Abstract Introduction: Malignant diseases of the biliopancreatic junction with failed ERCP due to tumor size or infiltration which does not allow cannulation of the sphincter of Oddi, can be resolved by means of a biliodigestive derivation guided by endoscopic ultrasound (DBD-EUS). In 2001, Dr. Giovannini reported the first procedures in which he placed plastic prostheses. Several meta-analyzes reported a technical and clinical success rate between 90-94%, superior to the percutaneous procedure, but it is operator dependent. The derivations that can be performed are: Coleodeco-duodenum or antral, hepatic-gastro, Rendezvous approach, cholecysto-antral, cholecysto-duodenum. The procedure is palliative in neoplastic diseases; It can be useful in benign conditions such as: hepatojejunal anastomosis stenosis, in Roux-en-Y surgery, Billroth II, Whipple, percutaneous drainage failure and patients with high surgical risk. Complications occur in 17% and are; perforation, hemorrhage, stent obstruction, cholangitis and biliary leak. Objective: Evaluate technical and clinical success rates and complications of the biliodigestive derivation guided by EUS in the endoscopy service of the “CMN 20 de Noviembre” ISSSTE. Material and methods: Retrospective, longitudinal study. All patients over 18 years of age with a diagnosis of malignant biliary obstruction and failed ERCP were included in those who underwent EUS-guided biliodigestive derivation. In the period between April 2017 and May 2019. Patient background, laboratory and image studies were evaluated. Descriptive statistics were used to obtain the technical and clinical success rates and to calculate the rate of complications. Results: Eleven patients were included, 7 were women, median age was 64 years. Technical success was achieved in 100% of the procedures and clinical success in 81%. Complications presented in only one patient (9%) due to septic shock secondary to cholangitis. Conclusions: Both the technical and clinical aspects are comparable with reports from the medical literature, the usefulness of this procedure was demonstrated and we concluded that this method is an effective therapeutic measure which is operator dependent.
Resumen Introducción: Los padecimientos malignos de la encrucijada biliopancreática con CPRE fallida debido a infiltración tumoral o por el tamaño que no permite la canulación del esfínter de Oddi, puede resolverse mediante una derivación biliodigestiva guiada por ultrasonido endoscópico (DBD-USE). Desde 2001 el Dr. Giovannini reportó los primeros procedimientos en los que colocó prótesis plásticas. En varios meta-análisis se reportó que hay éxito técnico y clínico de 90 – 94%, superior al procedimiento percutáneo, pero es operador dependiente. Las derivaciones que se pueden realizar son: Colédoco-duodeno o antral, hepático-gastro, abordaje de Rendezvous, colecisto-antral, colecisto-duodeno. Este procedimiento es paliativo en padecimientos neoplásicos; puede ser útil en padecimientos benignos como: estenosis de anastomosis hepato-yeyunal, en cirugía con Y de Roux, Billroth II, Whipple, falla de drenaje percutáneo, alto riesgo quirúrgico. Las complicaciones se presentan hasta el 17% y son: perforación, hemorragia, obstrucción del Stent, colangitis y fuga biliar. Objetivo: Evaluar la tasa de éxito técnico, clínico y complicaciones de la derivación biliodigestiva guiada por USE en el servicio de endoscopia del Centro Médico Nacional “20 de Noviembre” ISSSTE. Material y métodos: Estudio retrospectivo, longitudinal. Se incluyeron todos los pacientes mayores de 18 años con diagnóstico de obstrucción biliar maligna y CPRE fallida en los que se realizó derivación biliodigestiva guiada por USE. En el periodo comprendido entre abril de 2017 y mayo 2019. Se evaluaron los antecedentes de los pacientes, estudios de laboratorio e imagen. Se utilizó estadística descriptiva y se calculó tasa de éxito técnico, clínico y complicaciones. Resultados: Se incluyeron 11 pacientes, 7 fueron mujeres, mediana de edad de 64 años. Se tuvo éxito técnico en el 100% de los procedimientos y clínico en el 81%, complicaciones en una paciente (9%) finada por choque séptico secundario a colangitis. Conclusiones: Tanto el aspecto técnico y clínico es comparable con reportes de la literatura médica, se demuestra la utilidad de este procedimiento y se reconoce como una medida terapéutica eficaz dependiente de la experiencia del endoscopista.
