ABSTRACT BACKGROUND: Ankle taping (AT) is effective in preventing ankle sprain injuries in most common sports and is employed in rehabilitation and prevention sports. OBJECTIVE: This study aimed to investigate the effectiveness of AT to restricting excessive frontal plane ankle movements in semi-professional basketball players throughout the training session. DESIGN AND SETTING: A cross-sectional study was performed at the Universidad Europea de Madrid. METHODS: Forty male and female semi-professional basketball players were divided into two groups. The ankle dorsiflexion range of motion (ROM) and interlimb asymmetries in a weight-bearing lunge position were evaluated at four time points: 1) with no tape, 2) before practice, at 30 min of practice, and 3) immediately after practice. RESULTS: In male basketball players, no differences were observed in the right and left ankles between the baseline and 30 min and between baseline and 90 min of assessment. In female athletes, significant differences were reported between baseline and pre-training assessments for the right ankle and also significant differences between baseline and 90 min in both ankles. CONCLUSIONS: Ankle taping effectively decreased the ankle dorsiflexion ROM in male and female basketball players immediately after application. However, ROM restriction was very low after 30 and 90 min, as assessed in a single basketball practice. Therefore, the classic taping method should be revised to develop new prophylactic approaches, such as the implementation of semi-rigid bracing techniques or the addition of active stripes during training or game pauses.

ABSTRACT BACKGROUND: Current research supports the fact that prophylactic ankle taping (AT) is effective in preventing ankle injuries in amateur and elite sports athletes. OBJECTIVE: This study aimed to investigate the effect of AT on balance, knee valgus during drop jump and single-leg countermovement jump (SL-CMJ) landings, and ankle range of motion (ROM) restriction in healthy participants. DESIGN AND SETTING: A cross-sectional observational study was conducted at the Universidad Europea de Madrid, Madrid, Spain. METHODS: Participants: Thirty-nine healthy individuals participated in this study and performed the movements under two conditions (with and without tape). Outcome measurements: ankle ROM, balance, SL-CMJ height, flight time, ground time, and knee valgus. Before any intervention, a random process was developed with a 1:1 allocation ratio, and the participants were assigned to groups A (tape-no tape) and B (no tape-tape). RESULTS: Significant differences between tape and no-tape moments were observed for drop jump knee valgus flexion (P = 0.007), with an increase in knee valgus in participants with ankle taping. Similarly, the Y-balance testshowed a significant decrease in all variables (P = 0.001 and), ankle dorsiflexion (P = 0.001) in participants with ankle taping. CONCLUSIONS: AT is effective for immediate ankle ROM restriction. However, an increase in knee valgus during drop jump task and a decrease in lower limb balance were observed during drop jump task. Based on these results, it can be concluded that AT application in healthy individuals should not be recommended as it results in increase in injury risk factors.

ABSTRACT Introduction. The connection between news sources and the connotation of mental health articles in Paraguay during the COVID-19 pandemic (2020-2023) significantly impacted public opinion about mental health. Objective. This study analyzed news on mental health in Paraguay in the context of the COVID-19 pandemic. Material and methods. A descriptive retrospective study of all news articles on mental health published in the three primary newspapers of the country between May 2020 and May 2023 was conducted. The keywords were depressive disorders, anxiety disorders, bipolar disorder, schizophrenia, and mental health issues. Results. A total of 534 news were examined, 39.5% published by La Nación newspaper, 33.1% in 2023, and in 88.4% the author's identity and gender were not specified. Of the writing characteristics, 99.8% were written in the third person, 62.7% had a human-interest context, 48.5% had a neutral connotation, 65.7% focused on health and well-being, and 7.5% written by health experts. The year of publication was statistically associated to connotation (p=0.003) and context (p<0.001). Conclusion. The media has paid significant attention to mental health. Despite the objectivity provided by third-person narration, problems arise because of a lack of author identification. To encourage balanced and non-polarized reporting, it is crucial to incorporate expert voices and promote the framing of mental health stories as “human interest”.

RESUMEN Introducción. La conexión entre las fuentes de noticias y la connotación de los artículos sobre salud mental en Paraguay durante la pandemia de COVID-19 (2020-2023) impactó significativamente en la opinión pública sobre la salud mental. Objetivo. analizar las noticias sobre salud mental en Paraguay en el contexto de la pandemia COVID-19. Material y métodos. Se realizó un estudio descriptivo retrospectivo de todos los artículos periodísticos sobre salud mental publicados en los tres principales periódicos del país entre mayo de 2020 y mayo de 2023. Las palabras clave fueron trastornos depresivos, trastornos de ansiedad, trastorno bipolar, esquizofrenia y problemas de salud mental. Resultados. Se examinaron 534 artículos periodísticos, 39,5% publicados por el diario La Nación, 33,1% en 2023 y en 88,4% no se especificó la identidad ni el género del autor. De las características de redacción, el 99,8% estaban escritos en tercera persona, 62,7% pertenecían a un contexto de interés humano, 48,5% tenían una connotación neutra, 65,7% se centraban en la salud y el bienestar y el 7,5% escritos por expertos en salud. El año de publicación se asoció significativamente con la connotación (p=0,003) y con el contexto (p<0,001). Conclusión. Los medios de comunicación han prestado gran atención a la salud mental. A pesar de la objetividad que aporta la narración en tercera persona, surgen problemas por la falta de identificación del autor. Para fomentar una información equilibrada y no polarizada, es crucial incorporar voces expertas y promover el encuadre de las historias de salud mental como "interés humano".

Abstract Corn is the main crop of the basic basket in Mexico and one of the products of greatest commercial and economic importance nationwide. Among the main factors of agronomic management that determine its level of productivity is fertilization. Currently, as a result of the armed conflicts between Russia and Ukraine, there is a shortage and, consequently, a rise in the price of fertilizers. Therefore, this work studied the fertilization factor in the Papaloapan basin to analyze the effect of chemical nutrition on the corn crop based on its productivity in t ha-1 of grain. Three fertilization scenarios were simulated with the use of the SWAT model, and the results of the factors were mapped by statistically analyzing the difference in means of the treatments through an Anova, which yielded a value of P= 2e-16, showing a significant difference in the effects of the factors analyzed on the yield of the crop. In conclusion, the fertilization factor was higher than the non-fertilization factor, with a difference between means of 7.8 t ha-1 of grain, which indicates that the absence of fertilization work of the crop negatively impacts the productivity of corn grain. Nonetheless, the efficiency between half and full doses of nitrogen fertilization in terms of yield was found to be similar, with the half-dose factor being more efficient than the previous one.

Resumen El maíz es el principal cultivo de la canasta básica en México, así como uno de los productos de mayor importancia comercial y económica a nivel nacional. Dentro de los principales factores del manejo agronómico que determinan su nivel de productividad se encuentra la fertilización. Actualmente derivado de los conflictos bélicos entre Rusia y Ucrania, existe un desabasto y por ende una elevación del precio de los fertilizantes. Por lo anterior, en este trabajo se analizó el factor de fertilización en la cuenca del Papaloapan para analizar el efecto que ejerce la nutrición química en el cultivo de maíz en función de su productividad en t ha-1 de grano. Se simularon tres escenarios de fertilización con el uso del modelo SWAT y se mapearon los resultados de los factores analizando estadísticamente la diferencia de medias de los tratamientos por medio de una Anova la cual arrojó un valor de P= 2e-16 mostrando diferencia significativa en los efectos de los factores analizados sobre el rendimiento del cultivo. En conclusión, el factor de fertilización fue superior al de no fertilización con una diferencia entre medias de 7.8 t ha-1 de grano, lo cual indica que la ausencia de la labor de fertilización del cultivo impacta negativamente sobre la productividad de grano de maíz. Sin embargo, se encontró que la eficiencia entre media dosis y dosis completa de fertilización nitrogenada en términos de rendimiento es similar, siendo el factor de media dosis más eficiente que el anterior.

RESUMEN Introducción: La investigación sobre la autolesión no suicida permite aumentar la conciencia sobre este fenómeno, posibilita su detección temprana y la implementación de estrategias más eficaces en prevención y tratamiento. Esto conlleva a una reducción del sufrimiento individual, los costos económicos y el impacto en la sociedad en su totalidad. Objetivo: Determinar la frecuencia y factores asociados a autolesión no suicida en una muestra de población adulta de Paraguay. Metodología: Estudio descriptivo de asociación cruzada, de corte transversal y prospectivo. Una encuesta en línea fue difundida a través de redes sociales y aplicaciones de mensajería. Esta incluía preguntas sobre datos sociodemográficos y clínicos. La presencia de autolesiones se determinó a través de la versión en español de la escala Self-Harm Questionnaire. Se utilizó estadística descriptiva para todas las variables. Para buscar asociaciones se utilizó la prueba chi cuadrado. Resultados: Participaron 241 personas (media de edad=32±12 años, 74,7% mujeres). Se encontró una frecuencia de autolesiones de 24,5% (n=59). De estos, el 91,5% había tenido ideación suicida al menos una vez. Se encontró asociación entre el nivel de ingresos y la frecuencia de autolesiones (χ^2=16,9 gl=2 p<0,001). Los principales factores asociados a autolesiones fueron tener un diagnóstico previo de algún trastorno mental (OR=4,81) e historia de abuso físico o sexual (OR=3,00). Conclusión: A través de esta investigación se determinó que casi uno de cada cuatro adultos participantes se autolesionaba. La mayoría había tenido ideas suicidas al menos una vez. Además, los ingresos por debajo del salario mínimo y factores como tener un trastorno mental previo e historial de abuso físico o sexual se asociaron con las autolesiones.

ABSTRACT Introduction: Research on non-suicidal self-injury raises awareness of this phenomenon, enabling the early detection and implementation of more effective strategies in prevention and treatment. This leads to a reduction in individual suffering, economic costs, and impact on society. Objective: To determine the frequency and associated factors of non-suicidal self-injury in a sample of the adult population of Paraguay. Methodology: This was a descriptive, cross-sectional, and prospective study. An online survey was disseminated through social networks and messaging applications. The survey included questions on sociodemographic and clinical data. The presence of self-injury was determined by using the Spanish version of the Self-Harm Questionnaire. Descriptive statistics were used for all the variables. The chi-square test was used to examine associations. Results: There were 241 participants (mean age=32±12 years, 74.7%female). A non-suicidal self-injury frequency of 24.5% (n=59) was observed. Of these, 91.5% reported suicidal ideation at least once. An association was found between income level and the frequency of non-suicidal self-injury (χ^2=16,9 gl=2 p<0,001). The main factors associated with non-suicidal self-injury were a previous diagnosis of a mental disorder (OR=4.81) and a history of physical or sexual abuse (OR=3.00). Conclusion: Through this research, it was determined that almost one in four adult participants self-injured. Most of them had had suicidal thoughts at least once. In addition, income below the minimum wage and factors such as a previous mental disorder and a history of physical or sexual abuse were associated with non-suicidal self-injury.

ABSTRACT Cardiovascular diseases continue to be the leading cause of morbidity and mortality worldwide. Although there are therapeutic and pharmacological tools that help control the incidence of cardiovascular outcomes by reducing cardiovascular risk, inequities in timely access to healthcare services and social determinants of health prevent the entire population in need from receiving this therapeutic assistance. Thus, non-pharmacological interventions are prioritized, among which the diet stands out, as it has been shown to significantly influence cardiovascular health and life expectancy. Particularly, the Mediterranean diet, primarily based on the consumption of fruits, vegetables, grains, legumes, olive oil, spices, followed by fish and seafood-derived protein, has demonstrated a certain degree of protection against cardiometabolic diseases. However, the impact on a population with high cardiovascular risk and established cardiovascular disease remains uncertain. Considering the evolving evidence on dietary patterns and the need to understand how viable diets in our context affect individuals with a higher risk of morbidity and mortality from cardiovascular disease, the objective of this review is to present updated evidence on the benefits of the Mediterranean diet in individuals with high cardiovascular risk. For this purpose, a literature search was performed in the PubMed, Science Direct, and Scielo databases.

RESUMEN Las enfermedades cardiovasculares siguen siendo la principal causa de morbilidad y mortalidad en el mundo. Y aunque existen herramientas terapéuticas y farmacológicas que ayudan a controlar la incidencia de desenlaces cardiovasculares con la reducción del riesgo cardiovascular, las inequidades en el acceso oportuno a servicios de salud y de determinantes sociales de la salud impiden que toda la población con esta necesidad pueda recibir esta ayuda terapéutica. Es así, como se da prioridad a las intervenciones no farmacológicas, dentro de las que se destaca, la dieta, la cual ha demostrado influir significativamente en la salud cardiovascular y en la expectativa de vida del ser humano. Particularmente, la dieta mediterránea, basada esencialmente en el consumo de frutas, vegetales, granos, legumbres, aceite de oliva, especias, seguido de proteína derivada de pescado y comida de mar, ha demostrado proveer cierto grado de protección frente a enfermedades cardiometabólicas. Sin embargo, se desconoce con certeza cuál es el impacto en la población con elevado riesgo cardiovascular y, por ende, enfermedad cardiovascular establecida. Considerando la evolución de la evidencia en los patrones dietarios, así como la necesidad de conocer qué tanto impactan las dietas viables en nuestro medio, sobre todo en aquellos con mayor riesgo de morbilidad y mortalidad por enfermedad cardiovascular, el objetivo de esta revisión consiste en exponer evidencia actualizada sobre los beneficios de la dieta mediterránea en individuos con elevado riesgo cardiovascular. Para esto, se realizó una búsqueda bibliográfica en las bases de datos PubMed, Science Direct y Scielo.

Resumen Introducción: las clasificaciones de pie diabético (PD) son una herramienta que tienen el objetivo de mejorar la comunicación entre los profesionales, la referencia y contrarreferencia; proporcionar un pronóstico; ayudar en la valoración de las lesiones, y contribuir con fines estadísticos. Objetivos: describir las características de los pacientes que se presentaron al consultorio o a la guardia con un PD durante un período de 3 meses, determinar el riesgo según cinco clasificaciones (Texas, IDSA, San Elián, WIfI y SINBAD) y evaluar su evolución a 6 meses en relación con el grado de gravedad determinada por cada clasificación. Materiales y métodos: se analizaron 312 pacientes de 15 instituciones en Argentina. Para el análisis se utilizó la calculadora de clasificaciones de pie diabético/score de riesgo del Comité de Pie Diabético de la Sociedad Argentina de Diabetes. Resultados: el 43% de los pacientes (n=133) requirió internación al momento de la primera consulta y el 61% (n=189) había consultado previamente. El porcentaje de amputación mayor total fue de 8,33% (IC 95%; 5,5-11,9) (n=26) y el de amputación menor de 29,17% (IC 95%; 24,2-34,6) (n=91). A los 6 meses, el porcentaje de muerte fue de 4,49% (IC 95%; 2,5-7,4) (n=14), el 24,3% (IC 95%;19,6-29,5) presentaba la herida aún abierta (n=76), el 58,0% (IC 95%; 52,3-66,5) (n=181) cicatrizó y el 7,37% se perdió del seguimiento (n=23). Las clasificaciones de San Elián y WIfI se relacionaron con amputación mayor, cicatrización y muerte. En relación a la clasificación de Texas, el 49% de los pacientes presentó herida penetrante a hueso o articulación (Texas 3), con o sin infección. El 65,3% de las amputaciones mayores y el 78,6% de las muertes se produjeron en pacientes con isquemia. El punto de corte de San Elián para amputación mayor fue 20. Conclusiones: conocer los datos locales permite organizar los recursos para mejorar la atención de los pacientes.

Abstract Introduction: the classifications of diabetic foot (DF) are a tool that aims to improve communication between professionals, referral and counter-referral, provide a prognosis, help in the assessment of lesions, and contribute to statistical purposes. Objectives: to describe the characteristics of patients who presented to the clinic or emergency department with DF over a period of 3 months, determine the risk according to 5 classifications (Texas, IDSA, SEWSS, WIfI, and SINBAD), and evaluate their evolution at 6 months in relation to the severity degree determined by each classification. Materials and methods: 312 patients from 15 institutions in Argentina were analyzed. The Diabetic Foot Classification Calculator/Risk Score from the Diabetic Foot Committee of the Argentina Argentina Diabetes Society was used for the analysis. Results: 43% of patients (n=133) required hospitalization at the time of the first consultation and 61% (n=189) had previously consulted. The total major amputation percentage was 8.33% (95%CI; 5.5-11.9) (n=26), and the minor amputation percentage was 29.17% (95% CI; 24.2-34.6) (n=91). At 6 months, the death rate was 4.49% (95% CI; 2.5-7.4) (n=14), 24.3% (95% CI; 19.629.5) had an open wound (n=76), 58.0% (95% CI; 52.3-66.5) (n=181) had healed, and 7.37% were lost to follow-up (n=23). The SEWSS and WIfI classifications were related to major amputation, healing, and death. Regarding the Texas classification, 49% of patients had a penetrating wound to bone or joint (Texas 3), with or without infection. 65.3% of major amputations and 78.6% of deaths occurred in patients with ischemia. The SEWSS cut-off point for major amputation was 20. Conclusions: knowing local data allows organizing resources to improve patient care.

ABSTRACT Nutcracker syndrome caused by aorto-mesenteric compression of the left renal vein can be treated with surgical transposition. A review of this treatment was carried out at the Instituto de Previsión Social. Two women aged fifteen and twenty-six, who had low back pain and hematuria, were identified. They were discharged on the fourth and fifth postoperative day. The patients showed no symptoms after five and nine months of follow-up.

RESUMEN: El Síndrome de Nutcracker producido por compresión aorto-mesenterica de la vena renal izquierda puede tratarse con transposición quirúrgica. Se realizó revisión de dicho tratamiento realizado en el Instituto de Previsión Social. Se identificaron dos mujeres de 15 y 26 años, que presentaron dolor lumbar y hematuria. Egresaron al cuarto y quinto día de postoperatorio. Las pacientes no presentaron síntomas a los cinco y nueve meses de seguimiento.

Resumen En este artículo se presenta la aplicación de un enfoque híbrido que contempla un método de pronóstico y un método de toma de decisiones multicriterio para la selección de proveedores de materia prima en una PYME de tacos del Estado de México. El pronóstico se desarrolló mediante el software Minitab®18 a través un análisis de series de tiempo; evaluando el producto con mayor venta para un pronóstico de 15 días durante los tres principales meses donde la demanda es elevada, obteniendo como resultado el pronóstico de venta, el cual fue 54 tacos por día del tipo de taco más vendido de los cinco evaluados. Para la selección de proveedores se utilizó el método Electre I, se desarrollaron las matrices correspondientes para evaluar 13 proveedores de la PYME, se obtuvo como resultado que los proveedores 1, 3, 4, 5, 7, 9 y 12 sobresalen, correspondiente a un 53 %, descartando a los proveedores 2, 6, 8, 10, 11 y 13 correspondiente al 46 %, se logró seleccionar al mejor proveedor de acuerdo con los criterios evaluados.

Abstract This article presents the application of the multicriteria method and the forecast method in an SME in the State of Mexico for the selection of suppliers. It was developed using the Minitab®18 software, developing a time analysis, where the objective of increasing demand by 10 % was consolidated; evaluating the product with the highest sales in a period of 15 days during the three main months where the demand is high, obtaining as a result the sales forecast is 54 and 55 tacos per day. For the Electre I method, matrices were developed evaluating the 13 suppliers of the SME, the result was that supplier 1, 3, 4, 5, 7, 9 and 12 stand out, corresponding to 53 %, discarding supplier 2, 6 , 8, 10, 11 and 13 corresponding to 46 %, it was possible to increase the choice of suppliers with higher value.

Anthropometric, aerobic and muscle power profile of young Costa Rican basketball players. PENSAR EN MOVIMIENTO: Revista de Ciencias del Ejercicio y la Salud, 21(1), 1-17. The purpose of this study was to describe an anthropometric, aerobic and muscle power profile of young Costa Rican basketball players according to sex, play positions and age. The assessment was carried outa on 99 young players (43 women and 56 men; 18 guards, 51 forwards and 30 centers; 42 in the U16 category and 57 in the U18 category) from eight different teams. The sports experience of the players was 6.5 ± 3.1 years. The following parameters were assessed: height, weight, body mass index, skeletal muscle mass and body fat percentage, countermovement test (CMJ) and Yo-Yo Intermittent Recovery Test Level I. The main results showed significantly higher values in men than in women in terms of weight, height, skeletal muscle mass, distance covered in the yo-yo test, VO2max, height in CMJ and in flight time. Body fat percentage was higher in women than in men. Centers showed significantly higher height, weight and body fat percentage than other play positions. Guards and forwards covered more meters in the yo-yo test and showed higher VO2max than centers. No significant differences were found between the U16 and U18 categories in any variable. Regression models showed the influence of weight, body fat percentage, skeletal muscle mass and age on VO2max and CMJ. In conclusion, these indicators varied according to the sex and play position of the players. Additionally, this information will be useful for processes of promotion, development and training design of youth basketball in Costa Rica.

Este estudo teve como objetivo descrever um perfil antropométrico, aeróbico e de potência muscular de jogadores juvenis de basquete da Costa Rica segundo o sexo, as posições de jogo e a categoria de idade. Foram avaliados 99 jogadores juvenis (43 mulheres e 56 homens; 18 armadores, 51 alas e 30 pivôs; 42 na categoria U16 e 57 na categoria U18) de oito equipes diferentes. A experiência esportiva dos jogadores foi de 6,5 ± 3,1 anos. Foram realizadas as seguintes avaliações: altura, peso, índice de massa corporal, massa musculoesquelética e porcentagem de gordura, teste de salto com contramovimento (SCM) e teste de Yo-Yo de Recuperação Intermitente Nível I. Os principais resultados mostraram valores significativamente maiores em homens do que em mulheres com relação ao peso, tamanho, massa muscular esquelética, distância percorrida no teste Yo-Yo, VO2max, altura no SCM e no tempo de voo. A porcentagem de gordura foi maior em mulheres do que em homens. Os jogadores pivôs apresentaram significativamente maior estatura, peso corporal e porcentagem de gordura do que as outras posições de jogo. Os armadores e os alas cobriram mais metros no teste Yo-Yo e tiveram maior VO2max do que os pivôs. Entre as categorias U16 e U18 não foram encontradas diferenças significativas em nenhuma variável. Modelos de regressão mostraram a influência do peso, da porcentagem de gordura corporal, da massa musculoesquelética e da idade no VO2max e SCM. Conclui-se, portanto, que esses indicadores variam segundo o sexo e as posições dos jogadores. Além disso, essa informação será útil para processos de fomentação, desenvolvimento e desenho de treinamentos de basquete juvenil na Costa Rica.

El objetivo de este estudio fue describir un perfil antropométrico, aeróbico y de potencia muscular de basquetbolistas juveniles costarricenses según sexo, posiciones de juego y categoría de edad. Se evaluaron 99 jugadores juveniles (43 mujeres y 56 hombres; 18 bases, 51 aleros y 30 pívots; 42 categoría U16 y 57 categoría U18) de ocho equipos diferentes. La experiencia deportiva de los jugadores fue de 6.5 ± 3.1 años. Se realizaron las siguientes evaluaciones: altura, peso, índice de masa corporal, masa músculo-esquelética, porcentaje de grasa, prueba de contramovimiento (CMJ) y prueba de Yo-Yo de Recuperación Intermitente Nivel I. Los principales resultados mostraron valores significativamente mayores en hombres que en mujeres en peso, talla, masa músculo-esquelética, distancia recorrida en la prueba Yo-Yo, en el VO2max, altura en el CMJ y en el tiempo de vuelo. El porcentaje de grasa fue mayor en mujeres que en hombres. Los jugadores pívots presentaron significativamente mayor estatura, peso corporal y porcentaje de grasa que las otras posiciones de juego. Los bases y los aleros cubrieron más metros en la prueba Yo-Yo y tuvieron mayor VO2máx que los pívots. Entre las categorías U16 y U18 no se encontraron diferencias significativas en ninguna variable. Modelos de regresión mostraron la influencia del peso, porcentaje de grasa corporal, masa músculo esquelética y la edad sobre el VO2máx y CMJ. Como conclusión, según el sexo y las posiciones de juego de los jugadores, estos indicadores variaron. Además, esta información será útil para procesos de promoción, desarrollo y diseño de entrenamientos de baloncesto juvenil en Costa Rica.

Resumen Los sistemas agroforestales y la diversificación productiva son alternativas sostenibles que revierten los daños y riesgos de la agricultura convencional. Existen pocos estudios de sostenibilidad que permitan incorporarlos a proyectos destinados a aumentar la rentabilidad y mejorar el bienestar de los productores, restaurando los servicios ecosistémicos. El objetivo fue evaluar la sustentabilidad de dos sistemas agroforestales tradicionales, diversificados con cacao y vainilla, en los estados de Veracruz y Oaxaca. Se promovieron técnicas agroecológicas y estrategias de desarrollo empresarial con las familias de ambos productores. La investigación se llevó a cabo durante el periodo: febrero 2012 a julio 2020, las parcelas están ubicadas en los municipios de San Pedro Ixcatlán, Oaxaca y Papantla, Veracruz. Se utilizó la metodología del marco de evaluación de sistemas de gestión de recursos naturales. Los resultados mostraron que los estados iniciales tenían un manejo poco intensivo, bajo uso de insumos, alta diversificación y orientado a la agricultura de subsistencia. Tras la diversificación, manejo agroecológico y desarrollo empresarial, los estados de transición han mejorado en la mayoría de los indicadores de sostenibilidad. En particular la rentabilidad (índice beneficio-costo), que alcanzó 1.2 para San Pedro y 2.6 para Papantla cuando se da valor agregado a la vainilla y el cacao. Es relevante la participación de las mujeres en actividades emergentes derivadas de la diversificación y el interés de los jóvenes por continuar con actividades de transformación de materias primas y la comercialización de productos de valor agregado.

Abstract Agroforestry systems and productive diversification are sustainable alternatives that reverse the damages and risks of conventional agriculture. There are few sustainability studies that allow them to be incorporated into projects aimed at increasing profitability and improving the welfare of producers, restoring ecosystem services. The objective was to assess the sustainability of two traditional agroforestry systems, diversified with cacao and vanilla, in the states of Veracruz and Oaxaca. Agroecological techniques and business development strategies were promoted with the families of both producers. The research was carried out during the period: February 2012 to July 2020, the plots are located in the municipalities of San Pedro Ixcatlán, Oaxaca and Papantla, Veracruz. The methodology of the framework of assessment of natural resource management systems was used. The results showed that the initial states had a low intensive management, low use of inputs, high diversification and oriented to subsistence agriculture. After diversification, agroecological management and business development, transition states have improved in most sustainability indicators. In particular, profitability (benefit-cost index), which reached 1.2 for San Pedro and 2.6 for Papantla when added value is given to vanilla and cacao. The participation of women in emerging activities derived from diversification and the interest of young people to continue with activities of transformation of raw materials and the commercialization of value-added products are relevant.

ABSTRACT Background: Observational studies have described a high prevalence of depression and anxiety in rheumatoid arthritis: major depressive disorders are detected in 17 % of patients with the pathology, and local and systemic inflammation play an important role in anxiety and depression. Objectives: The overall objective of this research was to determine the frequency of anxiety, depression, and vulnerability to stress in patients diagnosed with rheumatoid arthritis. Materials and methods: This was an observational, descriptive, temporally prospective, cross-association study. Sampling was non-probabilistic of consecutive cases. We included adults with a diagnosis of rheumatoid arthritis who consulted at the Rheumatology Department of the Hospital de Clínicas between August and October 2022. The following instruments were used for psychiatric diagnosis: Generalized Anxiety Scale (GAD-7), Patient Health Questionnaire (PHQ-2) and Smith and Miller's Stress Vulnerability Scale. Results: Thirty-six patients, all women, aged between 20 and 77 years, were included in the study. The 27.8 % had depression, according to the cut-off points of the PHQ-2. Anxiety was present in 22.2 %, according to the GAD-7 cut-off points. Regarding stress, 22.2 % were vulnerable to stress and 5.6 % were seriously vulnerable. Conclusion: Depression, anxiety and vulnerability to stress are frequently observed comorbidities in patients with rheumatoid arthritis. Specific mental health interventions are required to address these issues and improve the quality of life of affected patients.

RESUMEN Antecedentes: Estudios observacionales han descrito una alta prevalencia de depresión y ansiedad en la artritis reumatoidea: los trastornos depresivos mayores se detectan en el 17 % de los pacientes con la patología, y la inflamación local y sistémica desempeña un papel importante en la ansiedad y la depresión. Objetivos: El objetivo general de esta investigación fue determinar la frecuencia de ansiedad, depresión y vulnerabilidad al estrés en pacientes con diagnóstico de artritis reumatoidea. Materiales y métodos: Este fue un estudio observacional, descriptivo de asociación cruzada y temporalmente prospectivo. El muestreo fue no probabilístico de casos consecutivos. Se incluyó a personas adultas con diagnóstico de artritis reumatoide que consultaron en el Departamento de Reumatología del Hospital de Clínicas, entre agosto y octubre del 2022. Para el diagnóstico psiquiátrico se utilizaron los siguientes instrumentos: Escala de Ansiedad Generalizada (GAD-7), Patient Health Questionnaire (PHQ-2) y Escala de Vulnerabilidad al Estrés de Smith y Miller. Resultados: Se incluyó a 36 pacientes, todas mujeres, con edades comprendidas entre los 20 y 77 años. El 27,8 % tenía depresión, según los puntos de corte del PHQ-2. El 22,2 % presentaba ansiedad, según los puntos de corte de GAD-7. En cuanto al estrés, el 22,2 % tenía vulnerabilidad a este y el 5,6 % era seriamente vulnerable. Conclusión: Depresión, ansiedad y vulnerabilidad al estrés son comorbilidades frecuentemente observadas en pacientes con artritis reumatoidea. Se requieren de intervenciones específicas de salud mental para abordar estas cuestiones y mejorar la calidad de vida de los pacientes afectados.

ABSTRACT The Interpersonal Theory of Suicide proposes that the presence of suicidal behavior is explained by the suicidal desire and capacity acquired in the individual. Fearlessness about death is a dimension of said capacity and is a potential risk factor for suicide. The objective was to realize Spanish translation and validation of the Acquired Capability for Suicide Scale Fearlessness About Death in Peruvian adults from the general population. The methodology of the study was cross-sectional study of quantitative and instrumental approach. A total of 929 Peruvian adults who responded the Acquired Capability for Suicide Scale - Fearlessness About Death (ACSS-FAD) participated. The reverse translation method was used for the English to Spanish version. Analyses were performed as from classical test theory and item response theory. The results indicate that the ACSS-FAD evidences a unidimensional structure (CFI = .99, RMSEA = .07 [90 % CI: .06 - .09], SRMR = .03], adequate reliability (α = .84 and H = .86), invariance and differential item functioning according to sex. In addition, all items discriminated adequately, with items 5 and 4 being more accurate in measuring fear lessness about death. In conclusion, the Spanish version of the ACSS-FAD presents sources of evidence of validity based on content, internal structure, reliability, invariance and item responses. Its use is recommended for the interpretation and inference of its scores in the assessment of stigma towards the suicide in the general adult Peruvian population.

RESUMEN La teoría interpersonal del suicidio propone que la presencia de una conducta suicida es explicada por el deseo de suicidio y capacidad adquirida en el individuo. La intrepidez ante la muerte es una dimensión de dicha capacidad y es un factor de riesgo potencial del suicidio. El objetivo fue realizar la traducción al español y validación del Acquired Capability for Suicide Scale Fearlessness About Death en adultos peruanos de población general. La metodología del estudio fue transversal con un enfoque cuantitativo e instrumental. Participaron 929 adultos que respondieron la Escala de Capacidad Adquirida para el Suicidio - Intrepidez ante la Muerte (ACSS-FAD). El método de traducción inversa se utilizó para la versión de inglés al español. Los análisis se realizaron a partir de la teoría clásica de los test y teoría de respuesta al ítem. Los resultados indican que la ACSS-FAD evidencia una estructura unidimensional (CFI = .99, RMSEA = .07 [IC 90 %: .06 - .09], SRMR = .03], adecuada confiabilidad (α = .84 y H = .86), invarianza y funcionamiento diferencial del ítem según sexo. Además, todos los ítems discriminaron adecuadamente, y los ítems 5 y 4 fueron los más precisos para medir la intrepidez ante la muerte. En conclusión, la versión en español del ACSS-FAD presenta fuentes de evidencias de validez basadas en el contenido, estructura interna, confiabilidad, invarianza y respuestas del ítem. Se recomienda su uso para la interpretación e inferencia de sus puntuaciones en la evaluación de la intrepidez ante la muerte en población general adulta peruana.

Resumen: Introducción: El intestino delgado es el sitio de asiento más frecuente del melanoma metastásico. Su diagnóstico es un desafío por cursar asintomático o con síntomas inespecíficos. Son pocos los casos que presentan complicaciones, siendo infrecuente la peritonitis por perforación. El objetivo del trabajo es comunicar el caso clínico de una peritonitis por perforación de una metástasis de melanoma en intestino delgado. Caso clínico: Paciente de sexo masculino de 66 años con diagnóstico de melanoma de cuello y secundario óseo, encefálico y pulmonar, fue intervenido de urgencia por peritonitis aguda por perforación de metástasis en intestino delgado. El estudio histológico confirmó secundarismo de melanoma cutáneo. Conclusión: Sabiendo que el yeyuno íleon es el sitio de asiento más frecuente de las metástasis de melanoma, ante la presencia de síntomas digestivos inespecíficos o anemia se debe sospechar su compromiso y evaluar posibles alternativas terapéuticas.

Abstract: Introduction: The small intestine is the most frequent site of metastatic melanoma. However, its diagnosis continues to be a challenge since it is usually asymptomatic or with non-specific symptoms. Few cases result in complications, peritonitis due to perforation being infrequent. The objective of the work is to report a clinical case of peritonitis due to perforation of a melanoma metastasis in the small intestine. Clinical case: A 66-year-old male patient diagnosed with melanoma of the neck and secondary bone, brain and lung melanoma, underwent emergency surgery for acute peritonitis due to perforation of metastasis in the small intestine, which was resected and anastomosed. The histology confirmed the secondary nature of the cutaneous melanoma. Conclusion: Knowing that the jejunum-ileum is the most frequent site of melanoma metastases, in the presence of non-specific digestive symptoms or anemia, its involvement should be suspected and possible therapeutic alternatives should be evaluated.

Resumo: Introdução: O intestino delgado é o local mais frequente de melanoma metastático. O diagnóstico é um desafio por ser assintomático ou apresentar sintomas inespecíficos. Há poucos casos que apresentam complicações, sendo pouco frequente a peritonite por perfuração. O objetivo deste trabalho é relatar um caso clínico de peritonite por perfuração de metástase de melanoma no intestino delgado. Caso clínico: Paciente do sexo masculino, 66 anos, diagnosticado com melanoma no pescoço com metástase óssea, cefálica e pulmonar. Foi submetido a cirurgia de emergência por peritonite aguda por perfuração de metástases do intestino delgado. O estudo histológico confirmou melanoma cutâneo. Conclusão: Sabendo que o jejuno e o íleo é o local mais frequente de metástase de melanoma, na presença de sintomas digestivos inespecíficos ou anemia deve-se suspeitar de seu acometimento e avaliar possíveis alternativas terapêuticas.
