Abstract Background: Recently, opioids have been related to trigger changes in cytokine release and tumor angiogenesis processes, influencing tumor growth, metastasis, and recurrence. Methods: This is a prospective randomized clinical study to test whether if exogenous opioids used in the anesthesia during cancer surgery can affect the systemic inflammatory and immunological patterns. Patients were randomly allocated to the OP (opioid-inclusive) or OF (opioid-free) anesthesia group. A total of 45 patients were selected, being carriers of prostate, stomach, pancreas, bile ducts, breast, colon, lung, uterus, kidneys, or retroperitoneum tumors. Plasma levels of IL-4, IL-12, IL-17A, and TNF-α, and their oxidative stress profile before and after surgery were evaluated in both groups. In vitro tests were performed by using healthy donor blood incubated with each isolated drug used in patients’ anesthesia for 1 hour, the same cytokines were measured in plasma. Results: There was a significant reduction in lipid peroxidation in both groups. Patients from OF group had a significant consumption of IL-12 in the perioperative period. The other cytokines evaluated did not vary. It was also observed a significant correlation between IL-12 and TNF-α levels in the OF-post group. Except for atracurium, all tested drugs led to a reduction in IL-12 levels. Conclusions: This study demonstrated that there is a reduction of IL-12 in the OF-post patients, suggesting acute consumption and that this seems to be a general mechanism of anesthetic drugs, as demonstrated in vitro. Also, these findings bring us to reflect if IL-12 changes may influence the disease progression and recurrence.

Resumo Calcado na leitura de Moore (2015) e nos conceitos de neoextrativismo e land grabbing, este trabalho caracteriza uma forma emblemática de concretização do capitalismo na América Latina, especialmente no estado brasileiro do Mato Grosso. Como manifestações concretas do land grabbing são discutidos os recentemente instituídos Fundos de Investimento do Agronegócio (Fiagro) e apresentadas as empresas listadas em bolsa vinculadas diretamente à especulação de terras e/ou ao monocultivo. Apresentam-se a geolocalização das fazendas administradas pelos Fiagro e sua proximidade dos conflitos por água no estado do Mato Grosso, além do mapa dos compartimentos geoambientais desse estado e o das terras indígenas. O objetivo é encontrar pistas de como - em resposta ao movimento geral de circulação e de valorização do capital em escala global - a construção e a operacionalização desses renovados instrumentos financeiros podem repercutir concretamente na existência de territórios de povos e comunidades que teimam em funcionar com outra lógica.

Abstract Based on the reading of Moore (2015) and the concepts of neo-extractivism and land grabbing, this paper characterizes an emblematic form of the realization of capitalism in Latin America, particularly in the Brazilian state of Mato Grosso. As concrete expressions of land grabbing, the recently established Investment Funds in Agroindustrial Productive Chains (Fiagro) are discussed, and companies listed on the stock exchange directly linked to land speculation and/or monoculture are also presented. The geolocation is also revealed of the farms managed by Fiagro and their proximity to water conflicts in the state of Mato Grosso, together with a map presenting the geoenvironmental compartments of Mato Grosso and the indigenous lands. The objective is to discover clues on how - in response to the general movement of circulation and appreciation of capital on a global scale - the construction and operationalization of these renewed financial instruments may have a concrete impact on the existence of territories of peoples and communities that insist on operating with another logic.

ABSTRACT Objective: To investigate whether family participation in intensive care unit interdisciplinary bedside rounds affects family satisfaction. Methods: A cross-sectional study was conducted at a 56-bed, adult, mixed intensive care unit of a tertiary hospital in Southern Brazil. From May to June 2019, family members of patients who stayed in the intensive care unit for at least 48 hours were invited to participate in the study at the time of patient discharge. The main exposure variable was participation in intensive care unit bedside rounds during the intensive care unit stay. Family satisfaction was assessed by using the Brazilian version of the Family Satisfaction in the Intensive Care Unit questionnaire. Results: Of the 234 screened individuals, 118 were included. Eleven participants withdrew consent. A total of 107 individuals were assessed; 58 (54%) reported being present during bedside rounds, and 49 (46%) reported never being present. General satisfaction and satisfaction with the decision-making process were higher among families who were present during rounds than among families who were not (p = 0.01 and p = 0.007, respectively). Conclusion: The presence during interdisciplinary rounds was associated with improved general satisfaction and satisfaction with the decision-making aspect. This outcome indicates that efforts must be directed to conduct studies with more robust methodologies to confirm this association.

RESUMO Objetivo: Investigar se a participação da família em rodadas interdisciplinares à beira do leito da unidade de terapia intensiva afeta a satisfação familiar. Métodos: Foi realizado um estudo transversal em uma unidade de terapia intensiva adulto mista com 56 leitos de hospital terciário no Sul do Brasil. De maio a junho de 2019, os familiares de pacientes internados na unidade de terapia intensiva por, pelo menos, 48 horas foram convidados a participar do estudo no momento da alta do paciente. A principal variável de exposição foi a participação em rodadas à beira do leito na unidade de terapia intensiva durante a internação na unidade de terapia intensiva. A satisfação familiar foi avaliada por meio da versão brasileira do Family Satisfaction in the Intensive Care Unit. Resultados: Dos 234 indivíduos selecionados, 118 foram incluídos. Destes, 11 participantes retiraram o consentimento. Foram avaliados 107 indivíduos; 58 (54%) relataram estar presentes durante as rodadas à beira do leito, e 49 (46%) relataram nunca estar presentes. A satisfação geral e a satisfação com o processo de tomada de decisão foram maiores entre as famílias presentes durante as rodadas do que entre as famílias ausentes (p = 0,01 e p = 0,007, respectivamente). Conclusão: A presença durante as rodadas interdisciplinares foi associada à melhora da satisfação geral e satisfação em relação ao processo de tomada de decisão. Esse desfecho indica que, para confirmar essa associação, devem-se direcionar esforços para realizar estudos com metodologias mais robustas.

Resumo Introdução O tabagismo é uma das principais causas evitáveis de doenças e um grande desafio para a saúde pública, sendo a Atenção Primária à Saúde (APS) o nível de atenção à saúde com maior potencial de manejo da cessação de uso de tabaco. Objetivo O objetivo foi descrever os resultados de cessação de tabaco em grupos inseridos em um programa público de tratamento do tabagismo ao longo de 11 anos. Método Estudo retrospectivo do tipo série histórica dos grupos de tabagismo desenvolvido no período de 2006 a 2016 nas 12 unidades de saúde do Serviço de Saúde Comunitária do Grupo Hospitalar Conceição (GHC), Porto Alegre-RS. Os dados foram coletados nos Relatórios Anuais de Indicadores de Saúde e no Sistema de Informações do GHC. Análises descritivas foram realizadas por meio de frequências absolutas e relativas. Resultados Dos 2.691 tabagistas acompanhados, 1.273 (47%) deixaram de fumar até a quarta sessão do grupo. Discussão A cessação de tabaco em grupos contribuiu para que mais da metade dos participantes conseguisse parar de fumar ao longo do período de 4 semanas de tratamento em grupo. Esta é uma importante estratégia terapêutica acessível e eficiente para auxiliar e oportunizar um atendimento integral aos usuários tabagistas no contexto da APS.

Abstract Background Smoking is one of the main preventable causes of diseases and a major challenge for public health. Primary Health Care (PHC) is the level of health care with the greatest potential for managing tobacco cessation. Objective The objective was to describe the results of smoking cessation in groups included in a public smoking treatment program over 11 years. Methods Retrospective study of the historical series type of smoking groups developed between 2006 and 2016 in the 12 health units of the Community Health Service of Grupo Hospitalar Conceição (GHC), Porto Alegre-RS. Data were collected in the Annual Health Indicator Reports and the GHC Information System. Descriptive analyzes were performed using absolute and relative frequencies. Results Of the 2,691 smokers monitored, 1,273 (47%) quit smoking until the fourth session of the group. Discussion Smoking cessation in groups contributed to more than half of the participants being able to quit smoking over 4 weeks of treatment groups. This is an important accessible and efficient therapeutic strategy to assist and provide comprehensive care to smokers in the context of PHC.

RESUMO O tratamento de lesões cavitadas de cárie em dentes decíduos anteriores é uma demanda frequente na clínica de odontopediatria, visto que o controle da doença na primeira infância ainda se apresenta como um desafio interfatorial nos cuidados em saúde infantil. Enquanto a evidência científica nos princípios minimamente invasivos de abordagem em Odontologia apresenta-se em nível crescente, as alternativas para tratamento restaurador em dentes decíduos anteriores ainda esbarram na baixa disponibilidade de relatos que associem a mínima intervenção às necessidades estéticas inerentes ao tratamento. Assim, este artigo apresenta uma série de casos em que as restaurações diretas em resina composta, sem a remoção de tecido cariado utilizando matrizes de polivinila foram alternativas de tratamento para lesões de cárie moderadas e severas em dentes decíduos anteriores. Observou-se que o uso desse artefato auxiliou a reabilitação dos elementos dentários comprometidos com rapidez e resultado satisfatório, sugerindo que essa é uma abordagem adequada para aplicação em Odontopediatria, já que oferece resistência, durabilidade, além de proporcionar estética e funcionalidade, apresenta menor custo e agilidade na medida que dispensa a fase laboratorial.

ABSTRACT The treatment of cavity carious lesions in anterior deciduous teeth is a frequent demand in the pediatric dentistry clinic, since the control of the disease in early childhood still presents itself as an inter-factor challenge in child health care. While the scientific evidence on the minimally invasive principles of approach in Dentistry is presented at an increasing level, the alternatives for restorative treatment in anterior deciduous teeth still face the low availability of reports that associate minimal intervention with the aesthetic needs inherent to the treatment. Thus, this article presents a series of cases in which direct restorations in composite resin, without removing carious tissue using polyvinyl matrices, were treatment alternatives for moderate and severe caries lesions in anterior deciduous teeth. It was observed, therefore, that the use of this artifact helped the rehabilitation of compromised dental elements with speed and satisfactory 50 result, suggesting that this is an appropriate approach for application in Pediatric Dentistry, 51 since it offers resistance, durability, besides providing aesthetics and functionality, it presents lower cost and agility as it eliminates the laboratory phase.

RESUMO: A hemipelvectomia parcial consiste na remoção de segmento pélvico, indicada para neoplasmas com envolvimento de ossos da pelve, fraturas graves ou má união óssea. Esse trabalho apresenta sete pacientes caninos, dos quais seis portadores de neoplasmas envolvendo a articulação coxofemoral e um com grave estreitamento do canal pélvico, resultado de traumatismo mal consolidado. Com o intuito de fornecer qualidade de vida aos cães e postergar a eutanásia, todos foram operados pela técnica de hemipelvectomia parcial média a caudal. Dos sete pacientes, seis necessitaram do uso de tela de polipropileno para fechamento da parede pélvica e apresentaram 85,71% de complicações manejáveis no pós-operatório. No entanto, a melhora na qualidade de vida dos animais operados foi significativa, tendo sido removida a causa da dor ou desconforto desencadeado pela alteração anatômica local. Conclui-se que a técnica é benéfica aos pacientes, tendo como principal complicação, a falta de parede pélvica para oclusão primária, contornada com uso de tela de polipropileno pelos autores.

ABSTRACT: Partial hemipelvectomy refers to the surgical removal of a pelvic segment, which is performed for pelvic bone neoplasms, severe fractures, or poor bone union. This study presents the cases of seven dogs, six of which have neoplasms involving the hip joint and one with severe pelvic canal narrowing due to a poorly consolidated trauma. Partial caudal hemipelvectomy was performed for all the dogs to improve the quality of life and delay euthanasia. Of the seven cases, six required the use of a polypropylene mesh for the pelvic wall closure, with 85.71% of them acquiring manageable complications in the postoperative period. Despite these complications, an overall improvement in the quality of life was reported to be significant in the operated animals due to the removal of the cause of pain or discomfort. Thus, it was concluded that partial hemipelvectomyis beneficial for dogs with pelvic conditions. Furthermore, the main complication due to the lack of a pelvic wall for primary occlusion was circumvented by using a polypropylene mesh, showing that possible complications in this procedure can be appropriately managed.

Abstract Soybean is a key crop in many countries, being used from human food to the animal industry due to its nutritional properties. Financially, the grain chain moves large sums of money into the economy of producing countries. However, like other agricultural commodities around the world, it can have its final yield seriously compromised by abiotic environmental stressors, like drought. As flowers imply in pods and in grains inside it to minimize damages caused by water restriction, researchers have focused on understanding flowering-process related genes and their interactions. Here a review dedicated to the soybean flowering process and gene network involved in it is presented, describing gene interactions and how genes act in this complex mechanism, also ruled by environmental triggers such as day-light and circadian cycle. The objective was to gather information and insights on the soybean flowering process, aiming to provide knowledge useful to assist in the development of drought-tolerant soybean lines, minimizing losses due to delays or anticipation of flowering and, consequently, restraining financial and productivity losses.

RESUMO Objetivo avaliar a prevalência de maloclusões e seus fatores associados em crianças atendidas em um Serviço de APS de Porto Alegre, Brasil. Método estudo transversal aninhado a uma coorte, realizado em 12 Unidades de Saúde. Das 414 crianças da coorte examinadas, 268 foram examinadas para maloclusão. A presença de mordida aberta anterior, mordida cruzada posterior e anterior foi avaliada pelos critérios de Foster e Hamilton. Variáveis socioeconômicas, hábitos de amamentação e informações sobre o uso de chupeta foram coletados por meio de questionário padronizado. A análise dos dados foi realizada por meio de uma abordagem hierárquica por Regressão de Poisson com variância robusta. Resultados do total de 268 avaliados, 135 (50,4%) eram meninos e a média de idade foi de 28,6 (± 11,9) meses. Dos 143 (53,4%) casos de maloclusão, 113 eram mordida aberta anterior, 16 eram mordida cruzada anterior, 27 eram mordida cruzada posterior e 38 tinham overjet acentuado. Na análise final, observou-se que houve maior prevalência de maloclusões em crianças que nunca foram amamentadas (RP = 1,44; IC95% 1,00-2,08) e que sempre usaram chupeta para dormir (RP = 1,81; IC95% 1,14 -2,86). Conclusão a prevalência de maloclusões nesta população foi elevada e esteve associada a hábitos comportamentais, como o uso de chupeta e a não amamentação.

The presence of anterior open bite, posterior and anterior crossbite was evaluated by the criteria of Foster and Hamilton. Socioeconomic variables, breastfeeding habits and pacifier use information were collected through a standardized questionnaire. Data analysis was performed using a hierarchical approach by Poisson Regression with robust variance. Results out of the total 268 evaluated, 135 (50.4%) were boys, and the average age was 28.6 (± 11.9) months. Out of the 143 (53.4%) cases of malocclusion, 113 were anterior open bite, 16 were anterior crossbite, 27 were posterior crossbite, and 38 had increased overjet. In the final analysis, it was observed that there was a higher prevalence of malocclusion in children who never breastfed (PR = 1.44; 95%CI 1.00-2.08) and who always used a pacifier to sleep (PR = 1.81; 95%CI 1.14-2.86). Conclusion the prevalence of malocclusion in this population was high and was associated with behavioral habits, such as the use of pacifier and not breastfeeding.

Abstract Introduction Cigarettes are the main cause of preventable death in the world, and primary health care services can contribute to the management of this habit. Objective To describe and analyze the factors associated with tobacco cessation in groups of smokers in primary health care. Methods The present is a cross-sectional study conducted in 12 primary health care units from July 2016 to May 2017. We investigated sociodemographic and health variables, as well as smoking characteristics and different interventions for tobacco cessation, with the outcome being studied and analyzed after the fourth group care session. Results We evaluated 329 smokers, of which 182 quit smoking after the fourth interview. Most of the individuals who quit smoking were women (n = 121, 66.5%), with a monthly income of 2 to 5 minimum wages (n = 88, 77.9%). After the multivariable analysis, we observed that tobacco cessation was significantly associated with depression (prevalence ratio [PR] = 1.11; 95% confidence interval [95%CI] = 1.02 to 1.22), as well as with belonging to the groups which used as approaches motivational interviewing (PR = 1.21; 95%CI = 1.13 to 2.01), patches (PR = 1.36; 95%CI = 1.24 to 1.48), and bupropion (PR = 1.16; 95%CI = 1.03 to 1.31). Conclusion Primary health care is the ideal site for the reduction of smoking rates, given that different technologies may be applicable and useful for tobacco cessation. The comprehensiveness and longitudinal care offered in primary care may provide opportunities for health professionals to understand which is the best technology for each health system user, thus contributing to personalized care.

RESUMO Objetivo: Avaliar a capacidade do Teste de Caminhada de 6 Minutos para predizer a melhora do estado funcional físico em longo prazo de pacientes sobreviventes à unidade de terapia intensiva. Métodos: Foram avaliados, de forma prospectiva, entre fevereiro de 2017 e agosto de 2018, em um ambulatório pós-unidade de terapia intensiva, 32 sobreviventes à unidade de terapia intensiva. Foram inscritos consecutivamente os pacientes com permanência na unidade de terapia intensiva acima de 72 horas (para admissões emergenciais) ou acima de 120 horas (para admissões eletivas) que compareceram ao ambulatório pós-unidade de terapia intensiva 4 meses após receberem alta da unidade de terapia intensiva. A associação entre a distância percorrida no Teste de Caminhada de 6 Minutos realizado na avaliação inicial e a evolução do estado funcional físico foi avaliada durante 8 meses, com utilização do Índice de Barthel. Resultados: A distância média percorrida no Teste de Caminhada de 6 Minutos foi significantemente mais baixa nos sobreviventes à unidade de terapia intensiva do que na população geral (405m versus 557m; p < 0,001). A idade (β = -4,0; p < 0,001) e a fraqueza muscular (β = -99,7; p = 0,02) se associaram com a distância percorrida no Teste de Caminhada de 6 Minutos. A distância percorrida no Teste de Caminhada de 6 Minutos se associou com melhora do estado funcional físico no período de 8 meses de acompanhamento desses pacientes (razão de chance para cada 10m: 1,07; IC95% 1,01 - 1,16; p = 0,03). A área sob a curva Característica de Operação do Receptor para predição da melhora funcional física pelo Teste de Caminhada de 6 Minutos foi de 0,72 (IC95% 0,53 - 0,88). Conclusão: O Teste de Caminhada de 6 Minutos, realizado 4 meses após a alta da unidade de terapia intensiva, predisse com precisão moderada a melhora do estado funcional físico de sobreviventes à unidade de terapia intensiva.

ABSTRACT Objective: To evaluate the ability of the 6-Minute Walk Test to predict long-term physical functional status improvement among intensive care unit survivors. Methods: Thirty-two intensive care unit survivors were prospectively evaluated from February 2017 to August 2018 in a post-intensive care unit outpatient clinic in Brazil. Individuals with intensive care unit stays > 72 hours (emergency admissions) or > 120 hours (elective admissions) attending the post-intensive care unit clinic four months after intensive care unit discharge were consecutively enrolled. The association between the 6-Minute Walk Test distance at baseline and physical functional status was assessed over 8 months using the Barthel Index. Results: The mean 6-Minute Walk Test distance was significantly lower in intensive care unit survivors than in the general population (405m versus 557m; p < 0.001). Age (β = -4.0; p < 0.001) and muscle weakness (β = -99.7; p = 0.02) were associated with the 6-Minute Walk Test distance. A 6-Minute Walk Test distance was associated with improvement in physical functional status over the 8-month follow-up (odds ratio for each 10m of 1.07; 95%CI 1.01 - 1.16; p = 0.03). The area under the Receiver Operating Characteristic curve for the 6-Minute Walk Test prediction of physical functional status improvement was 0.72 (95%CI 0.53 - 0.88). Conclusion: The 6-Minute Walk Test performed 4 months after intensive care unit discharge predicted long-term physical functional status among intensive care unit survivors with moderate accuracy.

RESUMO Introdução: Para melhor comparar a prevalência de esclerose múltipla (EM) nas diferentes regiões do Brasil, o Comitê Brasileiro para Tratamento e Pesquisa em Esclerose Múltipla implementou uma abordagem padronizada para avaliar a prevalência da doença em 5 cidades-chave, consideradas representativas de suas regiões em termos de características sociogeográficas e nas quais seria viável revisar cada caso pessoalmente. Objetivos: Descrever a prevalência pontual de EM em Passo Fundo, uma das cidades-chave, localizada no Sul do Brasil. Métodos: Buscamos identificar todos os pacientes com EM que viviam em Passo Fundo no dia 1( de julho de 2015. A fonte primária para identificação de casos foi os registros de consultórios de neurologistas e neurocirurgiões da cidade. Múltiplas fontes secundárias foram usadas para maximizar a identificação de casos. Todos os pacientes tiveram o diagnóstico revisado pessoalmente por um painel de neurologistas com experiência em EM. Resultados: Identificamos 52 pacientes com EM que viviam em Passo Fundo em 1( de julho de 2015. Assim, a prevalência pontual bruta de EM foi 26,4/100.000 habitantes (intervalo de confiança de 95%, 19,7 a 34,6/100.000). Entre os casos de EM, 42 (80,8%) eram mulheres, (razão de sexos: 4,2:1). Quarenta e seis casos (88,5%) foram categorizados como EM remitente-recorrente, e os 6 casos restantes como EM secundariamente progressiva (11,5%). As demais características epidemiológicas e clínicas foram comparáveis a populações de EM internacionais. Conclusões: A prevalência de EM em Passo Fundo é uma das maiores já relatadas no Brasil. Estudos em outras cidades-chave brasileiras, usando a mesma metodologia, estão em andamento.

ABSTRACT Background: To improve the comparability of multiple sclerosis (MS) prevalence across Brazilian regions, the Brazilian Committee for Treatment and Research in MS has implemented a standardized approach to assess the prevalence of the disease in five key cities, which were deemed representative of their regions in terms of socio-geographical features and where in-person revision of each case was feasible. Objective: To report the point-prevalence of MS in Passo Fundo, one of the key cities in Southern Brazil. Methods: We sought to identify all MS patients who were living in Passo Fundo on July 1st, 2015. The primary source for case ascertainment was records from the offices of neurologists and neurosurgeons practicing in the city. Multiple secondary sources were used to maximize identification of cases. All patients underwent in-person review of the diagnosis by a panel of neurologists with experience in MS. Results: We identified 52 MS patients living in Passo Fundo on July 1st, 2015. The point-prevalence rate for MS was 26.4/100,000 population (95% confidence interval, 19.7 to 34.6/100,000). Among the MS cases, 42 (80.8%) were female, for a sex ratio of 4.2:1. Forty-six cases (88.5%) were categorized as relapsing-remitting MS, and the remaining 6 cases, as secondary progressive MS (11.5%). Other epidemiological and clinical features were comparable to national and international MS populations. Conclusions: The prevalence of MS in Passo Fundo is one of the highest reported in Brazil so far. Studies in other key Brazilian cities, using the same methodology, are currently being carried out.

RESUMO Objetivo Verificar os fatores anatomofisiológicos, psicológicos, socioculturais do binômio mãe neonato e sua associação com o início da prática do aleitamento materno. Método Estudo transversal realizado em uma Maternidade de Lima Peru. A amostra foi de 304 neonatos sadios e suas respectivas mães. O desempenho na amamentação foi estimado por avaliação clínica utilizando-se a escala de Avaliação Clínica da Eficácia da Amamentação e a autopercepção materna pela Escala de Autoeficácia da Amamentação. Razões de Prevalência (RP) multivariadas foram estimadas pela Regressão de Poisson com Variância Robusta e intervalos de confiança (IC) de 95%. Resultados A prevalência de baixo desempenho clínico na amamentação foi de 27,6%. Primíparas associarem-se a maior prevalência de baixo desempenho quando não confiavam em ter sucesso [RP:2,02(IC95%:1,18-3,44)] e menor prevalência em ter boa pega [RP:0,52(IC95%:0,29-0,95)], assim como em enfrentar com êxito [RP:0,59(IC95%:0,37-0,91)]. As multíparas, apresentaram maior prevalência quando não confiavam em manter-se motivadas [RP:3,47(IC95%:1,67-7,22)] e em acalmar o neonato [RP:4,07(IC95%:1,83-9,95)]. Houve menor prevalência em manter o neonato acordado [RP:0,32(IC95%:0,14-0,75)] e quando não se sentiam confiantes na presença de seus familiares [RP:0,29(IC95%:0,13-0,64)]. Conclusão É importante que profissionais da saúde estejam atentos às questões emocionais, sociais e culturais para a promoção de um aleitamento materno com qualidade.

ABSTRACT Purpose To verify the anatomophysiological, psychological, and sociocultural factors of the mother-newborn binomial, as well as their association with the initiation of breastfeeding. Methods Cross-sectional study conducted in a maternity hospital in Lima, Peru. The sample consisted of 304 healthy neonates and their mothers. Breastfeeding performance was estimated by clinical assessment using the Clinical Evaluation of Breastfeeding Efficacy scale and maternal self-perception by the Breastfeeding Self-Efficacy Scale. Multivariate Prevalence Ratios (PR) were estimated by Poisson Regression with Robust Variance and 95% confidence intervals (CI). Results The prevalence of clinical low breastfeeding performance was 27.6%. Primiparous women were associated with higher prevalence of low performance when they did not trust to succeed [PR:2.02(95%CI:1.18-3.44)] and lower prevalence in having a good latch [PR:0.52(95%CI:0.29-0.95)], as well as in coping successfully [PR:0.59(95%CI:0.37-0.91)]. Multiparous women showed higher prevalence when they were not confident in staying motivated [PR:3.47(95%CI:1.67-7.22)] and in calming the neonate [PR:4.07(95%CI:1.83-9.95)]. There was lower prevalence in keeping the neonate awake [PR:0.32(95%CI:0.14-0.75)] and when they did not feel confident in the presence of their family [PR:0.29(95%CI:0.13-0.64)]. Conclusion It is important that health professionals be aware of emotional, social, and cultural issues to promote quality breastfeeding.

RESUMO: O fio cirúrgico não metálico com maior resistência à tração é o poliéster nº 5. Comparando a sua resistência com a do ligamento cruzado cranial de cães e do tendão gastrocnêmico canino até suas rupturas, é possível perceber uma resistência consideravelmente inferior da parte do implante. Com objetivo de alcançar grandes resistências mecânicas a partir de fios de sutura, estes foram agrupados e torcidos, resultando em uma corda cirúrgica ajustável de acordo com a necessidade do paciente. Foram analisadas a metodologia de confecção, a conformação final e a resistência à tração, baseados em três modelos de fios: “A” (poliglactina 910 nº 1), “B” (poliéster nº 1) e “C” (poliéster nº 5). Considerando as médias de espessura e carga final, os implantes “B” obtiveram menores valores do que “A”, indicando que cordas de poliglactina 910 são mais resistentes que as de poliéster. Ademais, os implantes “C” resultaram nos maiores valores de carga e espessura, indicando que a espessura final é preditiva para resistência à tração. O poliéster nº 1 foi o único a gerar regressão linear para carga suportada, garantindo o incremento de 25,34 Newtons (N) a cada fio acrescido ao implante, o que sugere seu uso para criação de cordas não absorvíveis. A utilização da poliglactina 910 nº 1 promove médias de carga superiores ao poliéster nº 1, portanto, é indicada para confecção de cordas cirúrgicas absorvíveis.

ABSTRACT: The most tension resistant, nonmetallic, surgical suture is 5 polyester. Comparing it’s resistance to those of the dogs’ cranial cruciate ligament and the canine gastrocnemius tendon’s up until they rupture, it’s possible to detect a considerably inferior resistance on the implant’s part. With the goal of achieving high mechanical resistances from sutures, these were grouped and twisted, resulting in a surgical rope that is adjustable to the patients needs. We analyzed manufacturing methodology, final conformation and tension resistance, based on three sutures models: “A” (1 polyglactin 910), “B” (1 polyester) and “C” (5 polyester). Considering averages of thickness and final load, the “B” implants obtained lower values than “A”, indicating that polyglactin 910 ropes are more resistant than those of polyester. Moreover, the “C” implants resulted in the highest values of load and thickness, indicating that final thickness is predictive towards tension resistance. Size 1 Polyester was the only one to generate linear regression for supported load, ensuring the 25,34 Newtons (N) increment to every suture added to the implant, which suggests its use for the creation of non absorbable ropes. The use of 1 polyglatctin 910 promotes load averages superior to 1 polyester;therefore, it is indicated for making absorbable surgical ropes.

RESUMO Introdução: embora a preservação do trânsito intestinal seja um dos objetivos principais na cirurgia do câncer retal, a anastomose colorretal pode ser considerada um procedimento de altíssimo risco, particularmente para pacientes com múltiplas comorbidades. Nosso objetivo foi avaliar as taxas de complicações cirúrgicas em pacientes com câncer retal de acordo com o tipo de procedimento a que foram submetidos. Materiais e Métodos: esta coorte incluiu todos os pacientes com câncer retal submetidos a ressecção eletiva em hospital universitário de referência ao longo de 16 anos. Houve três grupos de estudo de acordo com o tipo de operação realizada: (1) ressecção retal com anastomose, sem estoma desfuncionalizante (ED); (2) ressecção retal com anastomose e ED; e (3) procedimento de Hartmann (PH). Avaliamos as complicações pós-operatórias e os resultados clínicos. Resultados: estudamos 402 pacientes. O grupo 3 tinha 118 pacientes, estes sendo significativamente mais idosos (>10 anos), com pontuações mais altas no Índice de Comorbidade de Charlson e mais frequentemente classificados como ASA ≥ 3 do que os pacientes dos outros dois grupos. Sessenta e sete pacientes (16,7%) apresentaram complicações de Clavien-Dindo grau ≥ III, correspondendo à incidência de 11,8%, 20,9% e 14,4% nos grupos 1, 2 e 3, respectivamente (p = 0,10). Vinte e nove pacientes (7,2%) apresentaram complicações sépticas graves, necessitando reoperação, com incidência de 10,8%, 8,2% e 2,5% nos grupos 1, 2 e 3, respectivamente (p = 0,048). Vinte e um por cento dos pacientes do grupo 2 não foram submetidos ao fechamento do estoma após acompanhamento de 24 meses. Conclusão: o PH foi associado à menor incidência de reoperação por complicações sépticas intra-abdominais. Este procedimento continua sendo uma opção para pacientes com alto potencial de desenvolver complicações cirúrgicas graves.

ABSTRACT Background: although preservation of bowel continuity is a major goal in rectal cancer surgery, a colorectal anastomosis may be considered an unacceptably high-risk procedure, particularly for patients with multiple comorbidities. We aimed to assess rates of surgical complications in rectal cancer patients according to the type of procedure they had undergone. Materials and Methods: this cohort included all rectal cancer patients undergoing elective resection at a referral academic hospital over 16 years. There were three study groups according to the type of performed operation: (1) rectal resection with anastomosis without defunctioning stoma (DS); (2) rectal resection with anastomosis and DS; and (3) Hartmann’s procedure (HP). Postoperative complications and clinical outcomes were assessed. Results: four-hundred and two patients were studied. The 118 patients in group 3 were significantly older (>10 years), had higher Charlson Comorbidity Index scores, and more ASA class ≥3 than patients in the other two groups. Sixty-seven patients (16.7%) had Clavien-Dindo complications grade ≥ III, corresponding to an incidence of 11.8%, 20.9%, and 14.4% in groups 1, 2, and 3, respectively (p=0.10). Twenty-nine patients (7.2%) had major septic complications that required reoperation, with an incidence of 10.8%, 8.2% and 2.5% in groups 1, 2 and 3, respectively (p=0.048). Twenty-one percent of the group 2 patients did not undergo the stoma closure after a 24-month follow-up. Conclusion: HP was associated with a lower incidence of reoperation due to intra-abdominal septic complications. This procedure remains an option for patients in whom serious surgical complications are anticipated.
