ABSTRACT Objective: To report outcomes from the largest multicenter series of penile cancer patients undergoing video endoscopic inguinal lymphadenectomy (VEIL). Materials and Methods: Retrospective multicenter analysis. Authors of 21 centers from the Penile Cancer Collaborative Coalition-Latin America (PeC-LA) were included. All centers performed the procedure following the same previously described standardized technique. Inclusion criteria included penile cancer patients with no palpable lymph nodes and intermediate/high-risk disease and those with non-fixed palpable lymph nodes less than 4 cm in diameter. Categorical variables are shown as percentages and frequencies whereas continuous variables as mean and range. Results: From 2006 to 2020, 210 VEIL procedures were performed in 105 patients. Mean age was 58 (45-68) years old. Mean operative time was 90 minutes (60-120). Mean lymph node yield was 10 nodes (6-16). Complication rate was 15.7%, including severe complications in 1.9% of procedures. Lymphatic and skin complications were noted in 8.6 and 4.8% of patients, respectively. Histopathological analysis revealed lymph node involvement in 26.7% of patients with non-palpable nodes. Inguinal recurrence was observed in 2.8% of patients. 10y- overall survival was 74.2% and 10-y cancer specific survival was 84.8%. CSS for pN0, pN1, pN2 and pN3 were 100%, 82.4%, 72.7% and 9.1%, respectively. Conclusion: VEIL seems to offer appropriate long term oncological control with minimal morbidity. In the absence of non-invasive stratification measures such as dynamic sentinel node biopsy, VEIL emerged as the alternative for the management of non-bulky lymph nodes in penile cancer.

Abstract Background Major advances have been seen in techniques and devices for performing percutaneous coronary interventions (PCIs) for chronic total occlusions (CTOs), but there are limited real-world practice data from developing countries. Objectives To report clinical and angiographic characteristics, procedural aspects, and clinical outcomes of CTO PCI performed at dedicated centers in Brazil. Methods Included patients underwent CTO PCI at centers participating in the LATAM CTO Registry, a Latin American multicenter registry dedicated to prospective collection of these data. Inclusion criteria were procedures performed in Brazil, age 18 years or over, and presence of CTO with PCI attempt. CTO was defined as a 100% lesion in an epicardial coronary artery, known or estimated to have lasted at least 3 months. Results Data on 1196 CTO PCIs were included. Procedures were performed primarily for angina control (85%) and/or treatment of moderate/severe ischemia (24%). Technical success rate was 84%, being achieved with antegrade wire approaches in 81% of procedures, antegrade dissection and re-entry in 9%, and retrograde approaches in 10%. In-hospital adverse cardiovascular events occurred in 2.3% of cases, with a mortality rate of 0.75%. Conclusions CTOs can be treated effectively in Brazil by using PCI, with low complication rates. The scientific and technological development observed in this area in the past decade is reflected in the clinical practice of dedicated Brazilian centers.
Resumo Fundamento Tem sido observado um grande avanço nas técnicas e nos dispositivos para a realização de intervenções coronárias percutâneas (ICP) em oclusões totais coronarianas crônicas (OTC), mas existem poucos dados da prática do mundo real em países em desenvolvimento. Objetivos Relatar as características clínicas e angiográficas, os aspectos dos procedimentos e os resultados clínicos da ICP de OTC em centros dedicados a esse procedimento no Brasil. Métodos Os pacientes incluídos foram submetidos à ICP de OTC em centros participantes do LATAM CTO Registry, um registro multicêntrico latino-americano dedicado à coleta prospectiva desses dados. Os critérios de inclusão foram procedimentos realizados no Brasil, idade acima de 18 anos e presença de OTC com tentativa de ICP. A definição de OTC foi lesão de 100% em uma artéria coronária epicárdica, conhecida ou estimada como tendo pelo menos 3 meses de evolução. Resultados Foram incluídos dados de 1.196 ICPs de OTC. Os procedimentos foram realizados principalmente para controle da angina (85%) e/ou tratamento de uma grande área isquêmica (24%). A taxa de sucesso técnico foi de 84% e foi alcançada com técnicas de fios anterógrados em 81%, dissecção/reentrada anterógrada em 9% e retrógrada em 10% dos procedimentos. Os eventos cardiovasculares adversos intra-hospitalares ocorreram em 2,3% dos casos, sendo a mortalidade de 0,75%. Conclusões As OTC podem ser tratadas no Brasil por intervenção coronária percutânea de forma efetiva e com baixas taxas de complicações. O desenvolvimento científico e tecnológico observado nessa área na última década reflete-se na prática clínica de centros brasileiros dedicados a essa técnica.

Comparative analysis of entrepreneurial intention models: Self-efficacy versus entrepreneurial characteristics

Abstract Purpose: The purpose of this research is to analyze whether self-efficacy and entrepreneurial characteristics (e.g., risk-taking, planning, opportunity recognition, persistence, sociability, innovation, and leadership) differ in the explanation of entrepreneurial intention models, having entrepreneurial education as a moderator of relationships. Originality/value: This paper offers an in-depth perspective on which behavioral characteristics fit best into entrepreneurial intention models and helps to fill a theoretical and practical gap on the need for university education to increase its positive impact on human capital, particularly on students’ professional skills, demonstrating which characteristics are most impacted by entrepreneurial education. Design/methodology/approach: Quantitative methodology was employed, and data were analyzed with structural equation modeling. The sample consisted of 1,004 Brazilian university students from public and private universities. The research was conducted in a non-probabilistic way and with a convenience sample. In order to address the research hypothesis and objectives, all constructs were adapted from relevant literature on entrepreneurship. Findings: The results indicate it seems more appropriate to use a set of entrepreneurial characteristics when investigating an entrepreneurial intention model if you are looking for a higher explanatory factor and a more robust model; also, entrepreneurial characteristics represent a more sensitive model, which brings accurate assessments on factors that affect entrepreneurial intention. With these findings, entrepreneurial education can be planned to shape certain characteristics through actions in the university environment, making it possible to measure the impact of education on entrepreneurial intention.
Resumo Objetivo: O objetivo da pesquisa é analisar se a autoeficácia e as características empreendedoras (por exemplo, tomada de risco, planejamento, reconhecimento de oportunidade, persistência, sociabilidade, inovação e liderança) diferem na explicação dos modelos de intenção empreendedora, tendo a educação empreendedora como moderadora dos relacionamentos. Originalidade/valor: O estudo oferece uma perspectiva aprofundada sobre quais características comportamentais se encaixam melhor em modelos de intenção de empreendedorismo e auxilia a preencher uma lacuna teórica e prática sobre a necessidade de a educação universitária aumentar seu impacto positivo sobre o capital humano, particularmente nas habilidades profissionais dos alunos, demonstrando quais características são mais impactadas pela educação empreendedora. Design/metodologia/abordagem: Foi utilizada metodologia quantitativa e os dados foram analisados com modelagem de equações estruturais. A amostra foi composta por 1.004 universitários brasileiros, de universidades públicas e privadas. A pesquisa foi realizada de forma não probabilística e com amostra de conveniência. Para abordar as hipóteses e objetivos da pesquisa, todos os construtos foram adaptados da literatura de empreendedorismo. Resultados: Os resultados indicam que pode ser mais adequado utilizar um conjunto de características empreendedoras ao investigar um modelo de intenção empreendedora, quando se busca um fator explicativo superior e um modelo mais robusto; além disso, as características empreendedoras representam um modelo mais sensível, que traz avaliações precisas sobre os fatores que afetam a intenção empreendedora. Com esses resultados, a educação empreendedora pode ser planejada para moldar determinadas características, por meio de ações no ambiente universitário, sendo possível medir o impacto da educação na intenção empreendedora.

ABSTRACT Purpose To correlate the dysphagia quality of life and symptoms of anxiety and depression before and after thyroidectomy. Methods Observational, longitudinal, prospective, and experimental study. Twenty patients participated, with a mean age of 54 years, prevalence of females (n=17; 85%) and partial thyroidectomy (n=14; 70%). All subjects underwent laryngeal visual examination and answered the MD Anderson Dysphagia Questionnaire (MDADI) and the Hospital Anxiety and Depression Scale (HADS) in three different moments: preoperatively, immediately postoperatively (maximum one week) and three months after surgery. Results There was a significant difference in dysphagia quality of life for the physical and total domains in the three different moments. Regarding anxiety and depression, a statistically significant difference was observed between the scores in all domains, with a greater difference observed between the preoperative period and after 1 week. Higher values were observed in the preoperative period for mild anxiety traits, being more frequent in relation to depression, with a reduction after 1 week and an increase after three months of surgery. There was no significant correlation between the MDADI and HADS protocols. Conclusion Patients undergoing thyroidectomy self-report better quality of life in dysphagia and reduced anxiety/depression scores after three months of surgery. There was no correlation between anxiety, depression and quality of life in dysphagia at the moments evaluated.
RESUMO Objetivo Correlacionar a qualidade de vida em disfagia e sintomas de ansiedade e depressão pré e pós-tireoidectomia. Método Estudo observacional, longitudinal, prospectivo e quantitativo. Participaram 20 pacientes, com média de idade de 54 anos, maior prevalência do sexo feminino (n=17; 85%) e de tireoidectomia parcial (n=14; 70%). Todos realizaram exame visual laríngeo e responderam ao Questionário de Disfagia M. D. Anderson (MDADI) e à Escala Hospitalar de Ansiedade e Depressão (HADS), no pré-operatório, pós-operatório recente (máximo uma semana) e três meses pós-operatório. Resultados Houve diferença significante na qualidade de vida em disfagia, para os domínios físico e total, nos três momentos. Quanto à ansiedade e depressão foi observada diferença estatisticamente significante entre as pontuações em todos os domínios, com maior diferença observada entre o pré-operatório e pós-operatório recente. Foram observados valores mais altos no pré-operatório para traços leves de ansiedade, sendo mais frequentes em relação à depressão, com redução após 1 semana e aumento após três meses de cirurgia. Não houve correlação significante entre os protocolos MDADI e HADS. Conclusão Os pacientes submetidos à tireoidectomia autorreferem melhor qualidade de vida em disfagia e redução dos escores de ansiedade/depressão após três meses de cirurgia. Não houve correlação entre ansiedade, depressão e qualidade de vida em disfagia nos momentos avaliados.

Physicochemical Properties of Superabsorbent Hydrogels Formed by Polyelectrolytic Complexation of Carboxymethylcellulose-Chitosan at Basic pH

High water consumption during irrigation is one of the main problems in agriculture. The preparation of a material that prolongs the water retention capacity of the soil is important. This article aimed to prepare superabsorbent hydrogels by the complexation of chitosan (CHI), with high degree of deacetylation (DD), and carboxymethylcellulose (CMC), with high degree of substitution (SD). Different parameters (concentration and polymeric solutions addition order, polysaccharides’ ratio, pH) were studied. Morphology, thermal stability, chemical composition, mechanical strength, and swelling kinetics of hydrogels in water were evaluated. A hydrogel formation mechanism was discussed and proposed. Results showed that most samples prepared at acid pH disintegrated during swelling test. The morphology of the hydrogels varied with the pH. It was obtained superabsorbent hydrogels (swelling > 400%) based on CMC (SD = 2.23) and CHI (DD = 92%), at basic pH, with good compression strength, and applicable as soil conditioner. The best sample was the one prepared with CMC:CHI = 1:2, polymeric solutions 1.5% m/v and pH 8.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

RESUMO Objetivo: Estimar a epidemiologia do pterígio; sua correlação com sintomas de olho seco e com potenciais pre ditores sistêmicos e oculares. Métodos: Estudo transversal, de base populacional, no qual foram realizadas visitas domiciliares aleatórias a 600 participantes, com 40 anos ou mais de idade, em Ribeirão Preto-SP (n=420) e Cassia dos Coqueiros-SP (n=180), Brasil. Uma entrevista estruturada com um questionário detalhado foi usada para coletar informações sobre demografia e possíveis fatores de risco. Em um segundo momento, participantes aleatórios com pterígio (n=63) ou não (n=110) foram avaliados quanto a alterações na superfície ocular. Resultados: A frequência de pterígio em Ribeirão Preto foi de 21%; 15.7% entre as mulheres e 32.1% entre os homens (p=0,0002). Em Cássia dos Coqueiros, essa taxa foi de 19.4%; onde 17.3% eram mulheres e 25.5% eram homens (p=0,28). A média de idade naqueles afetados pelo pterígio foi superior à dos participantes sem pterígio, 65,6 ± 10,5 e 61,2 ± 12,0 anos, respectivamente (p=0,02). Houve uma correlação positiva entre o pterígio e história prévia de radioterapia e quimioterapia (p<0,0001 para ambos). Houve maior coloração de fluoresceína na córnea e maior coloração de lissamina verde na conjuntiva em olhos com pterígio (p=0,0003 e 0,0001, respectivamente). Conclusão: Encontramos uma alta frequência de pterígio em duas populações adultas brasileiras, principalmente em homens e idosos. Danos na superfície ocular e história prévia de radioterapia e/ou quimioterapia foram associados ao pterígio.
ABSTRACT Objective: To estimate the epidemiology of the pterygium and its correlation with dry eye symptoms and with the potential systemic and ocular predictors. Methods: This study is a population-based, cross-sectional study in which random visits were made to the 600 households of 600 participants of age ≥40 years in Ribeirão Preto-SP (n=420) and Cassia dos Coqueiros-SP (n=180) in Brazil. The participants were subjected to a structured interview with a detailed questionnaire to collect information on demography and the potential risk factors. Next, random participants with pterygium (n=63) or not (n=110) were evaluated for the ocular surface changes. Results: The frequency of pterygium in Ribeirão Preto was 21% (15.7% among women and 32.1% among men; p=0.0002). In Cássia dos Coqueiros, the corresponding frequency was 19.4% (17.3% among women and 25.5% among men; p=0.28). The mean age of the affected individuals was higher than that of the unaffected ones (65.6 ± 10.5 years vs. 61.2 ± 12.0 years, p=0.02). A positive correlation was noted between pterygium and any prior radiotherapy and chemotherapy (p<0.0001, for both). A higher score on corneal fluorescein and conjunctival lissamine green staining was asso ciated with pterygium (p=0.0003 and 0.0001, respectively). Conclusion: We noted a high frequency of pterygium in two Brazilian adult populations, mainly among the men and elderly. Ocular surface damage and a previous history of radiotherapy and/or chemotherapy were found to be associated with pterygium.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Wavelength dispersion X-ray fluorescence spectrometry (WDXRF) is a non-destructive technique and therefore attractive for gunshot residues (GSR) analysis. It is well known for determination of inorganic constituents of samples. However, X-ray scattering region spectral data is not commonly used, although it may provide information about organic constituents and their interactions. This work employed X-ray scattering region and inorganic elements spectral data for a better characterization of GSR. Swabs containing residues collected from the hand of people who fired (shooters group) and also from the hands of people which did not fire (control group) with guns were analyzed directly by the WDXRF. Brake pad and people who perform activities that favor the accumulation of characteristic elements of GSR on their hands were chosen to compose the control group. Principal components analysis (PCA) discriminated the GSR according to the firearm/cartridge used. However, similar GSR clustering did not occur without X-ray scatter data, showing the importance of X-ray scattering spectrum for GSR evaluation. The k-nearest neighbors (k-NN) method correctly classified all samples from shooters and control groups, employing from 1 to 5 nearest neighbors. No anomalous behavior was detected by PCA and hierarchical cluster analysis (HCA).

RESUMO Analisamos a presença de Leptospira spp. em tecido hepático e renal de marsupiais e roedores silvestres capturados em uma floresta periurbana no leste da Amazônia brasileira. Foram examinados 25 indivíduos de quatro espécies de marsupial e sete de roedor quanto à presença do gene 16S rRNA de Leptospira no DNA extraído de 47 amostras de tecido hepático e renal usando PCR. Detectamos amostras positivas em 12% (3/25) dos indivíduos, em fragmentos renais de duas espécies de marsupial (Didelphis marsupialis e Marmosops pinheiroi) e em um fragmento hepático de uma espécie de roedor (Echimys chrysurus). Estes são os primeiros registros de Leptospira spp. em M. pinheiroi e E. chrysurus, e é o primeiro levantamento molecular de marsupiais e roedores na Amazônia brasileira.
ABSTRACT We analyzed the presence of Leptospira spp. in liver and kidney tissue of wild marsupials and rodents trapped in a periurban forest in the eastern Brazilian Amazon. We examined 25 individuals of four marsupial and seven rodent species for the presence of the 16S rRNA gene of Leptospira in the DNA extracted from 47 liver and kidney tissue samples using PCR. We detected positive samples in 12% (3/25) of the individuals, in kidney fragments of two marsupial species (Didelphis marsupialis and Marmosops pinheiroi) and in a liver fragment of one rodent species (Echimys chrysurus). These are the first records of Leptospira spp. in M. pinheiroi and E. chrysurus and it is the first molecular survey of marsupials and rodents in the Brazilian Amazon.

Resumo O objetivo do presente estudo foi comparar o comportamento tático e técnico de jogadores iniciantes no voleibol no side-out de acordo com quatro situações distintas de jogos reduzidos com diferentes relações área/jogador: 4,5 m2 (3,0 m x 3,0 m), 8,0 m2 (4,0 m x 4,0 m), 10,58 m2 (4,6 m x 4,6 m) e 13,52 m2 (5,2 m x 5,2 m). Foram analisados 76 jogos disputados por 16 jogadores com média de idade de 12,2 ± 0,5 anos e experiência de 1,2 ± 0,8 anos de prática. O comportamento tático e técnico foi analisado por meio de uma adaptação do Game Performance Analysis Instrument. A análise foi realizada para a recepção, o levantamento e o ataque de acordo com os componentes relacionados ao ajuste técnico, eficiência técnica e a tomada de decisão. Os resultados mostraram que os jogadores apresentaram índices táticos e técnicos superiores nas habilidades de recepção e levantamento nas situações de 4,6 m x 4,6 m e 5,2 m x 5,2 m. Além disso, na situação de 3,0 m x 3,0 m foram encontrados índices técnicos mais elevados para o ataque. Não foram encontradas diferenças no comportamento tático do ataque para as situações analisadas. Concluímos que as restrições ambientais, impostas pelos pequenos jogos, implicam em comportamentos táticos e técnicos distintos. Assim, a estratégia pedagógica de utilização de pequenos jogos para o ensino do voleibol para jogadores iniciantes deve estar associada aos objetivos pretendidos para cada uma das habilidades básicas do voleibol.
Abstract The aim of the present study was to compare the tactical and technical behavior of beginner players in volleyball side-out between four distinct situations of reduced games with different area/player ratios: 4.5 m2 (3.0 m x 3.0 m), 8.0 m2 (4.0 m x 4.0 m), 10.58 m2 (4.6 m x 4.6 m), and 13.52 m2 (5.2 m x 5.2 m). We analyzed 76 games played by 16 players with a mean age of 12.2 ± 0.5 years and experience of 1.2 ± 0.8 years of practice. Tactical and technical behavior was analyzed using an adaptation of the Game Performance Analysis Instrument (GPAI). The analysis was conducted for passing, setting and attacking according to the components related to technical adjustment, technical efficiency and decision-making. The results showed that players presented higher technical and tactical indexes in the passing and setting skills in 4.6 m x 4.6 m and 5.2 m x 5.2 m situations. Also, in the 3.0 m x 3.0 m situation higher technical indexes were found for the attacking. No differences were found in the tactical behavior of attacking for tested situations. We conclude that the environmental constraints imposed by the small-sided games imply distinct technical and technical behaviors. Thus, the pedagogical strategy of using small-sided games for the teaching of volleyball for beginner players must be associated with the intended objectives for each basic volleyball skills.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

RESUMO Variações nas condições ambientais, como a disponibilidade luminosa, podem afetar a eficiência de controle dos herbicidas por alterar características biológicas das plantas relacionadas à sensibilidade aos herbicidas. Desse modo, objetivou-se avaliar a influência do cultivo em ambientes com diferentes disponibilidades luminosas e da aplicação isolada ou em mistura de glifosato e carfentrazone-etil, sobre a morfofisiologia e o controle de M. atropurpureum. O delineamento adotado foi o de blocos casualizados, em esquema de parcelas subdivididas com 5 repetições. As parcelas foram constituídas de três níveis de sombreamento (pleno sol, 50 e 70% de sombra) e as subparcelas de dois herbicidas (glifosato e carfentrazone-etil) combinados ou não. As doses de glifosato + carfentrazone-etil utilizadas foram 0+40 g ha-1 de carfentrazone-etil, 1,440+0 g ha-1 de glifosato, 1,080+30 g ha-1 de glifosato + carfentrazone-etil e mais um tratamento sem aplicação de herbicida. Plantas de M. atropurpureum cultivadas sob sombreamento apresentaram maior sensibilidade aos herbicidas, maior área dos folíolos e menor taxa fotossintética em comparação ao seu cultivo em ambiente a pleno sol. Em sombreamento, tanto os herbicidas aplicados isoladamente quanto a mistura são efetivos no controle do M. atropurpureum, o que indica maior susceptibilidade dessa espécie aos herbicidas em ambientes com restrição luminosa. A espécie é tolerante aos dois herbicidas testados, isolados ou em mistura, quando crescida a pleno sol.
ABSTRACT Variations in environmental conditions, such as the availability of light, can affect the efficacy of herbicides because they alter the biological characteristics of plants including those that are related to the plant’s sensitivity to herbicides. Therefore, the objective of the present study was to assess the influence of environments with different light availabilities, and of the application of glyphosate and carfentrazone-ethyl (separately or in combination) on the morphophysiology and control of Macroptilium atropurpureum. An experimental design of randomized blocks with five replicates was used, with treatments arranged in a split-plot design. The plots were composed of three levels of shading (full sunlight, 50% shading, and 70% shading); within each plot there were sub-plots, one for each of the two herbicides (glyphosate and carfentrazone-ethyl), used either separately or in combination. The doses of glyphosate + carfentrazone-ethyl applied in the treatments were 0+40 g ha-1 of carfentrazone-ethyl, 1.440+0 g ha-1 of glyphosate, 1.080+30 g ha-1 of glyphosate + carfentrazone-ethyl, and there was an additional treatment that did not include the application of an herbicide. Plants of M. atropurpureum cultivated under shading exhibited higher sensitivity to the herbicides, greater leaflet area, and lower photosynthetic rates than plants cultivated under full sunlight conditions. Under shading, both herbicides applied separately or in combination were effective in controlling M. atropurpureum, which indicates a greater susceptibility of this species to these herbicides in environments with light restriction. The species was tolerant to the two tested herbicides, used either alone or in combination, when grown under full sunlight conditions.
