RESUMEN Introducción: La Pandemia COVID-19, ha tenido impactos negativos en la salud física y mental de las personas, así como las medidas adoptadas por los gobiernos, para prevenir el contagio masivo de la población como el confinamiento, el aislamiento social, el trabajo y educación virtual. Dentro de los afectados por estos cambios, se encontrarían los estudiantes universitarios del área de la salud, que además de estudiar en estas condiciones de pandemia, no han podido hacer sus prácticas de los servicios de salud. Esta situación de exposición al aislamiento, podría afectar la salud mental de los jóvenes estudiantes universitarios. Objetivo: determinar los niveles de depresión, ansiedad y estrés en estudiantes universitarios y su relación con variables sociodemográficas y las características del aislamiento social durante la Pandemia COVID-19. Métodos: Se realizó un estudio descriptivo, observacional, de corte transversal y cuantitativo, con una muestra de 818 estudiantes universitarios de facultades de ciencias de la salud, mediante el cuestionario autoadministrado DASS-21. Resultados: Los principales hallazgos fueron: una mayor prevalencia en niveles altos (severos y extremadamente severos) en la ansiedad, alcanzado casi a 4 de cada 10 de los evaluados; por otro lado, en depresión y el estrés, 2 de cada 10 de los evaluados, se encontraron en los niveles mencionados. En relación con las variables sociodemográficas, las relacionadas con el COVID-19 y aislamiento social, se hallaron diferencias estadísticamente significativas con el género (mujeres > varones), el haber tenido COVID-19 (Sí > No) y vivir con familiares como antes de la pandemia (Sí > No) obtuvieron en promedio mayores puntajes en depresión, ansiedad y estrés. Conclusión: Los estudiantes que presentaron mayor depresión, ansiedad y estrés con una diferencia significativa fueron de género femenino, con antecedentes de haber tenido Covid-19, menor contacto con familiares y haber cumplido con aislamiento social total.

ABSTRACT Introduction: The COVID-19 Pandemic has had negative impacts on the physical and mental health of people, as well as the measures adopted by governments to prevent the massive contagion of the population, such as confinement, social isolation, virtual work, and virtual education. Among those affected by these changes would be university students in the health area who, in addition to studying in these pandemic conditions, have been unable to do their health service practices. This situation of exposure to isolation could affect the mental health of young university students. Objective: To determine the levels of depression, anxiety, and stress in university students, their relationship with sociodemographic variables, and the characteristics of social isolation during the COVID-19 Pandemic. Methods: A descriptive, observational, cross-sectional, and quantitative study was carried out with a sample of 818 university students from health sciences faculties using the DASS-21 self-administered questionnaire. Results: The main findings were a higher prevalence of high levels (severe and highly severe) in anxiety, reaching almost 4 out of 10 of those evaluated; On the other hand, in depression and stress, 2 out of 10 of those evaluated were found at the mentioned levels. Concerning the sociodemographic variables, those related to COVID-19 and social isolation, statistically significant differences were found with gender (women > men), having had COVID-19 (Yes > No), and living with relatives as before. On average, the pandemic (Yes > No) obtained higher scores in depression, anxiety, and stress. Conclusion: The students who presented greater depression, anxiety, and stress with a significant difference were female, with a history of having had Covid-19, less contact with family members, and having complied with total social isolation

Resumen: Objetivo: La fijación transpedicular ha evolucionado en los últimos 100 años. Una complicación común es el desplazamiento de tornillos, causante de déficits neurológicos o daños vasculares. Buscamos correlacionar el desplazamiento con síntomas radiculares mediante la escala de Gertzbein. Métodos: Se realizó un estudio observacional longitudinal retrospectivo de pacientes sometidos a cirugía de instrumentación lumbar a mano alzada asistida por fluoroscopia. Los pacientes fueron evaluados con tomografía computarizada postoperatoria y las posiciones de los tornillos se clasificaron con la escala de Gertzbein. Resultados: La muestra inicial incluyó 99 pacientes. De los 317 tornillos colocados, 201 no mostraron daño cortical, 105 mostraron invasión variable y 11 mostraron invasión severa. El 96,5% de los tornillos se colocaron en la zona segura, y el 8,6% de los pacientes mostraron debilidad transitoria. Se observó un 3,47% de tornillos con invasión grave en 7 pacientes, 2 de los cuales sufrieron discapacidad motora y dolor radicular persistente. Conclusión: Es crucial prestar atención a la inserción precisa de los tornillos para minimizar el riesgo de manifestaciones radiculares. Recomendamos la realización de tomografias de control para asegurar la correcta inserción de los tornillos y, en caso necesario, su recolocación debido al elevado riesgo de daño neurológico. ABSTRACT Objective: Transpedicular fixation has rapidly evolved over the last 100 years. A common complication is screw misplacement which can lead to neurological deficits, vascular damage, or organ perforation. We intend to assess the correlation between screw misplacement and radicular symptoms through the Gertzbein scale. Methods: We conducted an observational longitudinal retrospective study on patients who underwent free-hand lumbar instrumentation surgery with fluoroscopy assistance. The patients were evaluated with postoperative CT scans, and screw positions were classified with the Gertzbein scale. Results: The initial sample included 99 patients who underwent surgery. Of the 317 screws placed, 201 did not show cortical damage, 105 screws exhibited variable invasion, and 11 screws displayed severe invasion. 96.5% screws were placed in the safe zone, with 8.6% of patients (n=5.0) exhibiting transitory weakness. 3.47% of screws (n=11) with severe invasion were seen in 7 patients of which two patients suffered from motor deficient and persistent radicular pain. Conclusion: It is of the utmost importance to pay attention to the precise insertion of the screws to minimize the risk of radicular manifestations. We recommend performing control CT scans after the procedure to ensure the correct insertion of the screws, and in case of finding a screw in a no-safe zone or Getsbein 3 position, considering screw repositioning due to high-risk neurologic damage is highly encouraged. Pretendemos correlacionar o deslocamento do parafuso com sintomas radiculares usando a escala de Gertzbein. Métodos: Conduzimos um estudo retrospectivo longitudinal observacional em pacientes submetidos à cirurgia de instrumentação lombar à mão livre com assistência de fluoroscopia. Os pacientes foram avaliados com tomografia computadorizada pós-operatória e as posições dos parafusos foram classificadas com a escala de Gertzbein. Resultados: A amostra inicial incluiu 99 pacientes. Dos 317 parafusos colocados, 201 não mostraram danos corticais, 105 exibiram invasão variável e 11 invasão severa. 96,5% dos parafusos foram colocados na zona segura, com 8,6% dos pacientes apresentando fraqueza transitório. 3,47% dos parafusos com invasão severa foram observados em 7 pacientes, dos quais 2 sofreram de deficiência motora e dor radicular persistente. Resumen Antecedentes La arteritis es una complicación de la neurocisticercosis (NCC), que no siempre se conoce y podría desencadenar enfermedad cerebrovascular. La ultrasonografía Doppler transcraneal (DTC) es un método no invasivo que sirve para detectar y monitorizar enfermedades cerebrovasculares. No obstante, la utilidad de la DTC para evaluar los cambios hemodinámicos cerebrales que sugieren vasculitis asociada a NCC sigue siendo incierta. Objetivo Evaluar los cambios hemodinámicos cerebrales utilizando DTC en pacientes con NCC subaracnoidea y parenquimal. Métodos Se incluyeron 53 pacientes con NCC (29 con subaracnoidea y 24 con parenquimal) evaluados en un hospital de referencia para enfermedades neurológicas. Los participantes se sometieron a una entrevista clínica y serología para cisticercosis y a una DTC realizada dentro de las 2 semanas posteriores a la inscripción. Se registraron la velocidad media del flujo, la velocidad sistólica máxima, la velocidad diastólica final y el índice de pulsatilidad. Resultados Los participantes incluyeron 23 (43,4%) mujeres con una mediana de edad de 37 años (rango intercuartílico [RIC]: 29–48). Se detectaron cambios hemodinámicos cerebrales sugestivos de vasculitis en 12 pacientes (22,64%); el vaso más comprometido fue la arteria cerebral media, en 11 (91,67%) pacientes. Hubo más mujeres en el grupo con signos ecográficos de vasculitis (10/12, 83,33% versus 13/41, 31,71%; p = 0,002), y esto fue más frecuente en el grupo de NCC subaracnoidea (9/29, 31,03% versus 3/24, 12,5%; p = 0,187), aunque esta diferencia no alcanzó significancia estadística. Conclusión Los cambios hemodinámicos cerebrales sugestivos de vasculitis son frecuentes en pacientes con NCC y pueden evaluarse mediante DTC. Abstract Background Arteritis is a complication of neurocysticercosis (NCC), which is not well known and could trigger strokes. The transcranial Doppler ultrasound (TCD) is a noninvasive method for detecting, staging, and monitoring cerebrovascular diseases. Nonetheless, the utility of TCD to evaluate cerebral hemodynamic changes, suggesting vasculitis associated with NCC remains uncertain. Objective To evaluate cerebral hemodynamic changes using TCD in patients with subarachnoid and parenchymal NCC. Methods There were 53 patients with NCC evaluated at a reference hospital for neurological diseases included (29 with subarachnoid and 24 with parenchymal). Participants underwent a clinical interview and serology for cysticercosis and underwent TCD performed within 2 weeks of enrollment. Mean flow velocity, peak systolic velocity, end diastolic velocity, and pulsatility index were recorded. Results Among the participants, there were 23 (43.4%) women, with a median age of 37 years (IQR: 29–48). Cerebral hemodynamic changes suggesting vasculitis were detected in 12 patients (22.64%); the most compromised vessel was the middle cerebral artery in 11 (91.67%) patients. There were more females in the group with sonographic signs of vasculitis (10/12, 83.33% vs. 13/41, 31.71%; p = 0.002), and this was more frequent in the subarachnoid NCC group (9/29, 31.03% vs. 3/24, 12.5%; p = 0.187), although this difference did not reach statistical significance. Abstract (Objective) This investigation evaluates tomo graphic dimensional changes and histological changes after extraction with the use of two graft materials: Bio-O ss® xenograft and Bio-Gide® resorbable collagen membrane, and Puros® allograft and Biomed Extend ® resorbable collagen membrane for the preservation of the alveolar ridge. (Materials and Methods) This is an experimental study in which 20-single tooth extractions were performed and two different biomaterials randomly assigned in two groups were compared. Members of Group A received xenograft type material (Bio-Oss and Bio-Gide®), while members of group B received allograft-type material (Puros and Biomed®). Tomographic measurements were carried out on Day 0 and Day 210 after extraction. On Day 2 10 tissue samples were taken for histological analysis. (Results) Tomographic measurements did not show statistically significant variations except in the case of maximum width of the bone crest, which was smaller in the allograft group on day 210 with a value of p<0.05. Histological results show similarity between the two groups in cellular response, osteoid tissue, and mineralized new bone. Differences were observed in the amount of remaining material, with the highest presence of material found in the xenograft group. (Conclusion) The results of this study are similar to those found in the literature, and adequate preservation of the height and width of the bone ridge after an extraction was observed when xenograft or allograft bone substitute materials were employed, with very slight changes during the time of evaluation.

Resumen (Objetivo) Este estudio evalúa los cambios dimensionales tomográficos y cambios histológicos posexodoncia con el uso de dos materiales de injerto: xenoinjerto Bio-Oss® y membrana colágena reabsorbible Bio-Gide® y aloinjerto Puros® y membrana de colágeno reabsorbible Biomed Extend® en la preservación del reborde alveolar. (Materiales y Métodos) Estudio experimental en el que se realizaron 20 extracciones de diente único, y se comparan dos biomateriales diferentes repartidos aleatoriamente en dos grupos. Al grupo A se le colocó material tipo xenoinjerto (Bio-Oss y Bio-Gide®) y al grupo B se le colocó material tipo aloinjerto (Puros y Biomed®). Se realizaron medidas tomográficas al día 0 y al día 210 después de la extracción. En el día 210 se tomaron muestras de tejido para análisis histológico. (Resultados) Las medidas tomográficas no mostraron variaciones estadísticamente significativas excepto en la amplitud máxima del reborde óseo, siendo esta menor en el grupo del aloinjerto en el día 210 con valor de p<0.0 . En los resultados histológicos se observa similitud entre los dos grupos en la respuesta celular, tejido osteoide y hueso nuevo mineralizado. Se observaron diferencias en la cantidad de material remanente, presentando mayor presencia de material el grupo del xenoinjerto. (Conclusión) Este estudio presentó resultados similares a los encontrados en la literatura y se observa un comportamiento adecuado de la preservación del reborde óseo posterior a una extracción cuando se colocan materiales sustitutos óseos del tipo xenoinjerto o aloinjerto para las dimensiones de altura y amplitud, con cambios muy leves en el tiempo de evaluación.

Resumo (Objetivo) Este estudo avalia as alterações dimonais tomográficas e as alterações histológicas pós-exodônticas com o uso de dois materiais de enxer to: xenoenxerto Bio-Oss® e membrana de colágeno reabsorvível Bio-Gide® e aloenxerto Puros® e membrana de colágeno reabsorvível Biomed Extend® na preservação do rebordo alveolar. (Materiais e Métodos) Estudo experimental no qual foram realizadas 20 extrações de dentes unitários, comparando dois biomateriais diferentes divididos aleatoriamente em dois grupos. O grupo A foi equipado com material do tipo xenoenxerto (Bio-Oss e Bio-Gide®) e o grupo B foi equipado com material do tipo aloenxerto (Puros e Biomed®). As medições tomográficas foram realizadas no dia 0 e no dia 210 após a extração. No dia 210, amostras de tecido foram coletadas para análise histológica. (Resultados) As medições tomográficas não mostraram variações estatisticamente significativas, exceto pela largura máxima do reborde ósseo, que foi menor no grupo aloenxerto no dia 210, com valor de p<0,05. s resultados histológicos mostram semelhança entre os dois grupos na resposta celular, no tecido osteoide e no novo osso mineralizado. Foram observadas diferenças na quantidade de material remanescente, com mais material presente no grupo de xenoenxerto. (Conclusão) Este estudo apresentou resultados semelhantes aos encontrados na literatura e mostrou um comportamento adequado de preservação do rebordo ósseo após a extração quando materiais substitutos de xenoenxerto ou aloenxerto ósseo são colocados para as dimensões de altura e largura, com alterações muito pequenas no tempo de avaliação.

RESUMEN La paracoccidioidomicosis (PCM) es una infección causada por hongos patógenos humanos del género Paracoccidioides. Es una micosis sistémica que puede afectar cualquier órgano. Se describen con este reporte cuatro casos de paracoccidioidomicosis con diferentes presentaciones clínicas, tiempo de evolución, con afectación mucocutánea, pulmonar, glándulas suprarrenales, sistema nervioso entre otros, principalmente en adultos varones de diferentes edades y profesiones, tanto inmunosuprimidos como inmunocompetentes, teniendo en común el contacto con el suelo. Se demuestra de esta manera la importancia de considerar esta patología ante una sospecha clínica de micosis sistémica para así realizar una confirmación temprana y tratamiento oportuno ya que presenta buena respuesta terapéutica antimicótica y mejoría clínica.

ABSTRACT Paracoccidioidomycosis (PCM) is an infection caused by human pathogenic fungi of the genus Paracoccidioides. This report describes: 4 cases of paracoccidioidomycosis diagnosed with different clinical presentations, different times of evolution, with mucocutaneous, pulmonary, adrenal gland, and nervous system involvement, among others, mainly in male adults of different ages and professions, both immunosuppressed and immunocompetent, having in common contact with the ground. In this way, PCM is a systemic mycosis that can affect any organ and therefore the importance of considering this pathology when a diagnostic suspicion of systemic mycosis is presented in order to make an early diagnosis and timely treatment since it presents a good therapeutic response, antifungal and clinical improvement.

Abstract: Atrial fibrillation (AF) is the most prevalent arrhythmia and is related to significant morbidity, mortality and costs. In spite of relevant advances in the prevention of embolic events and rhythm control, little has been done to reduce its prevalence, progression and impact since it increases with aging as well as with common risk factors such as alcohol intake, tobacco use and stress as well as with arterial hypertension, diabetes mellitus, heart failure, sleep apnea, kidney failure, chronic pulmonary obstructive disease, ischemic heart disease and stroke, among other important comorbidities. Fortunately, new evidence suggests that lifestyle modifications and adequate risk factors and comorbidities control could be effective in primary and secondary AF prevention, especially in its paroxysmal forms. This is why a multidisciplinary approach integrating lifestyle modifications, risk factors and comorbidities control is necessary for conjunction with rhythm or rate control and anticoagulation. Unfortunately, that holistic approach strategy is not considered, is scarcely studied or is subtilized in general clinical practice. The present statement’s objectives are to: a) Review the relationship between habits, risk factors and illnesses with AF, b) Review the individual and common physiopathology mechanisms of each one of those conditions that may lead to AF, c) Review the effect of control of habits, risk factors and co-morbidities on the control and impact of AF, d) Supply guidelines and recommendations to start multidisciplinary and integrative AF treatment.

Resumen: La fibrilación auricular (FA) es la arritmia más frecuente y se asocia con importante morbilidad, mortalidad y costos. A pesar de los grandes avances en la prevención de eventos embólicos y en el control del ritmo, poco se ha realizado para reducir su prevalencia, progresión e impacto, debido a que incrementa con la edad y con la presencia de múltiples factores de riesgo muy comunes en la población, como obesidad, sedentarismo, alcoholismo, tabaquismo y estrés, así como hipertensión arterial sistémica, diabetes mellitus, insuficiencia cardiaca, apnea del sueño, enfermedad renal crónica, enfermedad pulmonar obstructiva crónica, cardiopatía isquémica y enfermedad vascular cerebral, entre otras comorbilidades importantes. Nuevas evidencias demuestran que las modificaciones en el estilo de vida y el control adecuado de factores de riesgo y comorbilidades pueden ser efectivos en la prevención primaria y secundaria de la FA, especialmente en sus formas paroxísticas; para ello es necesario un manejo multidisciplinario que integre las modificaciones en el estilo de vida, el manejo de factores de riesgo y control de comorbilidades en el tratamiento de la FA en conjunto con el control de ritmo o frecuencia y la anticoagulación. Desafortunadamente, en la práctica clínica, estas estrategias frecuentemente no son tomadas en cuenta, son subutilizadas y poco estudiadas. Los objetivos del presente posicionamiento son: a) revisar la relación entre factores de riesgo y comorbilidades con FA, b) revisar los mecanismos fisiopatológicos de cada una de estas condiciones, c) revisar el impacto del control de factores de riesgo y comorbilidades en el control e impacto de la FA y d) proporcionar guías y recomendaciones para la puesta en práctica de programas de tratamiento multidisciplinario e integral en pacientes con FA.

Special Article Introduction Hematopoietic stem cell transplantation is the only curative treatment for many disorders and international data shows a growing trend. Method We aimed to evaluate the temporal trends in HSCT transplant rates in Argentina. A time-series analysis was performed for the period 2009 to 2018 using the national database from the National Central Coordinating Institute for Ablations and Implants. Crude and standardized transplant rates were calculated. A permutation joinpoint regression model analysis was used to identify significant changes over time. Results Altogether, 8,474 transplants were reported to INCUCAI by 28 centers (autologous 67.5%); the main indication was multiple myeloma (30%). The WHO age-sex standardized HSCT rates for the entire country were 153.3 HSCT/10 million inhabitants (95% CI 141.7-165.8) in 2009 and 260.1 HSCT/10 million inhabitants (95% CI 245.5-275.5) in 2018. There was a large gap in HSCT rates among the states and regions. The transplant rate was higher for autologous transplants throughout the years. Within the allogeneic group, the related donor transplant rate was higher than the unrelated donor transplant rate. The joinpoint regression analysis of HSCT rates for the whole country over time showed an observed annual percentage change of 6.3% (95% CI 5.4-7.3; p< 0.01). No changes were observed for unrelated donors during the study period. Conclusions Age-sex standardized HSCT rates in Argentina are increasing, mainly due to autologous and family donor allogeneic transplants. Abstract Background: The topographic and climatic diversity of the Mexican Transition Zone (MTZ) has favored species richness, disjunctions and endemism. Tigridia durangensis is an endemic geophyte with disjunct distribution in the MTZ. Questions and / or Hypotheses: What is the genetic diversity of Tigridia durangensis? Does it have genetic and phylogeographic structure? Were there any changes in its demographic history? Did the Quaternary climatic oscillations affect its area of distribution? Methods: The cpDNA regions ndhF-rpL32, rpL32-trnL, and 3´trnV-ndhC of 55 individuals from 10 populations were sequenced. The genetic diversity and the genetic structure were estimated with Hd, π and F ST. The parameters G ST and N ST determined the phylogeographic structure. The genealogical relationships were inferred with a haplotype net. Phylogenetic hypotheses were generated with Bayesian inference and Maximum likelihood. The demographic history was determined by means of neutrality tests, analyses of mismatch distribution (AMD) and Bayesian skyline plot. The paleodistribution was estimated with ecological niche models (ENMs). Results: Tigridia durangensis showed genetic and phylogeographic structure. Nine haplotypes were identified; H1 and H2-H9 formed two intraspecific lineages. The neutrality tests were not significant. The AMD plot was congruent with the haplotype net. Tigridia durangensis experienced a bottleneck in the recent past and the ENMs displayed a disjunct distribution in all scenarios. Conclusions: In Tigridia durangensis, the bottleneck and the interruption of the gene flow between the haplogroups might have been associated with orogenic processes and volcanism of the Transmexican Volcanic Belt.

Resumen Antecedentes: La diversidad topográfica y climática de la Zona de Transición Mexicana (ZTM) favorecieron la riqueza de especies, la disyunción y el endemismo. Tigridia durangensis es una geófita endémica con distribución disyunta en la ZTM. Preguntas y / o Hipótesis: ¿Cuál es la diversidad genética de Tigridia durangensis? ¿Tiene estructura genética y filogeográfica? ¿Hubo cambios en su historia demográfica? ¿Las oscilaciones climáticas del Cuaternario afectaron su área de distribución? Métodos: En el ADNcp, las regiones ndhF-rpL32, rpL32-trnL y 3´trnV-ndhC en 55 individuos de 10 poblaciones fueron secuenciadas. La diversidad y estructura genética fueron calculadas con Hd, π y F ST. Los parámetros G ST y N ST estimaron la estructura filogeográfica. Las relaciones genealógicas fueron estimadas con una red de haplotipos. Hipótesis filogenéticas fueron inferidas con Inferencia bayesiana y Máxima verosimilitud. La demografía histórica fue determinada con pruebas de neutralidad, análisis de distribución de diferencias pareadas (ADDP) y de graficas de líneas de cielo bayesianas. La paleodistribución fue estimada con modelos de nicho ecológico (MNE). Resultados: Tigridia durangensis mostró estructura genética y filogeográfica. Nueve haplotipos fueron identificados, H1 y H2-H9 formaron dos linajes intraespecíficos. Las pruebas de neutralidad no fueron significativas. El gráfico ADDP fue congruente con la red de haplotipos. Tigridia durangensis experimentó un cuello de botella durante el pasado reciente. Los MNE estimaron una distribución disyunta en todos los escenarios. Conclusiones: Un cuello de botella y la interrupción del flujo genético entre los haplogrupos de Tigridia durangensis parecen estar asociados a procesos orogénicos y al volcanismo de la Faja Volcánica Transmexicana.

Abstract Background: Pulmonary thromboendarterectomy is the current treatment of choice in patients with chronic thromboembolic pulmonary hypertension. The objective of the present study was to analyze the clinical and hemodynamic outcomes and the risk factors for mortality in a cardiovascular center in Colombia. Methods: Cohort study, conducted between 2001 and 2019. All operated patients were included in the study. Risk factors associated with mortality were established by means of a multivariate regression using the COX method and survival was established using the Kaplan–Meier method. p < 0.05 was considered statistically significant. Results: Seventy-three patients were operated. Median age was 51 years, 55% of females, 79% had functional Class III and IV. The mean pulmonary arterial pressure was 50 mmHg and 640 dyn.s.cm−5 for pulmonary vascular resistance (PVR). After the intervention, there was a decrease in mean pulmonary artery pressure (p ≤ 0.001) and in PVR (p = 0.357); 21% had evidence of residual pulmonary hypertension. Only 8% and 6% continued with functional Class III and IV at 6 and 12 months, respectively. There were 15 deaths (19.1%; 12% at 30 days). The factors associated with mortality were the diastolic diameter of the right ventricle measured postoperatively (hazard ratio [HR] 10.88 95% confidence interval [CI] 1.97-62, p = 0.007), time of invasive mechanical ventilation (HR 1.06 95% CI 1.02-1.09 p = 0.004), and the presence of complications during the surgical procedure (HR 5.62 95% CI 1.94-16.22 p = 0.001). Conclusions: Pulmonary thromboendarterectomy is associated with excellent clinical and hemodynamic outcomes. The mortality risk factors found are not those usually described in the literature.

Resumen Antecedentes: La tromboendarterectomía pulmonar es el tratamiento de elección actual en pacientes con hipertensión pulmonar tromboembólica crónica. El objetivo del presente estudio fue analizar los resultados clínicos y hemodinámicos y los factores de riesgo de mortalidad en un centro cardiovascular de Colombia. Métodos: Estudio de cohorte entre 2001 y 2019. Se incluyeron todos los pacientes operados. Los factores de riesgo asociados a la mortalidad se establecieron mediante una regresión multivariante mediante el método COX y la supervivencia se estableció mediante el método de Kaplan-Meier. Los valores de p < 0.05 se consideraron estadísticamente significativos. Resultados: se operaron 73 pacientes. La mediana de edad fue de 51 años, 55% mujeres, 79% tenían clase funcional III y IV. La presión arterial pulmonar media fue de 50 mmHg y 640 dyn.s.cm−5 para la resistencia vascular pulmonar. Después de la intervención, hubo una disminución en la presión arterial pulmonar media (p ≤ 0.001) y en la resistencia vascular pulmonar (p = 0.357). El 21% tenía evidencia de hipertensión pulmonar residual. Solo el 8% y el 6% continuaron con clase funcional III y IV a los 6 y 12 meses respectivamente. Hubo 15 muertes (19.1%; 12% a los 30 días). Los factores asociados con la mortalidad fueron el diámetro diastólico del ventrículo derecho medido en el postoperatorio (HR 10.88 IC 95% 1.97-62, p = 0.007), el tiempo de ventilación mecánica invasiva (HR 1.06 IC 95% 1.02-1.09 p = 0.004) y el presencia de complicaciones durante el procedimiento quirúrgico (HR 5.62 IC 95% 1.94-16.22 p = 0.001). Conclusiones: La tromboendartectomía pulmonar se asocia con excelentes resultados clínicos y hemodinámicos. Los factores de riesgo de mortalidad encontrados no son los habitualmente descritos en la literatura.

Resumen Introducción. En Perú se usan múltiples definiciones de síndrome metabólico (SM); sin embargo, actualmente no hay un consenso sobre cuál definición usar en la práctica clínica. Objetivos. Comparar las estimaciones de riesgo cardiovascular (RCV), obtenidas mediante la calculadora de RCV de la ACC/AHA, y evaluar su grado de concordancia con diferentes definiciones de SM en pacientes atendidos en Lima, Perú. Materiales y métodos. Estudio transversal analítico. Se revisaron las historias clínicas, obtenidas por muestreo consecutivo, de 233 pacientes atendidos entre octubre y diciembre de 2019 en el Hospital Nacional Hipólito Unanue, Lima, Perú. El RCV se calculó mediante la calculadora virtual de RCV de la ACC/AHA y se consideraron las definiciones de SM de la OMS, NCEP-ATP III, IDF, AHA/NHLBI, JIS y ALAD para comparar el RCV según cada definición. La concordancia entre las distintas definiciones de SM se calculó mediante el coeficiente kappa con base en los seis niveles de fuerza de concordancia de Landis y Koch. Resultados. Las medianas de RCV en pacientes con SM según las definiciones de la OMS, NCEP-ATP III, IDF, AHA/NHLBI, ALAD y JIS fueron 9.6 (3.9-20.35), 7.9 (3.1-18.6), 7.3 (3-16.5), 7.8 (3-17.6), 7.1 (2.9-16.5) y 7.1 (3.1-16.5), respectivamente. La prevalencia de SM según las definiciones JIS, IDF, ALAD, AHA/NHLBI, NCEP-ATP III y OMS fue 81.97%, 80.26%, 74.68%, 67.81%, 65.67% y 51.14%, respectivamente. La concordancia entre las definiciones JIS e IDF, ALAD, NCEP-ATP III y AHA/NHLBI fue 0.944, 0.787, 0.592 y 0.567, respectivamente, pero entre la JIS y la OMS fue 0.286. Conclusiones. Existen diferencias entre las estimaciones de RCV según las distintas definiciones de SM usadas en Perú y consideradas en el presente estudio, lo que puede tener repercusiones en la intensidad de las intervenciones terapéuticas y preventivas realizadas en estos pacientes.

Abstract Introduction: Multiple definitions of metabolic syndrome (MS) are used in Peru, and there is currently no consensus on which definition should be used in clinical practice. Objectives: To compare cardiovascular disease (CVD) risk estimators, obtained using the ACC/AHA ASCVD Risk Calculator, and to assess their level of agreement with different definitions of MS in patients treated in Lima, Peru. Materials and methods: Analytical cross-sectional study. Medical records, collected through consecutive sampling, of 233 patients treated between October and December 2019 at the Hospital Nacional Hipólito Unanue, Lima, Peru, were reviewed. CVR risk was calculated using the online ACC/AHA ASCVD Risk Calculator, and the MS definitions of the WHO, NCEP-ATP III, IDF, AHA/NHLBI, JIS and ALAD were considered to compare CVD risk according to each definition. Agreement between the different MS definitions was calculated using the kappa coefficient based on the six levels of strength of agreement described by Landis and Koch. Results: The median CVD risk in patients with MS according to the definitions of the WHO, NCEP-ATP III, IDF, AHA/NHLBI, ALAD and JIS was 9.6 (3.9-20.35), 7.9 (3.1-18.6), 7.3 (3- 16.5), 7.8 (3-17.6), 7.1 (2.9-16.5), and 7.1 (3.1-16.5), respectively. The prevalence of MS according to JIS, IDF, ALAD, AHA/NHLBI, NCEP-ATP III and WHO definitions was 81.97%, 80.26%, 74.68%, 67.81%, 65.67%, and 51.14%, respectively. Agreement between the JIS criteria and the IDF, ALAD, NCEP-ATP III, and AHA/NHLBI criteria was 0.944, 0.787, 0.592, and 0.567, respectively, but it was 0.286 between the JIS criteria and the WHO criteria. Conclusions: In Peru, there are differences between CVD risk estimates depending on the MS definition used and considered in the present study, which may have an impact on the intensity of the therapeutic and preventive interventions performed in these patients.

Abstract Direct oral anticoagulants have emerged as the drugs that have changed the man agement of the antithrombotic treatment in the last 15 years. Their advantages, like a more friendly way of anticoagulation and their lower risk of bleeding, especially in the brain, have positioned these new anticoagu lants as the first drug of choice in the two most frequent indications of anticoagulation, atrial fibrillation, and the venous thromboembolic disease. However, not all the patients can receive these agents, not all the direct oral anticoagulants have the same characteristics, and most importantly, not all the diseases with an indication of an anticoagulant drug can be treated with them. Therefore, it is mandatory that all the faculties involved in the management of these drugs must know them in depth, to decide the best treatment for the patient. This position paper, from a group of experts in anticoagulation in Argentina, can help the general practitioner in the daily use of direct oral anticoagulants based on the new evidence and the experience of a wide group of professionals. The way we relate to the anticoagulant treatment has changed in the last years. The doctors who work with them must also do so.

Resumen Los anticoagulantes orales directos han surgido como una de las herramientas que ha cambiado el manejo de la enfermedad trombótica en los últimos 15 años. Sus ventajas, desde el punto de vista de la facilidad de uso y menor riesgo de sangrado, especialmente de sangrado cerebral, han posicionado a estos nuevos anticoagulantes como la primera alternativa de tratamiento en las dos indicaciones más frecuentes en que necesitamos estas drogas, la fibrilación auricular y la enfermedad tromboembólica venosa. Sin embargo, no todos los pacientes pueden recibir estos agentes, no todos los anticoagulantes directos tienen las mismas pro piedades y fundamentalmente, no todas las enfermedades con indicación de un anticoagulante pueden tratarse con ellos;con lo cual es necesario que todos los profesionales que están involucrados en el manejo de estos medicamentos estén obligados a conocerlos en profundidad, para poder decidir el mejor tratamiento en cada caso particular. Este documento de posición de expertos de diferentes especialidades de Argentina, presenta lineamientos para el uso correcto de los anticoagulantes directos en base a nueva evidencia y a la experiencia de uso de un amplio grupo de profesionales. La forma de relacionarnos con el tratamiento anticoagulante ha cambiado. Los médicos que trabajamos con ellos también debemos hacerlo.

Resumen Objetivo: El proyecto CoVIDA es una colaboración público-privada liderada por la Universidad de los Andes que contribuyó a la vigilancia epidemiológica del SARS-CoV-2 en Bogotá y municipios cercanos. Nuestro objetivo fue describir el desarrollo y rendimiento de la estrategia de tamizaje gratuito con RT-PCR mediante un modelo Drive/Walk through para SARS-CoV-2 implementado por CoVIDA. Materiales y métodos: Realizamos un análisis descriptivo de las características y desempeño de los centros de tamizaje Drive/Walk through de CoVIDA. El modelo y los indicadores de proceso para evaluar el desempeño del modelo se basaron en experiencias internacionales y la literatura científica. Se implementaron dos cen tros de tamizaje en centros comerciales del norte y sur de Bogotá. Se reportó la cantidad de pruebas tomadas, pruebas positivas y de participantes que utilizaron el modelo de acuerdo con el tipo de ocupaciones. Resultados : En total, se realizaron 36,689 pruebas RT-PCR nasofaríngeas para SARS-CoV-2 con una positividad acumulada del 5,75%. Los indicadores de proceso mostraron un excelente desempeño y una contribución importante en la reducción de las barreras de acceso a las pruebas. Conclusiones: los centros de tamizaje con modelo Drive/Walk through de CoVIDA apoyaron la vigilancia epidemiológica en población asintomática o con síntomas leves en Bogotá. Los países de ingresos bajos y medianos pueden utilizar este modelo como una estrategia innovadora y rentable para aumentar la realización de las pruebas y ayudar a mitigar la pandemia.

Abstract Objective: The CoVIDA project is a public-private collaboration led by Universidad de los Andes that contributed to the SARS-CoV-2 epidemiological surveillance in Bogotá and nearby municipalities. We aimed to describe the development and performance of the Drive/Walk-through free RT- PCR for SARS-CoV-2 testing strategy implemented by CoVIDA. Material and method: We performed a descriptive analysis of the characteristics and performance of the CoVIDA Drive/Walk-through testing centers. The model and the process indicators to assess the model’s performance were based on international experiences and scientific literature. Two screening centers were imple mented in shopping centers in the north and south of Bogotá. We reported the number of tests taken, the number of positive tests, and the number of participants that used the model by the type of occupations. Results: In total, 36,689 nasopharyngeal RT-PCR tests for SARS-CoV-2 were performed with a 5.75% cumulative positivity. Process indicators showed an excellent performance and an important contribution in reducing barriers to access to testing. Conclusions: the CoVIDA Drive/Walk-through testing centers supported the epidemiological surveillance in asymptomatic or mild-symptomatic population in Bo gotá. Low and middle-income countries can use this model as a cost-effective and innovative solution strategy to intensify testing and help mitigate the pandemic.

Introduction: It has been postulated the relationship between presence of cardiometabolic diseases with mental health problems, particularly depression. Objectives: To determine the association between cardiometabolic diseases and presence of depressive symptoms, evaluated by PHQ-9 questionnaire, in patients attended at the outpatient clinic of internal medicine service at Hospital Nacional Hipolito Unanue. Methods: Observational analytical cross-sectional study. Cardiometabolic disease was defined as the presence of diabetes mellitus, high blood pressure, dyslipidemia, and/or obesity. The PHQ-9 questionnaire was used to evaluate the presence of symptoms suggestive of depression. The association was evaluated crude and adjusted for possible confounders. For the multivariate analysis, a Poisson regression model was used to find prevalence ratios with their respective 95% confidence intervals. A p<0.05 was considered statistically significant. Results: 252 patients were included, of which 205 (81,4%) presented cardiometabolic diseases and 181 (71,9%) presented symptoms consistent with some grade of depression. The presence of cardiometabolic diseases was associated with depressive symptoms both in the crude analysis (PRc 1.43; CI 95% 1.08-1.89; p=0.012) and in the adjusted one (PRa 1.31; CI 95% 1.00-1.71; p=0.048). Additionally, an association was found between female sex and depressive symptoms (PRa 1,35; CI 95% 1,11-1,63; p=0,002). Conclusions: The presence of cardiometabolic diseases was associated with the presence of depressive symptoms in patients seen at the outpatient clinic of internal medicine. The mental health approach should be an integral part of the multidisciplinary management for the patient with cardiometabolic disease.

Resumen Introducción: Se ha postulado la relación entre la presencia de enfermedades cardiometabólicas con problemas de salud mental, en particular la depresión. Objetivos: Determinar la asociación entre la presencia de enfermedades cardiometabólicas y la presencia de sintomatología depresiva evaluada mediante el cuestionario PHQ-9 en pacientes atendidos en consultorios externos de medicina interna del Hospital Nacional Hipólito Unanue. Metodología: Estudio observacional, analítico, transversal. Se definió enfermedad cardiometabólica como la presencia de diabetes mellitus, hipertensión arterial, dislipidemia, y/u obesidad. Se utilizó el cuestionario PHQ-9 para evaluar la presencia de síntomas sugerentes de depresión. Se evaluó la asociación cruda y ajustada a posibles confusores. Para el análisis multivariado se utilizó un modelo de regresión de Poisson para hallar razones de prevalencia con sus respectivos intervalos de confianza al 95%. Se consideró un p<0,05 como estadísticamente significativo. Resultados: Se incluyeron 252 pacientes, de los que 205 (81,4%) presentaron enfermedades cardiometabólicas y 181 (71,9%) presentaron síntomas consistentes con algún grado de depresión. La presencia de enfermedades cardiometabólicas se asoció a síntomas depresivos tanto en el análisis crudo (RPc 1,43; IC 95% 1,08-1,89; p=0,012) como en el ajustado (RPa 1,31; IC 95% 1,00-1,71; p=0,048). Adicionalmente se encontró asociación entre el sexo femenino y sintomatología depresiva (RPa 1,35; IC 95% 1,11-1,63; p=0,002). Conclusiones: La presencia de enfermedades cardiometabólicas se asoció con la presencia de síntomas depresivos en pacientes atendidos en la consulta externa de medicina interna. El abordaje de la salud mental debe ser parte integral del manejo multidisciplinario del paciente con enfermedad cardiometabólica.

ABSTRACT BACKGROUND: The intensity of the thromboprophylaxis needed as a potential factor for preventing inpatient mortality due to coronavirus disease-19 (COVID-19) remains unclear. OBJECTIVE: To explore the association between anticoagulation intensity and COVID-19 survival. DESIGN AND SETTING: Retrospective observational study in a tertiary-level hospital in Spain. METHODS: Low-molecular-weight heparin (LMWH) status was ascertained based on prescription at admission. To control for immortal time bias, anticoagulant use was analyzed as a time-dependent variable. RESULTS: 690 patients were included (median age, 72 years). LMWH was administered to 615 patients, starting from hospital admission (89.1%). 410 (66.7%) received prophylactic-dose LMWH; 120 (19.5%), therapeutic-dose LMWH; and another 85 (13.8%) who presented respiratory failure, high D-dimer levels (> 3 mg/l) and non-worsening of inflammation markers received prophylaxis of intermediate-dose LMWH. The overall inpatient-mortality rate was 38.5%. The anticoagulant nonuser group presented higher mortality risk than each of the following groups: any LMWH users (HR 2.1; 95% CI: 1.40-3.15); the prophylactic-dose heparin group (HR 2.39; 95% CI, 1.57-3.64); and the users of heparin dose according to biomarkers (HR 6.52; 95% CI, 2.95-14.41). 3.4% of the patients experienced major hemorrhage. 2.8% of the patients developed an episode of thromboembolism. CONCLUSIONS: This observational study showed that LMWH administered at the time of admission was associated with lower mortality among unselected adult COVID-19 inpatients. The magnitude of the benefit may have been greatest for the intermediate-dose subgroup. Randomized controlled trials to assess the benefit of heparin within different therapeutic regimes for COVID-19 patients are required.
