The publication Cardiovascular Statistics – Brazil aims to provide an annual compilation of data and research on the epidemiology of CVDs in Brazil.  The report integrates official statistics from the Brazilian Ministry of Health and other governmental entities alongside data from the GBD project, coordinated by the IHME at the University of Washington. Additionally, it incorporates data derived from various sources and scientific studies, including cohorts and registries, that relate to CVDs and their associated risk factors. This publication is intended for a wide range of individuals, including researchers, clinicians, patients, healthcare policymakers, media professionals, the general public, and other interested parties seeking extensive national data about heart disease and stroke.  Volunteer researchers from various Brazilian universities and research institutions carry out the project. The group is led by a five-member steering committee (ALPR, CAP, DCM, GMMO, and LCCB). The Brazilian Society of Cardiology fully supports this initiative, and the project receives collaborative support from the GBD Brazil Network and an International Committee (GAR, PP, and TAG) from both the IHME/University of Washington (GAR) and the World Heart Federation (PP and TAG). 

A publicação Estatística Cardiovascular – Brasil tem por objetivo fornecer uma compilação anual dos dados e das pesquisas sobre a epidemiologia das DCV no Brasil.  Este documento integra as estatísticas oficiais do Ministério da Saúde do Brasil e outras entidades governamentais ao lado de dados do projeto GBD, coordenado pelo IHME da Universidade de Washington. Além disso, incorpora dados derivados de várias fontes e estudos científicos, inclusive coortes e registros, relacionados às DCV e fatores de risco associados. Esta publicação destina-se a um público variado, incluindo pesquisadores, clínicos, pacientes, formuladores de políticas de saúde, profissionais da mídia, o público em geral e todos aqueles que buscam dados nacionais abrangentes sobre DCV e acidente vascular cerebral. Pesquisadores voluntários de várias universidades e instituições de pesquisa brasileiros realizaram este projeto. O grupo é liderado por um comitê diretivo com cinco membros (ALPR, CAP, DCM, GMMO e LCCB). A Sociedade Brasileira de Cardiologia apoia integralmente esta iniciativa e o projeto recebe colaboração da Rede GBD Brasil1 e do GBD International Committee (GAR, PP e TAG) do IHME/Universidade de Washington (GAR) e da World Heart Federation (PP e TAG).

BACKGROUND Recurrence is a hallmark of ocular toxoplasmosis (OT), and conditions that influence its occurrence remain a challenge. Natural killer cells (NK) are effectors cells whose primary is cytotoxic function against many parasites, including Toxoplasma gondii. Among the NK cell receptors, immunoglobulin-like receptors (KIR) deserve attention due to their high polymorphism. OBJECTIVES This study aimed to analyse the influence of KIR gene polymorphism in the course of OT infection and its association with recurrences after an active episode. METHODS Ninety-six patients from the Ophthalmologic Clinic of the National Institute of Infectology Evandro Chagas were followed for up to five years. After DNA extraction, genotyping of the patients was performed by polymerase chain reaction sequence-specific oligonucleotide (PCR-SSO) utilising Luminex equipment for reading. During follow-up, 60.4% had a recurrence. FINDINGS We identified 25 KIR genotypes and found a higher frequency of genotype 1 (31.7%) with worldwide distribution. We note that the KIR2DL2 inhibitor gene and the gene activator KIR2DS2 were more frequent in patients without recurrence. Additionally, we observed that individuals who carry these genes progressed recurrence episodes slowly compared to individuals who do not carry these genes. MAIN CONCLUSIONS The KIR2DL2 and KIR2DS2 are associated as possible protection markers against ocular toxoplasmosis recurrence (OTR).

Abstract Events defined as major accidents emerged with the very industrialization process and alongside the development of new production technologies, becoming more complex in the early 21st century. This essay aimed to describe and contextualize the formulation of a concept that has sought to integrate topics related to workers’ safety and health with those of environmental health, workers’ struggles, and the democratization process in Brazil in a scenario of international division of labor, risks, and benefits. Considering the accidents and disasters in the 1980s and the more recent ones involving mine tailings dams, oil spills, and the Fukushima nuclear power plant, the authors identify more complex scenarios and new challenges for tackling this issue in the 21st century. Beyond dysfunctions in technological and organizational systems, the intensification of institutional vulnerabilities, added to the vulnerabilities produced by social inequalities, fuel the occurrence of major accidents and aggravate their effects, which, by being amplified beyond their spatial and temporal boundaries, especially affect countries in the Global South. We conclude that the recent events represent systemic expressions beyond organizational dysfunctions, revealing deeper layers of organizational and sociotechnical systems such as those forging the global economy and its profound asymmetries.

Resumo Os eventos definidos como acidentes ampliados surgem com o próprio processo de industrialização, assim como junto ao desenvolvimento de novas tecnologias de produção, tornando-se mais complexos neste início de século XXI. O objetivo deste ensaio foi apresentar e contextualizar a formulação de um conceito que buscou integrar os temas relacionados à segurança e saúde dos trabalhadores com os de saúde ambiental, bem como as lutas que envolviam os trabalhadores e o processo de democratização no país, em um cenário de divisão internacional do trabalho, riscos e benefícios. Considerando os acidentes e desastres ocorridos nos anos 1980 do século XX e, também, os recentes, envolvendo barragens de mineração, derrames de petróleo e uma usina nuclear de Fukushima, são apontados cenários mais complexos e novos desafios para enfrentamento desta questão no século XXI. Para além das disfunções dos sistemas tecnológicos e organizacionais encontradas, a intensificação das vulnerabilidades institucionais, somada às vulnerabilidades produzidas pelas desigualdades sociais, potencializam ocorrências e agravam os efeitos dos acidentes, ampliados para além de suas fronteiras espaciais e temporais, afetando sobretudo países do Sul Global. Conclui-se que os eventos recentes constituem expressões sistêmicas, indo além das disfunções organizacionais e revelando camadas mais profundas de sistemas organizacionais e sociotécnicos, como as que forjam a economia global e suas profundas assimetrias.

Abstract The aim of this study was to identify Cryptosporidium species found in cattle and sheep in Paraná, southern region of Brazil. Individual fecal samples from 458 bovines and 101 sheep were submitted for molecular analysis by PCR and nested PCR using specific primers for sequences of the 18S ribosomal unit (rRNA). Positive samples were analyzed using restriction fragment length polymorphism (RFLP), followed by genetic sequencing for species confirmation. The occurrence of Cryptosporidium was 11.27% (63/559). The highest occurrence was detected in lambs (12/59, 20.33%). From the 63 positive samples, it was possible to identify the species in 58 of them by RFLP and genetic sequencing. Five species of Cryptosporidium were identified: Cryptosporidium andersoni, Cryptosporidium bovis, Cryptosporidium ryanae, Cryptosporidium xiaoi, and Cryptosporidium parvum. The most prevalent species was C. andersoni (41.38%) and the least predominant was C. parvum (10.34%). The most abundant species of Cryptosporidium in dairy calves were C. andersoni (11/25) and C. ryanae (6/25). Of the 17 positive sheep, nine (52.94%) were infected with C. andersoni. This finding is the first report on the occurrence of C. andersoni in naturally infected sheep in Brazil and the first observation of a high absolute occurrence of this Cryptosporidium species in sheep.

Resumo O objetivo deste estudo foi identificar espécies de Cryptosporidium em bovinos e ovinos do Paraná, região sul do Brasil. Amostras de fezes de 458 bovinos e 101 ovinos foram individualmente submetidas à análise molecular por PCR e nested PCR, utilizando-se iniciadores específicos para sequências da unidade ribossomal 18S (rRNA). As amostras positivas foram analisadas pelo polimorfismo de comprimento de fragmento de restrição (RFLP), seguido de sequenciamento genético para confirmação da espécie. A ocorrência de Cryptosporidium foi de 11,27% (63/559). Observou-se maior ocorrência em cordeiros (20,33%). Das 63 amostras positivas, foi possível identificar as espécies em 58 amostras por RFLP e sequenciamento genético. Foram identificadas cinco espécies de Cryptosporidium: Cryptosporidium andersoni, Cryptosporidium bovis, Cryptosporidium ryanae, Cryptosporidium xiaoi e Cryptosporidium parvum. A espécie mais predominantemente encontrada foi C. andersoni (41,38%) e a menos foi C. parvum (10,34%). As espécies mais abundantes de Cryptosporidium, em bezerros leiteiros, foram C. andersoni (11/25) e C. ryanae (6/25). Dos 17 ovinos positivos, nove (52,94%) estavam infectados com C. andersoni. Este achado é o primeiro relato sobre a ocorrência de C. andersoni em ovinos naturalmente infectados no Brasil e a primeira observação de alta ocorrência absoluta desta espécie de Cryptosporidium em ovinos.

Diabetes among women with preterm births: outcomes of a Brazilian multicenter study

ABSTRACT Objective The objective was to compare the maternal and perinatal characteristics and outcomes between women with and without diabetes in a Brazilian cohort of women with preterm births. Methods This was an ancillary analysis of the Brazilian Multicenter Study on Preterm Birth, which included 4,150 preterm births. This analysis divided preterm births into two groups according to the presence of diabetes; pregestational and gestational diabetes were clustered in the same Diabetes Group. Differences between both groups were assessed using χ 2 or Student’s t tests. Results Preterm births of 133 and 4,017 women with and without diabetes, respectively, were included. The prevalence of diabetes was 3.2%. Pregnant women aged ≥35 years were more common in the Diabetes Group (31.6% versus 14.0% non-diabetic women, respectively). The rate of cesarean section among patients with diabetes was 68.2% versus 52.3% in non-diabetic cases), with a gestational age at birth between 34 and 36 weeks in 78.9% of the cases and 62.1% of the controls. Large-for-gestational-age babies were 7 times more common in the Diabetes Group. Conclusion Preterm birth among Brazilian women with diabetes was more than twice as prevalent; these women were older and had regular late preterm deliveries, usually by cesarean section. They also had a greater frequency of fetal morbidities, such as malformations and polyhydramnios, and a higher proportion of large-for-gestational-age and macrosomic neonates.

RESUMO A geração de cenários sintéticos de vazões afluentes é particularmente importante em países como o Brasil, onde a geração de energia hidrelétrica desempenha um papel fundamental e é crucial lidar adequadamente com a incerteza da vazão futura. Este artigo apresenta um esforço colaborativo do setor elétrico brasileiro para aprimorar os modelos de geração de cenários de vazão, com foco em horizontes de até um ano. Cinco instituições propuseram metodologias diversas e a sua eficácia foi avaliada através de uma avaliação comparativa estruturada. Os resultados revelam os pontos fortes e as áreas de possíveis aprimoramentos em cada modelo. O modelo GHCen apresentou o melhor desempenho, destacando-se nas análises de curto prazo e de média móvel, enquanto o modelo PARX demonstrou desempenho superior em regiões específicas do Brasil. O PAR(p)-A, metodologia oficial no Brasil, foi o segundo melhor modelo na análise de médias móveis. Este artigo oferece informações valiosas para países que enfrentam desafios semelhantes de programação hidrotérmica e geração de cenários. ABSTRACT Synthetic streamflow scenario generation is particularly important in countries like Brazil, where hydroelectric power generation plays a key role and properly handling the uncertainty of future streamflow is crucial. This paper showcases a collaborative effort within the Brazilian electrical sector to enhance streamflow scenario models, focusing on horizons up to one year. Five institutions proposed diverse methodologies, and their effectiveness was evaluated using a comparative framework. The results reveal the strengths and areas for improvement in each model. GHCen emerged as the top performer, excelling in both short-term and moving average analyses, while the PARX model demonstrated superior performance in specific regions. The PAR(p)-A, which is the official methodology in Brazil, was the second-best model in the moving average analysis. This research offers valuable insights for countries facing similar hydrothermal scheduling and scenario generation challenges. Resumo O objetivo deste trabalho foi avaliar se a puberdade e o parto alteram a contagem de folículos antrais (CFA) em fêmeas Nelore (Bos indicus) de corte. Foram realizados dois experimentos para comparar a CFA entre os períodos pré-púbere e púbere e entre os períodos pré-púbere e pós-parto. A CFA e a concentração folicular não diferiram entre os períodos pré-púbere e púbere. Entretanto, a área ovariana foi maior após a puberdade. Além disso, a CFA e a área do ovário foram maiores no período pós-parto quando comparado ao pré-púbere. Portanto, a CFA não é afetada pela puberdade de novilhas Nelore, mas aumenta significativamente quando as novilhas pré-púberes se tornam vacas. Abstract The objective of this work was to investigate whether puberty and parturition alter the antral follicle count (AFC) in female Nelore (Bos indicus) beef cattle. Two experiments were carried out to compare AFC between the prepubertal and pubertal periods and between the prepubertal and postpartum periods. AFC and follicle concentrations did not differ between the prepubertal and pubertal periods. However, the ovarian area increased after puberty. In addition, AFC and the ovarian area were greater in the postpartum than in the prepubertal period. ABSTRACT Objective: To evaluate the association between obesity and hospitalization in mild COVID-19 adult outpatients in Brazil. Subjects and methods: Adults with signs and symptoms suggestive of acute SARS-CoV-2 infection who sought treatment in two hospital (public and private) emergency departments were prospectively enrolled. Patients with confirmed COVID-19 at inclusion were followed by phone calls at days D7, D14 and D28. Multivariable logistic regression models were employed to explore the association between obesity and other potential predictors for hospitalization. Results: A total of 1,050 participants were screened, and 297 completed the 28-day follow-up and were diagnosed with COVID-19 by RT-PCR. The median age was 37.2 (IQR 29.7-44.6) years, and 179 (60.0%) were female. The duration of symptoms was 3.0 (IQR 2.0-5.0) days, and 10.0 (IQR 8.0-12.0) was the median number of symptoms at inclusion. Ninety-five (32.0%) individuals had obesity, and 233 (78.5%) had no previous medical conditions. Twenty-three participants (7.7%) required hospitalization during the follow-up period. After adjusting, obesity (BMI ≥ 30.0 kg/m2) (OR = 2.69, 95% CI 1.63-4.83, P < 0.001) and older age (OR = 1.05, 95% CI 1.01-1.09, P < 0.001) were significantly associated with higher risks of hospitalization. Conclusion: Obesity, followed by aging, was the main factor associated with hospital admission for COVID-19 in a young population in a low-middle income country. Our findings highlighted the need to promote additional protection for individuals with obesity, such as vaccination, and to encourage lifestyle changes.

RESUMO O artigo, em forma de ensaio, defende que as ameaças de vulnerabilização mais recentes contra os povos indígenas, intensificadas no contexto da pandemia de Covid-19, refletem um colonialismo persistente. Este se atualiza no contexto da inserção semiperiférica do Brasil no sistema-mundo capitalista, neoliberal e globalizado como exportador de commodities produzidas pelos dois setores estratégicos do neoextrativismo, a mineração e o agronegócio. O modelo neoextrativista beneficia principalmente grupos transnacionais e elites nacionais com grande poder econômico e político, além do próprio setor financeiro. Além disso, estabelece conexões com o submundo dos circuitos inferiores e ilegais vinculados a setores como o garimpo, e incluem desde práticas de violência até a lavagem de dinheiro com a participação de grupos locais que, nos últimos tempos, vêm assumindo crescente poder político e institucional. Tais grupos fazem parte do complexo mosaico do fortalecimento de ideologias de extrema-direita nos últimos anos no cenário nacional, que vêm reunindo alianças. O artigo tem por base experiências de pesquisa colaborativa nos últimos anos com o povo Munduruku na região do Médio Tapajós, com reflexões sobre a atual expansão de agenda política anti-indígena.

ABSTRACT This essay argues that the most recent threats of vulnerability against indigenous peoples, intensified in the context of the COVID-19 pandemic, reflect a persistent colonialism. This is updated in the context of Brazil’s semi-peripheral insertion into the capitalist, neoliberal, and globalized world-system as an exporter of commodities produced by the two strategic sectors of neo-extractivism, mining and agribusiness. The neo-extractivist model benefits mainly transnational groups and national elites with great economic and political power, in addition to the financial sector. Moreover, it establishes connections with the underworld of inferior and illegal circuits linked to sectors such as mining, and ranges from practices of violence to money laundering with the participation of local groups that, in recent times, have been assuming growing political and institutional power. Such groups are part of the complex mosaic of the strengthening of far-right ideologies in recent years on the national scene, which have been gathering alliances. This essay is based on collaborative research experiences in recent years with the Munduruku people in the Middle Tapajós region, along with reflections on the current expansion of the anti-indigenous political agenda.

Abstract Objective: to evaluate the accuracy of an antibody point-of-care lateral flow immunoassay (LFI -Wondfo Biotech Co., Guangzhou, China) in a pediatric population. Methods: children and adolescents (2 months to 18 years) with signs and symptoms suggestive of acute SARS-CoV-2 infection were prospectively investigated with nasopharyngeal RT-PCR and LFI at the emergency room. RT-PCR was performed at baseline, and LFI at the same time or scheduled for those with less than 7 days of the clinical picture. Overall accuracy, sensitivity and specificity were assessed, as well as according to the onset of symptoms (7-13 or ≥14 days) at the time of the LFI test. Results: In 175 children included, RT-PCR and LFI were positive in 51 (29.14%) and 36 (20.57%), respectively. The overall sensitivity, specificity, positive and negative predictive value was 70.6% (95%CI 56.2-82.5), 96.8% (95%CI 91.9-99.1), 90.0% (95%CI 77.2-96.0), and 88.9% (95%CI 83.9-92.5), respectively. At 7-13 and ≥14 days after the onset of symptoms, sensitivity was 60.0% (95%CI 26.2-87.8) and 73.2% (95%CI 57.1-85.8) and specificity was 97.9% (95%CI 88.7-99.9) and 96.1% (95%CI 89.0-99.2), respectively. Conclusion: Despite its high specificity, in the present study the sensitivity of LFI in children was lower (around 70%) than most reports in adults. Although a positive result is informative, a negative LFI test cannot rule out COVID-19 in children.

Os testes sorológicos no local de atendimento (point-of-care) para a infecção pelo SARS-CoV-2 têm sidos utilizados para o diagnóstico da COVID-19. Entretanto, não está plenamente elucidada a acurácia dos testes ao longo do tempo em relação ao início dos sintomas. Nosso objetivo foi de avaliar a acurácia, no local de atendimento, do imunoensaio de fluxo lateral (LFI). Pacientes com ≥ 18 anos de idade que apresentavam sintomas clínicos sugestivos de infecção aguda pelo SARS-CoV-2 foram testados uma vez com RT-PCR da nasofaringe e orofaringe, além do LFI. A acurácia do LFI foi avaliada com intervalos periódicos de 3 dias a partir do início dos sintomas. O ponto de corte ótimo foi definido como o número necessário de dias para atingir a melhor sensibilidade e especificidade. Esse ponto foi utilizado também para comparar a acurácia do LFI de acordo com a situação do paciente (ambulatorial ou hospitalizado). Foram incluídos 959 pacientes, dos quais 379 (39,52%) testaram positivos para SARS-CoV-2 pelo RT-PCR e 272 (28,36%) pelo LFI. Foi atingido o melhor desempenho para o LFI com 10 dias a partir do início dos sintomas, com sensibilidade e especificidade de 84,9% (IC95%: 79,8-89,1) e 94,4% (IC95%: 91,0-96,8), respectivamente. Embora a especificidade não tenha sido diferente entre os grupos de pacientes (94,6% vs. 88,9%, p = 0,051), a sensibilidade foi mais alta nos pacientes hospitalizados que nos ambulatoriais (91,7% vs. 82,1%, p = 0,032) no dia 10 depois do início dos sintomas. A melhor sensibilidade do LFI no local de atendimento ocorre 10 dias depois do início dos sintomas, o que pode limitar seu uso no atendimento agudo. A especificidade permanece alta, independentemente do número de dias desde o início dos sintomas.

Point-of-care serological tests for SARS-CoV-2 have been used for COVID-19 diagnosis. However, their accuracy over time regarding the onset of symptoms is not fully understood. We aimed to assess the accuracy of a point-of-care lateral flow immunoassay (LFI). Subjects, aged over 18 years, presenting clinical symptoms suggestive of acute SARS-CoV-2 infection were tested once by both nasopharyngeal and oropharyngeal RT-PCR and LFI. The accuracy of LFI was assessed in periodic intervals of three days in relation to the onset of symptoms. The optimal cut-off point was defined as the number of days required to achieve the best sensitivity and specificity. This cut-off point was also used to compare LFI accuracy according to participants’ status: outpatient or hospitalized. In total, 959 patients were included, 379 (39.52%) tested positive for SARS-CoV-2 with RT-PCR, and 272 (28.36%) tested positive with LFI. LFI best performance was achieved after 10 days of the onset of symptoms, with sensitivity and specificity of 84.9% (95%CI: 79.8-89.1) and 94.4% (95%CI: 91.0-96.8), respectively. Although the specificity was similar (94.6% vs. 88.9%, p = 0.051), the sensitivity was higher in hospitalized patients than in outpatients (91.7% vs. 82.1%, p = 0.032) after 10 days of the onset of symptoms. Best sensitivity of point-of-care LFI was found 10 days after the onset of symptoms which may limit its use in acute care. Specificity remained high regardless of the number of days since the onset of symptoms.

Los puestos de atención para pruebas serológicas del SARS-CoV-2 han sido usado para la diagnosis de la COVID-19. No obstante, su precisión a lo largo del tiempo, en lo que respecta a la aparición de los síntomas, no se ha comprendido completamente. Nuestro objetivo fue evaluar la precisión de un puesto de atención de inmunoanálisis de flujo lateral (LFI). Se hizo pruebas a individuos ≥ 18 años, presentando síntomas clínicos compatibles con una infección aguda de SARS-CoV-2, tanto vía nasofaríngea y orofaríngea RT-PCR, como LFI. La precisión de LFI fue evaluada en intervalos periódicos de 3 días con respecto a la aparición de los síntomas. El punto óptimo de corte se definió como el número de días requerido para alcanzar la mejor sensibilidad y especificidad. Este punto también se usó para comparar la precisión del LFI, según el estatus de los participantes: ambulatorios u hospitalizados. Se incluyeron a 959 pacientes, 379 (39,52%) dieron positivo en las pruebas de SARS-CoV-2 RT-PCR, y 272 (28,36%) fueron positivos en los LFI. Se alcanzó el mejor rendimiento de los LFI tras 10 días de la aparición de los síntomas, con una sensibilidad y especificidad de un 84,9% (IC95%: 79,8-89,1) y 94,4% (IC95%: 91,0-96,8), respectivamente. A pesar de que la especificidad no fue diferente (94,6% vs. 88,9%, p = 0,051), la sensibilidad fue mayor en pacientes hospitalizados que en los ambulatorios (91,7% vs. 82,1%, p = 0,032) tras 10 días desde la aparición de los síntomas. La mejor sensibilidad LFI del puesto de cuidado se produce tras 10 días de la aparición de los síntomas, lo que quizás limite su uso en el cuidado de urgencias. La especificidad permanece alta independientemente del número de días desde la aparición de los síntomas.
