Abstract Escherichia coli causes colibacillosis in farm animals that act as reservoirs for pathogenic strains. Antimicrobial resistance of E. coli producing Extended Spectrum Beta-lactamases [ESBL] is a serious public health problem that can be attributed to factors related to food consumption and contact with domestic animals. This study aimed to determine the presence and patterns of antimicrobial resistance of ESBL E. coli isolated from fecal samples from dairy cattle in the Pichincha province. A total of 182 bovine feces samples were analyzed, 112 samples from cattle slaughtered at the official slaughterhouse in Quito-Pichincha province, and 70 samples from the collection of the Foodborne Diseases and Antimicrobial Resistance Research Unit [UNIETAR]. The isolation of ESBL E. coli, biochemical identification, and resistance tests using the main antibiotics were carried out at UNIETAR. It was possible to identify 93 positive samples for ESBL E. coli (51%), phenotypic analysis revealed that antibiotics such as amoxicillin plus clavulanic acid, cefepime, ceftazidime, ciprofloxacin, amikacin, tetracycline, presented resistance higher than 80%. Furthermore, low resistance to nitrofurantoin, cefoxitin, and ertapenem was observed, while no isolate was resistant to tigecycline. One hundred percent of the isolates presented multi-resistance phenotypes, with the most frequent pattern being composed of 7 families of antibiotics. In conclusion, these results suggest that E. coli from dairy cattle origin could be an important reservoir of ESBL genes.

Resumen La bacteria Escherichia coli causa la colibacilosis en animales de granja que actúan como reservorios de cepas patógenas. La resistencia antimicrobiana de E. coli productor de betalactamasas de espectro extendido [BLEE] es un grave problema de salud pública y se puede atribuir a factores relacionados con el consumo de alimentos y el contacto con animales domésticos. El objetivo de este estudio fue determinar la presencia y patrones de resistencia antimicrobiana de E. coli BLEE aislado en muestras fecales provenientes de bovinos productores de leche de la provincia de Pichincha. Se analizaron un total 182 muestras de heces de bovinos: 112 muestras de bovinos faenados en el Camal Metropolitano de la provincia de Pichincha y 70 muestras de la colección de la Unidad de Investigación de Enfermedades Transmitidas por Alimentos y Resistencias a los Antimicrobianos [UNIETAR], se realizó el aislamiento de E. coli BLEE, la identificación bioquímica y pruebas de resistencia a los principales antibióticos utilizados. Se logró identificar 93 muestras positivas a E. coli BLEE (51 %), el análisis fenotípico reveló que los antibióticos amoxicilina más ácido clavulánico, cefepime, ceftazidima, ciprofloxacina, amikacina y tetraciclina presentaron porcentajes de resistencia mayores al 80 %. Además, se observó una baja resistencia a la nitrofurantoína, cefoxitin y ertapenem, mientras que ningún aislado fue resistente a la tigeciclina. El 100 % de los aislados presentaron fenotipos de multirresistencia y el patrón más frecuente estuvo compuesto por 7 familias de antibióticos. En conclusión, estos resultados sugieren que E. coli originaria de bovinos lecheros podría ser un reservorio de genes BLEE.

Resumo Avaliou-se a puericultura na ESF em município-sede de macrorregião de saúde do Nordeste brasileiro. Realizou-se estudo avaliativo com dois níveis de análise: a gestão municipal e local. Aprofundou-se a avaliação local em quatro EqSF mediante entrevistas com gestores, profissionais e puérperas, análise documental e observação não-participante. As fontes de dados foram trianguladas e processadas no software Nvivo, e o material empírico cotejado com uma situação-objetivo segundo os níveis de análise da pesquisa e elaborada pelos pesquisadores. No nível municipal, a definição clara de atribuições para os pontos de atenção em puericultura e fluxos assistenciais foi um dos critérios mais críticos e, no âmbito local, a integração de ações com outros serviços primários, a contrarreferência e ações educativas coletivas no território. A implementação das práticas de puericultura nas EqSF é fortemente dependente dos determinantes político-organizacionais entre o âmbito municipal e local, e os resultados aqui evidenciados podem se aplicar a outros sistemas municipais de saúde com configurações semelhantes. Este estudo contribuiu para a avaliação da puericultura sustentada na proposição de critérios que consideraram a interdependência municipal e local na organização das ações de avaliação do crescimento e desenvolvimento infantil.

This study contributed to the evaluation of childcare based on the proposition of criteria that considered municipal and local interdependence in the organization of actions to evaluate child growth and development.

Resumo: Ao compreender a mediação da leitura como uma atividade que exige do mediador uma atuação consciente e humanizadora, é necessário refletir sobre a importância de utilizar e articular os diversos dispositivos informacionais que podem auxiliar esse processo. Este estudo teve como objetivo evidenciar a utilização e a articulação de diferentes dispositivos informacionais no processo de mediação da leitura desenvolvido no espetáculo cênico-musical Crianceiras. Quanto à metodologia, a pesquisa configura-se como descritiva e teve como método o estudo de caso. Os dados foram coletados através de um questionário online e analisados com base na abordagem qualitativa. A análise dos resultados indicou que, ao articular diversos dispositivos informacionais, o Crianceiras tem conseguido alcançar o objetivo de envolver diferentes sujeitos nas atividades de mediação da leitura, de modo a apoiar esses sujeitos-leitores em sua formação e desenvolvimento crítico e cognitivo. Assim, é muito importante que o mediador da leitura considere e adote, em suas práticas, os diversos dispositivos informacionais para ampliar a ambiência que repercutirá na formação de leitores proficientes. Abstract Understanding the mediation of reading as an activity that requires a conscious and humanizing acting from the mediator, it is necessary to reflect on the importance of using and articulating the various informational devices that can help in this process. This study aimed to highlight the use and articulation of different informational devices in the process of mediation of reading developed in the Crianceiras scenic-musical spectacle. As for the methodology, this is a descriptive research and had the case study as a method. The data were collected through an online questionnaire and analyzed based on a qualitative approach. The analysis of the results inidcated that to the perception that by articulating various informational devices, the Crianceiras project has managed to reach the objective of involving different subjects-readers in mediation of reading activities, supporting them in their critical and cognitive development. Resumo Introdução Durante a pandemia de Covid-19, o uso do álcool a 70° GL foi amplamente recomendado para a higienização das mãos e superfícies. Acoplada à maior permanência das pessoas dentro do domicílio, essa recomendação pode contribuir para o aumento do risco de queimaduras. Objetivo Delinear os riscos de queimaduras no contexto da pandemia. Método Revisão por meio de análise documental. Resultados O álcool líquido 70° GL e o álcool isopropílico 70° GL são substâncias que apresentam maior risco de ignição no uso e armazenamento. O álcool 70° GL em gel deve ser priorizado tanto pela maior facilidade de uso para antissepsia das mãos como pelo menor risco de inflamabilidade. Esses materiais não devem ser manuseados durante o preparo de alimentos. Conclusão As autoridades sanitárias e governamentais devem se responsabilizar pela divulgação dos cuidados necessários para prevenção de queimaduras e incêndios no contexto da pandemia.

Abstract Background During the Covid-19 pandemic, the use of alcohol at 70° GL was widely recommended for hand and surface hygiene. Combined with greater permanency of individuals in the household, this recommendation may contribute to an increased risk of burns. Objective To outline the risks of burns in the context of the pandemic. Method A review through document analysis was carried out. Results Liquid alcohol 70° GL and isopropyl alcohol 70° GL are substances that present a greater risk of ignition during use and storage. Alcohol 70° GL in gel should be prioritized both for its greater easier of use for hand antisepsis and for its lower risk of flammability. These materials must not be handled during food preparation. Conclusion Health and government authorities must be responsible for disseminating the necessary care to prevent burns and fires in the context of the pandemic.

Resumo O objetivo deste artigo é descrever a distribuição de Centros Transplantadores (CTs) e transplantes de células-tronco hematopoiéticas (TCTH) no território brasileiro. Estudo descritivo, que reúne informações sobre a distribuição CTs e o número de procedimentos realizados entre 2001 e 2020, a partir das fontes dos dados: Sociedade Brasileira de Terapia celular e Transplantes de Medula Óssea (SBTMO); Associação Brasileira de Transplante de Órgãos (ABTO); Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH/SUS); e Ministério da Saúde (MS). Foram identificados 86 CTs, com predominância na região Sudeste do país (64%). A região Norte não possui CTs. No período contabilizaram-se mais de 30 mil procedimentos, concentrados nas regiões Sudeste e Sul. O TCTH do tipo alogênico foi prevalente. Constataram-se divergências entre os números de transplantes realizados a depender da fonte consultada. Apesar do crescimento do número de procedimentos no período do estudo, tanto a distribuição de CTs quanto o número de TCTHs se concentrou em regiões mais desenvolvidas. Essa heterogeneidade pode ter propiciado iniquidades no acesso ao tratamento pela população.

Abstract The scope of this article is to describe the distribution of Transplant Centers (TCs) and hematopoietic stem-cell transplants (HSCTs) in the Brazilian territory. It is a descriptive study, which brings together information on the distribution of TCs and the number of procedures performed between 2001 and 2020, based on the following data sources: the Brazilian Cell Therapy and Bone Marrow Transplant Society of (SBTMO); the Brazilian Organ Transplant Association (ABTO); the Hospital Information System of the Unified Health System (SIH/SUS); and the Ministry of Health (MS). A total of 86 TCs were identified, predominantly in the Southeastern region of the country (64%). There are no TCs in the Northern region. Throughout the period, there were more than 30,000 procedures, concentrated in the Southeastern and Southern regions. The allogeneic type of HSCT was prevalent. Differences were found between the numbers of transplants performed depending on the source consulted. Despite the increase in the number of procedures during the period studied, both the distribution of TCs and the number of HSCTs were concentrated in more developed regions. This heterogeneity may have led to inequities in the access of the population to treatment.

RESUMEN Introducción: Se han investigado y descrito numerosos tipos de artritis con el fin de clasificarlos en artritis no inflamatoria (osteoartritis) y artritis inflamatoria causada por cristales de depósito (seudogota, enfermedad del fosfato de calcio básico y gota), por bacterias e infecciones virales (Staphylococcus aureus, Neisseria gonorrohae, complicaciones de la enfermedad de Lyme, Parvovirus y Enterovirus) o por procesos autoinmunes. Objetivo: Presentar una visión global de la artritis reumatoide, con énfasis en el diagnóstico, fisiopatología y biomarcadores diagnósticos, pronósticos y predictivos en la artritis reumatoide, con el fin de contribuir la mejor atención de pacientes con enfermedades crónicas. Método: Se realizó una búsqueda bibliográfica relacionada con las palabras clave “artritis reumatoide” y otras variables de artritis. Se recuperaron 76 documentos que las incluían, de ellos utilizamos 42 que fueron útiles para el objetivo de la investigación. Conclusiones: El primer paso para un manejo efectivo de la enfermedad es un diagnóstico temprano y correcto, relacionado con que varios signos y síntomas también están asociados con otras enfermedades. Se debe incluir para este objetivo las estrategias de prevención de enfermedades, así como los programas de detección de personas en riesgo de desarrollarlas como el caso de la artritis reumatoide y la información de la enfermedad proporcionada a la población que pueden mejorar significativamente los parámetros epidemiológicos.

ABSTRACT Introduction: Numerous types of arthritis have been investigated and described in order to classify them into non-inflammatory arthritis (osteoarthritis) and inflammatory arthritis caused by deposition crystals (pseudogout, basic calcium phosphate disease, gout), by bacteria and viral infections (Staphylococcus aureus, Neisseria gonorrohae, complications of Lyme disease, Parvovirus, Enterovirus) or autoimmune processes. Objective: research is to present aspects related to the management of Digital Ulcers (DU) in scleroderma based on the experience of different authors, establishing a basis on which future research studies can be designed and improve the patient's quality of life. Method: A bibliographic search related to the keywords rheumatoid arthritis and other arthritis variables was carried out, recovering 76 documents that included them, of which we used 42 that were useful for the objective that we proposed, exposing the most prominent data in the investigation. Conclusions: The first step for an effective management of the disease is an early and correct diagnosis, related to the fact that several signs and symptoms are also associated with other diseases, disease prevention strategies as well as prevention programs should be included for this objective. detection of people at risk of developing them, as in the case of rheumatoid arthritis, monographs of the disease provided to the population can significantly improve epidemiological parameters.

Resumo O objetivo deste trabalho foi avaliar a resposta das cultivares de feijão (Phaseolus vulgaris) Triunfo, Garapiá e BRS-FC104 ao déficit hídrico representado pela fração de água transpirável no solo (FATS). O delineamento experimental foi inteiramente casualizado, em arranjo bifatorial 3×2 (cultivares × irrigação e sem irrigação). Avaliaram-se a transpiração, o crescimento e a produtividade na safra e na safrinha. Na safra, com baixa demanda atmosférica do ar (inferior a 15 hPa em 55% dos dias), 'Garapiá' apresentou fechamento precoce dos estômatos com FATS crítica (FATSc) de 0,36, mas teve a mesma produtividade que 'Triunfo', cujos estômatos fecharam tardiamente (FATSc = 0,23). Na safrinha, com alta demanda atmosférica do ar (superior a 15 hPa em 83,3% dos dias), 'Garapiá' apresentou fechamento estomático precoce (FATSc = 0,17) e maior produtividade (392,2 kg ha−1 a mais que 'Triunfo'). Na safra e na safrinha, 'Garapiá' apresentou tolerância ao déficit hídrico no período reprodutivo. O déficit hídrico resultou em menores valores de estatura, massa seca da parte aérea, massa seca da raiz e área foliar. Tanto 'Garapiá' como 'Triunfo' apresentaram alta produtividade na safra, mas apenas Garapiá na safrinha. 'Garapiá' apresenta tolerância ao déficit hídrico, com controle estomático eficiente e com altos crescimento e produtividade.

Abstract The objective of this work was to evaluate the response of the Garapiá, Triunfo, and BRS-FC104 bean (Phaseolus vulgaris) cultivars to the water deficit represented by the fraction of transpirable soil water (FTSW). The experimental design was completely randomized in a 3×2 bifactorial arrangement (cultivars × irrigation and no irrigation). Transpiration, growth, and yield were evaluated in the crop season and off-season. In the crop season, with a low atmospheric air demand (below 15 hPa in 55% of the days), 'Garapiá' presented early stomatal closure with the critical FTSW (FTSWc) of 0.36, but had the same yield as 'Triunfo', whose stomata closed late (FTSWc = 0.23). In the off-season, with a high atmospheric air demand (greater than 15 hPa in 83.3% of the days), 'Garapiá' presented an early stomatal closure (FTSWc = 0.17) and a higher yield (392.2 kg ha-1 more than 'Triunfo'). In the crop season and off-season, 'Garapiá' showed tolerance to water deficit in the reproductive period. Water deficit resulted in lower values of height, shoot dry mass, root dry mass, and leaf area. Both 'Garapiá' and 'Triunfo' showed a high yield in the crop season, but only 'Garapiá' in the off-season. 'Garapiá' presents tolerance to water deficit, with efficient stomatal control and high growth and yield.

RESUMO CONTEXTO: É recomendado alimentação saudável para pacientes com doença de Crohn (DC) em remissão. OBJETIVO: Avaliar a qualidade da dieta de pacientes com DC. MÉTODOS: Estudo transversal com pacientes com DC em remissão clínica e em uso do imunobiológico infliximabe. A qualidade da dieta foi avaliada pelo índice de qualidade da dieta revisado (IQD-R). O IQD-R foi calculado a partir do recordatório 24 horas, sendo classificado em “dieta inadequada” (≤40 pontos), “dieta que requer modificações” (41 a 64 pontos) e “dieta saudável” (≥65 pontos). Os pacientes foram avaliados quanto ao peso, altura e circunferência da cintura (CC). Para comparação entre grupos foi utilizado o test-t de Student ou Mann-Whitney. Para correlação entre variáveis contínuas foi utilizado o coeficiente de Pearson ou Spearman. Valores de P<0,05 indicaram significância estatística. RESULTADOS: Participaram do estudo 43 pacientes. A pontuação final do IQD-R foi de 49,1 pontos - “dieta que requer modificações”. Nenhum paciente recebeu a classificação de “dieta saudável” (pontuação máxima =59,7), 55,8% apresentaram “dieta que requer modificações” e 44,2% “dieta inadequada”. Ao comparar os grupos “dieta inadequada” e “dieta que requer modificações”, foi observado menor média de idade no grupo “dieta inadequada” (37,6±14,8 versus 47,4±10,5 anos, P=0,02). Verificou-se que 44,2% dos pacientes estavam acima do peso (índice de massa corporal (IMC) ≥25 kg/m²) e possuíam CC aumentada (mulheres: CC ≥80 cm e homens: CC ≥94 cm). Foi encontrada correlação positiva entre a pontuação final do IQD-R e o IMC (P=0,046; r=0,346). CONCLUSÃO: Os pacientes com DC em remissão clínica e em uso de infliximabe não estão adotando dieta com qualidade considerada saudável o que aponta a necessidade de abordagem nutricional individualizada.

ABSTRACT BACKGROUND: A healthy diet is recommended for patients with Crohn’s disease (CD) in remission. OBJECTIVE: To evaluate the diet quality of patients with CD. METHODS: Cross-sectional study with patients with CD and clinical remission using the biological agent infliximab. The diet quality was assessed using the Diet Quality Index-Revised (DQI-R). DQI-R was calculated based on 24-hour dietary recalls (24HR), being classified as “inadequate diet” (≤40 points), “diet requiring modifications” (41 to 64 points) and “healthy diet” (≥65 points). Weight, height and waist circumference (WC) of patients were assessed. For comparison between groups, Student’s t-test or Mann-Whitney was used. For correlation between continuous variables, Pearson or Spearman coefficient was used. Values of P<0.05 indicated statistical significance. RESULTS: A total of 43 patients participated in the study. The final DQI-R score was 49.1 points - “diet requiring modifications”. No patient received the classification of “healthy diet” (maximum score =59.7), 55.8% presented “diet requiring modifications” and 44.2% “inadequate diet”. When comparing the “inadequate diet” and “diet requiring modifications” groups, a lower mean age was observed in the “inadequate diet” group (37.6±14.8 versus 47.4±10.5 y, P=0.02). It was found that 44.2% of the patients were overweight (body mass index [BMI] ≥25 kg/m²) and had increased WC (women: WC ≥80 cm and men: WC ≥94 cm). A positive correlation was found between the final DQI-R score and BMI (P=0.046; r=0.346). CONCLUSION: Patients with CD in clinical remission using infliximab are not adopting a diet considered healthy, which points to the need for an individualized nutritional approach.

OBJECTIVES: Since there are difficulties in establishing effective treatments for COVID-19, a vital way to reduce mortality is an early intervention to prevent disease progression. This study aimed to evaluate the performance of patients with COVID-19 with acute hypoxic respiratory failure according to pulmonary impairment in the awake-prone position, outside of the intensive care unit (ICU). METHODS: A prospective observational cohort study was conducted on COVID-19 patients under noninvasive respiratory support. Clinical and laboratory data were obtained for each patient before the treatment and after they were placed in the awake-prone position. To identify responders and non-responders after the first prone maneuver, receiver operating characteristic curves with sensitivity and specificity of the PaO2/FiO2 and SpO2/FiO2 indices were analyzed. The maneuver was considered positive if the patient did not require endotracheal intubation for ventilatory assistance. RESULTS: Forty-eight patients were included, and 64.6% were categorized as responders. The SpO2/FiO2 index was effective for predicting endotracheal intubation in COVID-19 patients regardless of lung parenchymal damage (area under the curve 0.84, cutoff point 165, sensitivity 85%, specificity 75%). Responders had better outcomes with lower hospital mortality (hazard ratio [HR]=0.107, 95% confidence interval [CI]: 0.012-0.93) and a shorter length of stay (median difference 6 days, HR=0.30, 95% CI: 0.13-0.66) after adjusting for age, body mass index, sex, and comorbidities. CONCLUSIONS: The awake-prone position for COVID-19 patients outside the ICU can improve oxygenation and clinical outcomes regardless of the extent of pulmonary impairment. Furthermore, the SpO2/FiO2 index discriminates responders from non-responders to the prone maneuver predicting endotracheal intubation with a cutoff under or below 165.
