Resumo A pesquisa teve como objetivo identificar como acontece o Apoio Matricial realizado entre o Centro de Atenção Psicossocial (CAPS) e as equipes de Saúde da Família. Ocorreu a partir de uma revisão narrativa da literatura feita através de uma busca utilizando como descritores “apoio matricial AND saúde mental”, nas bases da SciELO e PubMed, entre 2016 e 2020, com 12 estudos selecionados. Após a leitura do conjunto do material, dois eixos temáticos foram construídos: apoio CAPS-ESF na prática e seus alcances e elementos facilitadores e desafiadores. O Apoio Matricial é ferramenta essencial à Reforma Psiquiátrica; recurso de fortalecimento entre os serviços de saúde mental especializada e Atenção Básica. Os desafios envolvem o campo das relações entre os sujeitos implicados nos serviços de saúde mental, incluindo a gestão, nas limitações do próprio trabalho em equipe, e nas fragilidades estruturais da rede de saúde mental. Por fim, os estudos convergem em destacar que o cuidado compartilhado entre apoiadores só é possível por meio de encontros e desencontros entre as equipes, sendo trabalho complexo por seu caráter de construção.

Abstract The research aimed to identify how the Matrix Support carried out between the Psychosocial Care Center (CAPS) and the Family Health teams takes place. It occurred from a narrative review of the literature carried out through a search using “matrix support AND mental health” as descriptors, in the SciELO and PubMed databases, between 2016 and 2020, with 12 studies selected. After reading the set of material, two thematic axes were constructed: CAPS-ESF support in practice and its scope and facilitating and challenging elements. Matrix Support is an essential tool for Psychiatric Reform; strengthening resource between specialized mental health services and Primary Care. The challenges involve the field of relationships between subjects involved in mental health services, including management, the limitations of teamwork itself, and the structural weaknesses of the mental health network. Finally, the studies converge in highlighting that shared care between supporters is only possible through meetings and disagreements between teams, being a complex work due to its construction nature.

ABSTRACT Overcoming malnutrition in all its forms, which is also manifested in the obesity epidemic, is one of the priorities of the 2030 Agenda and the Sustainable Development Goals. This article presents reflections on food insecurity in Brazil, based on topics such as population growth, climate change, biodiversity and traditional and regenerative food systems; it also addresses the role of social initiatives to guarantee the Human Right to Adequate Food. The Nutrition and Poverty Research Group, Planetary Health - Brazil Group, and the Center for Artificial Intelligence (C4AI) - AgriBio Axis, assume that the organized and systemic resumption of the initiatives mentioned out in this position paper can contribute to the articulation between research, public policies and services, to combat all forms of malnutrition, as a complex and priority problem to ensure social welfare.

RESUMO O combate à má alimentação em todas as suas formas, que também se expressa na epidemia da obesidade, é uma das prioridades da Agenda 2030 e dos Objetivos de Desenvolvimento Sustentável. Este artigo traz elementos sobre insegurança alimentar no Brasil, a partir de temas relativos ao crescimento populacional, mudanças climáticas, biodiversidade e sistemas alimentares tradicionais e regenerativos; trata também do papel dos movimentos sociais para a garantia do Direito Humano à Alimentação Adequada. O Grupo de Pesquisa em Nutrição e Pobreza, o Saúde Planetária Brasil, e o Centro de Inteligência Artificial (C4AI) - Eixo AgriBio, entendem que a retomada organizada e sistêmica das frentes apontadas neste artigo de posicionamento, pode constituir um caminho para a articulação entre pesquisa, serviço e políticas públicas na direção do combate à fome e às demais manifestações da má alimentação, enquanto problema complexo prioritário para a garantia do bem-estar social.

Abstract This case report describes a favorable healing outcome of the immediate management of an oblique tooth root fracture in the middle third concomitant with coronal fragment extrusion in a permanent maxillary left central incisor of a healthy 14-year-old female patient. The clinical exam showed incisal irregularity, local bleeding, increased mobility, and axial displacement of the maxillary left central incisor. The coronal fragment was repositioned and stabilized with a flexible splint for four weeks. The parents and the patient received medication prescriptions and instructions. Clinical and radiographic conditions remained satisfactory at the 4-week, 6-month, and 12-month follow-ups. Thermal and percussion tests confirmed pulp vitality after 12 months. Hard tissue formation between segments was observed in the radiographic examination and cone-beam computed tomography. In conclusion, the immediate conservative therapeutic showed satisfactory clinical and radiographic results.

Resumo Este relato de caso descreve resultados de cicatrização favorável para o tratamento imediato de fratura dentária radicular oblíqua no terço médio concomitante à extrusão do fragmento coronal de um incisivo central superior esquerdo permanente de uma paciente saudável de 14 anos de idade. O exame clínico demonstrou irregularidade incisal, sangramento local, aumento da mobilidade e deslocamento axial do incisivo central superior esquerdo. O fragmento coronal extruído foi reposicionado e estabilizado com contenção flexível por quatro semanas. Prescrições de medicamentos e instruções foram dadas aos pais e ao paciente. As condições clínicas e radiográficas permaneceram satisfatórias nos acompanhamentos de 4 semanas, 6 meses e 12 meses. A vitalidade pulpar foi confirmada por testes térmicos e de percussão após 12 meses. A formação de tecido duro entre os segmentos fraturados foi observada no exame radiográfico e na tomografia computadorizada de feixe cónico. Em conclusão, a terapêutica conservadora imediata apresentou resultados clínicos e radiográficos satisfatórios.

ABSTRACT Objective: Intrauterine environment can induce fetal metabolic programming that predisposes to adiposity-related chronic diseases in its lifespan. We examined the associations of parental nutritional status and gestational weight gain with offspring body composition in early adulthood. Materials and methods: This is cross-sectional analysis of female participants of the NutriHS who were submitted to questionnaires, clinical examinations and body composition assessed by DXA. Association of pre-conception parental BMI and maternal gestational weight gain (exposures) with body composition measurements (outcomes) were analyzed using multiple linear models adjusted for Directed Acyclic Graphs-based covariables (maternal and paternal educational level, maternal age, and tobacco, alcohol and/or drugs use). The sample included 124 women (median 28 (24-31) years) with a mean BMI of 25.4 ± 4.7 kg/m2. Results: No association between previous paternal BMI and offspring’s body composition was detected. In the fully adjusted linear regression model, maternal BMI was associated with offspring’s total lean mass (β = 0.66, p = 0.001), appendicular skeletal muscle mass index (ASMI) (β = 0.11, p = 0.003) and fat mass index (FMI) (β = 0.03, p = 0.039). Gestational weight gain was associated with increased offspring’s BMI (OR 1.12 [95% CI 1.02-1.20], p = 0.01). The linear regression model adjusted for maternal age and maternal and paternal education levels showed associations of gestational weight gain with offspring’s ASMI (β = 0.42, p = 0.046), FMI (β = 0.22, p = 0.005) and android-to-gynoid fat ratio (β = 0.09, p = 0.035). Conclusion: Our findings suggest that preconception maternal BMI could influence lean mass and general adiposity of young adult female offspring and that gestational weight gain could be useful for predicting centrally distributed adiposity.

Between Care and Custody: How Prison Officers Perceive Their Work in Minas Gerais reveals how prison officers in Minas Gerais perceive their work, whether it is focused on guaranteeing order or on care-a word traditionally associated with resocialization activities in prison. We analyzed whether the workers’ sex has any influence on how they perceive their work, through the analysis of 1,525 online questionnaires self-administered between 2014 and 2015, and semi-structured interviews carried out with 10 women and 13 men working in prisons between 2016 and 2018. In their perception, the maintenance of order and security is the main objective of the profession. The similarities of the discourses of women and men indicate the adoption of masculinity values, such as the use of force and brutality, seen as essential to maintain order in the prison.

Este artigo desvela como agentes prisionais em Minas Gerais percebem seu trabalho, se voltado à garantia da ordem ou ao cuidado - palavra tradicionalmente associada às atividades de ressocialização no cárcere. Para tanto, analisamos se o sexo do profissional influencia essa percepção, por meio de 1.525 questionários on-line autoaplicados entre 2014 e 2015, além de entrevistas semiestruturadas com dez mulheres e 13 homens em atividade de 2016 a 2018. Na percepção dessas pessoas, a manutenção da ordem e da segurança são os principais objetivos da profissão. As similaridades dos discursos de mulheres e homens indicam a adoção de valores atribuídos ao masculino, como uso da força e brutalidade, vistos como essenciais para manter a ordem no cárcere.

ABSTRACT BACKGROUND AND OBJECTIVES: Chronic obstructive pulmonary disease (COPD) is characterized by airflow obstruction, although it compromises the lungs, it also produces significant systemic consequences. The objective of this study was to analyze the relationship between pro-inflammatory cytokines and nociceptive threshold in rats with porcine pancreatic elastase-induced COPD. METHODS 144 animals were randomly distributed into 3 different models: nociceptive phasic pain at tail-flick test (TF), inflammatory pain in the Freund’s complete adjuvant model (CFA) and neuropathic pain in the sciatic nerve constriction model (CCI). 21 days after tracheal instillation of elastase the COPD was established, and the nociceptive threshold was evaluated at different times. RESULTS: The animals with COPD in TF had a shorter latency time and higher levels of IL-1b, IL-6 and TNF-alpha cytokines. In the CFA and CCI model, the animals with COPD showed an increase in the mechanical hyperalgesia and the levels of IL-1b, IL-6 and TNF-alpha were greater in plasma up to 24 hours. CONCLUSION: Animals with COPD have higher levels of pro-inflammatory cytokines and reduced nociceptive thresholds, suggesting a relationship between COPD and increased nociception.

RESUMO JUSTIFICATIVA E OBJETIVOS: A doença pulmonar obstrutiva crônica (DPOC) caracteriza-se pela obstrução do fluxo aéreo e, embora comprometa os pulmões, produz importantes consequências sistêmicas. O objetivo deste estudo foi analisar a relação entre citocinas pró-inflamatórias e limiar nociceptivo em ratos com DPOC induzida por elastase pancreática suína. MÉTODOS: Cento e quarenta e quatro animais foram distribuídos aleatoriamente em três modelos diferentes: dor fásica nociceptiva no teste de retirada de cauda (TF), dor inflamatória no modelo de adjuvante completo de Freund (CFA) e dor neuropática no modelo de constrição do nervo ciático (CCI). Vinte e um dias após a instilação traqueal de elastase a DPOC foi estabelecida e o limiar nociceptivo foi avaliado em diferentes períodos. RESULTADOS: Os animais com DPOC apresentaram menor tempo de latência de retirada e maiores níveis das citocinas IL-1β, IL-6 e TNF-alfa no T F. No modelo CFA e CCI, os animais com DPOC apresentaram aumento da hiperalgesia mecânica e os níveis de IL-1β, IL-6 e TNF-alfa foram maiores no plasma até 24 horas. CONCLUSÃO: Animais com DPOC apresentam níveis mais elevados de citocinas pró-inflamatórias e limiares nociceptivos reduzidos, sugerindo uma relação entre DPOC e aumento da nocicepção.

RESUMO Objetivo: Identificar as consequências do uso excessivo da tecnologia na infância. Fontes de dados: Foi realizada uma revisão sistemática nas bases de dados eletrônicas PubMed (National Library of Medicine — National Institutes of Health) e Biblioteca Virtual em Saúde (BVS) com artigos publicados de 2015 a 2020, em inglês, português e espanhol, utilizando os termos internet, child e growth and development. Síntese dos dados: Foram localizados 554 artigos, resultando em oito artigos incluídos nesta pesquisa. Os estudos foram avaliados quanto à sua qualidade metodológica pelos critérios Strengthening the Reporting of Observational Studies in Epidemiology (Strobe) e Consolidated Standards of Reporting Trials (Consort) e receberam pontuações que variaram de 17 a 22 pontos. Os artigos evidenciaram que há fatores positivos e negativos associados ao uso de tecnologias na infância, embora a maioria dos textos ressalte seu aspecto prejudicial. O uso excessivo de internet, jogos e exposição à televisão ocasionaram alterações intelectuais e da saúde mental, porém também possibilitaram o desenvolvimento psicossocial. Conclusões: Impedir o uso da internet é uma medida utópica, visto que a sociedade faz uso de tecnologias. Considerando que a internet pode trazer benefícios, mas também malefícios, são importantes a otimização do uso e a redução dos riscos, como a participação dos pais e responsáveis como moderadores dessa utilização, além da atualização dos profissionais da saúde para melhor orientá-los.

Abstract Objective: To identify the consequences of technology overuse in childhood. Data source: A systematic review was carried out in the electronic databases PubMed (National Library of Medicine of the National Institutes of Health) and BVS (Virtual Health Library), considering articles published from 2015 to 2020, in English, Portuguese and Spanish using the terms “Internet”, “Child” and “Growth and Development”. Data synthesis: 554 articles were found and 8 were included in the analysis. The studies’ methodological quality was assessed by the Strobe and Consort criteria, being scored from 17 to 22 points. The articles showed positive and negative factors associated with the use of technology in childhood, although most texts emphasize the harmful aspects. Excessive use of internet, games and exposure to television are associated with intellectual deficits and mental health issues, but can also enable psychosocial development. Conclusions: Preventing the use of the internet is a utopic measure ever since society makes use of technologies. The internet is associated with benefits as well as with harms. It is important to optimize the use of internet and reduce risks with the participation of parents and caregivers as moderators, and training of health professionals to better guide them.

Abstract Work organization and process are privileged spaces to recognize the forces that lead to psychological suffering. This study aims to analyze the perceptions of physicians of primary health care, who work in the More Doctors Program, on the relation between psychological suffering and work process during the COVID-19 pandemic. This research was conducted with a case study using qualitative data collection, description, and analysis. An intentional sample was selected in accordance with the theoretical saturation technique. Information was collected by semi-structured interviews, following a script built to meet the objectives of this research. The analysis of interviewees’ discourse articulated categories of analysis drawn from five thematic axes with points of articulation between them, consensuses and contradictions that give meaning to the visions and positions of the actors in health. Results evinced the pandemic as a health and traumatic catastrophe, resizing the work process and contributing to overload, conflicts, fear, feelings of helplessness, and psychological suffering. Numerous shortcomings, infrastructure problems, bureaucracy, political interference, mismatch between training and practice also contributed to this suffering.

Resumo A organização e o processo de trabalho são espaços privilegiados para reconhecer as forças que levam ao sofrimento psíquico. Este artigo pretende analisar as percepções de médicos(as) da atenção primária à saúde, que atuam no Programa Mais Médicos, sobre as relações entre sofrimento psíquico e processo de trabalho, no contexto da pandemia do coronavirus. Esta pesquisa foi realizada por meio de um estudo de caso, utilizando métodos e técnicas qualitativas para a coleta, descrição e análise dos dados. Foi selecionada uma amostra intencional em conformidade com a técnica de saturação teórica. A coleta de informações foi realizada com ajuda de entrevistas semiestruturadas, seguindo um roteiro construído para atender aos objetivos desta pesquisa. A análise do discurso dos entrevistados articulou categorias de análise extraidas de cinco eixos temáticos que têm pontos de articulação entre si, com consensos e contradições que dão sentido às visões e posições dos atores no campo da saúde. Os resultados evidenciaram a pandemia como catástrofe sanitária e traumática, redimensionando o processo de trabalho e contribuindo para a sobrecarga, conflitos, medo, sentimento de desamparo e sofrimento psíquico. Inúmeras carências, problemas de infraestrutura, burocracia, interferências políticas, descompasso entre a formação e a prática também contribuiram para esse sofrimento.

Abstract This article discusses the challenges faced by the state managers of the Brazilian National Health System (SUS) in the context of the COVID-19 pandemic and the consequences for their personal lives. From interviews with state managers, it became evident that the pandemic implied the replanning of health actions and services, the structuring of new hospital services and beds, the use of telemedicine, and the hiring and training of professionals. It also exposed the lack of integrated policies and national coordination, which led to states adopting different strategies. These factors, as well as the politicization of the pandemic, the denial of the disease, and the attempt of the federal government to impose ineffective treatments for COVID-19 were considered among the greatest difficulties experienced by managers. Managing the SUS became a risky exercise, with deleterious repercussions not only for themselves and their management, but also for their families and friends. If, on the one hand, the strenuous context affected the managers’ physical and mental health, who began dealing with overweight, anxiety, and anguish, on the other hand, it was a source of motivation for catalyzing solidarity, empathy, and sharing among political actors.

Resumo Este artigo discute os desafios enfrentados pelos gestores estaduais do Sistema Único de Saúde (SUS) no contexto da pandemia de covid-19 e as consequências produzidas em suas vidas pessoais. A partir de entrevistas com gestores estaduais, tornou-se evidente que a pandemia demandou o replanejamento das ações e serviços de saúde, a estruturação de novos serviços e leitos hospitalares, o uso de telemedicina e a contratação e capacitação de profissionais. Também emergiu a ausência de políticas integradas e de uma coordenação nacional, que levou à adoção de diferentes estratégias por parte dos estados. Estes fatores, assim como a politização da pandemia, a negação da doença e a tentativa do governo federal de impor tratamentos ineficazes para a covid-19 foram consideradas entre as maiores dificuldades vivenciadas pelos gestores. Fazer a gestão do SUS se tornou um exercício de risco, com repercussões deletérias não apenas para si e para a sua gestão, mas também para suas famílias e amigos. Se, por um lado, o contexto extenuante afetou a saúde física e mental dos gestores, que passaram a lidar com excesso de peso, ansiedade e angústia, de outro, foi fonte de motivação por catalisar solidariedade, empatia e o compartilhamento entre os atores políticos.

Abstract In addition to preserving the goods, packaging captures potential buyers' attention, provides information about product benefits, changes consumer beliefs, and increases the chances of a sale. Thus, this review aimed to provide information on the impact of color perception on food packaging by consumers. Using color on packaging results in product differentiation, increasing the advantage over others and building sales loyalty and emotional connections. Besides the product color, the colors of its packaging can play an essential role in the perception of product taste, as people tend to create associations of specific colors with specific products and associations with particular tastes.

ABSTRACT Objective: to identify and analyze the social representations and their dimensions about peripheral venous catheterization and its maintenance for anesthetic-surgical purposes in women undergoing cesarean section. Method: qualitative study of exploratory-descriptive type grounded by the Theory of Social Representations with the participation of 120 women and carried out in a public hospital in the state of Minas Gerais - Brazil, between 2019 and 2020. Evocations collected by interviews with application of the technique of free association of non-hierarchical words with prototypical analysis and by similarity in the EVOC and IRAMUTEQ software. Results: the present social representation addresses feelings in the behavioral dimension of stress, which are related to insecurity and fear of the pain of the needle, the surgical procedure, and the restriction of movements. Conclusion: the research contributes to a reflection on the need to resize nursing care before the difficulties faced by women in the triple experience of stress: cesarean section; venous catheterization; and the difficulty of performing postpartum care.

RESUMEN Objetivo: identificar y analizar las representaciones sociales y sus dimensiones sobre el cateterismo venoso periférico y su manejo con fines anestésico-quirúrgicos en mujeres sometidas a cesárea. Método: estudio cualitativo de tipo exploratorio-descriptivo fundamentado por la Teoría de las Representaciones Sociales con la participación de 120 mujeres y realizado en un hospital público del estado de Minas Gerais - Brasil, entre 2019 y 2020. Evocaciones recogidas mediante entrevistas con aplicación de la técnica de asociación libre de palabras no jerárquicas con análisis prototípico y por similitud en los programas informáticos EVOC e IRAMUTEQ. Resultados: la presente representación social aborda los sentimientos en la dimensión conductual del estrés, que están relacionados con la inseguridad y el miedo al dolor de la aguja, al procedimiento quirúrgico y a la restricción de movimientos. Conclusión: la investigación contribuye a la reflexión sobre la necesidad de redimensionar los cuidados de enfermería ante las dificultades enfrentadas por las mujeres en la triple experiencia de estrés: cesárea; cateterismo venoso; y la dificultad de realizar los cuidados posparto.

RESUMO Objetivo: identificar e analisar as representações sociais e suas dimensões sobre o cateterismo venoso periférico e sua manutenção para fins anestésico-cirúrgicos em mulheres submetidas à cesariana. Método: estudo qualitativo do tipo exploratório-descritivo alicerçado pela Teoria das Representações Sociais com a participação de 120 mulheres e realizada em um hospital público no estado de Minas Gerais - Brasil, entre 2019 e 2020. Evocações coletadas por entrevistas com aplicação da técnica de associação livre de palavras não hierarquizada com análise prototípica e por similitude nos softwares EVOC e IRAMUTEQ. Resultados: a presente representação social aborda sentimentos na dimensão comportamental de estresse, os quais são relacionados à insegurança e medo da dor da agulha, ao procedimento cirúrgico e à restrição de movimentos. Conclusão: a pesquisa contribui para uma reflexão da necessidade de redimensionamento da assistência de enfermagem ante as dificuldades enfrentadas pelas mulheres na tripla vivência de estresses: a cesárea; o cateterismo venoso; e à dificuldade de realizar os cuidados pós-parto.

Abstract Background Cardiovascular complications of COVID-19 are important aspects of the disease’s pathogenesis and prognosis. Evidence on the prognostic role of troponin and myocardial injury in Latin American hospitalized COVID-19 patients is still scarce. Objectives To evaluate myocardial injury as independent predictor of in-hospital mortality and invasive mechanical ventilation support in hospitalized patients, from the Brazilian COVID-19 Registry. Methods This cohort study is a substudy of the Brazilian COVID-19 Registry, conducted in 31 Brazilian hospitals of 17 cities, March-September 2020. Primary outcomes included in-hospital mortality and invasive mechanical ventilation support. Models for the primary outcomes were estimated by Poisson regression with robust variance, with statistical significance of p<0.05. Results Of 2,925 patients (median age of 60 years [48-71], 57.1% men), 27.3% presented myocardial injury. The proportion of patients with comorbidities was higher among patients with cardiac injury (median 2 [1-2] vs. 1 [0-2]). Patients with myocardial injury had higher median levels of brain natriuretic peptide, lactate dehydrogenase, creatine phosphokinase, N-terminal pro-brain natriuretic peptide, and C-reactive protein than patients without myocardial injury. As independent predictors, C-reactive protein and platelet counts were related to the risk of death, and neutrophils and platelet counts were related to the risk of invasive mechanical ventilation support. Patients with high troponin levels presented a higher risk of death (RR 2.03, 95% CI 1.60-2.58) and invasive mechanical ventilation support (RR 1.87, 95% CI 1.57-2.23), when compared to those with normal troponin levels. Conclusion Cardiac injury was an independent predictor of in-hospital mortality and the need for invasive mechanical ventilation support in hospitalized COVID-19 patients.

Resumo Fundamento As complicações cardiovasculares da COVID-19 são aspectos importantes da patogênese e do prognóstico da doença. Evidências do papel prognóstico da troponina e da lesão miocárdica em pacientes hospitalizados com COVID-19 na América Latina são ainda escassos. Objetivos Avaliar a lesão miocárdica como preditor independente de mortalidade hospitalar e suporte ventilatório mecânico em pacientes hospitalizados, do registro brasileiro de COVID-19. Métodos Este estudo coorte é um subestudo do registro brasileiro de COVID-19, conduzido em 31 hospitais brasileiros de 17 cidades, de março a setembro de 2020. Os desfechos primários incluíram mortalidade hospitalar e suporte ventilatório mecânico invasivo. Os modelos para os desfechos primários foram estimados por regressão de Poisson com variância robusta, com significância estatística de p<0,05. Resultados Dos 2925 pacientes [idade mediana de 60 anos (48-71), 57,1%], 27,3% apresentaram lesão miocárdica. A proporção de pacientes com comorbidades foi maior nos pacientes com lesão miocárdica [mediana 2 (1-2) vs. 1 (0-20)]. Os pacientes com lesão miocárdica apresentaram maiores valores medianos de peptídeo natriurético cerebral, lactato desidrogenase, creatina fosfoquinase, N-terminal do pró-peptídeo natriurético tipo B e proteína C reativa em comparação a pacientes sem lesão miocárdica. Como fatores independentes, proteína C reativa e contagem de plaquetas foram relacionados com o risco de morte, e neutrófilos e contagem de plaquetas foram relacionados ao risco de suporte ventilatório mecânico invasivo. Os pacientes com níveis elevados de troponina apresentaram um maior risco de morte (RR 2,03, IC95% 1,60-2,58) e suporte ventilatório mecânico (RR 1,87;IC95% 1,57-2,23), em comparação àqueles com níveis de troponina normais. Conclusão Lesão cardíaca foi um preditor independente de mortalidade hospitalar e necessidade de suporte ventilatório mecânico em pacientes hospitalizados com COVID-19.
