Abstract The pathogenesis of systemic lupus erythematosus (SLE) is complex. Few studies in Brazilian population have addressed cell phenotypes associated with immunological responses and their associations with SLE activity. The aim of this study is to investigate cell phenotypes associated to SLE diagnosis, treatment and activity. Twenty-eight SLE female patients (17 inactive, 11 active) and 10 healthy women were included in this study. Markers of natural killer (Nk), T and B cells in peripheral blood were evaluated by flow cytometry. Nkt cells were decreased only in SLE active patients. Activated CD4+, regulatory T FoxP3+ and B cells were decreased in both active and inactive SLE patients, compared to control group. The data corroborate the disruption of immune regulatory response in SLE patients and suggest phenotipic changes as possible biomarkers of SLE activity.

Experimental evaluation of the use of starch and carboxymethylcellulose in the prevention of intraperitoneal adhesions in hernia surgery with coated meshes

ABSTRACT Purpose: Laparoscopic hernia repair involves a risk of adhesion between coated mesh and viscera. Plant polysaccharides such as starch and carboxymethylcellulose (SC) make up a product that acts as a barrier and prevents intraperitoneal adhesions. This study aimed to evaluate whether topical SC can also reduce adhesions between mesh and intra-abdominal organs. Methods: Forty rats each received placement of two intraperitoneal mesh fragments, one on each side of the abdominal wall. Randomly, 20 animals received SC on the right and other 20 on the left, leaving the contralateral side as a control. Fourteen days after the surgery, the animals underwent an additional laparotomy in which macroscopic analysis was performed. Results: As for the percentage of the mesh area affected by adhesion, one (2.6%) animal had > 75% adhesion on the experimental side, and 11 animals (28.9%) on the control side. The adhesion intensity score showed firm adhesions in three (7.9%) animals on the experimental side and nine (23.7%) on the control side. Conclusions: The use of SC decreased the intensity of adhesions and the surface area of the mesh affected by adhesions. There was no negative interference of the product in the incorporation of the mesh into the abdominal wall.

Abstract The objective of this work was to select maize hybrids using the GGE biplot analysis, as well as to evaluate their stability and adaptability in different environments of the North and Midwest regions of Brazil. Thirty-six maize hybrids were evaluated in 2018, in the following five environments in the Northern and Midwestern regions, respectively: in the municipality of Vilhena, in the state of Rondônia; and in the municipalities of Sorriso, Sinop, Alta Floresta, and Carlinda, in the Northern region of the state of Mato Grosso. The experimental design was a randomized complete block design. The analysis of variance was performed, and adaptability and stability were estimated by the GGE biplot method based on grain yield performance. A significant interaction between genotypes and environments was detected, and the biplot analysis was efficient in explaining 62.74% of the total variation in the first two principal components, with the formation of three macroenvironments. The 1P2227, 'BRS 3042', and 1P2265 hybrids showed high yield, responsiveness, and stability in the evaluated environments. The DKB310VTPRO2 hybrid was the most unstable genotype. The recommended hybrids are: DKB310 for the Sorriso and Vilhena macroenvironment; 1M1810 and 1O2106 for the Carlinda environment; and 1M1807 for the Sinop environment.
Resumo O objetivo deste trabalho foi selecionar híbridos de milho, por meio da análise GGE biplot, bem como avaliar sua estabilidade e adaptabilidade em diferentes ambientes das regiões Centro-Oeste e Norte do Brasil. Trinta e seis híbridos de milho foram avaliados em 2018, nos seguintes cinco ambientes das regiões Norte e Centro-Oeste, respectivamente: no município de Vilhena, no estado de Rondônia; e nos municípios de Sorriso, Sinop, Alta Floresta e Carlinda, na região norte do estado de Mato Grosso. O delineamento experimental foi em blocos completos ao acaso. Realizou-se a análise de variância, e estimaramse a adaptabilidade e a estabilidade pelo método GGE biplot com base na produtividade. Detectou-se interação significativa entre genótipos e ambientes, e a análise biplot foi eficiente para explicar 62,74% da variação total nos dois primeiros componentes principais, com a formação de três macroambientes. Os híbridos 1P2227, 'BRS 3042' e 1P2265 apresentam alta produtividade, capacidade de resposta e estabilidade nos ambientes avaliados. O híbrido DKB310VTPRO2 foi o genótipo mais instável. Os híbridos recomendados são: DKB310 para o macroambiente Sorriso e Vilhena; 1M1810 e 1O2106 para o ambiente Carlinda; e 1M1807 para o ambiente Sinop.

Abstract The introgression of M. enterolobii resistance-related genes in guava breeding programs can be compromised by incompatibility among Psidium species. This study aimed to evaluate the female parent preference and genetic diversity of Psidium interspecific hybrids using morphoagronomic traits and resistance to M. enterolobii. There were evaluated cross successes and germination from crosses between accesses of P. cattleyanum, P. guineense and P. guajava and the genetic diversity by Ward-MLM method of hybrids according to descriptors developed for the genus. Crosses were more successful when P. cattleyanum was the female parent. Germination was more successful in crosses involving P. cattleyanum and P. guajava. Four groups were formed. The group IV clustered the most resistant genotypes, composed by genotypes of P. cattleyanum x P. guineense, while the group II was the most susceptible. The groups I and III grouped some genotypes of P. cattleyanum x P. guajava with low levels of susceptibility. There are preferences of female parent species among crosses. Some individuals of groups I and III can be used as source of resistance genes for the breeding program, due the presence of favorable alleles inherited from guava parent. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Resumo A vulnerabilidade dos recursos hídricos à curto e longo prazo está relacionada à diversos fatores como alterações no uso e cobertura da terra e mudanças climáticas. Dada a necessidade de mais informações para auxiliar na gestão da seca e no planejamento do uso da água no semiárido brasileiro, este trabalho segue duas linhas: (I) Analisar a variabilidade espaço-temporal das componentes hidroclimáticas da Bacia Hidrográfica do Submédio São Francisco; (II) Criar um índice de Vulnerabilidade Hídrica a partir da análise fatorial. Foram identificados dois fatores predominantes na bacia, solo e clima, os quais são constituídos por variáveis hidroclimaticas distribuídas espaço e temporalmente. Com o IVH, foi possível identificar as áreas que possuem o menor índice, ou seja, aquelas que são mais vulneráveis aos efeitos das mudanças climáticas, indicando que com a instabilidade no regime de chuvas deverá ocorrer menor recarga dos aquíferos, impedindo que, na época da estiagem, os reservatórios continuem a manter sua produção para o abastecimento, podendo ainda, apresentar riscos como aumento da temperatura e alterações no regime hídrico. climáticas brasileiro linhas I (I espaçotemporal temporal Francisco II (II fatorial bacia clima temporalmente IVH seja aquíferos estiagem abastecimento ainda hídrico
Abstract The vulnerability of water resources in the short and long term is related to several factors such as changes in land use and cover and climate change. Given the need for more information to assist in drought management and water use planning in the Brazilian semi-arid region, this work follows two lines: (I) To analyze the spatio-temporal variability of the hydroclimatic components of the Submédio São Francisco Watershed; (II) Create a Water Vulnerability Index from factor analysis. Two predominant factors were identified in the basin, soil and climate, which are constituted by hydroclimatic variables distributed in space and time. With the IVH, it was possible to identify the areas that have the lowest index, that is, those that are more vulnerable to the effects of climate change, indicating that with the instability in the rainfall regime, there should be less recharge of the aquifers, preventing that, at the time of the drought, the reservoirs continue to maintain their production for supply, and may also present risks such as increased temperature and changes in the water regime. change semiarid semi arid region lines I (I spatiotemporal spatio temporal Watershed II (II analysis basin IVH index regime aquifers supply

Abstract Forest burning susceptibility mapping is a tool to mitigate wildfires, with several methods to develop them. This study aimed to compare the Analytic Hierarchy Process (AHP), Multiple Linear Regression (MLR), and Random Forest (RF) methods for mapping. Several variables were used to generate the maps. For MLR and RF methods, fire frequency between 1990 and 2010 was used as the response variable in the models. To validate the methods (AHP, MLR and RF), fire data between 2011 and 2018 were used in four stages. RF was the best method employed. Correct and incorrect values for this method were 74% and 26% and AUC 0.66. The sensitivity and specificity for the highest risk class were 31% and 96%. The low sensitivity values can be attributed to the randomness attributed to anthropic fire. The high specificity values point to a good separation of the higher risk class compared to the others.

Aim To evaluate the impact of a dual-cured adhesive system on the in situ degree of conversion (DC), bond strength (BS) and failure mode (FM) of adhesive interfaces in dentin cavities restored with a bulk-fill resin composite. Methods 4-mm-deep dentin cavities with a 3.1 C-factor were created in 68 bovine incisors (n = 17 per group). The light-cured (Scotchbond™ Universal) or the dual-cured (Adper™ Scotchbond™ Multi-purpose Plus) adhesive system was applied to the cavities, which were then restored with a bulk-fill resin composite (Filtek™ Bulk Fill). In situ DC analysis was performed by means of micro Raman spectroscopy at the top and bottom interfaces. Push-out BS was measured in a universal testing machine after 24-h or 6-month water storage. FM was determined with a stereomicroscope. Data of in situ DC and BS were analyzed by two-way analysis of variance (ANOVA) and Tukey test (p<0.05), while the FM was analyzed descriptively. Results The groups that received the dual-cured adhesive system showed statistically higher in situ DC and BS than those that received the light-cured adhesive system. Cohesive failure mode was the most frequent in all conditions. Conclusion In situ DC and BS were influenced by the curing strategies of the adhesive systems with better performance of the dual-cured material.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

ABSTRACT The objective was to evaluate the effects of maturity at harvest and row spacing on fermentative profile, aerobic stability, and nutritional value of biomass sorghum (BRS 716) silage. The experiment was conducted using a split-plot completely randomized block design with three row spacings (45, 70, and 90 cm) and four maturities at harvest (70, 100, 130, and 160 days) and eight replications. Polyvinyl chloride silos of known weight measuring 50 cm length and 10 cm diameter were used for silage production. Dry matter and total carbohydrate contents of the silage increased linearly, whereas crude protein and ash decreased linearly with maturity at harvest. Row spacing did not influence pH, ammoniacal nitrogen (N-NH3), gas and effluent losses, and dry matter recovery of silage. The concentrations of malic, succinic, and acetic acids and ethanol responded quadratically to maturity at harvest. The levels of neutral detergent fiber, lignin, and indigestible neutral detergent fiber increased linearly with maturity at harvest. Ruminal degradation kinetics of dry matter of biomass sorghum silage was not influenced by row spacing. BRS 716 biomass sorghum should be planted at 70-cm row spacing and harvested at 160 days for silage production based on fermentative profile, dry matter losses, and nutritional characteristics.

ABSTRACT Mobile phones (MPs) have become an important work tool around the world including in hospitals. We evaluated whether SARS-CoV-2 can remain on the surface of MPs of first-line healthcare workers (HCW) and also the knowledge of HCWs about SARS-CoV-2 cross-transmission and conceptions on the virus survival on the MPs of HCWs. A cross-sectional study was conducted in the COVID-19 Intensive Care Unit of a teaching hospital. An educational campaign was carried out on cross-transmission of SARS-CoV-2, and its permanence in fomites, in addition to the proper use and disinfection of MPs. Herewith an electronic questionnaire was applied including queried conceptions about hand hygiene and care with MP before and after the pandemic. The MPs were swabbed with a nylon FLOQ Swab™, in an attempt to increase the recovery of SARS-CoV-2. All MP swab samples were subjected to SARS-CoV-2 RT-PCR; RT-PCR positive samples were subjected to viral culture in Vero cells (ATCC® CCL-81™). Fifty-one MPs were swabbed and a questionnaire on hand hygiene and the use and disinfection of MP was applied after an educational campaign. Most HCWs increased adherence to hand hygiene and MP disinfection during the pandemic. Fifty-one MP swabs were collected and two were positive by RT-PCR (4%), with Cycle threshold (Ct ) values of 34-36, however, the cultures of these samples were negative. Although most HCWs believed in the importance of cross-transmission and increased adherence to hand hygiene and disinfection of MP during the pandemic, SARS-CoV-2 RNA was detected in MPs. Our results suggest the need for a universal policy in infection control guidelines on how to care for electronic devices in hospital settings.

ABSTRACT The objective of this study was to evaluate the effects of wilting times and application of an enzymatic-bacterial inoculant on the fermentative profile and nutritional characteristics of BRS capiaçu grass silage in a semi-arid region. Four wilting times treatments (control, 6, 24, and 30 h), with or without the addition of an enzymatic-bacterial inoculant, were analyzed as a split-plot completely randomized design with eight replications. Parameters of the rumen degradability test were analyzed using a split-plot completely randomized block design with four replications. There was no interaction between wilting times and inoculant application on pH, ammoniacal nitrogen (NH3-N), and aerobic stability of BRS capiaçu silage. Aerobic stability was reduced by 1.2 h for every 1-h increase in wilting time. Inoculant application reduced the pH values by 2.59% and extended the aerobic stability of the silage by 19 h. There was a significant interaction of wilting times and inoculant application on the levels of malic, succinic, lactic, and acetic acids. Inoculant application increased the contents of dry matter, ash, crude protein, insoluble neutral detergent fiber, and total carbohydrates by 3.63, 6.13, 7.73, 6.39, and 9.97% compared with non-inoculated silages, respectively. Wilting times for up to 30 h and application of enzymatic-bacterial inoculant improves the fermentative profile and chemical composition and reduces dry matter losses of silage of BRS capiaçu grass harvested at 100 days of regrowth.

RESUMO Objetivo: Avaliar características e problemas pessoais, profissionais, médicos e de educação científica reportados por pediatras. Métodos: Estudo transversal com base em uma pesquisa online, incluindo 614 pediatras formados nos últimos 15 anos no Departamento de Pediatria de uma universidade brasileira. Resultados: A taxa de resposta foi de 331/614 (54%). A maioria dos participantes era do sexo feminino (82%), a mediana de idade foi de 33 anos (27-40 anos) e a mediana de tempo de prática pediátrica foi de 5 anos (1-13). Jornada de trabalho elevada (>60 horas/semana) foi relatada por 25% dos entrevistados e 47% recebiam ≥15 salários mínimos/mês. Os problemas relacionados ao trabalho mais frequentes foram jornadas longas de trabalho, vida social insatisfatória e sedentarismo (>50%). Os pediatras foram divididos em dois grupos de acordo com os anos de prática clínica pediátrica: grupo 1 (≤5 anos) e grupo 2 (>5 anos). A mediana de satisfação geral com a residência pediátrica [8 (0-10) versus 9 (4-10); p=0,002] foi significativamente menor no grupo 1. As frequências de jornada de trabalho >60 horas, trabalho na enfermaria pediátrica e na terapia intensiva pediátrica foram significativamente maiores no primeiro grupo (p<0,05). Quanto aos principais problemas relacionados à prática clínica no ano anterior, jornadas longas de trabalho (73 versus 53%; p<0,001), vida social insatisfatória (75 versus 62%; p=0,018) e assédio (23 versus 4%; p=0,003) foram significativamente mais elevados no grupo 1. Conclusões: Pediatras em início de carreira (≤5 anos) relataram maior jornada de trabalho, menor renda, problemas relacionados ao trabalho e diferentes locais de trabalho em comparação com pediatras mais experientes (>5 anos). A satisfação geral com a residência pediátrica foi boa, porém menor em pediatras do primeiro grupo.

The most work-related issues reported were long working hours, poor social life and a sedentary lifestyle (>50%). Pediatricians were further divided into two groups, according to years of pediatric clinical practice: group 1 (≤5 years) and group 2 (>5 years). The median of overall satisfaction with pediatric residency [8(0-10) vs. 9 (4-10); p=0.002] was significantly reduced in group 1. The frequencies of workload >60 hours, work on pediatric ward and pediatric intensive care were significantly higher in the first group (p<0.05). Regarding main issues related to clinical practice in the last year, long working hours (73 vs. 53%; p<0.001), poor social life (75 vs. 62%; p=0.018) and harassment (23 vs. 4%; p=0.003) were significantly higher in the first group. Conclusions: Very early career pediatricians (≤5 years) reported higher workload, lower income, work-related issues and different location of pediatric practice compared to early career pediatricians (>5 years). The overall satisfaction with pediatric residency was good, however, reduced in very early career pediatricians.
