FUNDAMENTOS: Métodos moleculares têm-se mostrados mais eficazes para o diagnóstico da LTA. OBJETIVOS: Comparar os resultados da intradermorreação de Montenegro (IRM), presença de leishmania em biópsia (Bc), reação de imunofluorescência indireta (Rifi), seqüenciamento de DNA e PCR-RFLP (-restriction fragment lenght polymorphism) para o diagnóstico da LTA. MÉTODOS: Foram estudados 152 pacientes com LTA. Para a PCR em Bc, utilizaram-se primers específicos para seqüência de 120bp do kDNA do minicírculo comum a todas as espécies de leishmanias. O produto da PCR, utilizado para seqüenciamento e para restrição enzimática com Hae III, foi comparado às culturas L. (L.) amazonensis e L. (V.) braziliensis. RESULTADOS: Houve predomínio do sexo masculino (75%), da cor branca (80%) e da profissão urbana (48%). A idade variou de três a 77 anos, com 56,5% entre 21 e 50 anos. 65,8% eram do Estado de São Paulo, prevalecendo a forma cutânea (79,6%). A IRM foi positiva em 73,4%, e a Rifi em 59,7%, enquanto a Bc evidenciou presença de leishmania em 30,6%. A PCR foi positiva em 81,6%, e a PCR-RFLP identificou L. (V.) braziliensis como espécie predominante (66%), o que também ocorreu com o seqüenciamento. Comparando PCR-RFLP e seqüenciamento, houve 61% de concordância entre os resultados, mostrando significância da PCR-RFLP para L. (V.) braziliensis. CONCLUSÕES: A IRM e a PCR foram estatisticamente equivalentes como métodos subsidiários para o diagnóstico da LTA, a PCR-RFLP e o seqüenciamento também o foram na identificação das espécies de leishmania, o primeiro apresentando menores custo e tempo de execução comparado ao seqüenciamento de DNA.

BACKGROUND: ATL is endemic in Brazil, and molecular methods have been shown more effective for its diagnosis. OBJECTIVES: Our purpose was to compare the results of Montenegro’s skin reaction (MR), presence of leishmania in skin biopsy (Bx), indirect immunofluorescence (IIF) for leishmania in sera samples, PCR, sequencing of DNA and PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) as subsidiary exams for ATL diagnose. METHODS: 152 patients were studied, with accomplishment of MR, Bx, IIF and PCR for leishmania in skin samples. For PCR, a specific pair of primers for a sequence of 120bp from kDNA minicircle, common to all leishmanias species, was used. The product of PCR was used for DNA sequencing and for RFLP with the enzyme Hae III. The analysis of the restriction pattern was compared to the cultures of L. (L.) amazonensis and L. (V.) braziliensis. RESULTS: The predominant sex was male (75%), the white color (80%) and urban professional occupation (48%). The age varied from 3 to 77 years, with prevalence from 21 to 50 years (56.5%). In relation to the origin, 65.8% was from the state of São Paulo, being the cutaneous form the more prevalent (79.6%). MR was positive in 73.4% and there was presence of leishmania in 30.6% of the samples, while IIF presented 59.7% of positivity. PCR was positive in 81.6% of skin samples, and PCR-RFLP identified L. (V.) braziliensis (66%) as predominant species, fact that also happened with DNA sequencing, with 64.4% of the positive results for L. (V.) braziliensis. Comparing PCR-RFLP and DNA sequencing there was 61% of agreement amongst the results, being significant in PCR-RFLP for L. (V.) braziliensis. CONCLUSION: MR and PCR were equivalent as subsidiary methods for the diagnosis of ATL, such as PCR-RFLP and DNA sequencing in the identification of the leishmania species. On the other hand, the method PCR-RFLP presented smaller cost and smaller time of execution compared to the sequencing of DNA.

FUNDAMENTOS: AA proteína MAX, componente salivar de Lutzomyia longipalpis, vetor do calazar ou leishmaniose sistêmica, tem sido empregada como vacina para leishmaniose tegumentar experimental, com funções de vasodilatação e imunomodulação. OBJETIVOS: Detectar anticorpos séricos antiMAX em pacientes com LTA e verificar a expressão de MAX em L. neivai, vetor da LTA na região em estudo. MÉTODOS: Anticorpos antiMAX foram detectados por Elisa em soro de 42 pacientes com LTA e 63 controles. A extração de proteínas e de DNA de L. longipalpis (controle positivo) e de L. neivai foi realizada pelo método Trizol e seguida pela detecção de proteínas por eletroforese e pela expressão gênica de MAX por PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) com as enzimas Hha I e Rsa I. RESULTADOS: Títulos maiores de anticorpos antiMAX foram observados na LTA (p=0,0132). A eletroforese de proteínas mostrou frações semelhantes para L. longipalpis e L. neivai, tendo-se observado para ambos fração protéica de peso molecular similar à proteína MAX. A expressão gênica de MAX em L. longipalpis e L. neivai foi confirmada por PCR-RFLP. CONCLUSÕES: A presença de antiMAX nos grupos em estudo tornou imprescindível a pesquisa de MAX no vetor de LTA da região, tendo sido registrada pela primeira vez expressão protéica e gênica de MAX em L. neivai. Detecção de antiMAX em controles confirma exposição a picadas de flebótomos. Títulos de anticorpos antiMAX maiores na LTA sugerem exposição prévia e natural à picada e, conseqüentemente, à proteína MAX, não protegendo da doença e desfavorecendo seu emprego em vacinação.

BACKGROUND: The protein MAX, salivary component of Lutzomyia longipalpis, vector of calazar or systemic leishmaniasis, has been used as vaccine for experimental tegumentar leishmaniasis, which vasodilatory and immunomodulatory functions are described. OBJECTIVES: Our purpose was to detect antibodies antiMAX in sera samples from patients with ATL and to verify the genetic and protein expression of MAX in L. neivai, phlebotomy vector of ATL in the area of study. METHODS: Antibodies antiMAX were detected by ELISA in sera from 42 patients with ATL and 63 controls. The extraction of proteins and of DNA from L. longipalpis (positive control) and L. neivai was accomplished by the method Trizol, following for the detection of proteins by electrophoresis, and genetic expression of MAX by PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) with the enzymes Hha I and Rsa I. RESULTS: Increased titles of antibodies antiMAX were observed in ATL, compared to controls (p=0,0132). Electrophoreses of proteins showed similar fractions for L. longipalpis and L. neivai, and for both a protein fraction with molecular weight similarity to MAX was observed. The genetic expression of MAX in L. longipalpis and L. neivai was confirmed by PCR-RFLP. CONCLUSIONS: The description of antibodies antiMAX in ATL patients and controls turned indispensable the research of MAX in the phlebotomy vector of ATL in the area of study. For the first time, it was registered protein and genetic expression of MAX in L. neivai. The antiMAX detection in controls confirms the previous exposition to prick of phlebotomies. Increased titles of antibodies antiMAX in ATL patients suggest previous and natural exposition to the bite and, consequently, to the protein MAX, not protecting them of disease and discouraging its employment in vaccination.

FUNDAMENTOS: O pênfigo foliáceo endêmico (PFE) é doença bolhosa auto-imune, em que o Estado de São Paulo (SP) destacou-se por sua alta incidência na década de 1940, a partir da qual é descrito seu declínio. OBJETIVOS: relatar casuística do PFE na região nordeste de SP. MÉTODO: levantamento de prontuários de pacientes com doenças bolhosas, Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto, Universidade de São Paulo, 1973 a 1998, anotando-se aspectos demográficos e epidemiológicos. RESULTADOS: Do total de 340 pacientes com doença bolhosa, em 245 foi diagnosticado PFE (72,1%), 9,4 casos/ano. Desses, 60,4% eram mulheres e 70,2%, brancos, de 7 a 82 anos de idade (29,4% entre 10 e 19 anos); 46,9% eram de zona rural. Predominou a profissão doméstica entre as mulheres (67,6%), e lavrador entre os homens (40,2%). Em 62,0%, a duração da doença foi menor que 1 ano; em 27,0%, entre 1 e 5 anos, e, em 11,0%, maior que 5 anos. 36,7% eram procedentes da Direção Regional de Saúde (DIR) XVIII de Ribeirão Preto, sendo que os municípios de Batatais e de Ribeirão Preto apresentaram maior número de casos: 33,3% e 23,3%, respectivamente; 22,0% da DIR XIII (Franca); 13,5% da DIR VII (Araraquara); 2,9% da DIR IX (Barretos); 4,1% de outras DIRs de SP, e 20,8% de outros Estados (16,7% de MG). Treze (5,3%) pacientes relataram PFE em algum familiar, e 4 (1,6%), em vizinhos. CONCLUSÕES: Os dados confirmam a região nordeste de SP como foco endêmico remanescente de PFE.

Endemic Pemphigus Foliaceus (EPF) is a bullous autoimmune skin disease whose incidence used to be high in the State of São Paulo (SP), Brazil, during the forties, but has declined thereafter. OBJECTIVES: to report a series of EPF patients from the northeastern region of SP. METHODS: a retrospective study concerning demographic and epidemiological data of patients seen from 1973 to 1998 was conducted at the University Hospital, Faculty of Medicine of Ribeirão Preto, SP. RESULTS: bullous disease was diagnosed in 340 patients, 245 with EPF (72.1%), 9.4 cases per year, 60.4% females, and 70.2% white, 7 to 82 year-old (29.4% in their teens); 46.9% lived in the rural zone. Concerning profession, housewives predominated among women (67.6%) and agricultural workers among men (40.2%). The time of disease was less than 1 year in 62.0% of cases, followed by 1 and 5 years (27%), and more than 5 years for the remaining patients (11%). 36.7% of patients were referred by the Direção Regional de Saúde (DIR) XVIII of Ribeirão Preto, with the largest number of cases being from Ribeirão Preto and Batatais: 33.3% and 23.3%, respectively; 22% from DIR XIII (Franca); 13.5% from DIR VII (Araraquara); 2.9% from DIR IX (Barretos); 4.1% from other DIRs of SP, and 20.8% from other States (16.7% from Minas Gerais). Thirteen (5.3%) patients reported occurrence of the disease in some relative, and 4 (1.6%) in neighbors. CONCLUSIONS: the present data characterize the northeastern region of the state of São Paulo as a remaining endemic focus of EPF.
