ABSTRACT The use of crop modeling can be useful to understand the interactions between the soil-plant-atmosphere system. The objective of this study was to evaluate sensitivity analysis of the AquaCrop model parameters for wheat crop in the Campos Gerais Region. The varietie tested was TBIO Sinuelo in Castro, Ponta Grossa and Itaberá cities. The analyzed parameters refer to crop phenology, transpiration, biomass production, yield formation, stresses and soil management. The sensitivity analysis was realized varying individually each input parameter in the AquaCrop for the calculation of the Relative Sensitivity Index (SI). The most sensitive parameters of the AquaCrop were: reference harvest index (HIo); water productivity normalized for evapotranspiration and CO2 concentration (WP*); crop coefficient when canopy expansion is complete (KcTR,x); fertility levels; and maximum canopy cover (CCx). The higher sensitivity of HIo and WP* is because they are directly related to two main equations of AquaCrop, linked to the estimates of dry above-ground biomass and yield formation, respectively. The AquaCrop counts WP* reflecting directly on dry above-ground biomass production and on final grain yield. The canopy decline coefficient (CDC) presented considerable sensitivity only in Castro due to the longer duration of the phenological cycle. Fertility levels and saturated hydraulic conductivity (Ksat) in Castro was the least sensitive parameters in the analysis.

Brazilian Journal of Cardiovascular Surgery
- Journal Metrics
ABSTRACT Introduction: Postoperative bleeding is one of the main causes of complications in cardiovascular surgery, which highlights the importance of ensuring adequate intraoperative hemostasis, providing a better patient outcome. This study aimed to improve the prevention of postoperative bleeding in the Cardiovascular Surgery Department of the Hospital Estadual Mário Covas (Santo André, Brazil) using an adapted version of the Papworth Haemostasis Checklist to assess the impact of this standardization on bleeding rate, postoperative complications, reoperation, and mortality. Methods: This is a non-randomized controlled clinical trial, whose non-probabilistic sample consisted of patients undergoing cardiac surgery in the abovementioned service within a two-year interval. The Papworth Haemostasis Checklist was adapted to the Brazilian laboratory parameters and the questions were translated into Portuguese. This checklist was used before the surgeon started the chest wall closure. Patients were followed up until 30 days after surgery. A P-value < 0.05 was considered statistically relevant. Results: This study included 200 patients. After the checklist, a reduction in 24-hour drain output, postoperative complications, and reoperation was observed, although statistical significance was not reached. Finally, there was a significant reduction in the number of deaths (8 vs. 2; P=0.05). Conclusion: The use of the adapted checklist in our hospital proved to be an effective intervention to improve the prevention of postoperative bleeding, with a direct impact in the number of deaths in the study period. The reduction in deaths was possible thanks to the reduction in the bleeding rate, postoperative complications, and reoperations for bleeding.

Abstract The objective of this work was to evaluate the data estimated by NASA Power in relation to that measured at surface weather stations under different climates, and to verify the effects of these data on reference evapotranspiration (ETo) estimation. For comparison, data measured at 21 surface weather stations, located in Brazil, Israel, Australia, Portugal, and the United States of America were used, representing different Köppen climate types. The following climatic variables were analyzed daily: maximum (Tmax), mean (Tmean), and minimum (Tmin) air temperatures; wind speed; incident solar radiation; and mean relative humidity (RHmean). Wind speed showed the highest variations and was overestimated in the Cfb, BWh, BSh, and Cfa climates. Tmean and mean wind speed were estimated accurately in the Csa and BWh climates, whereas Tmax and Tmin were underestimated in 13 and 9 climates, respectively; Tmin did not show adequate results in tropical climates. Incident solar radiation was overestimated in all climates, except in BSh, but presented the best statistical indicators among the analyzed variables. The scenarios in which ETo was estimated using the Penman-Monteith method and data from NASA Power were consistent even for the climate type that presented the worst association between measured and estimated data.
As seguintes variáveis climáticas foram analisadas diariamente: temperaturas máxima (Tmáx), média (Tméd) e mínima (Tmín) do ar; velocidade do vento; radiação solar incidente; e umidade relativa média do ar (URméd).

ABSTRACT Agricultural systems can alter the soil physical and hydraulic properties, and consequently its quality. We aimed to determine and associate the physical and hydraulic properties of four soil types under no-tillage system in the Subtropical region of Brazil, as well as to create a data set available to be explored by further studies. Disturbed and undisturbed soil samples were collected from three layers: 0.0 - 0.10 m; 0.10 - 0.25 m; and 0.25 - 0.40 m at six localities in the Paraná and São Paulo States. Soil texture, soil bulk density and particle density, porosity (total, macro and micro), volumetric water content (permanent wilting point and field capacity), available water capacity and saturated hydraulic conductivity were determined. The average soil bulk density was within 1000 kg m(3 to 1400 kg m(3, which is considered optimum for the respective soil textures. The average total porosity (between 0.47 m3 m-3 and 0.63 m3 m-3) was close to an optimal condition for annual crop development under no-tillage system. The proportion between micro and macropores was approximately 2:1. The average values of soil available water capacity ranged from 48 mm to 60 mm. The average for the saturated hydraulic conductivity was classified as “moderate”. The management adopted for more than 20 years in the studied areas did not bring about critical values for the eleven physical and hydraulic soil properties examined, when compared to acceptable ranges verified in literature.

Abstract The objective of this study was to verify the magnitude and trend of hourly reference evapotranspiration (EToh), as well as associate and analyze daily ETo (ETod) series and the sum of hourly ETo (ETo24h) in 24 h, estimated with the Penman-Monteith ASCE model for Paraná State (Cfa and Cfb climate type). Relative humidity (RH), temperature (T), solar radiation (Rs) and wind speed (u2) data were obtained from 25 meteorological stations from the National Meteorological Institute (INMET), between December 1, 2016 to November 8, 2018. The analyzes were performed by linear regression and associations considering the root mean square error, correlation coefficient and index of agreement. The EToh trend has a Gaussian distribution, with the highest values between 12:00 p.m. and 2:00 p.m., with the maximum average being 0.44 mm h−1 (Cfa climate type) and 0.35 mm h−1 (Cfb climate type). The average difference between the ETo24h and ETod values was small (5.1% for Cfa and 7.4% for Cfb), resulting in close linear associations. The results obtained indicate that EToh has good potential to be used in planning and management in the field of soil and water engineering, in Paraná State.

Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

ABSTRACT: The objective of this study was to calibrate and validate models to estimate dry biomass of maize and soybean crops in the Campos Gerais region, located in the subtropical zone in southern Brazil, with incident solar radiation (Rs), growing degree days (GDD), days after planting (DAP) and precipitation (P) as variables. The experiments were carried out in the 2006/2007 and 2007/2008 harvests in Castro and Ponta Grossa cities, in four phenological stages, to obtain dry biomass. The climate data came from the ABC Foundation agrometeorological stations, installed close to the experimental areas. The parameters adjustment was performed by the Least Squares Method and using a spreadsheet Solver. The soybean harvests with the highest DAP, Rs, and P until physiological maturation were not the ones that obtained the highest final dry biomass productivity, except for the GDD variable. As sowing was performed later, the soybean crop cycle decreased. With the exception of the models' performance for the precipitation variable (between "Terrible" and "Excellent") for soybean and maize crops, the other variables presented promising performances between "Bad" and "Excellent", with a predominance of "Excellent".

ABSTRACT Hypovitaminosis D is now considered a pandemic, especially among more vulnerable populations and in HIV-infected subjects, with 80% presenting levels below 30 ng/mL. As there is no consensus on the more adequate dosage needed to correct such deficiency, the objective of this study was to evaluate 25 (OH) vitamin D supplementation in HIV-1 patients deficient of vitamin D. A total of 73 HIV-1-infected patients were included, drawn from a cohort of 435 patients; 37 patients were randomized to the active group, supplemented once a week with 50,000 UI vitamin D by mouth (group 1) and 36 to the placebo group (group 2). The study period ranged from June 2016 to September 2017. Variables involved in vitamin D metabolism and risk factors associated with hypovitaminosis were evaluated. The mean age was 45 years and 31.5 % were women. Vitamin D supplementation was effective in normalizing serum levels after six months in group 1 (mean 35 ng/mL compared to 21 ng/mL for the placebo group; p= 0.04). No patient reached blood levels considered toxic (>100 UI). Efavirenz use can negatively influence vitamin D levels and supplementation is necessary as a likely adjunct to improving CD4+ T cells, resulting in greater effectiveness of the treatment. A weekly oral dose of 50,000 IU of vitamin D was sufficient to normalize the vitamin deficiency, safely and with good adherence among persons living with HIV/AIDS in Brazil.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Abstract The values of the physical-water attributes of soils for use in agricultural simulation models are usually obtained using difficult and time-consuming methods. The objective of this study was to analyze the performance of the AquaCrop model to estimate soybean and maize crop productivity in the region of Campos Gerais (Brazil), with the option of including soil physical-water attributes in the model. Real crop productivities and input data (soil, climate, crop and soil management) were obtained from experimental stations of the ABC Foundation for the crop years 2006 to 2014. Sixty-four yield simulations were performed for soybean (four municipalities) and 42 for maize (three municipalities), evaluating input soil data scenarios of AquaCrop as follows: i) all soil physical-water attributes were measured (standard) and ii) the attributes were measured only using textural classification of the area (alternative). Real and simulated yields were verified by simple linear regression analyses and statistical indices (r, d, c). The standard scenario yielded performances between very good and excellent (0.75<c≤1.0) for soyb ean and b etween bad and excellent (0.40<c≤1.0) for maize. The alternative scenario was more variable, with performances between terrible and excellent (0.0<c≤1.0) for soybean and terrible and medium (0.0<c≤0.65) for maize. Using only the soil texture classification in AquaCrop indicated an easier way to estimate crop yields, but low performances may restrict estimates of soybean and maize yields in Campos Gerais.

SARS-Cov-2 is a virus easily transmitted by air and fomites causing acute severe respiratory syndrome. Severity in some cases requires hospitalization and complex expensive intensive care treatments. Its rampant contagious led to a a fearful pandemic affecting the whole world with millions of infected humans and almost half a million deaths in a few months in the beginning of 2020 (until June 17, 2020). SARS-Cov2 is likely to have been spilled over from natural sylvatic cycles in bats from China.
Our purpose is to comment on the human individual immune inflammatory responses to the infection of SARS-Cov2 and the reflections of these individual immunoinflammatory profiles on patterns of the severity of the disease, time for therapeutical intervention, pathogenesis, candidate drugs and indicative comparative drug prices for covid-19.
Efficient treatment for covid-19 may require: 1) early disease detection, 2) a combination of drugs being used for 3) targeting the virus replication cycle and 4) specific/ individualized drug treatment for given immunoinflammatory human profile responses in a 5) timely manner.
Specific serum immuno-markers of covid-19 affected individuals at onset, in the follow-up, and in the resolution of the immunoinflammatory storm during the course of the disease may lead to individualized therapeutics with better outcomes.
Covid-19 is unlikely to be the last emergent human disease with fast pandemic potencial. To gather knowledge on the human host profiles and immunoinflamatory responses is an opportunity that could pave the way to faster, more efficient strategies to tackle upcoming diseases.
