por cisalhamento horizontal é o mais comum em lajes mistas com bases em numerosos estudos experimentais. Esta falha consiste na perda de aderência e contato mecânico do sistema misto e, frequentemente, ocorre antes do sistema atingir sua capacidade máxima à flexão plástica. Assim, o projeto deste sistema construtivo é determinado pela sua resistência ao cisalhamento longitudinal, que pode ser determinada pelo método semiempírico convencional m-k com base no ensaio de flexão de quatro pontos proposto por diversas normas. Um dos fatores conhecidos de maior influência na resistência ao cisalhamento longitudinal é a geometria das mossas presentes na fôrma metálica. Simulações computacionais por meio do método dos elementos finitos calibradas com base em resultados experimentais podem ser uma alternativa à realização deste tipo de ensaio. O presente trabalho modificou e adaptou uma metodologia que inclui explicitamente a geometria das mossas na análise de modo a usar materiais e produtos com características disponíveis para os engenheiros estruturais do Brasil e usou esta metodologia para derivar coeficientes m-k. Isto foi feito com o objetivo de simular numericamente o efeito de parâmetros relevantes para determinar a resistência ao cisalhamento longitudinal na interface aço-concreto de lajes mistas. Gráficos de força versus deslocamentos foram obtidos e os valores dos coeficientes m-k foram comparados com aqueles de outros autores na literatura. Abstract Slabs with cast-in-place steel formwork are composite structural elements composed of concrete and steel. The horizontal shear failure mode is the most common in composite slabs based on numerous experimental studies. This failure mode consists of loss of adherence and mechanical contact at the interface of the composite system and often occurs before the system reaches its full bending capacity. Thus, the design of this constructive system is determined by the value of the longitudinal shear strength, which can be computed by the semi-empirical so-called traditional m-k method based on the four-point bending test proposed by several standards. One of the most known factors to influence the longitudinal shear strength is the geometry of embossments present in the steel work. Calibrated computational simulation using the finite element method can be an alternative way to simulate this type of test. The present work modified and adapted a methodology explicitly including the geometry of embossments in the analysis in order to use materials and products with characteristics available to structural engineers in Brazil and used this methodology to derive m-k coefficients. This is being made with the objective of numerically simulating the effect of relevant parameters for determining the longitudinal shear strength at the concrete-steel interface of composite slabs. Force versus slip, force versus displacement graphs were obtained and the m-k coefficient values found were compared with those of other authors in the literature. ABSTRACT OBJECTIVE To describe the methodological challenges and strategies of a web survey on the working conditions and health among delivery workers. METHODS The study population consisted of Brazilian delivery workers operating in the national territory. Procedures include building solid and ongoing collaboration with worker representatives and conducting a four-month data collection from February to May 2022, sharing the link to the online questionnaire on social media such as social networks (Facebook, Instagram) and messaging apps (WhatsApp, Telegram). RESULTS The recruitment of 41 leaders or influencers of delivery workers increased the dissemination of the study, some of whom participated in the consensual validation of the questionnaire; the production of content for social media for the dissemination of the questionnaire link on social networks and applications, and the in-person dissemination of the study at the delivery workers’ meeting points during the workday played a fundamental role, totaling around 132 hours in 45 shifts. The strategies adopted for data collection with a hybrid approach to dissemination made it possible to carry out the web survey. After four months of the web survey, 564 delivery workers, 543 men and 18 women, responded to the online questionnaire. CONCLUSION The web survey presented methodological strategies to overcome the challenge of reaching workers, including hybrid work, to increase the participation of workers, on whom epidemiological research is still scarce.

Resumo A avaliação do atraso de colheita de sementes de soja pode auxiliar na seleção de cultivares mais resistentes à deterioração no campo. O objetivo deste trabalho foi avaliar a qualidade fisiológica e sanitária de sementes de três cultivares de soja, submetidas ao atraso da colheita. Sementes das cultivares ‘NS 5959’, ‘BMX Potência’ e ‘TMG 1175’ foram colhidas em quatro épocas (R8; R8 + 10 dias; R8 + 20 dias e R8 + 30 dias) e avaliadas quanto ao grau de umidade, germinação, emergência, crescimento de plântulas e incidência de fungos. Houve redução na germinação e vigor das sementes das diferentes cultivares de soja, principalmente quando colhidas em R8 + 20 e R8 + 30 dias, devido à deterioração em campo. Além disso, o atraso na colheita proporcionou maior incidência de fungos nas sementes, principalmente Phomopsis sp., Aspergillus sp. e Cercospora kikuchii. Em geral, a cultivar ‘TMG 1175’ foi mais resistente à deterioração das sementes em campo causada pelo atraso de colheita.

Abstract The assessment of delayed harvesting of soybean seeds may help in the selection of cultivars more resistant to deterioration in the field. This work aimed to evaluate the physiological and sanitary quality of seeds of three soybean cultivars submitted to harvest delay. Seeds of the cultivars ‘NS 5959’, ‘BMX Potência’, and ‘TMG 1175’ were harvested in four periods (R8; R8 + 10 days; R8 + 20 days, and R8 + 30 days) and evaluated for the moisture content, germination, emergence, seedling growth, and fungal incidence. There was a reduction in germination and vigor of the seeds of the different soybean cultivars, especially when harvested at R8 + 20 and R8 + 30 days, due to deterioration in the field. In addition, the delay in harvest provided a higher incidence of fungi in the seeds, mainly Phomopsis sp., Aspergillus sp., and Cercospora kikuchii. In general, the cultivar ‘TMG 1175’ was more resistant to seed deterioration in the field caused by delayed harvesting.

Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

ABSTRACT Scorpion envenoming is considered a public health problem in Brazil. A recent study described a variation in the clinical outcome of envenoming by Tityus obscurus in two populations separated by 850 km in the northeastern Amazon region. Our aim was to evaluate whether such clinical and toxinological variations are associated with underlying differences in genetic diversity between these two T. obscurus populations. We obtained DNA from five individuals of each population, in the municipalities of Belém and Santarém, located east and west of the state of Pará, Brazil, respectively. Gene regions encoding mitochondrial DNA (mtDNA) markers cytochrome oxidase subunit I (COI) and ribosomal 16S RNA (16S) were amplified and sequenced. Phylogenetic analyses were performed using maximum likelihood (ML) and Bayesian inferences (BA) for both molecular data (COI and 16S). The sampled T. obscurus populations corresponded to two distinct mtDNA lineages (genetic distance COI K2 P = 0.08 to 0.13; 16S K2 P = 0.10 to 0.11) with no shared mutations between groups and well supported by ML and BA inferences. Based on the divergence values found between eastern and western populations (COI, 0.07 to 0.12; 16S, 0.10), our study confirms the genetic heterogeneity of T. obscurus populations within the state of Pará, which correlates with observed venom and clinical differences, and reinforces the need for mapping the distribution of haplotypes throughout the geographic range of T. obscurus, to aid in future epidemiological, toxinological, and evolutionary studies.

RESUMO O envenenamento por escorpiões é considerado um problema de saúde pública no Brasil. Um estudo recente descreve a variação do quadro clínico de envenenamento por Tityus obscurus em duas populações separadas por uma distância de 850 km no nordeste da Amazônia. O objetivo deste estudo foi avaliar se tais variações clínicas e toxicológicas estão associadas a diferenças subjacentes na diversidade genética entre essas duas populações de T. obscurus. Obtivemos DNA de cinco indivíduos de cada população nos municípios de Belém e Santarém, localizados a leste e oeste do estado do Pará, Brasil, respectivamente. Regiões gênicas que codificam marcadores de DNA mitocondrial (mtDNA) citocromo oxidase subunidade I (COI) e RNA ribossômico 16S (16S) foram amplificadas e sequenciadas. As análises filogenéticas foram realizadas por máxima verossimilhança (ML) e inferência Bayesiana (BA) para ambos os dados moleculares (COI e 16S). As populações de T. obscurus amostradas corresponderam a duas linhagens distintas de mtDNA (distância genética COI K2 P = 0,08 a 0,13; 16S K2 P = 0,10 a 0,11) sem mutações compartilhadas entre os grupos, e bem corroboradas por inferências ML e BA. Com base nos valores de divergência encontrados entre as populações oriental e ocidental (COI, 0,07 a 0,12; 16S, 0,10), nosso estudo confirma a heterogeneidade genética das populações de T. obscurus no estado do Pará. Os resultados são congruentes com as diferenças observadas no quadro clínico dos acidentes e toxicidade do veneno, e reforçam a necessidade de mapear a distribuição de haplótipos em toda a distribuição geográfica de T. obscurus, para auxiliar em futuros estudos epidemiológicos, toxinológicos e evolutivos.

Abstract Objective Testing an experimental model for ischemic necrosis of the femoral head in Legg-Calvé-Perthes disease by evaluating gait, imaging and morphohistology. Methods The operation was done in 11 piglets. Necrosis by cerclage in the right femoral neck was induced. Piglets were divided into group A, with 8 animals, euthanizing two in the 2nd, 4th, 6th, and 8th weeks, respectively; and group B, with 2 animals (sham), submitted to the surgical procedure without cerclage of the right femoral neck. The gait classification used was that of Etterlin. The frozen femurs were submitted to digital radiography and computed tomography. The height and width of the epiphysis and epiphysary coefficient were measured at study times. Light microscopy and immunohistochemistry with TGF-β1 were performed. Results One animal died of sepsis in Group A. In this group, claudication was observed in all animals. On digital radiography and computed tomography, bone sclerosis, enlargement of the right femoral neck, flattening, collapse, and fragmentation of the right femoral head were observed. All epiphysis height and epiphysary coefficient values of the right femoral head were lower than the contralateral ones, in which were observed chondrocytes disordered and separated by gaps. A reduction in TGF-β1 expression was observed at 2 and 6 weeks in the right femoral head and at eight in the left. In group B, there were no signs of necrosis and gait was normal. Conclusions The model presented reproduced macroscopic necrosis on digital radiography, computed tomography, and microscopy. Gait evaluation showed a good correlation with other ischemia findings. Resumo Objetivo Testar um modelo experimental para necrose isquêmica da cabeça femoral na doença de Legg-Calvé-Perthes avaliando a marcha, exames de imagens e morfohistologia. Métodos Operaram-se 11 leitões. Induziu-se a necrose por cerclagem no colo femoral direito. Dividiram-se os leitões em grupo A com 8 animais, sacrificando-se dois na 2ª, 4ª, 6ª e 8ª semanas, respectivamente; e grupo B, com 2 animais (sham), submetidos ao procedimento cirúrgico sem a cerclagem do colo femoral direito. A classificação da marcha utilizada foi a de Etterlin. Os fêmures congelados foram submetidos à radiografia digital e tomografia computadorizada. Mediram-se a altura e largura da epífise e o coeficiente epifisário nos tempos de estudo. Realizou-se, microscopia de luz e imunohistoquímica com TGF-β1. Resultados Um animal morreu por sepse no grupo A. Neste grupo, observou-se claudicação em todos os animais. Na radiografia digital e tomografia computadorizada observaram-se: esclerose óssea, alargamento do colo femoral direito, achatamento, colapso e fragmentação da cabeça femoral direita. Todos os valores da altura da epífise e coeficiente epifisário da cabeça femoral direita foram menores que os contralaterais, nos quais observaram-se condrócitos desordenados e separados por lacunas. Observou-se redução da expressão do TGF-β1 com 2 e 6 semanas nas cabeças femorais direitas e nas esquerdas com oito. No grupo B, não ocorreram sinais de necrose e a marcha foi normal. Conclusões O modelo apresentado reproduziu a necrose macroscopicamente, na radiografia digital, tomografia computadorizada e microscopia. A avaliação da marcha demonstrou boa correlação com os demais achados de isquemia. Nível de EvidênciaV. Estudos diagnósticos.

ABSTRACT Background: Critical illness is a major ongoing health care burden worldwide and is associated with high mortality rates. Sodium-glucose cotransporter-2 inhibitors have consistently shown benefits in cardiovascular and renal outcomes. The effects of sodium-glucose cotransporter-2 inhibitors in acute illness have not been properly investigated. Methods: DEFENDER is an investigator-initiated, multicenter, randomized, open-label trial designed to evaluate the efficacy and safety of dapagliflozin in 500 adult participants with acute organ dysfunction who are hospitalized in the intensive care unit. Eligible participants will be randomized 1:1 to receive dapagliflozin 10mg plus standard of care for up to 14 days or standard of care alone. The primary outcome is a hierarchical composite of hospital mortality, initiation of kidney replacement therapy, and intensive care unit length of stay, up to 28 days. Safety will be strictly monitored throughout the study. Conclusion: DEFENDER is the first study designed to investigate the use of a sodium-glucose cotransporter-2 inhibitor in general intensive care unit patients with acute organ dysfunction. It will provide relevant information on the use of drugs of this promising class in critically ill patients. ClinicalTrials.gov registry: NCT05558098

RESUMO Antecedentes: A doença crítica é um importante ônus permanente da assistência médica em todo o mundo e está associada a altas taxas de mortalidade. Os inibidores do cotransportador de sódio-glicose do tipo 2 têm demonstrado consistentemente benefícios nos desfechos cardiovasculares e renais. Os efeitos dos inibidores do cotransportador de sódio-glicose do tipo 2 em doenças agudas ainda não foram devidamente investigados. Métodos: O DEFENDER é um estudo de iniciativa do investigador, multicêntrico, randomizado, aberto, desenhado para avaliar a eficácia e a segurança da dapagliflozina em 500 participantes adultos com disfunção orgânica aguda hospitalizados na unidade de terapia intensiva. Os participantes aptos serão randomizados 1:1 para receber 10mg de dapagliflozina e o tratamento padrão por até 14 dias ou apenas o tratamento padrão. O desfecho primário é um composto hierárquico de mortalidade hospitalar, início de terapia renal substitutiva e tempo de internação na unidade de terapia intensiva, até 28 dias. O monitoramento da segurança será rigoroso durante todo o estudo. Conclusão: O DEFENDER é o primeiro estudo desenvolvido para investigar o uso de um inibidor do cotransportador de sódio-glicose do tipo 2 em pacientes de unidade de terapia intensiva geral com disfunção orgânica aguda. ABSTRACT: The present study proposed to describe, through a literature review, the use of new therapeutic management which allows for offering a better quality of life to individuals affected by these pathologies. A bibliographic search was conducted in the main health databases PUBMED (www.pubmed.gov) and Scholar Google (www.scholar.google.com.br), in which studies published from 1987 to 2023 were collected. In the first stage, the list of retrieved articles was examined by reading the titles and abstracts. In the second stage, the studies were selected by reading the full contents. Two authors (JDMM and DAQ) performed stages 1 and 2. Experimental, clinical, case-control, randomized controlled, and laboratory cohort studies, case reports, systematic reviews, and literature reviews, which were developed in living individuals, were included. Therefore, articles that did not deal with the subject in question, letters to the editor, opinion articles, duplicated literature in databases, and literature that did not address the variables under study, we re excluded. Contemporary dentistry uses alternative treatments capable of improving the patient's condition since a cure is not always possible. Therefore, the possibility of improving the quality of life becomes an important point to be reached. Evidence-based healthcare has made great advances in recent decades, especially in the areas of orofacial pain, TMD, and occlusion, especially related to orthodontic, prosthetic, and restorative care.

RESUMEN: En el presente estudio se propuso describir, a través de una revisión bibliográfica, el uso de nuevos manejos terapéuticos que permitan brindar una mejor calidad de vida a los individuos afectados por estas patologías. Se realizó una búsqueda bibliográfica en las principales bases de datos de salud PUBMED (www.pubmed.gov) y Scholar Google (www.scholar.google.com.br), en las que se recopilaron estudios publicados entre 1987 y 2023. En la primera etapa, se examinó la lista de artículos recuperados mediante la lectura de los títulos y resúmenes. En la segunda etapa, los estudios fueron seleccionados mediante la lectura del contenido completo. Dos autores (JDMM y DAQ) realizaron las etapas 1 y 2. Se incluyeron estudios de cohortes experimentales, clínicos, de casos y controles, controlados aleatorios y de laboratorio, informes de casos, revisiones sistemáticas y revisiones de la literatura, que se desarrollaron en individuos vivos. Por lo tanto, se excluyeron artículos que no trataran el tema en cuestión, cartas al editor, artículos de opinión, literatura duplicada en bases de datos y literatura que no abordara las variables en estudio. La odontología contemporánea utiliza tratamientos alternativos capaces de mejorar el estado del paciente, ya que no siempre es posible la curación. Por lo tanto, la posibilidad de mejorar la calidad de vida se convierte en un objetivo importante. La atención médica basada en la evidencia ha logrado grandes avances en las últimas décadas, especialmente en las áreas de dolor orofacial, TMD y oclusión, especialmente en relación con la atención de ortodoncia, prótesis y restauración.
