Evaluation of five parasitological techniques for diagnosing phylum Ciliophora cysts in fecal samples from free-living wild artiodactyls

ABSTRACT: A variety of laboratory techniques are used in parasitological diagnosis. However, studies that analyze their laboratory efficiency are very scarce, especially with regard to biological samples from wild animals that are little known, with little popular attachment, such as artiodactyls. These can be infected by different parasites, including protozoa of the phylum Ciliophora, which includes the parasites Balantioides coli and Buxtonella sulcata. In this light, the aim of this study was to compare the efficiency of five coproparasitological techniques for diagnosing protozoan cysts of the phylum Ciliophora in the feces of free-living artiodactyls. To this end, 101 fecal samples were collected from trails in Pedra Selada State Park, Rio de Janeiro state, from 2020 to 2021. All the samples were analyzed using the qualitative techniques of modified Sheather floatation, modified Ritchie sedimentation and Lutz, as well as the quantitative techniques of Mini-FLOTAC and McMaster. Cyst recovery was best achieved using the modified Ritchie technique, in which 62.5% positivity was detected, followed by Lutz (47.5%), modified Sheather (37.5%) and the quantitative techniques of Mini-FLOTAC (30%) and McMaster (17.5%). In most of the comparisons between the techniques, reasonable agreement regarding the diagnosis was observed (Kappa 0.21 to 0.40), which was statistically significant (p≤0.05). McMaster showed higher mean and standard deviation values for counts of cysts per gram of feces than Mini-FLOTAC. However, there was no significant difference in the estimates for cyst counts (Wilcoxon p>0.05). Sedimentation qualitative techniques were more indicated for diagnosing cysts of protozoa of the phylum Ciliophora in the feces of free-living wild artiodactyls. These techniques can therefore be used as laboratory tools for environmental parasite monitoring. In addition, between the two quantitative techniques, Mini-FLOTAC presented better performance, thus showing its potential as a tool for estimating the abundance of cystic forms of the phylum Ciliophora in environmental samples.
RESUMO: Diversas são as técnicas laboratoriais utilizadas no diagnóstico parasitológico. No entanto, estudos que analisam a eficiência laboratorial das mesmas são muito escassos, principalmente quando se trata de amostras biológicas de animais silvestres, que possuem pouca divulgação e apego popular, como os artiodáctilos. Estes podem se infectar por diferentes parasitos, incluindo os protozoários do Filo Ciliophora, no qual se inclui Balantioides coli, bem como Buxtonella sulcata. Mediante o exposto, este estudo objetivou comparar a eficiência entre cinco técnicas coproparasitológicas para o diagnóstico de cistos de protozoário do Filo Ciliophora em fezes de artiodáctilos em vida livre. Entre 2020 e 2021 foram coletadas 101 amostras fecais em trilhas do Parque Estadual da Pedra Selada localizado no estado do Rio de Janeiro. Todas as amostras foram analisadas pelas técnicas qualitativas de flutuação de Sheather modificada, sedimentação de Ritchie modificada e Lutz, bem como por meio das técnicas quantitativas de Mini-FLOTAC e McMaster. Pode-se verificar uma superioridade na recuperação dos cistos por meio da técnica de Ritchie modificada, no qual foi detectado 62,5% de positividade, seguida pelo Lutz (47,5%), Sheather modificada (37,5%) e pelas técnicas quantitativas de Mini-FLOTAC (30%) e McMaster (17,5%). Na maioria das comparações entre as técnicas foi possível verificar uma concordância razoável (Kappa 0,21 a 0,40) em relação ao diagnóstico entre as mesmas, sendo este estatisticamente significativo (p≤0,05). McMaster em comparação com o Mini-FLOTAC foi a que apresentou os maiores valores médio e de desvio padrão na contagem dos cistos por grama de fezes. No entanto, não foi evidenciado diferença significativa na estimativa de contagem dos cistos (Wilcoxon=p>0,05). Pode-se verificar que as técnicas qualitativas de sedimentação foram consideradas as mais indicadas para o diagnóstico de cistos do protozoário do Filo Ciliophora em fezes de artiodáctilos silvestres em vida livre, podendo ser utilizadas como ferramentas laboratoriais de monitoramento parasitário ambiental. Além disso, dentre as técnicas quantitativas, o Mini-FLOTAC foi a que apresentou a melhor performance, denotando o seu potencial como uma ferramenta para se estimar a abundância de formas císticas do Filo Ciliophora em amostra ambiental.

Abstract Background: Patients using immunosuppressive drugs may have unfavorable results after infections. However, there is a lack of information regarding COVID 19 in these patients, especially in patients with rheumatoid arthritis (RA). Therefore, the aim of this study was to evaluate the risk factors associated with COVID 19 hospitalizations in patients with RA. Methods: This multicenter, prospective cohort study is within the ReumaCoV Brazil registry and included 489 patients with RA. In this context, 269 patients who tested positive for COVID 19 were compared to 220 patients who tested negative for COVID 19 (control group). All patient data were collected from the Research Electronic Data Capture database. Results: The participants were predominantly female (90.6%) with a mean age of 53 ±12 years. Of the patients with COVID 19, 54 (20.1%) required hospitalization. After multiple adjustments, the final regression model showed that heart disease (OR =4.61, 95% CI 1.06–20.02. P < 0.001) and current use of glucocorticoids (OR =20.66, 95% CI 3.09–138. P < 0.002) were the risk factors associated with hospitalization. In addition, anosmia was associated with a lower chance of hospitalization (OR =0.26; 95% CI 0.10–0.67, P < 0.005). Conclusion: Our results demonstrated that heart disease and the use of glucocorticoids were associated with a higher number of hospital admissions for COVID 19 in patients with RA. RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

The objective of this work was to carry out a bibliographic survey of secondary metabolites isolated from the Velloziaceae family, creating a bank of compounds. After the bank was created, four prediction models for potentially active compounds against pathogenic microorganisms (Candida albicans, Escherichia coli, Pseudomonas aeruginosa and Salmonella sp.) were obtained trying to identify which metabolites would be more active against the strains. Four sets of compounds with known activity for microorganisms were selected for the construction of predictive models from the CHEMBL database. Another bank with 163 unique molecules isolated from the Velloziaceae family was built. The Volsurf+ v.1.0.7 software obtained the molecular descriptors and Knime 3.5 generated the in silico model. The performances of the internal and external tests were also analyzed. The study contributed through the virtual screening of a bank of metabolites to select several compounds with potential antimicrobial activity, highlighting the biflavonoid amentoflavone which showed potential activity against the four strains.

A chemical investigation of Anaxagorea dolichocarpa Sprague & Sandwith, a member of Annonaceae family, was carried out. The ethanolic extract from the roots of this plant led, by chromatography tecniches, to isolation of the new azaphenanthrene alkaloids dolichocarpine (1) and 9-methoxyeupolauramine (2), besides the known alkaloids eupolauramine (3), 3-methoxyeupolauridine (4), eupolauramine (5) and 4-methylsampangine (6). The structures of isolated compounds were established by 1D and 2D NMR, HRESIMS, tandem MSn and IR data. The cytotoxicity of compounds 1 - 5 was evaluated against HCT-116 (human colorectal carcinoma) and L929 (murine fibroblast) cell lines.

RESUMO Nesta pesquisa objetivou-se avaliar os aspectos biológicos e o comportamento alimentar de Aphis gossypii em cultivares de melancia submetidas à aplicação de silício. O experimento foi conduzido no Instituto de Educação, Agricultura e Ambiente da Universidade Federal do Amazonas, Humaitá, Brasil. Utilizaram-se o delineamento experimental inteiramente ao acaso e o esquema fatorial 2×3 (sem silício e com silício; cultivares Crimson Sweet, Fairfax e Charleston), com dez repetições. A aplicação do ácido silícico (1%) foi realizada diretamente no substrato, com dose equivalente a 1 ton SiO2·ha-1, 25 dias após a semeadura. Os pulgões da criação foram mantidos em plantas de pepino, cultivar Caipira. Foram conduzidos ensaios de biologia do inseto para avaliação da duração dos períodos pré-reprodutivo, reprodutivo e pós-reprodutivo, longevidade, número de ninfas e comportamento alimentar por meio da técnica de secreção de honeydew. Realizou-se a análise de variância dos dados utilizando-se o programa estatístico SISVAR e as médias foram comparadas pelo teste de F e Scott–Knott (p ≤ 0,05). A aplicação de silício em plantas de melancia afeta a reprodução e a alimentação de A. gossypii. Plantas de melancia do cultivar Crimson Sweet tratadas com silício apresentam alta resistência à alimentação por A. gossypii.
ABSTRACT This research aimed to evaluate the biological aspects and the feeding behavior of Aphis gossypii in watermelon cultivars submitted to silicon application. The experiment was conducted at the Institute of Education, Agriculture and Environment of the Federal University of Amazonas, Humaitá, Brazil. The experimental design was completely randomized in a 2×3 factorial (with and without silicon; cultivars Crimson Sweet, Fairfax and Charleston), with ten replications. The application of silicic acid (1%) was carried out directly on the substrate using dose equivalent to 1 ton SiO2·ha-1, 25 days after sowing. The rearing of aphids was kept in cucumber plants, cultivar Caipira. Insect biology tests were conducted to evaluate the duration of the prereproductive, reproductive and postreproductive periods, longevity, number of nymphs, and feeding behavior using the honeydew secretion technique. Analysis of variance was performed using the statistical program SISVAR and the means were compared by the F and Scott–Knott test (p ≤ 0.05). The silicon application to watermelon plants affects the reproduction and feeding of A. gossypii. The watermelon plants cultivar Crimson Sweet treated with silicon has high resistance to feeding by A. gossypii.

Abstract We present postmortem evidence of invasive pulmonary aspergillosis (IPA) in a patient with severe COVID-19. Autopsies of COVID-19 confirmed cases were performed. The patient died despite antimicrobials, mechanical ventilation, and vasopressor support. Histopathology and peripheral blood galactomannan antigen testing confirmed IPA. Aspergillus penicillioides infection was confirmed by nucleotide sequencing and BLAST analysis. Further reports are needed to assess the occurrence and frequency of IPA in SARS-CoV-2 infections, and how they interact clinically.

Restrictive versus Liberal Fluid Therapy for Post-Cesarean Acute Kidney Injury in Severe Preeclampsia: a Pilot Randomized Clinical Trial

OBJECTIVES: The aim of this study was to determine whether a restrictive compared to a liberal fluid therapy will increase postoperative acute kidney injury (AKI) in patients with severe preeclampsia. METHODS: A total of 46 patients (mean age, 32 years; standard deviation, 6.8 years) with severe preeclampsia were randomized to liberal (1500 ml of lactated Ringer’s, n=23) or restrictive (250 ml of lactated Ringer’s, n=23) intravenous fluid regimen during cesarean section. The primary outcome was the development of a postoperative renal dysfunction defined by AKI Network stage ≥1. Serum cystatin C and neutrophil gelatinase-associated lipocalin (NGAL) were evaluated at postoperative days 1 and 2. ClinicalTrials.gov: NCT02214186. RESULTS: The rate of postoperative AKI was 43.5% in the liberal fluid group and 43.5% in the restrictive fluid group (p=1.0). Intraoperative urine output was higher in the liberal (116 ml/h, IQR 69-191) than in the restrictive fluid group (80 ml/h, IQR 37-110, p<0.05). In both groups, serum cystatin C did not change from postoperative day 1 compared to the preoperative period and significantly decreased on postoperative day 2 compared to postoperative day 1 (p<0.05). In the restrictive fluid group, NGAL levels increased on postoperative day 1 compared to the preoperative period (p<0.05) and decreased on postoperative day 2 compared to postoperative day 1 (p<0.05). CONCLUSION: Among patients with severe preeclampsia, a restrictive fluid regimen during cesarean section was not associated with increased postoperative AKI.

ABSTRACT Studies on mortality due to fishing based on isolating the effect caused by catches are scarce. The present study analyzed this aspect using the ghost shrimp Callichirus major (Say, 1818) as a biological model, which is heavily harvested along the Brazilian coast. For that purpose, density of burrows and population structure of C. major was examined at two adjacent beaches, Gonzaga and Itararé, from the State of São Paulo, Brazil. Gonzaga beach was considered as control because since 1992 a municipal law prevents the capture of C. major, whereas Itararé as treatment since in this place the species is harvested throughout the year. Burrow density and female CL size were significantly higher in Gonzaga than that Itararé. Sex-ratio did not differ significantly from 1:1 in Itararé, differing from the observed in the area without fishing mortality and also to the general pattern reported in other species of ghost shrimps in which females outnumbered than males. Reproductive potential was significantly smaller in Itararé than Gonzaga, as result of the reduced number of females present in this area relative to Gonzaga. A preliminary estimation indicates that 71 % of shrimps in Itararé died as result of fishing. This information suggests non-viability of C. major in this area given the current pressure of fishing. Presence of C. major in the beach of Itararé and probably in other nearby areas where exploitation occurs would be favored by the permanent supply of larvae from Gonzaga beach where the species is protected by and municipal surveillance.
RESUMEN La mortalidad por pesca ha sido pocas veces estudiada aislando el efecto causado por las capturas. En este estudio se analizó este aspecto usando como modelo biológico al camarón fantasma Callichirus major (Say, 1818), el cual es colectado intensamente en varias regiones de la costa de Brasil. Para ello, la densidad de las galerías y la demografía poblacional de C. major fue analizada en las playas de Gonzaga e Itararé, dos playas contiguas del Estado de São Paulo, Brasil. La playa de Gonzaga fue considerada como control debido a que desde 1992 una ley municipal impide las capturas de C. major, mientras que Itararé fue considerada como tratamiento ya que en este lugar este camarón es recolectado durante todo el año. Los resultados mostraron que la densidad de las galerías y el tamaño de las hembras fueron significativamente mayores en la playa de Gonzaga que en Itararé. En Itararé, la proporción sexual no mostró diferencias significativas de la relación 1:1, lo cual no se ajustó a lo observado en el área sin mortalidad por pesca y al patrón general reportado en otras especies de camarones fantasma del mundo en donde las hembras predominan sobre los machos. El potencial reproductivo fue significativamente menor en Itararé, como efecto del menor número de hembras presentes en esta área sometida a la pesca intensiva de camarones. De acuerdo con una estimativa inicial, un 71% de los individuos murieron por efecto de la pesca en Itararé. Esto hace inviable el sostenimiento ambiental de C. major en esta área dada la actual presión de extracción. La presencia de C. major en la playa de Itararé y probablemente en otras áreas próximas donde es capturada, estaría favorecida por el permanente suministro de larvas procedentes de la playa de Gonzaga, en Santos, que está protegida por ley y fiscalización municipal.

Resumo Objetivos Avaliar incidência de tuberculose e triagem para tuberculose latente em brasileiros com artrite reumatoide em uso de agentes biológicos na prática clinica. Pacientes e métodos Estudo de coorte com dados do Registro Brasileiro de Monitoração de Terapias Biológicas (BiobadaBrasil), de 01/2009 a 05/2013, abrangeu 1.552 tratamentos, 415 somente com drogas modificadoras do curso da doença (MMCDs) sintéticas, 942 MMCDs sintéticas em associação com anti-TNF (etanercepte, infliximabe, adalimumabe) e 195 MMCDs sintéticas em associação com outros biológicos (abatacepte, rituximabe e tocilizumabe). Avaliaram-se ocorrência de tuberculose, tempo de exposição às drogas e triagem para TB. Análise estatística: teste t não pareado e teste de Fisher bicaudal; p < 0,05. Resultados O tempo de exposição dos controles foi de 981 pacientes-ano, do grupo de anti-TNF foi de 1.744 pacientes-ano (adalimumabe = 676, infliximabe = 547 e etanercepte = 521 pacientes-ano) e o de outros biológicos de 336 pacientes-ano. A incidência de TB foi de 1,01/1.000 pacientes-ano nos controles e de 2,87 pacientes-ano nos usuários de anti-TNF (adalimumabe = 4,43/1.000 pacientes-ano; etanercepte = 1,92/1.000 pacientes-ano e infliximabe = 1,82/1.000 pacientes-ano). Não houve casos de tuberculose no grupo de outros biológicos. O tempo médio de exposição até a ocorrência de tuberculose foi de 27(11) meses para o grupo anti-TNF. Conclusões A incidência de tuberculose foi maior nos usuários de MMCDs sintéticas e anti-TNF do que nos usuários de MMCDs sintéticas e de MMCDs sintéticas e biológicos não anti-TNF, e também mais tardia, sugerindo infecção durante o tratamento, e não falha na triagem.
Abstract Objectives To assess the incidence of tuberculosis and to screen for latent tuberculosis infection among Brazilians with rheumatoid arthritis using biologics in clinical practice. Patients and methods This cohort study used data from the Brazilian Registry of Biological Therapies in Rheumatic Diseases (Registro Brasileiro de Monitoração de Terapias Biológicas - BiobadaBrasil), from 01/2009 to 05/2013, encompassing 1552 treatments, including 415 with only synthetic disease-modifying anti-rheumatic drugs, 942 synthetic DMARDs combined with anti-tumor necrosis factor (etanercept, infliximab, adalimumab) and 195 synthetic DMARDs combined with other biologics (abatacept, rituximab and tocilizumab). The occurrence of tuberculosis and the drug exposure time were assessed, and screening for tuberculosis was performed. Statistical analysis: Unpaired t-test and Fisher's two-tailed test; p < 0.05. Results The exposure times were 981 patient-years in the controls, 1744 patient-years in the anti-TNF group (adalimumab = 676, infliximab = 547 and etanercept = 521 patient-years) and 336 patient-years in the other biologics group. The incidence rates of tuberculosis were 1.01/1000 patient-years in the controls and 2.87 patient-years among anti-TNF users (adalimumab = 4.43/1000 patient-years; etanercept = 1.92/1000 patient-years and infliximab = 1.82/1000 patient-years). No cases of tuberculosis occurred in the other biologics group. The mean drug exposure time until the occurrence of tuberculosis was 27(11) months for the anti-TNF group. Conclusions The incidence of tuberculosis was higher among users of synthetic DMARDs and anti-TNF than among users of synthetic DMARDs and synthetic DMARDs and non-anti-TNF biologics and also occurred later, suggesting infection during treatment and no screening failure.

Resumo Introdução: A síndrome nefrite tubulointersticial e uveíte (síndrome TINU) é uma entidade clínica incomum e a maioria dos pacientes são adolescentes e mulheres jovens. O caso relatado a seguir refere-se a uma paciente idosa com manifestações oftalmológicas que antecederam às renais, sendo provavelmente o primeiro caso descrito no Brasil. Relato de Caso: Paciente feminina, 60 anos, procurou atendimento médico por queixa de "olhos vermelhos". Três meses após o primeiro episódio do quadro ocular, a paciente cursou com sintomas sistêmicos e disfunção renal. A biópsia renal evidenciou nefrite tubulointersticial com sinais de atividade. Discussão: A fisiopatologia da síndrome TINU permanece pouco entendida, provavelmente envolvendo a imunidade celular e humoral. Essa síndrome pode ser diferenciada de condições sistêmicas associadas à uveíte e à nefrite, sendo necessária a exclusão de outras doenças antes de se confirmar seu diagnóstico, especialmente na presença de achados oftalmológicos. Conclusão: A suspeição clínica e o conhecimento do manejo desta patologia por nefrologistas, internistas e oftalmologistas são mandatórios no tratamento do paciente com Síndrome TINU.
Abstract Introduction: Tubulointerstitial nephritis and uveitis syndrome (TINU Syndrome) is an uncommon clinical entity, and the majority of patients are adolescents and young women. The case reported refers to an elderly patient with ophthalmologic symptoms detected earlier than kidney manifestations, being probably the first case described in Brazil. Case Report: Female patient, 60 years-old, sought medical attention for complaints of "red eye". Three months after the first episode of eye manifestation, the patient presented with systemic symptoms and renal dysfunction. Renal biopsy showed tubulointerstitial nephritis with signs of activity. Discussion: The pathophysiology of TINU Syndrome remains poorly understood, probably involving both cellular and humoral immunity. This syndrome can be differentiated from systemic conditions associated with nephritis and uveitis, and prior exclusion of other diseases is necessary to confirm diagnosis, especially in the presence of ophthalmologic findings. Conclusion: The clinical suspicion and the knowledge of the management of the disease by nephrologists, internists and ophthalmologists is mandatory in the treatment of patients with TINU Syndrome.
