An observational study was conducted to know the serotypes and antimicrobial susceptibility of isolates of Streptococcus pneumonia responsible for meningitis in Cuba, where there is no vaccine yet to prevent invasive pneumococcal disease. The study included the total number of isolates submitted to the "Pedro Kourí" Institute between 2007 and 2012 (N=237). Serotypes identification was performed using capsular swelling test and antimicrobial susceptibility was studied by determining the minimum inhibitory concentration using the broth microdilution method. Predominant serotypes were 6A, 6B, 14, 19F and 23F and other non-vaccinal 18 serogroups/serotypes were identified in 29.1% of the isolates. A tendency to an increased resistance to penicillin (44.3 %) was observed; the most common resistance patterns were: penicillin-trimethoprim/sulfamethoxazole and penicillin-erythromycin (21.1% and 10.5%, respectively). The largest number of isolates resistant to penicillin was in serotypes 6B, 14, 19F and 23F and the possibility of resistant non-vaccine serotypes emergence should be considered. The results show that 70.4 % of the isolates studied corresponds to the serotypes included in 13-valent conjugated pneumococcal vaccine, but with 10-valent it would achieve a lower vaccination potential coverage (56.1%). This information must be considered when evaluating the decision to use in Cuba any commercially available vaccine or the proposal of another strategy of vaccination from autochthonous vaccine candidates.
Se realizó un estudio observacional para conocer los serotipos y la susceptibilidad antimicrobiana de los aislamientos de Streptococcus pneumoniae responsables de meningitis en Cuba, donde aún no se vacuna para prevenir la enfermedad neumocócica invasiva. El estudio incluyó el total de los aislamientos remitidos al Instituto “Pedro Kouríâ€� entre el 2007-2012 (N = 237). La identificación de los serotipos se realizó por la prueba de hinchazón capsular. Mediante la determinación de la concentración mínima inhibitoria, por el método de microdilución en caldo, se estudió la susceptibilidad a los antimicrobianos. Predominaron los serotipos 6A, 6B, 14, 19F y 23F, además en el 29,1% de los aislamientos se identificaron 18 serogrupos/serotipos no incluidos en las vacunas conjugadas. Se observó una tendencia al incremento de la resistencia a la penicilina (44,3%); los patrones de resistencia más frecuentes fueron: penicilina-trimetoprim/sulfametoxazol y penicilina-eritromicina (21,1% y 10,5%, respectivamente). Los resultados de la vigilancia demuestran que el 70,4% de los aislamientos estudiados se corresponden con los serotipos incluidos en la vacuna neumocócica conjugada de 13-valencias y que con las vacunas de 7 y 10 valencias se alcanzaría una cobertura potencial de vacunación inferior (52,3 y 56,1%, respectivamente). Esta información debe considerarse al evaluar la decisión del uso en Cuba de alguna de las vacunas comercialmente disponibles o la propuesta de otra estrategia de vacunación a partir de candidatos vacunales autóctonos.
