Knowledge and attitudes of rural healthcare providers regarding domestic violence against women: a systematic review

ABSTRACT BACKGROUND: Specific types of violence such as intimate partner sexual violence and intimate partner homicide occur more frequently in rural areas. OBJECTIVE: This study aimed to systematically review the literature on the knowledge and attitudes of rural healthcare providers regarding cases of domestic violence against women. DESIGN AND SETTING: Systematic review developed at Universidade Federal de Uberlândia. METHODS: We conducted an electronic search of six databases, which only included observational studies, regardless of the year, language, or country of publication, except for studies that used secondary data and were exclusively qualitative. Two reviewers performed the selection, data extraction, and risk of bias assessment using a specific Joanna Briggs Institute tool. RESULTS: Six studies met the inclusion criteria. All the studies had a low risk of bias. Approximately 38% of these professionals identified injuries caused by violence in patients. When asked about knowing the correct attitude to take in cases of confirmed violence, between 12% and 64% of rural healthcare providers answered positively; most of them would refer to specialized institutions and promote victim empowerment and counseling. The number of professionals with an educational background in the field ranged from 16% to 98%. CONCLUSIONS: The evident disparity across studies shows that some professionals have suboptimal knowledge and require training to adopt the correct attitude when identifying female victims of domestic violence in clinical practice. Abstract The pathogenesis of systemic lupus erythematosus (SLE) is complex. Few studies in Brazilian population have addressed cell phenotypes associated with immunological responses and their associations with SLE activity. The aim of this study is to investigate cell phenotypes associated to SLE diagnosis, treatment and activity. Twenty-eight SLE female patients (17 inactive, 11 active) and 10 healthy women were included in this study. Markers of natural killer (Nk), T and B cells in peripheral blood were evaluated by flow cytometry. Nkt cells were decreased only in SLE active patients. Activated CD4+, regulatory T FoxP3+ and B cells were decreased in both active and inactive SLE patients, compared to control group. The data corroborate the disruption of immune regulatory response in SLE patients and suggest phenotipic changes as possible biomarkers of SLE activity.

ABSTRACT Children have an increased likelihood of becoming carriers of the chronic hepatitis B virus. A total of 1,381 children and adolescents were assessed in five municipalities of Maranhao State, Brazil, for detection of anti-HBc, HBsAg and anti-HBs serologic markers and sociodemographic and behavioral features. Among those who were HBsAg negative and anti-HBc negative, the proportion of anti-HBs positives was calculated after the individuals had completed the vaccination schedule. The robust variance of the Poisson’s regression model was used in order to have adjusted tables and calculate the prevalence ratio. Multivariate analysis was performed to identify the factors associated with the prevalence of anti-HBc with or without HBsAg and the vaccine response. It was observed that 163 children were anti-HBc positive and nine individuals were HBsAg positive. The factors associated with the infection were: municipality of residence (residing in Morros municipality or Humberto de Campos municipality), residence in a rural area, aged between 13 and 15 years old, and illicit drug use. The percentage of individuals who were anti-HBc negative and received all three doses of the vaccine was 48.5%. Among these, only 276 (38.9%) had antibodies at protective concentrations. In an adjusted analysis, Morros municipality presented an increased positivity of vaccine response (p < 0.001), and the age ranging between 6 and 10 years old presented a reduced frequency of response. This study reveals a high prevalence of current and past HBV infection within the targeted age group which, in addition to the low vaccination coverage and serological responses, raises concerns about the management of prevention measures, especially the quality of vaccination in these locations.

Ti alloys are widely used in severe corrosion environments where corrosion resistance is required, as biomedical industry. Additive manufacturing produces customized and complexes products. Laser texturing is a process of structuring surfaces using laser pulses, that allows the creation of periodic patterns on the surfaces of materials, to modify them, functionally and/or aesthetically, in a precise and direct way, allowing parameterization, versatility and repeatability. Consequently, bringing together metallic additive manufacturing with laser texturing process could be an alternative to obtain parts with functional hydrophilic surfaces, which improves osteointegration and reduces bacteria adhesion. Thus, the aim of this work is to characterize and evaluate the influence of LASER parameters in as-built additive manufactured potential biomedical components. Ti6Al4V specimens were produced by L-PBF, using Ytterbium LASER with maximum power of 500 W, varying the laser power from 61 W to 244 W. The samples were characterized by SEM, Microhardness, and wettability. After that, some specimens were Laser textured using an Ytterbium optical fiber laser, and then evaluated by SEM, wettability, and 3D roughness. It was possible to observe that the surface of all studied samples was flattened after Laser texturing in comparison with as-built condition, due to the melting of the powder particles.

Abstract This study aimed to investigate the effect of growth and differentiation factor 9 (GDF-9) during the in vitro culture of isolated caprine early antral follicles. The isolated and selected early antral follicles were individually cultured for 18 days, and the following treatments were tested: α-MEM+ (control treatment) or α-MEM+ supplemented with 200 ng/mL GDF-9. The following endpoints were evaluated: follicular growth and morphology, estradiol production, oocyte nuclear maturation, and relative expression of key genes related to steroidogenesis (CYP19A1, CYP17, and insulin receptor) and basement membrane remodeling (MMP-9 and TIMP-2). In both treatments, a decrease was observed in the percentage of morphologically intact follicles with a concomitant increase in the rates of extruded and degenerated follicles (P < 0.05). The GDF-9 treatment showed higher rates of extruded follicles only on day 6 of culture (P < 0.05). Follicle diameter increased progressively throughout the culture period (P < 0.05) with similar diameters between treatments at all culture times (P > 0.05). Growth and differentiation factor 9 increased the daily growth rate from the first to the second third of culture, with higher values (P < 0.05) than control in the second third. Oocyte maturation rate as well as estradiol levels and relative mRNA expression for CYP19A1, CYP17, MMP-9, TIMP-2, and insulin receptor genes were similar between treatments (P > 0.05). This study shows for the first time that GDF-9 added to a culture medium increased the follicle growth rate of goat early antral follicles cultured in vitro.
Resumo Este estudo teve como objetivo investigar o efeito do GDF-9 durante o cultivo in vitro de folículos antrais iniciais caprinos isolados. Os folículos antrais iniciais isolados e selecionados foram cultivados individualmente por 18 dias, e os seguintes tratamentos foram testados: α MEM+ (tratamento controle) ou α-MEM+ suplementado com 200 ng/mL de GDF-9 (tratamento GDF-9). Os seguintes parâmetros foram avaliados: crescimento e morfologia folicular, produção de estradiol, maturação nuclear do oócito e expressão relativa de genes-chave relacionados a esteroidogênese (CYP19A1, CYP17 e receptor de insulina) e remodelamento da membrana basal (MMP-9 e TIMP-2). Em ambos os tratamentos, observou-se diminuição na porcentagem de folículos morfologicamente intactos com aumento concomitante nas taxas de folículos extrusos e degenerados (P < 0,05). O tratamento GDF-9 apresentou maiores taxas de folículos extrusos apenas no 6º dia de cultivo (P < 0,05). O diâmetro do folículo aumentou progressivamente ao longo do período de cultivo (P < 0,05) com diâmetros semelhantes entre os tratamentos em todos os tempos de cultivo (P > 0,05). O GDF-9 aumentou a taxa de crescimento diário do primeiro para o segundo terço de cultivo, sendo maior (P < 0,05) que o controle no segundo terço. A taxa de maturação oocitária assim como os níveis de estradiol e a expressão relativa de RNAm para os genes CYP19A1, CYP17, MMP-9, TIMP-2 e receptor de insulina foram similares entre os tratamentos (P > 0,05). Em conclusão, este estudo mostra pela primeira vez que GDF-9 adicionado a um meio de cultivo aumentou a taxa de crescimento de folículos antrais iniciais caprinos cultivados in vitro.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

RESUMO A mão em fenda é uma deformidade congênita rara caracterizada por uma deficiência longitudinal dos raios centrais da mão, podendo estar associada a outras malformações. Devido ao amplo espectro de manifestações, o tratamento de mãos em fenda é desafiador. Este estudo objetiva apresentar as classificações, técnicas cirúrgicas mais indicadas e seguimentos adotados conforme a manifestação clínica. Foi realizada uma pesquisa nos bancos de dados Web of Science, PubMed, Scopus, Cochrane e Embase, descritores e termos relacionados à anomalia mão em fenda típica. Trinta e dois artigos foram incluídos, sendo analisados quanto a classificação da anomalia, classificação da gravidade de expressão, técnicas cirúrgicas e estudos com informações da intervenção cirúrgica adotada para uma coorte de pacientes. Considerando que estudos sobre mão em fenda são diretamente afetados pelas descobertas embriológicas, genéticas e de biologia molecular, diferentes classificações foram descritas e diversos estudos de complementação de técnicas cirúrgicas já existentes foram encontrados. Estudos inovadores são escassos. A padronização na descrição das técnicas e resultados, além de pesquisas de melhor qualidade, poderiam elucidar lacunas ainda existentes em torno das opções de tratamento.
ABSTRACT Cleft hand is a rare congenital deformity characterized by a longitudinal deficiency of the central rays of the hand, which may be associated with other malformations. Due to the wide spectrum of manifestations, the treatment is challenging. This study aims to present the most suitable classifications, surgical techniques and follow-up adopted according to the clinical manifestation. A search was performed in the databases Web of Science, PubMed, Scopus, Cochrane and Embase, descriptors and terms related to the hand anomaly in a typical cleft. Thirty-two articles were included and analyzed regarding the classification of the anomaly, classification of the severity of expression, surgical techniques and studies with information on the surgical intervention adopted for a cohort of patients. Considering that studies about cleft hand could be directly affected by embryological, genetic and molecular biology discoveries, different classifications have been described and several studies to complement existing surgical techniques have been found. Innovative studies are scarce. Standardization in the description of techniques and results, in addition to better quality research, could elucidate gaps that still exist around treatment options gaps.

ABSTRACT Objective: To evaluate the association between obesity and hospitalization in mild COVID-19 adult outpatients in Brazil. Subjects and methods: Adults with signs and symptoms suggestive of acute SARS-CoV-2 infection who sought treatment in two hospital (public and private) emergency departments were prospectively enrolled. Patients with confirmed COVID-19 at inclusion were followed by phone calls at days D7, D14 and D28. Multivariable logistic regression models were employed to explore the association between obesity and other potential predictors for hospitalization. Results: A total of 1,050 participants were screened, and 297 completed the 28-day follow-up and were diagnosed with COVID-19 by RT-PCR. The median age was 37.2 (IQR 29.7-44.6) years, and 179 (60.0%) were female. The duration of symptoms was 3.0 (IQR 2.0-5.0) days, and 10.0 (IQR 8.0-12.0) was the median number of symptoms at inclusion. Ninety-five (32.0%) individuals had obesity, and 233 (78.5%) had no previous medical conditions. Twenty-three participants (7.7%) required hospitalization during the follow-up period. After adjusting, obesity (BMI ≥ 30.0 kg/m2) (OR = 2.69, 95% CI 1.63-4.83, P < 0.001) and older age (OR = 1.05, 95% CI 1.01-1.09, P < 0.001) were significantly associated with higher risks of hospitalization. Conclusion: Obesity, followed by aging, was the main factor associated with hospital admission for COVID-19 in a young population in a low-middle income country. Our findings highlighted the need to promote additional protection for individuals with obesity, such as vaccination, and to encourage lifestyle changes.

ABSTRACT The objective of this work was to determine and model the drying kinetics of organic corn seeds at different temperatures, as well as to evaluate their physiological quality after drying. The drying of organic corn seeds was carried out by keeping controlled temperatures of 30, 40, 50 and 60 °C during periods of 84.3; 20.0; 14.3 and 8.6 hours, respectively. With the experimental data, several mathematical models were adjusted to describe the drying kinetics, and the best model was chosen by statistical criteria. After drying, the seeds were subjected to analyzes of electrical conductivity, germination, emergence e and germination speed index (IVG). The Modified Midilli model was the one that best represented the drying kinetics of organic corn seeds. It was verified that the increase in the temperature of the drying air promotes greater damage to the seeds, due to the increase in the electrical conductivity of the solution. Germination, emergence and IVE had their values reduced by temperature increase. It is concluded that the physiological quality of organic corn seeds is affected by the drying air temperature and that higher temperatures promote greater damage to the seeds. A temperature of 30 °C is recommended for drying the seeds.
RESUMO Objetivou-se, com o presente trabalho, determinar e modelar a cinética de secagem de sementes de milho em diferentes temperaturas bem como avaliar a qualidade fisiológica das mesmas após a secagem. A secagem das sementes de milho foi realizada mantendo-se as temperaturas controladas de 30, 40, 50 e 60 °C durante os períodos de 84,3; 20,0; 14,3 e 8,6 horas, respectivamente. Com os dados experimentais, ajustaram-se diversos modelos matemáticos para descrever a cinética de secagem e, o melhor modelo, foi escolhido por critérios estatísticos. Após a secagem, as sementes foram submetidas às análises de condutividade elétrica, germinação, emergência e índice de velocidade de germinação (IVG). O modelo Midilli Modificado foi o que melhor representou a cinética de secagem das sementes de milho. Constata-se que o incremento de temperatura do ar de secagem afeta a viabilidade das sementes, pelo aumento da condutividade elétrica da solução. A germinação, emergência e o IVE tiveram seus valores reduzidos pelo aumento de temperatura. Conclui-se que a qualidade fisiológica das sementes de milho é afetada pela temperatura do ar de secagem e que as temperaturas mais elevadas promovem maiores danos às sementes. Recomenda-se a temperatura de 30°C para realização da secagem das sementes.

Abstract Objective: to evaluate the accuracy of an antibody point-of-care lateral flow immunoassay (LFI -Wondfo Biotech Co., Guangzhou, China) in a pediatric population. Methods: children and adolescents (2 months to 18 years) with signs and symptoms suggestive of acute SARS-CoV-2 infection were prospectively investigated with nasopharyngeal RT-PCR and LFI at the emergency room. RT-PCR was performed at baseline, and LFI at the same time or scheduled for those with less than 7 days of the clinical picture. Overall accuracy, sensitivity and specificity were assessed, as well as according to the onset of symptoms (7-13 or ≥14 days) at the time of the LFI test. Results: In 175 children included, RT-PCR and LFI were positive in 51 (29.14%) and 36 (20.57%), respectively. The overall sensitivity, specificity, positive and negative predictive value was 70.6% (95%CI 56.2-82.5), 96.8% (95%CI 91.9-99.1), 90.0% (95%CI 77.2-96.0), and 88.9% (95%CI 83.9-92.5), respectively. At 7-13 and ≥14 days after the onset of symptoms, sensitivity was 60.0% (95%CI 26.2-87.8) and 73.2% (95%CI 57.1-85.8) and specificity was 97.9% (95%CI 88.7-99.9) and 96.1% (95%CI 89.0-99.2), respectively. Conclusion: Despite its high specificity, in the present study the sensitivity of LFI in children was lower (around 70%) than most reports in adults. Although a positive result is informative, a negative LFI test cannot rule out COVID-19 in children.

Abstract Microbialites are the most abundant life evidence in Precambrian sedimentary rocks. They are produced by microbial interaction activity and sedimentary processes reflecting paleoenvironmental conditions. The Ediacaran-Cambrian carbonate and siliciclastic successions in the Southern Amazon Craton in Central Brazil, provide a key opportunity to understand how the metazoan life coexisted with the microbial communities. The spatial and temporal distribution of microbialites as well as morphological and paleoenvironmental changes have been assessed, reinterpreting previous works and including new data from the Araras-Alto Paraguai and Corumbá basins. The deposition was controlled by subsidence and sea-level changes that affected these basins, considered extensions of epicontinental seas during the Gondwana assembly. The stromatolites are restricted to coastal deposits and experienced thriving flourishment intervals after the Marinoan Glaciation (635 Ma). Post-glacial transgression was marked by microbial colonization in shallow platforms represented by stratiform and giant domical stromatolites in the Araras-Alto Paraguai Basin. The continuity of the transgression generated a moderately deep aragonite sea at about 622 Ma. A progressive sea-level fall caused the implantation of coastal environments under greenhouse conditions with tidal flat and sabkha settings colonized by centimetric-scale stromatolites. The sea retreat was accompanied by progressive uplift, causing a moderate inversion of the basin and erosion of the succession until ~560 Ma with the deposition of the last preserved tidal flat deposits with the occurrence of thrombolites. The subsiding Corumbá Basin was the site of microbially-induced deposition of carbonates in a shallow platform connected to an offshore setting with the proliferation of metazoan straddling the Ediacaran-Cambrian boundary. Microbial communities were restricted to lagoon deposits during the Lower Cambrian transgression in the Araras-Alto Paraguai Basin and the last phase refers to the sea retreat towards southeast, developing a fluvial system connected with the arid and evaporitic tidal flats colonized by microbialites that lasted until the upper Cambrian. Except for the post-glacial stromatolites, the columnar and domal microbialite indicate that the coastal settings dominated the Ediacaran-Cambrian transition. The preservation of microbialites in the post-glacial intervals can be associated with the Mg-Ca-CO3 oversaturation in dolomitic platforms. The rapid calcification and ability to resist the dissolution and replacement have increased the stromatolites’ preservation potential reported here, where its well-preserved occurrence in tidal flats and sabkha occurs due to intense early diagenetic silicification. The change from carbonate accumulation to siliciclastic-rich environments contributed to the demise of microbially-induced strata. In general, the scarce coexistence between coastal stromatolite and metazoan-bearing marine deposits makes it challenging to establish a competitive relationship between these organisms, as previously postulated.

The article discusses the reconfiguration of work in Primary Health Care (PHC) based on changes implemented by the Brazilian federal government since 2017, which have aggravated precarious work conditions. Methodologically it is inspired by Michael Burawoy’s expanded case ethnography, dialoguing with studies on productive restructuring and New Public Management (NGP), linked to the concept of difficulty. Participant observation and semi-structured interviews with workers from a health team from a basic unit of the Unified Health System (SUS) in Rio de Janeiro were used. Difficulty has been characterized as: the constant demand for changes in the organization of daily work and overload due to the extensive workday, and the demand for reaching established goals despite the work conditions. This paper emphasized the need to continue analyzing the reconfiguration of health work, considering the impacts of the Constitutional Amendment 95 (2016); the labor reform (2017); and the COVID-19 pandemic.
Cet article discute la reconfiguration du travail en Soins de Santé Primaires (SSP) selon des changements mis en ouvre par le gouvernement fédéral brésilien depuis 2017, lesquels aggravent des circonstances de la précarisation du travail. Du point de vue méthodologique, la recherche s’inspire de l’ethnographie de cas élargi, selon Michael Burawoy, en dialogue avec des études sur la restructuration productive et la Nouvelle Gestion Publique (NGP), liées au concept de la pénibilité au travail. Nous utilisons l’observation participante et des entretiens semi-structurées avec des travailleuses d’un établissement de soins de santé de base du Système de Santé Unifié brésilien (SUS) à Rio de Janeiro. Ont été caractérisées comme types de pénibilité: l’exigence constante des changements dans l’organisation du travail quotidien et la surcharge causée par la longue journée du travail; et la demande pour répondre des objectifs fixés sans considérer des particularités du contexte de travail. L’étude souligne la nécessité de continuer des analyses de la reconfiguration du travail en santé en consonance avec des impacts de l’Amendement Constitutionnel 95 (2016) ; de la réforme du travail (2017); et de la pandémie de COVID-19.
O artigo trata da reconfiguração do trabalho na Atenção Primária à Saúde (APS) a partir de mudanças implementadas pelo governo federal desde 2017, que agravam as circunstâncias de precarização do trabalho. Metodologicamente, inspira-se na etnografia de caso ampliado de Michael Burawoy, dialogando com estudos sobre a reestruturação produtiva e a Nova Gestão Pública (NGP), articulados ao conceito de penosidades. Utilizou-se a observação participante e entrevistas semiestruturadas com trabalhadoras de uma equipe de saúde de uma unidade básica do Sistema Único de Saúde (SUS) do Rio de Janeiro. Caracterizam-se como penosidades: exigência constante de mudanças na organização do cotidiano do trabalho e sobrecarga devido à extensa jornada de trabalho, e cobrança pelo alcance de metas a despeito das condições de trabalho. Ressalta-se a necessidade de continuar análises da reconfiguração do trabalho em saúde, considerando os impactos da Emenda Constitucional nº 95 (2016); da reforma trabalhista (2017) e da pandemia de Covid-19.

Resumo Este trabalho objetivou analisar a tendência e a distribuição espacial da mortalidade de motociclistas em acidentes de transporte no estado de Alagoas. Trata-se de um estudo ecológico referente a todos os óbitos decorrentes de acidentes motociclísticos no estado no período 2001-2015. Os dados de mortalidade foram obtidos do Sistema de Informações sobre Mortalidade (SIM). As taxas de mortalidade foram calculadas e estratificadas por sexo. Para a análise de tendência, foi empregado o modelo de regressão por pontos de inflexão. Calculou-se a Variação Percentual Anual (VPA). Significância de 5%. Para a análise espacial, aplicou-se modelagem bayesiana empírica local, estatística de Moran e estatística de varredura espacial. Foram registrados 1.458 óbitos de motociclistas no período estudado, sendo 91,3% homens. Três comportamentos temporais foram observados nessa população: crescimento (2001-2005), padrão estacionário (2005-2013) e declínio a partir de 2013. As maiores taxas foram observadas no agreste e sertão. Cinco aglomerados espaciais foram evidenciados no que se refere à mortalidade geral e masculina, todos situados no agreste e sertão alagoanos. A modelagem mostrou redução da mortalidade a partir de 2013 e a análise espacial evidenciou que o problema é mais grave no interior do estado.
Abstract The scope of this work was to analyze the trend and distribution of mortality among motorcyclists in traffic accidents in the State of Alagoas. It involved an ecological study relating to all deaths resulting from motorcycle accidents in the state in the period from 2001 to 2015. Mortality data were obtained from the Mortality Information System (MIS). Mortality rates were calculated and stratified by gender. The joinpoint regression model was used for trend analysis and the Annual Percentage Variation (APV) was calculated with a significance rate of 5%. For the spatial analysis, local empirical Bayesian modeling and Moran statistics and spatial scanning statistics were applied. There were 1,458 deaths of motorcyclists in the period studied, 91.3% of which were men. Three temporal behaviors were observed in this population group: growth (2001-2005), stationary pattern (2005-2013) and decline from 2013 onwards. The highest rates were observed in the ‘agreste’ and ‘sertão’ regions of the state of Alagoas. Five spatial clusters were revealed with relation to general and male mortality, all located in the ‘agreste’ and ‘sertão’ hinterlands of Alagoas. The modeling showed a reduction of mortality from 2013 onwards and the spatial analysis revealed that the problem is more acute in the interior of the state.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.
