Objectives: A multicenter study was performed with the aim of determining the frequency of nasal carriers of S. aureus, antimicrobial susceptibility and risk factors of the carrying in Health staff of Intensive Care Units of Adults (UCIA). Design: Prospective observational study of transverse cut, made in November of 2002 in Health staff of UCIA from four public Hospitals (Clinical Hospital, National Hospital, Social Preventive Institute and Medical Emergency Center). Variable: age, sex, hospitality labor, and admission, preview surgery, preview use of antibiotics and topics corticoids, nasal carrying of S. aureus and antimicrobial susceptibility. It was made nasal swabs to detect carriage of S. aureus in 142 individuals. The isolations were confirmed as S. aureus based on catalase, coagulas and agglutination test. The susceptibility testing was performed by the Disk diffusion method for methicillin (with 1 ?g oxacilin disc), vancomycin (VAN) in agar Mueller- Hinton. Results were interpreted with guidelines established through the Clinical and Laboratory Standards Institute (CLSI). Results: It was included 142 persons of the UCIA Health-staff. Average age: 32 ± 6.3 years, feminine sex: 78.2% and the distribution by function were: 23.3% doctors, 57.0% nursing and 19.7% assistants. The risk factors of nasal carrying were internment and preview surgery in the last year, in 8.5% and 22.5% respectively; the utilization of systemic antibiotics 46.5% and topics corticoids 11.3%, both were in the last six months. They presented positive culture of S. aureus in 60 people that represent 42.3% of nasal carrying, methicillin-resistant S. aureus was 33.3% and non resistant to VAN was found. Risk factors for nasal carriers for people working less time in an UCIA was of OR 2.76 (1.33 - 35.75) p 0.006 and for S. aureus methicillin resistant (MRSA) nasal carriers working in Social Preventive Institute and National Hospital was of OR: 4.71 (1.4 - 15.8) p 0.01. Conclusions: Nasal carriers of S. aureus healthstaff of UCIA were 42.3%, 33.3% of the strains were methicillin-resistant S. aureus. The lesser spent of time working in an UCIA was a risk factor for S. aureus nasal carriers and for people working in Social Preventive Institute and National Hospital was for MRSA
Objetivo: determinar la frecuencia de portación nasal de Staphylococcus aureus (S. aureus), su sensibilidad antimicrobiana y factores de riesgo de portación en personal de salud en Unidades de Cuidados Intensivos de adultos (UCIA) Diseño: estudio prospectivo observacional de corte transverso, realizado en noviembre del año 2002. Población: médicos, licenciados en enfermerías y asistentes de UCIA de 4 hospitales públicos (Hospital de Clínicas, Nacional de Itauguá, Instituto de Previsión Social, Emergencias Médicas). Variables: edad, sexo, función hospitalaria, internación y cirugía previas, uso previo de antibióticos y corticoides tópicos, portación nasal de S. aureus y sensibilidad antimicrobiana. Fueron realizados hisopados nasales para detectar portación de S. aureus en 142 personas. Los aislamientos confirmados como S. aureus fueron basados en las pruebas de catalasa, coagulasa y test de aglutinación. Los test de susceptibilidad fueron desarrollados por método de difusión en discos para meticilina (con discos para oxacilina de 1 ug), vancomicina en agar Mueller-Hinton. Los resultados fueron interpretados según normas de la Clinical and Laboratory Standards Institute (CLSI). Resultados: Se incluyeron 142 individuos, la edad promedio fue 32 ± 6,3 años, sexo femenino 78,2% y la distribución según función: 23,2% médicos, 57% licenciadas en enfermería, 19,7% asistentes; 8,5% tuvo alguna internación y 22,5% cirugía previa en el último año; utilizaron antibióticos sistémicos y corticoides tópicos en los últimos 6 meses 46,5 % y 11,3 % respectivamente. Presentaron cultivos positivos a S. aureus 60 personas que representa 42,3 % de portación nasal. La resistencia a la meticilina fue 33,3 % y ninguna a la vancomicina. Factor de riesgo para portación nasal un menor tiempo de trabajo en la terapia con RR: 2.76 (1.33 - 5.75) p 0.006 y para portación de meticilino resistente los hospitales de Instituto de Previsión Social y Nacional de Itauguá RR 4.71 (1.4 - 15.8) p = 0.01. Conclusiones: La portación nasal de S. aureus en personal de salud de las unidades de cuidados intensivos fue de 42,3 % con una resistencia a la meticilina de 33.3 % y ninguna a vancomicina. Factor de riesgo para portación nasal un menor tiempo de trabajo en UCIA y para meticilino resistente los hospitales Nacional de Itauguá e Instituto de Previsión Social.
