Objetivo: Avaliar a factibilidade da implantação precoce de um programa de reabilitação da deglutição em pacientes traqueostomizados com disfagia e sob ventilação mecânica. Métodos: Estudo prospectivo realizado em unidades de terapia intensiva de um hospital universitário. Incluímos pacientes hemodinamicamente estáveis e submetidos à ventilação mecânica por pelo menos 48 horas e há no mínimo 48 horas com traqueostomia e nível adequado de consciência. Os critérios de exclusão foram cirurgia prévia na cavidade oral, faringe, laringe e/ou esôfago, presença de doenças degenerativas ou história pregressa de disfagia orofaríngea. Todos os pacientes foram submetidos a um programa de reabilitação da deglutição. Antes e após o tratamento de reabilitação da deglutição, foram determinados um escore estrutural orofaríngeo, um escore funcional de deglutição, e um escore otorrinolaringológico estrutural e funcional. Resultados: Foram incluídos 14 pacientes. A duração média do programa de reabilitação foi de 12,4 ± 9,4 dias, com média de 5,0 ± 5,2 dias sob ventilação mecânica. Onze pacientes puderam receber alimentação por via oral enquanto ainda permaneciam na unidade de terapia intensiva após 4 (2 - 13) dias de tratamento. Todos os escores apresentaram melhora significante após o tratamento. Conclusão: Neste pequeno grupo de pacientes, a implantação de um programa precoce de reabilitação da deglutição foi factível, mesmo em pacientes sob ventilação mecânica.

Objective: The aim of the present study was to assess the feasibility of the early implementation of a swallowing rehabilitation program in tracheostomized patients under mechanical ventilation with dysphagia. Methods: This prospective study was conducted in the intensive care units of a university hospital. We included hemodynamically stable patients under mechanical ventilation for at least 48 hours following 48 hours of tracheostomy and with an appropriate level of consciousness. The exclusion criteria were previous surgery in the oral cavity, pharynx, larynx and/or esophagus, the presence of degenerative diseases or a past history of oropharyngeal dysphagia. All patients were submitted to a swallowing rehabilitation program. An oropharyngeal structural score, a swallowing functional score and an otorhinolaryngological structural and functional score were determined before and after swallowing therapy. Results: We included 14 patients. The mean duration of the rehabilitation program was 12.4 ± 9.4 days, with 5.0 ± 5.2 days under mechanical ventilation. Eleven patients could receive oral feeding while still in the intensive care unit after 4 (2 - 13) days of therapy. All scores significantly improved after therapy. Conclusion: In this small group of patients, we demonstrated that the early implementation of a swallowing rehabilitation program is feasible even in patients under mechanical ventilation.

Estudo de casos para caracterizar os benefícios da aplicação de toxina botulínica em glândulas salivares, associada à fonoterapia em pacientes disfágicos graves. Foram analisados cinco prontuários de pacientes neurológicos, em uso exclusivo de via alternativa de alimentação, com idades entre 17 e 70 anos, sendo quatro do gênero masculino e um do gênero feminino. Do total, quatro pacientes eram traqueostomizados. Foi considerado como critério de inclusão apresentar disfagia grave, com manifestações clínicas de escape extra oral e/ou acúmulo de saliva em cavidade oral e aspiração traqueal maciça de saliva, com limitação da fonoterapia. Quanto à avaliação clínica da deglutição, foram coletados dados pré e pós-fonoterapia associada à aplicação de toxina botulínica, quanto aos seguintes aspectos: mobilidade e força das estruturas orofaríngeas (lábios, língua, bochechas), elevação laríngea, grau da disfagia, uso de via alternativa de alimentação e traqueostomia. Quanto aos resultados pós- fonoterapia foi observado, em quatro pacientes, melhora da mobilidade e força de lábios, língua, bochechas e laringe. Quatro pacientes apresentaram deglutição funcional e um teve modificação do grau de gravidade da disfagia. Desta forma, a maioria foi capaz de receber dieta exclusiva por via oral e apenas um permaneceu com dieta mista, ou seja, gastrostomia e dieta via oral na consistência pastosa. Todos os pacientes traqueostomizados tiveram a cânula de traqueostomia removida. O estudo mostrou que o tratamento descrito acima contribui para a reabilitação da deglutição, reintrodução de alimentos por via oral e retirada da cânula de traqueostomia.

Case report with the aim to characterize the benefits of botulinum toxin injection into salivary glands in association with swallowing therapy in patients with severe dysphagia. The medical records of five neurological patients (four male and one female, aged between 17 and 70 years) who exclusively used alternative feeding were analyzed. Four patients were tracheostomized. Inclusion criterion was to present severe dysphagia associated to clinical manifestations of drooling and/or sialorrhea with significant aspiration of saliva, restricting the improvement in swallowing rehabilitation. Data were collected before and after intervention associated with botulinum toxin injection, regarding the following aspects: mobility and strength of oropharyngeal structures (lips, tongue and cheeks), laryngeal elevation, severity degree of dysphagia, use of alternative tube feeding and tracheostomy. After swallowing therapy, four patients showed improvement in mobility and strength of the lips, tongue, cheeks and larynx. Four patients presented functional swallowing and one of them modified had the severity degree of dysphagia changed. Therefore, most patients were able to receive exclusive oral feeding, and only one remained on mixed feeding, that is, gastrostomy and oral feeding with pasty consistence. All tracheostomized patients had the tracheostomy cannula removed. The study showed that the treatment described contributed to swallowing rehabilitation, reintroduction of oral feeding, and withdrawal of the tracheostomy cannula.

OBJETIVO: verificar se as condições referentes à estrutura geral dos asilos, recursos humanos e rotinas diárias de alimentação em instituições asilares, podem potencializar alterações da dinâmica da deglutição em idosos. MÉTODOS: foi aplicado questionário aos dirigentes de cinco instituições asilares do município do Rio de Janeiro, no qual constavam perguntas em relação aos recursos materiais, humanos e rotina alimentar. RESULTADOS: o asilo do sistema privado foi o único a aproximar-se das condições ideais da estrutura geral para o atendimento do idoso. Em relação aos recursos humanos existentes, nenhum dos asilos possuía todos os profissionais exigidos pela portaria nº810/89. Em relação às rotinas diárias de alimentação, em uma das instituições pesquisadas, cuja maioria dos residentes não faziam uso de próteses dentárias, não havia restrição na consistência alimentar oferecida. Outro fator significativo diz respeito à negligência referente à higiene bucal, favorecendo a colonização de bactérias na cavidade oral, podendo agravar infecções pulmonares no caso de microaspirações. O fato dos idosos deitarem para dormir logo após o término das refeições, como ocorrido em três das instituições pesquisadas, tem grande importância na medida em que favorece a ocorrência de episódios de refluxo gastroesofágico. CONCLUSÃO: foram observados que em todas as instituições pesquisadas há fatores que podem potencializar um distúrbio de deglutição, como problemas em relação à estrutura geral e ou aos recursos humanos e ou relativos à rotina alimentar estabelecida.

PURPOSE: to check if the conditions related to general structure, human resources and daily routine of feeding in asylums can increase the chances for an alteration in the deglutition process of the elderlies. METHODS: a questionnaire was elaborated to be applied to the directors of five institutions in the city of Rio de Janeiro. RESULTS: it can be observed that the sole factor related to the structure of the place that had relevance in the obtained results was the nature of the institution, as the private system was the only one that was considered near the ideal conditions. Regarding the existing human resources, none of the institutions had all the professionals demanded by the Ordinance nº810/89. In relation to the daily feeding routines, in one of the investigated institutions, where the majority of the residents did not use denture, there was no restriction as for the offered food consistency. Another significant factor was the carelessness in terms of oral hygiene, favoring the settling of bacteria in the mouth, which can cause serious consequences in case of microaspirations. In three of the investigated institutions, it was observed that the elderlies lay down to sleep soon after the meals, fact that also had great importance, as this favors the incidence of gastroesophageal reflux episodes that can lead to gradual dysphagia. CONCLUSION: despite the fact that the asylum directors did not mention any referring problem in the process of deglutition of the elderlies, they show proper characteristics that can increase the possibilities for occurrence of alterations in the deglutition of these elderlies.

CONTEXTO: Muitos pacientes em uso de ventilação mecânica apresentam complicações clínicas por apresentarem aspiração traqueal. Métodos de avaliação e detecção precoce são necessários para que medidas preventivas e de segurança se estabeleçam nessa população. OBJETIVO: Pesquisar a incidência de aspiração traqueal de saliva em indivíduos traqueostomizados internados em unidades de terapia intensiva fazendo uso de dois modos de ventilação mecânica e com diferentes níveis de sedação. MÉTODO: Estudo prospectivo realizado com 14 pacientes não-neurológicos traqueostomizados em uso de ventilação mecânica. A amostra foi dividida em dois grupos baseada nos modos ventilatórios: ventilação por pressão de suporte e ventilação por pressão controlada. Estes dois grupos foram subdivididos em dois subgrupos, de acordo com o nível de sedação. Foi feita a avaliação fonoaudiológica por meio do teste de anilina culinária azul, a fim de analisar a incidência da aspiração traqueal de saliva. RESULTADOS: O nível de sedação e tempo de ventilação mecânica em relação à aspiração traqueal não foram estatisticamente significantes nesse estudo. Já a relação entre modo ventilatório e aspiração traqueal apresentou significância estatística, sendo que houve incidência maior de aspiração traqueal no modo ventilação por pressão controlada. CONCLUSÃO: Pôde-se observar relação significativa entre a incidência da aspiração traqueal e o modo ventilatório ventilação por pressão controlada, o que pode implicar na inclusão desses pacientes no grupo de risco para disfagia orofaríngea e sua inserção nos protocolos de prevenção. A relação entre aspiração traqueal e níveis de sedação, bem como aspiração traqueal e tempo de ventilação mecânica não foram estatisticamente significantes nessa amostra, necessitando de futuros estudos.

CONTEXT: Many patients in use of mechanical ventilation show clinical complications due to tracheal aspiration. Assessment and early methods are necessary, so that preventive and safety measures apply to this patients OBJECTIVE: To study the incidence of tracheal aspiration of saliva in tracheotomized patients treated in intensive care unit using two modes of mechanical ventilation and with different sedation levels. METHOD: Prospective study with 14 tracheotomized non-neurological patients using mechanical ventilation. The sample was divided into two groups based on ventilation mode: pressure support ventilation and pressure controlled ventilation. Those two groups were subdivided into two others according to sedation level. The speech pathology evaluation was completed via the blue dye test in order to analyze the incidence of tracheal aspiration of saliva. RESULTS: Sedation levels and mechanical ventilation time related to tracheal aspiration were not statistically significant in this study. On the other hand, ventilation mode and tracheal aspiration showed statistical significance, and there was a higher incidence of tracheal aspiration in the pressure controlled ventilation mode. CONCLUSION: It was possible to observe a significant relationship between tracheal aspiration incidence and pressure controlled ventilation mode, which means the inclusion of those patients in the risk group for oropharyngeal dysphagia and their insertion in prevention protocols. The relationship between tracheal aspiration and sedation level, as well as tracheal aspiration and mechanical ventilation, were not statistically significant in this sample, needing further research.

In this work titanium sol was prepared from tetraethylorthotitanate (TEOT) in ethanol, stabilized with beta-diketonate 2,4 pentanedione in molar ratio 1:1 homogenized by magnetic stirring, europium ion was add as structural probe. The xerogels were heat treated at 500, 750 and 1000 °C and the characterization was realized by x-ray diffraction (XRD), transmission electron microscopy (TEM), thermogravimetric analysis (TGA/DSC) and photoluminescence (PL). The excitation spectra of Eu (III) ion present maximum in 394 nm correspondent to 5L6 level and emission spectra present bands characteristic transitions arising from the 5 D0 -> 7F J (J = 0, 1, 2, 3, 4) manifolds to samples treat at 500 and 750 °C. The Eu (III) emission disappear, when heated at 1000 °C, probably due to phase transition anatase to rutile and migrations of ions to the external surface that was proved by x-ray diffraction, transmission electronic microscopy and the thermogravimetric analyses of xerogels.
