Abstract Background: Patients using immunosuppressive drugs may have unfavorable results after infections. However, there is a lack of information regarding COVID 19 in these patients, especially in patients with rheumatoid arthritis (RA). Therefore, the aim of this study was to evaluate the risk factors associated with COVID 19 hospitalizations in patients with RA. Methods: This multicenter, prospective cohort study is within the ReumaCoV Brazil registry and included 489 patients with RA. In this context, 269 patients who tested positive for COVID 19 were compared to 220 patients who tested negative for COVID 19 (control group). All patient data were collected from the Research Electronic Data Capture database. Results: The participants were predominantly female (90.6%) with a mean age of 53 ±12 years. Of the patients with COVID 19, 54 (20.1%) required hospitalization. After multiple adjustments, the final regression model showed that heart disease (OR =4.61, 95% CI 1.06–20.02. P < 0.001) and current use of glucocorticoids (OR =20.66, 95% CI 3.09–138. P < 0.002) were the risk factors associated with hospitalization. In addition, anosmia was associated with a lower chance of hospitalization (OR =0.26; 95% CI 0.10–0.67, P < 0.005). Conclusion: Our results demonstrated that heart disease and the use of glucocorticoids were associated with a higher number of hospital admissions for COVID 19 in patients with RA. RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

RESUMO Unidades de diálise são ambientes potencialmente propensos à disseminação da Covid-19. Os pacientes não podem suspender o tratamento, e são frequentemente portadores de comorbidades, o que confere maior risco e pior prognóstico. A Sociedade Brasileira de Nefrologia elaborou esse documento de boas práticas, cujas recomendações técnicas tratam de medidas gerais que podem ser implementadas para reduzir o risco de transmissão e evitar a disseminação da doença na unidade.
ABSTRACT Dialysis units are environments potentially prone to the spread of Covid-19. Patients cannot suspend treatment, and they often have comorbidities, which assigns them a higher risk and worse prognosis. The Brazilian Society of Nephrology prepared this document of good practices, whose technical recommendations deal with general measures that can be implemented to reduce the risk of transmission and prevent the spread of the disease in the unit.

RESUMO A pandemia da Covid-19 trouxe desafios ao sistema de saúde em diversas esferas: diagnóstico, tratamento e medidas para evitar a disseminação da doença. Com a maior disponibilização e variedades de testes diagnósticos, torna-se fundamental sua adequada interpretação. Este posicionamento pretende orientar unidades de diálise em relação ao uso de critérios clínicos e testes diagnósticos para a tomada de decisão referente à descontinuação do isolamento de pacientes com suspeita ou confirmação de Covid-19, assim como para o retorno às atividades laborais de colaboradores com suspeita ou confirmação de Covid-19.
ABSTRACT The Covid-19 pandemic brought several challenges to the healthcare system: diagnosis, treatment and measures to prevent the spread of the disease. With the greater availability and variety of diagnostic tests, it is essential to properly interpret them. This paper intends to help dialysis units concerning the use of clinical criteria and diagnostic tests for decision making regarding the discontinuation of isolation of patients with suspected or confirmed Covid-19, as well as the return to work activities for employees with suspected or confirmed Covid-19.

RESUMO Em razão de resultados preliminares promissores, a hidroxicloroquina e a cloroquina têm sido discutidas como opção terapêutica para pacientes com Covid-19. Apesar da ausência de estudos robustos que evidenciem o benefício e justifiquem o uso de uma dessas drogas, a decisão final compete ao médico assistente, devendo ser individualizada e, sempre que possível, compartilhada. A presente nota pretende orientar o ajuste posológico dessas drogas no contexto da disfunção renal.
ABSTRACT Chloroquine and hydroxychloroquine have shown promising preliminary results and have been discussed as therapeutic options for patients with Covid-19. Despite the lack of robust evidence demonstrating the benefits and justifying the use of one of these drugs, the final decision is the responsibility of the attending physician and should be individualized and shared, whenever possible. This position statement recommends dosage adjustment for these drugs in the context of renal impairment.

Human-modified landscape acts as refuge for mammals in Atlantic Forest

Resumo: Paisagens antropicamente modificadas (HMLs) são compostas por fragmentos florestais pequenos, isolados e defaunados, imersos em áreas agrícolas e/ou urbanas. Informações sobre as espécies que habitam essas paisagens são importantes para o direcionamento de estratégias de conservação em escalas local e regional. Uma vez que as HMLs são as paisagens dominantes na Mata Atlântica, o objetivo deste trabalho foi avaliar a diversidade de mamíferos em uma HML do sudeste do Brasil e propor estratégias para sua conservação. Foram coletados dados de mamíferos terrestres (pequenos, médios e grandes) e voadores em três fragmentos florestais (10, 14 e 26 ha) e áreas adjacentes, entre 2003 e 2016, usando métodos complementares: busca ativa, armadilhamento fotográfico, armadilhas de captura e redes de neblina. Adicionalmente, foram utilizados dados de literatura para complementar a lista de espécies. Foram registradas 35 espécies de mamíferos nativos (6 de pequenos, 16 de médios e grandes e 13 de morcegos) e sete espécies exóticas. A assembleia de mamíferos registrada (terrestres e voadores), embora composta por espécies generalistas, apresentou três espécies de médio e grande porte ameaçadas de extinção nacionalmente (Leopardus guttulus, Puma concolor and Puma yagouaroundi) e duas deficientes em dados (Galictis cuja and Histiotus velatus), destacando a importância dessa HML para conservação e manutenção das populações de mamíferos. Embora inserida em uma paisagem extremamente modificada, a área de estudo abriga uma riqueza significativa de mamíferos de médio e grande porte, sendo um importante refúgio para a biodiversidade na região. Entretanto, essa biodiversidade está ameaçada pela baixa qualidade dos habitats, por atropelamentos e por abundantes populações de cães e gatos domésticos. Portanto, enfatizamos a necessidade de estratégias de conservação focadas nos mamíferos de médio e grande porte como grupo "guarda-chuva", o que pode beneficiar as demais espécies na paisagem. Recomendamos ações de conservação visando a restauração biológica, para melhorar a composição estrutural e conectividade dos fragmentos florestais, reduzir o número de atropelamentos e controlar as populações de cães e gatos domésticos, afim de manter e aumentar a diversidade local de mamíferos em longo prazo.
Abstract: Human-modified landscapes (HMLs) are composed by small, isolated and defaunated forest fragments, which are surrounded by agricultural and urban areas. Information on species that thrives in these HMLs is essential to direct conservation strategies in local and regional scales. Since HMLs are dominant in the Atlantic Forest, we aimed to assess the mammalian diversity in a HML in southeastern Brazil and to propose conservation strategies. We collected data of terrestrial (small-, medium- and large-sized) and volant mammals in three small forest fragments (10, 14 and 26 ha) and adjacent areas, between 2003 and 2016, using complementary methods: active search, camera trapping, live-traps, mist nets and occasional records (i.e., roadkills). In addition, we used secondary data to complement our species list. We recorded 35 native mammal species (6 small-sized, 16 medium- and large-sized, and 13 bats) and seven exotic species in the HML. The recorded mammal assemblage (non-volant and volant), although mainly composed of common and generalist species, includes three medium- and large-sized species nationally threatened (Leopardus guttulus, Puma concolor and Puma yagouaroundi) and two data deficient species (Galictis cuja and Histiotus velatus), highlighting the importance of this HML for the maintenance and conservation of mammal populations. Despite highly impacted by anthropogenic disturbances, the study area harbors a significant richness of medium- and large-sized mammals, being an important biodiversity refuge in the region. However, this biodiversity is threatened by the low quality of the habitats, roadkills and abundant populations of domestic cats and dogs. Therefore, we stress the need of conservation strategies focusing on the medium- and large-sized mammals as an umbrella group, which could benefit all biodiversity in the landscape. We recommend actions that promotes biological restoration, aiming to increase structural composition and connectivity of the forest fragments, reducing roadkills and controlling the domestic cats and dogs' populations, in order to maintain and improve the diversity of mammals in long-term.

Endophytic Microbiota Associated with the Root Tips and Leaves of Baccharis dracunculifolia

ABSTRACT Although endophytic bacteria impact the health, and ultimately the fitness, of their hosts, our understanding of the diversity of endophytic species remains limited. Here we report on the endophytic microbiota inhabiting the roots, healthy leaves and leaves attacked by a gall-inducing insect of Baccharis dracunculifolia, a species of major economic relevance in South America, using 16S rRNA gene new generation sequencing. Rhodoplanes and Nitrospira were well represented in the communities of roots and leaves; known to be important for nitrogen cycling. The difference in bacterial diversity between healthy and galled leaves was not pronounced. The leaves seem to harbor specialized bacteria with high tolerances to abiotic stresses such as wide variation in temperature, low humidity, shallow and nutrient-poor soils and high solar irradiation. These findings suggest taxon-specific ecological niches in the leaves and roots, which may be the result of different physicochemical characteristics between these structures. This study provides a basis for further investigations and adds significant new information to the current knowledge of the endophytic bacterial composition in B. dracunculifolia.

CHEMICAL CHARACTERIZATION AND EVALUATION OF ANTIBACTERIAL, ANTIFUNGAL, ANTIMYCOBACTERIAL, AND CYTOTOXIC ACTIVITIES OF Talinum paniculatum

RESUMO Neste estudo foi avaliada a bioatividade de Talinum paniculatum, planta amplamente utilizada na medicina popular. O extrato das folhas (EF) de T. paniculatum foi obtido por percolação com etanol-água e, em seguida, submetido à partição líquido-líquido, obtendo-se as frações hexânica (HX), acetato-etílica (AcOEt), butanólica (BuOH) e aquosa (Aq). A triagem para a atividade antimicrobiana do EF e de suas frações foram avaliadas in vitro através do método de microdiluição em caldo contra treze micro-organismos patogênicos e não-patogênicos e, a atividade antimicobacteriana, foi avaliada através do teste de difusão em ágar. As concentrações citotóxicas (CC90) do EF e das frações HX e AcOEt foram obtidas sobre células da linhagem BHK-21 através do ensaio de redução do MTT. O EF mostrou atividade contra Serratia marcescens e Staphylococcus aureus, com valores de concentração inibitória mínima (CIM) de 250 e 500 µg/mL, respectivamente. Além disso, HX demonstrou excelente atividade contra Micrococcus luteus e Candida albicans com uma CIM de 31,2 µg/mL, em ambos os casos. Contra Escherichia coli, a CIM para AcOEt foi também de 31,2 µg/mL. Por outro lado, as frações BuOH e Aq foram inativas contra todos os micro-organismos testados, assim como o EF contra Mycobacterium tuberculosis e Mycobacterium bovis. Campesterol, estigmasterol e sitosterol foram as estruturas propostas como principais compostos presentes no EF e nas frações HX e AcOEt, evidenciadas através de espectrometria de massas. Portanto, o extrato da folha e as frações HX e AcOEt provenientes de T. paniculatum apresentaram potencial como possíveis fontes de compostos antimicrobianos, HX principalmente, por ter apresentado uma baixa toxicidade sobre células BHK-21 com um bom índice de seletividade (IS = CC90/MIC) de 17,72 contra C. albicans.
SUMMARY In this study, the bioactivity of Talinum paniculatum was evaluated, a plant widely used in folk medicine. The extract from the T. paniculatum leaves (LE) was obtained by percolation with ethanol-water and then subjecting it to liquid-liquid partitions, yielding hexane (HX), ethyl acetate (EtOAc), butanol (BuOH), and aqueous (Aq) fractions. Screening for antimicrobial activity of the LE and its fractions was evaluated in vitro through broth microdilution method, against thirteen pathogenic and non-pathogenic microorganisms, and the antimycobacterial activity was performed through agar diffusion assay. The cytotoxic concentrations (CC90) for LE, HX, and EtOAc were obtained on BHK-21 cells by using MTT reduction assay. The LE showed activity against Serratia marcescens and Staphylococcus aureus, with Minimum Inhibitory Concentration (MIC) values of 250 and 500 µg/mL, respectively. Furthermore, HX demonstrated outstanding activity against Micrococcus luteus and Candida albicans with a MIC of 31.2 µg/mL in both cases. The MIC for EtOAc also was 31.2 µg/mL against Escherichia coli. Conversely, BuOH and Aq were inactive against all tested microorganisms and LE proved inactive against Mycobacterium tuberculosisand Mycobacterium bovisas well. Campesterol, stigmasterol, and sitosterol were the proposed structures as main compounds present in the EF and HX/EtOAc fractions, evidenced by mass spectrometry. Therefore, LE, HX, and EtOAc from T. paniculatumshowed potential as possible sources of antimicrobial compounds, mainly HX, for presenting low toxicity on BHK-21 cells with excellent Selectivity Index (SI = CC90/MIC) of 17.72 against C. albicans.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Controversial predictors of biochemical recurrence after radical prostatectomy: a study from a Latin American (Brazilian) Institution

Purpose To analyze controversial clinicopathologic predictors of biochemical recurrence after surgery: age, race, tumor extent on surgical specimen, tumor extent on needle biopsy, Gleason score 3 + 4 vs 4 + 3, and amount of extent of extraprostatic extension and positive surgical margins. Materials and Methods The needle biopsies and the correspondent surgical specimens were analyzed from 400 patients. Time to recurrence was analyzed with the Kaplan-Meier curves and risk of shorter time to recurrence using Cox univariate and multivariate analysis. Results Except for age, race, maximum percentage of cancer per core, and number of cores with cancer, all other variables studied were significantly predictive of time to biochemical recurrence using the Kaplan-Meier curves. In univariate analysis, except for focal extraprostatic extension, age, race, focal positive surgical margins, and maximum extent and percentage of cancer per core, all other variables were significantly predictive of shorter time to recurrence. On multivariate analysis, diffuse positive surgical margins and preoperative PSA were independent predictors. Conclusions Young patients and non-whites were not significantly associated with time to biochemical recurrence. The time consuming tumor extent evaluation in surgical specimens seems not to add additional information to other well established predictive findings. The higher predictive value of Gleason score 4 + 3 = 7 vs 3 + 4 = 7 discloses the importance of grade 4 as the predominant pattern. Extent and not simply presence or absent of extraprostatic extension should be informed. Most tumor extent evaluations on needle biopsies are predictive of time to biochemical recurrence, however, maximum percentage of cancer in all cores was the strongest predictor.

Os indicadores convencionais de fertilidade de solo, mais comumente utilizados, enfatizam o balanço ácido-base e o conteúdo de elementos químicos, mas não consideram adequadamente as propriedades biológicas do solo, responsáveis por grande parte do seu funcionamento. Os indicadores biológicos, ainda pouco difundidos, levam em consideração a atividade microbiana do solo que é mais sensível à dinâmica do solo. Em Marapanim-PA, e utilizando um desenho experimental inteiramente casualizado, com quatro repetições, avaliou-se o comportamento dos indicadores do solo carbono e nitrogênio da biomassa microbiana, fosfatase ácida, matéria orgânica, carbono orgânico e fósforo disponível, em um solo com cultivo sequencial de mandioca e milho, preparo com corte-e-trituração da capoeira original. Os fatores corte-e-trituração, fosfato natural e adubação verde foram arranjados nos seguintes tratamentos: capoeira; testemunha; fosfato natural (FN); feijão-de-porco [Canavalia ensiformis (L.) DC] (FP); FN + FP; FN + guandu [Cajanus cajan (L.) Millspaugh]. (G); FN + titônia [Tithonia diversifolia (Hemsley) A. Gray] (T); FP + G; FP + T. Em quatro épocas de amostragens, foram tiradas amostras compostas do solo (0 - 10 cm) de cada parcela para análise dos indicadores. Os indicadores estudados foram influenciados pela época de amostragem do solo. Os indicadores fosfatase ácida, nitrogênio da biomassa microbiana e carbono da biomassa microbiana foram capazes de detectar os efeitos de maior número de tratamentos de recuperação do solo testados, enquanto a matéria orgânica do solo foi o menos capaz. Ao longo do tempo, a resposta dos indicadores variou em função dos fatores estudados.
The more commonly used traditional soil fertility indicators emphasize the acid-basic balance and the content of chemical elements, but do not consider adequately the biologic soil properties, largely responsible for soil functioning. The biologic indicators, not yet sufficiently used, take into consideration the microbial activity that is more sensible to soil dynamics. This study was carried out in Marapanim-PA and using a completely randomized plot design, with four replications, to evaluate the behavior of some indicators such as microbial biomass, acid phosphatase, soil organic matter, organic carbon and available phosphorus - in a soil cultivated with cassava and corn, and in which the secondary vegetation was prepared by slash-and-trituration practice. The slash-and-trituration, rock phosphate and green manure factors were arranged in the following treatments: secondary vegetation; control; rock phosphate (RP); wonderbean Canavalia ensiformis (L.) DC (W); RP + W; RP + pigeonpea Cajanus cajan (L.) Millspaugh (P); RP + tithonia Tithonia diversifolia (Hemsley) A. Gray (T); W + P; W + T. In four season of sampling, soil samples were taken from the 0 - 10 cm layer of each parcel to measure the indicator values. The indicators studied were influenced by the time of soil sampling. The acid phosphatase, microbial biomass nitrogen and microbial biomass carbon soil indicators were capable of detecting the effects of a higher number of soil reclamation treatments, while soil organic matter was the least capable. With the passing of time, the response of the indicators varied according to the factors studied.

CONTEXTUALIZAÇÃO: Pacientes com doença pulmonar obstrutiva crônica (DPOC) queixam-se de dispneia nas atividades de vida diária (AVD) com os membros superiores (MMSS). A hiperinsuflação dinâmica (HD) é um dos mecanismos ventilatórios que contribuem para a dispneia. Para minimizar a HD, propõe-se a utilização de sistemas de ventilação não-invasiva (VNI). OBJETIVOS: Verificar se existe HD e dispneia durante a realização de uma AVD com os MMSS com e sem o uso da VNI. MÉTODOS: Participaram 32 pacientes com DPOC de moderada a muito grave, com idades entre 54 a 87 anos (69,4±7,4). Os pacientes elevaram potes com pesos de 0,5 a 5 kg durante 5 minutos, iniciando a elevação a partir da cintura pélvica em direção a uma prateleira localizada acima da cabeça, com e sem o uso da VNI (BiPAP®; IPAP 10 cmH2O; EPAP 4 cmH2O). Foram avaliadas a capacidade inspiratória (CI) e a dispneia (Escala de Borg). A CI foi mensurada antes e após a simulação da AVD. Na análise dos dados foram utilizados o teste t de Student para amostras dependentes e o teste de Wilcoxon. RESULTADOS: Houve redução significativa da CI após a AVD com e sem VNI (p=0,01). A dispneia aumentou após a AVD com e sem a VNI, mas entre ambos os protocolos não houve diferença. CONCLUSÕES: A simulação da AVD com os MMSS resultou em aumento da HD e dispneia. A VNI ofertada com pressões preestabelecidas não foi suficiente para evitar a HD e a dispneia.
BACKGROUND: Patients with chronic obstructive pulmonary disease (COPD) complain of dyspnea on activities of daily living (ADLs) with the upper limbs. Dynamic hyperinflation (DH) is one of the ventilatory mechanisms that may contribute towards dyspnea. To minimize the DH it is indicated the use of noninvasive ventilation (NIV). OBJECTIVES: To verify whether there is DH and dyspnea during the performance of ADL with the upper limbs with and without the use of NIV. METHODS: 32 patients with moderate-to-severe COPD, aged 54 to 87 years (mean 69.4, SD 7.4) were evaluated. The subjects lift up containers weighing between 0.5 and 5.0 kg over a five-minute period, starting from the waist level and putting them onto a shelf located above head height, with and without the use of NIV (BiPAP®; IPAP 10cmH2O; EPAP 4 cmH2O). The inspiratory capacity (IC) and dyspnea (Borg scale) were evaluated on all the patients. The IC was measured before and after simulation of the ADL. In order to analyze the data, Student's t test for dependent samples and the Wilcoxon test were used. RESULTS: There were statistically significant reductions in IC after the ADL with and without NIV (p=0.01). The dyspnea increased after the ADL with and without the NIV, however between both interventional procedures protocols no between-group difference was observed. CONCLUSIONS: The simulation of an ADL with the upper limbs resulted in an increase in DH and dyspnea. The NIV supplied with pre-established pressure was not enough to prevent the DH and dyspnea.
