ABSTRACT This study aims to report the diagnostic course and treatment of a fast-growing mycobacteria infection after cesarean delivery. We report the case of a 37-year-old woman admitted to the Infectious Diseases’ Clinic of the Hospital das Clinicas da Universidade Federal de Pernambuco, Pernambuco State, Brazil, four months after a cesarean section, presenting with healing difficulties and located pain outside the surgical site. The first diagnosis was a probable rejection of the sutures that were not absorbed, but based on the clinical signs, reported history, complementary laboratory tests and no response to treatment with the conventional antibiotic therapy (cephalosporins/quinolones) prescribed, the ultimate diagnosis was a mycobacteriosis caused by Micobacterium fortuitum. Since fast-growing mycobacteria do not easily penetrate host tissues, they is mainly related to post-trauma or post-surgical procedures. It is extremely important to call attention to these occurrences in the gynecological-obsthetric field. Treatment for mycobacteriosis is often complicated because of the side effects of antibiotics, especially the ototoxicity of amikacin.

OBJETIVO: A maioria das neoplasias malignas de cabeça e pescoço é diagnosticada em estágios avançados. O alto risco de desenvolver um segundo tumor primário (STP) diminui a taxa de sobrevida dos pacientes. STP piora significativamente o prognóstico e estudos sobre ele devem ser realizados para se descobrir seus fatores de risco e melhor forma de tratamento. O principal objetivo deste estudo foi analisar a ocorrência de STP em pacientes com carcinoma epidermóide bucal (CEB) tratados no Hospital Erasto Gaertner (HEG), Curitiba, PRP, Brasil, num período de 16 anos. METODOLOGIA: Um estudo retrospectivo foi realizado a fim de revisar os pacientes com CEB que desenvolveram STP entre Janeiro de 1990 e Dezembro de 2005. Os dados demográficos e clínicos foram coletados através dos prontuários médicos dos pacientes e foram analisados por estatística descritiva. RESULTADOS: Neste período, 34.637 pacientes foram admitidos no HEG. Um total de 1637 (4,4%) pacientes recebeu diagnóstico de CEB, dos quais 37 (2,7%) desenvolveram STP. Os pacientes que desenvolveram STP eram 29 (78,4%) homens e 8 (21,6%) mulheres, com média de idade de 58 anos. Os sítios anatômicos mais frequentes para o STP foram a boca, orofaringe e esôfago; a taxa de sobrevivência foi de 27%. CONCLUSÃO: Pacientes tratados de CEB devem ser examinados cuidadosamente e monitorados regularmente por causa do alto risco de desenvolver um STP, especialmente aqueles que mantêm os hábitos de tabagismo e etilismo.

PURPOSE: Most head and neck malignant neoplasias are diagnosed in advanced stages. Another threatening element to the patients' survival chances and quality of life is the high risk of developing a second primary tumor (SPT). SPT significantly worsens prognosis, and for that reason patients must be monitored for early diagnosis. The main objective of this study was to analyze the occurrence of SPTs in patients with oral squamous cell carcinoma (OSCC) treated in Erasto Gaertner Hospital (EGH), Curitiba, PR, Brazil, in a period of 16 years. METHODS: Design: retrospective study. The sample comprised patients with OSCC who developed SPT between January 1990 and December 2005. Demographic and clinical data were recorded form the patients' medical charts and analyzed with descriptive statistics. RESULTS: During this period, 34,637 patients were admitted at EGH. A total of 1,637 (4.4%) patients were diagnosed with OSCC, and 37 (2.7%) developed a SPT. Patients who developed SPT were 29 (78.4%) male and 8 (21.6%) female, with a median age of 58 years old. The most frequent anatomical SPT site was the mouth, oropharynx and oesophagus, with an overall survival of 27%. CONCLUSION: Patients treated from OSCC must be carefully monitored because of the increased risk of SPT, especially when there is a continuous history of tobacco and alcohol consumption.

OBJETIVO: Quantificar as fístulas após cirurgia de câncer da cavidade oral e identificar fatores de risco. MÉTODOS: Estudo retrospectivo, interessando pacientes submetidos à cirurgia. Seguimento pós-operatório mínimo de dois anos. Variáveis estudadas: sexo, comorbidades, tabagismo, etilismo, risco anestésico e pulmonar, estadiamento clínico, linfadenectomia cervical, tratamento radioterápico, acidentes cirúrgicos, infecção ou deiscência de ferida operatória, seroma ou hematoma de sítio cirúrgico, infecção respiratória no pós-operatório, tipo de cirurgia e reconstrução realizadas. RESULTADOS: Estudados 159 pacientes. Ocorreu fístula orocutânea em 30,3% (48 pacientes). Pacientes T3 tiveram fístula em 16% dos casos, T4 em 40,3% e naqueles estádio T1 ou T2, 26,6% e 1,8% respectivamente (p=0,0138). Os casos N+ evoluíram com fístula em 22.9% (N2c com 42,8%, p=0,0136), os com radioterapia pré-operatória em 63,6% (p=0,0346). Aqueles com infecção de sítio cirúrgico em 47,3% (p=0,0146) e aqueles com deiscência de ferida operatória em 53,7% (p=0,0030). O índice de fístula foi de 60% nos retalhos regionais mucocutâneos, de 39,2% nos miocutâneos e de 12,5% com retalho microcirúrgico (p=0,0286). CONCLUSÃO: O índice de fístulas foi de 30,3%. Foram estatisticamente significativos para ocorrência de fístulas: estádio T, linfadenectomia cervical bilateral, radioterapia pré ou pós-operatória, infecção e deiscência de ferida operatória, e o uso de retalhos para reconstrução.

OBJECTIVE: To quantify the oral cutaneous fistulae after surgery and to identify possible risk factors. METHODS:A retrospective study, interesting patients that were submitted to surgery, with a two years minimum post-operative follow up. The considered variables were: sex, concomitant diseases, tabacco and alcohol use, the anesthesic and pulmonary risks, clinical stage, cervical linphadenectomy, pre or postoperative radiotherapy, accidents during the surgery, wound infection and or hematoma, pulmonary infection, surgery and reconstruction extension. RESULTS: In 159 patients, oral cutaneous fistulae occurred in 48 patients (30,3%): Patients stage T1 in 26,6 %,T2 in 1,8 %,T3 in 16%, and T4 in 40,3% (p=0,0138). The cases N+ developed fistulae in 22.9%, (N2c with 42,8%, (p=0,0136), those with preoperative radiotherapy in 63,6% (p=0,0346) Those with wound infection in 47,3% (p=0,0146), and those with wound deiscense in 53,7 % (p=0,0030). The fistulae rate was of 60% in the regional mucocutaneous flaps reconstruction cases, 39,2% in the myocutaneous ones and 12,5% of microsurgery ones (p=0,0286). CONCLUSION: The general rate of oral cutaneous fistulae was 30,3%. The significant factors were: T stage, cervical linphadenectomy, pre or postoperative radiotherapy, wound infection and deiscense, and the use of flaps.

O carcinoma bucal é o sexto tipo mais comum de câncer no mundo e o sétimo no Brasil, onde ocorre a maior incidência da América Latina. A sobrevida média de aproximadamente cinco anos permanece como uma das menores entre os principais cânceres, justificando estudos que auxiliem no delineamento de estratégias de prevenção. Este estudo objetivou avaliar em uma amostra de 91 pacientes portadores de carcinomas bucais características epidemiológicas; fatores de risco, clínicos e histopatológicos. A média de idade foi de 58,62 ± 10,46 anos e a razão sexual de 6,6:1,0 (79 homens e 12 mulheres). A etnia euro-descendente foi predominante com 79 (86,8%) pacientes. Oitenta e cinco (93,4%) indivíduos eram tabagistas e 70 (76,9%) etilistas. As localizações anatômicas prevalentes foram: 27 tumores (29,7%) de língua; 18 (19,8%) de assoalho; 11(12,1%) de orofaringe e 11 (12,1%) de mucosa. Cinqüenta e sete (62,6%) pacientes apresentaram os linfonodos comprometidos e três apresentaram (3,3%) metástases à distância. A maioria dos pacientes (43,2%) recebeu tratamento cirúrgico e radioterápico. Com exceção da proporção sexual, nossos dados concordam com os freqüentemente descritos para portadores de carcinomas bucais.

Mean five-year survival remains one of the lowest among the main cancers, thus justifying studies that contribute to the development of preventive strategies. The aim of this study was to compare the epidemiological, clinical, and histological characteristics of 91 patients with oral carcinoma. Mean age was 58.62 ± 10.46 years, and male-to-female ratio was 6.6:1.0 (79 men and 12 women). European descendants predominated with 79 patients (86.8%). Eighty-five individuals (93.4%) smoked and 70 (76.9%) consumed alcohol regularly. Anatomical distribution of tumors was: 27 (29.7%) tongue; 18 (19.8%) floor of mouth; 11 (12.1%) oropharynx; and 11 (12.1%) oral mucosa. Fifty-seven patients (62.6%) presented lymph node involvement and three (3.3%) had distant metastases. Surgery and radiotherapy were used in 43.2% of patients. With the exception of the male/female ratio (which was higher), our data are consistent with previous studies on oral carcinoma patients.

OBJETIVO: Descrever as dificuldades da endoscopia de contato aplicada à boca e ao lábio (microestomatoscopia de contato) e aplicar nas lesões do lábio os critérios usados na endoscopia de contato de outras topografias. MÉTODO: Estudo prospectivo utilizando o endoscópio de contato Storz 8715A e o corante azul de toluidina. Vinte exames foram feitos para aprendizado da técnica e depois mais 31 (21 lesões do lábio e 10 da boca), todos exames in vivo. Os critérios avaliados foram: Celularidade, Arquitetura, Estratificação e Morfologia. RESULTADOS: A falta de nitidez, os tremores finos e o deslizamento do aparelho dificultaram o procedimento em 100% dos casos, mas permitiram um exame adequado. Os 13 diagnósticos de malignidade das lesões do lábio apresentaram todos os critérios alterados. CONCLUSÃO: as dificuldades encontradas foram o contato do aparelho com a superfície anatômica, o deslizamento do aparelho, os tremores finos e a falta de nitidez da imagem, que não inviabilizaram a realização e a interpretação do exame. Os critérios são aplicáveis às lesões da boca e do lábio inferior.

BACKGROUND: 1- To describe difficulties of contact endoscopy related to mouth and lip (contact microstomatoscopy) and 2- apply the criteria used in contact endoscopy of other topographies when lip lesions are diagnosed. METHODS: A prospective study was performed with the contact endoscope Storz 8715A and toluidine blue was used. Twenty examinations were accomplished in order to learn the technique; after that, other 31 were included (21 lip lesions and 10 oral lesions), all in vivo examinations. The criteria assessed were: cellularity, architecture, stratification and morphology. RESULTS: Lack of image clearness, slight tremors and sliding of the equipment made the procedure difficult in 100% of the cases; even though, accomplishment of proper examination was obtained. The 13 cases of lip malignancy lesions had the criteria altered as a whole. CoNCLUSION: Difficulties found were the contact of the equipment with anatomic surface, the sliding of the equipment, the slight tremors, and the lack of image clearness. Nevertheless, such elements did not interrupt the accomplishment and the examination analysis. The criteria are applied to oral and lower lip lesions.

Doença de Madelung ou lipomatose simétrica múltipla é uma doença rara, que é caracterizada por acúmulo de gordura não encapsulada, geralmente localizada simetricamente ao redor do pescoço e ombros. A etiologia é desconhecida, mas é freqüentemente observada em pacientes com história de alcoolismo crônico e é acompanhada por anormalidades metabólicas. Os autores apresentam um caso de lipomatose cervical associada a polineuropatia e alcoolismo. A remoção cirúrgica do tecido adiposo por lipectomia ou lipoaspiração é terapia paliativa, já que ele não é encapsulado, a recorrência é comum.

Madelung's disease or multiple symmetric lipomatosis is a rare disease, that is characterized by non-encapsulated accumulation of fat, generally located symmetrically around the neck and shoulders. The aetiology is unknown, but is often seen in patients with a history of chronic alcoholism and it is accompanied by metabolic abnormalities. The authors present a case of cervical lipomatosis association with polineuropathy and alcoholism. The surgical removal of the fatty tissue by lipectomy or liposuction is palliative therapy, as it is unencapsulated, recurrence is common.

Os estudos publicados quanto aos exames pré-operatórios mostram-se controversos e poucos abordam os riscos especificamente em cirurgias de cabeça e pescoço. O objetivo deste estudo foi identificar fatores de risco em cirurgia de cabeça e pescoço para, a partir destes, estabelecer uma rotina pré-operatória racionalizada que permita identificar o grupo de pacientes mais afetados e diminuir as taxas de complicação e mortalidade. Foram avaliados em 17 meses, 382 pacientes submetidos a cirurgia de cabeça e pescoço por neoplasias benignas ou malignas. Os fatores avaliados no estudo foram: a idade, tratamento prévio à cirurgia, o estado geral, os riscos anestésico, cardíaco e pulmonar, o potencial de contaminação, o porte e o caráter da cirurgia em relação às taxas de complicação e mortalidade. Após a análise estatística foi realizada regressão logística até se obter uma fórmula numérica, na qual se incluíram os fatores mais significativos. Encontramos em nosso serviço uma taxa de morbidade de 27,2% e uma mortalidade de 3,9%. As principais complicações foram infecção pulmonar e infecção da ferida operatória. Através dos resultados observamos que os fatores estado geral, porte e caráter da cirurgia foram significativos quanto à morbidade. Quanto à mortalidade foram significativos a idade, o estado geral e o porte cirúrgico. Isto nos permite uma abordagem pré-operatória, atuando especificamente sobre estes fatores ou suas implicações com a intenção de diminuir as taxas de morbidade e mortalidade.

The published studies about the pre-surgery examinations show themselves in controversy, and only a few of the studies mention the risks of head and neck surgery. The objective of this report was to identify the risk factors on head and neck surgeries, and after that establish a rationalized pre-operative routine that allows us to identify the most and the least affect group ofpatients. In the period of17 months, 382 patients were submitted to head and neck surgery because ofmalign and benign neoplasms. The evaluatedfactors were: the age, the previous treatment to surgery, the general aspect, the cardiac, the pulmonary and anaesthetic risk, the potential of contamination, the port and the character of the surgery related to the complication and mortality rufes. After the statistical analysis a logistical regression was realized to obtain the numeric formula, in which was included the most significantfasctors. We havefounded in our service, a morbidity rale of27,2% and a mortality rufe of 3,9%. The Mo main complications were: pulmonary infection and infection ofthe operative wound. We could observe through the results that the general aspect, the surgical port and character were significant in relation to the morbidity. In relation to the mortality the age, the general aspect and the surgical port were significant. This conclusion allows us to obtain a pro-surgical approach. that will act specifically on the factors or implications, with the intention of decreasing the mortality and morbidity rates.

O retalho nasogeniano com pedículo inferior tem sido utilizado, por via transbucal (supramandibular), na reconstrução do soalho da boca. A descrição da técnica de confecção do retalho nasogeniano musculocutâneo em ilha por via submandibular e a viabilidade do mesmo, tendo-se como parâmetro a necrose do retalho, são os objetivos deste estudo. Complementarmente, fez-se uma avaliação da mobilidade da língua, da capacidade de articulação dos fonemas orais no pós-operatório e procurou-se identificar através de análise estatística os fatores que pudessem estar correlacionados com a necrose do retalho. Foram operados nove pacientes considerados como T2, cinco como T3 e três como T4, sendo que todos foram submetidos a algum tipo de esvaziamento cervical. Dos dezessete retalhos, um teve necrose total, e outro, necrose parcial, proporcionando uma viabilidade de 88,3%. Em nenhum dos casos ocorreu fístula orocutânea. A avaliação da mobilidade da língua e da capacidade de articulação dos fonemas orais demonstrou resultados próximos do normal.

Even though not supplying the same quantity of tissue as the frontal, deltopectoral and cervical flaps, the nasolabial flap is an excelent option for the correction of defects of small to moderate extension because of its spread, tissue composition and safety in relation to its blood nutrition. The present study tries to demonstrate the surgical technique used and the results obtained in seventeen nasolabial musculocutaneous submandibular island flaps used for mouth floor reconstruction after exeresis of malign tumors of the lower floor, in the period from December 1990 to July 1995, in the Service of Surgery of Head and Neck of Erasto Gaertner Hospital, in Curitiba. The basic objectives were to reconstruct the continuity of the buccal mucous membrane and preserve the mobility of the tongue (anterior 1/3) to the maximum. Only one out of the seventeen surgery cases is female and the predominant age range is within the sixties. The first surgery occurred in December 1990. One patient presented Zubrod II and the others Zubrod I. As to staging, there were 9 T2 cases, 5 T3 and 3 T4, 12 N0, 4 N2A and 1 N3. In relation to the histological type, one case of fibrosarcoma and sixteen cases of spinous cellular carcinoma were verified. One of the cases developed with total necrosis and another with partial necrosis, due to irrigation and draining respectively. In seven cases hair growth (beard) occurred because of the lower prolongament of the flap. The cases with infection (six) were due to necrosis of flap (two), cervical lymphadenectomy (two) and proximity of tracheostomy. In none of the cases cervical fistula was present. The main conclusions taken from the acquired experience after seventeen confectionated flaps are (a) that the surgical performance of the technique is the domain of the surgeon of head and neck, (b) that the viability of the flap, considering the occurrence of necrosis in it, was of 88.3%, (c) that the incidence of orocutaneous fistula was null, despite the presence of necrosis and infection in some cases, and (d) that the mobility of the tongue and the capacity to articulate oral phonemes comes close to normal, mainly if the primary tumor affects basically the mouth floor.
