Simultaneous Detection of Mycobacterium tuberculosis and Mycobacterium avium Complex by a Multiplex PCR

Abstract Infection disease caused by nontuberculous mycobacteria (NTM) have been increasingly reported and often manifest with the same symptoms of tuberculosis (TB). The identification of the causative agent is fundamental for the determination of an appropriate therapy, since each species of mycobacteria requires a specific treatment. Thus, rapid and accurate tests to identify mycobacteria of medical interest, such as Mycobacterium tuberculosis complex (MTBC) and Mycobacterium avium complex (MAC), are necessary in the clinical routine. The present study evaluated an in-house multiplex PCR to detect, in reference and clinical strains, the genus Mycobacterium and also M. tuberculosis and MAC. To identify the Mycobacterium genus, M. tuberculosis and MAC, it was amplified a fragment of hsp65 gene, esat-6 gene, and the internal transcribed spacer between the 16S and 23S rRNA genes, respectively. In total, 87 mycobacteria strains were used, being 10 reference and 77 clinical strains, previously identified as MTBC (n = 66), M. avium (n = 8) or other NTM specie (n = 13). The hsp65 gene fragment was amplified for all mycobacteria strains evaluated (87/87). This multiplex PCR presented sensitivity of 100% and specificity of 95.2% for M. tuberculosis detection, and sensitivity of 100% and specificity of 100% for MAC detection. The multiplex PCR evaluated is an important tool for the differentiation between M. tuberculosis and NTM, as well as for the identification of MAC, a complex composed by species with high prevalence in the world.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

ABSTRACT Background: Tuberculosis (TB) remains a serious public health problem, with approximately 10 million new cases reported annually. Knowledge about the quantitative evolution of theses and dissertations (T&Ds) examining human TB in Brazil can contribute to generating strategic planning for training professionals in this field and disease control. Therefore, this study highlights the role of T&Ds on TB in national scientific disclosures. Methods: An integrative review related to TB was performed, including T&Ds produced in Brazil and completed between 2013 and 2019. Results: A total of 559,457 T&Ds were produced, of which 1,342 were associated with TB, accounting for 0.24% of the total number of T&Ds in Brazil. This was evidenced by a predominance of themes such as attention/health care, epidemiology, and TB treatment, and 80.2% of the T&Ds on TB were related to the large areas of health and biological sciences. Only 19.7% of T&Ds were associated with groups of patients considered at risk for TB, and 50.9% were produced in southeastern Brazil. The 1,342 T&Ds on TB were developed in 416 postgraduate programs linked to 121 higher education institutions (HEIs). We highlight that 72.7% of T&Ds on TB were produced in federal HEIs, 27.4% in state HEIs, and 8.5% in private HEIs. Conclusions: Strategic themes, such as TB control, require public policies that aim to increase the number of doctors and masters with expertise in TB, with geographic uniformity, and in line with the priorities for disease control.

Micro-CT evaluation of root canal preparation with rotary instrumentation on prototyped primary incisors

Abstract This study aimed to evaluate the endodontic instrumentation outcomes with asymmetrical files compared to reciprocating and hand files (HFs) in 3D-printed prototypes of upper primary incisors using micro-computed tomography (micro-CT). For this purpose, 50 prototypes were randomly divided (n = 10) according to the instrumentation technique as follows: HFs, a reciprocating file (WaveOne® Gold [WOG]), and three asymmetrical movement files: XP-Endo® Shaper (XPS), XP-Endo® Finisher (XPF), and XP Clean (XPC). The specimens were scanned and, after registration of the baseline and instrumented volumes, changes in the root canal volume (RCV), debris accumulation, removed root material volume (RRMV), non-instrumented areas, and the presence of cracks/perforations were quantified. Data were analyzed by analysis of variance and Student’s t-test, while the effect size was calculated for statistically significant outcomes. All groups showed an increase in RCV after instrumentation (p < 0.05), but this was higher with HFs (p < 0.05). Accumulated debris was higher for WOG and XPS (p < 0.05), but WOG exhibited more in the medium and apical third areas. HFs showed the highest RRMV (p < 0.05), especially at the apical third. The non-instrumented areas were lower for HFs and XPC than for the other systems (p < 0.05). Cracks were present in a few WOG (n = 2) and HF specimens (n = 3) and in this group, one of the cracked specimens and two others showed perforations. The asymmetric systems resulted in conservative dentin removal and fewer cracks/perforations as compared to HFs and a reciprocation file in prototyped primary upper incisors. XPC showed the best compromise between RRMV and non-instrumented areas with a low accumulation of debris.

Resumo A pandemia de COVID-19 tem desafiado pesquisadores e gestores a encontrar medidas de saúde pública que evitem o colapso dos sistemas de saúde e reduzam os óbitos. Esta revisão narrativa buscou sistematizar as evidências sobre o impacto das medidas de distanciamento social na epidemia de COVID-19 e discutir sua implementação no Brasil. Foram triados artigos sobre o efeito do distanciamento social na COVID-19 no PubMed, medRXiv e bioRvix, e analisados atos do poder público nos níveis federal e estadual para sumarizar as estratégias implementadas no Brasil. Os achados sugerem que o distanciamento social adotado por população é efetivo, especialmente quando combinado ao isolamento de casos e à quarentena dos contatos. Recomenda-se a implementação de medidas de distanciamento social e de políticas de proteção social para garantir a sustentabilidade dessas medidas. Para o controle da COVID-19 no Brasil, é imprescindível que essas medidas estejam aliadas ao fortalecimento do sistema de vigilância nos três níveis do SUS, que inclui a avaliação e uso de indicadores adicionais para monitorar a evolução da pandemia e o efeito das medidas de controle, a ampliação da capacidade de testagem, e divulgação ampla e transparente das notificações e de testagem desagregadas.
Abstract The COVID-19 pandemic has challenged researchers and policy makers to identify public safety measures forpreventing the collapse of healthcare systems and reducingdeaths. This narrative review summarizes the available evidence on the impact of social distancing measures on the epidemic and discusses the implementation of these measures in Brazil. Articles on the effect of social distancing on COVID-19 were selected from the PubMed, medRXiv and bioRvix databases. Federal and state legislation was analyzed to summarize the strategies implemented in Brazil. Social distancing measures adopted by the population appear effective, particularly when implemented in conjunction with the isolation of cases and quarantining of contacts. Therefore, social distancing measures, and social protection policies to guarantee the sustainability of these measures, should be implemented. To control COVID-19 in Brazil, it is also crucial that epidemiological monitoring is strengthened at all three levels of the Brazilian National Health System (SUS). This includes evaluating and usingsupplementary indicators to monitor the progression of the pandemic and the effect of the control measures, increasing testing capacity, and making disaggregated notificationsand testing resultstransparentand broadly available.

Abstract Coronavirus disease (COVID-19) is a pandemic caused by a new coronavirus, called SARS-CoV-2. This disease was first identified in December 2019 and rapidly developed into a challenge to the public health systems around the world. In the absence of a vaccine and specific therapies, disease control and promotion of patient health are strongly dependent on a rapid and accurate diagnosis. This review describes the main laboratory approaches to making a diagnosis of COVID-19 and identifying those previously infected with SARS-CoV-2.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

VENOUS INSUFFICIENCY AND THROMBOEMBOLIC DISEASE IN BARIATRIC SURGERY PATIENTS

Contexto A obesidade mórbida está associada a várias comorbidades, dentre elas a estase venosa crônica. A cirurgia bariátrica é a única modalidade efetiva de tratamento, porém com riscos e possibilidade de complicações, inclusive o tromboembolismo venoso. Objetivos Determinar a prevalência de sinais clínicos e ultrassonográficos de insuficiência venosa crônica no paciente obeso mórbido candidato a cirurgia bariátrica e a incidência de trombose venosa profunda. Métodos Foram selecionados para este estudo os pacientes candidatos a cirurgia bariátrica das equipes Centro Terapêutico Especializado em Fígado (CETEFI) e Pró-Gastro do Hospital Real e Benemérita Sociedade Portuguesa de Beneficência de São Paulo no período de março de 2007 a dezembro de 2009. Os dados analisados foram os achados clínicos (classificação CEAP - clínica, etiologia, anatomia e patofisiologia - classificação da estase venosa crônica) e sinais ultrassonográficos, o tipo de cirurgia (aberta ou laparoscópica), a circunferência abdominal, o índice de massa corpórea (IMC), o tempo de internação hospitalar e presença de comorbidades. A avaliação foi realizada para encontrar fatores independentes relacionados com a estase venosa. Resultados Entre março de 2007 e dezembro de 2009, 95 pacientes candidatos a cirurgia bariátrica foram avaliados clinicamente e por ultrassonografia duplex scan de sistema venoso de membros inferiores. Dos 95 pacientes, 53 acabaram sendo submetidos à cirurgia. Houve predominância de mulheres (77,9%), a idade média foi de 38,5 anos, a média de peso pré-operatório 124,6 kg e IMC médio de 45,5 kg/m2. Quanto à obesidade, 16,8% eram obesos graves, 78,9% obesos mórbidos e 4,2% mega-obesos. Em relação à avaliação venosa clínica, 86,3% dos pacientes se enquadravam na classificação CEAP menor que 3 e 13,7% maior ou igual a 3. Dentre as complicações pós-operatórias, houveram quatro casos de infecção de ferida. Não houve nenhum caso de trombose venosa e embolia pulmonar no pós-operatório. Conclusão Não foi encontrada correlação entre IMC, classificação CEAP e achados ultrassonográficos. Apesar de ter sido feita profilaxia para trombose venosa distal pós-operatória em todos os pacientes, a incidência foi consideravelmente elevada.
Context Morbid obesity is associated with various co-morbidities, including chronic venous insufficiency. Bariatric surgery is the only effective treatment for morbid obesity, but with potential risks and possible complications, including venous thromboembolism. Objective To determine the prevalence of clinical and ultrasonographic signs of chronic venous insufficiency in morbid obese patients in preparation for bariatric surgery and the incidence of post-operative venous thromboembolic disease. Methods Patients on work-up for bariatric surgery of Centro Terapêutico Especializado em Fígado (CETEFI) and Pro-Gastro surgical teams of the Hospital Beneficência Portuguesa de São Paulo were included. The analysed data were pre-operative findings for venous insufficiency (CEAP - clinical, etiological, anatomical, physiopathologic - classification and venous ultrassonographic findings), type of surgery (open or laparoscopic), abdominal circumference, body mass index (BMI) and post-operative ultrassonography search for venous insufficiency and deep venous thrombosis. Results Between March 2007 and December 2009, 95 patients candidates for bariatric surgery had clinical and duplex scan evaluation of the lower limbs venous system. Of the 95 patients, 53 were submitted to the surgical procedure. There was a predominance of women (77.9%), the average age was 38.5 years, average preoperative weight 124.6 kg and average BMI of 45.5 kg/m2. Regarding obesity, 16.8% were obese, and 83.1% were morbidly obese. In relation to the venous findings, 86.3% of the patients did fit CEAP classification less than 3 and 13.7% greater than or equal to 3. Among the post-operative complications, there were four cases of wound infection. Three patients developed post-operative distal venous thrombosis (7.5%), but no one had clinically manifested pulmonary embolism. Conclusion No relation between BMI, CEAP classification and venous ultrassonographic findings were found. Although prophylaxis was used in all patients, the incidence of post-operative distal venous thrombosis was considerably high.

Objective: Functional neuroimaging techniques represent fundamental tools in the context of translational research integrating neurobiology, psychopathology, neuropsychology, and therapeutics. In addition, cognitive-behavioral therapy (CBT) has proven its efficacy in the treatment of anxiety disorders and may be useful in phobias. The literature has shown that feelings and behaviors are mediated by specific brain circuits, and changes in patterns of interaction should be associated with cerebral alterations. Based on these concepts, a systematic review was conducted aiming to evaluate the impact of CBT on phobic disorders measured by functional neuroimaging techniques. Methods: A systematic review of the literature was conducted including studies published between January 1980 and April 2012. Studies written in English, Spanish or Portuguese evaluating changes in the pattern of functional neuroimaging before and after CBT in patients with phobic disorders were included. Results: The initial search strategy retrieved 45 studies. Six of these studies met all inclusion criteria. Significant deactivations in the amygdala, insula, thalamus and hippocampus, as well as activation of the medial orbitofrontal cortex, were observed after CBT in phobic patients when compared with controls. Conclusion: In spite of their technical limitations, neuroimaging techniques provide neurobiological support for the efficacy of CBT in the treatment of phobic disorders. Further studies are needed to confirm this conclusion.

Caries experience in young children with congenital heart disease in a developing country

Oral care is frequently suboptimal in children from developing countries, especially those suffering from severe systemic diseases. The aim of the present study was to analyze the oral epidemiological profile of 3-to-5-year-old children with congenital heart disease. Dental and medical records of children evaluated at the Dental Service of the National Institute of Cardiology, Rio de Janeiro, Brazil, were reviewed. Caries experience was reported using the dmft index. Negative behavior towards dental management was recorded. The sample consisted of 144 children aged 4.41 ± 0.95 years. The mean dmft value was 5.4 ± 4.9, and 80.5% had at least one caries lesion. Dmft index was greater in the presence of cyanotic cardiac disease and in children with negative behavior. An increase in the “missing” component of the dmft index was also found in children using medicine on a daily basis. A higher caries experience was associated with children whose fathers had only an elementary education. In conclusion, children with congenital heart disease had high levels of caries experience at a young age. Cyanosis, negative behavior, daily use of medicine, one-parent family and the educational level of fathers seem to influence caries experience in children with congenital cardiac disease.

Os autores relatam a experiência de adoção de ações interdisciplinares no processo de ensino-aprendizagem no curso de Odontologia da Universidade Severino Sombra (USS), as quais foram realizadas com fomento do Pró-Saúde. O objetivo do trabalho é descrever as etapas da construção da proposta, a metodologia adotada, as dificuldades e resistências enfrentadas, assim como os resultados e seus desdobramentos. A partir do relato é possível concluir que as ações interdisciplinares podem ser praticadas no ensino superior desde que haja uma diretriz no projeto pedagógico, uma equipe docente comprometida com a inovação metodológica e gestores institucionais empenhados em efetuar mudanças.
The authors report on their experience with the adoption of interdisciplinary activities in the teaching-learning process at the Dentistry School of Severino Sombra University (USS), promoted through the Pró-Saúde project. The study describes the stages in developing the proposal, the methodology, difficulties and resistance, and the results and spinoffs. The authors conclude that interdisciplinary activities can be applied to a higher education project as long as the teaching project has guidelines, a faculty body committed to the methodological innovation, and institutional policymakers dedicated to implementing the changes.

Objetivou-se avaliar o valor nutritivo de três espécies forrageiras tropicais: capim-tanzânia (Panicum maximum Jacq.), capim-marandu (Brachiaria brizantha) e capim-tifton 85 (Cynodon spp), em duas épocas do ano (janeiro-março e abril-junho) e em três idades de rebrota (28, 35 e 42 dias), por meio da composição química, do fracionamento de proteínas e carboidratos e da digestibilidade in vitro da matéria seca (DIVMS) e da matéria orgânica (DIVMO). O capim-marandu destacou-se no período de janeiro-março, com menores conteúdos de parede celular e fração B2 dos carboidratos e maiores valores de proteína bruta, fração A + B1, DIVMS e DIVMO, em comparação aos capins tanzânia e tifton 85, independentemente da idade de corte. O aumento da concentração de parede celular em detrimento ao conteúdo celular com o avanço da maturidade das plantas foi evidente no capim-marandu no período de janeiro-março, quando foram observados maior valor da fração B2, maior conteúdo de fibra em detergente neutro (FDN) e menor concentração da fração carboidratos não-fibrosos. No período de abril-junho, a composição em parede celular não apresentou diferenças evidentes com aumento da idade, devido às condições ambientais observadas. O capim-tanzânia apresenta, de modo geral, baixos valores de parede celular e altos valores de carboidratos não-fibrosos, DIVMS e DIVMO nesse período, seguido pelos capins marandu e tifton 85, respectivamente.
This trial was conducted with the objective of evaluating the nutritive value of three tropical forage species: tanzania grass (Panicum maximum Jacq.), marandu grass (Brachiaria brizantha) and Tifton 85 bermudagrass (Cynodon spp) in two different periods of the year (January-March and April-June) and in three cutting ages (28, 35 and 42 days), based on the chemical composition, protein and carbohydrate fractions, and in vitro digestibility of dry matter (DMD) and organic matter (OMD). Marandu grass, in the period January-March, had the lowest cellular wall contents and B2 fraction of carbohydrates, and higher values of crude protein, A + B1 fraction, DMD and OMD in comparison to tanzania grass and Tifton 85 bermudagrass, regardless of the cutting age. The increase of concentration of the cell wall in detriment to the cell content as plant maturity advanced was evident in marandu grass during January-March when higher values of B2 fraction, higher content of neutral detergent fiber and lower concentrations of non fibrous carbohydrate fractions were observed. In April-June period, the cellular wall contents did not show evident differences as age increased due to environmental conditions. The tanzania grass presents, in general, lower values of the cell wall and higher values of non fibrous carbohydrate, DMD and OMD during this period, followed by marandu grass and Tifton 85 bermudagrass, respectively.

Avaliaram-se a cinética de fermentação e taxas de degradação de três espécies forrageiras tropicais (capins tanzânia, marandu e tifton 85), com três idades de corte (28, 35 e 42 dias) nos períodos de janeiro-março e abril-junho, mediante o uso da técnica de produção de gases in vitro. No período de janeiro-março, o capim-marandu apresentou os maiores valores de fração solúvel, degradabilidade potencial e degradabilidade efetiva a taxa de passagem de 2%/hora. No período de abril-junho, os capins marandu e tanzânia apresentaram resultados semelhantes para essas mesmas variáveis. Independentemente do período, o capim-tifton 85 apresentou os menores valores de fração solúvel, degradabilidade potencial e degradabilidade efetiva a 2%/hora e valores superiores de fração não-degradável. Em abril-junho, a idade de corte influenciou a fração solúvel e a produção de gases após 96 horas de incubação, cujos menores valores foram observados, de modo geral, aos 28 dias. As menores taxas de fermentação foram observadas para o capim-tifton 85 no período de abril-junho, independentemente da idade de corte.
Fermentation kinetics and degradation rates of three tropical forage species (Tanzania grass, Marandu grass and Tifton 85 grass) were evaluated by the in vitro gas production technique. The forages were harvested at 28, 35 and 42 days of regrowth during two periods, January-March and April-June. In the January-March period, Marandu grass presented the greatest values of fraction A, potential degradability (PD) and effective degradability for a passage rate of 2 %/hour (ED2). In the April-June period, Marandu and Tanzania grasses presented similar results for the variables tested above. In both periods, Tifton 85 had the lowest values of fraction A, PD and ED2, and the greatest value of fraction C. In the April-June period, there was an age effect in relation to gas production potential and 96 hours of incubation with the lowest value was found for 28 days of regrowth. The lowest fermentation rates were observed in general for Tifton 85 grass in the April-June period, regardless of the regrowth age.
