RESUMO Objetivos: Avaliar a percepção do risco de exposição da Doença de Coronavírus 2019 (COVID-19), conhecimento sobre medidas de proteção pessoal entre os profissionais de oftalmologia latino-americanos e priorização de pacientes com Covid-19. Métodos: Pesquisa anônima voluntária autoadministrada (formulários do Google Drive) distribuída por mensagem de texto para profissionais de oftalmologia em 1º a 5 de maio de 2020. Resultados: Trezentos e setenta e um profissionais completaram a pesquisa (taxa de resposta de 45%), composta por 118 residentes (27,6%), 111 oftalmologistas (40,5%) e 142 subespecialistas (32,8%). 106 profissionais (32,6%) sentiram-se em alto risco de adquirir o COVID-19 no trabalho. 273 (69,1%) acreditavam que as diretrizes atuais não são suficientes para identificar os pacientes com COVID-19. 265 (59,5%) não tinham treinamento para usar os equipamentos de proteção individual (EPI) e, mesmo com seu uso correto, 341 (91,5%) ainda se sentiram em risco de adquirir COVID-19. 80% consideraram que a equipe de trabalho não tem conhecimento de protocolos nacionais para o atendimento aos pacientes com COVID-19. Apenas 9 dos profissionais (2%) consideraram mudar a profissão para minimizar o risco de contágio por COVID-19. Conclusão: Esta pesquisa mostra a escassez de pessoal e treinamento específico que os praticantes de oftalmologia na América Latina enfrentam em sua prática diária. Essas preocupações e ansiedade parecem ser as mesmas em todo o mundo com a pandemia de COVID- 19. É importante reforçar a confiança dos profissionais de oftalmologia nas diretrizes atuais de atendimento ao paciente com COVID-19 e também disponibilizar programas de treinamento sobre o uso de EPI e também itens de EPI disponíveis em todos os momentos para garantir a qualidade do atendimento e a disseminação mínima da doença.

ABSTRACT Purpose: To assess the perception of Latin American ophthalmology practitioners regarding coronavirus disease 2019 (COVID-19) exposure risk, knowledge about personal protection measures, and prioritization of patients. Methods: Self-administered voluntary anonymous survey (Google Drive forms) was distributed via text message to ophthalmology practitioners from May 01 to May 05, 2020. Results: Three hundred seventy-one practitioners (45% response rate) comprising 118 (27.6%) residents, 111 (40.5%) ophthalmologists, and 142 (32.8%) sub-specialists completed the survey. Among them, 106 (32.6%) thought that they were at a high risk of acquiring COVID-19 during the course of work. Furthermore, 273 (69.1%) believed that the current guidelines were insufficient to identify COVID-19 patients. The survey also revealed that 265 (59.5%) were not trained to use personal protective equipment (PPE), and even with its correct use, 341 (91.5%) still felt that they were at risk of acquiring COVID-19. Moreover, 80% of the respondents were of the view that staff members were not knowledgeable about national protocols for attending COVID-19 patients. However, only 9 (2%) considered changing their profession to ameliorate COVID-19 contagion risk. Conclusion: This survey has revealed the issues faced by ophthalmology practitioners in Latin America during their routine practice. These concerns and anxiety about COVID- 19 pandemic seem to be the same worldwide. It is important to reinforce the confidence of ophthalmology practitioners on current guidelines for attending COVID-19 patients. It is also necessary to conduct training programs on PPE usage and ensure that PPE items are available at all times to enhance the quality of care and minimize the spread of the disease.

Abstract Soil salinity is one of the most devastating problems which reduces crop production and increases desertification. New approaches to overcome the negative effect of salinity on plants include the use of plant biostimulants, such as Xyloglucan oligosaccharides (XGOs) derived from the breakdown of xyloglucans from plant cell walls. The present study aimed at verifying the influence of exogenous XGOs derived from Tamarindus indica L. cell walls, on Arabidopsis thaliana’s tolerance to salt stress by understanding the gene expression, enzymatic and metabolic changes resulting from its application. A. thaliana plants were grown in liquid media and after 15 days they were treated by a salt shock with 100 mM of sodium chloride, with or without XGOs at 0.1 mg L-1. Gene expression of four oxidative stress markers as well as catalase and peroxidase activities and content of glutathione, total carbonyl, polyphenolics and chlorophyll were quantified. Bioinformatic models were used to obtain the co-expression network of the four oxidative stress response gene markers from microarray data of Arabidopsis under salt stress. Results showed that in saline conditions, XGOs dramatically increased catalase gene expression and enzymatic activity, peroxidase activity, and chlorophyll a/b ratio, while reducing protein oxidation and total polyphenols. Thus, XGOs may act to counteract negative effects of oxidative stress under saline conditions.

OBJETIVO: presentar el proceso y los resultados de una investigación- acción en el ámbito de la educación, en el programa de crecimiento y desarrollo de una institución prestadora de servicios de salud de la ciudad de Medellín. METODOLOGIA: siguiendo los planteamientos de Stenhouse sobre un educador-investigador, se llevó a cabo un proceso reflexivo donde los miembros de los equipos del programa, investigaron sus prácticas pedagógicas y las analizaron a la luz de su propia experiencia y de la teoría proveniente de investigaciones realizadas en su institución, y de la literatura científica. RESULTADOS: mediante la reflexión y la investigación de su práctica pedagógica los educadores lograron concebir el programa desde una perspectiva más amplia: la crianza. Así mismo consiguieron situarse de manera diferente hacia la educación y el educando y generar propuestas alternativas. DISCUSION: la experiencia investigativa muestra la dificultad para realizar cambios en las perspectivas pedagógicas en el ámbito de de la educación para salud, generalmente sustentadas en modelos tradicionales o conductistas. Teniendo en cuenta los resultados obtenidos, se presenta esta propuesta investigativa como una vía para avanzar en el fortalecimiento de la dimensión educativa en el ámbito de salud desde una perspectiva alternativa.

OBJECTIVE: to present the process and results of an action research in the educational dimension, the growth and development program of a health institution in Medellin. METHODOLOGY: following Stenhouse’s ideas about a teacher-researcher, it was developed a reflective process where the program’s teams, researched their pedagogical practices, and analyzed them taking in consideration their own experience as well as theory coming from past research done in their institution, and the scientific literature. RESULTS: through a reflective process and by investigating their pedagogical process, educators were able to design the program from a wider perspective: child rearing. Also they were able to locate themselves in a different way with respect to education and to parents generating alternative proposal for education DISCUSSION: research experience shows the difficulty of making changes in pedagogical perspectives in health education, usually based on traditional or behavioral models. According to the results, this research proposal is presented as a way to forward in the strengthening of a educational dimension in health field from an alternative perspective.

OBJECTIVES: To identify the principal sociodemographic, clinical, and behavioral characteristics of patients participating in diabetic care programs in the city of Medellín, Colombia, and to evaluate progress toward the goal of metabolic control and diabetic care standards. METHODS: A cross-sectional, descriptive study was done, analyzing the clinical records of patients who had been participating for at least 6 months in the diabetic care programs of nine health care centers in Medellín. The study period ran from January 2001 to December 2003. The following data were collected: sociodemographic (age and sex), clinical (time since disease onset, diabetes type and treatment, and concurrent illnesses and their treatment), lifestyle habits (exercise routine, smoking, and sugar and fat consumption), chronic complications, and laboratory test results from the prior year. Questionable or missing data were categorized as unavailable. RESULTS: Of the 3 583 clinical histories evaluated, we were able to confirm the form of diabetes for 3 554 patients. Of those 3 554, 95.1% had type 2 diabetes. Overall, 56.9% (95% confidence interval (95% CI): 55.2% to 58.6%) of the patients exercised regularly, 15.1% (95% CI: 13.9% to 16.3%) were current smokers, 17.7% (95% CI: 16.4% to 19.0%) were former smokers, and 67.2% (95% CI: 65.6% to 68.8%) had never smoked. In all, 21.0% (95% CI: 19.6% to 22.5%) of the patients consumed sugar regularly, while 24.8% (95% CI: 23.3% to 26.3%) consumed fats and only 19.5% (95% CI: 17.8% to 21.3%) were self-monitoring their glucose levels at least weekly. Among the type 2 diabetics, 68.8% had high blood pressure, and 98.2% had dyslipidemia. The average body mass index was 28.0 kg/m² (95% CI: 27.8 to 28.2). The average number of medications that the patients were taking to treat type 2 diabetes was 1.3; to treat high blood pressure, 1.9; and to treat dyslipidemia, 0.6. There were no data on low-density lipoprotein (LDL) cholesterol in 44.8% of the clinical records, and no data on high-density lipoprotein (HDL) cholesterol in 16.4% of the records. Among the complications, the most common were ocular ones (31.8%; 95% CI: 30.1% to 33.5%), followed by renal ones (25.9%; 95% CI: 24.4% to 27.5%), and cardiovascular ones (22.5%; 95% CI: 21.1% to 23.8%). Cholesterol control achievement was inadequate for LDL (reached by only 14.2% of the patients), for triglycerides (36.9% of patients), and HDL (47.4% of patients). CONCLUSIONS: Among the study population in Medellín, the three chronic complications with the greatest impact on prognosis and health care costs of the diabetics were nephropathy, retinopathy, and dyslipidemia. The noticeable amount of unavailable data in the clinical records could be greatly reduced by standardizing the clinical record forms and by periodic quality checks of the records themselves.

OBJETIVOS: Identificar las principales características sociodemográficas, clínicas y conductuales de los pacientes que participaban en los programas de atención al diabético en Medellín, Colombia, y evaluar el cumplimiento de las metas de control metabólico y de los estándares de control en el diabético. MÉTODOS: Se realizó un estudio descriptivo transversal mediante el análisis de las historias clínicas de todos los pacientes que asistían a los programas de atención al diabético desde al menos 6 meses en nueve instituciones de salud entre enero de 2001 y diciembre de 2003. Se tomaron datos sociodemográficos (edad y sexo), clínicos (tiempo de evolución de la enfermedad, tipo y tratamiento de la diabetes y enfermedades concurrentes y su tratamiento), sobre los hábitos de vida (práctica de ejercicios físicos, hábito de fumar y consumo de azúcar y grasas) y sobre las complicaciones crónicas y los resultados de las pruebas de laboratorio del último año. Todos los datos confusos o faltantes se registraron como ausentes. RESULTADOS: De las 3 583 historias clínicas evaluadas, se logró conocer el tipo de diabetes que padecían 3 554 pacientes, de ellos 95,1% correspondieron a pacientes con diabetes tipo 2. En general, 56,9% (intervalo de confianza de 95% [IC95%]: 55,2 a 58,6%) de los pacientes realizaban ejercicios periódicos; 15,1% (IC95%: 13,9 a 16,3%) aún fumaba en el momento del corte, 17,7% (IC95%: 16,4 a 19,0%) había dejado de fumar y 67,2% (IC95%: 65,6 a 68,8%) nunca había fumado. En total, 21,0% (IC95%: 19,6 a 22,5%) de los pacientes consumía azúcar regularmente, mientras 24,8% (IC95%: 23,3 a 26,3%) consumía grasas y solo 19,5% (IC95%: 17,8 a 21,3%) realizaba automonitoreo de la glucemia con una frecuencia semanal o mayor. La prevalencia general de hipertensión arterial fue de 68,8% y de 98,2% de dislipidemia en los diabéticos tipo 2. El promedio general del índice de masa corporal fue de 28,0 kg/m² (IC95%: 27,8 a 28,2). El promedio del número de medicamentos utilizados fue de 1,3 para el tratamiento de la diabetes tipo 2, de 1,9 para la hipertensión arterial y de 0,6 para la dislipidemia. No se encontraron los datos de dislipidemia de colesterol de lipoproteínas de baja densidad (LDL) en 44,8% de las historias clínicas y de colesterol de lipoproteínas de alta densidad (HDL) en 16,4%. Las complicaciones oculares fueron las más frecuentes (31,8%; IC95%: 30,1 a 33,5%), seguidas de las afecciones renales (25,9%; IC95%: 24,4 a 27,5%) y las cardiovasculares (22,5%; IC95%: 21,1 a 23,8%). El cumplimiento de las metas de control del colesterol de LDL, los triglicéridos y el colesterol de HDL fue insuficiente (14,2%, 36,9% y 47,4%, respectivamente). CONCLUSIONES: En la población estudiada, las tres complicaciones crónicas que más influyen en el pronóstico y el costo de salud en los diabéticos fueron la nefropatía, la retinopatía y la dislipidemia. El gran subregistro de datos encontrado en las historias clínicas se puede reducir si se toman medidas para estandarizar las historias clínicas y se realizan evaluaciones periódicas de la calidad de las mismas.

Realizou-se estudo comparativo da freqüência e etiologia das mortes súbitas ocorridas no Município de Uberaba, Minas Gerais, em maiores de 15 anos de idade, nos anos de 1980 e de 1990, com o objetivo fundamental de analisar a freqüência atual do óbito súbito em nossa região e o eventual papel de medidas profiláticas e terapêuticas na morte súbita por doença de Chagas. Dos 1226 óbitos de 1980, 54 (4,4%) foram súbitos e destes, 13 (24,1%) etiologia atribuída à doença de Chagas. Em 1990foram pesquisados 1740 óbitos, dos quais 44 (2,5%) foram súbitos; destes, somente 3 (6,8%) devidos â tripanosomose cruzi. Os resultados indicam redução significativa na freqüência tanto da morte súbita em geral, como daquela devida à doença de Chagas, no ano de 1990, em relação ao de 1980, discutindo-se suas prováveis razões.

A comparison was made between the years 1980 and 1990for the frequency and causes of sudden death occurring in the urban and rural areas of the city of Uberaba in individuals older than 15 years. It aims mainly to analyse the current frequency of sudden death in that region and to evaluate the impact, it any, of prophylaxis and therapy on sudden death due to Chagas' disease. For the 1226 deaths cases studied from our 1980, 54 (4.4%) were sudden ones; out of these, 13 (24.1%) were supposedly due to Chagas' disease. For the 1740 death cases studied form our 1990 series, 44 (2.5%) were sudden ones; out of these, only 3 (6.8%) were considered to be due to Chagas' disease. The results indicate a significant decrease in the frequency both for sudden death in general and for sudden death due to Chagas' disease when the year 1990 is compared with 1980. Probable explanations for the findings are discussed.
