OBJETIVO: revisar a literatura mais atual, dos últimos 15 anos, em busca de estudos clínicos que relatem a relação entre a disfunção temporomandibular (DTM) e o tratamento ortodôntico e/ou a má oclusão. A intenção foi verificar se o tratamento ortodôntico aumentaria o aparecimento de sinais e sintomas de DTM, e se o tratamento ortodôntico seria um recurso para o tratamento ou prevenção dos sinais e sintomas de DTM. MÉTODOS: artigos dos tipos revisão de literatura, editorial, carta, estudo experimental em animais e comunicação foram excluídos dessa revisão. Foram incluídos artigos prospectivos, longitudinais, caso-controle ou retrospectivo com amostra maior, com relevante análise estatística. Estudos que abordassem deformidades e síndromes craniofaciais e tratamento por cirurgia ortognática também foram excluídos, bem como aqueles que relatassem apenas a associação entre má oclusão e DTM. RESULTADOS: foram encontrados 20 artigos relacionando Ortodontia à DTM, segundo os critérios adotados. Os estudos, então, associando sinais e sintomas de DTM ao tratamento ortodôntico apresentaram resultados heterogêneos. Alguns encontraram efeitos positivos do tratamento ortodôntico para os sinais e sintomas de DTM; entretanto, nenhum deles apresentou diferença estatisticamente significativa. CONCLUSÕES: todos os estudos citados nessa revisão de literatura relataram que o tratamento ortodôntico não forneceu risco ao desenvolvimento de sinais e sintomas de DTM, independentemente da técnica utilizada para tratamento, da exodontia ou não de pré-molares e do tipo de má oclusão previamente apresentada pelo paciente. Alguns estudos realizados com acompanhamento em longo prazo concluíram que o tratamento ortodôntico não seria preventivo ou uma modalidade de tratamento para DTM.

OBJECTIVE: The objective of this study was to review the most recent studies from the last 15 years, in search of clinical studies that report the relationship between TMD and orthodontic treatment and/or malocclusion. Our intention was to determine whether orthodontic treatment would increase the incidence of signs and symptoms of TMD, and whether orthodontic treatment would be recommended for treating or preventing signs and symptoms of TMD. METHODS: Literature reviews, editorials, letters to the editor, experimental studies in animals and short communications were excluded from this review. Were included only prospective, longitudinal, case-control or retrospective studies with a large sample and significant statistical analysis. Studies that dealt with craniofacial deformities and syndromes or orthognathic surgery treatment were also excluded, as well as those that reported only the association between malocclusion and TMD. RESULTS: There were 20 articles relating orthodontics to TMD according to the inclusion criteria. The studies that associated signs and symptoms of TMD to orthodontic treatment showed discrepant results. Some have found positive effects of orthodontic treatment on signs and symptoms of TMD, however, none showed a statistically significant difference. CONCLUSIONS: All studies cited in this literature review reported that orthodontic treatment did not provide risk to the development of signs and symptoms of TMD, regardless of the technique used for treatment, the extraction or non-extraction of premolars and the type of malocclusion previously presented by the patient. Some studies with long-term follow-up concluded that orthodontic treatment would not be preventive or a treatment option for TMD.

Este trabalho descreve uma nova forma de ancoragem por meio de miniplacas denominada SAO®, Sistema de Apoio Ósseo para Mecânica Ortodôntica. Após a descrição do sistema, protocolos de tratamento para mordidas abertas esqueléticas são apresentados. A aplicação de cantiléveres e alças apoiadas diretamente nos tubos do sistema de ancoragem permite que associações de problemas verticais e sagitais (Classe II e III) sejam tratadas de formas distintas. A aplicação de forças leves e constantes e o controle tridimensional das forças aplicadas são o grande diferencial desse novo sistema.

This paper describes the SAO®, Sistema de Apoio Ósseo para Mecânica Ortodôntica (Skeletal Supported System for Orthodontic Mechanics), a new approach for anchorage by means of mini-plates. After the description of the system, treatment protocols for skeletal openbite cases are presented. The use of cantilevers and different spring configurations attached directly in the tubes of the anchorage system allows various approaches for vertical and sagittal discrepancies.

OBJETIVO: avaliar a resistência de braquetes metálicos colados em dentes humanos com resina polimerizada com luz halógena por meio de ensaios mecânicos de cisalhamento. METODOLOGIA: para este estudo foram realizados ensaios in vivo com dinamômetro portátil digital e in vitro com máquina de ensaios mecânicos universal com e sem termociclagem, complementado pelo Índice de Adesivo Remanescente (IAR). Braquetes Edgewise Standard (Abzil) foram colados utilizando adesivo Transbond Plus Self Etching Primer (SEP) e Resina Transbond XT. Foram formados 3 grupos com 10 dentes em cada um deles. No GI os braquetes foram colados nos segundos pré-molares dos pacientes. Nos GII e GIII utilizaram-se primeiros pré-molares extraídos por motivos ortodônticos. Os ensaios mecânicos do GI foram realizados 24 horas após a polimerização diretamente na boca dos pacientes com dinamômetro portátil digital. No GII os corpos-de-prova foram armazenados em água destilada e levados à estufa a 37ºC durante 24 horas e, posteriormente, submetidos à termociclagem, com 1000 ciclos a 5 e 55ºC. No GIII os corpos-de-prova foram armazenados em água destilada em temperatura ambiente por 24 horas e posteriormente submetidos aos ensaios mecânicos. RESULTADOS: os valores médios da resistência ao cisalhamento em Megapascal foram de: GI = 4,39; GII = 7,11 e GIII = 7,35. Após a descolagem foram realizadas fotografias das áreas de colagem, tanto dos dentes submetidos a testes in vivo quanto in vitro e ampliadas 5x para facilitar a visualização. As imagens obtidas foram analisadas, classificadas de acordo com o IAR e, por meio de gráficos de dispersão, foi verificada a relação entre a resistência ao cisalhamento e este índice. CONCLUSÃO: a média dos ensaios mecânicos realizados in vivo foi estatisticamente menor em relação aos ensaios in vitro. Não houve diferenças na resistência ao cisalhamento in vitro entre o grupo termociclado e o não-termociclado. Não houve relação entre tensão de ruptura e tipo de falha.

OBJECTIVE: The purpose of this research was to assess the strength of metallic brackets bonded to human teeth, with lightcuring resin with halogen light, by means of mechanical tests of shear bond strength. METHODS: In vivo tests with a portable digital dynamometer and in vitro tests with Universal testing machine were performed, with and without thermocycling, complemented by Adhesive Remaining Index (ARI). Edgewise standard brackets (Abzil) were bonded using Transbond Plus Self Etching Primer (SEP) and Transbond XT resin. Three groups of ten teeth each have been formed. In GI the brackets were bonded to second premolars in the patients’ mouths. In GII and GIII the brackets were bonded to first premolars extracted for orthodontic reasons. The mechanical tests of GI were performed 24 hours after the polymerization, directly in the patients’ mouths with a portable digital dynamometer. In GII the specimens were stored in distilled water and taken to sterilizer at 37ºC for 24 hours and afterwards, submitted to thermocycling with 1000 cycles at 5 and 55º C. In GIII the specimens were stored in distilled water at room temperature for 24 hours, and then submitted to mechanical tests. RESULTS: The average values of the shear strength in Megapascal were: GI=4.39; GII=7.11 and GIII=7.35. Pictures of the bonding areas in both in vivo and in vitro samples were taken after the tests and they were amplified 5 times to be better visualized. The images obtained were analyzed, classified according to IAR and it was verified, by means of dispersion graphs, the relation between the shear bond strength and this index. CONCLUSION: The average of the in vivo mechanical tests was statistically lower than the in vitro tests. There were no differences in the in vitro shear bond strength between the thermocycled and non-thermocycled groups. There was no relation between shearing stress and kind of failure.

Os autores apresentam uma revisão da literatura referente às alternativas de tratamento cirúrgicas e não-cirúrgicas para a Síndrome da Apnéia e Hipopnéia Obstrutiva do Sono (SAHOS) e para a Síndrome da Resistência das Vias Aéreas superiores (SRVAS) com enfoque no princípio de ação e evolução dos aparelhos de avanço mandibular e, em particular, no mecanismo dinâmico de ação do Aparelho Anti-Ronco® (AAR-ITO).

The authors present a literature review of the surgical and non-surgical treatment alternatives to the Obstructive Sleep Apnea and Hypopnea Syndrome (OSAHS) and snoring with focus in the mechanism of action and progress of mandibular advancement devices mainly in the dynamic action mechanism of the Mandibular Advancement Device® (MAD-ITO).

Realizou-se um estudo das alturas faciais anterior e posterior em 79 crianças brasileiras, sendo 46 do gênero feminino e 33 do masculino, com idades de 8 a 11 anos, portadoras de má oclusão Classe I de Angle, na fase de dentadura mista, não submetidas a tratamento ortodôntico. Esta investigação utilizou duas radiografias cefalométricas em norma lateral, de cada criança, com intervalo de 8 a 16 meses entre uma radiografia e outra, para comparação dos dados cefalométricos. Foram estudadas as proporções faciais anteriores, ou seja, a relação entre as dimensões N-Me (AFT), N-ENA (AFS), ENA-Me (AFI) e o relacionamento entre as dimensões Ar-GO (AFP) e a distância entre o plano palatino ao mento (AFA) , isto é, o Índice da Altura Facial (IAF). Os resultados obtidos, possibilitaram constatar que as tendências de crescimento foram equilibradas em todas as situações estudadas; a dimensão altura facial inferior, contribuiu de forma mais significativa, nas alterações observadas na altura facial total; as proporções faciais anteriores se mantiveram em torno de 42% para a altura facial superior e 58% para a altura facial inferior; o índice da altura facial ou se manteve constante em torno de 0,66 ou apresentou uma tendência a aumentar com a idade; não ocorreu dimorfismo sexual com relação às proporções faciais e índice da altura facial.

The objective of this study was to evaluate the anterior and posterior facial heights in 79 non orthodontic brazilian children, 46 females and 33 males, from 8 to 11 years old, presenting Angle Class I malocclusion, in the mixed dentition. Two lateral cephalometric radiographs were taken, with a time interval ranging from 8 to 16 months between radiographs in order to compare the cephalometric data. The anterior facial proportions were studied, based in following dimensions: N-Me (AFT), N-ENA (AFS) e ENA-Me (AFI). The Facial Height Index (IAF) was also evaluated taking in account the relationship between the Ar-Go (AFP) and the distance between the palatal plane and the menton (AFA). The results showed that growth tendencies were balanced; the Lower Facial Height dimension contributed more significantly to the Total Facial Height changes; the anterior facial proportions were around 42% for the Upper Facial Height and 58% for the Lower Facial height; the Facial Height Index was stable, around 0,66 or had a tendency to increase, according to the age; there were no sexual dimorphism in Anterior Facial Proportion and Facial Height Index.

O objetivo desse artigo é descrever a análise cefalométrica que vem sendo aplicada nos cursos de mestrado, doutorado e especialização da Faculdade de Odontologia de Araraquara - UNESP. A mesma foi desenvolvida utilizando medidas cefalométricas já existentes e descritas na literatura as quais foram agrupadas em campos a fim de permitir que o profissional faça a interpretação adequada de cada área e tenha, no final da mesma, todas as informações necessárias para um correto diagnóstico e plano de tratamento. Da mesma forma, após o tratamento, a comparação das diferentes medidas e áreas de análise permitirão a interpretação de quais foram os efeitos provocados pelo tratamento aplicado.

The purpose of this paper is to describe the cephalometric analysis that have been used in Master, PhD and Certificate Programs at Araraquara Dental School - UNESP. This analysis was developed by cephalometric measurements existent and described in literature, organized by fields to allow the professional to do a correct interpretation of each area and to have all necessary information to the correct diagnose and treatment plane in the end of the analysis. In the same way the comparison among the different measurements and areas, before and after treatment, will allow the interpretation of the treatment results.
