ABSTRACT: This study described the pathological aspects of a metastatic grade II simple tubulopapillary mammary carcinoma in a free-living senile female maned wolf (Chrysocyon brachyurus). The diagnosis was based on histopathological findings of malignant epithelial cells arranged in tubular and papillary patterns, with marked anisocytosis and anisokaryosis and metastases to regional lymph nodes, lungs, parietal pleura, and pericardium. Also associated to positive immunolabeling of neoplastic cells for Cytokeratin AE1/AE3 and negative for P63, Calponin and Cox-2, added to the cell proliferation index of 26.8%. The findings demonstrated that free-ranging wild canids develop mammary neoplasms similar to those observed in domestic canids. RESUMO: Este estudo descreve os aspectos patológicos de um carcinoma mamário simples túbulo-papilar metastático grau II em um lobo-guará, fêmea, senil de vida livre. O diagnóstico foi baseado nos achados histopatológicos de células malignas epiteliais arranjadas em um padrão tubular e papilar, com acentuada anisocitose e anisocariose e metástases para os linfonodos regionais, pulmão, pleura parietal e pericárdio. Também associado a marcação imuno-histoquímica positiva das células neoplásicas para Citoqueratina AE1/AE3 e negativas para P63, Calponina e Cox-2, somado ao índice de proliferação celular de 26,8%. Os achados demonstram que canídeos silvestres de vida livre podem desenvolver neoplasias mamárias similares às observadas em canídeos domésticos. Aim This present study aims to compare the data from the Brazilian Unified Health System on the number of clinical consultations of Oral Medicine from the first 6 months (March-August 2020) of the COVID-19 pandemic in Brazil with the last 6 months (September-February 2020/2021) to update the data, verify the measures’ effectiveness to return clinical activities in the following months. Methods perform a literature review of recent articles that report the impact of the COVID-19 pandemic on Oral Medicine. Results There was an increase in the number of Oral Medicine clinical consultations in the second half of the pandemic throughout Brazil (+64.2%), representing over 9,235 appointments in this period. Conclusion measures for the return of health assistance and the practice of Telemedicine proved to be effective after the second period of the pandemic. Even so, strengthening security measures against the coronavirus is essential to ward off a new wave since the Omicron variant emerged in the country and, consequently, possible new lockdowns that might affect healthcare in Brazil. Abstract Objective To review current literature to support the use of mesna as a preventive therapy for hemorrhagic cystitis and bladder cancer in patients with systemic autoimmune diseases and systemic vasculitis treated with cyclophosphamide. Materials and methods The search for articles was conducted systematically through MEDLINE, LILACS, Cochrane Library, and Embase databases. Only articles in English were selected. For available records, titles and abstracts were selected independently by two investigators. Results Eighteen studies were selected for analysis. The known adverse effects of cyclophosphamide were hematological toxicity, infections, gonadal toxicity, teratogenicity, increased risk for malignancy and hemorrhagic cystitis. Long-term toxicity was highly dependent on cyclophosphamide cumulative dose. The risk of bladder cancer is especially higher in long-term exposure and with cumulative doses above 36 g. The risk remains high for years after drug discontinuation. Hemorrhagic cystitis is highly correlated with cumulative dose and its incidence ranges between 12 and 41%, but it seems to be lower with new regimens with reduced cyclophosphamide dose. No randomized controlled trials were found to analyze the use of mesna in systemic autoimmune rheumatic diseases and systemic vasculitis. Retrospective studies yielded conflicting results. Uncontrolled prospective studies with positive results were considered at high risk of bias. No evidence was found to support the use of mesna during the treatment with cyclophosphamide for autoimmune diseases or systemic vasculitis to prevent hemorrhagic cystitis and bladder cancer. In the scenarios of high cumulative cyclophosphamide dose (i.e., > 30 g), patients with restricted fluid intake, neurogenic bladder, therapy with oral anticoagulants, and chronic kidney disease, mesna could be considered. Conclusion The current evidence was found to be insufficient to support the routine use of mesna for the prophylaxis of hemorrhagic cystitis and bladder cancer in patients being treated for systemic autoimmune diseases and systemic vasculitis with cyclophosphamide. The use may be considered for selected cases. ABSTRACT Background: Alcoholic liver disease (ALD) and metabolic-dysfunction associated steatotic liver disease (MASLD) are common, and gut microbiota (GM) is involved with both. Here we compared GM composition in animal models of MASLD and ALD to assess whether there are specific patterns for each disease. Methods: MASLD model- adult male Sprague Dawley rats, randomized into two groups: MASLD-control (n=10) fed a standard diet; MASLD-group (n=10) fed a high-fat-choline-deficient diet for 16 weeks. ALD model- adult male Wistar rats randomized: ALD-control (n=8) fed a standard diet and water+0.05% saccharin, ALD groups fed with sunflower seed and 10% ethanol+0.05% saccharin for 4 or 8 weeks (ALC4, n=8; ALC8, n=8). ALC4/8 on the last day received alcoholic binge (5g/kg of ethanol). Afterwards, animals were euthanized, and feces were collected for GM analysis. Results: Both experimental models induced typical histopathological features of the diseases. Alpha diversity was lower in MASLD compared with ALD (p<0.001), and structural pattern was different between them (P<0.001). Bacteroidetes (55.7%), Firmicutes (40.6%), and Proteobacteria (1.4%) were the most prevalent phyla in all samples, although differentially abundant among groups. ALC8 had a greater abundance of the phyla Cyanobacteria (5.3%) and Verrucomicrobiota (3.2%) in relation to the others. Differential abundance analysis identified Lactobacillaceae_unclassified, Lachnospiraceae_NK4A136_group, and Turicibacter associated with ALC4 and the Clostridia_UCG_014_ge and Gastranaerophilales_ge genera to ALC8. Conclusion: In this study, we demonstrated that the structural pattern of the GM differs significantly between MASLD and ALD models. Studies are needed to characterize the microbiota and metabolome in both clinical conditions to find new therapeutic strategies. RESUMO Contexto: A doença hepática alcoólica (DHA) e a doença hepática esteatótica associada à disfunção metabólica (MASLD) são comuns, e a microbiota intestinal (MI) está envolvida em ambas. Aqui, comparamos a composição da MI em modelos animais de MASLD e DHA para avaliar se existem padrões específicos para cada doença. Métodos: Modelo de MASLD - ratos machos adultos da linhagem Sprague Dawley, randomizados em dois grupos: MASLD-controle (n=10) alimentados com uma dieta padrão; grupo MASLD (n=10) alimentados com uma dieta rica em gordura e deficiente em colina por 16 semanas. Modelo de DHA - ratos machos adultos da linhagem Wistar randomizados: DHA-controle (n=8) alimentados com uma dieta padrão e água+0,05% de sacarina; grupos DHA alimentados com semente de girassol e 10% de etanol+0,05% de sacarina por 4 ou 8 semanas (DHA4, n=8; DHA8, n=8). DHA 4/8 no último dia receberam binge alcoólico (5 g/kg de etanol). Posteriormente, os animais foram sacrificados, e as fezes foram coletadas para análise da MI. Resultados: Ambos os modelos experimentais induziram características histopatológicas típicas das doenças. A diversidade alfa foi menor na MASLD em comparação com a DHA (P<0,001), e o padrão estrutural foi diferente entre elas (P<0,001). Bacteroidetes (55,7%), Firmicutes (40,6%) e Proteobactérias (1,4%) foram os filos mais prevalentes em todas as amostras, embora com abundâncias diferenciadas entre os grupos. ABSTRACT Pigment dispersion syndrome is associated with clinical features such as Krukenberg's spindles, trabecular pigmentation, Scheie's stripe and Zentmayer's ring. Another less common feature of this syndrome is retrolental pigment deposits due to anterior hyaloid detachment or a defect in the Wieger's ligament. We present two cases of pigment deposits on the posterior lens capsule. In both cases, there is bilateral dispersion of pigment throughout the anterior segment. The retrolental deposits are unilateral in the first case and bilateral in the second. Both patients report a history of ocular trauma. This is a possible important clinical sign of pigment dispersion syndrome, rarely described. RESUMO A síndrome de dispersão pigmentar associa-se a sinais clínicos característicos como fuso de Krukenberg, hiperpigmentação da malha trabecular, linha de Scheie e anel de Zentmeyer. Um sinal menos comum dessa síndrome é o depósito de pigmento posterior ao cristalino, que ocorre por um descolamento da hialoide anterior ou um defeito no ligamento de Wieger. Apresentamos dois casos de depósitos de pigmento posterior à cápsula posterior do cristalino. Em ambos os casos, existia dispersão bilateral de pigmento por todo o segmento anterior. No primeiro caso, os depósitos eram unilaterais e, no segundo, estavam presentes em ambos os olhos. ABSTRACT In ecotonal Cerrado areas, the addition of Amazonian and Atlantic Forest species mainly occurs through connections between forest areas and forest physiognomies. In this context, the biome is a unique region for research on the influence of geographical and historical factors on its biota. In tropical ecotonal regions, integrated communities can exist on soils that present variations in fertility, physical properties and depth, which results in heterogeneity of physiognomies. It was tested the hypothesis that edaphic conditions related to calcium availability are significantly related to species composition and structural parameters in two physiognomies. The study was conducted in the Cerrado of the Chapada das Mesas National Park (CMNP) in the city of Carolina, Northeast Brazil, and the sample universe consisted of 18 areas in two physiognomies. The initial hypothesis was confirmed. SUMMARY OBJECTIVE: Increasing evidence suggests that exercise programs are of great value in the rehabilitation and survivorship of patients with cancer. However, challenges remain regarding maintaining patients more physically active. This study aimed to evaluate the impact of a supervised exercise program on quality of life, fatigue, physical performance, and levels of physical activity of patients with cancer. METHODS: An observational longitudinal study, with a 1-year prospective follow-up, was developed. SETTING: This is a university-based outpatient rehabilitation program in a high-complexity cancer care center in Sao Paulo. RESULTS: After the program, patients showed a significant gain in quality of life (p<0.0001), physical performance (p<0.0001), and improvement in fatigue (p<0.0001). After 12 months, 81.1% of the patients remained active, and only 4.5% declared themselves to be sedentary. CONCLUSION: The results of this study confirm that exercise programs are an important tool in the rehabilitation of patients with cancer and that an initial supervised exercise program, in combination with follow-ups, can help increase the levels of physical activity of this population. Abstract Sjogren’s disease (SjD) is an autoimmune disease that is characterized not only by the sicca symptoms it causes but also by its systemic nature, which is capable of several and not yet fully understood extraglandular manifestations. To gain a clearer understanding of these manifestations as well as a better practical approach, a panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis on the identification of epidemiologic and clinical features of the extraglandular manifestations present in ESSDAI (EULAR Sjogren´s syndrome disease activity index), followed by a voting panel with recommendations for clinical practice. This publication is complementary to others already published and covers cutaneous and hematological manifestations, with prevalence data generated by a meta-analysis of 13 clinical or laboratory manifestations and 6 clinical management recommendations. Abstract Objective To develop the second evidence-based Brazilian Society of Rheumatology consensus for diagnosis and treatment of lupus nephritis (LN). Methods Two methodologists and 20 rheumatologists from Lupus Comittee of Brazilian Society of Rheumatology participate in the development of this guideline. Fourteen PICO questions were defined and a systematic review was performed. Eligible randomized controlled trials were analyzed regarding complete renal remission, partial renal remission, serum creatinine, proteinuria, serum creatinine doubling, progression to end-stage renal disease, renal relapse, and severe adverse events (infections and mortality). The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to develop these recommendations. Recommendations required ≥82% of agreement among the voting members and were classified as strongly in favor, weakly in favor, conditional, weakly against or strongly against a particular intervention. Other aspects of LN management (diagnosis, general principles of treatment, treatment of comorbidities and refractory cases) were evaluated through literature review and expert opinion. Results All SLE patients should undergo creatinine and urinalysis tests to assess renal involvement. Kidney biopsy is considered the gold standard for diagnosing LN but, if it is not available or there is a contraindication to the procedure, therapeutic decisions should be based on clinical and laboratory parameters. Fourteen recommendations were developed. Target Renal response (TRR) was defined as improvement or maintenance of renal function (±10% at baseline of treatment) combined with a decrease in 24-h proteinuria or 24-h UPCR of 25% at 3 months, a decrease of 50% at 6 months, and proteinuria < 0.8 g/24 h at 12 months. Hydroxychloroquine should be prescribed to all SLE patients, except in cases of contraindication. Glucocorticoids should be used at the lowest dose and for the minimal necessary period. In class III or IV (±V), mycophenolate (MMF), cyclophosphamide, MMF plus tacrolimus (TAC), MMF plus belimumab or TAC can be used as induction therapy. For maintenance therapy, MMF or azathioprine (AZA) are the first choice and TAC or cyclosporin or leflunomide can be used in patients who cannot use MMF or AZA. Rituximab can be prescribed in cases of refractory disease. In cases of failure in achieving TRR, it is important to assess adherence, immunosuppressant dosage, adjuvant therapy, comorbidities, and consider biopsy/rebiopsy. Resumo Uma espécie nova de Cambeva com ocorrência em tributários dos rios Piquiri e Ivaí, bacia do alto rio Paraná, Brasil, é descrita utilizando a combinação de dados morfológicos e moleculares. A espécie nova pode ser diferenciada de suas congêneres pela presença de um entalhe na porção posterior do metapterigóide, número de raios branquiostegais, odontódeos operculares e interoperculares, e de costelas. Além disso, os resultados corroboraram a existência de uma única espécie com ampla variação intraespecífica no padrão de coloração corporal. A localidade-tipo está situada na área de influência da Reserva Biológica das Perobas, uma Unidade de Conservação no estado do Paraná, composta por duas fisionomias vegetais da Mata Atlântica. Considerando que a bacia do alto rio Paraná é uma área de influência antrópica, é fundamental descrever e preservar suas espécies para conhecer sua ictiofauna. Abstract A new species of Cambeva occurring in the rio Piquiri and Ivaí, upper rio Paraná basin, Brazil, is described using the combination of morphological and molecular data. The new species is distinguished from most congeners by the presence of a notch in the posterior portion of the metapterygoid, number of branchiostegal rays, opercular and interopercular odontodes, and ribs. In addition, the results corroborated the existence of a single species with wide intraspecific variation in body coloration. The type-locality is within the area of influence of the Perobas Biological Reserve, a Conservation Unit in the Paraná State, composed of two Atlantic Forest physiognomic forms. Considering that the upper rio Paraná basin is an area of significant anthropic influence, it is crucial to describe and preserve species to understand their ichthyofauna. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Introduction The P300 auditory evoked potential is a long-latency cortical potential evoked with auditory stimulation, which provides information on neural mechanisms underlying the central auditory processing. Objectives To identify and gather scientific evidence regarding the P300 in adult cochlear implant (CI) users. Data Synthesis A total of 87 articles, 20 of which were selected for this study, were identified and exported to the Rayyan search software. Those 20 articles did not propose a homogeneous methodology, which made comparison more difficult. Most articles (60%) in this review compare CI users with typical hearing people, showing prolonged P300 latency in CI users. Among the studies, 35% show that CI users present a smaller P300 amplitude. Another variable is the influence of the kind of stimulus used to elicit P300, which was prolonged in 30% of the studies that used pure tone stimuli, 10% of the studies that used pure tone and speech stimuli, and 60% of the studies that used speech stimuli. Conclusion This review has contributed with evidence that shows the importance of applying a controlled P300 protocol to diagnose and monitor CI users. Regardless of the stimuli used to elicit P300, we noticed a pattern in the increase in latency and decrease in amplitude in CI users. The user’s experience with the CI speech processor over time and the speech test results seem to be related to the P300 latency and amplitude measurements. RESUMEN Los individuos toman decisiones en todo momento, ya sean simples o complejas, rápidas o lentas, dependiendo de la pequeña o gran disponibilidad de información, de carácter único o constante, entre otros aspectos. Buscando comprender la lógica detrás del proceso de toma de decisiones de las personas, los académicos han propuesto varios modelos. El presente estudio tiene como objetivo realizar la traducción, adaptación transcultural y validación para el contexto brasileño del Inventario General de Estilos de Toma de Decisiones - GDMS. Para eso, el instrumento pasó por seis etapas de traducción y adaptación. Luego, después de realizar el pre-test, se aplicó la versión final del instrumento en un estudio piloto con una muestra de 250 individuos. Así, a partir de los datos encontrados, se realizó un análisis factorial exploratorio, con el fin de validar la versión final del cuestionario. Finalmente, se concluyó que el modelo propuesto en este estudio se mostró adecuado para la investigación del constructo estilos individuales de toma de decisiones, presentando índices de validez y confiabilidad adecuados. ABSTRACT Individuals always make decisions, whether simple or complex, fast or slowly, depending on the small or large availability of information, of a unique or constant nature, among other aspects. Seeking to understand the logic behind people's decision-making process, several models have been proposed by scholars. The present study aims to carry out the translation, cross-cultural adaptation, and validation for the Brazilian context of the General Decision-Making Styles Inventory - GDMS. To this end, the instrument underwent six stages of translation and adaptation. Then, after carrying out the pre-test, the final version of the instrument was applied in a pilot study with a sample of 250 individuals. Thus, based on the data found, an exploratory factor analysis was carried out, to validate the final version of the questionnaire. Finally, it was concluded that the model proposed in this study proved to be adequate for the investigation of the construct individual decision-making styles, presenting appropriate validity and reliability indices. RESUMO Os indivíduos tomam decisões a todo momento, de caráter simples ou complexo, de forma rápida ou demorada, da pequena ou grande disponibilidade de informações, de natureza única ou constante, entre outros aspectos. Buscando compreender a lógica de desenvolvimento do processo de decisão das pessoas, diversos modelos foram propostos por estudiosos. O presente estudo objetiva realizar a tradução, adaptação transcultural e validação para o contexto brasileiro do Inventário Geral de Estilos de Tomada de Decisão - GDMS. Para tanto, o instrumento passou por seis etapas de tradução e adaptação e, em seguida, após a realização do pré-teste, a versão final do instrumento foi aplicada em um estudo piloto com uma amostra de 250 indivíduos. A partir dos dados encontrados, procedeu-se à análise fatorial exploratória, visando validar a versão final do questionário.
