ABSTRACT For several decades, the issue of coverage and access to health services has been frequently debated. The objective of this communication is to contribute to the debate based on the concrete experiences of the Cuban National Health System in the last six decades. A bibliographic and official document review of the Cuban National Health System was carried out. A brief synthesis of the Cuban experience is presented. It can be said that Cuba reached universal coverage around 2010-2015 with the model of the family doctor and nurse.

RESUMEN Desde hace varias décadas se debate con frecuencia el tema de la cobertura y acceso a los servicios de salud. El objetivo de la presente comunicación es contribuir al debate partiendo de las experiencias concretas del Sistema Nacional de Salud de Cuba en las últimas seis décadas. Se realizó una revisión bibliográfica y de documentos oficiales del Sistema Nacional de Salud cubano. Se presenta una síntesis breve de la experiencia cubana. Se puede decir que Cuba alcanzó la cobertura universal alrededor de 2010-2015 con el modelo del médico y enfermera de la familia.

Resumen Introducción El análisis de la actividad eléctrica cerebral mediante electrodos ubicados sobre el cuero cabelludo con electroencefalografía (EEG) podría permitir conocer la profundidad anestésica de un paciente durante cirugía. Sin embargo, los equipos de EEG convencionales, por su precio y tamaño, no son una alternativa práctica en quirófanos y los equipos comerciales usados en cirugía no permiten acceder a la actividad eléctrica. Disponer de tecnologías portables y de bajo costo aumentaría el número de investigaciones sobre la actividad cerebral bajo anestesia general y facilitaría la búsqueda de nuevos marcadores para la profundidad anestésica. Objetivo Evaluar la capacidad de una tecnología EEG portable de adquirir ritmos cerebrales relacionados con el estado consciente y el estado de anestesia general de pacientes en cirugía anestesiados con propofol. Métodos Estudio observacional de corte transversal en el que se analizaron datos de 10 registros EEG obtenidos mediante tecnología portable y de bajo costo OpenBCI, de pacientes de sexo femenino que fueron sometidas a anestesia general con propofol. La señal obtenida de los electrodos frontales se analizó mediante análisis espectral y se contrastaron los resultados con lo descrito en la literatura. Resultados La señal obtenida con electrodos frontales, especialmente el ritmo α, permitió diferenciar el reposo con ojos cerrados y ojos abiertos en estado consciente, del estado de mantenimiento de la anestesia durante cirugía. Conclusiones Se logra la diferenciación de estado de reposo y de mantenimiento de la anestesia replicando hallazgos previos de tecnologías convencionales. Estos resultados abren la posibilidad de utilizar las tecnologías portables como el OpenBCI para investigar la dinámica cerebral durante la anestesia.

Abstract Introduction The analysis of the electrical activity of the brain using scalp electrodes with electroencephalography (EEG) could reveal the depth of anesthesia of a patient during surgery. However, conventional EEG equipment, due to its price and size, are not a practical option for the operating room and the commercial units used in surgery do not provide access to the electrical activity. The availability of low-cost portable technologies could provide for further research on the brain activity under general anesthesia and facilitate our quest for new markers of depth of anesthesia. Objective To assess the capabilities of a portable EEG technology to capture brain rhythms associated with the state of consciousness and the general anesthesia status of surgical patients anesthetized with propofol. Methods Observational, cross-sectional study that reviewed 10 EEG recordings captured using OpenBCI portable low-cost technology, in female patients undergoing general anesthesia with propofol. The signal from the frontal electrodes was analyzed with spectral analysis and the results were compared against the reports in the literature. Results The signal captured with frontal electrodes, particularly α rhythm, enabled the distinction between resting with eyes closed and with eyes opened in a conscious state, and sustained anesthesia during surgery. Conclusions It is possible to differentiate a resting state from sustained anesthesia, replicating previous findings with conventional technologies. These results pave the way to the use of portable technologies such as the OpenBCI tool, to explore the brain dynamics during anesthesia.

ABSTRACT Cuba’s maternity homes were founded in 1962 as part of the general movement to extend health services to the whole population in the context of the post-1959 social transformations. The over-arching goal of the homes was to improve the health of pregnant women, mothers and newborns. Hence, in the beginning when there were few hospitals in Cuba’s rural areas, their initial purpose was to increase institutional births by providing pregnant women a homelike environment closer to hospitals. There, they lived during the final weeks before delivery, where they received medical care, room and board free of charge. Over time, and with expanded access to community and hospital health facilities across Cuba, the numbers, activities, modalities and criteria for admission also changed. In particular, in addition to geographical considerations, expectant mothers with defined risk factors were prioritized. For example, during the 1990s economic crisis, the maternity homes’ role in healthy nutrition became paramount. The purpose of this essay is to provide a historical perspective of this process, describe the changes and results during the 55 years examined, and take a critical look at the challenges to successful implementation of this model, a mainstay at the primary healthcare level of the public health system’s Maternal–Child Health Program.

The research was conducted in the Francisco Vivar Castro University Park (PUFVC), owned by the National University of Loja, located in the canton and province of Loja; with the objective of valuing their environmental goods and services ecologically and economically. For ecological assessment, we considered: flow, leaf litter thickness, floristic and faunal diversity, physical and chemical properties of the soil, ecological functions assigning an excellent, good and bad rating considering the current status and attributes of each function. For the total economic assessment, the value of direct use was considered, using the market price of wood, seeds, ornamental and medicinal plants; and the values of indirect use, option and existence, through the contingent valuation by applying 100 surveys to visitors on weekends. The ecological assessment indicates that flow for León Huayco is 5.14 l/s and Los Nogales 2.16 l/s; the leaf litter thickness is 9.1 cm; the floristic diversity with 137 species; 80 species of birds, 9 species of mammals and 5 of reptiles; the soil is clayey loam, extremely acid pH, organic matter varies from medium to high, nitrogen is medium, phosphorus and potassium low. Eight ecological functions were identified. The total economic valuation is $ 641 259.50; and the contributions are: direct value $ 626 359.5, indirect value $ 700, option value $ 1720 and existence value $ 12 480. The ecological-economic valuation allowed to have an integral vision on the value and importance of the natural resources and generated arguments that support the management of the Francisco Vivar Castro University Park

La investigación se realizó en el Parque Universitario Francisco Vivar Castro (PUFVC), propiedad de la Universidad Nacional de Loja, ubicado en el cantón y provincia de Loja; con el objetivo de valorar ecológica y económicamente sus bienes y servicios ambientales. Para la valoración ecológica, se consideraron: caudales, espesor de hojarasca, diversidad florística y faunística, propiedades físicas y químicas del suelo, funciones ecológicas asignando una calificación de excelente, bueno y malo considerando el estado actual y atributos de cada función. Para la valoración económica total, se consideró el valor de uso directo, usando el precio en el mercado de madera, semillas, plantas ornamentales y medicinales; y los valores de uso indirecto, opción y existencia, mediante la valoración contingente aplicando 100 encuestas a visitantes los fines de semana. La valoración ecológica indica que el caudal para León Huayco es 5,14 l/s y Los Nogales 2,16 l/s; el espesor de hojarasca de 9,1 cm; la diversidad florística de 137 especies; 80 especies de aves, 9 especies de mamíferos y 5 de reptiles; el suelo es franco arcilloso, pH extremadamente ácido, materia orgánica varía de media a alta, nitrógeno es mediano, fósforo y potasio bajos. Se identificaron ocho funciones ecológicas. La valoración económica total es de $ 641 259,50; y los aportes son: valor directo $626 359,5, valor indirecto $ 700, valor de opción $ 1720 y valor de existencia $ 12 480. La valoración ecológica-económica permitió tener una visión integral sobre el valor e importancia de los recursos naturales y generó argumentos que sustentan el manejo del Parque Universitario Francisco Vivar Castro

ABSTRACT The text refers to the contents of Geography as a science, its areas and the existence in the planet of the ´´human animal´´. It analyzes how this human animal (man), although its intelligence carry out non smart things as wars, terrorism, addictions with their consequent predatory and pollutant actions. These last have been studied by Geography as a science developing specialties as Human Geography and Health’s Geography. It is remarkable the existence and validity of Health’s Geography that deals with the changes in fertility patterns, migrations, pathogenic urbanism, unhealthy neighborhoods, lack of drinkable water, food and road insecurity, among others that represent risks and aggressions to health and that the specialist in this field analyzes as potential outbreaks of obesity, stress, violence and inequality. In the training of health workers should be included the study of this problems from the wide view that Health’s Geography offers.

RESUMEN El texto alude al contenido de la geografía como ciencia, sus espacios y de la existencia en la superficie del planeta del animal humano. Analiza como este animal humano, el hombre, a pesar de su inteligencia realiza prácticas no inteligentes entre ellas, guerras, terrorismo, adicciones y su acción depredadora y contaminante. Estas últimas han sido objeto de estudio por la geografía, que ha avanzado hasta desarrollar la geografía humana y la geografía de la salud. Destaca la existencia y vigencia de esta última, que se ocupa, entre otros, de los cambios en los patrones de fecundidad, migraciones, urbanismo patogénico, barrios insalubres, carencia de agua potable, inseguridad alimentaria y vial, todos riesgos y agresiones a la salud que el biologicista ve convertirse en epidemias de obesidad, estrés, violencia y desigualdades. En la preparación de los salubristas, debe incluirse el estudio de estos problemas desde la visión más amplia que proporciona la geografía de la salud.
