Resumen En todo el mundo, la población de edad avanzada está en aumento y con ello el número de personas con enfermedades crónicas y progresivas, tales como el trastorno neurocognitivo mayor (TNM); este tipo de trastorno se constituye como un problema de salud pública. En atención a esta situación surgen múltiples estrategias de intervención dirigidas a reducir el malestar que representa el cuidado. El presente artículo tiene como propósito revisar las intervenciones dirigidas a cuidadores de pacientes con Alzheimer desde las áreas de psicología, fonoaudiología y fisioterapia, para proyectar un análisis desde un enfoque transdisciplinar por medio del uso de una investigación documental. Se emplearon bases de datos especializadas Psychology and Behavioral Sciences Collection, Scopus, Pubmed, Publindex, Lilacs, SciELO, EBSCO, ENFISPO, PEDro, ASHA, y se utilizó la búsqueda de palabras clave por medio de la estrategia PICO, durante los meses de enero a junio de 2017. Los descriptores utilizados fueron: “cuidadores familiares”, “cuidador primario”, “dolor”, “impacto físico”, “family caregiver”, “physical therapy”. Se obtuvieron 317 artículos, en español, inglés y francés, de los cuales tras una revisión profunda se seleccionaron 30. Entre los hallazgos se encuentran programas de intervención psicopedagógicos y psicoterapéuticos con diferentes modelos de aplicación. Se concluye que, a pesar de las diferencias metodológicas de los programas, siempre se tendrá un cambio positivo en la experiencia del cuidado. Además, se sugiere diseñar propuestas transdisciplinares en respuesta a las necesidades específicas del cuidador informal.

Abstract In the world, the elderly population increases and with it the number of people with chronic and progressive diseases, such as the Major Neurocognitive Disorder; this type of alteration is a phenomenon is starting to constitute a public health problem. In response to this, multiple intervention strategies have emerged, aimed at reducing the discomfort that care represents, but these have been developed in an isolated and fragmented way by the disciplines involved in the well-being of this population group. This article aims to review the intervention programs for caregivers of patients with Alzheimer’s produced in the areas of psychology, phonoaudiology and physiotherapy, projecting an analysis from a transdisciplinary perspective using documentary research. Specialized databases were used: Psychology and Behavioral Sciences Collection, Scopus, Pubmed, Publindex, Lilacs, SciELO, EBSCO, ENFISPO, PEDro, ASHA, using the search for keywords through the PICO strategy which allows the selection of the best scientific evidence of health programs, between the months of January to June 2017. The descriptors used in Spanish and English were “family caregivers”, “primary caregiver”, “pain”, “physical impact”, “low back pain”, “physiotherapy”. A total of 317 articles were obtained, in Spanish, English and French, of which 30 were selected after an in-depth review. Among them, psychopedagogical and psychotherapeutic intervention programs with different application models were found. It is concluded that, despite the methodological differences of the programs, there will always be a positive change in the experience of care; it is also suggested to design transdisciplinary proposals in response to the specific needs of the informal caregiver.

Resumen Los significados sobre el conflicto armado se construyen a partir de la cercanía y las experiencias que las poblaciones tienen frente a dicho fenómeno. El presente estudio tuvo como objetivo explorar los significados que tenían del conflicto armado 22 adolescentes y jóvenes y 20 adultos mayores de Bucaramanga. Con un diseño cualitativo, enfoque fenomenológico y muestreo a conveniencia, se utilizó una entrevista a profundidad y se construyó un grupo focal. Los resultados evidenciaron categorías sobre percepción, conocimiento, valoración e ideas de solución. Se encontró que los adultos mayores refieren más hechos relacionados al conflicto en comparación con los adolescentes y los jóvenes. En relación con la valoración del conflicto, ambos grupos lo perciben como algo atroz e injusto. También señalan que estos delitos los generaban grupos como la guerrilla, las FARC, el ELN, o el M-19. La corrupción también se destacó en el discurso de los participantes. Respecto a las soluciones al conflicto armado, resaltó el diálogo. Se concluye que existen diferencias entre los significados de las dos generaciones, en las que los adultos mayores presentan mayor fuerza en el discurso sobre esta problemática en comparación con los adolescentes y los jóvenes. Sin embargo, el impacto del conflicto en la población joven incluye en la percepción de la realidad.

Abstract The implications of the armed conflict are dependent upon the proximity to the conflict and the unique experiences that populations have in the face of the conflict. The present study had the objective of exploring the significance of the armed conflict for 22 adolescents and youth and for 20 older adults in Bucaramanga. Using a qualitative design, a phenomenological focus, and convenience sampling, researchers carried out an in-depth interview and constructed a focus group. The results yielded categories regarding perception, knowledge, valuation, and ideas of solutions. Researchers found that older adults refer to facts regarding the conflict more often, in comparison to adolescents and youth. In relation to the valuation of the conflict, both groups perceive it as something atrocious and unjust. Both groups also noted that guerrilla groups such as the FARC, the ELN, and M-19 generated these crimes. Corruption was also highlighted within the participants' interviews. Respect to solutions to the armed conflict, dialogue stood out as a principal solution. From these results, researchers conclude that differences exist between the two generations in terms of the significance of the armed conflict. Older adults demonstrate greater strength in their interviews regarding the conflict, in comparison to adolescents and youth. However, the impact of the conflict on the youth population influences their perception of reality.

Abstract Leptospirosis is important in Uruguay due to the economic loss caused by the diseases of production animals, mainly bovines, and also due to frequent human infection. We decided to study anti-Leptospira antibodies in the sera of dairy workers, rice laborers, veterinarians, suburban slum dwellers and garbage recyclers. Our aims were to estimate the seroprevalence of infection by Leptospira spp. in these people at risk, the relative importance of the known risk factors associated with infection, and the impact of human infections in each setting. Groups at risk were identified and 35 visits to their locations were made, conducting field surveys and exchange talks for information and education. Simple epidemiological questionnaires were administered and sera samples were taken from 308 persons. The microagglutination Technique (MAT) and the IgM Indirect Immunofluorescence (IIF) assay were employed to detect antibodies. Environmental water samples, canine and equine sera were also examined. More than 45% of human sera were reactive and the studied groups were confirmed to be widely exposed to infection. Female sera were frequently reactive, though most illnesses occur in men, and the most severe cases in elderly males; the emergence and evolution of the disease may strongly depend on the host condition and functions. Animal contact and unsafe water usage were the main identified risk factors to be considered in prevention. Fifty per cent of the studied horses showed a positive MAT reaction. The underdiagnosis of the illness and its long-term symptoms require further study, as well as greater health and social attention efforts.

Resumen La leptospirosis es importante en Uruguay por las pérdidas económicas que ocasionan las enfermedades en los animales de producción, principalmente en los bovinos, y a causa de la frecuente infección humana. Decidimos estudiar anticuerpos anti -Leptospira en trabajadores de tambo y de plantaciones de arroz, así como en veterinarios, habitantes de asentamientos y recicladores de residuos. Buscamos evaluar la importancia de distintos factores de riesgo conocidos asociados con la infección por Leptospira spp., y estimar la frecuencia y el impacto de las infecciones humanas en la población expuesta. Se efectuaron 35 visitas a colectivos de riesgo, realizando relevamientos de campo e intercambios educativos, llenando formularios epidemiológicos sencillos y tomando muestras de sangre a 308 personas. Se investigaron anticuerpos séricos con técnicas de microaglutinación (MAT) e inmunofluorescencia indirecta para IgM. Se examinaron también, muestras de agua ambiental, y sueros caninos y equinos. Más del 45% de los sueros humanos fueron reactivos y se confirmó que los grupos estudiados estaban ampliamente expuestos a la infección. Los sueros de mujeres fueron frecuentemente reactivos, aunque la mayoría de las enfermedades ocurren en varones, y los casos más graves en varones añosos. La emergencia y la evolución de la afección pueden depender fuertemente del estado del huésped y su respuesta. El contacto con animales y el uso de agua insegura fueron los principales factores de riesgo identificados para considerar en la prevención. El 50% de los sueros equinos fueron reactivos por MAT. Es necesario incrementar los esfuerzos de atención social y sanitaria en estos sectores, superar el subdiagnóstico y estudiar la evolución y la sintomatología a largo plazo de los pacientes.

ABSTRACT Laboratory diagnosis of human leptospirosis usually relies on indirect methods exploring specific immune response. Isolation and identification of the involved strains are cumbersome, but can provide biological resources for pathogenic studies and relevant information for guiding prevention and control measures. The aim of the research we are hereby reporting was the characterization of Leptospira isolates obtained from humans and the environment in Uruguay. Blood cultures were performed from early samples of 302 Uruguayan patients, mainly rural workers, and from 36 water samples taken from their living or working environments. Eight human isolates and seven environmental isolates were obtained and analyzed by end point Polymerase Chain Reaction (PCR), Multilocus Variable Number of Tandem Repeat Analysis (MLVA) and other molecular methods. Human isolates corresponded to several serogroups and serovars of Leptospira interrogans and Leptospira kirschneri species, probably reflecting the infection with similar involved Leptospira species and serovars of an extended animal reservoir in rural settings of the country, mostly dedicated to meat and dairy production. Culture-positive patients were older than usually affected workers, and presented signs and symptoms of severe illness. A high organic and circulating bacterial burden may explain an easier positive result from these workers’ samples. Environmental isolates were mainly identified as Leptospira biflexa strains, with a single L. meyeri isolate of uncertain significance.

Los procesos de articulación entre las universidades y las empresas son el resultado de la identificación de necesidades e intereses en común, que pueden generar la transferencia y gestión de conocimiento entre las partes. En este artículo se exponen los resultados de la evaluación de la calidad de las relaciones universidad-empresa en el departamento de Boyacá (Colombia) bajo el enfoque de marketing relacional. Para su evaluación fue necesario la realización de análisis multivariado y el uso de ecuaciones estructurales a partir de la técnica Partial Least Square (PLS). Entre los resultados se encuentra la importancia de la comunicación constante y permanente para que pueda existir la confianza necesaria para el establecimiento de acuerdos de trabajo y el compromiso en la articulación que pueda dar como resultado un beneficio mayor de la competitividad regional.

The processes of articulation between universities and industry resulting from the identification of needs and interests in common, to allow for the transfer and management of knowledge between the parties. This paper presents the results of the evaluation of the quality of the relationship university - industry in the Department of Boyacá low in relationship marketing approach. Was necessary for evaluation conducting multivariate analysis and the use of structural equations from technical Partial Least Square (PLS). Among the findings is the importance of consistent and constant communication, so you can be the confidence in establishing working arrangements and commitment in the joint that may result in a greater benefit of regional competitiveness.

La investigación se planteó como objetivo general identificar las áreas integrantes de las ciencias de la información y las relaciones que se establecen entre ellas, con el fin de evaluar posibilidades de existencia; dilucidar las diferencias terminológicas, las características y similitudes. La metodología desarrollada fue cualitativa, interrelacionó el trabajo de análisis documental, la reflexión grupal y la discusión con profesionales reunidos en foros para apoyar la profundización en el tema por parte del equipo de investigadores, las discusiones sobre los términos, objetos, relaciones y dilemas epistemológicos surgidos de las fuentes documentales. Se expone primero la postura epistemológica y la metodología; luego los resultados del análisis sobre los campos de conocimiento, en específico el de las ciencias de la información documental y los resultados de los encuentros epistemológicos. Finalmente, se presenta una apuesta interpretativa del subcampo de las ciencias de la información documental. Entre las conclusiones expuestas se analizan como la sociedad contemporánea ha convertido la información en 'una nueva fe', hecho que propició la configuración de un campo de conocimiento cuyo objeto es la información y donde se reúnen agentes, estudiosos y profesionales de las diversas ciencias relacionadas con ella. Por otra parte, el campo de la información deberá identificar diversos énfasis de la información y con ello establecer relaciones más cooperantes y diferenciadas entre agentes, ciencias, propósitos y perspectivas.

This research postulated as its general objective to identify the areas that integrate the sciences of Information and the relationships they establish between themselves, with the goal of evaluating the possibilities of existence, elucidating the terminological differences, their characteristics and likenesses. It worked on the basis of a qualitative methodology, that related the work of documental analysis, group reflection, and discussion with professionals reunited in forums, as a means to support a deeper consideration by the research team, of the discussions about the terms, objects, relationships and epistemological dilemmas that arose from the consideration of documental sources. First, the paper exposes the epistemological point of departure of the research and its methodology. Then, it considers the results of the analysis of the fields of knowledge, specifically the field of information sciences, and the results of the epistemological encounters. Finally, it presents an interpretative proposal of the subfield of documental information sciences. Between the conclusions exposed, it analyzes how contemporary society has turned information into a ''new faith'', a fact that has favored the configuration of a field of knowledge whose object is information and which gathers agents, scholars and professionals of different sciences related to information. Besides, the field of information shall identify diverse emphasis on information, and through this, to establish more cooperative and differentiated relationships between agents, sciences, purposes and perspectives.

Se presenta el caso de una mujer joven, que tuvo varios episodios de sangrado digestivo oscuro manifiesto por lo que requirió soporte transfusional, realización de dos esofagogastroduodenoscopias y dos colonoscopias, en las que no se identificó causa del sangrado. Se realizó enteroscopia de doble balón encontrando una lesión subepitelial, ulcerada, de aspecto neoplásico en el íleon, la cual se marcó con tinta china y se tomaron biopsias que no fueron diagnósticas. Después de realizar estudios de extensión que fueron negativos, se llevó a laparotomía diagnóstica y terapéutica resecando el segmento de ileon comprometido por la lesión y cuyo diagnóstico histopatológico y por inmunohistoquímica fue conclusivo de carcinoma neuroendocrino mal diferenciado de alto grado de célula grande. Al final presentamos además, una revisión de sangrado oscuro manifiesto y tumor neuroendocrino de intestino delgado.

Young woman who is having episodes of overt obscure gastrointestinal bleeding that requires transfusions. The endoscopic study consists of 2 endoscopies of the upper digestive system and two colonoscopies. The tests do not find the cause of the digestive hemorrhage. A double-balloon enteroscopy is performed and it is found that the Ileum has an ulcerate subepitelial lesion with neoplasic appearance which is marked with Chinese ink and biopsies are taken from the tissue which are not diagnosed. Studies of staging are performed ant the result is negative. A laparatomy is performed for diagnosis and treatment which includes the intestinal resection of ileum where the tumor is placed. The result of the test shows to be a neuroendocrine carcinoma of high degree of large cells undifferentiated. One appears in addition a revision to overt obscure gastrointestinal bleeding and neuroendocrine tumor of small bowel.

El bloqueo peribulbar es una técnica anestésica que consiste en colocar anestésicos locales por fuera del cono muscular de la órbita. Los anestésicos utilizados, con diferentes grados de toxicidad sistemica, han obligado la búsqueda de nuevos agentes como la ropivacaina. Se realizó un ensayo clínico controlado triple ciego para determinar las diferencias en calidad anestésica de la mezcla tradicional de anestésicos locales y ropivacaina, evaluando aquinesia, presión intraocular (PIO) y analgesia. Se tomaron 103 pacientes sometidos a extracción extracapsular de catarata más colocación de lente intraocular (EEC+LIO), recolectados entre noviembre y diciembre de 2003 en la Clínica Rafael Uribe de la ciudad de Cali. Los pacientes fueron asignados a dos grupos de manera seudoaleatoria. Grupo 1: ropivacaina 0.75% 10cc y grupo 2: bupivacaina 0.75% 5cc más lidocaina 2% 5cc más hialuronidasa 30 UI. ( Mezcla tradicional). Los grupos fueron similares en cuanto a las variables demográficas (sexo y raza), con una media de edad de 70 años para ambos. A partir del rango promedio y suma de rangos, se evaluaron: (1) Aquinesia, donde el grupo 2 mostró mayor inmovilidad que el grupo 1 ( p =0,008) y (2) dolor a través de la escala visual análoga sin diferencias significativas (p=0,571). La PIO previo bloqueo se analizó a partir de la media, que fue para ambos grupos 14.1mmHg (p=0.784.). La PIO después del bloqueo fue menor para ambos grupos, la media en el grupo 1 fue 10.8mmHg y grupo 2: 10.4mmHg ( p =0.409.) En conclusión, ambas soluciones anestésicas proporcionaron excelentes condiciones quirúrgicas en bloqueo peribulbar .

Peribulbar blockade is an anesthetic technique which consists in applying a local anesthetic outside the muscular layer of the orbit. The different levels of systemic toxicity of the local anesthetics currently used, has pushed the search of new agents like ropivacain. A 1. Pseudorandomized clinical trial, triple blind was designed. 2. The objective was to compare the differences in quality of anesthesia of the traditional mixture of local anesthetics and ropivacaine. Aquinesia, intraocular pressure and analgesia were evaluated. 103 patients operated of extracapsular extraction of cataract and insertion of intraocular lens, reervided between November and December 2003 at the Rafael Uribe Uribe clinic in Cali, Colombia. The patients were randomly assigned to two groups: Group 1: Ropivacain 0.75% 10 cc and group 2: Bupivacain 0.75% 5 cc. plus lidocain 2% 5cc. plus hyaluronidase 30 Units (traditional mixture). Groups were similar in demographic variables (sex, gender, race), with a mean age of 70 years. Starting at the mean range and the sum of ranges we evaluated: aquinesia, were group 2 showed more immobility than group 1 (p=0.008) and, pain through the analogue visual scale without difference between the two groups (p=0.075). The intraocular pressure before the blockade was analyzed starting at the mean, which was for the both groups 14.1 mm Hg. (p=0.0784). The intraocular pressure after the blockade was less for both groups, mean for group 1 was 10.8 mm Hg. And for group 2: 10.4 mm Hg. (p=0.409) In conclusion both anesthetic solutions provided excellent surgical conditions in peribulbar blockade .
