ABSTRACT Introduction: Health personnel represent the first response of the health system in the COVID-19 pandemic. Therefore, it is important to assess their knowledge about the identification and care of patients with COVID-19. Objective: To identify the factors associated with the perception of knowledge of Peruvian resident physicians regarding the care of patients with COVID-19 and to describe the perception of mental health during the COVID-19 pandemic. Methods: Cross-sectional study, carried out in 2020 in hospitals in Peru. The "Perception of Knowledge in Health Workers" scale was used (Cronbach's Alpha: 0.77). And also, the "Mental Health COVID-19" scale was used (Cronbach's Alpha: 0.88). Bivariate and multivariate statistics were performed. Results: Of the 200 respondents, 12% and 5% stated that they were able to recognize a suspected case and care for a diagnosed patient, respectively; 41% and 35% strongly agreed with the fear/concern of infecting their family and feeling mistreatment due to the lack of personal protective equipment, respectively. In the multivariate analysis, the perception of knowledge was associated with working in emergencies (p= 0.007), with the number of positive friends for COVID-19 (p= 0.012) and with having recently undergone intubation (p= 0.035); these were adjusted by the mental health perception score. Conclusions: Working in emergencies, having had contact with infected patients and having performed medical-surgical procedures influence the perception of knowledge of doctors about the care of patients with COVID-19.

RESUMEN Introducción: El personal sanitario representa la primera respuesta del sistema de salud en la pandemia de la COVID-19, por ello, es importante valorar su conocimiento sobre la identificación y atención de pacientes con la COVID-19. Objetivo: Identificar los factores asociados a la percepción del conocimiento de médicos residentes ante la atención de pacientes con la COVID-19 y describir la percepción de la salud mental durante la pandemia de la COVID-19. Métodos: Estudio transversal, realizado en 2020 en hospitales del Perú. Se empleó la escala “Perception of Knowledge in Health Workers” (alpha de Cronbach: 0,77). También, se empleó la escala “Mental Health COVID-19” (alpha de Cronbach: 0,88). Se realizó estadística bivariada y multivariada. Resultados: De los 200 encuestados, el 12 % y 5 % manifestaron que eran capaces de reconocer a un caso sospechoso y atender un paciente diagnosticado, respectivamente. El 41 % y 35 % estuvo muy de acuerdo con el miedo/preocupación de contagiar a su familia y sentir el maltrato por los pocos equipos de protección personal, respectivamente. En el análisis multivariado, la percepción del conocimiento se asoció a trabajar en emergencias (p= 0,007), a la cantidad de amistades positivos a la COVID-19 (p= 0,012) y a realizar recientemente una intubación (p= 0,035); estas fueron ajustadas por el puntaje de la percepción de salud mental. Conclusiones: Trabajar en emergencias, haber tenido contacto con pacientes contagiados y haber realizado procedimientos médico-quirúrgicos influyen en la percepción de conocimientos de los médicos sobre la atención de pacientes con la COVID-19.

This, added to other factors, is possibly associated with an increased risk of the appearance and concurrence of autoimmune diseases. The simultaneous occurrence of GBS and ITP has been rarely reported in the literature, and GBS is rarely associated with another autoimmune disease. We present the case of a man who, one month after his recovery from acute moderate COVID-19, presented concurrent GBS and ITP with an adequate response to treatment.

RESUMEN Durante la infección aguda por el SARVS-CoV-2 se produce una desregulación del sistema inmune que puede durar hasta ocho meses después de controlado el cuadro agudo. Esto, sumado a otros factores, posiblemente este asociado con un aumento del riesgo de aparición y concurrencia de enfermedades autoinmunes. La aparición simultanea del síndrome de Guillain-Barré (SGB) y púrpura trombocitopénica (PTI) se ha reportado poco en la literatura, y el SGB raramente se asocia con otra enfermedad autoinmune. Presentamos el caso de un varón que luego de un mes de tener un cuadro agudo de COVID-19 moderado, presentó concurrentemente SGB y PTI con respuesta adecuada al tratamiento.

ABSTRACT Hantavirosis is a zoonotic infection transmitted mainly by rodents. In Peru, a lethality of 40-60% is calculated in inhabitants of the Peruvian Amazon jungle, especially in Loreto. Despite this, this disease continues to be under-diagnosed despite the fact that it represents a serious problem for public health in Latin America. We present a systematic revision of case reports and series of cases of Hantavirus infection in the Peruvian jungle. The most important characteristic of the cases presented are mean age 25.7 years, predominance of female sex (5/6), clinical presentation of headache, myalgias, nausea and vomiting (6/6) and unfavorable evolution to acute respiratory distress syndrome (ARDS), renal failure, septic shock and multiple organ failure in 3 of the cases presented.

RESUMEN La hantavirosis es una infección zoonótica transmitida principalmente por roedores. En el Perú, se calcula una letalidad del 40-60% en pobladores de la selva amazónica peruana, especialmente en Loreto. A pesar de ello, esta enfermedad continúa siendo sub-diagnosticada a pesar que representa un problema serio para la salud pública en Latinoamérica. Presentamos una revisión sistemática de reportes y series de casos de infección por Hantavirus procedentes de la selva peruana. Las características más importantes de los casos presentados son: media de edad 25.7 años, predominio del sexo femenino (5/6), presentación clínica de cefalea, mialgias, nausea y vómitos (6/6), y evolución desfavorable a distrés respiratorio agudo (SDRA), falla renal, shock séptico y falla multiorgánica en 3 de los casos presentados.

ABSTRACT Systemic or cutaneous-visceral loxoscelism is the most severe clinical presentation of loxoscelism. Currently there is no validated laboratory diagnostic method that allows us to confirm the presence of this disease in the face of arachneism. However, there is a clinical prediction rule (CPR), which allows us to predict the evolution of a bite towards a systemic condition and which would be useful in emergency rooms. We present the case of a woman who developed the systemic picture of loxoscelism to whom a new CPR was applied for the early detection of systemic loxoscelism, presenting adequate performance for early evaluation in emergency services.

RESUMEN El loxoscelismo sistémico o cutáneo-visceral, es la presentación clínica más severa del loxoscelismo. Actualmente no se cuenta con algún método diagnóstico laboratorial validado que nos permita confirmar la presencia de esta enfermedad frente a un aracneismo. Sin embargo, existe una regla de predicción clínica (RPC), que nos permite predecir la evolución de una picadura hacia un cuadro sistémico y que sería de utilidad en las salas de emergencia. Presentamos el caso de una mujer que desarrollo el cuadro sistémico del loxoscelismo a la que se le aplicó una nueva RPC para la detección temprana del loxoscelismo sistémico, presentando un rendimiento adecuado para la evaluación temprana en los servicios de emergencia.

ABSTRACT Accidents caused by spider bites are a public health problem in Peru. Of these, those related to Loxosceles spider bites have been the cause of numerous deaths, mainly on the coast. These bites generally result in a cutaneous lesion or systemic involvement, which may threaten the person's life if not treated timely. An international protocol is not yet available for the diagnosis, prediction or management of Loxosceles spider bites. Therefore, therapeutic options are applied which are not supported by evidence. Still, timely and appropriate support management is crucial in severe cases. An unusual case of viscerocutaneous loxoscelism is presented, also known as systemic loxoscelism, which occurred in a rural area. The case was characterized by severe renal involvement requiring hemodialysis, but its outcome was favorable, despite not using antiloxoscelic serum, which shows the importance of timely management based on appropriate support measures.

RESUMEN En el Perú los accidentes por mordedura de araña representan un problema de salud colectiva; de estos, el producido por la araña Loxosceles ha sido causa de numerosas muertes sobre todo en la costa. Esta tiende a ocasionar, ya sea una lesión cutánea o un cuadro sistémico, que puede llevar a la muerte del individuo si no es tratado a tiempo. Hasta la fecha no se cuenta con un protocolo de diagnóstico, predicción ni manejo a nivel internacional, por lo que se utilizan opciones terapéuticas sin respaldo de evidencia. Sin embargo, el manejo de soporte oportuno y adecuado es crucial para los cuadros severos. Se presenta el caso inusual de un loxoscelismo cutáneo-visceral o también llamado sistémico ocurrido en una zona rural. Hubo un compromiso renal severo que requirió hemodiálisis con un desenlace favorable a pesar del no uso de suero antiloxoscélico, lo que evidencia la importancia del manejo oportuno con las medidas de soporte adecuadas.

ABSTRACT Critical reading from the point of view of evidence-based medicine is a structured reading that allows us to evaluate the validity and relevance of the results and the applicability of a clinical study in our patient. However, the level of critical reading among residents is low, as described in a number of studies. A study of oncology residents revealed an inadequate level of critical reading of clinical research articles. Similarly, inadequate levels were observed among residents of cardiology and family practice. In our country, this situation is unknown and it is most likely deficient. For this reason, we have prepared a small review in order to share practical bases to help the resident of medical specialties to apply in a simpler way the critical reading of clinical studies.

RESUMEN La lectura crítica desde el punto de vista de la medicina basada en la evidencia es una lectura estructurada que nos permite evaluar la validez y pertinencia de los resultados y la aplicabilidad de un estudio clínico en nuestro paciente. Sin embargo, el nivel de lectura crítica en los médicos residentes es baja, como se ha descrito diversos estudios. En un estudio realizado en médicos residentes de oncología, se encontró un nivel deficiente de lectura crítica de artículos de investigación clínica. Y de la misma forma, se encontró niveles deficientes en residentes de cardiología y medicina familiar. En nuestro país, se desconoce esta situación y lo más probable es que sea deficiente. Motivo por el que elaboramos una pequeña revisión para poder compartir bases prácticas que ayuden al residente de especialidades clínicas aplicar de una manera más sencilla la lectura crítica de estudios clínicos.

ABSTRACT Introduction: Rategravir belongs to integrase inhibitors, being demonstrated and approved by several clinical trials as a powerful and safe antiretroviral drug for the treatment of patients infected with human immunodeficiency virus (HIV), with good tolerance and low toxicity, including in the third line or rescue scheme and it starts when the first and second lineas schemes have failed. Objective: To evaluate the efficacy and safety in real clinical conditions of the use of Raltegravir within the HAART schemes in patients with HIV infection in a reference hospital of social insurance in Peru. Methods: A retrospective observational study was performed in patients with a diagnosis of HIV infection who started treatment within the TARGA scheme based on Raltegravir with follow-up and control at 6 months. We presented summary measures of frequencies and percentages for the qualitative variables, as well as means and standard deviation for the quantitative variables based on the results of the normality tests. The data was processed and analyzed in the statistical software SPSS version 22. Results: The male gender was the most affected with 76% (n = 119) of the total. The most frequent age range was between 45 to 55 years (25.4%, n = 40). The most frequent comorbidities were Diabetes mellitus and arterial hypertension, with exponential reduction in viral load and elevation of CD4 lymphocyte levels. Conclusion: Raltegravir is effective for the treatment of HIV patients.

RESUMEN Introducción: El Rategravir pertenece a los inhibidores de integrasas, quedando demostrado y aprobado por diversos ensayos clínicos como un potente antirretroviral seguro y eficaz para el tratamiento de pacientes infectados con el virus de inmunodeficiencia humana (VIH), con buena tolerancia y baja toxicidad, incluyéndose en el esquema de tercera línea o rescate y se inicia cuando los esquemas de primera y segunda línea han fracasado. Objetivo: Evaluar la eficacia y seguridad en condiciones clínicas reales del uso de Raltegravir dentro de los esquemas de la Terapia Antiretroviral de Gran Actividad (TARGA) en pacientes con infección por VIH en un hospital de referencia del seguro social en Perú. Métodos: Se realizó un estudio observacional retrospectivo en pacientes con diagnóstico de infección por VIH que iniciaron tratamiento dentro del esquema TARGA basados en Raltegravir con seguimiento y control a los 6 meses. Se presentaron medidas de resumen de frecuencias y porcentajes para las variables cualitativas, así como medias y desviación estándar para las variables cuantitativas en base a los resultados de las pruebas de normalidad. Los datos fueron procesados y analizados en el software estadístico SPSS versión 22. Resultados: El género masculino fue el más afectado con un 76%(n=119) del total. El rango de edad más frecuente fue el comprendido entre los 45 a 55 años (25,4%; n=40). Las comorbilidades más frecuentes fueron Diabetes mellitus e Hipertensión arterial, con reducción exponencial de la carga viral y elevación de los niveles de linfocitos CD4. Conclusión: El Raltegravir es eficaz para el tratamiento de pacientes VIH.

ABSTRACT Introduction: Systemic loxoscelism is the most severe complication of loxoscelism. The management of the cadre by health personnel presents a high variability due to factors that are currently unknown. There is no standard of reference or a clinical prediction model that can guide our decisions when approaching a spider bite patient. Objective: Develop and validate a clinical prediction rule for systemic loxoscelism. Methods: An observational study of derivation and validation of a clinical prediction model was carried out with diagnostic test validation based on a historical single-arm cohort in patients treated at Vitarte Hospital between 2007 and 2016 and international clinical reports published. Results: Systemic loxoscelism occurred only in 32.9% (n = 24) of cases. For the bivariate analysis, the variables that showed a statistically significant association (P <0.05) were sex, bite in an independent abdomen in relation to other parts of the body, bite in other parts of the body than the abdomen, vomiting , fever and hemoglobinuria. The regression analysis included in the analysis the variables: sex, vomit, fever and hemoglobinuria. Bootstrapping determined the internal validity of the model. The area under the curve was 0.91 (P <0.05) and the sensitivity, specificity, LR + and LR- were 79.1%, 93.8%, 12.9 and 0.22 respectively. Conclusions: The protocol of prediction of systemic derived loxoscelism is valid, for the moment.

RESUMEN Introducción: El loxoscelismo sistémico es la complicación más severa del loxoscelismo. El manejo del cuadro por parte del personal de salud presenta una alta variabilidad por factores que se desconocen actualmente. No se cuenta con un estándar de referencia ni con un modelo de predicción clínica que pueda guiar nuestras decisiones al momento del abordaje de un paciente con mordedura de araña Objetivo: Elaborar y validar una regla de predicción clínica para loxoscelismo sistémico. Métodos: Se llevó a cabo un estudio observacional de derivación y validación de un modelo de predicción clínica con validación de prueba diagnóstica basada en una cohorte histórica de un solo brazo en pacientes atendidos en el Hospital Vitarte entre los años 2007 al 2016 y reportes clínicos internacionales publicados. Resultados: El loxoscelismo sistémico se presentó solo en el 32,9 % (n=24) de casos. Para el análisis bivariado, las variables que demostraron presentar una asociación estadísticamente significativa (P<0,05) fueron el sexo, mordedura en abdomen independiente en relación a otras partes del cuerpo, mordedura en otras partes del cuerpo que no sea el abdomen, vómito, fiebre y hemoglobinuria. El análisis de regresión incluyó en el análisis a las variables: sexo, vómito, fiebre y hemoglobinuria. El bootstrapping determinó la validez interna del modelo. El área bajo la curva fue de 0,91 (P<0,05) y la sensibilidad, espeficidad, LR+ y LR- fueron de 79,1%, 93,8%, 12,9 y 0,22 respectivamente. Conclusión: El protocolo de predicción del loxoscelismo sistémico derivado es válido, por el momento.

Mucormycosis is a highly lethal fungal infection occurring in patients with some degree of immunosuppression. The rhinocerebral form is the most frequent presentation. This is the first report in Peru of a case of disseminated mucormycosis in a patient with diabetic ketoacidosis. This is a 47-year-old diabetic male subject who was referred from the Amazon jungle and presented with respiratory insufficiency receiving mechanical ventilation. On admission, the patient had leukocytosis, metabolic acidosis with high anion gap, hyperglycemia, and hypoalbuminemia. Soon afterwards, the patient underwent surgery because of acute abdomen, and the anatomopathological examination revealed necrosis and hollow viscus perforation, and he ultimately died. At this time, he was receiving amphotericin B deoxycholate. Straight angle hyphae compatible with mucormycosis were found in stomach, intestine, and lungs.

La mucormicosis es una infección fúngica altamente mortal que se presenta en pacientes con algún grado de inmunosupresión, siendo la forma rinocerebral la más común. Es el primer reporte en el Perú de mucormicosis diseminada con compromiso multisistémico en un paciente con cetoacidosis diabética. Se presenta el caso de un varón de 47 años diabético procedente de la selva peruana con cuadro de insuficiencia respiratoria en ventilación mecánica. A su ingreso presenta leucocitosis, acidosis metabólica anion gap elevado, hiperglicemia e hipoalbuminemia. Posteriormente, es intervenido quirúrgicamente en tres oportunidades por presentar abdomen agudo con hallazgos en la patología de necrosis y perforación de varios órganos, falleciendo a los pocos días. Paciente se encontraba recibiendo su quinta dosis de anfotericina B deoxicolato. Se observaron hifas con angulación recta compatible con mucormicosis en estómago, intestino y pulmón.

ABSTRACT Infectious meningitis is a medical emergency. Within the spectrum of infectious agents, the most important is Streptococcus pneumoniae, the most frequent etiological agent of bacterial meningitis. The initiation of empirical antimicrobial treatment bears great importance and considers third-generation cephalosporins as the first alternative. However, cases of ceftriaxone resistance have been reported in several regions of the world. This has become an emerging problem in need of reconsideration of the current empirical antibiotic treatment schemes. We present the case of a 56-year old man with acute infectious meningitis caused by ceftriaxone-resistant Streptococcus pneumoniae, who responded favorably to combined empirical treatment with ceftriaxone and vancomycin and to whom, during his hospital stay, the presence of hypothyroidism and mega cisterna magna was diagnosed.

RESUMEN La meningitis infecciosa es una emergencia médica. Dentro del espectro de agentes infecciosos, el más importante es el Streptococcus pneumoniae, agente etiológico más frecuente de la meningitis bacteriana. El inicio de tratamiento antimicrobiano empírico es de gran importancia y considera a las cefalosporinas de tercera generación como la primera alternativa. Sin embargo, casos de resistencia a ceftriaxona han sido reportados en diversas partes del mundo, siendo un problema emergente, por lo que necesita una reconsideración de los esquemas antibióticos empíricos actuales. Presentamos el caso de un varón de 56 años que presenta meningitis aguda infecciosa por Streptococcus pneumoniae resistente a ceftriaxona, que respondió favorablemente al tratamiento empírico combinado con ceftriaxona y vancomicina y que durante su estadía hospitalaria se detectó la presencia de hipotiroidismo y megacisterna magna.

Loxoscelism is a condition produced by the bite of spiders from the genus Loxosceles. It is considered to be an accident that occurs mostly in the night, due to the spider's nocturnal habits and good adaptation to domestic environments, preferably dark, dry spaces. It presents in two clinical scenarios: cutaneous (83.3 %) and visceral or systemic (16 %), with a variation in the cutaneous manifestation known as predominantly edematous loxoscelism. The objective of the study was to report an unusual case of scrotal loxoscelism. Diagnosis was based on the clinical and epidemiological characteristics of the accident. It is concluded that scrotal loxoscelism is a very infrequent condition. Management of the patient greatly influences its evolution and progress. Exhaustive treatment with sulfadiazine twice daily and intravenous antibiotics yields very favorable results.

Loxoscelismo es el cuadro clínico originado por la mordedura de araña del género Loxosceles. Es considerado un accidente que ocurre con mayor frecuencia en las noches, debido al hábito nocturno de la araña y buena adaptación a los ambientes domésticos, preferentemente en espacios oscuros y secos. Clínicamente presenta dos escenarios, cutáneo (83,3 %) y visceral o sistémico (16 %), con una variación del cuadro cutáneo denominado loxoscelismo predominantemente edematoso. El objetivo es informar un caso inusual de loxoscelismo escrotal. El diagnóstico se realizó mediante las características clínicas y epidemiológicas del accidente. En conclusión, el loxoscelismo escrotal es una entidad muy infrecuente, el manejo del paciente influye mucho en su evolución y progresión; el tratamiento exhaustivo con sulfadiazina 2 veces al día y antibióticos intravenosos, muestra resultados muy favorables.
