ABSTRACT Introduction: the postgraduate educational climate (CE) in the hospital environment can determine the way in which resident physicians (MRs) adapt to their new stage, during which professional burnout syndrome (PAS) is highly likely to occur at some point. Objective: to determine the relationship between the educational climate and professional burnout in physicians in the medical residency programs of the National Hospital 2023-2024. Methodology: cross-sectional observational study of a sample of MR convenience of both sexes from all medical specialties of the National Hospital between November 2023 and January 2024, MR with external rotation and/or vacations were excluded. Sample calculation; 136 MR. An online form from Google was sent to the MR email requesting voluntary participation. The SAP was measured with the standard Maslach Burnout Inventory (MBI) method with subscales; emotional exhaustion (EA), depersonalization (DESP), personal fulfillment (PR), with PAS with high scores of AE, DESP and low RP. The EC was assessed with the measure of the hospital postgraduate educational environment (PHEEM) with subscales; perception of role autonomy (PAR), perception of teaching (PE), perception of social support (PSS), the higher the score, the better the EC. Demographic and academic variables, MBI and PHEEM scores were collected, and the SAP was considered dependent, as the main result was the predictive relationship of the PHEEM on the SAP. Data were compared with χ², t-test or Mann-Whitney U, ANOVA and linear regression (p-significant value =<0.05). Results: of 233 participants, the mean age was 28.75 (±2.20), 63.94 % (149) women, the general prevalence of PAS; 70.38 % (164/233), PAS in men 75 % (63/84) and women 67.78 % (101/149) p=0.0014, the first 2 years with the highest frequency of PAS; 65 (85.52 %), 59 (86.76 %) respectively p=0.0009, 63 (90 %) p=0.0071 surgical specialties. The subscales of the MBI, the AE; 79.82 % (186) with an average of 35.39 (± 11.61) points. The CE of MRs with SAP; 107.9 (19.6) dots and without PAS; 136.3 (12.1) dots 95 % CI 18.1-25.0 p= 0.0001. The PHEEM predicted the AE with R2=0.619, B= -0.18 (±0.009) p=0.001, and the DESP; R2= 0.385, B= -0.001(0.004) p=0.009, PSS being a significant predictor with inverse correlation in both. Conclusion: there is an inverse relationship between the educational climate and burnout syndrome in the population studied, with a high prevalence of burnout in male residents of surgical specialties, with the social support of the hospital educational environment being a significant predictor for the development of burnout syndrome.

RESUMEN Introducción: el clima educacional (CE) de posgrado en el ambiente hospitalario puede determinar la manera en como los médicos residentes (MR) se adaptan a su nueva etapa, durante el cual el sindrome agotamiento profesional (SAP) es altamente probable de producirse en algún momento. Objetivo: determinar la relación entre el clima educacional y el agotamiento profesional en médicos de los programas de residencias médicas del Hospital Nacional 2023-2024. Metodología: estudio observacional transversal de una muestra por conveniencia de MR de ambos sexos de todas las especialidades médicas del Hospital Nacional entre noviembre 2023 y enero 2024, se excluyeron MR con rotación externa y/o vacaciones. Calculo muestral; 136 MR. Se envío un formulario en línea de Google al correo electrónico del MR solicitando la participación voluntaria. El SAP fue medido con el método estándar Maslach Burnout Inventory (MBI) con subescalas; agotamiento emocional (AE), despersonalización (DESP), realización personal (RP), existiendo SAP con puntuaciones altas de AE, DESP y bajas de RP. El CE fue valorado con la medida del entorno educativo de posgrado hospitalario (PHEEM) con subescalas; percepción autonomía de roles (PAR), percepción enseñanza (PE), percepción soporte social (PSS), a mayor puntaje mejor CE. Se recopiló variables demográficos, académicos, puntajes del MBI y PHEEM, el SAP fue considerado dependiente, como resultado principal la relación predictiva del PHEEM sobre el SAP. Se compararon datos con χ², prueba t o U de Mann-Whitney, ANOVA y regresión lineal (p-valor significativo =<0,05). Resultados: de 233 participantes, la edad media fue de 28,75 (± 2,20), el 63,94 % (149) mujeres, la prevalencia general del SAP; 70,38 % (164/233), SAP en hombres 75 % (63/84) y mujeres 67,78 % (101/149) p=0,0014, los 2 primeros años con mayor frecuencia de SAP; 65 (85,52 %), 59 (86,76 %) repectivamente p=0,0009, de especialidades quirúrgicas 63 (90 %) p=0,0071. Las subescalas del MBI el AE;79,82 % (186) con un promedio de 35,39 (± 11,61) puntos. El PHEEM de los MR con SAP; 107,9(( 19,6) puntos y sin SAP; 136,3(( 12,1) puntos IC 95 % 18,1-25,0 p= 0,0001. El PHEEM predijo el AE con R2=0,619, PPS; B= -0,18 (±0,009) p=0,001, y la DESP; R2= 0,385, PPS; B= -0,001 (±0,004) p=0,009 siendo PSS predictor significativo con correlación inversa en ambas. Conclusión: existe relación inversa entre el clima educativo y el sindrome de agotamiento profesional en la población estudiada, con alta prevalencia de este en residentes hombres de especialidades quirúrgicas, siendo el soporte social del ambiente educativo hospitalario un predictor significativo para el desarrollo del sindrome de agotamiento profesional.

Resumen: el presente texto es una breve muestra de la obra poética Tenbilal antsetik / Mujeres olvidadas (2022), de Susi Bentzulul, publicada por el Fondo de Cultura Económica, en coedición con el Fondo Editorial Tierra Adentro. El libro se compone de tres apartados, escrito en tsotsil/ español, y emerge con una voz que remueve aquellas “bisagras” sostenidas por imaginarios construidos socialmente: el padre, el hermano, la familia.

Resumen Las anomalías dentales (ADs) son defectos en la estructura, número, forma y tamaño de los dientes. En el presente estudio se evaluó la prevalencia de estas anomalías, tanto en la dentición primaria como permanente, en dos poblaciones mexicanas de 2 a 14 años de edad. Además, se determinó el nivel de conocimiento sobre ADs entre los estudiantes de la carrera de cirujano dentista, en ambas universidades. Se realizó un estudio transversal en niños que acudieron por primera vez a las Clínicas Odontopediátricas de las Universidades de Nuevo León y San Luis Potosí (México), entre marzo y mayo de 2017. Se estimó la prevalencia de las ADs, y se aplicó un cuestionario de 20 ítems relacionados con éstas a 100 estudiantes de licenciatura para determinar su nivel de conocimiento. Se incluyeron 94 niños (54 niños y 42 niñas; edad media 9.73±2.1 años). Veintidós pacientes (23.4%) presentaban al menos una AD. Las más frecuentes fueron macrodoncia e hipoplasia del esmalte. La agenesia ocurrió en tres ocasiones, y hubo dos casos de mesiodens. En cuanto al sexo, 10 pacientes eran niñas (45,4%) y 12 niños (54,6%). La prevalencia de ADs en dientes permanentes (57.5%) fue superior a la de dientes temporales (42.5%). Sólo dos pacientes presentaron ADs en los dientes temporales. Sólo dos pacientes presentaban ADs en ambas denticiones. No se detectaron anomalías de forma. En cuanto al nivel de conocimientos entre los estudiantes universitarios, el porcentaje de respuestas correctas fue de 61.6%. El 23.4% de los participantes presentaba al menos una AD en los dientes primarios o permanentes, como macrodoncia e hipoplasia del esmalte. Estos resultados demuestran lo crucial que es alentar a los padres a visitar al odontopediatra a una edad temprana y la importancia de realizar una evaluación integral de los pacientes.

Abstract Dental anomalies (DAs) in children refer to conditions affecting the structure, number, shape, and size of teeth. This study aimed to assess the prevalence of these anomalies in both primary and permanent dentitions among two Mexican pediatric populations aged 2-14 years. Additionally, the level of knowledge about DAs among students pursuing a degree in dentistry in both universities was determined. A cross-sectional study was conducted on children making their first visit to the Pediatric Dental Clinics of the Universities of Nuevo León and San Luis Potosí in México between March and May 2017. The prevalence of DAs was estimated. In addition, a questionnaire comprising 20 closed items regarding DAs was administered to 100 undergraduate students to evaluate their level of knowledge. Ninety-four children (54 boys and 42 girls; mean age 9.73±2.1 years) were included. Twenty-two patients (23.4%) exhibited at least one DA in their primary or permanent teeth. The most common DAs were in size (macrodontia) and structure (enamel hypoplasia). Agenesis occurred three times, and there were two cases of mesiodens. Regarding gender, 10 patients were girls (45.4%), and 12 were boys (54.6%). The prevalence of DAs in permanent teeth (57.5%) was higher than that in primary teeth (42.5%, p=0.031). Only two patients presented DAs in both primary and permanent teeth. No tooth-shaped anomalies were detected. Regarding the level of knowledge among the university students, the percentage of correct answers was 61.6%. Our findings emphasize the importance of encouraging parents to initiate pediatric dental visits early. Additionally, the study highlights the significance of conducting a thorough evaluation of the patients.

ABSTRACT This work investigates the potentiality of the 2D electrical resistivity tomography method to delineate an altered intrusive rock domain. The detection and characterization of such altered domains are crucial to understanding local hydrogeological conditions. Here, the target is the San Luis area, located in the northern part of the Estibaná River watershed, within the Los Santos Province in the central region of the Isthmus of Panama. First, a detailed geological survey was carried out to better constrain the tectonic structures. Then, two geological sections were developed based on two 2D electrical resistivity tomographies. A geoelectrical simulation process was conducted in both sections to set theoretical models whose model responses (synthetic field data set) would be consistent with the observations. After appropriate selection of the inversion parameters by using the Boundless Electrical Resistivity Tomography (BERT) code, the observations were processed, resulting in low calculation errors (χ2 < 0.51). Based on the superficial geological information and the results of the geophysical modelling, two profiles were established, depicting the lateral and in-depth distribution of rocks and sediments. The limit of the Ocú-Parita fault zone was identified as well. The results of the 2D inversion of the synthetic data, obtained with the established theoretical models, are consistent with the 2D inversion of the observations. Electrical resistivity tomography studies can be useful in establishing groundwater management models, particularly in areas that experience severe water scarcity during the dry season.

RESUMEN Este trabajo investiga la capacidad del método de tomografía de resistividad eléctrica 2D para delinear un dominio de roca intrusiva alterada. La detección y caracterización de este tipo de intrusivos juega un papel importante en esclarecer el contexto hidrogeológico local. La zona de estudio corresponde al área de San Luis, ubicada en la parte norte de la cuenca del río Estibaná, provincia de Los Santos, región central del istmo de Panamá. Para reconocer las estructuras tectónicas de la zona, se realizó primero un levantamiento geológico superficial de detalle. Posteriormente, se efectuaron dos tomografías de resistividad eléctrica 2D a fin de confeccionar dos secciones geológicas. Un proceso de simulación geoeléctrica fue llevado a cabo en ambas secciones para establecer modelos teóricos cuyas respuestas (conjunto de datos de campo sintéticos) fuesen acordes con las observaciones. Posterior a la selección apropiada de los parámetros de inversión mediante el código BERT (Boundless Electrical Resistivity Tomography), las observaciones fueron procesadas, obteniéndose errores de cálculo bajos (χ2 < 0,51). Al combinar la información geológica de superficie y los resultados del modelado geofísico, se crearon dos perfiles geológicos donde es posible observar la distribución lateral y en profundidad de rocas y sedimentos, así como el límite de la zona de falla Ocú-Parita. Los resultados de la inversión 2D de los datos sintéticos obtenidos mediante los modelos teóricos establecidos concuerdan con aquellos de la inversión 2D de las observaciones. Estudios de tomografía de resistividad eléctrica pueden ser útiles en proporcionar modelos de gestión de agua subterránea, particularmente en áreas que experimentan escasez hídrica severa durante la temporada seca.

Resumen Introducción : Objetivo: comparar sensibilidad diag nóstica de videocolposcopia con inteligencia artificial (IA) auxiliar, con la videocolposcopia común realizada por colposcopistas. Métodos : Estudio descriptivo de corte transversal retrospectivo, en 782 historias clínicas anonimizadas del Sistema Informático para el Tamizaje (SITAM), de mujeres a las cuales se les efectuaron videocolposcopia con IA y colposcopías con videocolposcopio común rea lizadas por especialistas, con sus biopsias (gold standard). Se evaluó la relación entre los resultados de videocol poscopia con IA y videocolposcopia común con resulta dos de las biopsias. Se calculó precisión global de cada procedimiento diagnóstico. Se determinó sensibilidad y concordancia de los resultados de la videocolposcopia con IA, con el gold standard. Resultados : Se analizaron 395 historias clínicas de pacientes con videocolposcopia con IA y 387 con vi deocolposcopia común. La precisión diagnóstica de resultados fue 80% (IC 95%: 75-83%) en videocolposco pias con IA y 65% (IC 95%: 60-69%) en videocolposcopia común (p<0.001). Los resultados de videocolposcopia con IA y colposcopia común se correlacionaron signifi cativamente con los resultados de las biopsias, rs=0.75 vs. rs=0.57 respectivamente (p<0.001). La sensibilidad de videocolposcopia con IA fue 96% (IC 95%: 94-98%), y 93% (IC 95%: 89-95%) en colposcopías comunes. La concordancia general de las impresiones colposcópicas clasificadas por videocolposcopia con IA y enfermedad fue mayor que la de la interpretación colposcópica de los colposcopistas (90% frente a 83%, Kappa 0.59 frente a 0.47, p<0.001). Conclusión : La alta precisión diagnóstica de video colposcopia con IA permite aumentar la sensibilidad del estudio y mejorar la detección precoz de lesiones precursoras de neoplasias cervicouterinas.

Abstract Introduction : To compare the diagnostic sensitivity of artificial intelligence (AI) assisted videocolposcopy with standard videocolposcopy performed by specialist colposcopists. Methods : A descriptive retrospective cross-sectional study, 782 anonymized medical records from the Com puterized System for Screening (SITAM) of women who underwent videocolposcopy with AI and colposcopy with common videocolposcopy performed by specialists, with their corresponding biopsies (gold standard) were analyzed. The relationship between the results of IA videocolposcopy and regular videocolposcopy and the results of biopsies was evaluated. The overall accuracy of each diagnostic procedure was calculated. The sensitiv ity and concordance of the results of AI videocolposcopy with the gold standard (biopsy) were determined. Results : A total of 395 patient records of patients with IA videocolposcopy and 387 with regular vi-deocolposcopy were analyzed. The accuracy of results was 80% (IC 95%: 75-83%) in IA videocolposcopy and 65% (IC 95%: 60-69%) in regular videocolposcopy (p<0.001). Videocolposcopy results with IA and com mon colposcopy were significantly correlated with bi opsy results, rs=0.75 vs. rs=0.57 respectively (p<0.001). The sensitivity of videocolposcopy with AI was 96% (95% CI: 94-98%), and 93% (95% CI: 89-95%) for regu lar colposcopy. The overall agreement of colposcopic impressions classified by videocolposcopy with AI and disease was higher than that of colposcopic interpretation by colposcopists (90% vs. 83%, Kappa 0.59 vs. 0.47, p<0.001). Conclusion : The high diagnostic accuracy of AI vi-deocolposcopy allows obtaining highly sensitive studies that help in the early detection of precursor lesions of cervical neoplasia.

Resumen Introducción: El lupus eritematoso sistémico (LES) es una enfermedad autoinmune que causa inflamación sistémica y alteraciones en la tolerancia inmunológica. La activación de los genes inducibles por interferón (IFN), contribuye en más del 50% de su patogenia. Objetivo: relacionar el papel del IFN-λ en la patogenia del LES. Materiales y Métodos: Búsqueda sistémica en base de datos; a través de las palabras claves del MeSH and DeCS. Fue incluido adicionalmente la palabra “Interferón Lambda”. Resultados: Se encontró que la producción aberrante de interferón tipo I contribuye a la desregulación de IFN-λ, producido principalmente por células dendríticas plasmocitoides. Este proceso conduce a la estimulación inmunológica por autoanticuerpos y a un aumento de IFNλR-1 en células B, potenciando la generación de anticuerpos. IFN-λ3 se asocia particularmente con nefritis lúpica, y el IFN-λ en general aumenta la expresión de MHC-I, intensificando la respuesta de células T CD8+ y posiblemente afectando la tolerancia central y la regulación en el timo. Conclusión: Se destaca que el IFN-λ favorece la activación inmune, formación de inmunocomplejos, inflamación crónica y producción de autoanticuerpos, vinculándose niveles altos de IFN-λ3 con mayor actividad de la enfermedad.

Abstract Introduction: Systemic lupus erythematosus (SLE) is an autoimmune disease that causes systemic inflammation and alterations in immunological tolerance. The activation of interferon (IFN)-inducible genes contributes to more than 50% of its pathogenesis. Objective: to review the role of IFN-λ in the pathogenesis of SLE. Materials and Methods: Systemic search in database; through the MeSH and DeCS keywords. The word “Lambda Interferon” was additionally included. Results: Aberrant production of type I interferon was found to contribute to the deregulation of IFN-λ, produced mainly by plasmacytoid dendritic cells. This process leads to immunological stimulation by autoantibodies and an increase in IFNλR-1 in B cells, enhancing the generation of antibodies. IFN-λ3 is particularly associated with lupus nephritis, and IFN-λ generally increases MHC-I expression, enhancing the CD8+ T cell response and possibly affecting central tolerance and regulation in the thymus. Conclusion: It is highlighted that IFN-λ favors immune activation, formation of immune complexes, chronic inflammation and production of autoantibodies, linking high levels of IFN-λ3 with greater disease activity.

Resumen: En el libro Todo cambió de Josías López Gómez está en juego la construcción del Yo a través de la memoria autobiográfica. Esta noción, al referirse a una “experiencia de vida” de cada sujeto en la investigación experimental, en la literatura la concibo como parte de una “experiencia estética” de configuración del sujeto narrativo a través del tiempo. En el relato literario, la memoria autobiográfica la analizo, por un lado, mediante elementos de la narratología que me permiten ver el movimiento del Yo discursivo en el tiempo y espacio textual; por otro lado, es necesario complementar la investigación con datos obtenidos a partir del contexto de producción de la obra y de entrevistas al autor para engarzar su experiencia de vida con algunos personajes que le permiten construir un nuevo sujeto narrativo a la luz de un sujeto ficticio que se piensa y cuestiona desde la modernidad.

Abstract: The book Todo cambió (Everything Changed) by Josías López Gómez engages in the construction of self through autobiographical memory. This notion can refer to the “life experience” of each subject in an experimental investigation; in literature, I conceive it as part of an “aesthetic experience” of the narrative subject’s configuration across time. In López Gómez’s literary works, I analyze autobiographical memory, on the one hand, through narratological elements that permit me to see the movement of the discursive self in textual time and space; on the other hand, it is necessary to complement the research with information from the work’s creative context and interviews with the author. The analysis shows how connects his own experience with some of his characters to construct a new narrative subject in light of a fictional subject conceived in and questioning from modernity.

Abstract Objective: The study aims to apply the CLS in patients undergoing left-sided colorectal surgery. Method: Retrospective study in patients with the left-sided colorectal surgery and primary anastomosis without diverting stoma. CLS was calculated in patients, who were classified in AL and NO-AL groups. Predictive value of CLS was evaluated by receiver operator characteristic. Correlation between CLS and AL was determined. 208 patients (55% male, mean age 59 years) were included in the study. Results: Overall, AL was 7.2%. Mean CLS of all patients was 7.2 ± 3.2 (0-17). Patients with AL had a higher CLS (11.8 ± 2.3) than NO-AL patients (6.8 ± 3) (p = 0.0001). The area under the curve for the prediction of AL by CLS was 0.898 ([CI] 0.829-0.968, p = 0.0001). A CLS of 8.5 had 93% sensitivity and 72% specificity. There was a statistically significant odds ratio for CLS and AL (0.58: [CI] 0.46-0.73, p = 0.0001). Conclusions: CLS is a useful tool to predict AL in the left-sided colorectal surgery.

Resumen Objetivo: Este estudio tiene el objetivo de aplicar el CLS en pacientes con cirugía colorrectal de lado izquierdo. Método: Estudio retrospectivo en pacientes con cirugía colorrectal izquierda y anastomosis primaria sin estoma de derivación. Se calculó el CLS en los pacientes, los cuales fueron clasificados en los grupos con AL y sin AL. Resultados: La media del CLS de todos los pacientes fue de 7.2 ± 3.2 (0-17). Los pacientes con AL tenían un CLS más alto (11.8 ± 2.3) que los pacientes sin AL (6.8 ± 3) (p = 0.0001). El área bajo la curva para la predicción de la AL mediante el CLS fue de 0.898 (intervalo de confianza (CI) 0.829-0.968; p = 0.0001). Un CLS de 8.5 tuvo una sensibilidad del 93% y una especificidad del 72%. Además, se obtuvo un Odds Ratio con una diferencia estadísticamente significativa para el CLS y AL (0.58; CI 0.46-0.73; p = 0.0001). Conclusiones: La CLS es una herramienta útil para predecir la AL en la cirugía colorrectal del lado izquierdo.

Resumen Este trabajo documenta la presencia de Eristalis tenax en un establecimiento lechero de la provincia de Salta. Las larvas de E. tenax, conocidas como "gusanos de cola de rata", se encontraron específicamente en el comedero y la fosa de desechos. La observación de las características morfológicas de la especie permitió su identificación taxonómica. Este hallazgo amplía la distribución geográfica conocida de E. tenax en Argentina. Aunque la importancia médica de estos sírfidos es relativa, se destaca su capacidad de actuar como vectores mecánicos de micobacterias, lo que sugiere la necesidad de controlar sus poblaciones en entornos antropizados. Se enfatiza la importancia de medidas de higiene para prevenir la ingestión accidental de larvas y se sugiere incorporar supervisión en depósitos de agua y piletones de decantación. En conclusión, se subraya la relevancia de reconocer la naturaleza facultativa de esta especie como posible vector y la contribución de su control en las medidas de saneamiento en rodeos afectados.

Abstract The presence of Eristalis tenax in a dairy farm in the province of Salta is described. Larvae of E. tenax, known as "rat-tailed maggots", were found in large quantities on the premises, specifically in the feeding trough and waste pit. Taxonomic identification revealed distinctive morphological characteristics of the species. This finding expands the known geographical distribution of E. tenax in Argentina. Although the medical significance of these syrphids is relative, their ability to act as mechanical vectors of certain pathogens, such as mycobacteria, suggests the need to control their populations in anthropized environments. The importance of hygiene measures to prevent accidental ingestion of larvae is stressed, and monitoring of water tanks and pits is suggested. In conclusion, the relevance of recognizing the facultative nature of infestations is highlighted, along with the potential contribution of E. tenax control in sanitation measures of affected herds.

ABSTRACT Objective: To determine the standard surgical times of the four most common general surgery procedures (unilateral inguinal hernioplasty, bilateral inguinal hernioplasty, umbilical hernioplasty and cholecystectomy) in a second-level hospital and to estimate the probability of extending the time of each of the procedures. Efficiency is a widely studied subject in economics. It involves the need for fewer elements in the production of a certain level of goods and services. Therefore, it is extremely important to consider it in the field of surgery. Materials and methods: An observational, descriptive and retrospective study. It used the operating room records from 2017 to 2019 of the General Surgery service in a second-level hospital. Based on this information, the time required for each procedure was standardized using the mean for each one (umbilical hernioplasty, unilateral or bilateral inguinal hernioplasty and cholecystectomy). The probability of extending surgical times was estimated based on the obtained data and confidence interval. Results: The mean for unilateral inguinal hernioplasty was 76 min (95.00 % CI: 72-80 min, SD 23), for bilateral inguinal hernioplasty 104.38 min (95.00 % CI: 91-116 min, SD 41.7), for umbilical hernioplasty 59.31 min (95.00 % CI: 54-63 min, SD 29.99) and for cholecystectomy 85.735 min (95.00 % CI: 83-88 min). The probability of scheduling three surgical interventions and completing all of them on time (upper limit of the CI) is 92.69 %, and the probability of scheduling three surgical interventions and extending the time of all of them is 0.0016 % (lower limit of the CI). Conclusions: Planning scheduled operations using standardized surgical times is feasible. Updated statistics on surgical procedures (average time for each procedure) are required since it is possible to more accurately detect and supervise operating room dynamics by identifying opportunity areas. This will make operating room time more efficient for the benefit of health care systems and patients.

RESUMEN Objetivo: Determinar los tiempos quirúrgicos estándar de los cuatro procedimientos más comunes en cirugía general (hernioplastia inguinal unilateral, hernioplastia inguinal bilateral, hernioplastia umbilical y colecistectomía) de un hospital de segundo nivel y calcular la probabilidad de extensión de cada uno de los procedimientos. La eficiencia es un fenómeno ampliamente estudiado en el ámbito económico, pues hace referencia a la necesidad de menor cantidad de factores para la producción de un determinado nivel de bienes y servicios, por ello, es de vital importancia incluirlo en el ámbito quirúrgico. Materiales y métodos: Estudio observacional, descriptivo y retrospectivo. Se utilizaron los registros de quirófano de un hospital de segundo nivel del año 2017 al 2019 del servicio de Cirugía General. A partir de esta información, se estandarizó el tiempo necesario para cada procedimiento mediante la media de cada uno (hernioplastia umbilical, hernioplastia inguinal unilateral o bilateral y colecistectomía). Se calculó la probabilidad de extensión de las cirugías tomando en consideración los datos obtenidos y el intervalo de confianza. Resultados: Para el procedimiento de hernioplastia inguinal unilateral se obtuvo una media de 76 min (IC 95,00 %: 72-80 min, DE 23); en hernioplastia inguinal bilateral, una media de 104,38 min (IC 95,00 %: 91-116 min, DE 41,7); en hernioplastia umbilical, una media de 59,31 min (IC 95,00 %: 54-63 min, DE 29,9), y en colecistectomía, una media de 85,735 min (IC 95,00 %: 83-88 min). La probabilidad de que se programen tres cirugías y todas estén a tiempo (límite superior del IC) es de 92,69 %, la probabilidad de que se programen tres cirugías y todas se prolonguen es de 0,0016 % (límite inferior del IC). Conclusiones: Es posible realizar la planeación de las cirugías programadas mediante el uso de tiempos quirúrgicos estandarizados. Se requiere contar con estadística actualizada de los procedimientos quirúrgicos (promedios del tiempo de realización de cada procedimiento), ya que es posible detectar y supervisar de manera más precisa la dinámica de quirófano mediante la detección de las áreas de oportunidad, de esta manera, se eficientizará el tiempo de quirófano para beneficio de los sistemas de salud y los pacientes.

ABSTRACT Objective. To identify the presence of the SARS-CoV-2 virus in wastewater from hospitals in Peru. Materials and methods. Water samples were collected from the effluents of nine hospitals in Peru during March and September 2022. SARS-CoV-2 was identified by using Illumina sequencing. Variant, lineage and clade assignments were carried out using the Illumina and Nextclado tools. We verified whether the SARS-CoV-2 variants obtained from wastewater were similar to those reported by the National Institute of Health of Peru from patients during the same period and region. Results. Eighteen of the 20 hospital wastewater samples (90%) provided sequences of sufficient quality to be classified as the Omicron variant according to the WHO classification. Among them, six (30%) were assigned by Nextclade to clades 21K lineage BA.1.1 (n=1), 21L lineage BA.2 (n=2), and 22B lineages BA.5.1 (n=2) and BA .5.5 (n=1). Conclusions. SARS-CoV-2 variants were found in hospital wastewater samples and were similar to those reported by the surveillance system in patients during the same weeks and geographic areas. Wastewater monitoring could provide information on the environmental and temporal variation of viruses such as SARS-CoV-2.

RESUMEN Objetivo. Identificar la presencia del virus SARS-CoV-2 en aguas residuales de hospitales en Perú. Materiales y métodos. Se recolectaron muestras de agua en los efluentes de nueve hospitales del Perú durante marzo y septiembre de 2022 y se realizó la identificación de SARS-CoV-2 mediante secuenciación Illumina. Las asignaciones de variantes, linajes y clados se llevaron a cabo con las herramientas Illumina y Nextclado. Verificamos si las variantes de SARS-CoV-2 obtenida de las aguas residuales fueron similares a las reportadas por el Instituto Nacional de Salud del Perú procedentes de pacientes durante el mismo período y región. Resultados. Dieciocho de las 20 muestras de aguas residuales hospitalarias (90%) proporcionaron secuencias con la calidad suficiente para ser clasificadas como variante Ómicron según la clasificación de la OMS. Entre ellos, seis (30%) fueron asignados por Nextclade a los clados 21K linaje BA.1.1 (n=1), 21 L linaje BA.2 (n=2) y 22B linajes BA.5.1 (n=2) y BA.5.5 (n=1). Conclusiones. Se encontraron variantes del SARS-CoV-2 en muestras de aguas residuales hospitalarias y que fueron similares a las reportadas por el sistema de vigilancia en pacientes durante las mismas semanas y áreas geográficas. El monitoreo de aguas residuales podría proporcionar información sobre la variación ambiental y temporal de virus como el SARS-CoV-2.

RESUMEN Introducción: En 2020, el cáncer de pulmón se convirtió en la segunda causa de muerte, según datos de la Organización Mundial de la Salud. Aunque los pulmones son un asiento frecuente de metástasis de cánceres originados en órganos extratorácicos, el cáncer primario pulmonar también es muy frecuente. Objetivo: Caracterizar a los pacientes con cáncer pulmonar según variables clínicas y epidemiológicas. Métodos: Se realizó un estudio observacional descriptivo y retrospectivo para caracterizar la presencia de cáncer pulmonar de acuerdo con dos variables (clínicas y epidemiológicas), en 110 pacientes adultos atendidos en el servicio de Neumología del Hospital Universitario Clínico-Quirúrgico «Arnaldo Milián Castro», en el período comprendido de octubre 2020 a octubre 2022. Resultados: La mayoría de los pacientes diagnosticados con cáncer pulmonar correspondieron al año 2020 (38,18 %): femeninos (59,09 %), blancos (59,09 %), grupo de edad de 60 - 69 años (44,50 %), jubilados (63,63 %), fumadores (82,72 %), y con antecedentes familiares de esta enfermedad (97,27 %). La tos fue el síntoma principal (95,45 %) y predominó el carcinoma escamoso poco diferenciado infiltrante (19,09 %). El tipo de cáncer pulmonar fue el de células no pequeñas(84,54 %) y como tratamiento, la quimioterapia (49,09 %). Conclusiones: Los pacientes con cáncer de pulmón fueron diagnosticados con más frecuencia en 2020, predominó el sexo femenino y el color de la piel blanco, mayores de 60 años, jubilados y fumadores. La variedad histológica más frecuente fue el carcinoma de células no pequeñas, y de ellos, el escamoso poco diferenciado infiltrante.

ABSTRACT Introduction: lung cancer became the second cause of death in 2020 according to data from the World Health Organization. Primary lung cancer is very common although the lungs are also a frequent site of metastasis originated from extrathoracic organs. Objective: to characterize patients with lung cancer according to clinical and epidemiological variables. Methods: an observational, descriptive and retrospective study was carried out to characterize clinically and epidemiologically the presence of lung cancer in 110 adult patients treated in the Pulmonology service at “Arnaldo Milıán Castro” Clinical and Surgical University Hospital from October 2020 to October 2022. Results: most of the patients diagnosed with lung cancer corresponded to the year 2020 (38.18%): female (59.09%), white (59.09%), age group 60 - 69 years (44.50 %), retirees (63.63%), smokers (82.72%) and with a family history of this disease (97.27%). Cough was the main symptom (95.45%) and infiltrating poorly differentiated squamous cell carcinoma predominated (19.09%). The type of lung cancer was non-small cell cancer (84.54%) and the treatment was chemotherapy (49.09%). Conclusions: patients with lung cancer were diagnosed more frequently in 2020. Subjects over 60 years old, female gender, white skin color, retirees and smokers predominated. The most common histological variety was non-small- cell lung cancer and infiltrating poorly differentiated squamous cell carcinoma predominated.

ABSTRACT Introduction: Brachial plexus neuropathy is a rare complication of therapeutic pronation at the intensive care unit. Few studies explore the association between ICU-managed COVID-19 and the development of plexopathies of the upper limbs. Objective: Synthesize the medical literature on the relationship between COVID-19 requiring prone ventilation and brachial plexus neuropathy. Methods: A scoping review including observational studies in PubMed and Scopus. Results were summarized based on the general characteristics of each included article. Results: Eleven full-text articles were included, identifying case series (n = 4), case reports (n = 3), retrospective (n = 2) and prospective (n = 2) cohort studies, with a total sample of 361 patients. The brachial plexus injury is a rare complication of prone ventilation in the ICU. Males with a history of high blood pressure, diabetes mellitus and obesity are more likely to develop this injury. The main clinical manifestations were weakness, paresthesia, and neuropathic pain with an onset time from one to four weeks after the end of pronation. Conclusion: Prone ventilation in the treatment of patients with severe COVID-19 has been increasing along with neurological complications such as brachial plexus neuropathy.

RESUMEN Introducción: La neuropatía del plexo braquial es una complicación rara de la pronación terapéutica en la Unidad de Cuidados Intensivos. Pocos estudios exploran la asociación entre la COVID-19 manejada en la UCI y el desarrollo de plexopatías de las extremidades superiores. Objetivo: Sintetizar la literatura médica sobre la relación entre el COVID-19 que requiere ventilación en decúbito prono y la neuropatía del plexo braquial. Métodos: Una revisión de alcance que incluye estudios observacionales en PubMed y Scopus. Los resultados se resumieron en función de las características generales de cada artículo incluido. Resultados: Se incluyeron 11 artículos a texto completo; se identificaron series de casos (n = 4), reportes de casos (n = 3), estudios de cohortes retrospectivos (n = 2) y prospectivos (n = 2) con muestra total de 361 pacientes. La lesión del plexo braquial es una complicación rara de la ventilación en decúbito prono en la UCI. Los varones con antecedentes de hipertensión arterial, diabetes mellitus y obesidad son más propensos a desarrollar esta lesión. Las principales manifestaciones clínicas fueron debilidad, parestesia y dolor neuropático con un tiempo de aparición de una a cuatro semanas después del final de la pronación. Conclusión: La ventilación de prono en el tratamiento de pacientes con COVID-19 grave ha ido en aumento junto con complicaciones neurológicas como la neuropatía del plexo braquial.

Abstract: This research seeks to know the experience of participants -15 caregivers of children between 4 and 12 years old from two schools with a high vulnerability index in Santiago de Chile and 4 facilitators - in the GenerationPMTO program adapted to a remote modality, in the context of the COVID-19 health crisis. A descriptive, exploratory, qualitative study was carried out. Semi-structured interviews with 15 caregivers from both schools participating in the program; one focus group with 4 facilitators who implemented the online program; and a documentary review of the journals registered by the facilitators after each session were conducted. The information collected was analyzed qualitatively through the open coding procedures of Grounded Theory. After that, the perspectives of both groups were gathered through an integrated analysis. From the experience of participants, the online adaptation of the program was successful since the facilitators were able to develop the contents and strategies of the intervention in virtual mode, and the families were able to participate and learn them from their homes. This was compatible with physical distancing (given the COVID-19 pandemic) and allowed caregivers' participation in the program to be compatible with other life activities. Among challenges, strengthening group participation and cohesion were highlighted, as well as overcoming barriers related to unstable internet connection and/or inappropriate home conditions to participate. These results show a successful adaptation of the program from the perspective of its participants, providing insights that contribute to improve the adaptation of this and other programs to virtual modality.

Resumen: Esta investigación buscó conocer la experiencia de los participantes -15 cuidadores de niños entre 4 y 12 años de dos escuelas con alto índice de vulnerabilidad en Santiago de Chile y 4 facilitadores - del programa GenerationPMTO en su adaptación a modalidad remota, en contexto de crisis sanitaria causada por COVID-19. Se realizó un estudio cualitativo exploratorio descriptivo, en el cual se realizaron entrevistas semiestructuradas a 15 cuidadores de ambas escuelas participantes del programa, un grupo focal con 4 facilitadoras que implementaron el programa de forma online y una revisión documental de las bitácoras completadas por las facilitadoras luego de cada sesión. La información recolectada fue analizada cualitativamente a través de los procedimientos de codificación abierta de la teoría fundamentada, para luego reunir las perspectivas de ambos grupos en un análisis integrado. Desde la experiencia de los participantes, la adaptación del programa a formato online resultó exitosa, ya que las facilitadoras lograron desarrollar los contenidos y estrategias de la intervención en modalidad virtual y los cuidadores pudieron participar y aprender desde sus casas, siendo esto compatible con el distanciamiento físico (por COVID-19) y con otras actividades de la vida de los cuidadores. Se destacan como desafíos potenciar la participación y cohesión grupal y superar barreras relacionadas con la conexión a Internet inestable y/o lugar físico poco adecuado en algunos hogares para participar. Estos resultados muestran una exitosa adaptación del programa desde la perspectiva de sus participantes, aportando aprendizajes que permitirán mejorar la adaptación de este y otros programas a modalidad virtual.
