ABSTRACT This article summarizes the clinical practice guideline (CPG) for the management of acute pancreatitis in the Social Security of Peru (EsSalud), to provide evidence-based clinical recommendations for the management of acute pancreatitis in EsSalud. A guideline development group (GEG) was formed that included medical specialists and methodologists. The GEG formulated 7 clinical questions to be answered by this CPG. Systematic searches of systematic reviews and -when considered relevant-primary studies were carried out in PubMed during 2022. The evidence was selected to answer each of the clinical questions posed. The certainty of the evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work meetings, the GEG used the GRADE methodology to review the evidence and formulate the recommendations, the points of good clinical practice, and the corresponding ow charts. Finally, the CPG was approved with Resolution No. 105-IETSI-ESSALUD-2022. This CPG addressed 7 clinical questions on buid therapy, start of enteral nutrition, analgesia, type of nutrition, antibiotic, and surgical treatment. Based on these questions, 8 recommendations (1 strong and 7 conditional), 13 BPCs, and 1 flowchart were formulated. This article summarizes the methodology and evidence-based conclusions of the CPG for the management of acute pancreatitis in EsSalud.

RESUMEN El presente artículo resume la guía de práctica clínica (GPC) para el manejo de la pancreatitis aguda en el Seguro Social del Perú (EsSalud). Su objetivo es proveer recomendaciones clínicas basadas en evidencia para el manejo de la pancreatitis aguda en EsSalud. Se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 7 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y -cuando fue considerado pertinente- estudios primarios en PubMed durante el 2022. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y flujogramas correspondientes. Finalmente, la GPC fue aprobada con Resolución N° 105-IETSI-ESSALUD-2022. La presente GPC abordó 7 preguntas clínicas sobre fluidoterapia, momento de inicio de nutrición enteral, analgesia, tipo de nutrición, tratamiento antibiótico y quirúrgico. En base a dichas preguntas se formularon 8 recomendaciones (1 fuerte y 7 condicionales), 13 BPC, y 1 flujograma. El presente artículo resume la metodología y las conclusiones basadas en evidencia de la GPC para el manejo de la pancreatitis aguda en EsSalud.

ABSTRACT Hepatocellular carcinoma (HCC) is the third most common cause of cancer-related mortality worldwide. The Brazilian Society of Hepatology (SBH) published in 2020 the updated recommendations for the diagnosis and treatment of HCC. Since then, new data have emerged in the literature, including new drugs approved for the systemic treatment of HCC that were not available at the time. The SBH board conducted an online single-topic meeting to discuss and review the recommendations on the systemic treatment of HCC. The invited experts were asked to conduct a systematic review of the literature on each topic related to systemic treatment and to present the summary data and recommendations during the meeting. All panelists gathered together for discussion of the topics and elaboration of the updated recommendations. The present document is the final version of the reviewed manuscript containing the recommendations of SBH and its aim is to assist healthcare professionals, policy-makers, and planners in Brazil and Latin America with systemic treatment decision-making of patients with HCC.

RESUMO O carcinoma hepatocelular (CHC) é uma das principais causas de mortalidade relacionada a câncer no Brasil e no mundo. A Sociedade Brasileira de Hepatologia (SBH) publicou em 2020 a atualização das recomendações da SBH para o diagnóstico e tratamento do CHC. Desde então, novas evidências científicas sobre o tratamento sistêmico do CHC foram relatadas na literatura médica, incluindo novos medicamentos aprovados que não estavam disponíveis na época do último consenso, levando a diretoria da SBH a promover uma reunião monotemática on-line para discutir e rever as recomendações sobre o tratamento sistêmico do CHC. Um grupo de experts foi convidado para realizar uma revisão sistemática da literatura e apresentar uma atualização, baseada em evidências científicas, sobre cada tópico relacionado ao tratamento sistêmico e a apresentar os dados e recomendações resumidas durante a reunião. Todos os painelistas se reuniram para discutir os tópicos e elaborar as recomendações atualizadas. O presente documento é a versão final do manuscrito revisado, contendo as recomendações da SBH, e seu objetivo é auxiliar os profissionais de saúde, formuladores de políticas e planejadores no Brasil e na América Latina na tomada de decisões sobre o tratamento sistêmico de pacientes com CHC.

ABSTRACT Soil quality can be understood as its capacity to provide several essential services within the ecosystem and has been used to understand the impact of different managements, providing information that proves the benefits and your maintenance of the agroecosystem. To understand the impact of different managements, this study aimed to compare the chemical, physical, and biological soil properties in pasture areas managed with different recovery times in an integrated crop-livestock system about perennial pastures and secondary forest. The following management systems were evaluated: Secondary Forest (SF), Perennial Pasture (PP), pasture recovered to five years through the intercropping corn + Brachiaria brizantha (Palisade grass) (ICL5), and pasture recovered to eight years through the intercropping corn + Brachiaria brizantha (Palisade grass) (ICL8). Different soil properties were evaluated, namely: Chemical: pH, H+Al, Al 3+ , P, K + , Ca 2+ , Mg 2+ , TOC, SB, CEC, V, and m; Physical: soil bulk density (Bd), total porosity (Tp), macroporosity (Ma), microporosity (Mi), soil resistance to penetration (Pr), and gravimetric soil water content (GWc); and biological: soil microbial biomass carbon (SMB-C), basal soil respiration (BSR), metabolic quotient (qCO 2 ), and microbial quotient (qMic). Perennial pasture and ICL8 areas were the ones that most contributed to the increase in nutrients (Ca 2+ , Mg 2+ , and K + ), TOC and sorption complex. The ICL8 area showed the best results in soil physical variables Ma, Tp, Pr, and GWc were the best results for the ICL8 area. Secondary forest and ICL8 areas presented the best results from SMB-C and qMic. Between periods of pasture recovery through the integration of crops and livestock, the longer the recovery time, the greater its beneficial effects on the different chemical, physical and biological soil properties, overcoming secondary forest and perennial pasture.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

ABSTRACT. In Maranhão State, Brazil, soils are naturally acidic, nutrient-deficient, and prone to cohesion and erosion. Removing the natural cover to establish pastures causes physical, chemical, and biological changes in the soil. Therefore, this study aimed to evaluate the contents and stocks of carbon (C), nitrogen (N), and particle-size fractions of soil organic matter (SOM) in pastures with different years of recovery, and compare them with a secondary forest in the Legal Amazon. Four treatments were evaluated: secondary forest, perennial pasture, and perennial pastures recovered for five years and eight years, both of the latter through corn + brachiaria intercropping. The contents and stocks of total organic carbon, total nitrogen, C, and N from the soil organic matter particle-size fractions, as well as the carbon management indexes (CMI) of the 0.00-0.10, 0.10-0.20, 0.20-0.30, and 0.30-0.40 m layers were evaluated. The perennial pasture environment presented the highest total soil C and N contents; however, when observing the granulometric fractions and CMI, these increases were qualitative in relation to the secondary forest. Pasture recovery over eight years contributed to an improvement of soil quality similar to secondary forest, indicating that an increase in SOM quality, quantity, and recovery time related to increased pasture capacity to accumulate C and N in the soil.

Resumo Trata-se de uma análise qualitativa das percepções de atores sociais sobre as respostas públicas durante a pandemia de COVID-19. Foram conduzidas entrevistas semiestruturadas com 11 representantes da sociedade civil indicados por lideranças de movimentos sociais. Para garantir a análise de uma diversidade de vozes, também realizamos a sistematização de artigos de opinião de autoria de organizações humanitárias e lideranças da sociedade civil, publicados em jornais de grande circulação. Nossa análise se inspirou nos princípios éticos da justiça social, solidariedade e cidadania. Utilizamos da análise temática, na qual dois temas principais emergiram: 1) as populações desproporcionalmente afetadas seguem invisibilizadas para cuidados e proteções; 2) há um projeto intencional de aniquilamento das populações consideradas indesejáveis. Para cuidados e mitigação dos efeitos durante emergências sanitárias, os movimentos comunitários devem ser incorporados como parte essencial das respostas. Cuidado, solidariedade e participação cidadã são centrais para a construção de respostas para proteção em saúde dentro do marco da justiça social. As respostas para transformação do futuro pós-pandêmico se localizam nas iniciativas da sociedade civil e de lideranças comunitárias dentro dos territórios.

Abstract This qualitative study aimed to analyze the social actors’ perceptions of public responses during the COVID-19 pandemic. We conducted semi-structured interviews with eleven civil society representatives and social movement leaders. We also performed triangulation based on the systematization of opinion papers authored by humanitarian organizations and civil society leaders and published in large-circulation newspapers. Our analysis was inspired by the ethical principles of social justice, solidarity, and citizenship. Two main themes emerged from the thematic analysis: 1) disproportionately affected populations remain invisible to care and protection; 2) there is an intentional project to annihilate “undesirable” populations. Community movements must be incorporated as an essential part of the responses to provide care and protection and mitigate the effects during health emergencies. Care, solidarity, and social participation are central to building health protection responses within the framework of social justice. The responses to transforming the future in the aftermath of the pandemic will occur through the initiatives of civil society and community leaders within the territories.

Resumo O enfrentamento da covid-19 suscitou uma série de problemas na área da saúde, em razão do aumento da demanda de cuidados intensivos. Para solucionar a crise causada pela escassez de recursos de alta complexidade, a tomada de decisão tem se norteado por escores prognósticos, porém esse processo inclui uma dimensão moral, ainda que esta seja menos evidente. Mediante revisão integrativa, este artigo buscou refletir sobre a razoabilidade da utilização de indicadores de gravidade para definir a alocação de recursos escassos na saúde. Observou-se que o trabalho realizado em situações de escassez de recursos provoca sobrecarga moral, convergindo para busca por soluções padronizadas e objetivas, como a utilização de escores prognósticos. Conclui-se que seu uso isolado e indiscriminado não é eticamente aceitável e merece avaliação cautelosa, mesmo em situações emergenciais, como a da covid-19.

Abstract Facing COVID-19 caused many problems in the healthcare field, due to the rise in the intensive care demand. To solve this crisis, caused by the scarcity of resources of high complexity, decision-making has been guided by prognostic scores; however, this process includes a moral dimension, although less evident. With na integrative review, this article sought to reflect on the reasonability of using severity indicators to define the allocation of the scarce resources in healthcare. We observed that the work carried out on resource scarcity situations causes moral overload, converging to the search for standard and objective solutions, such as the use of prognostic scores. We conclude that their isolated and indiscriminate use is not ethically acceptable and deserves cautious evaluation, even in emergency situations, such as COVID-19.

Resumen La lucha contra el Covid-19 implicó una serie de problemas en el área de la salud, debido al aumento de la demanda de cuidados intensivos. Para solucionar la crisis provocada por la escasez de recursos de alta complejidad, la toma de decisiones estuvo orientada por puntuaciones pronósticas, pero este proceso incluye una dimensión moral aún menos evidente. A partir de una revisión integradora, este artículo buscó reflexionar sobre la razonabilidad de utilizar indicadores de gravedad para definir la asignación de recursos escasos en salud. El trabajo realizado en situaciones de escasez de recursos genera sobrecarga moral, llevando a la búsqueda de soluciones estandarizadas y objetivas, como el uso de puntuaciones de pronóstico. Se concluye que su uso aislado e indiscriminado no es éticamente aceptable y merece una cuidadosa evaluación, incluso en situaciones de emergencia, como la del Covid-19.

ABSTRACT For soccer managers, player selection lineups are a key process for better performance, both on financial and sports matters. However, to determine a quality solution to this problem it becomes more complex, as the number of alternatives and criteria increases and the number of viable solutions grows ex ponentially. This paper proposes a multicriteria method with genetic algorithm for the evaluation of soccer teams based on Brazilian Championship data. A team complementation percentage was calculated con sidering a total of 322 athletes and 18 criteria. The results presented a 3-6-1 format as the ideal for this case study, obtaining a team complementation value of 43.04%. The method adaptability for the decision-maker is highlighted, showing it was possible to determinate the most complementary team according to the desired tactical formation and importance attributed for each criterion.

ABSTRACT Before being reused, industrial waste needs to have its composition determined so that in the future, it will not cause harm to the people and to the environment. The present study proposes the characterization and reuse of the residue remaining in the production of leveling masses and textures as a component in the Portland cement mortar specimens in order to analyze the effects on the axial compressive strength. The residue generated was characterized by Infrared (IR) Absorption Spectroscopy, X-ray Diffraction (XRD), Atomic Emission (AE) and Thermogravimetric Analysis (TG/DTG). All analyzes confirmed the predominant presence of dolomite (CaMg(CO3)2) as the main component and absence of potentially toxic metals. The addition of 10% of the residue on the cement and sand mass or the replacement of 10% of the sand mass in the mortars had small impact on the axial compression strength. The standard deviation values found lead to the conclusion that the compressive strength of the studied mortars can be considered statistically similar.

Resumo A concentração do turismo no litoral brasileiro é amplamente reconhecida, porém seu desenvolvimento não ocorre de forma homogênea ao longo de toda costa. Assim, o objetivo do presente artigo é analisar a difusão espacial da atividade turística no litoral brasileiro, procurando identificar as localidades onde assume maior expressividade e os fatores que contribuíram para seu desenvolvimento desigual. Os procedimentos adotados incluíram a exploração de informações sobre os destinos turísticos de duas principais fontes: o Guia Quatro Rodas, da Editora Abril; e o Mapa do Turismo Brasileiro, do Ministério do Turismo, edição 2019. O mapeamento e a análise dos dados permitiram concluir que a espacialidade do turismo litorâneo foi condicionada pela estrutura urbana preexistente, concentrando-se nas grandes cidades. Entretanto, municípios de pequeno e médio porte, após intervenções seletivas do poder público, tornaram-se destinos de destaque no mercado turístico regional, nacional e internacional.

Abstract The concentration of tourism on the Brazilian coast is widely recognised, despite the fact that its development does not take place in an even manner along the entirety of the coast. As a result, the goal of this article is to examine the spatial diffusion of tourism-related activities on the Brazilian shore and seek to identify the places where it gains more prominence, along with the factors that contributed to its uneven growth. The procedures adopted included the study of tourism-related data as contained in two sources: the Guia Quatro Rodas [‘Quatro Rodas’ Motoring Guide] of the Abril Publishing House and the Brazilian Tourism Map, as published by the Brazilian Department of Tourism in its 2019 edition. The mapping and the analysis of the data allowed us to conclude that the spatial character of coastal tourism was conditioned by the pre-existing urban structure, and concentrated on the major cities. It should be noted that small and mid-sized cities, following specific interventions from the Government, became relevant destinations in the regional, domestic and foreign markets.

Resumen La concentración del turismo en la costa brasileña es ampliamente reconocida, pero su desarrollo no es homogéneo a lo largo de toda la costa. Así, el objetivo de este artículo es analizar la difusión espacial de la actividad turística en la costa brasileña, buscando identificar los lugares donde asume mayor expresión y los factores que contribuyeron a su desigual desarrollo. Los procedimientos adoptados incluyeron la exploración de información sobre destinos turísticos de dos fuentes principales: Guía Quatro Rodas, de Editora Abril; y el Mapa de Turismo de Brasil, del Ministerio de Turismo, edición 2019. El mapeo y análisis de datos permitió concluir que la espacialidad del turismo costero estaba condicionada por la estructura urbana preexistente, concentrándose en las grandes ciudades. Sin embargo, los municipios pequeños y medianos, luego de intervenciones selectivas por parte del gobierno, se han convertido en destinos destacados en el mercado turístico regional, nacional e internacional

Resumo O objetivo deste ensaio, dividido em duas partes, é investigar a estrutura geral do vínculo terapêutico. A primeira parte descreve agentes sociais como sujeitos moralmente interpelados pela norma fundamental da justiça (“a cada qual o que lhe é devido”), que na cena particular do cuidado se desdobra em potenciais conflitos intersubjetivos. A segunda representa a relação terapêutica articulada com os “três níveis do juízo médico” apontados por Paul Ricoeur – prudencial, deontológico e reflexivo –, conferindo às prescrições desse âmbito o estatuto de relação francamente moral.

Abstract This article investigates the general structure of the therapeutic bond in two parts: a pragmatic description of social agents as subjects morally challenged by the fundamental norm of justice (“to each one what is due”), which in healthcare may also have potential intersubjective conflicts; and a representation of the therapeutic relationship based on Paul Ricœur’s “three levels of medical judgment” (prudential, deontological and reflexive), giving the prescriptions in this scope the status of a moral relationship.

Resumen El objetivo de este ensayo, que se divide en dos partes, es investigar la estructura general del vínculo terapéutico. La primera parte describe a los agentes sociales como sujetos a los que la norma fundamental de la justicia (“a cada uno lo suyo”) interpela moralmente, lo que, en la escena particular de la atención médica, se desdobla en posibles conflictos intersubjetivos. La segunda parte muestra la relación terapéutica que se articula mediante lo que el filósofo Paul Ricoeur denomina “los tres niveles de juicio médico” –prudencial, deontológico y reflexivo–, confiriendo a las prescripciones de este ámbito el estatuto de una relación francamente moral.

Resumen Objetivo. Apreender las representaciones sociales de las trabajadoras sexuales sobre su sexualidad. Métodos. Estudio cualitativo, basado en la Teoría de las Representaciones Sociales. Aceptaron participar 69 mujeres de una región productiva bahiana de salud en el Sertão (Brasil). Para la producción de los datos empíricos, se utilizaron las técnicas de Asociación Libre de Palabras y entrevistas a profundidad. Las respuestas se analizaron a partir del Análisis de Contenido por Objetivo de Constelación o de Contenido Semántico. Resultados. Surgieron dos categorías temáticas: "Representación negativa de la sexualidad" y "Mi placer es el dinero". Para las trabajadoras sexuales, la sexualidad tiene una representación negativa cuando se asocia con la satisfacción sexual con el cliente, además de la alusión al sexo como fuente de ingreso económico. Conclusión. Las representaciones sociales de la sexualidad construidas por las trabajadoras sexuales se refieren al sentimiento de negación del placer y a la obtención de dinero para la subsistencia. Reflexionar sobre la sexualidad aporta caminos para repensar el cuidado a un grupo estigmatizado por la sociedad y con alta vulnerabilidad social

Abstract Objective. To know the social representations of female sex workers about their sexuality. Methods. Qualitative study based on the Theory of Social Representations. Thirty-nine women from a health region of Alto Sertão Produtivo Baiano - Brazil agreed to participate. For the production of empirical data, the techniques of Free Word Association and in-depth interviews were used. The answers were analyzed based on Constellation Target Content Analysis and Semantic Content Analysis. Results. Two thematic categories emerged: “negative representation of sexuality”; “my pleasure is the money”. Therefore, the theme sexuality and meanings derived from the social representations elaborated by the sex workers about sexuality, based on their experiences and daily life, showed that the work involved a negative representation of sexuality when associated with sexual satisfaction with the client, in addition to the allusion to sex as a source of income. Conclusion. The social representations about sexuality constructed by sex workers are linked to the feeling of denial of pleasure and obtaining money for subsistence. Reflecting on sexuality points out ways to rethink the care to be provided for a stigmatized and vulnerable group.

Resumo Objetivo. Apreender as representações sociais de trabalhadoras sexuais sobre sua sexualidade. Métodos. Estudo qualitativo, fundamentado na Teoria das Representações Sociais. Aceitaram participar 69 mulheres de uma região de saúde do Sertão Produtivo Baiana (Brasil). Para a produção dos dados empíricos utilizou-se as técnicas de Associação Livre de Palavras, e a entrevista em profundidade. As respostas foram analisadas a partir da Análise de Conteúdo por Alvo de Constelação e de Conteúdo Semântica. Resultados. Surgiram duas categorias temáticas: “representação negativa da sexualidade” e “meu prazer é o dinheiro”. Para as trabalhadoras sexuais, a sexualidade envolve a representação negativa quando associado à satisfação sexual com o cliente, além da alusão ao sexo como fonte de renda. Conclusão. As representações sociais sobre a sexualidade construídas por trabalhadoras sexuais remetem ao sentimento de negação do prazer e a obtenção do dinheiro para a subsistência. Refletir sobre a sexualidade apontam caminhos para repensar o cuidado a um grupo estigmatizado e de vulnerabilidade pela sociedade.
