Abstract The study focuses on the use and non-use of bibliographic managers by members of the university community that participated in the training carried out by the Central Library of the National University of San Marcos, which have been developed throughout the 2021. Likewise, it tries to identify the differences in use by type of academic profile, area of study, age and scientific production. An online questionnaire was sent and a set of 953 responses was collected and analyzed. 66.5 % identified themselves as non-users and only 33.5% as users. Among the latter, 27.6% are undergraduate students, as well as Mendeley (49.5%) and Zotero (26.0 %) turned out to be the most used managers. The lack of knowledge of these tools and the difficult handling were the main causes of not using bibliographic managers. Regarding the characteristics, the analysis revealed that a relationship prevails in the use of bibliographic managers with the aspects of age (young users) and scientific production (greater number of publications), however, aspect such as the area of study and profile academic does not represent a conclusive relation in the use of these tools.

Resumen El estudio se enfoca en el uso y preferencias de los gestores bibliográficos por parte de participantes en las capacitaciones de ALFIN realizadas por la Biblioteca Central de la Universidad Nacional Mayor de San Marcos, con el propósito de mejorar los programas de soporte de la biblioteca. Se envió un cuestionario en línea, se recopiló y analizó un conjunto de 953 respuestas. Los resultados revelan que sólo el 33.5 % de los participantes utilizaba los gestores bibliográficos. De ellos, el 27.6 % eran estudiantes de pregrado y el 24.1 % de posgrado, y se demostró que Mendeley (49.5 %) y Zotero (26.0 %) son los gestores más utilizados. Así también, por parte de los encuestados, el nivel de manejo es básico y de dominio regular. El 66.5 % de ellos no usa ningún gestor bibliográfico, siendo las principales causas el desconocimiento de estas herramientas y su difícil manejo. Se concluye que hay deficiencias en el uso de gestores bibliográficos, a pesar de ser una herramienta de apoyo en las competencias informacionales para el desarrollo de los trabajos de investigación.

Abstract Objectives The present study aimed to evaluate the measurement invariance of a general measure of the perception of governmental responses to COVID-|19 (COVID-SCORE-10) in the general population of 13 Latin American countries. Methods A total of 5780 individuals from 13 Latin American and Caribbean countries selected by non-probabilistic snowball sampling participated. A confirmatory factor analysis was performed and the alignment method was used to evaluate invariance. Additionally, a graded response model was used for the assessment of item characteristics. Results The results indicate that there is approximate measurement invariance of the COVID-SCORE-10 among the participating countries. Furthermore, IRT results suggest that the COVID-SCORE-10 measures with good psychometric ability a broad spectrum of the construct assessed, especially around average levels. Comparison of COVID-SCORE-10 scores indicated that participants from Cuba, Uruguay and El Salvador had the most positive perceptions of government actions to address the pandemic. Thus, the underlying construct of perception of government actions was equivalent in all countries. Conclusion The results show the importance of initially establishing the fundamental measurement properties and MI before inferring the cross-cultural universality of the construct to be measured.

Abstract: This research study aims to conduct electromagnetic and thermal simulations of a portable shelter prototype against lightning strikes to determine risk factor for the user, based on the regulation IEC 60479-2. Colombia is one of the countries in the world with the highest rates of lightning strikes and victims of this phenomenon, especially in the Colombian National Army. Electromagnetic and thermal simulations of the prototype are performed by using the simulation software CST Microwave Studio®. The results show the induced step voltages and the energy levels achieved by the user, depending on the soil’s resistivity and the prototype’s distance from the impact site of a lightning strike. It is concluded that the shelter prototype is not designed to withstand a direct lightning strike; instead, it can only mitigate the effects of step voltages of an indirect lightning strike.

Resumen: Esta investigación tiene como objetivo realizar simulaciones electromagnéticas y térmicas de un prototipo de refugio portátil contra rayos para determinar los factores de riesgo para el usuario, con base en la norma IEC 60479-2. Colombia es uno de los países en el mundo con los mayores índices de ocurrencia de impactos de rayos y de víctimas por este fenómeno, especialmente en sus fuerzas armadas. Se realizan simulaciones electromagnéticas y térmicas del prototipo en el software de simulación CST Microwave Studio®. Los resultados muestran las tensiones de paso inducidas y los niveles de energía alcanzados por el usuario, en función de la resistividad del suelo y la distancia de ubicación del prototipo con respecto al sitio de impacto del rayo. Se concluye que el prototipo de refugio no fue diseñado para soportar un impacto de rayo de tipo directo; sólo mitiga los efectos por tensiones de paso ante un impacto de rayo de tipo indirecto.

Abstract Objectives: in Colombia, sedation by non-anesthesiologists for endoscopic procedures outside the operating room has been implemented. A description of an experience in the gastroenterology unit of a tertiary referral hospital in Cali, Colombia, was conducted. Materials and methods: an analytical cohort observational study to describe the frequency and type of adverse events associated with sedation procedures performed by general practitioners and evaluate the factors related to their occurrence in patients who attended the endoscopy unit of Fundación Valle del Lili for endoscopic studies under intravenous sedation. Between November 2018 and June 2019, non-anesthesiologist physicians performed this procedure due to the minimal risk implied. A descriptive analysis was completed, and the median and interquartile range were calculated for numerical variables and frequencies for qualitative variables. Results: There were 1506 participants, 59.4% ASA I and 40.6% ASA II in this study. On average, the starting dose of propofol was 60 mg, and the total dose was 140 mg. Forty-six patients (3.05%) reported non-severe adverse events; the most common occurrence was transient desaturation (80.4%). No patients experienced severe adverse events. The average initial Aldrete scale score was 8, while at discharge, the average score was 10. Conclusions: sedation for endoscopic procedures performed by non-anesthesiologists is safe provided that it is performed by trained personnel conducting a correct assessment of the patient’s (cardiovascular, gastrointestinal, and neurological) history and risk factors within the framework of the current institutional guidelines.

Resumen Objetivos: en Colombia se ha venido implementando la sedación por médicos no anestesiólogos para procedimientos endoscópicos fuera del quirófano. Se describió la experiencia en la unidad de gastroenterología de una clínica de alto nivel de atención en Cali, Colombia. Materiales y métodos: estudio observacional, de tipo cohorte analítica para describir la frecuencia y el tipo de eventos adversos asociados a los procedimientos de sedación por médicos generales, y evaluar los factores asociados a su ocurrencia en pacientes que acudieron a la unidad de endoscopia de la Fundación Valle del Lili para la realización de estudios endoscópicos bajo sedación intravenosa que, por ser de bajo riesgo, fue aplicada por un médico no anestesiólogo entre noviembre de 2018 y junio de 2019. Se realizó análisis descriptivo, se calcularon mediana y rango intercuartílico para las variables numéricas, y frecuencias para las variables cualitativas. Resultados: se incluyeron 1506 participantes, 59,4 % ASA I y 40,6 % ASA II. En promedio, la dosis inicial de propofol fue de 60 mg y la dosis total, de 140 mg. Se registraron eventos adversos no serios en 46 pacientes (3,05 %) y el más común fue la desaturación transitoria (80,4 %). Ningún paciente presentó eventos adversos serios. El puntaje inicial promedio de la escala de Aldrete fue 8, mientras que al alta el puntaje promedio fue de 10. Conclusiones: la sedación para procedimientos endoscópicos dada por médicos no anestesiólogos es segura, siempre y cuando sea realizado por personal entrenado que realice una adecuada valoración de los antecedentes (cardiovasculares, gastrointestinales y neurológicos) y factores de riesgo del paciente dentro del marco de los lineamientos institucionales vigentes.

Resumen Introducción. El virus de la viruela del mono (MPXV) está compuesto por un genoma de ADN bicatenario, aproximadamente, de 197.209 pb. La clasificación actual agrupa el MPXV en tres clados: clado I (de la cuenca del Congo en África central), y clados IIa y IIb (de África occidental). Objetivo. Reportar el genoma completo y el análisis filogenético de un caso humano de viruela símica detectado en Colombia. Materiales y métodos. Se obtuvo exudado de lesiones vesiculares de un paciente varón con el antecedente de un viaje reciente a España. Se implementó un enfoque directo, en el cual se purificó el ADN total de la muestra mediante un método basado en columnas, seguido de la secuenciación directa en la plataforma Nanopore GridION. Las lecturas se alinearon con el genoma de referencia del MPXV, utilizando minimap2, v.2.24, y la inferencia filogenética fue realizada mediante la estimación por máxima verosimilitud. Resultados. Un total de 11.951 lecturas se alinearon directamente con el genoma de referencia con una cobertura del 96,8 % (190.898 pb). Conclusión. El análisis filogenético del MPXV circulante en Colombia demostró su estrecha relación con el clado de África occidental (clado IIb) responsable del brote en múltiples países en el 2022.

Abstract Introduction: Monkeypox virus (MPXV) is an enveloped double-stranded DNA virus with a genome of approximately 197.209 bp. The current classification divides MPXV into three clades: Clade I (Central African or Congo Basin clade) and clades IIa and IIb (West African clades). Objective: To report the complete genome and phylogenetic analysis of a human monkeypox case detected in Colombia. Materials and methods: Exudate from vesicular lesions was obtained from a male patient with recent travel history to Spain. A direct genomic approach was implemented in which total DNA from the sample was purified through a column-based method, followed by sequencing on the Nanopore GridION. Reads were aligned against the MPXV reference genome using minimap2 v.2.24 and phylogenetic inference was performed using maximum likelihood estimation. Results: A total of 11.951 reads mapped directly to a reference genome with 96.8% of coverage (190.898 bp). Conclusion: Phylogenetic analysis of the MPXV circulating in Colombia demonstrated its close relationship to clade IIb responsible for the multi-country outbreak in 2022.

Resumen Plagioscion magdalenaees el recurso íctico más importante del embalse El Guájaro, en el norte de Colombia, y para sus pobladores ribereños, fundamental para las pesquerías de subsistencia de la región, lo que ha llevado a su sobreexplotación. En este artículo se describen los aspectos reproductivos deP. magdalenae,en este embalse. Se realizaron diez muestreos de recolección mensuales, entre abril de 2018 y enero de 2019, con el fin de abarcar cada momento del pulso de inundación. Se calcularon la proporción sexual, relación gonadosomática (RGS), fecundidad, diámetro de ovocitos y talla media de madurez sexual (L50). Se analizaron 207 ejemplares, y se establecieron siete intervalos de tallas; 126 individuos fueron machos y 81 hembras, con una proporción sexual de 1:1.6 (hembra-macho). La RGS indicó que este pez tiene varios picos reproductivos, los principales en julio y septiembre. La fecundidad absoluta fue 1909 ovocitos para aguas bajando, 1590 para aguas bajas, 2435 para aguas subiendo y 2132 para aguas altas, con un diámetro promedio de ovocito de 0.29 mm. Los machos maduraron primero que las hembras y la L50estuvo por debajo de la talla media de captura (30 cm), lo cual refleja que la normativa legal está bien determinada, pero no hay regulación para su pesca.

Abstract Plagioscion magdalenaeis the most important fish resource of the El Guájaro reservoir, Colombia, and its riparian inhabitants, fundamental for subsistence fisheries in the region, which has led to its overexploitation. This paper describes and evaluates the reproductive aspects ofP. magdalenaein this reservoir. Ten monthly collection samplings were made between April 2018 and January 2019, in order to cover each moment of the flood pulse. We calculated sex ratio, gonadosomatic ratio (RGS), fecundity, oocyte diameter and minimum and mean size at sexual maturity (L50). We analyzed 207 specimens and established seven size intervals; 126 individuals were males and 81 were females, with an overall sex ratio 1:1.6 (female-male). The RGS indicated that this fish has several reproductive peaks, the main ones being in July and September. Absolute fecundity was 1909 oocytes for falling water, 1590 for low water, 2435 for rising water and 2132 for high water; with an average oocyte diameter of 0.29 mm. Males matured earlier than females and their mean size at sexual maturity was below the mean catch size (30 cm), reflecting that legal regulations are well stipulated, but there is no strict control for fishing.

Abstract Background: Functional neuroimaging studies may aid to our understanding of the pathophysiology of the takotsubo cardiomyopathy. Objective: The aim of the study was to review the available evidence of brain functional connectivity in takotsubo cardiomyopathy patients. Methods: This was a systematic review. We searched MEDLINE, LILACS, Ovid (Cochrane), Scopus, and Science Direct for studies conducting functional magnetic resonance imaging (fMRI) in takotsubo patients. After reviewing title/abstract and full text, we selected relevant studies, extracted methodological characteristics, and their main findings, and assessed their risk of bias with the Newcastle-Ottawa scale. We present a narrative review. Results: We included five case-control studies from 600 registries. The risk of bias was low; comparability was the main issue. Resting-state fMRI findings suggest significant differences for the hippocampus, the Insula, the amygdala, and the para-hippocampal gyrus. Task fMRI findings suggest significant differences for the Insula, the superior occipital gyrus, and the amygdala. Studies were heterogeneous about the laterality and directionality of these differences. Conclusion: Brain connectivity alterations involving elements relevant for autonomic control like the Insula and the Amygdala provide evidence in favor of the role of functional networks in the neurocardiology of stress-related cardiomyopathies. However, it is not possible to determine if this role is causal or consequential.

Resumen Antecedentes: Los estudios de neuroimagen funcional podrían ayudar a clarificar la fisiopatología de la miocardiopatía de takotsubo. Objetivo: Revisar la evidencia disponible sobre conectividad funcional cerebral en pacientes con cardiomiopatía de takotsubo. Métodos: Revisión sistemática. Se buscaron en MEDLINE, LILACS, Ovid (Cochrane), Scopus, y ScienceDirect estudios de imagen por resonancia magnética funcional (IRMf) en pacientes con cardiomiopatía de takotsubo. Tras revisar títulos, resúmenes y textos completos se seleccionaron los estudios relevantes, se extrajeron sus características metodológicas y resultados principales, y se valoró su riesgo de sesgo mediante la escala Newcastle-Ottawa. Se presenta revisión narrativa de los resultados. Resultados: Se incluyeron cinco estudios de casos y controles de entre 600 registros. El riesgo de sesgo fue bajo, la comparabilidad fue la principal limitante. Los estudios de IRMf en estado de reposo sugieren diferencias significativas en el Hipocampo, la Ínsula, la Amígdala y el Giro parahipocampal. Los estudios de IRMf bajo paradigma sugieren diferencias en la Ínsula, el Giro occipital superior y en la Amígdala. Los estudios fueron heterogéneos respecto a la lateralización y direccion de estas diferencias. Conclusión: Alteraciones en la conectividad cerebral de zonas relevantes para el control autonómico como la ínsula y la Amígdala provén evidencia a favor del rol de redes funcionales en la neurocardiología de miocardiopatías relacionadas con el estrés. Sin embargo, aún no es posible determinar si esto obedece a un rol causal o consecuencial.

RESUMEN El objetivo del estudio fue investigar la relación entre los grupos sanguíneos A, B y O y la mortalidad en pacientes hospitalizados por una infección grave por SARS-CoV-2. La investigación fue observacional retrospectiva en un hospital de tercer nivel en Lima, Perú. Se incluyó a 203 pacientes, con una edad media de 62,58 ± 16,45 años, y el 71,92 % eran varones. La frecuencia de los grupos sanguíneos O, A y B fue del 75,37 %, 17,24 % y 7,39 %, respectivamente. Se encontró asociación con la mortalidad por infección grave por COVID-19 con los grupos sanguíneos que no son A (grupo O, grupo B), con un PR (razón de prevalencia) de 2,25 IC (intervalo de confianza) 95 % 1,07-4,71. Al ajustar por las principales variables, la asociación con RP persistió en 2,78 IC 95 % 1,06-7,24. En conclusión, en los pacientes hospitalizados por una infección grave por SARS-CoV-2, los grupos sanguíneos O y B estarían asociados con una mayor mortalidad que los pacientes del grupo sanguíneo A.

ABSTRACT This study aimed to determine the relationship between ABO blood groups and mortality in patients hospitalized for severe SARS-CoV-2 infection. An observational and retrospective research was conducted in a tertiary care hospital in Lima, Peru. A total of 203 patients with a mean age of 62.58 ± 16.45 years were included in the research, out of whom 71.92 % were males. The frequency of O, A and B blood groups were 75.37 %, 17.24 % and 7.39 %, respectively. An association with mortality from severe COVID-19 infection was found with non-A blood groups (O group or B group), with a PR (prevalence ratio) of 2.25 and 95% CI (confidence interval) of 1.07 - 4.71. When adjusting the main variables, the association with PR remained in 2.78 and 95% CI in 1.06 - 7.24. In conclusion, patients hospitalized for severe SARS-CoV-2 infection with O and B blood groups seem to be associated with higher mortality rates than those with A blood group.

SUMMARY The use of Magnetic Resonance Imaging (MRI) in the diagnosis and follow-up of patients with Multiple Sclerosis (MS) has optimized the care of the affected patients. Several international working groups have tried to clarify and standardize the global use of MRI but, on many occasions, data are extrapolated from other regions, do not contemplate local realities or are difficult to implement. Objective: To reach a consensus on aspects related to the use of MRI in the diagnosis and follow-up of patients with MS in Peru. Material and Methods: A group of Peruvian experts (neurologists and radiologists) worked on the elaboration of the consensus using a remote survey round methodology. Results: The recommendations, established on the basis of published evidence and on the experts’ criteria, focused on the role of both, the conventional MRI techniques and the measurement of brain atrophy in MS patients both at the time of diagnosis and during the follow-up period Conclusions: The consensual recommendations could potentially assist in the standardization and optimization of the care and follow-up of patients with MS in our country.

RESUMEN El uso de la resonancia magnética (RM) en el diagnóstico y seguimiento de pacientes con esclerosis múltiple (EM) ha optimizado el cuidado de los pacientes afectados. Diversos grupos internacionales de trabajo han intentado clarificar y normatizar el uso global de la RM pero, en muchas ocasiones, se extrapolan datos de otras regiones que no contemplan la realidad de cada lugar o son difíciles de implementar. Objetivo: Consensuar aspectos relacionados con el uso de RM en el diagnóstico y seguimiento de pacientes con EM en el Perú. Material y Métodos: Un grupo de expertos peruanos, conformado por neurólogos y radiólogos, condujo la elaboración del consenso mediante metodología de ronda de encuestas a la distancia. Resultados: Las recomendaciones, basadas en la evidencia publicada y en el criterio de los expertos, enfocaron tanto el rol de las técnicas convencionales de RM como el de la medición de la atrofia cerebral en pacientes con EM al momento del diagnóstico y durante el periodo de seguimiento. Conclusiones: Las recomendaciones del consenso podrán potencialmente homogenizar y optimizar el cuidado y seguimiento de pacientes con EM en nuestro país.

Abstract: The aim of this research study was to describe and classify French bread from neighbourhood bakeries and grocery stores in the Lima Metropolitan Area (Peru) by cluster and discriminant analyses. Mass, volume, specific volume, moisture, acidity, and salt content of 198 French bread samples were collected from 66 bakeries selected by convenience sampling. Samples were classified into three groups by using multivariate analyses. The highest mass and volume averages per bread unit were found in grocery store French breads. Specific volume ranged from 3.59 to 14.67 cm3/g. Grocery store breads were within humidity parameters set by Peruvian national regulations. However, neighbourhood bakery breads showed low humidity and high specific volume. It is concluded that French bread from the Lima Metropolitan Area can be classified into three groups according to their physicochemical characteristics.

Resumen: El objetivo de esta investigación fue describir y clasificar el pan Francés de panaderías de barrio y de supermercados de la Lima Metropolitana (Perú), mediante el análisis de conglomerados y discriminante. Se determinó la masa, volumen, volumen específico, humedad, acidez y contenido de sal de 198 muestras de pan Francés recolectadas de 66 panaderías por medio del muestreo de conveniencia. Usando los análisis multivariados, las muestras fueron clasificadas en tres grupos. Los mayores promedios de masa y volumen por unidad de pan correspondieron a los panes de supermercado; mientras que el volumen específico varió de 3.59 a 14.67 cm3/g. Los panes de supermercado estuvieron dentro del parámetro de humedad dado por la normativa nacional peruana; en cambio, los de panaderías de barrio presentaron baja humedad y elevado volumen específico. Se concluye que el pan Francés en Lima Metropolitana puede ser clasificado en tres grupos según sus características fisicoquímicas.

Abstract The aimed of this study was quantified total phenols content, total flavonoids, and antioxidant activity in extruded ready-to-eat products made from yellow corn added with apple pomace powder. A central composite rotatable experimental design was used. The variables tested were exit die temperature (140-180 °C), feed moisture (14-20 g/100 g) and apple pomace concentration (0-30 g/100 g). The effect of the process variables on the total contents of phenols, flavonols, flavanols and flavonoids and antioxidant activity was evaluated using the response surface methodology. The data of each response were fitted to a second-order model with determination coefficients ≥ 0.71. The extruded product with no added apple bagasse showed the lowest values of the evaluated phytochemicals but had higher antioxidant activity by DPPH. The addition of apple pomace increased the phytochemical content and antioxidant activity in extruded ready-to-eat products made with yellow corn. This makes it possible to obtain a formulation of an extruded food rich in phenolic compounds with potential to contribute to prevention of oxidative stress, while making use of agro-industrial waste from apple juice extraction.

Abstract: The main goal of this study is to characterize Chilean distance education processes during the COVID-19 pandemic from the experience of professors and students. Theoretical aspects are assessed in relation to the best-know distance education practices, gender, and field of study. Two online closed-answer instruments are applied to assess perceptions on technological conditions, surrounding environment, digital skills, professor-student relationships, teaching and evaluation processes, and emotions. The sample consists of 1750 students and 654 professors from 34 Chilean universities. The results show multiple cross-cutting elements that differ according to year of study, student gender, and university program. It is concluded that there is a need for professor training in pedagogy, for reinforcing student-professor dialogue, and for developing improved assessment practices that integrate feedback, collaborative work, and the inclusion of distance education teaching strategies that are different from that of face-to-face education.

Resumen: El objetivo de este estudio es caracterizar el proceso educativo remoto en Chile durante la pandemia COVID-19 desde la experiencia de docentes y estudiantes. Se consideran aspectos teóricos sobre las mejores prácticas de educación remota, el género, y el área disciplinar. Se aplicaron dos instrumentos (en línea) de respuesta cerrada sobre la percepción de condiciones físicas-tecnológicas, competencias digitales, la relación profesor-estudiantes, el proceso de enseñanza y evaluación, y las emociones en educación remota en pandemia. La muestra está compuesta por un grupo de 1750 estudiantes y 654 profesores de 34 universidades chilenas. Los resultados dan cuenta de elementos transversales, con ciertas diferencias de acuerdo a niveles formativos, al género y al área disciplinar. Se concluye que existe la necesidad de formación pedagógica para docentes, la necesidad de más diálogo profesor-estudiante, y la mejora de las prácticas evaluativas que integran la retroalimentación, el trabajo colaborativo y la incorporación de estrategias de enseñanza diferentes a la educación presencial.

Resumen Aleaciones cristalinas desordenadas Fe50Mn25+xSn25_x, con x = -1.25, 0.0, 2.5, 5.0, 7.5, cercanas a las composiciones de Heusler-211, fueron preparadas por fusión en atmósfera inerte, subsecuentemente, sometidas a recocido térmico durante 4 días a 1173 K y, finalmente, enfriadas en agua helada. Todas las aleaciones han sido sistemáticamente analizadas mediante difracción de rayos X, espectroscopias Mössbauer de 57Fe y 119Sn, y medidas de magnetización a temperatura ambiente. Los análisis Rietveld de los difractogramas de las muestras con x = -1.25 y 0.0 muestran la presencia de dos fases cristalográficas hexagonales: (i) la solución solida químicamente desordenada ε-(Fe/Mn)3Sn (mayoritaria), y (ii) el intermetálico ε-Fe5Sn3 (minoritaria); mientras que, las aleaciones con x = 2.5, 5.0 y 7.5, presentan solo la solución sólida desordenada, ε-(Fe/Mn)3(Sn/Fe/Mn). Si bien, las composiciones de las aleaciones Fe50Mn25+xSn25-x son cercanas a las de Heusler-211, ninguna tiene la estructura cúbica L2 1 característica de estas. Los tamaños medios de los cristalitos, calculados por el método de Williamson-Hall, están dentro del rango 62-256 nm y disminuyen cuando x aumenta. Los resultados Mössbauer de todas las aleaciones muestran la característica de magnetismo localizado a través de la distribución de campos magnéticos hiperfinos en los sitios de Fe, y de magnetismo itinerante a través de los campos transferidos desde los átomos de Fe en los núcleos de 119Sn. Los campos coercitivos y remanentes, obtenidos por medidas de magnetización bajo campo magnético aplicado entre -2200 y +2200 Oe, disminuyen cuando x aumenta. Los valores de los campos coercitivos están en el rango de los magnetos blandos (1-20 Oe).

Abstract Disordered crystalline Fe50Mn25+xSn25-x alloys, with x = -1.25, 0.0, 2.5, 5.0, 7.5 (close to the full-Heusler alloys), were arc-melted in a high purity argon atmosphere and the molten pellets were individually sealed in quartz tubes also under argon atmosphere. Subsequently, they were annealed at 1173 K for 4 days, being finally quenched in a bath with cold water. Structural and magnetic properties have systematically been studied using X-ray diffraction, 57Fe, and 119Sn Mössbauer spectroscopies, and magnetization measurements recorded at room temperature. Rietveld refinement of the X-ray diffraction patterns of the annealed samples with x = -1.25 and 0 has revealed the presence of two hexagonal crystallographic phases: (i) a chemically disordered solid solution identified as ε-(Fe/Mn)3Sn (majority fraction) and (ii) the ε-Fe5Sn3 intermetallic compound (minority fraction). For samples with x = 2.5, 5.0, and 7.5, the Rietveld analysis has only indicated the presence of a chemically disordered solid solution identified as ε -(Fe/Mn)3(Sn/Fe/Mn). Although compositions of the Fe50Mn25+xSn25_x alloys are close to that of full-Heusler alloys, none of them has the expected L2 1 structure. The average crystallite sizes, estimated from the Williamson-Hall method, are in the range of 256-62 nm. The average sizes has gradually decreased as the x-content is increased. Mössbauer results have shown localized-type magnetism from Fe non-equivalent sites, and itinerant-like magnetism on 119Sn-probes. Magnetic hysteresis loops, recorded at 300 K for a maximum field of 2200 Oe, have indicated that the remanent and coercive fields have systematically decreased as the x-parameter has increased. Coercive fields are in the range for soft magnets (1-20 Oe).

Abstract We performed a single center retrospective study in patients with pulmonary embolism (PE) undergoing catheter directed thrombolysis (CDT) from 2014 to 2020. Efficacy was defined by mean pulmonary pressure drop, and safety was assessed by intracranial and severe bleeding (defined by GUSTO). Forty-three patients were included, aged 64 (56-79) years old, 5 (12%) with shock, most with right ventricle dilation (95%) and bilateral PE (95%) or unilateral (5%) in patients with only one functional lung. CDT was used as first treatment (53%), upscale after anticoagulation alone (42%), or after failed systemic thrombolytics (5%). Median recombinant tissue plasminogen activator (rtPA) dose was 30 (25-35) mg over 20 (20-20) hours, and rtPA bolus was used after catheter placement in 38 cases (89%), consisting of 5 mg (95%) or 1 mg (5%). Only one lung was treated for technical reasons, and 4 (9%) were repositioned in the same lung for continuation of infusion. A significant reduction in mean pulmonary pressure was observed (pre 35 [29-41] mmHg vs. post 24 [20-34] mmHg, p<0.001) with no intracranial bleeding. One patient (2%) experienced severe bleeding, while 5 (12%) presented access site bleeding, and 3 (7%) required blood transfusions. In-hospital mortality was 12% but only one case (2%) due to PE. Our results are similar to previously reported studies.

Resumen Se realizó un estudio unicéntrico retrospectivo para evaluar la eficacia y seguridad de trombolisis dirigida por catéter (TDC) en pacientes con tromboembolismo pulmonar agudo (TEP) de 2014 a 2020. Se analizó la efectividad (mejoría de presión pulmonar), y seguridad (sangrado intracraneal y grave definido por compromiso hemodinámico). Se incluyeron 43 pacientes, de 67(56-79) años, 5 (12%) con shock, 41 (95%) con dilatación del ventrículo derecho y TEP bilateral. La decis ión de TDC fue: tratamiento inicial (53%), escalada de anticoagulación (42%) y rescate de trombolisis sistémica (5%). Se utilizó TDC facilitada por ultrasonido en 40 casos (93%), utilizándose 30 (25-35) mg de activador tisular del plasminógeno recombinante (rtPA) durante 20 h. Se administró un bolo de rtPA en 38 (89%) casos, que fue 5 mg (95%) o 1 mg (5%). Se utilizó un solo catéter por paciente. En 4 (9%) se decidió recolocación (mismo pulmón) para continuar infusión en otro sector. Se observó una disminución significativa de la presión media pulmonar (pre 35 [29-41] mmHg vs. post 24 [20-34] mmHg, p<0.001). No se observó ningún caso de hemorragia intracraneal, y un caso (2%) de sangrado grave. Se observó hematoma del sitio de punción en 5 (12%) (incluyendo el sangrado grave), y requirió transfusiones en 3 (7%). La mortalidad intrahospitalaria fue 12%, siendo un solo c aso (2%) atribuido al TEP. El tratamiento con TDC fue efectivo asociándose a una reducción significativa de la presión pulmonar, sin observarse ningún sangrado intracraneal y con un sangrado grave. Nuestros resultados se asemejan a lo publicado en otros estudios.

Recognizing the role of technology in the development of medicine and the impact of telecommunication advances, we reflect on the meaning and ethics of the use of Telemedicine, both in its general dimension for the use and distribution of knowledge, as well as in the delivery of health actions, scientific research, and data management. Teleconsultation is discussed in greater detail, analyzing its process and application, reviewing its possible advantages and disadvantages, from the point of view of providers and patients. We highlight the need to carry out an appropriate evaluation of each instance, from the point of view of both the patient and the professional who uses it. The importance of maintaining a doctor-patient relationship in agreement with the nature and practice of Medicine, respecting people’s dignity, is emphasized. We mention the ethical conditions that must be bore in mind for the proper use of telemedicine. We discuss the eventual influence that this practice will have on the concept and practice of medical care, while suggesting the need to legislate on the matter.
