Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

Abstract The Chapada das Mesas National Park is a federal reserve located in the State of Maranhão, in an ecotonal region of the Brazilian Northeast Cerrado. Aiming to assess the influence of biogeographic patterns and support restoration actions for regions where information is scarce, we analyzed the floristic composition of savanna and forest Cerrado physiognomies, the floristic relationships with other Brazilian Cerrado areas and investigated the floristic relationships between the Cerrado and the Amazon, and the Caatinga and Atlantic Forest biomes. A total of 242 species in 181 genera and 64 families are recorded in our study. Fabaceae was the most representative family. We report 50 new records of species for Maranhão, four new records for the Cerrado biome and one vulnerable species. The separation of the gallery forest from cerradão and typical cerrado by ordination shows the influence of water availability and soil characteristics on floristic composition. In Cerrado ecotonal regions, such as Mato Grosso and Maranhão, the presence of species shared with the Amazon is expressive. In addition to the Amazon, physiognomies had greater floristic connection with the Caatinga flora. Our data demonstrates that the marginal areas of the Brazilian Cerrado may present high species diversity.

In this study, we investigated the chemical compositions and antioxidant and antimicrobial activities of propolis produced by the stingless bee Frieseomelitta longipes and the honeybee Apis mellifera collected from colonies in North Brazil. In terms of volatile composition, both mono- and sesquiterpenes were detected in the propolis of F. longipes while only sesquiterpenes were detected in that of A. mellifera. Out of 50 volatiles identified in all samples, 26 were found exclusively in F. longipes propolis and 8 were found exclusively in A. mellifera propolis. The chemical profiles of the propolis extracts were determined by atmospheric pressure chemical ionization-mass spectrometry and liquid chromatography-electrospray ionization-tandem mass spectrometry allowed to identify several prenylated benzophenones. A. mellifera extracts exhibited major antioxidant activity as assessed by the 2,2-diphenyl-1-picrylhydrazyl method and all extracts exhibited antioxidant activity as assessed by the β-carotene/linoleic acid method. The ethanolic extracts of the propolis showed promisor activity against all tested microorganisms.

Type C waterborne botulism outbreaks in buffaloes (Bubalus bubalis) in the Amazon region

RESUMO: Botulismo é uma intoxicação causada por neurotoxinas botulínicas (BoNTs). Os sorotipos C e D de BoNTs estão envolvidos em surtos de botulismo em bovídeos em vários países. Apesar do elevado número de búfalos em todo o mundo, o real impacto do botulismo em búfalos não é conhecido; pois não é uma doença de notificação obrigatória no Brasil e poucos estudos avaliaram a incidência desta doença em búfalos. Além disso, estes estudos não realizaram testes diagnósticos para confirmar a presença de BoNTs. O objetivo do presente estudo foi descrever três surtos de botulismo em búfalos na região amazônica brasileira, considerando dados epidemiológicos e clínicos, bem como o diagnóstico laboratorial para confirmar a presença de BoNTs. Os resultados do bioensaio em camundongos foram negativos em todos os tecidos e nas amostras de alimentos testados; no entanto foram positivos para BoNT C nas amostras de água. A confirmação da ocorrência de botulismo em búfalos permite a implementação de estratégias preventivas nos rebanhos. O botulismo hídrico nos búfalos pode ser prevenido assegurando-se que coleções de água fossem mantidas limpas, sem a presença de animais mortos e ossadas no seu interior e não permitindo o acúmulo de matéria orgânica e condições de anaerobiose favoráveis à multiplicação de Clostridium. botulinum. Outra medida que pode ser adotada é o sombreamento das pastagens, a fim de manter o conforto térmico dos búfalos e assim evitar o excesso de sua permanência dentro das fontes de água.
ABSTRACT: Botulism is a poisoning caused by botulinum neurotoxins (BoNTs). BoNTs serotypes C and D are involved in botulism outbreaks in cattle in several countries. Despite the high number of buffaloes worldwide, the real impact of botulism in buffaloes is not known, because it is not a notifiable disease in Brazil and only few studies have evaluated the occurrence of the disease in buffaloes. Those studies did not conduct diagnostic tests to confirm the presence of BoNTs. The objective of the present study was to describe three outbreaks of botulism in buffaloes in the Brazilian Amazon region considering epidemiological and clinical data as well as laboratory diagnosis to confirm the presence of BoNTs. The results of the bioassay were negative in the tissues and in feed samples, but positive for BoNT C in water samples. Confirmation of the occurrence of botulism in buffaloes allows the implementation of preventive strategies in susceptible herds. Waterborne botulism in buffaloes is prevented by ensuring the constant circulation of water collections and restricting the presence of dead animals and bones in order to prevent the accumulation of organic matter and the development of anaerobic conditions, which might favor the replication of Clostridium botulinum. Another measure that can be adopted is the shading of the pasture, in order to maintain the thermal comfort for the buffaloes and to avoid the excess of permanence of them in the water pools.

Resumo: Com o objetivo de realizar um estudo dos teores de cobre (Cu), zinco (Zn) e ferro (Fe) em búfalas com paratuberculose (PTB) foram utilizadas 13 búfalas, das raças Murrah, Mediterrâneo e seus mestiços acima de três anos de idade, pertencentes a duas propriedades localizadas nos municípios de São Luiz e São Mateus, no Estado do Maranhão. Os animais foram selecionados de acordo com a presença de sinais clínicos sugestivos de paratuberculose, caracterizados por estado nutricional regular a ruim, diarreia crônica líquida a semi-líquida, desidratação, edema submandibular, anestro prolongado, mastites e verminose gastrintestinal. Foi realizada biópsia retal em todos os animais, para detecção de Mycobacterium avium subsp. paratuberculosis (Map) por meio da qPCR, e exames histopatológicos (HE e Ziehl-Neelsen). No Grupo1 sete animais foram positivos para presença de Map, e no Grupo 2 seis foram negativos. Todos os búfalos foram eutanasiados e necropsiados para coleta de diversos tecidos. Parte dos fragmentos foram fixados em formol a 10% para histopatologia e fragmentos de tecido hepático foram congelados para as dosagens dos microminerais (Cu, Zn e Fe). À necropsia todos os animais positivos para PTB apresentavam linfonodos mesentéricos de coloração castanha sugestiva de hemossiderose. Adicionalmente, em um animal foram observados pequenos pontos de cor marrom distribuídos difusamente na mucosa do intestino delgado. Na histopatologia foi observada hemossiderose moderada a acentuada no baço dos animais do Grupo 1. Na dosagem dos microminerais todos os animais com PTB apresentaram níveis abaixo dos valores de referência para Cu e Zn. Observou-se que a média dos teores de Cu dos búfalos com PTB foi 18,0ppm e de Zn 68,6ppm. No Grupo 2 a média dos teores de Cu foi 113,7ppm e de Zn 110,0ppm. Os teores de Fe em ambos os grupos foram elevados (>670ppm). Baseado nos achados clínico-patológicos e nas dosagens de minerais realizadas neste estudo, conclui-se que na região estudada, a PTB agravou o quadro clínico de animais com deficiência de Cu e Zn. Em áreas menos deficientes desses minerais sugere-se que a doença seja capaz de induzir quadros de deficiência mineral secundária.
Abstract: In order to study copper, zinc and iron concentration in the liver of buffaloes with paratuberculosis (PTB), 13 buffalo cows above three years of age of the Murrah and Mediterranean races or their crosses were used. They originated from two farms in the municipalities of São Luís and São Mateus, state of Maranhão, Brazil. The animals were selected according to clinical signs of paratuberculosis, as diarrhea, dehydration and submandibular edema. Rectal biopsies for detection of Mycobacterium avium subsp. paratuberculosis (Map) through PCR in real time (qPCR) and Ziehl-Neelsen (ZN) stain were performed on all animals. Seven buffaloes revealed positive and six were negative for PTB. They were separated into two groups: Group 1 contained seven buffaloes positive for paratuberculosis, and Group 2 contained six animals negative for paratuberculosis. They were euthanized and postmortem examination was performed for the collection of various tissue samples to be fixed in 10% formalin for histopathology; also liver tissue samples were collected to be frozen for chemical analysis of trace minerals (Cu, Zn and Fe). Postmortem showed that all buffaloes with PTB had brown colored mesenteric lymphnodes, indicating hemosiderosis. One buffalo had brown spots on the mucosa of the small intestine. Histopathology revealed moderate to severe hemosiderosis of the spleen in buffaloes of Group 1. All animals positive for PTB showed micromineral levels below the reference values. The average Cu concentration in buffaloes with PTB was only 18.0ppm and the one of Zn only 68.6ppm. In Group 2, negative for PTB, the average Cu concentration was 113.7ppm and the one of Zn 110.0ppm. Fe concentrations in buffaloes of both groups were high (>669ppm). The analysis of trace elements in the liver confirmed copper deficiency, but also showed low levels of zinc and high levels of iron. Based on clinical and pathological findings, and the micromineral concentrations found in the liver, it is concluded that paratuberculosis could lead to mineral deficiency and worsen the clinical condition of the buffaloes.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Skull radiographs and computed tomography scans in children and adolescents with mild head trauma

OBJETIVO: Identificar quais os pacientes na faixa pediátrica com trauma craniencefálico leve são candidatos para a realização de radiografia simples ou tomografia computadorizada do crânio (TCC). MÉTODO: Pacientes com trauma craniano leve, entre 0 e 19 anos, admitidos em um centro de referência em traumatologia, na cidade do Salvador, Brasil, entre maio 2007 e maio 2008. RESULTADOS: Foram atendidos 1888 pacientes com trauma craniano leve, com idade média de 7,4 (±5,5) anos. Um total de 1956 radiografias simples e 734 TCC foram realizadas. Em 44,4% dos pacientes com escala de coma de Glasgow (GCS) 13 e 55.4% com GCS 14, tiveram TCC com achados anormais. Em pacientes com múltiplos traumas, 16% possuíam alterações na TCC. CONCLUSÃO: Recomendamos TCC em pacientes com GCS 13 e 14 ou naqueles com traumas múltiplos, independente do GCS. Radiografias simples do crânio como rotina, não foram identificadas como úteis, no presente estudo.
OBJECTIVE: To identify which pediatric patients with mild head trauma are candidates for skull radiographs or cranial computed tomography (CCT) scans. METHOD: Patients with mild head trauma aged from 0 to 19 years presenting to the Emergency Department of a trauma centre from Salvador City, Brazil, between May 2007 and May 2008. RESULTS: A total of 1888 mild head trauma patients were admitted; mean age was 7.4 (±5.5) years. A total of 1956 skull radiographs and 734 CCT scans were performed. About 44.4% patients with Glasgow coma score (GCS) 13 and 55.4% with GCS 14 had abnormal CCT scans. In patients with multiple traumas, 16% had abnormal findings on CCT scans. CONCLUSION: We strongly recommend routine CCT studies to patients with GCS of 13 and 14 or to multiple trauma victims, independently of score. Routine screening skull radiographs were not useful in the evaluation of mild head trauma patients in this study.
