RESUMO Antecedentes: A Pandemia por COVID-19 tem trazido desafios subtanciais para a prática clínica no tratamento das doenças neuromusculares hereditárias (DNMh). A infecção não tem sido a única preocupação para os pacientes. O distanciamento social tem comprometido a assistência multidisciplinar, atividade física e tem trazido problemas mentais em decorrência do próprio isolamento. Nós apresentamos aqui um seguimento de 363 pacientes com DNMh de um centro terciário Brasileiro durante o pico da Pandemia de Covid-19. Objetivos: Mostrar a frequência e gravidade da infecção por Sars-Cov-2 em pacientes com DNMh e demonstrar os efeitos da pandemia nos hábitos de vida, na progressão da doença e no cuidado multidisciplinary. Métodos Trezentos e sessenta e três pacientes (58% homens and 42% mulheres) foram acompanhados por 3 meses através de 3 teleconsultas durante o pico da Pandemia de Covid-19 no Brasil. Resultados Houve um decréscimo no número de pacientes que faziam terapia física, respiratória e fonoaudiológica. Em muitos pacientes, o apetite (33%) e hábitos do sono (25%) se alteraram. Exercícios físicos e terapias foram interrompidas pela maioria dos pacientes. Physical exercises and therapies were interrupted for most of the patients. Eles relataram piora ou aparecimento de fadiga (17%), dor (17%), retrações (14%), e escoliose (7%). Irritabilidade, mudanças no sono, peso e apetite, sendo principalmente diminuição do apetite e peso foram mais frequentemente encontrados em pacientes que apresentaram piora clinica da doença. Houve uma baixa taxa de contaminação por Covid-19 (0.8%), e todos os pacientes infectado apresentaram quadro clinico leve. Conclusão O isolamento por si só se mostrou protetor na perspectiva de infecção por Covid-19, mas pode desencadear um cenário complexo com mudanças nos hábitos de vida e curso desfavorável da doença de base.

ABSTRACT Background The COVID-19 pandemic has brought substantial challenges for current practices in treating hereditary neuromuscular disorders (hNMDs). However, this infection has not been the only concern for these patients. Social distancing has compromised multidisciplinary assistance and physical activity, and has brought about several mental health issues. We presented a follow-up on 363 patients with hNMDs at a Brazilian tertiary center during the peak of the COVID-19 pandemic. Objective We aimed to show the frequency and severity of SARS-CoV-2 infection among hNMD patients and to demonstrate the effects of the pandemic on life habits, disease progression and multidisciplinary supportive care status. Methods Three hundred and sixty-three patients (58% male and 42% female) were followed for three months through three teleconsultations during the peak of the COVID-19 pandemic in Brazil. Results There were decreases in the numbers of patients who underwent physical, respiratory and speech therapies. For several patients, their appetite (33%) and sleep habits (25%) changed. Physical exercises and therapies were interrupted for most of the patients. They reported new onset/worsening of fatigue (17%), pain (17%), contractions (14%) and scoliosis (7%). Irritability and sleep, weight and appetite changes, and especially diminished appetite and weight loss, were more frequent in the group that reported disease worsening. There was a low COVID-19 contamination rate (0.8%), and all infected patients had a mild presentation. Conclusion The isolation by itself was protective from a COVID-19 infection perspective. However, this isolation might also trigger a complex scenario with life habit changes that are associated with an unfavorable course for the NMD.

Se analizan algunos temas relacionados con la selección y la construcción de los datos de costos unitarios para utilizar en análisis económicos regionales, con ejemplos ilustrativos de un análisis de la vacuna conjugada infantil antineumocócica realizado en 10 países de América Latina y el Caribe en 2006. El objetivo del análisis fue obtener un conjunto consistente de costos unitarios de recursos empleados en el tratamiento de la enfermedad neumocócica, según una bien ponderada combinación de datos publicados, tasas de reembolso, datos de cobro y nuevos métodos de determinación de costos detallados. Los pasos previos al análisis abarcaron la determinación del grado de detalle requerido, la definición del intervalo de costos de interés y la selección de conceptos de medición apropiados. Se determinó el costo detallado de los ítems mediante la descomposición de los componentes del tratamiento. Para el análisis se utilizaron los costos promedio ponderados por la población. En este informe se describen los diferentes métodos de determinación del costo empleados en el estudio y los problemas conceptuales y prácticos encontrados para su aplicación. También se identifican los posibles retos para generalizar este enfoque a otros escenarios de determinación de costos con otras enfermedades.

OBJECTIVE: To conduct a comprehensive review of data on pneumococcal disease incidence in Latin America and the Caribbean and project the annual number of pneumococcal disease episodes and deaths among children < 5 years of age in the region. METHODS: We carried out a systematic review (1990 to 2006) on the burden of pneumococcal disease in children < 5 years of age in the region. We summarized annual incidence rates and case fatality ratios using medians and interquartile ranges for invasive pneumococcal disease (IPD) (including all-IPD and separately abstracting pneumococcal meningitis, pneumonia, bacteremia, and sepsis data), pneumonia (all cause and radiologically confirmed), and acute otitis media by age group: < 1 year, < 2 years, and < 5 years. We modeled age-specific cumulative incidence of disease obtained from standard Kaplan-Meier analysis and projected data to obtain regional estimates of disease burden. We adjusted burden estimates by serotype coverage, vaccination coverage, and vaccine efficacy to estimate the number of cases and deaths averted. RESULTS: Of 5 998 citations identified, 26 papers from 10 countries were included. The estimated annual burden of pneumonia, meningitis, and acute otitis media caused by pneumococcus in children < 5 years of age ranged from 980 000 to 1 500 000, 2 600 to 6 800, and 980 000 to 1 500 000, respectively. An estimated 12 000 to 28 000 deaths due to pneumococcal disease occur in the region annually. Pneumococcal conjugate vaccine could save 1 life per 1 100 and prevent 1 case per 13 children vaccinated. CONCLUSION: A substantial burden of pneumococcal disease in the region is potentially preventable with pneumococcal conjugate vaccines and should be considered in regional vaccine decision making. Results are limited by the very few studies, conducted in selected settings, included in this review.

OBJETIVO: Realizar una revisión amplia de los datos sobre la incidencia de la enfermedad neumocócica en América Latina y el Caribe y proyectar el número anual de episodios de la enfermedad y de defunciones entre niños menores de 5 años de edad en la región. MÉTODOS: Se llevó a cabo una revisión sistemática (1990-2006) sobre la carga de la enfermedad neumocócica en niños < 5 años en la región. Las incidencias anuales y las tasas de letalidad se compendiaron mediante las medianas y los rangos intercuartiles de la enfermedad neumocócica invasiva (en su conjunto y por separado para meningitis, neumonía, bacteremia y sepsis neumocócicas), la neumonía (todos los casos confirmados mediante radiología) y la otitis media aguda, por grupos de edad: < 1 año, < 2 años y < 5 años. Se modeló la incidencia acumulada de la enfermedad específica para la edad mediante el análisis estándar de Kaplan-Meier y se proyectaron los datos para obtener estimados regionales de la carga de la enfermedad. Para estimar el número de casos y muertes evitados se ajustaron los estimados de la carga según la cobertura de los serotipos bacterianos, la cobertura de la vacunación y la eficacia de la vacuna. RESULTADOS: De las 5 998 referencias identificadas se seleccionaron 26 artículos de 10 países. La carga anual estimada de neumonía, meningitis y otitis media aguda causadas por neumococos en niños < 5 años varió entre 980 000 y 1 500 000, 2 600 y 6 800, y 980 000 y 1 500 000, respectivamente. Se estimó que en la región podrían morir anualmente entre 12 000 y 28 000 niños debido a la enfermedad neumocócica. La vacuna antineumocócica conjugada podría salvar una vida por cada 1 100 niños vacunados y evitar un caso de enfermedad por cada 13. CONCLUSIONES: Se podría evitar una parte substancial de la carga de enfermedad neumocócica en la región mediante la aplicación de vacunas antineumocócicas conjugadas y esto se debe tener en cuenta al tomar decisiones sobre la vacunación en la región. Estos resultados están limitados por los pocos estudios -realizados en ubicaciones específicas- analizados en esta revisión.

OBJECTIVE: In Latin America and the Caribbean, routine vaccination of infants against Streptococcus pneumoniae would need substantial investment by governments and donor organizations. Policymakers need information about the projected health benefits, costs, and cost-effectiveness of vaccination when considering these investments. Our aim was to incorporate vaccine, demographic, epidemiologic, and cost data into an economic analysis of pneumococcal vaccination of infants in Latin America and the Caribbean. METHODS: We previously used a structured literature review to develop regional estimates of the incidence of disease. Cost data were collected from physician interviews and public fee schedules. We then constructed a decision analytic model to compare pneumococcal conjugate vaccination of infants with no vaccination across this region, examining only vaccine's direct effects on children. RESULTS: Pneumococcal vaccination at the rate of diphtheria-tetanus-pertussis vaccine coverage was projected to prevent 9 500 deaths per year in children aged 0 to 5 years in the region, or approximately one life saved per 1 100 infants vaccinated. These saved lives as well as averted cases of deafness, motor deficit, and seizure result in 321 000 disability-adjusted life years (DALYs) being averted annually. At vaccine prices between US$5 and US$53 per dose, the cost per DALY averted from a societal perspective would range from US$154 to US$5 252. CONCLUSION: Pneumococcal conjugate vaccine was highly cost-effective up to $40 per dose. Introduction of pneumococcal vaccine in the Latin American and Caribbean region is projected to reduce childhood mortality and to be highly cost-effective across a range of possible costs.

OBJETIVO: En América Latina y el Caribe, la vacunación sistemática de niños contra Streptococcus pneumoniae podría requerir inversiones considerables de los gobiernos y organizaciones donantes. Al evaluar estas inversiones se requiere información sobre los posibles beneficios sanitarios, costos y rentabilidad (relación costo-efectividad) de la vacunación. Se presenta un análisis económico de la vacunación infantil antineumocócica en América Latina y el Caribe a partir de la información de la vacuna y de datos demográficos, epidemiológicos y de costos. MÉTODOS: Se realizó una revisión bibliográfica estructurada previa para llegar a estimados regionales de la incidencia de la enfermedad. Los costos se tomaron de entrevistas a médicos y tarifas públicas. Se construyó un modelo analítico de decisión para comparar la vacunación de los niños de esta región con la vacuna antineumocócica conjugada y la no vacunación, tomando en cuenta solamente el efecto directo de la vacuna en los niños. RESULTADOS: Se proyectó la vacunación antineumocócica con las tasas de cobertura de la vacuna contra la difteria, el tétanos y la tosferina para prevenir 9 500 muertes anuales en menores de 5 años en esta región, o aproximadamente 1 vida salvada por cada 1 100 niños vacunados. Las vidas salvadas y los casos prevenidos de sordera, trastornos psicomotores y convulsiones equivalen a evitar anualmente 321 000 años de vida ajustados por incapacidad (AVAD). A un precio de la vacuna de US$ 5,00 a US$ 53,00 por dosis, el costo por AVAD evitado desde el punto de vista de la sociedad sería de US$ 154,00 a US$ 5 252,00. CONCLUSIONES: La vacuna antineumocócica conjugada fue altamente rentable hasta un precio de US$ 40,00 por dosis. La introducción de esta vacuna en América Latina y el Caribe puede reducir la mortalidad infantil con una alta rentabilidad en un amplio espectro de posibles costos.

Measles is one of the most infectious diseases. Before measles vaccine was introduced, nearly everyone contracted the disease at some point in childhood. By the late 1980s, most countries had incorporated measles vaccine into their routine immunization programmes. Globally, about 800 000 children nevertheless still die from measles annually, half of them in Africa. Eradicating measles would therefore play an important role in improving children's survival. The 24th Pan American Sanitary Conference in 1994 established a goal of eradicating measles from the Americas. Progress to date has been remarkable and the disease is no longer endemic in the Americas, with most countries having documented interruption of transmission. As of November 2003, 12 months had elapsed since the last indigenous case was detected in Venezuela. This experience shows that measles transmission can be interrupted, and that this can be sustained over a long period of time. Global eradication is feasible if an appropriate strategy is implemented. Even under a new paradigm in which immunization is not discontinued after measles is eradicated, eradication will be a good investment to avoid expensive epidemics and save the lives of almost one million children annually. A world free of measles by 2015 is not a dream.

El sarampión es una de las enfermedades más infecciosas que existen. Hasta que se introdujo la vacuna antisarampionosa, casi todo el mundo contraía la enfermedad en algún momento de la infancia. A finales de los años ochenta, la mayoría de los países habían incorporado la vacuna en sus programas de vacunación sistemática. Sin embargo, cada año siguen muriendo por esa causa unos 800 000 niños en todo el mundo, la mitad en África. La erradicación del sarampión, por consiguiente, es un factor decisivo para mejorar la supervivencia infantil. En 1994, la 24ª Conferencia Sanitaria Panamericana estableció la meta de erradicar el sarampión de las Américas. Los progresos logrados hasta la fecha han sido notables, y la enfermedad ya no es endémica en las Américas, de tal manera que la mayoría de los países han confirmado la interrupción de la transmisión. A noviembre de 2003 habían transcurrido 12 meses desde la detección del último caso autóctono en Venezuela. Esta experiencia muestra que es posible interrumpir la transmisión del sarampión, y que esa situación puede mantenerse durante mucho tiempo. La erradicación mundial es factible si se pone en práctica una estrategia apropiada. Aun bajo un nuevo paradigma en el que no se interrumpa la inmunización una vez erradicado el sarampión, la erradicación será una buena inversión para evitar el alto costo de nuevas epidemias y salvar la vida de casi un millón de niños cada año. Un mundo libre de sarampión para 2015 no es ningún sueño.

La rougeole est l'une des maladies les plus contagieuses. Avant l'introduction du vaccin antirougeoleux, rares étaient les enfants qui échappaient à la maladie. Vers la fin des années 80, la plupart des pays avaient inclus la vaccination antirougeoleuse dans leurs programmes de vaccination systématique. Chaque année dans le monde, environ 800 000 enfants - dont la moitié en Afrique - meurent encore de la rougeole. L'éradication de cette maladie permettra donc d'améliorer sensiblement la survie de l'enfant. En 1994, la XXIVe Conférence sanitaire panaméricaine s'est fixé comme objectif d'éradiquer la rougeole des Amériques. Les résultats obtenus à ce jour ont été remarquables et la maladie n'est plus endémique dans les Amériques, la plupart des pays ayant prouvé que la transmission était interrompue. Le dernier cas de transmission indigène, détecté au Venezuela, remonte à novembre 2002, ce qui montre que la transmission de la rougeole peut être interrompue, et ce sur une longue période. L'éradication mondiale est réalisable avec une stratégie adaptée. Même dans l'hypothèse où l'on poursuit la vaccination après l'éradication de la rougeole, l'éradication reste un bon investissement qui permet d'éviter les épidémies coûteuses et de sauver la vie de près d'un million d'enfants chaque année. Un monde sans rougeole d'ici 2015 n'est pas un vœu pieux.

The Division of Vaccines and Immunization of the Pan American Health Organization (PAHO) is promoting epidemiological surveillance of bacterial pneumonias in children in Latin America in order to generate scientific evidence to support future decisions concerning using vaccines to control such pneumonias in the countries of the Region of the Americas. The diagnosis of these diseases rarely includes bacteriological documentation of the causative agent. Therefore, studies of this type that are carried out around the world accept radiological images of alveolar consolidation as a confirmatory criterion for a presumptively bacterial pneumonia. This piece examines the theoretical rationale and requirements for using thorax radiology as an instrument for epidemiological surveillance of bacterial pneumonias. The piece also summarizes the activities carried out during 2 years of joint efforts between the Center for Vaccine Development (Centro para Vacunas en Desarrollo) of Chile and PAHO's Division of Vaccines and Immunization. During those 2 years, the two groups encouraged the epidemiological study of bacterial pneumonias in Latin American children, using internationally accepted criteria and definitions as well as tools and practical solutions adapted to the reality of the Region of the America. The activities carried out so far show both the need for and the feasibility of standardizing the interpretation of chest radiographs so that they can be used in epidemiological studies.

La División de Vacunas e Inmunización de la Organización Panamericana de la Salud (OPS) está promoviendo la vigilancia epidemiológica de neumonías bacterianas en niños latinoamericanos, con el objetivo de generar pruebas científicas que sustenten futuras decisiones de control mediante vacunas en los países de la Región. Como es poco frecuente que el diagnóstico de estas enfermedades abarque la documentación bacteriológica del agente causal, los estudios de este tipo que se realizan en el mundo aceptan la imagen radiológica de consolidación alveolar como criterio confirmatorio de una neumonía presuntamente bacteriana. En este informe se examinan la racionalidad teórica y los requisitos necesarios para utilizar la radiología de tórax como instrumento para la vigilancia epidemiológica de neumonías bacterianas. Además, se ofrece un resumen de las actividades realizadas en dos años de trabajo conjunto entre el Centro para Vacunas en Desarrollo, en Chile, y la División de Vacunas e Inmunización de la OPS. Durante ese período se fomentó el estudio epidemiológico de las neumonías bacterianas en niños latinoamericanos utilizando los criterios y definiciones acordados a escala internacional, así como herramientas y soluciones prácticas ajustadas a la realidad de la Región. Las actividades desarrolladas hasta ahora evidencian la necesidad y factibilidad de estandarizar la interpretación de las radiografías de tórax para utilizarlas en estudios epidemiológicos.

Congenital rubella syndrome (CRS) is associated with substantial morbidity and mortality and with high costs. Today, as a result of improved vaccination and epidemiological surveillance efforts directed at eradicating measles from the Western Hemisphere, there has been a notable increase in the ability to detect, prevent, and control rubella and CRS. The importance of these measures is undeniable, and this piece examines the components that are essential in moving ahead to reduce these major public health problems in Latin America and the Caribbean. One step in that direction would be to integrate the surveillance of measles with that of rubella and CRS.

In October 2000, the Ministries of Health of the Dominican Republic and Haiti notified two cases of acute flaccid paralysis (AFP) in rural areas, one of them in a 9-month-old female, and the other in a 2-year-old female, respectively. Stool samples that were obtained from these cases, which occurred in July and August 2000, after a 9-year interruption of wild poliovirus circulation in the Western Hemisphere, revealed the presence of type 1 poliovirus. Genetic sequencing, which was later performed at the Centers for Disease Control and Prevention, in Atlanta, Georgia, United States of America, revealed an atypical descendant of the virus used in the manufacture of the oral polio vaccine (OPV), but with 3% genetic divergence with respect to the parent strain. Normally, viral isolates that derive from vaccine components show 99.5% genetic agreement with the parent strain; in wild polioviruses, on the other hand, this agreement is usually less than 82.0%. Thus, the 3% genetic divergence detected in this study suggests that, in areas with low vaccine coverage, the virus used in the vaccine remained in circulation for at least two years, during which it recovered the neurovirulence and communicability of wild poliovirus type 1. This report describes the characteristics and results of the active search for cases of AFP that was sparked by the detection of the two index cases. It also looks at the public health implications of this outbreak for the entire Region of the Americas.

In some countries, the invasive disease caused by Haemophilus influenzae type b (Hib) has been practically eliminated thanks to vaccination. However, in much of the developing world, meningitides and pneumonias caused by these bacteria continue to be a major cause of childhood morbidity and mortality, as well as high hospitalization costs. Because safe and effective conjugate vaccines are now available, the Special Program for Vaccines and Immunization of the Pan American Health Organization has recommended introducing them into the regular vaccination regimen of as many countries as possible. This has been done in Chile and Uruguay, where the Hib vaccine now forms part of the regular vaccination routine. When the vaccine was being introduced, both countries had difficulties they could have avoided if they had known of the experiences of other nations. Therefore, these two countries now offer the lessons they learned to other nations considering introducing the vaccine into their immunization programs. The most important lessons were to: strengthen the epidemiological surveillance system sufficiently in advance of introducing the vaccine; with th support of sicentific societies, present the technical information that justifies introducing the vaccine; seek community backing and acceptance; precisely establish in advance the presentation and dosage of the vaccine that is most appropriate for the country; and be certain to have the political and legal decisions needed to ensure the continuity of Hib vaccination in the future.
